Backgroud: Discharge planning from the hospital of frail older patients is an important step to avoid inappropriate long‐stay hospitalizations and to prevent the risks related to the prolonged hospitalization. In this frame, we developed an experimental trial—'PRO‐HOME', a multicomponent programme of interventions for multimorbid and polytreated hospitalized older patients. Aim: The main aim of the study was to develop a protected discharge facility using a mini apartment equipped with advanced architectural and technological components to reduce the length of hospital stay of older participants (aged 65+ years old) admitted to the hospital for an acute event, deemed stable and dischargeable. Materials and Methods: This is a pilot randomized controlled study, comparing 30 hospitalized participants included in a multidimensional, transitional care programme based on information and communication technologies to 30 patients in standard usual care until hospital discharge. Results: We presented the study design of the PRO‐HOME programme, including architectural and technological components, the enrolment procedures, the components of the intervention that is physical activity, cognitive training and life‐style education and the evaluation method of the intervention based on the Comprehensive Geriatric Assessment to explore the changes in the individual domains that are target of the multicomponent intervention. Conclusions: The final results will suggest whether the PRO‐HOME programme represents a useful and feasible intervention to reduce the length of hospital stay of multimorbid and polytreated hospitalized older patients and improve their physical and cognitive performances and overall quality of life. Patient or Public Contribution: Due to the characteristics of the population of interest of the PRO‐HOME study, we involved in the study design and programme of the activities the participants enrolled in a previous smart home‐based project named MoDiPro carried‐out during a 3‐year period. Sustainable development is a perspective on economic progress that takes into account the equilibrium among social, economic, and environmental elements. This implies that endeavours aimed at development should guarantee the satisfaction of present generations' requirements while safeguarding the capacity of future generations to fulfil their own necessities. In the context of an aging society, sustainable development involves providing solutions, services, and technologies that address the needs of older people while also minimizing negative impacts on the environment and future generations. The aim of this article is to identify, analyse and evaluate technologies that improve the quality of life of older people without compromising the well-being of future generations from ecological, social, ethical and other perspectives. Technologies that enhance the quality of life for older people are often referred to as gerontechnologies in the literature. The article identifies and then selects 3 gerontechnologies that can improve the quality of life of older people while remaining sustainable. Further, 42 criteria for evaluating gerontechnologies were identified. These gerontechnologies were then evaluated by potential users. A ranking of gerontechnologies was further developed. Dysphagia is a common geriatric syndrome that might induce serious complications and death. Standard diagnostics using the Videofluoroscopic Swallowing Study (VFSS) or Fiberoptic Evaluation of Swallowing (FEES) are expensive and expose patients to risks, while bedside screening is subjective and might lack reliability. An affordable and accessible instrumented screening is necessary. This study aimed to evaluate the classification performance of Transformer models and convolutional networks in identifying swallowing and non-swallowing tasks through depth video data. Different activation functions (ReLU, LeakyReLU, GELU, ELU, SiLU, and GLU) were then evaluated on the best-performing model. Sixty-five healthy participants (n = 65) were invited to perform swallowing (eating a cracker and drinking water) and non-swallowing tasks (a deep breath and pronouncing vowels: "/eɪ/", "/iː/", "/aɪ/", "/oʊ/", "/u:/"). Swallowing and non-swallowing were classified by Transformer models (TimeSFormer, Video Vision Transformer (ViViT)), and convolutional neural networks (SlowFast, X3D, and R(2+1)D), respectively. In general, convolutional neural networks outperformed the Transformer models. X3D was the best model with good-to-excellent performance (F1-score: 0.920; adjusted F1-score: 0.885) in classifying swallowing and non-swallowing conditions. Moreover, X3D with its default activation function (ReLU) produced the best results, although LeakyReLU performed better in deep breathing and pronouncing "/aɪ/" tasks. Future studies shall consider collecting more data for pretraining and developing a hyperparameter tuning strategy for activation functions and the high dimensionality video data for Transformer models. There has been a subsequent increase in the number of elderly people living alone, with contribution from advancement in medicine and technology. However, hospitals and nursing homes are crowded, expensive, and uncomfortable, while personal caretakers are expensive and few in number. Home monitoring technologies are therefore on the rise. In this study, we propose an anonymous elderly monitoring system to track potential risks in everyday activities such as sleep, medication, shower, and food intake using a smartphone application. We design and implement an activity visualization and notification strategy method to identify risks easily and quickly. For evaluation, we added risky situations in an activity dataset from a real-life experiment with the elderly and conducted a user study using the proposed method and two other methods varying in visualization and notification techniques. With our proposed method, 75.2% of the risks were successfully identified, while 68.5% and 65.8% were identified with other methods. The average time taken to respond to notification was 176.46 min with the proposed method, compared to 201.42 and 176.9 min with other methods. Starting from the concept of intergenerational solidarity, this paper examines how relations of 36 elderly people with younger generations of their families influence their appropriations and uses of ICT. A multiqualitative approach - non-participant observation in ICT classes attended by the participants, followed by focus groups and semi-structured interviews - conducted in Brazil and Portugal reveals that the family contexts influence how older people deal with ICT in distinctive ways. In both countries, the influence happens either by building up social interactions and increasing communication inside and outside the family or by contributing to social and emotional isolation, even when family generations are physically close. According to the latest statistics, the proportion of the elderly (+65) is increasing and is expected to double within the European Union in a period of 50 years. This ageing is due to the improvement of quality of life and advances in medicine in the last decades. Gerontechnology is receiving a great deal of attention as a way of providing the elderly with sustainable products, environments, and services combining gerontology and technology. One of the most important aspects to consider by gerontechnology is the mobility/rehabilitation technologies, because there is an important relationship between mobility and the elderly's quality of life. Telerehabilitation systems have emerged to allow the elderly to perform their rehabilitation exercises remotely. However, in many cases, the proposed systems assist neither the patients nor the experts about the progress of the rehabilitation. To address this problem, we propose in this paper, a fuzzy‐semantic system for evaluating patient's physical state during the rehabilitation process based on well‐known standard for patients' evaluation. Moreover, a tool called FINE has been developed that facilitates the evaluation be accomplished in a semi‐automatic way first asking patients to carry out a set of standard tests and then inferencing their state by means of a fuzzy‐semantic approach using the data captured during the rehabilitation tasks. Background non-pharmacological interventions to prevent delirium are useful in hospitalised older adults. However, they are poorly implemented in clinical practice. We aimed to develop a software for bedside use by hospitalised older adults and to improve their access to these interventions. Methods a transdisciplinary team composed of healthcare professionals, designers, engineers and older adults participated in the development of the software. Scrum methodology was used to coordinate the work of the team, and the software was evaluated in a feasibility study. Results a software for touchscreen mobile devices that supports Android 5.0 or later was produced, including modules for time-spatial re-orientation, cognitive stimulation, early mobilisation, sensorial support use promotion, sleep hygiene and pain management optimisation. Horizontal disposition, use of colour contrast and large interaction areas were used to improve accessibility. The software's usability and accessibility were evaluated in 34 older adults (average age 73.2 ± 9.1 years) showing that 91.1% of them got access to all the software functions without previous instructions. The clinical feasibility assessment showed that 83.3% of the 30 enrolled hospitalised patients (76 ± 8 years) completed the 5-day protocol of software usage during hospitalisation. Software use was associated with a decreased trend in delirium incidence of 5 of 32 (15.6%) at baseline to 2 of 30 (6.6%) after its implementation. Conclusion a highly accessible and implementable software, designed to improve access to non-pharmacological interventions to prevent delirium in hospitalised older adults, was developed. Recently, the interest of industry, government agencies and healthcare professionals in technology for aging people has increased. The challenge is whether technology may play a role in enhancing independence and quality of life and in reducing individual and societal costs of caring. Information and communication technologies, i.e. tools aimed at communicating and informing, assistive technologies designed to maintain older peoples' independence and increasing safety, and human–computer interaction technologies for supporting older people with motility and cognitive impairments as humanoid robots, exoskeletons, rehabilitation robots, service robots and companion-type are interdisciplinary topics both in research and in clinical practice. The most promising clinical applications of technologies are housing and safety to guarantee older people remaining in their own homes and communities, mobility and rehabilitation to improve mobility and gait and communication and quality of life by reducing isolation, improve management of medications and transportation. Many factors impair a broad use of technology in older age, including psychosocial and ethical issues, costs and fear of losing human interaction. A substantial lack of appropriate clinical trials to establish the clinical role of technologies to improve physical or cognitive performances and/or quality of life of subjects and their caregivers may suggest that the classical biomedical research model may not be the optimal choice to evaluate technologies in older people. In conclusion, successful technology development requires a great effort in interdisciplinary collaboration to integrate technologies into the existing health and social service systems with the aim to fit into the older adults' everyday life. Transportation infrastructure is a critical component to a nation’s economy, security, and wellbeing. In order to keep up with the rising population, there is a great need for more efficient and cost-effective technologies and techniques to not only repair the infrastructure, but also to advance and expand the transportation infrastructure to sustain the growing population. Building Information Modeling (BIM) has been widely adopted in the building industry, and its established methods and technologies show enormous potential in benefiting the transportation industry. The purpose of this paper is to present a literature review and critical analysis of BIM for transportation infrastructure. A total of 189 publications in the area of BIM for transportation infrastructure were reviewed, including journal articles, conference proceedings, and published reports. Additionally, schemas and file formats from 9 main categories and 34 areas related to transportation infrastructure were reviewed. An application was developed to collect, store, and analyze the publications. Various algorithms were developed and implemented to help in the automation and analysis of the review. The goal of this paper is to provide a comprehensive, up-to-date literature review and critical analysis of research areas regarding BIM for transportation infrastructure to further facilitate research and applications in this domain. Based on the results of the analysis, current topics and trends, applications and uses, emerging technologies, benefits, challenges and limitations, research gaps, and future needs are discussed. Significantly, the contribution of this paper is providing the foundation of current research, gaps, and emerging technologies needed to facilitate further research and applications for both academia and industry stakeholders to develop more efficient and cost-effective techniques necessary to repair, advance, and expand the transportation infrastructure. Furthermore, the results show that the use of BIM for transportation infrastructure has been increasing, although the research has mainly been focusing on roads, highways, and bridges. The results also reveal a major need for a standard neutral exchange format and schema to promote interoperability. Most importantly, the continuing collaboration between academia and industry is required to mitigate most challenges and to realize the full potential of BIM for transportation infrastructure. Gerontechnology is a new cross-disciplinary and multidisciplinary field dedicated to implementation of novel technologies in order to meet the complex needs of daily life of older people. It becomes more and more important due to the phenomenon of demographic aging that is present in Romania too. Various methods of adapting environment and assisting older people in order to help them remain as long as possible in their own homes are presented. A novel ecosystem to promote the physical, emotional and psychic health and well-being of the elderly is presented. Our proposal was designed to add several services developed to meet the needs of the senior population, namely services to improve social inclusion and increase contribution to society. Moreover, the solution monitors the vital signs of elderly individuals, as well as environmental parameters and behavior patterns, in order to seek eminent danger situations and predict potential hazardous issues, acting in accordance with the various alert levels specified for each individual. The article shows how sociomaterial practices of ordering temporality can become part of labor and workers identities, when homecare workers who work with 'time-saving' technology experience a lack of sufficient time to do their work. The article offers the concept 'time labour' to describe this practice, and defines it as homecare workers methods of saving time, by cutting visits to care recipients as short as possible, and by using technology to strategically produce and maintain quantitative representations of themselves as skilled workers. The term time labor conceptualizes these methods as sociomaterial enactments of temporality that are an intrinsic part of the workers ability to perform that work as well as their ability to produce and maintain a professional identity. The article argues that the role of technology is never predictable, but always emerges in use, and suggests a turn toward domestication theory in practice theoretical studies of work with technology. Advancements in medical science and technology, medicine and public health coupled with increased consciousness about nutrition and environmental and personal hygiene have paved the way for the dramatic increase in life expectancy globally in the past several decades. However, increased life expectancy has given rise to an increasing aging population, thus jeopardizing the socio-economic structure of many countries in terms of costs associated with elderly healthcare and wellbeing. In order to cope with the growing need for elderly healthcare services, it is essential to develop affordable, unobtrusive and easy-to-use healthcare solutions. Smart homes, which incorporate environmental and wearable medical sensors, actuators, and modern communication and information technologies, can enable continuous and remote monitoring of elderly health and wellbeing at a low cost. Smart homes may allow the elderly to stay in their comfortable home environments instead of expensive and limited healthcare facilities. Healthcare personnel can also keep track of the overall health condition of the elderly in real-time and provide feedback and support from distant facilities. Background: Aging at home rather than in an institution is now considered the gold standard. Public health figures document an important demographic transition to an increasingly elderly society. At the same time, this is accompanied by the emergence of significant numbers of innovative technologies to help and support home-dwelling older adults in declining health who wish to remain at home. Study aim: To explore the acceptability of intelligent wireless sensor system (IWSS) among home-dwelling older adults in rapidly detecting their health issues. Methods: Data were sourced from a pilot 3-month randomized clinical trial that involved 34 older patients in the experimental group (EG) using an IWSS to rapidly detect falls and other health issues at home. The effectiveness of the IWSS was assessed by comparing it to participants' functional and cognitive status, as measured both before and after the trial. The Resident Assessment Instrument for Home Care, Confusion Assessment Method, Cognitive Performance Scale, Geriatric Depression Scale, and Informed Questionnaire on Cognitive Decline in the Elderly were used for the assessments. Acceptability of the IWSS was explored at the end of the study. Results: Both older adults and their informal caregivers considered the performance and usefulness of the IWSS intervention to be low to moderate. A majority of the participants were unsatisfied with its ease of use and found multiple obstacles in using and having an intention to use the IWSS. However, their informal caregivers were more satisfied with the program and gave higher scores for usefulness, ease of use, and intention to use IWSS technology. Conclusion: The IWSS displayed low-to-moderate acceptability among the older participants and their informal caregivers. Background: Around the world, populations are aging and there is a growing concern about ways that older adults can maintain their health and well-being while living in their homes.Objectives: The aim of this paper was to conduct a systematic literature review to determine: (1) the levels of technology readiness among older adults and, (2) evidence for smart homes and home-based health-monitoring technologies that support aging in place for older adults who have complex needs.Results: We identified and analyzed 48 of 1863 relevant papers. Our analyses found that: (1) technology-readiness level for smart homes and home health monitoring technologies is low; (2) the highest level of evidence is 1b (i.e., one randomized controlled trial with a PEDro score ≥6); smart homes and home health monitoring technologies are used to monitor activities of daily living, cognitive decline and mental health, and heart conditions in older adults with complex needs; (3) there is no evidence that smart homes and home health monitoring technologies help address disability prediction and health-related quality of life, or fall prevention; and (4) there is conflicting evidence that smart homes and home health monitoring technologies help address chronic obstructive pulmonary disease.Conclusions: The level of technology readiness for smart homes and home health monitoring technologies is still low. The highest level of evidence found was in a study that supported home health technologies for use in monitoring activities of daily living, cognitive decline, mental health, and heart conditions in older adults with complex needs. Background: As gait speed and transfer times are considered to be an important measure of functional ability in older adults, several systems are currently being researched to measure this parameter in the home environment of older adults. The data resulting from these systems, however, still needs to be reviewed by healthcare workers which is a time-consuming process.Methods: This paper presents a system that employs statistical process control techniques (SPC) to automatically detect both positive and negative trends in transfer times. Several SPC techniques, Tabular cumulative sum (CUSUM) chart, Standardized CUSUM and Exponentially Weighted Moving Average (EWMA) chart were evaluated. The best performing method was further optimized for the desired application. After this, it was validated on both simulated data and real-life data.Results: The best performing method was the Exponentially Weighted Moving Average control chart with the use of rational subgroups and a reinitialization after three alarm days. The results from the simulated data showed that positive and negative trends are detected within 14 days after the start of the trend when a trend is 28 days long. When the transition period is shorter, the number of days before an alert is triggered also diminishes. If for instance an abrupt change is present in the transfer time an alert is triggered within two days after this change. On average, only one false alarm is triggered every five weeks. The results from the real-life dataset confirm those of the simulated dataset.Conclusions: The system presented in this paper is able to detect both positive and negative trends in the transfer times of older adults, therefore automatically triggering an alarm when changes in transfer times occur. Research problem: The purpose of this study was to investigate the effects of two types of motivational elements—verification steps and personal stories—in an instruction manual for a cell phone targeted at senior users (between 60 and 70 years). Research questions: What are the effects of adding verification steps and personal stories to an instruction manual on: (1) Seniors' confidence in being able to work with a cell phone? (2) Seniors' motivation to work with a cell phone? (3) Seniors' effectiveness and efficiency in performing tasks, and their satisfaction with the cell phone and the instruction manual (usability)? Literature review: To optimize products and services for older users, the literature suggests that two specific motivational elements may be beneficial: (1) verification steps, which provide information at the end of procedures to allow users to check if these procedures were performed correctly. These appear to users as part of conventional procedural information in user instructions and (2) personal stories, which are anecdotes or testimonials that describe how a fictitious user succeeded in completing a procedure. Methodology: We conducted a quantitative experiment in which 59 seniors between 60 and 70 years of age participated. They performed a number of tasks with a cell phone, using a version of the instruction manual with added verification steps, or a version with additional personal stories, or a control version without motivational elements. We measured participants' confidence, their motivation, and usability by asking them to answer several questionnaires and by measuring task performance. Results and conclusions: Results support the use of verification steps or personal stories as motivational elements in user instructions as they can positively affect confidence, motivation, and usability, especially when senior users encounter setbacks. The incidence of cognitive impairment in older age is increasing, as is the number of cognitively impaired older adults living in their own homes. Due to lack of social care resources for these adults and their desires to remain in their own homes and live as independently as possible, research shows that the current standard care provisions are inadequate. Promising opportunities exist in using home assistive technology services to foster healthy aging and to realize the unmet needs of these groups of citizens in a user-centered manner. ISISEMD project has designed, implemented, verified, and assessed an assistive technology platform of personalized home care (telecare) for the elderly with cognitive impairments and their caregivers by offering intelligent home support services. Regions from four European countries have carried out long-term pilot-controlled study in real-life conditions. This paper presents the outcomes from intermediate evaluations pertaining to user satisfaction with the system, acceptance of the technology and the services, and quality of life outcomes as a result of utilizing the services. The population of older adults is growing rapidly worldwide, but technical communicators have not accounted for the needs of these audiences, nor drawn from the wide range of research on aging. This article suggests four challenges that practitioners, educators, and researchers must undertake to accommodate older adults' physical, cognitive, and emotional needs: refine the demographic variable of age, operationalize age to enrich current methods of audience analysis, investigate multidisciplinary sources of aging research, and participate in research on aging by offering our expertise in document design and communication strategies. Wearable devices play a growing role in healthcare applications and disease prevention. We conducted a retrospective study to assess cardiovascular and pre-frailty risk during the Covid-19 shelter-in-place measures on human activity patterns based on multimodal biomarkers collected from smartwatch sensors. For methodology validation we enrolled five adult participants (age range: 32 to 84 years; mean 57 ± 22.38; BMI: 27.80 ± 2.95 kg/m2) categorized by age who were smartwatch users and self-isolating at home during the Covid-19 pandemic. Resting heart rate, daily steps, and minutes asleep were recorded using smartwatch sensors. Overall, we created a dataset of 464 days of continuous measurement that included 50 days of self-isolation at home during the Covid-19 pandemic. Student's t-test was used to determine significant differences between the pre-Covid-19 and Covid-19 periods. Our findings suggest that there was a significant decrease in the number of daily steps (−57.21%; −4321; 95% CI, 3722 to 4920) and resting heart rate (−4.81%; −3.04; 95% CI, 2.59 to 3.51) during the period of self−isolation compared to the time before lockdown. We found that there was a significant decrease in the number of minutes asleep (−13.48%; −57.91; 95% CI, 16.33 to 99.49) among older adults. Finally, cardiovascular and pre-frailty risk scores were calculated based on biomarkers and evaluated from the clinical perspective. [ABSTRACT FROM AUTHOR]

The emerging wearable medical devices open up new opportunities for the provision of health services and promise to accelerate the development of novel telemedical services. The main objective of this study was to investigate the desirable features and applications of telemedical services for the Polish older adults delivered by wearable medical devices. The questionnaire study was conducted among 146 adult volunteers in two cohorts (C.1: <65 years vs. C.2: ≥65 years). The analysis was based on qualitative research and descriptive statistics. Comparisons were performed by Pearson's chi-squared test. The questionnaire, which was divided into three parts (1-socio-demographic data, needs, and behaviors; 2-health status; 3-telemedicine service awareness and device concept study), consisted of 37 open, semi-open, or closed questions. Two cohorts were analyzed (C.1: n = 77; mean age = 32 vs. C.2: n = 69; mean age = 74). The performed survey showed that the majority of respondents were unaware of the telemedical services (56.8%). A total of 62.3% of C.1 and 34.8% of C.2 declared their understanding of telemedical services. The 10.3% of correct explanations regarding telemedical service were found among all study participants. The most desirable feature was the detection of life-threatening and health-threatening situations (65.2% vs. 66.2%). Background: Most seniors wish to live independently for as long as possible. Gerontechnologies such as personal alarms or remote control systems, have the potential to help them age in place. For seniors, assessing what is the most appropriate technology for their aging in place needs can be difficult. Professionals specifically tasked with matching seniors' needs with technology solutions can greatly help here. Yet not much is known about the challenges these professionals face or how they can optimize their matchmaking service.Methods: Participatory action research was conducted in the Netherlands, in two phases. In phase one, ten matchmaking dialogues between municipal technology consultants and seniors were observed, followed by interviews with both technology consultants and seniors to understand the current matchmaking service. In phase two, a new matchmaking tool was co-created with technology consultants and other professionals over the course of four co-creation session. Variants of the tool were tested out in nine additional matchmaking dialogues. The Cycle of Technology Acquirement by Independent-Living Seniors (C-TAILS) model, which can be used to understand both origins and consequences of technology acquirement by independent-living seniors, was used as a theoretical lens.Results: Important challenges for municipal technology consultants in their current matchmaking practice are: making the matchmaking service more demand oriented and creating an accurate and complete overview of relevant factors within the seniors' individual situation so that an optimal match can be made. Together with technology consultants and other professionals, a new Gerontechnologies Matchmaking (GTM) tool was created to help overcome these challenges. Evaluation of the tool showed that it better includes each senior's personal, social, physical and technological context, within the matchmaking service.Conclusion: Professionals who help seniors match gerontechnology to their aging in place needs experience a variety of challenges in the delivery of their service. Currently, few tools are available for them to overcome these challenges. The newly developed GTM tool can help overcome challenges and optimize matchmaking services. Introduction In order to take into account the societal problem of aging, the Direction de l’Autonomie (social section for elderly and disabled people) of the Var Conseil Général expressed the needs to get a 3D tool using Virtual Reality enabling elderly and/or disabled people to experiment assistive technologies and to inform and educate professionals involved in housing affordability, in order to facilitate information on in-home support/care services, know-how's, existing innovative technical solutions. Material/method 3D design of the apartment is the result of a discussion between patient, family, caregiver, therapist. It is carried out with the 3DSMAX Autodesk software by integrating AEC (Architecture Extended Objects) tool, which makes it completely open to all requests. The 3D integration of all elements in the virtual apartment is carried out with UNITY3D game creation engine (Unity Technologies), enabling to do animations and actions on environment with scripts programmed in C# or JavaScript. All orders are made thanks to a virtual tablet, integrated to ATOLL. Results ATOLL enables a fully customizable virtual apartment whereas user can move as if he is its tenant. Codesign involving both the designer who conceives and the future real life user seeks an optimum autonomy. It implements the latest techniques used in home automation and allows the choice of works to be done concerning new constructions, purchase of technical aids, case studies of use. to achieve an optimized adaptive solution to the situation of the elderly/disabled, the formalization of his needs and likes through virtual testing. All the elements are a basis for discussion among the elderly/disabled, members of his family, an architect, an artisan. and sustainability of funding. Discussion Our society takes time to adapt to this “revolution” of advanced age. In fact, the elderly are too often victims of discrimination and appear only as individuals with a minor or non-existent social role. It therefore becomes necessary to adapt our mentalities and this requires a greater work against discriminations and adaptation of housing for older people to remain in their homes as long as possible.
