Summary: Background: No study has compared the virological and immunological status of young people with perinatally-acquired HIV infection (P-HIV) with that of people with HIV adulthood (A-HIV) having a similar duration of infection. Methods: 5 French cohorts of P-HIV and A-HIV patients with a known date of HIV-infection and receiving antiretroviral treatment (ART), were used to compare the following proportions of: virological failure (VF) defined as plasma HIV RNA ≥ 50 copies/mL, CD4 cell percentages and CD4:CD8 ratios, at the time of the most recent visit since 2012. The analysis was stratified on time since infection, and multivariate models were adjusted for demographics and treatment history. Findings: 310 P-HIV were compared to 1515 A-HIV (median current ages 20.9 [IQR:14.4–25.5] and 45.9 [IQR:37.9–53.5] respectively). VF at the time of the most recent evaluation was significantly higher among P-HIV (22.6%, 69/306) than A-HIV (3.3%, 50/1514); p ≤ 0.0001. The risk of VF was particularly high among the youngest children (2–5 years), adolescents (13–17 years) and young adults (18–24 years), compared to A-HIV with a similar duration of infection: adjusted Odds-Ratio (aOR) 7.0 [95% CI: 1.7; 30.0], 11.4 [4.2; 31.2] and 3.3 [1.0; 10.8] respectively. The level of CD4 cell percentages did not differ between P-HIV and A-HIV. P-HIV aged 6–12 and 13–17 were more likely than A-HIV to have a CD4:CD8 ratio ≥ 1: 84.1% vs. 58.8% (aOR = 3.5 [1.5; 8.3]), and 60.9% vs. 54.7% (aOR = 1.9 [0.9; 4.2]) respectively. Interpretation: P-HIV were at a higher risk of VF than A-HIV with a similar duration of infection, even after adjusting for treatment history, whereas they were not at a higher risk of immunological impairment. Exposure to viral replication among young patients living with HIV since birth or a very early age, probably because of lower adherence, could have an impact on health, raising major concerns about the selection of resistance mutations and the risk of HIV transmission. Nickel oxide nanoparticles (NiO NPs) have been the focus of many toxicity studies. However, acute toxicity studies that identify toxicological dose descriptors, such as an LC50 or LD50, are lacking. In this paper, the acute toxicity of NiO NPs was evaluated in albino-derived Sprague-Dawley rats through OECD guideline studies conducted by both the oral and inhalation routes of exposure. The animals were assessed for mortality, body weight, behavioral observations, and gross necropsy. Results from previously conducted (unpublished) acute inhalation studies with larger NiO microparticles (MPs) are also included for comparison. Mortality, the primary endpoint in acute toxicity studies, was not observed for rats exposed to NiO NPs via either the oral or inhalation exposure routes, with a determined LD50 of >5000 mg/kg and an LC50 > 5.42 mg/L, respectively. Our results suggest that these NiO NPs do not exhibit serious acute toxicity in rats or warrant an acute toxicity classification under the current GHS classification criteria. This aligns with similar results for NiO MPs from this and previously published studies.

The recent development and mass administration of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) vaccines allowed for disease control, reducing hospitalizations and mortality. Most of these vaccines target the SARS-CoV-2 Spike (S) protein antigens, culminating with the production of neutralizing antibodies (NAbs) that disrupt the attachment of the virus to ACE2 receptors on the host cells. However, several studies demonstrated that the NAbs typically rise within a few weeks after vaccination but quickly reduce months later. Thus, multiple booster administration is recommended, leading to vaccination hesitancy in many populations. Detecting serum anti-SARS-CoV-2 NAbs can instruct patients and healthcare providers on correct booster strategies. Several in vitro diagnostics kits are available; however, their high cost impairs the mass NAbs diagnostic testing. Recently, we engineered an ACE2 mimetic that interacts with the Receptor Binding Domain (RBD) of the SARS-2 S protein. Here we present the use of this engineered mini-protein (p-deface2 mut) to develop a detection assay to measure NAbs in patient sera using a competitive ELISA assay. Serum samples from twenty-one patients were tested. Nine samples (42.8%) tested positive, and twelve (57.1%) tested negative for neutralizing sera. The data correlated with the result from the standard commercial assay that uses human ACE2 protein. This confirmed that p-deface2 mut could replace human ACE2 in ELISA assays. Using bacterially expressed p-deface2 mut protein is cost-effective and may allow mass SARS-CoV-2 NAbs detection, especially in low-income countries where economical diagnostic testing is crucial. Such information will help providers decide when a booster is required, reducing risks of reinfection and preventing the administration before it is medically necessary.

This paper presents a description of the adjective class in Dinka, and an exploration of its typological relevance to the existence of adjectives cross-linguistically. In Dinka, a subset of intransitive verbs can be defined by two morphophonological characteristics. The overwhelming majority of these are property concepts, despite the fact that the defining characteristics are unrelated to semantic properties. This calls into question whether or not properties provide a useful semantic context in the investigation of adjectives, even in languages where the adjective class is clearly a subclass of another lexical category. While the analysis is based primarily on the Bor South dialect of Dinka, it has been corroborated using evidence from other dialects. Therefore, it is likely that this characterization holds across the language.

Myoepithelial cells prevent tumour growth and invasion in DCIS. Here, the authors show that p63 and TCF7 cooperate to regulate a transcription factor network for the maintenance of normal myoepithelial function and altered expression of these genes perturb myoepithelial function in DCIS to promote invasive progression.

Background: Spontaneous osteonecrosis of the knee (SONK) is a poorly understood but debilitating disease entity. Total knee arthroplasty (TKA) is the standard of care for those patients who fail conservative management, but considering SONK’s predilection for affecting a single knee compartment, unicompartmental knee arthroplasty (UKA) appears to be a more tailored option. Unfortunately, conflicting data exist on the utility of UKA in SONK. Thus, the purpose of this study was to evaluate functional outcomes and revision rates of UKA in the setting of SONK. Methods: A systematic literature search was performed to evaluate all studies examining patients who underwent UKA for SONK. Screening of the articles was performed using multiple Boolean search strings, methodological index for non-randomized studies criteria, and other selected exclusion criteria. Results: Seven studies were included, with a total of 276 knees (273 patients). The mean age was 68 years (64–74 years), with a mean body mass index of 26 kg/m 2 (25–29 kg/m 2 ). The final range of motion was 125° (124–126°). Standardized mean difference (SMD) of clinical improvement pre- and post-outcome was 3.39 ( p < 0.001). The improvement in the visual analog score was 57.03 points, with an SMD of 4.57 ( p < 0.001). Revision rates were determined to be 5.51% (95% confidence interval of 1.96–10.69%). Conclusion: Our meta-analysis demonstrated that in properly selected patients, UKA could be an excellent alternative to TKA for patients with SONK. These data show that UKA has few complications, significant improvements on functional outcomes, and good survivorship at a mean follow-up of 6 years.

The contamination of the aquatic ecosystem with heavy metals arising from the discharge of electronic waste (e-waste) or seepage of leachate from open waste dumpsites is on the increase. The impact of e-waste leachate on the micronucleus (MN) of Tilapia guineensis and the levels of heavy metals ( Pb, Cd, Cr, As, Hg, Al and Ba) in the sediments, water, leachate and aquatic fauna (Tilapia guineensis, Callinectes amnicola and Cardiosoma armatum) found in and around e-waste dumpsites in Lagos and Osun States was studied. The levels of heavy metals were determined using PyeUnicam model 969 Atomic Absorption Spectrophotometry and Inductive Coupling Plasma Atomic Absorption Spectrophotometry (ICP-AAS).The heavy metal concentrations decreased in the order: Pb ˃ Cd ˃ Cr ˃ Hg ˃ Al ˃Ba ˃As in the samples. The level of Cd in fish muscle from Lagos (1.4mg/kg) exceeded the Environment Canada threshold effect level (EC TEL, 2006) recommended for sea food (1.0mg/kg). The calculated bioconcentration factor (BCF) for fish gills (1.4 and1.0) and fish muscles. (3.6 and 1.2) from Lagos and Osun States respectively as well as the bioaccumulation factor (BAF) for crabs (1.78 and 1.36) are strong indications of the ability of the organisms to bioaccumulate the heavy metals. The MN assay showed an initial increase in the frequency of mionccronuclei on the 7th day which gradually decreased on the 14th and 21st day. The result of this study reveals the toxic impacts of e-waste contamination on ecological and human health in a developing economy.

Background. Heavy metal contamination has become a challenge in Nigerian cities. Objective. This review updates our current understanding of the environmental health effects of metal pollution in Nigeria. Methods. The review was conducted by systematically searching the databases of GOOGLESCHOLAR, MEDLINE, PUBMED CENTRAL, and PUBMED libraries for original research using search terms such as ‘metal pollution in Nigeria’, ‘bio-monitoring of lead, cadmium, nickel and chromium in Nigeria’, ‘lead in Nigeria’, ‘cadmium in Nigeria’, ‘nickel in Nigeria, and ‘chromium in Nigeria’. Discussion. This review highlights the need for the study of the toxicological implications of chronic, low-level exposure to heavy metals in African markets. Elevated blood lead levels (BLLs) have been documented in Nigerian populations across all ages, socioeconomic classes, sex, etc. Conclusions. There is a need to assess BLLs across populations in Nigeria by undertaking sound scientific studies using appropriate sample sizes and methodologies. In particular, information is needed on both non-occupational and occupational exposures to adults that may result in children being exposed. A comprehensive strategy is needed to reduce lead exposure in order to reduce the future economic and disease burden in Nigeria. Competing Interests. Our previous studies showed that renal proximal tubular cells (RPTC) express Ca2+-independent phospholipase A2γ (iPLA2γ) in endoplasmic reticulum (ER) and mitochondria and that iPLA2γ prevents and/or repairs lipid peroxidation induced by oxidative stress. Our present studies determined the importance of iPLA2γ in mitochondrial and cell function using an iPLA2γ-specific small hairpin ribonucleic acid (shRNA) adenovirus. iPLA2γ expression and activity were decreased in the ER by 24 h and in the mitochondria by 48 h compared with scrambled shRNA adenovirus-treated cells. Lipid peroxidation was elevated by 2-fold at 24 h and remained elevated through 72 h in cells with decreased iPLA2γ. Using electrospray ionization-mass spectrometry, primarily phosphatidylcholines and phosphatidylethanolamines were increased in iPLA2γ-shRNA-treated cells. At 48 h after exposure to the iPLA2γ shRNA, uncoupled oxygen consumption was inhibited by 25% and apoptosis was observed at 72 and 96 h. RPTC with decreased iPLA2γ expression underwent apoptosis when exposed to a nonlethal concentration of the oxidant tert-butyl hydroperoxide (TBHP). Exposure of control cells to a nonlethal concentration of TBHP induced iPLA2γ expression in RPTC. These results suggest that iPLA2γ is required for the prevention and repair of basal lipid peroxidation and the maintenance of mitochondrial function and viability, providing further evidence for a cytoprotective role for iPLA2γ from oxidative stress.

Sacro occipital technique (SOT) is a method of chiropractic that integrates whole body kinematic system care, viscerosomatic reflex therapies, and cranial/temporomandibular joint (TMJ) treatment. This would be essential before using a patient's posture to initiate chiropractic care for curve restoration or begin dental care for a patient's possible TMD and airway dysfunction. Forward head posture (FHP) tends to be mysterious, and depending on the observer (healthcare practitioner), it may be considered to have completely different causations and, hence, treatments. Considering the generally low priority given to educating consumers in Nepal and also as a part of its commitment towards consumer awareness, the Drug Information Unit of Tribhuvan University Teaching Hospital (TUTH) developed and distributed a consumer education pamphlet on basic do's and don'ts about medicines. The methodology of the development of this pamphlet included: (1) a questionnaire interview of visitors (n=604) of patients admitted to TUTH to find out the existing level of knowledge on basic facts about medicines, (2) development of a draft pamphlet, mainly based on the findings of this baseline survey, (3) pretesting of the draft pamphlet on patients’ visitors (n=614) at TUTH and subject experts (n=13) to get their comments and suggestions on the text, language, drawings and presentation, and (4) revision of the pamphlet based on the pretest and its final printing. The present work thus represents a research-based approach to the development of public education material. Although there is some understanding of the shared characteristics and predictors of psychological distress of women participating in hereditary breast and ovarian cancer registries, these same characteristics are only beginning to be identified in research on community women seeking genetic testing for BRCA1/BRCA2 gene mutations. This study provides an initial exploration of characteristics associated with family environments for 51 community women waiting to receive such genetic testing results. Thirty-four of the 36 women classified on family environment type of the Family Environment Scale (FES) were from Personal Growth-Oriented families. Comparisons of women with and without personal cancer histories resulted in a trend for women with personal cancer histories to be classified as from Independence-Oriented families. Reported distress appears to vary for different family emphases based on family and personal cancer history. A moral–religious family emphasis consistently appeared to be associated with decreased psychological distress. Preliminary analyses comparing these community women (who were not part of a hereditary registry but were self- or physician-referred) seeking genetic testing with normal and distressed family means found that increased cohesion and expressiveness may be related to decreased conflict, indicative of potentially supportive family environments for these women. When compared with normative data, a subset consisting of the Ashkenazi Jewish women showed a trend of less cohesion than normal families, but similar cohesion levels as distressed families. A cDNA, designated celF, encoding a cellulase (CelF) was isolated from the anaerobic fungus Orpinomyces PC-2. The open reading frame contains regions coding for a signal peptide, a carbohydrate-binding module (CBM), a linker, and a catalytic domain. The catalytic domain was homologous to those of CelA and CelC of the same fungus and to that of the Neocallimastix patriciarum CELA, but CelF lacks a docking domain, characteristic for enzymes of cellulosomes. It was also homologous to the cellobiohydrolase IIs and endoglucanases of aerobic organisms. The gene has a 111-bp intron, located within the CBM-coding region.
