Objective: This study investigated the effect of neuromuscular electrical stimulation on the frontal ankle motor control in individuals with chronic ankle instability during drop landing. Design: This was a randomized, controlled, double-blind trial. Thirtysix individuals with chronic ankle instability were randomly assigned to each group. Participants received 6-wk neuromuscular electrical stimulation intervention and sham stimulation in the neuromuscular electrical stimulation and control groups, respectively. Data were collected at week0 and week6. A mixed-effects model and analysis of covariance were employed to investigate the between-group differences in continuous and discrete outcome variables at week6, with the outcome variables at week0 as covariates. Results: Compared to control group, neuromuscular electrical stimulation group exhibited a 2.66° (2.45, 2.86) reduction in frontal ankle inversion angle, a 47.41°/sec (−16.05, −78.77) decrease in peak ankle inversion angular velocity, and a 0.43 Nm/kg (0.18, 0.68) increase in peak ankle eversion moment during drop landing at week6. Conclusions: Applying 6-wk neuromuscular electrical stimulation to the fibularis longus resulted in decreased ankle inversion angle and ankle inversion angular velocity and increased peak ankle eversion moment during drop landing. Consequently, neuromuscular electrical stimulation could be considered an effective modality for individuals with chronic ankle instability to enhance the frontal ankle movement patterns and overall ankle motor control. The purpose of this study was to assess the effects of stroke rate and sex on trunk and shoulder kinematics of Olympic athletes during rowing on an ergometer. Fifty-eight participants (31 females and 27 males) from the Chinese National Rowing Team were recruited. Trunk (i.e., the pelvis, lumbar and thoracic spine) and shoulder kinematics were measured using an inertial measurement unit system for three stroke rates (18, 26, and 32 strokes/min). Range of motion and angles at the catch and finish were assessed using mixed model ANOVA and correlation analyses with rowing power. Range of motion increased significantly at higher rates for both female and male athletes. This may be a strategy used by athletes when dealing with higher demand for power during training, because a greater range of motion with a longer stroke length could reduce the demand for force generation and possibly delay fatigue. Female rowers exhibited greater range of motion in the lumbar spine, thorax and shoulders than males due to more extended positions at the finish. The sex-related kinematic differences may be attributed to differences in body size, muscle strength and endurance. Treadmill training is useful for racewalking training; however, it may affect gait characteristics and lead to non-legal techniques. The aim of this study was to determine the kinematic differences between treadmill and overground conditions during racewalking at different speeds. Twenty-two elite racewalkers participated in this study. They racewalked under treadmill and overground conditions at high and medium speeds. A 12-camera motion analysis system was used to record the racewalking trials. Significant condition by speed interactions were detected in step frequency and pelvis rotation angle; step frequency decreased while pelvis rotation angle increased from overground to treadmill conditions at high speed. Compared to overground conditions, racewalkers decreased the ankle dorsiflexion angle at heel strike and increased hip flexion, shoulder hyperextension, and elbow flexion angles at heel strike and hip and shoulder extension angles at toe-off under treadmill conditions. Compared to medium speed, racewalkers decreased the contact time, hip flexion, and shoulder hyperextension at heel strike, and ankle plantarflexion and shoulder extension angles at toe-off, and increased flight time, step length, and elbow flexion angle at heel strike at high speed. Several kinematic differences during racewalking were detected between treadmill and overground conditions, with more differences detected at high speed, indicating that treadmill racewalking, especially at high speed, has different gait characteristics. However, no differences were detected in flight time and knee angle under treadmill conditions compared to overground conditions, indicating that racewalking on a treadmill does not increase the risk of disqualification. Background: Anterior cruciate ligament (ACL) injury can lead to changes in tibiofemoral kinematics during gait, but the detailed short-term kinematic changes after ACL injury are still unknown. Purpose: To measure tibiofemoral kinematics during gait in ACL-deficient (ACLD) knees over time after ACL injury. Study Design: Controlled laboratory study. Methods: The authors categorized 76 patients with unilateral ACLD knees into 4 groups based on the time from injury: <3 months (group 1), 3 to 6 months (group 2), >6 to 12 months (group 3), and >12 months (group 4). The controls were 20 participants with ACL-intact knees. Changes in the knee kinematics and range of motion during gait were compared among ACLD groups and those with ACL-intact knees. Results: Compared with controls, the range of motion of flexion in group 1 was significantly lower (6°; P =.033), and the mean knee flexion was significantly increased (0.7°-3.4°) in groups 1 to 4 (all P ≤.004). There was more internal tibial rotation (2.9°-4.3°) in group 1 and 2, and more anterior tibial translation (4.3 mm) in group 1 during the stance or swing phases than in controls (P ≤.049 for all). The mean internal tibial rotation and anterior tibial translation significantly decreased from group 1 to group 4 (P <.001 for both). Compared with controls, the mean medial tibial translation was significantly greater (1.2-2.5 mm) in all groups, and more medial tibial translations (2.4-3.7 mm) were observed during the stance phase in groups 1, 3, and 4 (P ≤.047 for all). Conclusion: ACLD knees displayed a motion impairment walking strategy within 3 months, and a higher-flexion walking strategy increased with time after injury. Excessive anterior translation and internal rotation of the tibia tended to return to normal, while excessive medial translation of the tibia increased in ACLD knees after 6 months postinjury. This study determined the acute changes in kinematics and kinetics when an additional load equivalent to 5% body mass was attached to the torso during change of direction (COD). In this within-subject repeated measures study, 14 male soccer players (age: 18.29 ± 0.32 years) volunteered to participate. Subjects performed COD under two conditions in randomized order: (1) no WR, and (2) with WR. No significant differences between the loaded and unloaded conditions in actual COD angle, approach speed, braking time, propulsive time, contact time, COD completion time (all p > 0.05, ES = 0.05–0.11), and all measured kinematic parameters (all p > 0.05, ES = 0–0.18). Nonetheless, ankle plantar/dorsi flexion ROM had possibly small increase in the loaded condition (ES = 0.24). Kinetics analysis has shown that the loaded condition was likely to have small increase in relative peak vertical propulsive ground reaction force (GRF, p = 0.11, ES = 0.41), and possible small increases in relative peak braking GRF (vertical: p = 0.21, ES = 0.42; total: p = 0.22, ES = 0.38), relative peak total propulsive GRF (p = 0.24, ES = 0.26), and relative braking impulse (horizontal, vertical, and total; p = 0.27–0.41, ES = 0.26–0.28). The central aim of this experiment was to compare acoustic parameters, formant frequencies and vowel space area (VSA), in adolescents with hearing-impaired (HI) and their normal-hearing (NH) peers; for kinematic parameters, the movements of vocal organs, especially the lips, jaw and tongue, during vowel production were analysed. The participants were 12 adolescents with different degrees of hearing impairment. The control group consisted of 12 age-matched NH adolescents. All participants were native Chinese speakers who were asked to produce the Mandarin vowels /a/, /i/ and /u/, with subsequent acoustic and kinematic analysis. There was significant difference between the two groups. Additionally, the HI group produced more exaggerated mouth and less tongue movements in all vowels, compared to their NH peers. Results were discussed regarding possible relationship between acoustic data, articulatory movements and degree of hearing loss to provide an integrative assessment of acoustic and kinematic characteristics of individuals with hearing loss. This study examined the effects of collar height and heel counter-stiffness of basketball shoes on ankle stability during sidestep cutting and athletic performance. 15 university basketball players wore customized shoes with different collar heights (high and low) and heel counter-stiffness (regular, stiffer and stiffest) for this study. Ankle stability was evaluated in sidestep cutting while athletic performance evaluated in jumping and agility tasks. All variables were analysed using two-way repeated ANOVA. Results showed shorter time to peak ankle inversion for both high collar and stiff heel counter conditions (P < 0.05), while smaller initial ankle inversion angle, peak inversion velocity and total range of inversion for wearing high collar shoes (P < 0.05). No shoe differences were found for performance variables. These findings imply that the collar height might play a larger role in lateral stability than heel counter-stiffness, while both collar height and counter-stiffness have no effect on athletic performance. Background: Numerous studies have investigated kinematic alterations in patients with an isolated anterior cruciate ligament (ACL) injury. However, a substantial proportion of patients with injured ACLs also have concomitant meniscal tears. Purpose: To evaluate the in vivo alteration of knee kinematics after an ACL tear, with or without a combined medial or lateral meniscal tear, during level walking activity. Study Design: Controlled laboratory study. Methods: Fifty-six patients with unilateral ACL-deficient (ACLD) knees were studied. Among these patients, 15 had isolated ACL injuries (group 1), 15 had combined ACL and medial meniscal injuries (group 2), 15 had combined ACL and lateral meniscal injuries (group 3), and 11 had combined ACL and medial/lateral meniscal injuries (group 4). The kinematics of each knee was determined using an optical tracking system during treadmill gait. Range of motion (ROM) and kinematic alterations were compared between the contralateral ACL-intact (ACLI) and ACLD knees. Results: All ACLD knees, with or without meniscal deficiency, had significantly less flexion than the ACLI knees (~3°-8°; P < .05). In groups 1, 3, and 4, the injured knees exhibited more femoral external rotation by approximately 1° to 2° (P < .05). Group 4 showed posterior femoral translation (anterior tibial translation) in the swing phase (~13 mm), while groups 2 and 3 showed increased anterior femoral translation (~2-3 mm). During medial-lateral translation, patients in group 2 had a more medial femoral shift (~4 mm) relative to the tibia initially, while those in group 4 exhibited a lateral femoral shift. Conclusion: The results indicate that meniscal injuries alter the kinematics of the ACLD knee when compared with knees with an isolated ACL injury. The location of the meniscal tear also affects knee kinematics. Clinical Relevance: Considering the varying effects of meniscal injuries on knee joint kinematics, these data provide insight into the pathological function of the ACL-injured knee joint during walking. Purpose: Models of speech production often abstract away from shared physiology in pitch control and lingual articulation, positing independent control of tone and vowel units. We assess the validity of this assumption in Mandarin Chinese by evaluating the stability of lingual articulation for vowels across variation in tone. Method: Electromagnetic articulography was used to track flesh points on the tongue (tip, body, dorsum), lips, and jaw while native Mandarin speakers (n = 6) produced 3 vowels, /pa/, /pi/, /pu/, combined with 4 Mandarin tones: T1 “high,” T2 “rising,” T3 “low,” and T4 “falling.” Results: Consistent with physiological expectations, tones that begin low, T2 and T3, conditioned a lower position of the tongue body for the vowel /a/. For the vowel /i/, we found the opposite effect, whereby tones that begin low, T2 and T3, conditioned a higher tongue body position. Conclusions: The physiology of pitch control exerts systematic variation on the lingual articulation of /a/ across tones. The effects of tone on /i/ articulation are in the opposite direction predicted by physiological considerations. Physiologically arbitrary variation of the type observed for /i/ challenges the assumption that phonetic patterns can be determined by independent control of tone (source) and vowel (filter) production units. The purpose of this study was to investigate in vivo threedimensional tibiofemoral kinematics and femoral condylar motion in knees with anterior cruciate ligament (ACL) deficiency during a knee bend activity. Ten patients with unilateral ACL rupture were enrolled. Both the injured and contralateral normal knees were imaged using biplane radiography at extension and at 15°, 30°, 60°, 90°, and 120° of flexion. Bilateral knees were next scanned by computed tomography, from which bilateral three-dimensional knee models were created. The in vivo tibiofemoral motion at each flexion position was reproduced through image registration using the knee models and biplane radiographs. A joint coordinate system containing the geometric center axis of the femur was used to measure the tibiofemoral motion. In ACL deficiency, the lateral femoral condyle was located significantly more posteriorly at extension and at 15° (p < 0.05), whereas the medial condylar position was changed only slightly. This constituted greater posterior translation and external rotation of the femur relative to the tibia at extension and at 15° (p < 0.05). Furthermore, ACL deficiency led to a significantly reduced extent of posterior movement of the lateral condyle during flexion from 15° to 60° (p < 0.05). Coupled with an insignificant change in the motion of the medial condyle, the femur moved less posteriorly with reduced extent of external rotation during flexion from 15° to 60° in ACL deficiency (p < 0.05). The medial-lateral and proximaldistal translations of the medial and lateral condyles and the femoral adduction-abduction rotation were insignificantly changed after ACL deficiency. The results demonstrated that ACL deficiency primarily changed the anterior-posterior motion of the lateral condyle, producing not only posterior subluxation at low flexion positions but also reduced extent of posterior movement during flexion from 15° to 60°. Background: The restoration of knee rotational stability after anatomic double-bundle anterior cruciate ligament (ACL) reconstruction has been demonstrated in the cadaveric model and with passive stress tests on humans but not yet with dynamic functional biomechanical tests performed by human participants.Purpose: To prospectively investigate the range of tibial rotation of ACL-deficient and ACL-reconstructed knees during a pivoting task. The authors hypothesized that there would be a significant increase in tibial internal rotation in the ACL-deficient knee compared with the contralateral knee and that the increased rotation would return to normal after anatomic double-bundle ACL reconstruction.Study Design: Case series; Level of evidence, 4.Methods: Ten men with unilateral ACL injury performed a high-demand jump-landing and pivoting task before and after ACL reconstruction with mean follow-up of 11 months. The range of tibial rotation of the injured, reconstructed, and intact knees during the pivoting movement was measured by an optical motion analysis system. Paired t tests were performed to investigate any significant difference between the 2 limbs preoperatively and postoperatively and within the injured limb before and after the surgical treatment. Statistical significance was set at P < .05.Results: The range of tibial rotation was higher in the ACL-deficient knee (12.6° ± 4.5°) than in the intact knee (7.9° ± 3.1°) preoperatively (P < .05). The increased rotation was reduced in the reconstructed knee (8.9° ± 3.0°) after ACL reconstruction versus the intact knee postoperatively (8.2° ± 2.6°) (P < .05). There was no significant difference in the tibial rotation between the intact knee and the reconstructed knee postoperatively (P > .05).Conclusion: As assessed with a dynamic functional pivoting movement, the anatomic double-bundle ACL reconstruction successfully restores knee rotational stability from an impaired level.
