Attrition from Canadian graduate programs is a point of concern on a societal, institutional, and individual level. To improve retention in graduate school, a better understanding of what leads to withdrawal needs to be reached. This paper uses logistic regression and discrete-time survival analysis with time-varying covariates to analyze data from the Youth in Transition Survey. The pre-entry attributes identified in Tinto's (1993) model of attrition are examined to help uncover who is most likely to withdraw from graduate school. A good academic background is shown to be the strongest predictor of entering graduate school. Upon entry, demographic and background characteristics, such as being married and having children, are associated with a reduced likelihood of completing. Policy recommendations at the department and institution level are provided as well as directions for future research.

This paper examines the barriers to technology integration for Manitoban K-12 inservice science educators (n = 430) based on a 10-item online survey; results are analyzed according to teaching stream using the Technology, Pedagogy, and Content Knowledge (TPACK) framework. Quantitative descriptive statistics indicated that the leading barriers experienced by all teachers are inadequate: access; time; resources; training; budget; and support. Upon further examination, Middle Years and Senior Years teachers are considerably more likely to report that access and time hinder technology integration than their Early Years counterparts. Nearly 80% of all teachers remarked that technology was available to them, but about one quarter of respondents expressed frustration about the barriers that hinder effective technology integration in their classrooms. Implications of this study inform school division teacher support programs and planning, inservice professional development opportunities, and preservice teacher education.

The authors investigated the effectiveness of online versus paper assignments using final examination scores in three cohorts of theory of finance. In particular, two cohorts were exposed to online assignments while another cohort was exposed to traditional assignments. The central result is that exposure to online assignments robustly leads to higher final exam scores, all else being equal. The authors also found a lower level of engagement as shown by low assignment completion rates for students exposed to online assignments.

This paper describes faculty perceptions of the impacts of a professional development (PD) programme for faculty called Interculturalising the curriculum. Over 70 faculty members have participated since 2008. Participants in the study represented four cohorts from 2008, 2011, 2012, 2013, from a broad range of academic disciplines. We begin with a review of the literature relevant to the rationale for, and challenges of, intercultural faculty development. We describe the theoretical frameworks and design of the programme. Next we present the results of a survey and one-on-one interviews with 20 past participants. The findings of the study combined with real changes in pedagogy and curriculum provide evidence that well-planned, theoretically grounded, intercultural PD for faculty can promote long-lasting impacts for both the educators and their students.

Background: This paper reviews what is currently known about mortality among Canadians with intellectual and developmental disabilities and describes opportunities for ongoing monitoring. Methods: In-hospital mortality among adults with intellectual and developmental disabilities in Ontario was examined using hospital data. Mortality was compared between age-, sex- and residence area-matched groups of Manitobans with and without intellectual and developmental disabilities using linked administrative data. A retrospective cohort study of mortality among individuals with intellectual and developmental disabilities in a region of Ontario focused on measuring excess mortality and risk factors. Findings: There is evidence of excess mortality in persons with intellectual and developmental disabilities in Canada. Some of the excess is attributable to comorbidities that are more common in this population. Women may have a greater risk of death than men. Excess mortality occurs at all ages but is more pronounced in early life. Discussion: High-quality ongoing monitoring of mortality among individuals with intellectual and developmental disabilities is possible in Canada. Examination of sex differences should be a priority.

Teachers are one of the most important factors in successful inclusion of students with a learning disability in mainstream classrooms. Attitudes towards inclusion of certain students in classrooms have been mixed, and many attitudes are often developed during the pre-service training and early teaching years. This study investigated the attitudes of trainee teachers towards students with learning disabilities by analysing their attributional responses to hypothetical students. Accordingly, 181 graduating Canadian trainee teachers were surveyed and the results indicated that as students' ability levels decrease, teachers' sympathy levels rise, and the expectation of future failure increases. Moreover, as students' expended efforts increase, the teacher feedback becomes more positive, the frustration decreases, the sympathy levels rise, and the expectation of future failure decreases. With regards to differences between students with and without a learning disability, as students' ability levels increase the difference in trainee teachers' sympathy level increases. Furthermore, as students' expended efforts increase, the difference in teacher feedback given to students with and without a learning disability decreases, the difference in frustration and sympathy levels decrease, and the difference in expectations of future failure increases. Implications and recommendations for practice and research conclude the paper.

This paper explores the efficacy of employing the cohort model of teacher education for preparing pre-service teachers to address issues of race within an urban university setting. The results of the research indicate that the cohort facilitates a degree of familiarity and support amongst the teacher candidates, provides exposure to diverse experiences and requires staff who are knowledgeable about discourses of difference to provide a foundation for addressing and challenging notions of race. Additionally, ensuring that members of the cohort engage in a comprehensive analysis of the systemic and institutional aspects of oppression is central to effectively understanding social inequities.

Background: In Canada, publicly-funded home care programs enable older adults to remain and be cared for in their home for as long as possible but they often differ in types of services offered, and the way services are delivered. This paper examines whether these differing approaches to care shape the pathway that home care clients will take. Older adult client pathways refer to trajectories within, and out of, the home care system (e.g., improvement, long term care (LTC) placement, death). Methods: A retrospective analysis of home care assessment data (RAI-HC was linked with health administrative data, long-term care admissions and vital statistics in Nova Scotia Health (NSH) and Winnipeg Regional Health Authority (WRHA). The study cohort consists of clients age 60 + years, admitted to home care between January 1, 2011 to December 31, 2013 and up to four years from baseline. Differences in home care service use, client characteristics and their pathways were tested across the two jurisdictions overall, and among the four discharge streams within jurisdictions using t-tests and chi-square tests of significance. Results: NS and WHRA clients were similar in age, sex, and marital status. NS clients had higher levels of need (ADL, cognitive impairment, CHESS) at base line and were more likely discharged to LTC (43% compared to 38% in WRHA). Caregiver distress was a factor correlated with being discharged to LTC. While a third remained as home care clients after 4 years; more than half were no longer in the community – either discharged to LTC placement or death. Such discharges occurred on average at around two years, a relatively short time period. Conclusions: By following older clients over 4 years, we provide enhanced evidence of client pathways, the characteristics that influence these paths, as well as the length of time to the outcomes. This evidence is central to identification of clients at risk in the community and aids in planning for future home care servicing needs that will allow more older adults to remain living in the community. In this paper we describe the process we used to reactivate a cohort study that began in 1973 but had not been contacted since 1995. In 2018, we began efforts to trace cohort members who had been involved in the last wave of the study. While we had old contact information, we also employed internet search strategies to try to find individuals. We discuss our strategy and the limits that we have as Canadians working in an extremely limited funding landscape and a data infrastructure that does not allow access to government data sources, like those described by researchers of other similar longitudinal studies spanning decades in the UK and the US. Despite our considerable attrition, we performed some analyses that demonstrates our remaining cohort is not that dissimilar from either the original cohort or in terms of general characteristics of Ontarians in their mid-60s. Background: Understanding the complex interaction of risk factors that increase the likelihood of developing common diseases is challenging. The Canadian Partnership for Tomorrow Project (CPTP) is a prospective cohort study created as a population-health research platform for assessing the effect of genetics, behaviour, family health history and environment (among other factors) on chronic diseases.Methods: Volunteer participants were recruited from the general Canadian population for a confederation of 5 regional cohorts. Participants were enrolled in the study and core information obtained using 2 approaches: attendance at a study assessment centre for all study measures (questionnaire, venous blood sample and physical measurements) or completion of the core questionnaire (online or paper), with later collection of other study measures where possible. Physical measurements included height, weight, percentage body fat and blood pressure. Participants consented to passive follow-up through linkage with administrative health databases and active follow-up through recontact. All participant data across the 5 regional cohorts were harmonized.Results: A total of 307 017 participants aged 30-74 from 8 provinces were recruited. More than half provided a venous blood sample and/or other biological sample, and 33% completed physical measurements. A total of 709 harmonized variables were created; almost 25% are available for all participants and 60% for at least 220 000 participants.Interpretation: Primary recruitment for the CPTP is complete, and data and biosamples are available to Canadian and international researchers through a data-access process. The CPTP will support research into how modifiable risk factors, genetics and the environment interact to affect the development of cancer and other chronic diseases, ultimately contributing evidence to reduce the global burden of chronic disease. Objective: Red and processed meat is considered risk factors of gestational diabetes mellitus (GDM), but the evidence is inconclusive. We aimed to examine the association between red and processed meat intake and odds of GDM among South Asian and White European women living in Canada. Methods: This is a cross-sectional analysis of pregnant women from two birth cohorts: SouTh Asian biRth cohorT (START; n = 976) and Family Atherosclerosis Monitoring In earLY life (FAMILY; n = 581). Dietary intake was assessed using a validated 169-item semi-quantitative food-frequency questionnaire (FFQ). Multivariate logistic regression models were used to examine the associations between gestational diabetes and: 1) total red and processed meat; 2) unprocessed red meat; 3) processed meat and GDM after adjustment for potential confounders. Results: There were 241 GDM cases in START and 91 in FAMILY. The median total red and processed meat intake were 1.5 g/d (START) and 52.8 g/d (FAMILY). In START, the multivariable-adjusted odds ratio (OR) showed neither lower nor higher intakes of unprocessed red meat (p-trend = 0.68), processed meat (p-trend = 0.90), or total red and processed meat (p-trend = 0.44), were associated with increased odds of GDM, when compared with medium intake. Similar results were observed in FAMILY except for processed meat intake [OR = 0.94 (95% CI 0.47–1.91), for medium versus low and OR = 1.51 (95% CI 0.77–2.29) for medium versus high; p-trend = 0.18] after adjusting for additional dietary factors such as the diet quality score, total fiber, saturated fat and glycemic load. Conclusion: Medium compared with low or high red and processed meat intake is not associated with GDM in White Europeans and South Asians living in Canada. [ABSTRACT FROM AUTHOR]

Background: The Patented Medicine Prices Review Board (PMPRB), the agency that regulates the prices of patented medicines in Canada, published proposed amendments to the regulatory framework in December 2017. Because of a series of changes and delays, the revised policy has not yet been finalized. We sought to evaluate the potential early impact of the uncertainty about the PMPRB policy on patented-medicine launches. Methods: We developed a retrospective cohort of patented medicines (molecules) sold in Canada and the 13 countries that the PMPRB currently uses or has proposed to use as price comparators, from sales data from the IQVIA MIDAS database for 2012–2021. The outcome was whether a molecule was launched (i.e., sold) in a specific country within 2 years of its global first launch (2-yr launch). We compared the change of 2-year launch before (2012–2017) and after the proposed amendments were published ("uncertain period," 2018–2021) in Canada with the change in the United States and the other 12 countries as a group ("other-countries group"), using interrupted time series and logistic regressions, respectively. We further conducted analyses for each individual country and subgroups by molecule characteristics, such as therapeutic benefit, separately. Results: We included 242 and 107 new molecules launched before publication of the proposed amendments and during the uncertain period, respectively. The corresponding 2-year launch proportions were 45.0% and 30.8% in Canada, 81.4% and 82.2% in the US, and 83.9% and 70.1% in the other-countries group. All analyses showed changes in 2-year launch during the uncertain period in the US and in the other-countries group that were similar to the changes in Canada. Greater decreases were observed in Norway and Sweden than in Canada. The 2-year launch proportion for molecules with major therapeutic benefit decreased from 45.8% to 31.3% in Canada during the uncertain period and from 87.5% to 62.5% in the other-countries group, but increased from 91.7% to 100% in the US. Interpretation: No negative impact of the PMPRB-policy uncertainty on molecule launches was observed when comparing Canada with price-comparator countries, except for molecules with major therapeutic benefit. Human life expectancy has risen in most developed countries over the last century, causing the observed demographic shifts. Babel, Bomsdorf and Schmidt (forthcoming) introduce a stochastic mortality model using panel data procedures which distinguishes between a common time effect and a common age effect of mortality evolvement. Using this mortality model, the present paper provides forecasts of future life expectancy for 17 countries divided into 12 regions: Australia, Alps, Bene, Canada, England and Wales, France, Germany, Italy, Japan, Spain, Scandinavia and the United States of America. We consider (traditional) period life expectancies as well as cohort life expectancies, the latter being a more realistic approach but less common. It turns out that a continuing increase of life expectancy is expected in all considered countries. Further, we show that the probabilistic uncertainty of forecast life expectancies is different if either period life expectancies or cohort life expectancies are considered and, moreover, the uncertainty increases substantially if the error of parameter estimation is included. This paper investigates empirically the role of age and arrival cohort effects on immigrant earnings differentials. The dataset used consists of a sample of 5,069 adult Canadian male employees from the 1973 Job Mobility Survey, a non-census dataset that provides information on actual work experience, language attributes, and numerous parental family background characteristics. It thus allows a more general specification of foreign-born/native-born earnings differentials, particularly the effects of age, work experience and years since immigration. The results confirm the importance of cross-sectional age cohort effects and refine previous findings on arrival cohort effects. Sample selectivity bias is investigated and found to be significant only when respondents' occupation is not controlled for. Background: In Canada, only 15% of patients requiring palliative care receive such services in the year before death. We describe health care utilization patterns among home care users in their last 6 months of life to inform care planning for older people with varying mortality risks and evolving care needs as they decline. Methods: Using population health administrative data from Ontario, we performed a retrospective cohort study involving home care clients aged 50 years and older who received at least 1 interRAI (Resident Assessment Instrument) Home Care assessment between April 2018 and September 2019. We report the proportion of clients who used acute care, long-term care, and palliative home care services within 6 months of their assessment, stratified by their predicted 6-month mortality risk using a prognostic tool called the Risk Evaluation for Support: Predictions for Elder-life in their Communities Tool (RESPECT) and vital status. Results: The cohort included 247 377 adults, 11.9% of whom died within 6 months of an assessment. Among decedents, 50.6% of those with a RESPECT-estimated median survival of fewer than 3 months received at least 1 nonphysician palliative home care visit before death. This proportion declined to 38.7% and 29.5% among decedents with an estimated median survival between 3 and 6 months and between 6 and 12 months, respectively. Interpretation: Many older adults in Ontario do not receive any palliative home care before death. The purpose of this study was to develop a harmonized method to collect, compare and quantify regulatory approval, patenting and chemical data from multiple cohorts of new and follow-on drugs. The range of drugs studied encompassed all drug classes enumerated, described and prioritized by drug regulators in developed nations, with a specific focus on Canada. A related purpose was to address uncertainty in the pharmaceutical innovation literature regarding approval nomenclature, such as whether certain drugs should be called new or follow-on, follow-on or Me Too, and whether the Me Too and First in Class designations refer only to drugs approved via the new drug approval route or should also refer to drugs that are line extensions. In total, 2,087 drug approvals, 5,011 patents and 130 chemical components were analyzed. Drugs approved via new drug, line extension (or "supplemental"), and generic approval routes were studied. The first major observation was that the greatest fraction of all approval, patenting and chemical indicators were associated with line extension drugs generally and line extension Me Too drugs in particular. Conversely, the smallest fraction of all three indicators observed was in relation to the most innovative drugs class identified. The second trend is in relation to drugs in classes other than the Me Too class. As one moved from approvals, to patenting, to chemical components, an increasing fraction of all indicators studied, particularly for the most profitable drugs, was associated with drug products going through some form of expedited review or drugs containing a new active substance (also referred to as a new chemical entity). Thus, while brand firms are putting most of their effort into developing line extension and Me Too drugs, therapeutic products moving through expedited review and drugs with a new active substance designation are also attracting significant drug development activity. Third, the percentage of most innovative approvals, patents and chemicals was limited for each indicator by the comparatively lower number of drugs that were First in Class, especially in the new drug category. This result suggests that a focus on new drugs that are First in Class would be an efficient way for brand firms to increase the level of innovation in their pipelines. The same conclusion applies to government incentives for drug development, including both patent and regulatory rights incentives. Fourth, the level of innovation increased steadily as one moved from drug approvals to drug patents and chemical components. This result suggests that the rank order of functional utility of indicators studied is chemicals > patents > approvals. Having said this, only approvals and patents demarcate the boundaries of legal protection for pharmaceuticals. A surprisingly large number of generic approvals, patents and chemical indicators was observed across cohorts. This result suggests that generic firms are accruing greater number of approvals, patents and chemical components than may have been previously recognized. The data also support the conclusion that generic firms are following the lead of their sister brand firms by creating clusters of related products and patents. BJOG 2011;118:966-977. To examine the relationship between delivery mode and postpartum depression at 6 weeks following hospital discharge. A prospective cohort study. Eleven hospitals in Ontario, Canada. A total of 2560 women ≥16 years of age who delivered singleton, live infants at term. Women completed a questionnaire in hospital and 74% ( n = 1897) participated in a structured telephone interview 6 weeks after discharge. Additional data were extracted from labour and delivery records. Generalised estimating equations (GEEs) were used to investigate factors associated with postpartum depression. Women were screened for depression at 6 weeks following hospital discharge using the Edinburgh Postnatal Depression Scale (EPDS). A score of ≥12 on the EPDS was used as a measure of the primary outcome, depression. Mode of delivery was not independently associated with postpartum depression, and did not factor into the main-effects model. The multivariable analysis identified 11 predictor variables for depression: young maternal age (OR 5.27; 95% CI 2.73-10.15); maternal hospital readmission (OR 3.02; 95% CI 1.46-6.24); non-initiation of breastfeeding (OR 2.02; 95% CI 0.99-4.11); good, fair, or poor self-reported postpartum health (OR 1.82; 95% CI 1.19-2.80); urinary incontinence (OR 1.79; 95% CI 1.06-3.03); multiparity (OR 1.59; 95% CI 1.22-2.08); low mental health functioning (OR 1.20; 95% CI 1.15-1.25); low subjective social status (OR 1.16; 95% CI 1.02-1.33); high number of unmet learning needs in hospital (OR 1.12; 95% CI 1.03-1.22); low social support (OR 1.06; 95% CI 1.03-1.09); and low physical health functioning (OR 1.03; 95% CI 1.003-1.055). An exploratory interaction model revealed that caesarean section was associated with higher odds of becoming depressed in Canadian-born women, but that in women born outside of Canada it was associated with a lower risk of becoming depressed. Delivery mode had no significant impact on the development of postpartum depression in the main-effects model. The health and social conditions of First Nations, Inuit, and Métis peoples in Canada remain important policy concerns. The life course has been proposed by some as a framework for analysis that could assist in the development of policies that would improve the economic and social inclusion of Aboriginal peoples. In this paper we support the goal of applying a life-course perspective to policies related to Aboriginal peoples but suggest that the framework needs to consider the unique relationship between Aboriginal peoples and public policies. Abstract: The unprecedented demographic change of the aging Canadian society has raised numerous questions, including the provision of health care and the national pension plan to an increasingly large older population. Surprisingly, however, there is little Canadian literature regarding the travel behavior of its older population, an oversight that this paper addresses. Using the 1986, 1992, and 1998 General Social Surveys and pseudo-cohort methods, this paper addresses changing driving behavior among older Canadians, and compares the ‘old’ and ‘transitional old’ to younger-aged cohorts. Results indicate that while older Canadians undertake fewer trips, and travel for different reasons than those in the labor force, their reliance upon the private automobile for transportation is no less significant. Specifically, we demonstrate that the number of trips by car with older drivers increase over the study period as the population ages. [Copyright &y& Elsevier]

This paper examines the patterns of timing of births of women using data from the Canadian Fertility Survey of 1984. Semi-Markovian schemes are applied to the maternity histories of the 5315 sample women in their reproductive ages in order to measure the intensity and timing of childbearing. Age-and-duration-specific transition probabilities are found to have substantially changed for the more recent birth cohorts compared to the older cohorts. [ABSTRACT FROM PUBLISHER]

This paper examines the broad movements of Canadian period and cohort fertility over the last hundred years or so, and compares them with corresponding trends in the United States and other industrialized countries. The main movement in Canada was a decline in fertility extending from the nineteenth century to the present time, interrupted in the 1940s and 1950s by a ‘baby boom’. The long decline in cohort fertility is largely explained by the decrease in the proportions of families of six or more children. This decrease continued during the baby boom, but in these years was more than offset, though not for Catholics, by the effects of increases in the proportions of families with three, four, and five children. This paper presents tests of hypotheses derived from the assimilation thesis according to which immigrants are expected to exhibit lower levels of participation in mass political activities than non-immigrants. Immigrant background is measured three ways, birthplace, generational cohorts, over four generations, and length of residence in Canada if foreign-born, but special emphasis is placed on generational patterns. Seven different measures of political participation are employed in multivariate controlled analyses of adult sample survey data from the 1984 Canadian National Election Study. Only generational status shows relationships with participation for more than two of the measures of the dependent variable after controls. For some forms of participation the offspring of migrants show significantly higher levels than any other category, replicating results from international studies. Thus, there is no support for the ideas that deeply rooted Canadians are the most participatory, that there is a progressive increase in participation with generations in Canada, and that the native-born are more involved than the foreign-born.

Gastroschisis has increased globally over recent decades, and this increase has not been explained by identified risk factors. We conducted a population-based study of infants born in Canada, 2004–2020. We used "winter" months (i.e., September through June) and northern areas of residence as indicators of less sunlight/less active lifestyle, while "summer" (i.e., July and August) and southern areas were considered as reference. Rate of gastroschisis for infants conceived in winter (3.4 per 10,000) was higher than for infants conceived in summer (2.2 per 10,000; p < 0.001). Exposure to winter, and northern area, hypothyroidism, substance or tobacco uses and depressive disorder were initially identified as risk factors for gastroschisis. There was a significant interaction between women < 24 years of age and 2-month conception intervals (rate ratio (RR): 1.42 (95% confidence interval [CI] 1.19–1.70). The association of maternal depression (mean ratio 2.19, 95% CI 0.87–3.50, p = 0.001) with infant gastroschisis was mediated by hypothyroidism (mean ratio 1.04, 95% CI 1.01–1.07, p < 0.001), whereas substance use, hypothyroidism, tobacco smoking and gestational diabetes showed 5.5-, 3.1-, 2.7-, and 1.2-fold associations, respectively, with maternal depression. In contrast to the summer conception interval of low gastroschisis risk, an elevated risk of gastroschisis spans the other ten months in association with higher levels of stress adaptation, thermoregulation and metabolism, reproduction, and growth effector hormones. Background: All longitudinal cohort studies strive for high participant retention, although attrition is common. Understanding determinants of attrition is important to inform and develop targeted strategies to improve study participation. We aimed to identify factors associated with research participation in a large children's primary care cohort study. Methods: In this longitudinal cohort study between 2008 and 2020, all children who participated in the Applied Research Group for Kids (TARGet Kids!) were included. TARGet Kids! is a large primary care practice-based pediatric research network in Canada with ongoing data collection at well-child visits. Several sociodemographic, health, and study design factors were examined for their associations with research participation. The primary outcome was attendance of eligible research follow-up visits. The secondary outcome was time to withdrawal from the TARGet Kids! study. Generalized linear mixed effects models and Cox proportional hazard models were fitted. We have engaged parent partners in all stages of this study. Results: A total 10,412 children with 62,655 total eligible research follow-up visits were included. Mean age at enrolment was 22 months, 52% were male, and 52% had mothers of European ethnicity. 68.4% of the participants attended at least 1 research follow-up visit. Since 2008, 6.4% of the participants have submitted a withdrawal request. Key factors associated with research participation included child age, ethnicity, maternal age, maternal education level, family income, parental employment, child diagnosis of chronic health conditions, certain study sites, and missingness in questionnaire data. Conclusions: Socioeconomic status, demographic factors, chronic conditions, and missingness in questionnaire data were associated with research participation in this large primary care practice-based cohort study of children. Results from this analysis and input from our parent partners suggested that retention strategies could include continued parent engagement, creating brand identity and communication tools, using multiple languages and avoiding redundancy in the questionnaires. Background: Several studies indicate adverse effects of selected heat exposures on semen quality, but few studies have directly evaluated fertility as an endpoint. Objective: We evaluated prospectively the association between male heat exposures and fecundability, the per‐cycle probability of conception. Materials and methods: We analyzed data from 3041 couples residing in the United States or Canada who enrolled in a prospective preconception cohort study (2013–2021). At enrollment, males reported on several heat‐related exposures, such as use of saunas, hot baths, seat heaters, and tight‐fitting underwear. Pregnancy status was updated on female follow‐up questionnaires every 8 weeks until conception or a censoring event (initiation of fertility treatment, cessation of pregnancy attempts, withdrawal, loss to follow‐up, or 12 cycles), whichever came first. We used proportional probabilities regression models to estimate fecundability ratios (FR) and 95% confidence intervals (CIs) for the association between heat exposures and fecundability, mutually adjusting for heat exposures and other potential confounders. Results: We observed small inverse associations for hot bath/tub use (≥3 vs. 0 times/month: FR = 0.87, 95% CI: 0.70–1.07) and fever in the 3 months before baseline (FR = 0.94, 95% CI: 0.79–1.12; one cycle of follow‐up: FR = 0.84, 95% CI: 0.64–1.11). Little association was found for sauna use, hours of laptop use on one's lap, seat heater use, time spent sitting, and use of tight‐fitting underwear. Based on a cumulative heat metric, FRs for 1, 2, 3, and ≥4 versus 0 heat exposures were 0.99 (95% CI: 0.87–1.12), 1.03 (95% CI: 0.89–1.19), 0.94 (95% CI: 0.74–1.19), and 0.77 (95% CI: 0.50–1.17), respectively. Associations were stronger among men aged ≥30 years (≥4 vs. 0 heat exposures: FR = 0.60, 95% CI: 0.34–1.04). Conclusion: Male use of hot tubs/baths and fever showed weak inverse associations with fecundability. Cumulative exposure to multiple heat sources was associated with a moderate reduction in fecundability, particularly among males aged ≥30 years. [ABSTRACT FROM AUTHOR]

Background: Persons with disability (PWD) experience disproportionately high poverty rates in Canada. This trend is apparent especially among youth compared to those who develop disabilities later in life. PWD in poverty have additional needs that increase barriers to full participation in society and translate to higher basic costs for daily living. Despite the existence of income assistance programs in Canada to mitigate income inequalities faced by PWDs, access to these programs can be limited. Objective: To describe use of income assistance for young adults with disability in British Columbia for the development of potential approaches to improve realized access to these programs. Methods: We conducted a population-based retrospective cohort study using British Columbia linked administrative data. We described differences in income assistance use among PWD by the level of special education funding received during primary school education (from most to least; Level 1, Level 2, Level 3, Unfunded, and no special education) and family composition. We also provided longitudinal patterns of income assistance use. Results: Of 218,324 young adults, 88% received no special education, 0.1% used Level One, 1.6% used Level Two, 2.9% used Level Three, and 7.1% used Unfunded special education coding. Young adults with Level One special education funding had the highest rates of hospitalizations and continuing care, with no hospitalization due to homelessness. Those with Level Three special education coding had higher rates of hospitalization and hospitalization due to homelessness than Level Two young adults. When transitioning to adulthood initially, Level One and Two funded individuals used relatively more disability income assistance than individuals from the other funding levels. Nearly all BCEA users with higher funded special education codes used this disability-specific program, while lesser funded special education codes used the Temporary Assistance more frequently, for a longer duration and were more likely to be persistent Temporary Assistance users. Background: The COVID-19 pandemic has affected older adults disproportionately, and delirium is a concerning consequence; however, the relationship between delirium and corticosteroid use is uncertain. The objective of the present study was to describe patient characteristics, treatments and outcomes among older adults hospitalized with COVID-19, with a focus on dexamethasone use and delirium incidence. Methods: We completed this retrospective cohort study at 7 sites (including acute care, rehabilitation and long-term care settings) in Toronto, Ontario, Canada. We included adults aged 65 years or older, consecutively hospitalized with confirmed SARS-CoV-2 infection, between Mar. 11, 2020, and Apr. 30, 2021. We abstracted patient characteristics and outcomes from charts and analyzed them descriptively. We used a logistic regression model to determine the association between dexamethasone use and delirium incidence. Results: During the study period, 927 patients were admitted to the acute care hospitals with COVID-19. Patients' median age was 79.0 years (interquartile range [IQR] 72.0-87.0), and 417 (45.0%) were female. Most patients were frail (61.9%), based on a Clinical Frailty Scale score of 5 or greater. The prevalence of delirium was 53.6%, and the incidence was 33.1%. Use of restraints was documented in 20.4% of patients. In rehabilitation and long-term care settings (n = 115), patients' median age was 86.0 years (IQR 78.5-91.0), 72 (62.6%) were female and delirium occurred in 17 patients (14.8%). In patients admitted to acute care during wave 2 of the pandemic (Aug. 1, 2020, to Feb. 20, 2021), dexamethasone use had a nonsignificant association with delirium incidence (adjusted odds ratio 1.38, 95% confidence interval 0.77-2.50). Overall, in-hospital death occurred in 262 (28.4%) patients in acute care settings and 28 (24.3%) patients in rehabilitation or long-term care settings. Interpretation: In-hospital death, delirium and use of restraints were common in older adults admitted to hospital with COVID-19. Further research should be directed to improving the quality of care for this population with known vulnerabilities during continued waves of the COVID-19 pandemic. [ABSTRACT FROM AUTHOR]

Objectives: To determine the relationship between ergonomic demands of the job at conception and fetal loss (miscarriage or stillbirth). Methods: Women with a welding or electrical trade apprenticeship were identified across Canada for the Women's Health in Apprenticeship Trades–Metal and Electrical study. They completed a reproductive and employment history at recruitment and every 6 months for up to 5 years to provide details on pregnancies and work demands. Job at conception was identified and fetal loss examined in relation to ergonomic exposures/demands, allowing for potential confounders. Results: A total of 885 women were recruited; 447 in welding and 438 in electrical trades. Of these, 574 reported at least one pregnancy. Analysis of 756 pregnancies since the woman started in her trade suggested no increased risk of fetal loss in those choosing welding rather than electrical work. Among 506 pregnancies conceived during a period working in a trade, 148 (29.2%) ended in fetal loss: 31.2% (73/234) in welding, and 27.6% (75/272) in electrical work. Detailed exposure information was available for 59% (299/506) of these pregnancies. In welders, the risk of fetal loss was increased with whole‐body vibration (prevalence ratio [PR] = 2.14; 95% confidence interval [CI] 1.39–3.31) and hand‐arm vibration for > 1 hour/day (PR = 2.15; 95% CI 1.33–3.49). In electrical workers risk increased with more than 8 days working without a rest day (PR = 2.29; 95% CI 1.25–4.17). Local exhaust ventilation reduced risk in welders. Conclusions: There was no significant increase in fetal loss in welding trades compared to electrical work. Vibration, largely from grinding, and extended work rotations appear to be potentially modifiable factors of some importance. [ABSTRACT FROM AUTHOR]

In an era of expanding postsecondary markets and heightened student and institutional competition, students' field of study decisions may be becoming an increasingly important point of differentiation in the process of social mobility. Drawing on the two most recent cohorts of the Baccalaureate and Beyond and National Graduates Surveys, this paper examines and compares field of study choices among American and Canadian baccalaureate degree-holders. Consistent with existing research, gender remains an important and consistent predictor of field of study choices. In Canada, the analyses show some evidence that the gender gap for business and management is shrinking, but the engineering and mathematics gap remains significant. In the U.S. the situation was reversed, as the engineering gap shrunk and the business and management gap did not change across cohorts. Moderate family background effects, strong and consistent academic ability effects and growing academic aspiration effects were found across most analyses, lending support to theories that predict family background has direct and indirect effects on higher education choices. This research examines mobility in the Canadian educational system and how it has evolved over the last 50 years of the twentieth century. Mixtures of ordered probability models which control for unobserved heterogeneity are estimated for both male and female respondents for two age groups: those between age 25 and 49 and those aged 50 or older in 2001. The main result is that educational attainments depend significantly less on family background variables for the younger cohorts. This is a qualitatively different result from what Canadian sociologists have found. They argue that there is little mobility in the Canadian educational system which has remained much as it was at the beginning of the twentieth century.
