Objective: The aim of this study was to investigate the effect of some maternal and neonatal clinical parameters on the neonatal intensive care unit admission rates of neonates born to mothers who had preeclampsia. Study Design: Study included 402 singleton pregnant women with preeclampsia who admitted to Maternal-Fetal Medicine Unit of Zeynep Kamil Children and Women’s Health Training and Research Hospital. Pregnancies with uterine rupture, chorioamnionitis and congenital malformations were excluded. Some maternal and neonatal clinical characteristics were assessed to predict neonatal intensive care unit admission. Results: Among 402 neonates, 140 (35%) of them had an indication for neonatal intensive care unit admission, among 140 neonates, 136 (97%) of them were preterm neonates. Comparison of groups with and without neonatal intensive care unit admission indicated significant differences between groups in terms of gestational age, Apgar scores at 1st and 5th minutes, birth weight, some maternal laboratory parameters (Hemoglobin, hematocrit, alanine aminotransferase, aspartate aminotransferase, albumin). In multivariate analysis, among all study population, gestational age at delivery, birth weight and Apgar scores were found to be significantly associated with neonatal intensive care unit admission. On the other hand, in subgroup of term neonates, none of the variables was shown to be associated with neonatal intensive care unit admission. Conclusion: Gestational age at delivery and the birth weight are the main risk factors for neonatal intensive care unit admission of neonates born to mothers who had preeclampsia.

Amaç: Plasenta akreata spektrumu (PAS), özellikle gelişmekte olan ülkelerde fetomaternal morbidite ve mortalitenin ana sebebi olan obstetrik kanama için önemli bir risk faktörüdür. Bu çalışmada; PAS olan ve olmayan plasenta previa olgularının özellikleri, insidansı, maternal ve fetal sonuçlarının araştırılması amaçlanmıştır. Ek olarak, PAS ile başvuran plasenta previa vakalarının klinik özellikleri ve risk faktörlerinin analiz edilmesi amaçlanmıştır.Gereç ve Yöntemler: Plasenta previa ile birlikte PAS olan ve olmayan gebelikleri analiz etmek amacıyla retrospektif kohort bir çalışma planlandı. Ocak 2005 ile Aralık 2014 arasında yazılı ve elektronik olarak tıbbi kayıtlar gözden geçirildi. Plasenta previa tanısı ultrasonografik olarak, PAS ise klinik olarak plasentanın uterustan ayrılma aşamasında zorluk olarak tanımlandı.Bulgular: Ocak 2005 ile Aralık 2014 arasında toplam 11351 doğumun retrospektif analizi yapıldı. 387 plasenta previa tanısı konulmuş olgu izlendi. Bu olguların 11'inde çoğul gebelik saptandı ve bu vakalar çalışma dışı bırakıldı. Kurumumuzda plasenta previa insidansı ‰3.41 idi. PAS riskini ≥4 gebelik sayısı 1,56 kat, ≥2 geçirilmiş sezeryan sayısı 9,74 kat, ≥3 abort sayısı 3,83 kat ve gestasyonel hipertansiyon varlığı 29,72 kat, gestasyonel diabet varlığı 49 kat arttırmıştır. Risk faktörlerinin lojistik regresyon analizinde ≥3 abort sayısı, ≥2 geçirilmiş sezaryen sayısı ve gestasyonel hipertansiyon varlığı anlamlı olarak değerlendirilmiştir.Sonuç: Plasenta previa vakalarında; ≥3 abort sayısı, geçirilmiş sezaryen sayısının ≥2 ve gestasyonel hipertansiyon varlığı PAS gelişimi açısından en önemli risk faktörleridir. Bu risk faktörlerine sahip plasenta previa olgularının gebeliği boyunca PAS gelişimi açısından sıkı takibi maternal-neonatal morbidite ve mortalite üzerine azalan bir etkiye sahip olabileceğini düşünmekteyiz.

Puerperal hematomas occur in 12 per 1000 deliveries and rarely cause maternal mortality. A 21-year-old female patient, gravida 2 parity 2, was admitted to our clinic with bilateral vulvar hematoma after a spontaneous vaginal delivery. Extensive hematomas are primarily treated with surgical debridement. Due to the consistency of hematoma after debridement, primary retention sutures were placed at the vulva and a silicon catheter was placed between the vulva and retention sutures. The patient was hospitalized for five days. The early diagnosis of puerperal hematomas is very important because many complications can be prevented with early diagnosis and treatment. Partial rotation of the uterus not more than 45 degrees to the right is considered to be normal. Since all cases are not reported, the incidence of uterine torsion in pregnancy is not known exactly. In the literature, there have been reports of cases with uterine torsion ranging from 45 to 720 degrees. This is a case report of uterine torsion of 720 degrees with accompanying bladder torsion, developing after two caesarean sections, and developing of uterine atony after the operation.
