This study investigated how data series length and gaps in human kinematic data impact the accuracy of Lyapunov exponents (LyE) calculations with and without cubic spline interpolation. Kinematic time series were manipulated to create various data series lengths (28% and 100% of original) and gap durations (0.05–0.20 s). Longer gaps generally resulted in significantly higher LyE% error values in each plane in noninterpolated data. During cubic spline interpolation, only the 0.20-second gap in frontal plane data resulted in a significantly higher LyE% error. Data series length did not significantly affect LyE% error in noninterpolated data. During cubic spline interpolation, sagittal plane LyE% errors were significantly higher at shorter versus longer data series lengths. These findings suggest that not interpolating gaps in data could lead to erroneously high LyE values and mischaracterization of movement variability. When applying cubic spline, a long gap length (0.20 s) in the frontal plane or a short sagittal plane data series length (1000 data points) could also lead to erroneously high LyE values and mischaracterization of movement variability. These insights emphasize the necessity of detailed reporting on gap durations, data series lengths, and interpolation techniques when characterizing human movement variability using LyE values. Context: Early identification of incoming military personnel at elevated odds for bone stress injury (BSI) is important for the health and readiness of the US military. Design: Prospective cohort study. Methods: Knee kinematic data of the incoming US Military Academy cadets were collected while performing a jump-landing task (The Landing Error Scoring System) using a markerless motion capture system and depth camera. Data on incidence of lower-extremity injury, including BSI, were collected throughout the study period. Results: A total of 1905 participants (452 females, 23.7%) were examined for knee valgus and BSI status. A total of 50 BSI occurred during the study period (incidence proportion = 2.6%). The unadjusted odds ratio for BSI at initial contact was 1.03 (95% confidence interval [CI], 0.94–1.14; P =.49). Adjusted for sex, the odds ratio for BSI at initial contact was 0.97 (95% CI, 0.87–1.06; P =.47). At the instant of maximum knee-flexion angle, the unadjusted odds ratio was 1.06 (95% CI, 1.02–1.10; P =.01), and the odds ratio was 1.02 (95% CI, 0.98–1.07; P =.29) after adjusting for sex. This suggests that there was not a significant enough association for an increase in the odds of BSI based on either degree of knee valgus. Conclusions: Our results did not demonstrate an association between knee valgus angle data during a jump-landing task and future increased odds of BSI in a military training population. Further analysis is warranted, but the results suggests the association between kinematics and BSI cannot be effectively screened by knee valgus angle data in isolation. [ABSTRACT FROM AUTHOR]

Soccer, one of the most popular sports in the world, has one of the highest rates of sports-related concussions. Additionally, soccer players are frequently exposed to nonconcussive impacts from intentionally heading the ball, a fundamental component of the sport. There have been many studies on head impact exposure in soccer, but few focus on soccer practices or practice activities. This study aimed to characterize the frequency and magnitude of head impacts in National Collegiate Athletic Association Division I female soccer practice activities using a custom-fit instrumented mouthpiece. Sixteen players were instrumented over the course of 54 practice sessions. Video analysis was performed to verify all mouthpiece-recorded events and classify practice activities. Category groupings of practice activities include technical training, team interaction, set pieces, position-specific, and other. Differences in head impact rates and peak resultant kinematics were observed across activity types and category groupings. Technical training had the highest impact rate compared to other category groupings. Impacts occurring during set piece activities had the highest mean kinematic values. This study compared lower extremity, trunk, and upper extremity kinematics between tee and front toss hitting in youth baseball athletes. Twenty youth baseball athletes (14.3±2.9 yrs) performed three maximal effort swings off front toss and tee. Kinematic data were collected during the preparatory and acceleration phases. Lower extremity, trunk, and upper extremity kinematics were compared between tee and front toss hitting using 1-dimensional statistical parametric mapping (SPM). There was a significant difference in trunk kinematics between tee and front toss during the preparatory phase (p=.001); the trunk rotated more toward the back side when hitting off a tee compared to front toss (p<0.001). There was also a significant difference in trunk kinematics between tee and front toss for 67% of the acceleration phase; the trunk rotated more towards the back side from 0 to 67% when hitting off the tee (p<0.001). Significant differences were found in trunk kinematics between tee and front toss hitting in youth baseball players, where the trunk is less rotated toward the pitcher in the tee than in the front toss. Coaches utilize various training modalities to enhance hitting performance; however, differences in trunk kinematics should be considered between modalities when developing fundamental hitting techiques in youth baseball athletes. A common cause of traumatic brain injuries (TBIs) is the head’s kinematic response to rapid movement, which can be reduced with dynamic neck strengthening. This study aimed to determine the most effective neck-strengthening program by comparing a traditional and nontraditional program. Isometric neck strength was assessed in 32 subjects randomly assigned to one of the traditional and nontraditional neck-strengthening programs. The nontraditional program used a novel neck-strengthening device. After weeks 6 and 10 of training, isometric neck strength was reassessed. With the collected data, linear mixed models were established to compare the changes in neck strength between the 2 groups during the 10-week training period. Statistical analysis results suggest that, for both cervical extension (CE) and cervical flexion (CF), subjects in the novel neck-strengthening device group had a significantly higher gain of strength during the 10 weeks than the traditional group. With test statistics of 22.691 and 23.203 and corresponding 2-sided p-value of 0.01289 and 0.003889, respectively, we conclude that there is a statistically significant difference in the linear slopes of increase for both CE and CF between the 2 groups. As to left cervical lateral flexion and right cervical lateral flexion, the novel neck strength group had increased strength gains compared with the traditional group. However, the increase was not enough to demonstrate significant findings. Results were considered significant at p, 0.05. Altering running cadence is commonly done to reduce the risk of running-related injury/reinjury. This study examined how altering running cadence affects joint kinetic patterns and stride-to-stride kinetic variability in uninjured female runners. Twenty-four uninjured female recreational runners ran on an instrumented treadmill with their typical running cadence and with a running cadence that was 7.5% higher and 7.5% lower than typical. Ground reaction force and kinematic data were recorded during each condition, and principal component analysis was used to capture the primary sources of variability from the sagittal plane hip, knee, and ankle moment time series. Runners exhibited a reduction in the magnitude of their knee extension moments when they increased their cadence and an increase in their knee extension moments when they lowered their cadence compared with when they ran with their typical cadence. They also exhibited greater stride-to-stride variability in the magnitude of their hip flexion moments and knee extension moments when they deviated from their typical running cadence (ie, running with either a higher or lower cadence). The objective was to examine the efficacy of the Get aHEAD Safely in Soccer™ intervention on head impact kinematics and neck strength in female youth soccer players. The control group (CG) consisted of 13 players (age: 11.0 ± 0.4 yrs), while the experimental group (EG) consisted of 14 players (age: 10.6 ± 0.5 yrs). Head impact kinematics included peak linear acceleration (PLA), peak rotational acceleration (PRA), and peak rotational velocity (PRV). Pre- and post-season measures included strength measures of neck/torso flexion (NF/TF) and extension (NE/TE). Data were analysed using a multilevel linear model and ANOVA techniques. No differences in PLA, PRA, or PRV were observed between groups. The EG showed significant improvement in NF strength while the CG showed significant improvement in NE strength. Both groups significantly improved in TF pre- to post-season. The foundational strength components of the Get aHEAD Safely in Soccer program appear to show a benefit in youth soccer players beginning to learn the skill of purposeful heading. BACKGROUND: Work-related low back pain (LBP) increases the workforce disability and healthcare costs. This study evaluated the LBD risk level associated with handling the ACGIH TLVs in lifting tasks corresponding to various horizontal and vertical zones. OBJECTIVE: The aim of this study was to compare the low-risk ACGIH TLV to risk outcomes from various validated lifting assessment methods, including the OSU LBD Risk Model, NIOSH Lifting Equation, and LiFFT. METHODS: Twenty-four subjects were recruited for this study to perform various lifting conditions. The various ergonomic assessment methods were then used to obtain the risk assessment outcomes. RESULTS: The selected assessment methods showed that the ACGIH-defined TLVs are associated with less than high-risk for LBD for all the assessed tasks. The findings showed a moderate agreement (Kendall's W = 0.477) among the various assessment methods risk outcomes. The highest correlation (ρ= 0.886) was observed between the NIOSH Lifting Equation and LiFFT methods risk assessment outcomes. CONCLUSION: The findings showed that ACGIH-defined TLVs possesses less than high-risk for LBD. The outcomes of the selected ergonomic assessment methods moderately agree to each other. The label 'Stop' potentially generates conflict—signifying important corrective action, or a warning not to touch. To examine potential conflict between an incongruent label (i.e. STOP) and an imperative command (i.e. MOVE!), 18 participants used a computer mouse to move a crosshair cursor to targets with superimposed labels. Trials systematically varied Imperative (blank or MOVE!), Label (+GO + or STOP) and movement Distance. Kinematic analyses examined response latency, movement duration and accuracy. Incongruent labels had little impact upon response latencies, but they affected cursor deceleration and the variability of cursor placement. Although reading is assumed to be obligatory, the impact of written labels is not immediate, instead affecting cursor deceleration. Indeed, responses to controls labelled STOP were less accurate than those labelled + GO+. As labelled interfaces can create error versus command confusions, enhancing the discriminability of controls to afford more obvious visible cues as to method of use is recommended. Practitioner summary: Emergency stop and shutdown controls can cause response conflict as their labels signify both urgent corrective actions and 'don't touch'. Response conflict caused by confusing superimposed labels is resolved as cursors near the target control and may result in reduced movement accuracy. Changes in forefoot bending stiffness have been shown to affect metatarsophalangeal peak bending angles as well as athletic performance. Increasing bending stiffness tends to reduce peak bending angles, which could potentially reduce hyperextension injuries such as turf toe. Limited information is available, however, on the efficacy of increasing forefoot bending stiffness on large-sized athletes such as those that participate in American Football, with prior studies being conducted on smaller athlete populations. Therefore, the purpose of this study was to determine the influence of increased forefoot bending stiffness on metatarsophalangeal joint extension and athletic performance of grid-iron football players. Ten varsity grid-iron football players performed four National Football League combine movements in a motion capture laboratory in three footwear conditions of varying bending stiffness: Soft (12.7 N/mm), Control (23.8 N/mm), Stiff (42.2 N/mm). None of the footwear conditions significantly altered the maximum metatarsophalangeal bending. Therefore, to reduce metatarsophalangeal hyperextension injuries in American football players a greater amount of forefoot bending stiffness may be required. Performance differences were present only during the five-metre sprint acceleration, with athletes having an improved performance in the Control and Stiff conditions. Background: Baseball pitchers employ various contralateral trunk tilt (CTT) positions when pitching depending on if they have an overhand, three-quarter, or sidearm delivery. There are no known studies that have examined how pitching biomechanics are significantly different in professional pitchers with varying amounts of CTT, which may provide insight into shoulder and elbow injury risk among pitchers with different CTT. Purpose: To assess differences in shoulder and elbow forces and torques and baseball pitching biomechanics in professional pitchers with maximum 30° to 40° CTT (MaxCTT), moderate 15° to 25° CTT (ModCTT), and minimum 0° to 10° CTT (MinCTT). Study Design: Controlled laboratory study. Methods: In total, 215 pitchers were examined, including 46 pitchers with MaxCTT, 126 pitchers with ModCTT, and 43 pitchers with MinCTT. All pitchers were tested using a 240-Hz, 10-camera motion analysis system, and 37 kinematic and kinetic parameters were calculated. Differences in kinematic and kinetic variables among the 3 CTT groups were assessed with a 1-way analysis of variance (P <.01). Results: Maximum shoulder anterior force and maximum elbow proximal force were significantly greater in ModCTT (403 ± 79 N) than MaxCTT (369 ± 75 N) and MinCTT (364 ± 70 N), while maximum elbow flexion torque and shoulder proximal force, respectively, were significantly greater in ModCTT (69 ± 11 N·m and 1176 ± 152 N, respectively) than MaxCTT (62 ± 12 N·m and 1085 ± 119 N, respectively). During arm cocking, maximum pelvis angular velocity was greater in MinCTT than MaxCTT and ModCTT, and maximum upper trunk angular velocity was greater in MaxCTT and ModCTT than MinCTT. At ball release, trunk forward tilt was greater in MaxCTT and ModCTT than MinCTT and greater in MaxCTT than ModCTT, while arm slot angle was less in MaxCTT and ModCTT than MinCTT and less in MaxCTT than ModCTT. Conclusion: The greatest shoulder and elbow peak forces occurred in ModCTT, which occurs in pitchers who throw with a three-quarter arm slot. More research is needed to assess if pitchers with ModCTT are at a higher risk of shoulder and elbow injury compared with pitchers with MaxCTT (overhand arm slot) and MinCTT (sidearm arm slot), although in the pitching literature, excessive elbow and shoulder forces and torques have been shown to correlate with elbow and shoulder injuries. Clinical Relevance: The results from the current study will help clinicians better understand if differences in kinematic and kinetic measures differ with pitching, or if differences in force, torque, and arm position occur at different arm slots. The purpose of this study was to evaluate the influence of technical and core stability parameters on rowing ergometer performance defined as mean power at the handle. Twenty-four high-level rowers were evaluated at their competitive stroke rate on an instrumented RowPerfect 3 ergometer to determine leg, trunk and arm power output, while trunk and pelvis 3D kinematics were measured. Linear mixed models revealed that mean power at the handle was predicted by the power output of legs, trunk and arms (r2 = 0.99), with trunk power being the best predictor. Time to peak power, work ratio and mean to peak power ratio were relevant technical parameters significantly predicting the different segments' power. In addition, a greater trunk range of motion significantly influenced the power produced by this segment. Accordingly, achieving an earlier peak power together with enhanced work production at the trunk and arm levels, as well as distributing the segments power over the whole drive phase, could serve as recommendations for technical training of rowers on dynamic ergometers in order to produce higher power output. Physical function decline due to aging or disease can be assessed with quantitative motion analysis, but this currently requires expensive laboratory equipment. We introduce a self-guided quantitative motion analysis of the widely used five-repetition sit-to-stand test using a smartphone. Across 35 US states, 405 participants recorded a video performing the test in their homes. We found that the quantitative movement parameters extracted from the smartphone videos were related to a diagnosis of osteoarthritis, physical and mental health, body mass index, age, and ethnicity and race. Our findings demonstrate that at-home movement analysis goes beyond established clinical metrics to provide objective and inexpensive digital outcome metrics for nationwide studies. Purpose: This study aimed to identify associations between dorsiflexion range of motion (DFROM), functional hop test performance, and hopping biomechanics with the magnitude of talar cartilage deformation after a standardized hopping protocol in individuals with and without chronic ankle instability (CAI). Methods: Thirty CAI and 30 healthy individuals participated. Ankle DFROM was assessed using the weight-bearing lunge test. Four different functional hop tests were assessed. Three-dimensional kinematics and kinetics were sampled during a 60-cm single-leg hop. We calculated cartilage deformation after a dynamic loading protocol consisting of sixty 60-cm single-leg forward hops by assessing the change in average thickness for the overall, medial, and lateral talar cartilage. Linear regressions examined the associations between cartilage deformation magnitude and DFROM, functional hop tests, and hop biomechanical variables after accounting for body weight and time since the initial ankle sprain. Results: In CAI group, lesser static DFROM (Δ R 2 = 0.22) and smaller peak ankle dorsiflexion angle (Δ R 2 = 0.17) was associated with greater medial deformation. Greater peak vertical ground reaction force (vGRF) (Δ R 2 = 0.26–0.28) was associated with greater medial and overall deformation. Greater vGRF loading rate (Δ R 2 = 0.23–0.35) was associated with greater lateral and overall deformation. Greater side hop test times (Δ R 2 = 0.31–0.36) and ankle plantarflexion at initial contact (Δ R 2 = 0.23–0.38) were associated with greater medial, lateral, and overall deformation. In the control group, lesser side hop test times (Δ R 2 = 0.14), greater crossover hop distances (Δ R 2 = 0.14), and greater single-hop distances (Δ R 2 = 0.21) were associated with greater overall deformation. Conclusions: Our results indicate that lesser static DFROM, poorer functional hop test performance, and hop biomechanics associate with greater talar cartilage deformation after a dynamic loading protocol in those with CAI. Purpose: The purpose of this study was to examine the relationship between whole body and throwing arm composition and length with throwing shoulder distraction and anterior force. Literature shows a relationship between shoulder force and pain; therefore, as well as between body mass and pain, it was hypothesized that pitchers with greater body fat mass would display increased throwing shoulder force during the acceleration phase of the softball pitch. Methods : Thirty-eight high school softball pitchers from southern United States agreed to participate (1.69 +/- 0.07 m, 74.71 +/- 16.36 kg, 15.2 +/- 1.1 yr). All procedures were completed within one visit. Pitchers underwent a dual-energy x-ray absorptiometry scan then threw fastballs at regulation distance while motion was measured using an electromagnetic motion capture system. Data from the three fastest pitches were averaged and analyzed. Peak shoulder distraction and anterior force during the acceleration phase of the softball pitch were extracted and used as dependent variables in two separate regression analyses. Whole body and throwing arm fat mass and lean mass, as well as throwing arm length were entered as independent variables into both regression equations. Results : The first regression equation was statistically significant (F (5,37) = 13.28, P < 0.001) and explained approximately 62% of the variance in peak shoulder distraction force (R 2 = 0.675, adjusted R 2 = 0.624). Increased whole body fat mass (B = 13.76, SE = 5.39, [beta] = 0.91, t = 2.553, P = 0.016) significantly predicted peak throwing shoulder distraction force. The second regression equation predicting peak anterior force was not significant. Ladder training is a form of multidirectional lower limb plyometric training utilized by coaches and athletes in a variety of sports. The purpose of this study was to examine the effects of ladder training (LT) on sprint (20-m) and change of direction (COD) (L-drill) kinematics. Fourteen NCAA D-2 Basketball players were matched on baseline performance rankings and randomly assigned to a LT (n = 7) or conventional training (CT) (n = 7) group. The LT group performed all the CT exercises with the addition of 25-35 minutes of LT performed 3 times per week, for 4 weeks. Within-group analyses showed significant improvements (p < 0.001) in 20-m sprint performance from pretest to posttest in LT (+6.71%) and CT (+2.16%). No statistically significant difference was found in COD performance. Significant enhancements (p < 0.005) were found in stride frequency from pretest to posttest for 20-m sprint and COD as a result of LT (+13.58%, and +12.26%) and CT (+0.97%, and -2.33%), respectively. LT resulted in substantially better results (between-group changes) in both the 20-m sprint (ES = 1.45) and COD test (ES = 0.97). Furthermore, LT resulted in substantially greater enhancements in stride frequency (ES = 2.43, 1.65), and ground contact time (ES = 1.82, 1.25) in the 20-m sprint and COD performance respectively. LT may be more effective than CT in improving performance and kinematics. LT should be implemented as a warm up or neural priming exercise to induce improvements in stride frequency and ground contact time. [ABSTRACT FROM AUTHOR]

On-slope pilot testing of snow tubes was conducted at two ski areas in the United States to examine the effects of deceleration mats. Snow tube and rider kinematics were measured using an instrumented bodysuit and a GPS system worn by the rider. For each test, the riders descended a tubing run with minimal input and stopped in the run-out area. Snow tube and rider speeds when entering the run-out area were controlled to be approximately 9.5 m/s. Test trials were conducted with and without deceleration mats. Four deceleration mat conditions were tested, including two raised surface protuberances (ribs and projections) and two mat geometry parameters (flat and folded). The deceleration and effective coefficient of friction (COF) were determined for each trial. Data were recorded for 75 test trials with a mean (± standard deviation) speed entering the run-out area of 9.5 (±1.8) m/s. There were no significant differences in the deceleration or effective coefficient of friction between the surface protuberance conditions. The peak deceleration and effective COF for the folded mats (5.1 ± 1.6 m/s2 and 0.26 ± 0.14) was greater than for the flat (3.3 ± 0.8 m/s2 and 0.10 ± 0.07) and no mat (0.06 ± 0.3 m/s2 and 0.08 ± 0.03) conditions (all p < 0.05). Deceleration mats in run-out areas slow snow tube riders faster than without deceleration mats. Background: The estimated cost per year of injuries in collegiate athletics has been reported to be billions of dollars in the United States. Injury prevention programs are often assessed only by their ability to reduce injuries, and there is little evidence of any potential reduction in associated health care costs. Purpose: To investigate changes in injury-related health care costs at a National Collegiate Athletic Association (NCAA) Division I university after the implementation of an injury prevention program. Study Design: Cohort study; Level of evidence, 3. Methods: Included were 12 sports teams that used the injury prevention program (user group) and 16 teams that did not implement the program (nonuser group). The injury surveillance and prevention system (Sparta Science) utilized a commercially available force-plate system to assess kinematic variables, flag high-risk athletes, and guide individual conditioning programs. Data were obtained from 3 academic years before (2012-2014) and 2 academic years after (2015-2016) implementation of the Sparta Science system. The number of injuries and associated health care costs (surgery, clinic visits, imaging, and physical therapy) were compared between users and nonusers. Results: Total average annual injuries did not change significantly between users and nonusers after implementation of the program; however, users demonstrated a 23% reduction in clinic visits as compared with a 14% increase for nonusers (P =.049). Users demonstrated a 13% reduction in associated health care encounters, compared with a 13% increase for nonusers (P =.032). Overall health care costs changed significantly for both groups, with an observed 19% decrease ($2,456,154 to $1,978,799) for users and an 8% increase ($1,177,542 to $1,270,846) for nonusers (P <.01 for both). Costs related to associated health care encounters also decreased by 20% for users as compared with a 39% increase for nonusers (P =.027). Conclusion: This study demonstrated the ability to significantly reduce injury-related health care costs in NCAA Division I athletes via a comprehensive injury surveillance and prevention program utilizing force-plate technology. Given the substantial and appropriate focus on value of care delivery across the US health care system, we recommend the continued study of sports injury surveillance and prevention programs for reducing injury-related health care costs. BACKGROUND: The excessive use of hand-held mobile devices (HHMD) leads to a postural phenomenon known as text neck. OBJECTIVE: The aim of this paper is to discuss the anatomical, biomechanical and muscle activation changes within the cervical and thoracic regions associated with the sustained, forward, flexed neck posture, observed with excessive usage of hand-held mobile devices. Additionally, this paper examines the relationship of gender, as well as the effects of carrying backpack loads by youth, on this forward, flexed neck posture. METHODS: Multiple aspects of the text neck position that occur when an individual uses a HHMD are described. RESULTS: Prolonged use of hand-held mobile devices results in adverse anatomical and biomechanical changes in the cervical and thoracic spine, muscular imbalances, and postural compensations, all of which contribute to muscular overuse and fatigue resulting in pain. CONCLUSIONS: Physical therapists must educate their patients about proper posture while using hand-held mobile devices. Proper posture includes: holding the device close to eye level, using the device while standing or sitting and holding the device with a line of sight perpendicular to the surface of the device, using a larger screen, and texting with both hands. Also, because children are using hand held mobile devices at younger ages, parents and teachers must be educated about the dangers of prolonged use of hand-held devices. [ABSTRACT FROM AUTHOR]

Background: In-season return to play after anterior glenohumeral instability is associated with high rates of recurrent instability and the need for surgical stabilization. We are not aware of previous studies that have investigated in-season return to play after posterior glenohumeral instability; furthermore, as posterior shoulder instability in collision athletes occurs frequently, understanding the expected outcome of in-season athletes may improve the ability of physicians to provide athletes with a better understanding of the expected outcome of their injury and their ability to return to sport.Questions/purposes: (1) What proportion of athletes returned to play during the season after posterior instability in collegiate football players? (2) How much time did athletes lose to injury, what proportion of athletes opted to undergo surgery, and what proportion of athletes experienced recurrent instability after a posterior instability episode during a collegiate football season?Methods: A multicenter, prospective, observational study of National Collegiate Athletic Association (NCAA) Division 1 Football Bowl Subdivision athletes was performed at three US Military Service Academies. Ten athletes who sustained a posterior instability event during the regular football season and who pursued a course of nonoperative treatment were identified and prospectively observed through the subsequent season. All athletes in the observed cohort attempted an initial course of nonoperative treatment during the season. All athletes sustained subluxation events initially identified through history and physical examination at the time of injury. None of the athletes sustained a dislocation event requiring a manual reduction. Intraarticular pathology consisting of posterior labral pathology was further subsequently identified in all subjects via MRI arthrogram. Return to play was the primary outcome of interest. Time lost to injury, surgical intervention, and subsequent instability were secondary outcomes.Results: Of the 10 athletes who opted for a trial of initial nonoperative management, seven athletes were able to return to play during the same season. Although these seven athletes returned within 1 week of their injury (median of 1 day), 5 of 7 athletes sustained recurrent subluxation events with a median (range) of four subluxation events per athlete (0 to 8) during the remainder of the season. Seven athletes were treated surgically after the completion of their season, four of whom returned to football.Conclusion: This study suggests that although collegiate football players are able to return to in-season sport after a posterior glenohumeral instability event, they will likely sustain multiple recurrent instability events and undergo surgery after the season is completed. The results of this study can help guide in-season management of posterior shoulder instability by allowing more appropriate postinjury counseling and decision making through the identification of those athletes who may require additional attention from medical staff during the season and possible modifications to training regimens to minimize long-term disability. Further prospective studies involving a larger cohort over several seasons should be performed through collaborative studies across the NCAA that better assess function and injury risk factors before beginning collegiate athletics. The purpose of this study was to examine the combined impact of experience and decision making on frontal plane knee joint biomechanics during a cutting maneuver. Kinematic and kinetic data were collected from 12 recreationally active and 18 NCAA Division I female athletes during execution of anticipated and unanticipated single-leg land-and-cut maneuvers. Knee joint abduction angles and external knee joint abduction torques were calculated and discrete peak stance-phase variables were extracted. Angle and torque time-series data were also submitted to separate functional data analyses. Variables derived from the functional data analyses indicated that decision making influenced knee abduction angle and torque time series in the recreational group only. Specifically, these variables pointed to greater knee abduction at the end of stance as well as a greater, albeit delayed peak in knee abduction torque at the beginning of landing in the recreational athletes during the unanticipated condition. In addition, the recreational athletes displayed greater discrete peak knee abduction angles than the Division I athletes regardless of condition. An improved understanding of common differences between tornadic and nontornadic supercells is sought using a large set of observations from the operational NEXRAD WSR-88D polarimetric radar network in the contiguous United States. In particular, data from 478 nontornadic and 294 tornadic supercells during a 7-yr period (2011–17) are used to produce probability-matched composite means of microphysical and kinematic variables. Means, which are centered on echo-top maxima and in a horizontal coordinate system rotated such that storm motion points in the positive x dimension, are created in altitude relative to ground level at times of peak echo-top altitude and peak midlevel rotation for nontornadic supercells and times at and prior to the first tornado in tornadic supercells. Robust differences between supercell types are found, with consistent characteristics at and preceding tornadogenesis in tornadic storms. In particular, the mesocyclone is found to be vertically aligned in tornadic supercells and misaligned in nontornadic supercells. Microphysical differences found include a low-level radar reflectivity hook echo aligned with and ~10 km right of storm center in tornadic supercells and displaced 5–10 km down-motion in nontornadic supercells, a low-to-midlevel differential radar reflectivity dipole that is oriented more parallel to storm motion in tornadic supercells and more perpendicular in nontornadic supercells, and a separation between enhanced differential radar reflectivity and specific differential phase (with unique displacement-relative correlation coefficient reductions) at low levels that is more perpendicular to storm motion in tornadic supercells and more parallel in nontornadic supercells. Background: When pitching a baseball, pelvic and trunk pitching kinematics play an integral role in momentum transfer from the lower extremity to the upper extremity. However, it is unknown how hip and trunk ROM and hip shoulder separation interplay with pelvic and trunk pitching kinematics. Hypothesis/Purpose: To determine the relationship between clinical trunk and hip range of motion (ROM) and pitching biomechanical pelvis and trunk kinematics, and kinematic sequencing. Study Design: Controlled biomechanical study Methods: High school pitchers were assessed for trunk rotation via motion capture and hip ROM via a goniometer prior to pitching. Trunk rotation was designated as dominant and non-dominant sides, and hips as stance and lead limbs. Pitchers threw four fastballs during three dimensional biomechanical assessment. Spearman's Rho correlations were performed between trunk and hip ROM, and trunk and hip biomechanical kinematics, and kinematic pitching sequence. Results: Thirty-two pitchers (mean age: 16.3 ± 1.2 years, height = 184.0 ± 6.9 cm, mass = 76.8 ± 20.8 kg) were included in this study. Their mean pitch velocity was 34.7 ± 2.3 m/s, peak pelvis rotation velocity: 669.1 ± 95.5 deg/s, and peak trunk rotation velocity: 1084.7 ± 93.0 deg/s. There were no differences between dominant and non-dominant side trunk rotation, or between stance and lead hip ROM. There were no significant relationships between trunk or hip ROM and pitching kinematics. There was a significant relationship between hip shoulder separation and peak trunk rotation velocity (r = 0.390, p=0.027). There was a significant relationship between pitch velocity and peak trunk rotation velocity (r = 0.478, p = 0.006). There were no other significant relationships between pitching kinematics or kinematic sequencing. Conclusion: Hip shoulder separation is related to trunk rotation velocity, and ultimately pitch velocity. Professional American football games are recorded in digital video with multiple cameras, often at high resolution and high frame rates. The purpose of this study was to evaluate the accuracy of a videogrammetry technique to calculate translational and rotational helmet velocity before, during and after a helmet impact. In total, 10 football impacts were staged in a National Football League (NFL) stadium by propelling helmeted 50th percentile male crash test dummies into each other or the ground at speeds and orientations representative of concussive impacts for NFL players. The tests were recorded by experienced sports film crews to obtain video coverage and quality typically available for NFL games. A videogrammetry procedure was used to track the position and rotation of the helmet throughout the relevant time interval of the head impact. Compared with rigidly mounted retroreflective marker three dimensional (3-D) motion tracking that was concurrently collected in the experiments, videogrammetry accurately calculated changes in translational and rotational velocity of the helmet using high frame rate (two cameras at 240 Hz) video (7% and 15% error, respectively). Low frame rate (2 cameras at 60 Hz) video was adequate for calculating pre-impact translational velocity but not for calculating the translational or rotational velocity change of the helmet during impact. Background: Some cadaveric studies have indicated that the anterior cruciate ligament (ACL) consists of anteromedial and posterolateral bundles that display reciprocal function with regard to knee flexion. However, several in vivo imaging studies have suggested that these bundles elongate in parallel with regard to flexion. Furthermore, the most appropriate description of the functional anatomy of the ACL is still debated, with the ACL being described as consisting of 2 or 3 bundles or as a continuum of fibers. Hypothesis: As long as their origination and termination locations are defined within the ACL attachment site footprints, ACL bundles elongate in parallel with knee extension during gait. Study Design: Descriptive laboratory study. Methods: High-speed biplanar radiographs of the right knee joint were obtained during gait in 6 healthy male participants (mean ± SD: body mass index, 25.5 ± 1.2 kg/m2; age, 29.2 ± 3.8 years) with no history of lower extremity injury or surgery. Three-dimensional models of the right femur, tibia, and ACL attachment sites were created from magnetic resonance images. The bone models were registered to the biplanar radiographs, thereby reproducing the in vivo positions of the knee joint. For each knee position, the distances between the centroids of the ACL attachment sites were used to represent ACL length. The lengths of 1000 virtual bundles were measured for each participant by randomly sampling locations on the attachment site surfaces and measuring the distances between each pair of locations. Spearman rho rank correlations were performed between the virtual bundle lengths and ACL length. Results: The virtual bundle lengths were highly correlated with the length of the ACL, defined as the distance between the centroids of the attachment sites (rho = 0.91 ± 0.1, across participants; P < 5 × 10-5). The lengths of the bundles that originated and terminated in the anterior and medial aspects of the ACL were positively correlated (rho = 0.81 ± 0.1; P < 5 × 10-5) with the lengths of the bundles that originated and terminated in the posterior and lateral aspects of the ACL. Conclusion: As long as their origination and termination points are specified within the footprint of the attachment sites, ACL bundles elongate in parallel as the knee is extended. PURPOSE: Performance status (PS) is a key factor in oncologic decision making, but conventional scales used to measure PS vary among observers. Consumer-grade biometric sensors have previously been identified as objective alternatives to the assessment of PS. Here, we investigate how one such biometric sensor can be used during a clinic visit to identify patients who are at risk for complications, particularly unexpected hospitalizations that may delay treatment or result in low physical activity. We aim to provide a novel and objective means of predicting tolerability to chemotherapy. METHODS: Thirty-eight patients across three centers in the United States who were diagnosed with a solid tumor with plans for treatment with two cycles of highly emetogenic chemotherapy were included in this single-arm, observational prospective study. A noninvasive motion-capture system quantified patient movement from chair to table and during the get-up-and-walk test. Activity levels were recorded using a wearable sensor over a 2-month period. Changes in kinematics from two motion-capture data points pre- and post-treatment were tested for correlation with unexpected hospitalizations and physical activity levels as measured by a wearable activity sensor. RESULTS: Among 38 patients (mean age, 48.3 years; 53% female), kinematic features from chair to table were the best predictors for unexpected health care encounters (area under the curve, 0.775 ± 0.029) and physical activity (area under the curve, 0.830 ± 0.080). Chair-to-table acceleration of the nonpivoting knee (t = 3.39; P =.002) was most correlated with unexpected health care encounters. Get-up-and-walk kinematics were most correlated with physical activity, particularly the right knee acceleration (t = −2.95; P =.006) and left arm angular velocity (t = −2.4; P =.025). CONCLUSION: Chair-to-table kinematics are good predictors of unexpected hospitalizations, whereas the get-up-and-walk kinematics are good predictors of low physical activity. [ABSTRACT FROM AUTHOR]

Agricultural work is associated with increased risk of adverse musculoskeletal health outcomes. The purpose of this study was to quantify exposure to biomechanical factors among a sample (n = 55) of farmers in the Midwest region of the U.S. while they performed a variety of routine agricultural activities, and to compare exposure levels between these activities. Surface electromyography was used to estimate activity levels of the erector spinae, upper trapezius, forearm flexor, and forearm extensor muscle groups. Simultaneously, inertial sensors were used to measure kinematics of the trunk, upper arm, and wrist. In general, lower muscle activity levels, less extreme postures, and slower movement speeds were observed during activities that involved primarily the use of agricultural machinery in comparison to manual activities, suggesting a potential advantage of mechanization relative to musculoskeletal health. Median wrist movement speeds exceeding recently proposed exposure thresholds were also observed during many manual activities, such as milking animals and repairing equipment. Upper arm postures and movement speeds did not appear to confer excessive risk for shoulder-related outcomes (on the whole), but interpretation of the results is limited by a sampling approach that may not have captured the full extent of exposure variation. Not surprisingly, substantial variation in exposure levels were observed within each agricultural activity, which is related to substantial variation in the equipment, tools, and work practices used by participants. Ultimately, the results of this study contribute to an emerging literature in which the physical demands of routine agricultural work have been described on the basis of sensor-based measurements rather than more common self-report or observation-based approaches. Clinical measurement; 22 subjects with no upper limb disability completed the Jebsen-Taylor Hand Function Test (JHFT). To realize the potential of 3D motion capture to augment evaluation of individuals with upper limb disability/impairment, it is important to understand the expected kinematic motion that characterizes performance during functional evaluation. To assess kinematic variability and establish kinematic patterns for the JHFT. Upper body joint kinematics were collected using a Vicon motion capture system. Average range of motion and maximum angle were calculated for all tasks. Intrasubject and intersubject variability were assessed by calculating Pearson's correlation coefficient, adjusted coefficient of multiple correlation (CMC adj), and standard deviation for 10 joint angles at the wrist, elbow, shoulder, and torso. The writing and picking up small objects tasks generally had high intrasubject variability, with most joint angles having median Pearson's correlation coefficients lower than 0.7. The CMC adj values were generally greater than 0.5 for elbow, shoulder, and torso joints during can-lifting tasks, indicating high consistency in those kinematic trajectories across subjects. Low consistency across subjects in all joint angles was observed for writing (CMC adj < 0.07; SD max > 10°). Kinematic patterns for the JHFT tasks were analyzed. With kinematic patterns for the JHFT tasks analyzed, optimal patterns of activity performance can be defined, allowing for easier identification and adjustment of atypical motion. Background: Unstable footwear designs are popular as training devices to strengthen human neuromuscular control, and many studies have evaluated their effect on gait parameters in comparison to conventional footwear designs. However, there is minimal research on variability of gait measures during walking with unstable shoes. Therefore, the study objective was to compare variability of gait measures between stable and unstable shoe configurations, in conjunction with kinematic and kinetic changes.Methods: Fifteen healthy male subjects walked in both a stable and unstable footwear device configuration while full-body gait kinematic and kinetic data was collected. Averages and standard deviations of gait trials were compared between the two configurations at different stages of each step.Results: Comparison of gait variability between both footwear configurations revealed that variability of frontal-plane foot center of pressure offset, transverse-plane ankle moment, and frontal-plane shoulder angle decreased significantly while walking in the unstable configuration, while transverse-plane spine angle variability increased. No changes in variability of gait measures at the knee, hip, or pelvis were observed. Kinematic and kinetic changes were observed throughout the whole body with the unstable shoe.Conclusion: Our findings suggest that the unstable device used in the study may reduce gait variability at the two extremes of the kinematic chain (i.e., foot, ankle, and shoulders), but increase variability of spine rotation angle. This may suggest a compensatory mechanism to maintain both stability and adaptability, and may have potential clinical implications for gait retraining and enhancing dynamic gait stability and joint stability, pending further investigation. Vehicle run-off-road is one of the most frequent and fatal traffic accidents in the United States. Various lane-keeping assistance (LKA) systems have been developed in the last decade to help drivers stay on the road. Most of them are built upon linear driver–vehicle–road (DVR) models and treat road curvature as a disturbance. Albeit effective, their control performance would degrade if road curvature varies rapidly. This paper proposes a novel nonlinear DVR model by integrating a driver steering model into vehicle–road kinematics, which explicitly considers road curvature. Particularly, this nonlinear DVR system has been proven to be differentially flat, and a flatness-based LKA system is designed. Additionally, Model-Free Control is introduced to compensate for system modeling errors. Objective The aim of this study was to explore whether the Variable Lifting Index (VLI) can be corrected for cumulative mass and thus test its efficacy in predicting the risk of low-back pain (LBP). Background A validation study of the VLI method was published in this journal reporting promising results. Although several studies highlighted a positive correlation between cumulative load and LBP, cumulative mass has never been considered in any of the studies investigating the relationship between manual material handling and LBP. Method Both VLI and cumulative mass were calculated for 2,374 exposed subjects using a systematic approach. Due to high variability of cumulative mass values, a stratification within VLI categories was employed. Dummy variables (1-4) were assigned to each class and used as a multiplier factor for the VLI, resulting in a new index (VLI_CMM). Data on LBP were collected by occupational physicians at the study sites. Logistic regression was used to estimate the risk of acute LBP within levels of risk exposure when compared with a control group formed by 1,028 unexposed subjects. Results Data showed greatly variable values of cumulative mass across all VLI classes. The potential effect of cumulative mass on damage emerged as not significant ( p value = .6526). Conclusion When comparing VLI_CMM with raw VLI, the former failed to prove itself as a better predictor of LBP risk. Application To recognize cumulative mass as a modifier, especially for lumbar degenerative spine diseases, authors of future studies should investigate potential association between the VLI and other damage variables. Objective: To investigate whether a novel, task-specific training intervention that focused on correcting pain-producing movement patterns was feasible and whether it would improve hip and knee kinematics, pain, and function in women with patellofemoral pain. Design: Prospective, non-randomized, within-group, double baseline, feasibility intervention study. Subjects: A total of 25 women with patellofemoral pain were enrolled. Intervention: The intervention, delivered 2×/week for six weeks, consisted of supervised, high-repetition practice of daily weight-bearing and recreational activities. Activities were selected and progressed based on participants’ interest and ability to maintain optimal alignment without increasing pain. Main measures: Primary feasibility outcomes were recruitment, retention, adherence, and treatment credibility (Credibility/Expectancy Questionnaire). Secondary outcomes assessing intervention effects were hip and knee kinematics, pain (visual analog scale: current, average in past week, maximum in past week), and function (Patient-Specific Functional Scale). Results: A total of 25 participants were recruited and 23 were retained (92% retention). Self-reported average daily adherence was 79% and participants were able to perform their prescribed home program correctly (reduced hip and knee frontal plane angles) by the second intervention visit. On average, treatment credibility was rated 25 (out of 27) and expectancy was rated 22 (out of 27). Hip and knee kinematics, pain, and function improved following the intervention when compared to the control phase. Conclusion: Based on the feasibility outcomes and preliminary intervention effects, this task-specific training intervention warrants further investigation and should be evaluated in a larger, randomized clinical trial. Background: Bone loss in patients undergoing revision THA poses a considerable challenge for orthopaedic surgeons. Often, to achieve better fixation in remaining bone, larger diameter acetabular components and reaming superiorly may be necessary. However, this is likely to raise the hip center of rotation, which may lead to altered biomechanics, specifically, insufficiency of the abductor muscles, altered gait, and increased risk of dislocation from impingement. More recently, a newer acetabular shell has been designed to more closely replicate the native hip center of rotation in these circumstances while maintaining adequate fixation.Questions/purposes: The purpose of this study was to compare the radiographic parameters of this newer design with conventional hemispheric cups in revision THA. Specifically, we assessed the differences in (1) vertical center of rotation (COR) displacement and (2) horizontal COR displacement.Methods: Between January 2016 and April 2016, five reconstructive surgeons at five institutions utilized a newer highly porous acetabular shell designed with peripheral screw holes and vertically eccentric COR to allow for restoration of center of hip rotation in revision THA. We included all patients who received this device. During this time, the general indications at these sites for using the new device included Paprosky Stage IIA, IIB, IIC, or IIIA acetabular defects. This yielded 29 patients who were subsequently matched (one to two) by cup size and sex to a cohort who underwent revision THA with conventional hemispheric cups between January 2015 and May 2016. To determine hip COR, radiographic measurements were performed. A circle contiguous to the acetabulum was drawn and the center was determined as the hip COR. All measurements were made from the interteardrop line for both the revised and native hips. A line through the teardrops was used for all horizontal measurements. Center position adjustments were made based on the manufacturer-specified values. Comparisons were performed using chi-square tests for categorical and t-tests for continuous variables. There was no difference in the severity of bone loss before the revision in the groups, as evidenced by Paprosky staging of preoperative radiographs.Results: The mean vertical COR displacement was smaller in patients who had the novel cup (3.5 mm; range, -12 to 15 mm; mean difference, -7.3 mm; 95% confidence interval [CI], -13.2 to -1.5) as compared with those who had the conventional cup (10.5 mm; range, -4 to 50 mm; mean difference, 7.3 mm; 95% CI, -12.5 to -2.2; p = 0.003). There was no difference in mean horizontal displacement between the two groups (-0.06 ± 6.1 versus 1.7 ± 7.1; mean difference, -1.8; p = 0.903).Conclusions: Although hip COR was improved based on radiographic measurements with the use of this novel acetabular design, and although this may improve hip biomechanics, more studies are required before its widespread adoption for revision cases of this nature can be recommended. Objective: This study compared biomechanical responses of a normally seated Hybrid III dummy on conventional and all belts to seat (ABTS) seats in 40.2 km/h (25 mph) rear sled tests. It determined the difference in performance with modern (≥2000 MY) seats compared to older (<2000 MY) seats and ABTS seats.Methods: The seats were fixed in a sled buck subjected to a 40.2 km/h (25 mph) rear sled test. The pulse was a 15 g double-peak acceleration with 150 ms duration. The 50th percentile Hybrid III was lap-shoulder belted in the FMVSS 208 design position. The testing included 11 <2000 MY, 8 ≥2000 MY, and 7 ABTS seats. The dummy was fully instrumented, including head accelerations, upper and lower neck 6-axis load cells, chest acceleration, thoracic and lumbar spine load cells, and pelvis accelerations. The peak responses were normalized by injury assessment reference values (IARVs) to assess injury risks. Statistical analysis was conducted using Student's t test. High-speed video documented occupant kinematics.Results: Biomechanical responses were lower with modern (≥2000 MY) seats than older (<2000 MY) designs. The lower neck extension moment was 32.5 ± 9.7% of IARV in modern seats compared to 62.8 ± 31.6% in older seats (P =.01). Overall, there was a 34% reduction in the comparable biomechanical responses with modern seats. Biomechanical responses were lower with modern seats than ABTS seats. The lower neck extension moment was 41.4 ± 7.8% with all MY ABTS seats compared to 32.5 ± 9.7% in modern seats (P =.07). Overall, the ABTS seats had 13% higher biomechanical responses than the modern seats.Conclusions: Modern (≥2000 MY) design seats have lower biomechanical responses in 40.2 km/h rear sled tests than older (<2000 MY) designs and ABTS designs. The improved performance is consistent with an increase in seat strength combined with improved occupant kinematics through pocketing of the occupant into the seatback, higher and more forward head restraint, and other design changes. The methods and data presented here provide a basis for standardized testing of seats. However, a complete understanding of seat safety requires consideration of out-of-position (OOP) occupants in high-speed impacts and consideration of the much more common, low-speed rear impacts. Objective: Subjective crepitus is the reporting of hearing grating, cracking, or popping sounds in and/or around a joint. We aimed to evaluate whether there is an association between crepitus and incident symptomatic knee osteoarthritis (OA) in the Osteoarthritis Initiative (OAI), a multicenter longitudinal US cohort.Methods: Knees without baseline symptomatic OA were included. Crepitus frequency was assessed using a question from the Knee Injury and Osteoarthritis Outcome Score at baseline and at 12, 24, and 36 months. Frequent knee pain and radiographs were assessed at baseline and at annual visits up to 48 months. Radiographic OA was defined as a tibiofemoral Kellgren/Lawrence grade ≥2. Symptomatic OA was defined as a knee with both frequent symptoms and radiographic OA. We performed a repeated-measures analysis with a predictor of crepitus and outcome of incident symptomatic OA, adjusting for age, sex, and body mass index (BMI), with those never reporting crepitus as the referent group.Results: There were a total of 3,495 participants (42.2% male), with mean ± SD age of 61.1 ± 9.2 years and a mean ± SD BMI of 28.2 ± 4.7 kg/m². The odds of incident symptomatic OA were higher with greater frequency of crepitus (never, rarely, sometimes, often, and always, with adjusted odds ratios of (referent), 1.5, 1.8, 2.2, and 3.0, respectively; P < 0.0001 for trend). The group with radiographic OA at OAI baseline but without symptoms contributed 26% of the observations but more than 75% of the incident symptomatic OA cases.Conclusion: In those without symptomatic OA, subjective knee crepitus predicts incident symptomatic OA longitudinally, with most cases occurring in those with preexisting tibiofemoral radiographic OA but without frequent knee pain. However, an important limitation is that patellofemoral OA was not systematically evaluated within the OAI. Context: Several tasks have been used to examine landing biomechanics for evaluation and rehabilitation, especially as related to anterior cruciate ligament injuries. However, comparing results among studies in which different tasks were used can be difficult, and it is unclear which task may be most appropriate. Objective: To compare lower extremity biomechanics across 5 commonly used landing tasks. Design: Descriptive laboratory study. Setting: University-operated US Air Force Special Operations Forces human performance research laboratory. Patients or Other Participants: A total of 65 US Air Force Special Tactics Operators (age ± 27.7 6 5.0 years, height ± 176.5 6 5.7 cm, mass ± 83.1 6 9.1 kg). Intervention(s): Kinematic and kinetic analysis of doubleand single-legged drop landing, double- and single-legged stop jump, and forward jump to single-legged landing. Main Outcome Measure(s): Hip-, knee-, and ankle-joint kinematics; knee-joint forces and moments; and ground reaction forces (GRFs) were the dependent measures. We used repeated-measures analyses of variance or Friedman tests, as appropriate, to assess within-subject differences across tasks. Results: Peak vertical GRF and peak knee-flexion angle were different among all tasks (P , .001). Single-legged landings generated higher vertical GRF (v2 ± 244.68, P , .001) and lower peak knee-flexion values (F4,64 ± 209.33, P , .001) except for forward jump to single-legged landing, which had the second highest peak vertical GRF and the lowest peak knee-flexion value. The single-legged drop landing generated the highest vertical (v2 ± 244.68, P , .001) and posterior (v2 ± 164.46, P , .001) GRFs. Peak knee-valgus moment was higher during the double-legged drop landing (v2 ± 239.63, P , .001) but similar for all others. Conclusions: Different landing tasks elicited different biomechanical responses; no single task was best for assessing a wide range of biomechanical variables related to anterior cruciate ligament injuries. Slips and falls represent a serious public safety concern in older adults, with the segment of the United States population over the age of 65 accounting for about three quarters of all fall related deaths. The majority of falls in older adults are due to trips and slips. The objective of this study was to investigate how age affects arm reactions generated in response to unexpected slips. Thirty-three participants divided into two age groups (16 young, 17 old) participated in this study. Participants were exposed to two conditions: known dry walking (baseline) and an unexpected slip initiated when stepping onto a glycerol-contaminated floor. The upper extremity parameters of interest included the timing and amplitude of the shoulder flexion moment generated in response to the slip as well as the resulting angular kinematics (trajectories). The analysis of the kinetic data revealed a delayed shoulder flexion reaction to slips in older adults compared to their young counterparts, as well as a greater flexion moment magnitude. Knowledge of such upper body reaction mechanisms to unexpected slips may help to improve balance recovery training in older adults, as well as aid in the implementation of environmental modifications, e.g. handrails, to reduce falls-related injuries. Background: Large-herd dairy parlor workers experience a high prevalence of musculoskeletal symptoms in the upper extremity. The purpose of this study was to evaluate the effect of milking unit design on upper extremity muscle activity during milking unit attachment.Methods: Upper extremity muscle activity was recorded among U.S. large-herd parlor workers (n=11) using surface electromyography. Participants performed several milking unit attachment cycles with each of six milking unit designs. Muscle activity levels were then compared between unit designs.Results: Mean muscle activity levels (in %MVE) across milking units ranged from 6.8 to 8.2 for the upper trapezius, 8.2 to 10.3 for the anterior deltoid, 13.8 to 17.2 for the forearm flexors, and 9.9 to 12.4 for the forearm extensors. Pairwise comparisons between milking units did not reveal statistically significant differences in muscle activity levels across milking unit designs. However, a general pattern of higher muscle activity was observed with specific milking units. Milking unit weight, milk tube spread, and teat cup shape may explain differences in muscle activity levels.Conclusions: Milking unit design may influence muscle activity levels among parlor workers. Small reductions in muscle activity associated with milking unit design have the potential to delay the onset of fatigue or development of musculoskeletal health outcomes among parlor workers. STUDY DESIGN: Reliability study. BACKGROUND: Video analysis of running gait is frequently performed in orthopaedic and sports medicine practices to assess biomechanical factors that may contribute to injury. However. the reliability of a whole-body assessment has not been determined. OBJECTIVE: To determine the ¡ntrarater and ¡nterrater reliability of the qualitative assessment of specific running kinematics from a 2-dimensional video. METHODS: Running-gait analysis was performed on videos recorded from 15 individuals (8 male, 7 female) running at a self-selected pace (3.17 ± 0.40 m/s, 8:28 ± 1:04 min/mi) using a high-speed camera (120 frames per second). These videos were independently rated on 2 occasions by 3 experienced physical therapists using a standardized qualitative assessment. Fifteen sagittal and frontal plane kinematic variables were rated on a 3- or 5-point categorical scale at specific events of the gait cycle, including initial contact (n = 3) and midstance (n = 9), or across the full gait cycle (n = 3). The video frame number corresponding to each gait event was also recorded. Intrarater and interrater reliability values were calculated for gait-event detection (¡ntraclass correlation coefficient [ICC] and standard error of measurement [SEM]) and the individual kinematic variables (weighted kappa [Kw]). RESULTS: Gait-event detection was highly reproducible within raters (ICC = 0.94-1.00; SEM, 0.3-1.0 frames) and between raters (ICC = 0.77-1.00; SEM, 0.4-1.9 frames). Eleven of the 15 kinematic variables demonstrated substantial (Kw = 0.60-0.799) or excellent (Kw>0.80) intrarater agreement, with the exception of foot-to-center-of-mass position (Kw = 0.59), forefoot position (Kw = 0.58), ankle dorsiflexion at midstance (Kw = 0.49), and center-of-mass vertical excursion (Kw = 0.36). Interrater agreement for the kinematic measures varied more widely (Kw = 0.00-0.85), with 5 variables showing substantial or excellent reliability. CONCLUSION: The qualitative assessment of specific kinematic measures during running can be reliably performed with the use of a high-speed video camera. Detection of specific gait events was highly reproducible, as were common kinematic variables such as rearfoot position, foot-strike pattern, tibial inclination angle, knee flexion angle, and forward trunk lean. Objective: Pain and functional decline are hallmarks of knee osteoarthritis (OA). Nevertheless, longitudinal studies unexpectedly reveal stable or improved physical function. The aim of this study was to impute missing and pre-total knee replacement (TKR) values to describe physical function over time among people with symptomatic knee OA.Methods: We included participants from the Multicenter Osteoarthritis Study (MOST) and the Osteoarthritis Initiative (OAI) with incident symptomatic knee OA, observed during the first 30 months in MOST and 36 months in OAI. Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) physical function (WOMAC-PF), the 5-times sit-to-stand test, and the 20-meter-walk test were assessed at 4 and 5 years in MOST and at 6 years in OAI. We used a multiple imputation method for missing visits, and estimated pre-TKR values close to the time of TKR, using a fitted local regression smoothing curve. In mixed-effect models, we investigated the physical function change over time, using data before and after imputation and calculation of pre-TKR values.Results: In MOST, 225 (8%) had incident knee OA, with corresponding 577 (12.7%) in OAI. After adjusting for pre-TKR values and imputing missing values, we found that WOMAC-PF values remained stable or slightly declined over time, and the 20-meter-walk test results changed from stable in nonimputed analyses to worsening using imputed data.Conclusion: Data from MOST and OAI showed stable to worsening physical function over time in people with incident symptomatic knee OA after imputing missing values and adjusting pre-TKR values. Background: Polyethylene liner dissociation is a rare but catastrophic event in total hip arthroplasty (THA), and certain implant designs are known to be at greater risk. Although the DePuy Pinnacle (Warsaw, IN, USA) modular acetabular construct has an excellent record of fixation and wear, an unexpectedly high number of liner dissociations has been noted.Questions/purposes: The purposes of this study were (1) to characterize the clinical parameters observed in a large group of patients who have experienced liner dissociations with the DePuy Pinnacle acetabular component; (2) to describe the radiographic findings in this group of patients; and (3) to calculate a minimum frequency of this complication.Methods: Since 2001, 23 patients with previously well-functioning THAs presented with sudden atraumatic polyethylene liner dissociation at four separate institutions. These THAs were performed between 2001 and 2013. Eight different arthroplasty specialists had performed the index hip arthroplasties using the DePuy Pinnacle acetabular component with a polyethylene liner. Polyethylene failures were evaluated for liner type and radiographic cup position. For three of the surgeons who contributed cases, institutional registries allowed the calculation of the number of components of this type that they used during the period in question, which provided a conservative estimate of the frequency of this type of failure.Results: All 23 liner failures occurred atraumatically in previously asymptomatic THAs at a mean of 48 months (range, 3-138 months). Patients characteristically reported a new and sudden onset of discomfort with audible, reproducible squeaking. Surgical inspection of dissociated liners demonstrated displacement of polyethylene with shearing of the peripheral locking tabs. Radiographic evaluation demonstrated that 14 cups were well positioned and nine cups were malpositioned outside the so-called safe zone. Conservative estimates of the frequency of this complication from the three surgeons' practices whose institutional registries allowed calculation of the lowest possible frequency were 0.32% (six of 1888), 0.77% (three of 391), and 0.82% (three of 367).Conclusions: With this report of 23 additional liner dissociations, we suggest that surgeons should be aware of the problem and take extra precautions when using this implant to ensure locking mechanism integrity at the time of surgery. Background: Hard-on-hard bearings for total hip arthroplasty continue to warrant analysis even though crosslinked polyethylene is performing very well. Ceramic-on-metal (CoM) has low in vitro wear and did well in an early clinical trial. We report on a prospective, randomized, multicenter investigational device trial comparing CoM with metal-on-metal (MoM).Questions/purposes: (1) Is there a difference in the number or type of revisions comparing CoM with MoM? (2) Are cobalt and chromium metal levels different for CoM and MoM THA?Methods: Between August 2005 and October 2006, of 1015 patients screened, 390 patients were enrolled at 11 centers and randomized to 194 CoM and 196 MoM bearings. There was no difference in the preoperative patient demographics between the study groups. Mean followup was 50 months (range, 22-75 months). Seventy-two patients from two centers had metal level analysis.Results: With the numbers available, there was no difference in the proportion of patients undergoing revisions between the MoM and the CoM cohorts (MOM: 3% [six of 196]; COM: 1.5% [three of 194]; p = 0.50). Four MoM revisions were unrelated to the bearing surface. Two had bearing surface-related reoperations, one for an aseptic lymphocyte-dominated vasculitis-associated lesion and one for elevated metal levels with acetabular malposition. None of the CoM revisions were related to the bearing surface. The metal level analysis revealed that in contrast to the CoM, the MoM bearing group had increasing values of erythrocyte and serum cobalt from 1 to 5 years (CoM erythrocyte 0.45-0.55 ppb, p = 0.11 and CoM serum 0.88-0.85, p = 0.55, and MoM erythrocyte 0.32-0.51 ppb, p < 0.01 and MoM serum 0.65-1.01 ppb, p < 0.01). In addition, the MoM cobalt levels in erythrocytes and serum at 5 years were more variable than at 1 year (erythrocyte interquartile range [IQR], 0.26-0.44 to 0.31-1.21 ppb and serum IQR, 0.42-0.80 to 0.64-2.20 ppb, p < 0.02 for both).Conclusions: Although both bearings performed well at short-term followup, the CoM bearing group had no wear-related revisions and maintained consistently low metal levels. The MoM cobalt elevations may be important considering recent reports of taper corrosion. This CoM bearing was approved by the FDA but withdrawn from the market because of low sales. If it were available, the authors would not use CoM until long-term data were available. Background: Modular revision femoral components allow the surgeon to make more precise intraoperative adjustments in anteversion and sizing, which may afford lower dislocation rates and improved osseointegration, but may not offer distinct advantages when compared with less expensive monoblock revision stems.Questions/purposes: We compared modular and monoblock femoral components for revision of Paprosky Type I to IIIA femoral defects to determine (1) survivorship of the stems; and (2) complications denoted as intraoperative fracture, dislocation, or failure of osseointegration.Methods: Between 2004 and 2010, participating surgeons at three centers revised 416 total hip arthroplasties (THAs) with Paprosky Type I to IIIA femoral defects. Of those with minimum 2-year followup (343 THAs, mean followup 51 ± 13 months), 150 (44%) were treated with modular stems and 193 (56%) were treated with monoblock, cylindrical, fully porous-coated stems. During this time, modular stems were generally chosen when there was remodeling of the proximal femur into retroversion and/or larger canal diameters (usually > 18 mm). A total of 27 patients died (6%) with stems intact before 2 years, 46 THAs (13%) were lost to followup before 2 years for reasons other than death, and there was no differential loss to followup between the study groups. The modular stems included 101 with a cylindrical distal geometry (67%) and 49 with a tapered geometry (33%). Mean age (64 versus 68 years), percentage of women (53% versus 47%), and body mass index (31 versus 30 kg/m(2)) were not different between the two cohorts, whereas there was trend toward a slightly greater case complexity in the modular group (55% versus 65% Type 3a femoral defects, p = 0.06). Kaplan-Meier survivorship was calculated for the endpoint of aseptic revision. Proportions of complications in each cohort (dislocation, intraoperative fracture, and failure of osseointegration) were compared.Results: Femoral component rerevision for any reason (including infection) was greater (OR, 2.01; 95% CI, 1.63-2.57; p = 0.03) in the monoblock group (27 of 193 [14%]) compared with the modular cohort (10 of 150 [7%]). Femoral component survival free from aseptic rerevision was greater in the modular group with 91% survival (95% CI, 89%-95%) at 9 years compared with 86% survival (95% CI, 83%-88%) for the monoblock group in the same timeframe. There was no difference in the proportion of mechanically relevant aseptic complications (30 of 193 [16%] in the monoblock group versus 34 of 150 [23%] in the modular group, p = 0.10; OR, 1.47; 95% CI, 0.86-2.53). There were more intraoperative fractures in the modular group (17 of 150 [11%] versus nine of 193 [5%]; OR, 2.2; 95% CI, 1.68-2.73; p = 0.02). There were no differences in the proportions of dislocation (13 of 193 [7%] monoblock versus 14 of 150 [9%] modular; OR, 0.96; 95% CI, 0.67-1.16; p = 0.48) or failure of osseointegration (eight of 193 [4%] monoblock versus three of 150 [2%] modular; OR, 1.92; 95% CI, 0.88-2.84; p = 0.19) between the two groups with the number of hips available for study.Conclusions: Although rerevisions were less common in patients treated with modular stems, aseptic complications such as intraoperative fractures were more common in that group, and the sample was too small to evaluate corrosion-related or fatigue concerns associated with modularity. We cannot therefore conclude from this that one design is superior to the other. Larger studies and pooled analyses will need to be performed to answer this question, but we believe modularity should be avoided in more straightforward cases if possible.Level Of Evidence: Level III, therapeutic study. Background: Adverse local tissue reactions (ALTRs) around hip arthroplasties are an important reason for failure of metal-on-metal (MoM) hip implants. Little is known about capsular dehiscence patterns as ALTRs decompress from the hip into the surrounding tissue planes; these patterns may also influence the onset and severity of patient symptoms.Questions/purposes: Through a multicenter study approach, we asked: (1) Is ALTR location related to the surgical approach used for arthroplasty in patients who underwent hip arthroplasty (resurfacing or THA) with a single, recalled hip arthroplasty system? (2) Do ALTR severity and location affect patient-reported outcomes in these patients? (3) Is ALTR severity different between patients who received the resurfacing version of this component (Articular Surface Replacement [ASR]) and those who received the THA implant in this system (ASR XL)?Methods: In a multicenter prospective study of patients who had undergone surgery with use of the ASR and ASR XL hip system (DePuy Orthopaedics, Warsaw, IN, USA), 288 patients (333 hips) from two centers had a metal artifact reduction sequence MRI of the hip performed at a mean time of 6 years postsurgery. Procedures included 166 hips (50%) with ASR resurfacing and 167 hips (50%) with ASR XL THA performed between 2004 and 2010. One hundred twenty-nine hips (39%) had been operated on using a direct lateral approach and 204 using a posterior approach (61%). The EQ-5D, Harris hip score, UCLA activity score, and visual analog scale pain score were obtained for each patient. ALTRs were classified using the Anderson ALTR grading system, and the location, synovial thickness, and diameter of the ATLRs were assessed. The relationship between ALTR location and surgical approach as well as for ALTR severity and patient-reported outcomes were evaluated, and logistic regression was used to identify predictors for moderate-to-severe ALTRs.Results: Moderate or severe ALTRs were identified in 79 hips (24%); 41 of these hips had been operated on using the direct lateral approach and 38 using the posterior approach. In patients in whom the lateral approach was used, 83% had an anterior ALTR. Similarly, 71% of patients in the posterior approach group had posterior ALTRs. There were no differences in patient-reported outcome measures between patients with moderate-to-severe ALTRs and those with no ALTR findings on MRI (p > 0.09). Use of ASR XL was an independent risk factor for moderate-to-severe ALTRs (odds ratio, 2.8; 95% confidence interval, 1.4-5.5 p = 0.004) and patients with ASR XL also had a thicker synovium (median ASR XL = 3.6 mm [1.2-10.6 mm], median ASR = 2.6 mm [1.2-10.7 mm], p < 0.001) and larger maximal ALTR diameter (median ASR XL = 47.6 mm [14-109.70 mm], median ASR = 38.4 [17.2-118.0 mm], p = 0.02) than patients treated with ASR.Conclusions: The location of ALTRs can be predicted based on the previous surgical approach to the hip. Patients with ASR XL are more likely to develop moderate-to-severe ALTRs compared with ASR patients. An extensive range of patient-reported outcome measures may not identify all patients with ALTRs further supporting the use of MRI as a screening measure for ALTRs.Level Of Evidence: Level II, therapeutic study. Background: Preoperative depressive symptoms have been shown in some but not all studies to be associated with poor self-reported pain and function outcomes. In addition, depressive symptoms after surgery have been shown to improve relative to preoperative levels.Questions/purposes: We hypothesized that (1) preoperative depressive symptoms would predict postoperative pain; (2) depressive symptoms would decrease after surgery; and (3) preoperative depressive symptoms would increase as the scheduled surgery date approached.Methods: Data from the Osteoarthritis Initiative, a National Institutes of Health-funded prospective multiyear cohort study, were used in this retrospective analysis. Persons from four communities were eligible if they had radiographic knee osteoarthritis or were at risk for developing knee osteoarthritis based on occupational, medical history, or body weight risk factors. A total of 4796 persons participated and rates of followup were 80% or greater over the course of the study. Participants completed a validated depressive symptom scale and the Knee Injury and Osteoarthritis Outcome Scale pain scale each year for 3 years before and 3 years after TKA. Latent growth curve modeling was used to model intercepts and slopes of pre- and postoperative depression and pain. Preoperative trajectories and intercepts were then used to predict postoperative pain and depressive symptoms adjusting for confounding variables.Results: After adjustment for potential confounding, we found no evidence that preoperative depressive symptoms predicted postoperative pain with function (estimate, 0.1; 95% confidence interval, -0.31 to 0.50; p = 0.64) or that depressive symptoms were reduced after surgery (z = 0.06, p = 0.80). We also found no evidence to indicate that preoperative depressive symptoms increased as the date of surgery approached (linear slope = 0.28, SE = 0.19, p = 0.15).Conclusions: Preoperative and postoperative depressive symptoms in patients before and after TKA did not appreciably change over a 6-year perioperative period. Patient depressive symptoms were not reduced after surgery and did not appear to be related to less pain postoperatively. Our findings of no association between preoperative depressive symptom severity and postoperative pain and no reduction in postoperative depressive symptoms run counter to other available evidence, potentially attributable, in part, to a data collection process that occurred outside of orthopaedic surgeons' offices. Future research is needed to more fully explore the potential role of social desirability, the concept that patients respond in a way that they think the researcher or clinician wants them to respond in lieu of responding in a way that truly reflects the patient's status. Background: Patients with shoulder and rotator cuff pathology who exhibit greater levels of psychological distress report inferior preoperative self-assessments of pain and function. In several other areas of orthopaedics, higher levels of distress correlate with a higher likelihood of persistent pain and disability after recovery from surgery. To our knowledge, the relationship between psychological distress and outcomes after arthroscopic rotator cuff repair has not been similarly investigated. Questions/purposes: (1) Are higher levels of preoperative psychological distress associated with differences in outcome scores (visual analog scale [VAS] for pain, Simple Shoulder Test, and American Shoulder and Elbow Surgeons score) 1 year after arthroscopic rotator cuff repair? (2) Are higher levels of preoperative psychological distress associated with less improvement in outcome scores (VAS for pain, Simple Shoulder Test, and American Shoulder and Elbow Surgeons score) 1 year after arthroscopic rotator cuff repair? (3) Does the prevalence of psychological distress in a population with full-thickness rotator cuff tears change when assessed preoperatively and 1 year after arthroscopic rotator cuff repair? Methods: Eighty-five patients with full-thickness rotator cuff tears were prospectively enrolled; 70 patients (82%) were assessed at 1-year followup. During the study period, the three participating surgeons performed 269 rotator cuff repairs; in large part, the low overall rate of enrollment was related to two surgeons enrolling only two patients total in the initial 14 months of the study. Psychological distress was quantified using the Distress Risk Assessment Method questionnaire, and patients completed self-assessments including the VAS for pain, the Simple Shoulder Test, and the American Shoulder and Elbow Surgeons score preoperatively and 1 year after arthroscopic rotator cuff repair. Fifty of 85 patients (59%) had normal levels of distress, 26 of 85 (31%) had moderate levels of distress, and nine of 85 (11%) had severe levels of distress. Statistical models were used to assess the effect of psychological distress on patient self-assessment of shoulder pain and function at 1 year after surgery. Results: With the numbers available, distressed patients were not different from nondistressed patients in terms of postoperative VAS for pain (1.9 [95% confidence interval {CI}, 1.0-2.8] versus 1.0 [95% CI, 0.5-1.4], p = 0.10), Simple Shoulder Test (9 [95% CI, 8.1-10.4] versus 11 [95% CI, 10.0-11.0], p = 0.06), or American Shoulder and Elbow Surgeons scores (80 [95% CI, 72-88] versus 88 [95% CI, 84-92], p = 0.08) 1 year after arthroscopic rotator cuff repair. With the numbers available, distressed patients also were not different from nondistressed patients in terms of the amount of improvement in scores between preoperative assessment and 1-year followup on the VAS for pain (3 [95% CI, 2.2-4.1] versus 2 [95% CI, 1.4-2.9], p = 0.10), Simple Shoulder Test (5.2 [95% CI, 3.7-6.6] versus 5.0 [95% CI, 4.2-5.8], p = 0.86), or American Shoulder and Elbow Surgeons scale (38 [95% CI, 29-47] versus 30 [95% CI, 25-36], p = 0.16). The prevalence of psychological distress in our patient population was lower at 1 year after surgery 14 of 70 (20%) versus 35 of 85 (41%) preoperatively (odds ratio, 0.36; 95% CI, 0.17-0.74; p = 0.005). Conclusions: Mild to moderate levels of distress did not diminish patient-reported outcomes to a clinically important degree in this small series of patients with rotator cuff tears. This contrasts with reports from other areas of orthopaedic surgery and may be related to a more self-limited course of symptoms in patients with rotator cuff disease or possibly to a beneficial effect of rotator cuff repair on sleep quality or other unrecognized determinants of psychosocial status. Objectives: Characterize the distribution of foot-strike (FS) patterns in U.S. Army Soldiers and determine if FS patterns are related to self-reported running injuries and performance.Methods: 341 male Soldiers from a U.S. Army Combined Arms Battalion ran at their training pace for 100 meters, and FSs were recorded in the sagittal plane. Participants also completed a survey related to training habits, injury history, and run times. Two researchers classified FS patterns as heel strike (HS) or nonheel strike (NHS, combination of midfoot strike and forefoot strike patterns). Two clinicians classified the musculoskeletal injuries as acute or overuse. The relationship of FS type with two-mile run time and running-related injury was analyzed (p ≤ 0.05).Results: The Soldiers predominately landed with an HS (87%) and only 13% were characterized as NHS. Running-related injury was similar between HS (50.3%) and NHS (55.6%) patterns (p = 0.51). There was no difference (p = 0.14) between overuse injury rates between an HS pattern (31.8%) and an NHS pattern (31.0%). Two-mile run times were also similar, with both groups averaging 14:48 minutes.Conclusion: Soldiers were mostly heel strikers (87%) in this U.S. Army Combined Arms Battalion. This study quantified the mechanical interactions between an American football cleat and eight surfaces used by professional American football teams. Loading conditions were applied with a custom-built testing apparatus designed to represent play-relevant maneuvers of elite athletes. Two natural grass and six infill artificial surfaces were tested with the cleated portion of a shoe intended for use on either surface type. In translation tests with a 2. 8-kN vertical load, the grass surfaces limited the horizontal force on the cleats by tearing. This tearing was not observed with the artificial surfaces, which allowed less motion and generated greater horizontal force (3.2 kN vs. 4.5 kN,p < 0.05). Similarly, rotation tests generated less angular displacement and greater torque on the artificial surfaces (145 N m vs. 197 N m,p < 0.05). Translation/drop tests, in which the foot-form was launched into the surfaces with both horizontal and vertical velocity components generated less peak horizontal force on the natural surfaces than on the artificial surfaces (2.4 kN vs. 3.0 kN,p < 0.05). These results suggest a force-limiting mechanism inherent to natural grass surfaces. Future work should consider implications of these findings for performance and injury risk and should evaluate the findings' sensitivity to cleat pattern and playing conditions. Two states in the United States now have legislation requiring that laying hens be provided with sufficient space to perforin particular behaviors. To provide a framework for translating these performance standards into a space requirement, kinematic analysis was used to measure the amount of space needed for White Leghorn hens to stand, turn around 180°, lie down, and wing flap. Hyline W-36 hens (n = 9) were marked on the tops of their heads and the tips of both wings and 3 toes with black livestock marker. Each hen was then placed in a floor pen (91.4 x 91.4 cm) and filmed using 2 high-speed cameras. The resulting images were processed using a software program that generated 3-dimensional space use for each behavior. Because none of the hens lay down in the test pen, the 2-dimensional space required for lying was determined by superimposing a grid over videos of the hens lying down in their home cages. On average, hens required a mean area of 563 (± 8) cm2 to stand, 1,316 (± 23) cm² to turn around, 318 (± 6) cm² to lie down, and 1,693 (± 136) cm² to wing flap. The mean heights used were 34.8 (± 1.3) cm for standing, 38.6 (± 2.3) cm for turning, and 49.5 (± 1.8) cm for wing flapping. However, space requirements for hens housed in multiple-hen groups in cage or noncage systems cannot be based simply on in-formation about the space required for local movement by a single hen. It must also incorporate consideration of the tendency of hens in a flock to synchronize their behaviors. In addition, it must include not just local movement space but also the space that hens may need to use for longer-distance movements to access resourc-es such as food, water, perches, and nest boxes. Background and purpose The purpose of the study was to investigate whether individuals with anterior glenohumeral joint (GHJ) instability exhibit different scapular orientation from individuals with healthy shoulders in a planar shoulder movement and a functional three-dimensional (3D) reaching task. Methods This is a between-groups study with repeated measures. Ten people with anterior GHJ instability and 15 people with healthy shoulders participated in the study. For the planar shoulder movement task, participants were asked to abduct the shoulder from the resting position to pre-determined target angles (45°, 90°, and 135° of shoulder abduction) in the frontal plane. For the 3D reaching task, participants were asked to move the unconstrained arm to a remembered target location in space. Scapular orientation about three rotational axes was analysed at the target location and compared between the two groups of subjects. Results For planar abduction, participants with GHJ instability and those with healthy shoulders exhibited similar scapular orientation changes in upward-downward rotation, anterior-posterior tipping and medial-lateral rotation. For 3D reaching, participants with GHJ instability and those with healthy shoulders also exhibited similar scapular orientation changes pointing to targets at various locations. Conclusions Results of this study show that scapular orientation changes associated with shoulder abduction and functional reaching activities were similar in people with GHJ instability and those with healthy shoulders. The current study suggests that anterior GHJ instability has little impact on scapular kinematics during upper limb movements in a chronic condition without pain, muscle weakness or limited range of motion.
