The aim of this work was to analyze the spatial and temporal features of the epidemic process dynamics of tick-borne encephalitis (TBE) in Irkutsk Region in 2001–2021.Materials and methods. The data of the Rospotrebnadzor Administration for the Irkutsk Region, the Reference Center of the Irkutsk Research Anti-Plague Institute, the materials of the state statistical reporting form No. 2 “Information on infectious and parasitic diseases”, and scientific publications were analyzed. The chronological and chorological features of the change in the incidence of TVE have been traced. In the latter case, the administrative-territorial formations (ATF) of the subject were differentiated by epidemiological risk groups over two ten-year periods: 2001–2010 and 2011–2020. At each time interval, the grouping of areas according to the level of TBE incidence was carried out using the calculation of 95% confidence interval. Time sequences of observations were analyzed using the construction of regression equations. Standard methods of variation statistics in the Microsoft Excel were deployed.Results and discussion. Over the course of the 21st century, the incidence of TBE in Irkutsk Region has been declining. Based on 2001–2010 data, there were no cases of TBE in six ATF, and endemic areas were divided into three groups: with low (15 districts), medium (6) and high (8) incidence of TBE. A separate assessment of morbidity rates was conducted in the administrative center of the entity, the city of Irkutsk. In 2011–2020, the structure of ATF with varying epidemiological risk of TBE underwent a change. Six non-endemic areas, groups of low (12 ATFs), medium (9), high (6) and very high (2) epidemiological risk were identified. The spatial arrangement of high epidemiological risk zones changed, and their area increased. Each ATF cluster with different TBE incidence in 2011–2020 is characterized by the number of TBE cases and the volume of measures to prevent the infection. It is concluded that preventive measures were insufficient in ATF groups of high and very high epidemiological risk.

Abstract Background Babesiosis is a globally growing tick-borne disease in humans. Severe babesiosis caused by Babesia divergens has been reported in two patients from Asturias (Northwestern Spain), suggesting an undetected risk for the disease. To analyze this risk, we retrospectively evaluated the seroprevalence of babesiosis in the Asturian population from 2015 through 2017, a period covering the intermediate years in which these two severe cases occurred. Methods Indirect fluorescent assay (IFA) and Western blot (WB) were performed to detect B. divergens IgG antibodies in 120 serum samples from Asturian patients infected with the tick-transmitted spirochete Borrelia burgdorferi sensu lato, a condition that indicates exposure to tick bites. Results This retrospective study confirmed a B. divergens seroprevalence rate of 39.2% according to IFA results. B. divergens incidence was 7.14 cases/100,000 population, exceeding previously reported seroprevalence rates. No differences in epidemiology and risk factors were found between patients infected solely with B. burgdorferi s.l. and those infected with B. burgdorferi s.l. and with IgG antibodies against B. divergens. This last group of patients lived in Central Asturias, had a milder clinical course and, according to WB results, developed different humoral responses against B. divergens. Conclusions Babesia divergens parasites have circulated for several years in Asturias. Epidemiological evidence of babesiosis makes Asturias an emerging risk area for this zoonosis. Human babesiosis could also be relevant in other Spanish and European regions affected by borreliosis. Hence, the potential risk of babesiosis on human health in Asturias and other European forest regions needs to be addressed by the health authorities. Graphical Abstract

Tick-borne viral encephalitis (TBVE) is one of the most significant natural-focal infections in the Russian Federation.The aim of the study was to analyze the current epidemiological situation on TBVE in the Buryat Republic in 2010–2020 with a subsequent differentiation of municipalities by epidemiological risk groups in order to elaborate proposals for optimization of preventive measures.Materials and methods. A retrospective analysis of the epidemiological situation on TBVE in the Buryat Republic was carried out using forms of federal statistical surveillance No. 2 “Information on infectious and parasitic diseases” over 2010–2020 and the data from the Reference Center of the Irkutsk Research Anti-Plague Institute of Siberia and Far East of the Rospotrebnadzor on the epidemiological situation and preventive measures in the municipalities of the constituent entity. Statistical processing was performed applying conventional methods of variation statistics. Based on calculated 95 % parametric confidence interval for the data on variability of the long-term average TBVE incidence in the municipalities of the Republic of Buryatia over a 10-year period, the entities were differentiated by epidemiological risk groups. QGis 2.18.28 and a set of open geodata OpenStreetMap were used for mapping.Results and discussion. All municipalities have been classified into five groups by the level of epidemiological risk: with zero TBVE incidence – 2 districts, with a low level – 4, medium – 8, high – 5, very high – 2. In addition, the administrative center has been placed into a separate group. Each individual group of municipalities is characterized by the number of TBVE cases, the level of morbidity, the frequency of seeking medical aid because of tick bites, the scope of specific and non-specific prevention measures. Recommendations for optimizing the tactics of TBVE prevention in certain municipal districts have been provided.

Toxoplasmosis is a disease caused by a protozoan called Toxoplasma gondii, it manifests itself with great morbidity and mortality in fetuses, newborns and immunocompromised patients, however, toxoplasmosis can also manifest itself in immunocompetent patients suffering from: chorioretinitis, lymphadenopathy, pneumonia, brain abscess, myositis, myocarditis and hepatitis. The objective of this investigation was to identify the clinical manifestations of toxoplasmosis in pork meat and the possible risks to human health. A literature review was performed in the Scopus, Cochrane, Web of Science, PubMed, Library, and Embase databases up to October 2022. Reference lists of all major articles found electronically were also hand searched, which allowed obtaining 85 records, of which 19 were selected. Toxoplasmosis is a common infectious disease in developing, tropical, and subtropical countries around the world. The incidence has increased in rural areas and in lower socioeconomic groups. The aim of the work was information-analytical assessment of the epidemiological situation on infectious diseases that are potentially or truly dangerous in terms of occurrence of emergencies of sanitary-epidemiological nature in the Region of Americas.Materials and methods. The study was based on the official reports of the WHO, the Pan American Health Organization, the Centers for Disease Control and Prevention, the national Ministries of Health, data from the ProMED information portal, the Global Network for the Epidemiology of Infectious Diseases, and published scientific papers.Results and discussion. By the model of the Americas, regional epidemiological features have been established, including the endemicity (enzooticity) of territories according to the most relevant nosological forms and the intensity of the epidemic process manifestations. It is shown that the main epidemiological risks in the countries of Central, South America and the Caribbean are associated with dengue, Zika, Chikungunya fevers characterized by a wide territorial dissemination and the ability to cause large-scale epidemic outbreaks, in the countries of North America – West Nile fever. Other infections of international concern include: cholera, that twice caused epidemics of imported origin during the seventh pandemic, which changed the structure of world morbidity; plague, manifested in an annual incidence, including with a complication by the pneumonic form, which determines an increased potential danger of anthropogenic spread; malaria, demonstrating an upward trend in morbidity and the number of intra-continental imported cases; yellow fever, characterized by the activation of natural foci and the expansion of the territories of potential pathogen transmission. The data obtained can serve as a basis for assessing the risks of infectious disease introduction from the American Region into safe territories, improving epidemiological forecasting and validity in making managerial decisions when conducting sanitary and anti-epidemic (preventive) measures.

The region of Campania, South Italy rose to prominence in the mid-2000s due to the illegal disposal of waste by the Camorra during the local waste management crisis. Several lines of evidence have identified a link between exposure to toxic waste and adverse health outcomes in the local populations. Critically, studies since 2017 have strongly suggested that this link is causal in nature. The uncertainty of evidence polarised the Italian epidemiological community and partly undermined the precautionary principle in public health policy, leading to years of delay in the deployment of appropriate interventions. The crisis also sparked concerns about pollution of soil, water, and agricultural products. The contrast between political responses and protests from local communities shows analogies with environmental emergencies of a larger scale. Beyond law enforcement actions to prosecute illegal waste disposal activity, future mitigation of risks for affected populations will require coordinated efforts in environmental policy (land reclamation, improved waste management) and public health (i.e. extensive epidemiological surveillance, screening and prevention programs). By summarising evidence over the last two decades, this review aims to construct a cohesive interdisciplinary narrative of the events in the Campanian waste crisis.

Background. Dirofilariasis is a vector-borne helminthiasis that tends to expand spatial boundaries in areas with temperate and cold climates. In the Voronezh oblast, located in the Central Black Earth Region of the Russian Federation there were previously isolated reports of cases of dirofilariasis in humans and dogs, but this issue was not specifically studied.Aims. To determine the degree of infection of dogs with Dirofilaria immitis and Dirofilaria repens in the Voronezh oblast in order to determine the level of epidemiological risk.Materials and methods. For seven years, targeted work was carried out to study the spread of different types of dirofilariae in dogs in the Voronezh region. Blood from 3 498 dogs of different breeds, ages, conditions of keeping and household use was examined. Dirofilaria larvae were counted using the Fuchs-Rosenthal camera. The type of dirofilaria larvae isolated from the blood of dogs was determined by the histochemical method. The antigen of the imaginal form of D. immitis was detected using a chromatographic test system the ImmunoRun Antigen Detection Kit Caninae Heartworm Biogal chromatographic test system (Israel). Statistical processing of the material was carried out in the Microsoft Excel 2010 program. Studies were carried out with the consent of animal owners and in accordance with the European Convention for the Protection of Vertebral Animals used for experimental and other scientific purposes (Strasbourg, 1986).Results. As a result of the conducted studies, it was found that in the Voronezh oblast, the average infection of domestic dogs (Canis familiaris) with dirofilariasis is 14.14 ± 0.31 %. The indicator depends on the conditions of keeping animals and makes 27.15 % in stray dogs, 25.26 % – in service dogs and 10.54 % – in apartment dogs. In service dogs, the species D. repens occurs in 52.39 % of cases, D. immitis – in 42.95 %, mixtinvasia – in 4.80 % of cases. In stray dogs, the species D. repens prevails (80.10 %). The second type of D. immitis was established in 10.0 % of cases. At the same time, both types were also established in 10.0 % of cases. The maximum number of cases of dirofilariasis occurs in urban dogs (65.87 %), which is due to the high density of their population and close contact with the vector of transmission in urbanized areas. The seasonal dynamics of infestation is determined by the year-round detection of cases with a peak in October (46.77 %). The number of cases of dirofilariasis increases with age, in dogs from one to six years old it reaches maximum of 63.26 %. There is an epidemiological risk of dirofilariasis in the Voronezh oblast. This is due to the existence and active functioning of synanthropic and natural foci of invasion on the territory of the Voronezh oblast.

In 2018, between June14 and July 15, final matches of the FIFA World Cup-2018 were held in 11 cities of the Russian Federation. That event was the biggest mass event with international participation in the history of Russia. During the period, in the process of sanitary-epidemiological welfare provision, developed earlier scintific-and-practical achievementds for prevention of emergency situations of sanitary-epidemiologoical character were implemented to the fullest extent. Objective of the study – analysis of evolution of the developed in Russia scientifically-substantiated approaches in the sphere of sanitary-epidemiological welfare provision of mass events and aggregation of the results of their implementation during preparation and holding of FIFA World-Cup-2018. Utilized were information-analytical materials from the Rospotrebnadzor Administrations by the constituent entities of the Russian Federatuion, Centers of Hygiene and Epidemiology in the entities of the Russian Federation, RusRAPI “Microbe” of the Rospotrebnadzor, data from Organizing Committee “Russia-2018”, Federal Turism Agency, information published in periodical press. Scientifically substantiated model of sanitary-epidemiological welfare provision during mass events was developed in the Russian Federation. It includes expert evaluation and quantification of potential epidemic hazard of mass event which allows for targeted prophylactic and anti-epidemic measures with rationalized loads and adequate frequency of repeat. The paper discusses the complex of measures carried out during preparation and holding of FIFA World Cup-2018 by the functional areas: sanitary protection of the territory; epidemiological surveillnace over relevant anthropozoonotic, natural-focal zoonotic, sapronotic infectious diseaseas; sanitary surveillance over communal facilities, catering facilities, sites of accomodation of the participants and guests; sanitary-hygienic monitoring of ambient environment objects; provision of anti-epidemic preparadeness and readiness of the Rospotrebnadzor authorities and institutions and medical organizations; provision of readiness of laboratory facilities; involvement of additional forces and capacities and interagency cooperation in response to emergency situations of sanitary-epidemiological character.

Since 1933 in the Rostov region, the official registration of tularemia began. In 1964-1949 the highest incidence was noted. Since 1947, they have been vaccinated against tularemia. In 1966 to 1973, there were no cases of tularemia. In July-August 1993, a large outbreak of tularemia (more than 200 people) was recorded. Until early 2017, the epidemic situation in tularemia in the Rostov region, according to the data of long-term monitoring, was assessed as stable, but low coverage with vaccinations against tularemia, both in the population of endemic territories and in individuals of certain professional categories was recorded. In january 2017, two residents of Rostov-on-Don received a clinical diagnosis of «tularemia». In june and july 2017, three cases of tularemia were reported. The most effective mechanism for preventing the spread of tularemia remains vaccination of the population from the contingent of risk. We consider it advisable to conduct studies of the immunity to tularemia in the population vaccinated in the last five years, to significantly increase the reliability of short- and long-term prognosis for tularemia in the region.

In the submitted report from modern theoretical positions the concept of risk of epidemiology is surveyed. Definitions are given to categories: «epidemiological risk», «risk factor», «risk group», «territory of risk» and «risk time». Hierarchical systematization of the discussed concepts is given.

Objective of the study was to develop the methodology for identification of administratively independent areas under potential risk of “external” epidemiological threat realization, by the example of cholera.Materials and methods. Analysis was conducted using free software package with open source code (QGIS 2.8 and GRASS GIS 7.0) on the basis of the data received from Rosgranitsa and the Federal State Statistics Service. The construction of risk cartogram was performed on the base of Euclidean distance and estimation of nuclear density.Results and conclusions. In accordance with the obtained results, the GIS containing information about the checkpoints on the Russian border, settlements, roads and railway lines was worked out. The method of identification of risk areas due to importation of infectious diseases based on the cholera model has been developed, and the total area of such territories was less than 1 % of the total area of the country. It was found that in some cases the risk area is located at a certain distance from the checkpoint, but the existence of checkpoint does not lead to the formation of risk areas. The developed GIS is available on the geo-information portal of FGHI Rostov-on-Don Research Anti-Plague Institute of Rospotrebnadzor.

This work is devoted to the assessment of the vulnerability of the Sary-Jaz autonomous focus of plague by using of GIS technologies. This focus of the plague covers a very large and difficult to access territory. The annual survey and disarming of this territory is very expensive. Therefore, it is necessary to select sectors that need more attention. That why, it is necessary to assess the vulnerability of the population in various sectors. Now in the world literature there is a lot of work on the assessment of vulnerability from these or those dangers, but there is no definition of hazard, risk and vulnerability indices for the focus of plague. The paper presents the results of the determination of these indices and calculates indices for each sector of the natural focus of the plague. As a result of the work, the final map of the Index of the epidemiological vulnerability of the Sary-Jaz autonomous focus of the plague was obtained. Detail results of the assessment and the final map for the study area are given. The authors hope that the calculations and results obtained in the course of this work will find their application both for theoretical assessments of the vulnerability of various territories from plague and for practical actions to reduce vulnerability.

Introduction: Water is a convenient environment for the spread of pathogenic bacteria. Their presence may result from contact with water through soil, air, and animal and human activity. The identification of microorganisms allows us to determine the biological properties, the possible epidemiological risk, and the sanitary condition of the surface water. Aim of the research: To analyse the presence of microbiological indicators in selected water samples collected in autumn, winter, and spring. Material and methods: The research sites included three rivers and two water reservoirs located in Świętokrzyskie voivodeship. The water samples were tested based on the presence of coliform, mesophilic, and psychrophilic bacteria. Results: The presence of E. coli was confirmed in all water reservoirs, but values above the standard limit were confirmed in just three cases (Wisła, Bernatka, and Emerald Lake). The results show that the changing microbiological parameters of the analysed water samples can result from different factors. The large number of bacteria in water can come from other local streams and groundwater, human and animal activity, and especially from influx of domestic wastewater. The sanitary state of surface water in Świętokrzyskie is consistent with data from other Polish voivodeships published in the literature. Conclusions: Microbiological pollution of surface water in Poland is a constant and underestimated problem. It can be a serious epidemiological threat, especially in mild winters and warmer summers. The article presents the results of the case follow-up of the frequency, structure and dynamics of the malignant tumors in the course of the regional social hygienic monitoring. We have considered the unified research principles and technologies of epidemiological type with the use of modern methods of retrospective analysis, prediction and the real risk assessment. It is shown that in the period 1999-2013 at the downward trend in cancer mortality in Rostov Region, the primary cancer incidence had a stable growth trend. Among the priority locations an unfavorable upward trend of the frequency of malignancies in children under 14 years old was formed, namely: malignant tumors of the esophagus, colon, rectum, melanoma of skin, breast, cervix and uterus, malignant lymphoma. On the basis of the regional criteria risk of morbidity of the population in Rostov region, the amount of malignant tumors and cancer-related deaths within the period 1999-2013 years was assessed as moderate. The increased risk for a specified period was detected only in relation to cancer incidence in the cities of the regional subordination, among which are priority cities: Volgodonsk, Taganrog and Azov.

To improve the sanitary and epidemiological surveillance at the Olympic Games has developed a system of GIS for monitoring objects and situations in the region of Sochi. The system is based on software package ArcGIS, version 10.2 server, with Web-java.lang.Object, Web-server Apach, and software developed in language java. During the execution of the tasks are solved: the stratification of the region of the Olympic Games for the private and aggregate epidemiological risk OCI various etiologies, ranking epidemiologically important facilities for the sanitary and hygienic conditions, monitoring of infectious diseases (in real time according to the preliminary diagnosis). GIS monitoring has shown its effectiveness. Information received from various sources, but focused on one portal. Information was available in real time all the specialists involved in ensuring epidemiological well-being and use at work during the Olympic Games in Sochi.

The epidemiological risk factors causing calcification of aortic valves have also been found to cause atherosclerosis. Aim: To analyse the epidemiological risk factors causing aortic valve sclerosis which have been studied in details and analysed to see whether they cause any significant increase in the incidence of cardiovascular events. Materials and Methods: This prospective case-control study was conducted between 1st Jan 2015 to 31st Dec 2015 in NEIGRIHMS hospital and data for age, gender, socioeconomic status, hypertension, diabetes, tobacco use, Body Mass Iindex (BMI), cholesterol levels, Electrocardiography (ECG) changes and Ejection Fraction (EF) were collected and analysed by using SPSS software version 22. Results: Hypertension, diabetes, weight, BMI, hyperglycaemia and hyperlipidemia were not found to be significantly associated with aortic valve sclerosis in patients presenting with acute coronary syndromes. The presence of aortic valve sclerosis was also not associated with increased risk of cardiovascular mortality and morbidity. Conclusion: The risk factors for atherosclerosis were found to be associated with the presence of aortic valve sclerosis more in the control group and hence finding of a sclerosed aortic valve in the apparent normal population might identify those persons at increased risk of developing coronary artery disease and appropriate preventive measures should be taken before the disease sets in.

The purpose of this study was to determine priority directions of anti-epidemic readiness of the Russian Federation Region for emergency situations associated with infectious diseases dangerous for humans, at the present stage. Readiness of the Russian Federation region for emergency situations associated with infectious diseases dangerous for humans (anti-epidemic readiness) implies a set of preventive and anti-epidemic measures, implementation of relevant management decisions at the Regional level, which aim to ensure the safety of people living (temporarily located) in conditions of epidemiological risks. Anti-epidemic readiness is provided through targeted distribution of logistics, human resources, scientific methods, and informational resources of public health of the region, taking into account the priority of the organization of preventive measures especially for those infectious diseases that define dominant epidemiological risk in a particular municipality. Efficient use of resources is not possible without administrative, legal and financial support. As the result of the research priority directions of anti-epidemic readiness of the Russian Federation region for emergency situations associated with infectious diseases dangerousfor humans, taking into account the dominant epidemiological risks, are formulated.

Mass gatherings (MGs) always incur some kind of potential hazard associated with incipiency of emergency situations with sanitary-epidemiological bias (ES). Upcoming Universiade (World Student Games) in Kazan (July, 2013) raises an issue of implementation of a unified system of approaches to the effective provision of sanitary-epidemiological welfare of the population, taking due account of peculiarities and specific conditions under which the MG will be conducted. Therewith key objective of this study has been formulated as elaboration of the system based on epidemiological assessment of real and potential threats at the MG. Analyzed have been the data on 37 MGs carried out over the period of 25 years since 1987 up to 2012; investigated is epidemiological situation in 170 participating states. Worked out is a scheme of qualitative determination of potential epidemiological threat on the basis of comprehensive assessment of epidemiological risks in the endemic territories of the troubled regions. Distinguished is a complex of measures for the provision of sanitary-epidemiological welfare when managing the Universiade in Kazan heedful of high potential epidemiological hazard of this event.

In the last decade outbreaks and epidemics of infectious diseases dangerous for humans cause tension epidemiological situation in the world and in the Russian Federation. Epidemic manifestation of these diseases has symptoms of emergency situations and requires the use of additional forces and means of Public Health, as well as administrative, legal and financial support. The purpose of this study is formulation of the concept of "public health emergency situations associated with diseases dangerous for humans". In considering of definitions "emergency situation", "disease" and "event" in international and national law, the thesis is confirmed that epidemic manifestation of infectious diseases dangerous for humans, are signs of an emergency. In addition, the interpretation of these terms focuses on the distinction between potential risk and its actual implementation. Consequently, the threat of public health emergency situations should be viewed from the risk methodology, by which we proposed to evaluate internal and external epidemiological risks and to define its dominance for each municipality of the Russian Federation Region. Infectious diseases dangerous for humans which able to cause public health emergency situations, can be divided into three groups. 1. Diseases that form the external epidemiological risk - dangerous infectious diseases requiring measures for sanitary protection of the territory of the Russian Federation. 2. Diseases that form the internal epidemiological risk - dangerous infectious diseases, registration both single and mass indigenous events requires emergency response and using of additional forces and means of public health of the Region. 3. Infectious diseases of importance in regional pathology, prevention of which is to implement long-term programs. In the framework of this paper the definition of "public health emergency situations associated with diseases dangerous for humans" is proposed. It focuses on the feasibility of upgrading approaches to priority directions of anti-epidemic readiness of the Russian Federation Region, taking into account the probability of epidemiological risks.

Emergency situations associated with infectious diseases dangerous for humans are actual for the Russian Federation. The probability of the occurrence of these events varies for different Russian Federation regions, but also for the individual municipalities of one region. Complex approach to the organization of anti-epidemic readiness for such situations is necessary. This approach should be based on differentiation of municipalities of a Russian Federation region according to the estimation of external and internal epidemiological risks associated. with diseases dangerous for humans. At the federal level the system of epidemiological surveillance is determined for diseases requiring measures for sanitary protection of the territory, and separately for almost every infectious disease, affecting the sanitary-epidemiological welfare of the population of the Russian Federation. In the frames of this study it is offered to consider the probability of the importation of infectious diseases, requiring measures for sanitary protection of the territory, as the external epidemiological risk. Diseases common to man and. animals, other endemic to the Russia infectious diseases, including relevant in regional pathology, are viewed as the potential and real internal epidemiological risk. The purpose of this study is the development of the control system of these epidemiological risks at the Municipal level. The three-level control system of the epidemiological risks according to differentiation of municipalities of a Russian Federation region by criteria of external and. internal epidemiological risks is proposed as a result of this work. The control system allows suggesting a priority orientation of organizational, preventive and anti-epidemic actions in municipalities and. promotes rational distribution of forces and means of Public Health Institutions in cases of emergency situations in the field of sanitary-epidemiological welfare associated with diseases dangerous for humans.

The risk of emergency situations in the sphere of sanitary-epidemiological welfare associated with diseases dangerous for population varies for different municipalities of a region of Russian Federation. Therefore a differentiation of a region for three types of territories (A, B, C) is necessary. Adequate number of sanitary protection measures is determined for each type of territories. The purpose of this study was the development of methodical reception for differentiation of a region of Russian Federation. Point scoping of criteria of external and. internal epidemiological risks for each of municipality is proposed as this method. Criteria of external epidemiological risk allow to take into account the probability of the importation of infectious diseases, requiring measures for sanitary protection of the territory. Criteria of internal epidemiological risk reflect both actual and. potential dangers: morbidity, the registration of particular nosologic forms of diseases in animals and. detection of pathogens in the living and. non-living environment objects. Assessment of internal risk is carried out in the analysis of information about epizootic and. epidemiological situation of infectious diseases, the list of which is proposed, with, consideration of their epidemiological significance and. can be used for any Russian Federation region. According to the results of total assessment of risks for each of municipalities the complex index of epidemiological risk (CIER) is determined. On the basis of CIER the territory type (A, B, C) is determined. Type A, B or C defines for the municipality the volume of measures for sanitary protection of the territory on the basis of a sufficient level for preparedness of forces and means of public health institutions. The method, based, on the assessment of external and. internal epidemiological risks, that allows to differentiate the region of Russian Federation, at municipal level for risk of diseases dangerous for people, is proposed, as the result of this work. A new methodical approach, will contribute to the increase of the preparedness of public health, institutions to emergency situations in the sphere of sanitary-epidemiological welfare associated, with, diseases dangerous for people.

Background: Urinary bladder cancer is the fourth most common malignancy in Western world. It is a heterogeneous disease with variable natural history. There is paucity of data in our country. Aims and Objectives: The present study aims to assess the risk factors contributing to urothelial carcinoma of bladder in the southern region of West Bengal and histopathological grading (WHO/ISUP). Materials and Methods: It was a retrospective study by multivariate analysis of case record proforma(CRP), agreed upon by a team of experts from the departments of Urosurgery and Pathology,CNMC&H, Kolkata from December, 2007 to November, 2009. Total 88 subjects who had at-tended in the department of urosurgery and al-ready been diagnosed either by cystoscopic bi-opsy or Transurethral Resection of Bladder Tumour (TURBT), were enrolled. Results: There was male preponderance (86.4 % male vs 13.6% female); the age range being 30-81 years(median 65). Hindus (81.8 %) were affected more compared to Muslims (18.2 %). Blood group analysis showed highest incidence in B group (41%) followed by O (32%). Tobacco smoking (75% cases) revealed a strong relationship with bladder cancer and that was quantity and duration dependant. Less water intake(53.4%) and poor socioeconomic status (82%)were significant contributory factors. Analysis of occupational factors gave no epidemiological clue. Histopathological Examination (HPE)revealed low grade urothelial carcinoma (51cases), high grade (24 cases) and papilloma with low malignant potential (13 subjects). Conclusion: Proper epidemiological survey plays a pivotal role in bladder cancer to validate it in the disease control programme.

The criteria approach has been proposed for the development of the database on the risks in the sphere of biological safety in the constituent unit of the Russian Federation. This approach includes application of the following indices: epidemiological, social and demographic ones, those regarding external and internal menaces to the biological safety, characteristics of biological and chemical objects of potential danger. The database is oriented for work as a part of geographical information systems.

The region of Campania, South Italy rose to prominence in the mid-2000s due to the illegal disposal of waste by the Camorra during the local waste management crisis. Several lines of evidence have identified a link between exposure to toxic waste and adverse health outcomes in the local populations. Critically, studies since 2017 have strongly suggested that this link is causal in nature. The uncertainty of evidence polarised the Italian epidemiological community and partly undermined the precautionary principle in public health policy, leading to years of delay in the deployment of appropriate interventions. The crisis also sparked concerns about pollution of soil, water, and agricultural products. The contrast between political responses and protests from local communities shows analogies with environmental emergencies of a larger scale. Beyond law enforcement actions to prosecute illegal waste disposal activity, future mitigation of risks for affected populations will require coordinated efforts in environmental policy (land reclamation, improved waste management) and public health (i.e. extensive epidemiological surveillance, screening and prevention programs). This paper discusses different needs and approaches to establishing "causation" that are relevant in legal cases involving statistical input based on epidemiological (or more generally observational or populationbased) information. We distinguish between three versions of "cause": the first involves negligence in providing or allowing exposure, the second involves "cause" as it is shown through a scientifically proved increased risk of an outcome from the exposure in a population, and the third considers "cause" as it might apply to an individual plaintiff based on the first two. The population-oriented "cause" is that commonly addressed by statisticians, and we propose a variation on the Bradford Hill approach to testing such causality in an observational framework, and discuss how such a systematic series of tests might be considered in a legal context. We review some current legal approaches to using probabilistic statements, and link these with the scientific methodology as developed here. In particular, we provide an approach both to the idea of individual outcomes being caused on a balance of probabilities, and to the idea of material contribution to such outcomes. Statistical terminology and legal usage of terms such as "proof on the balance of probabilities" or "causation" can easily become confused, largely due to similar language describing dissimilar concepts; we conclude, however, that a careful analysis can identify and separate those areas in which a legal decision alone is required and those areas in which scientific approaches are useful. Background: Identifying risk factors for Triatominae infestation is essential for the development of vector control interventions., Methods: To determine the intra- and peridomiciliary risk factors associated with triatomine infestation, a cross-sectional analytical study was carried out with random cluster sampling in two stages, which included the identification of risk factors by survey and direct observation, as well as the search and capture of triatomines. The detection of trypanosomes in triatomines was carried out by observing the rectal content and then by conventional polymerase chain reaction (PCR)., Results: In 21 of the 207 houses inspected, 13 specimens of R. colombiensis and 19 specimens of P. geniculatus were found. Entomological indices included: dispersion 36%, infestation 10%, infection 65%, colonization 4.7%, density 15%, and concentration 152%. An association was found between the presence of Triatominae and the existence of branches and fissures in the floors, as well as with the presence of accumulated objects and with knowledge about Chagas disease. The risk of having triatomines in urban homes is 5.7 times higher than the risk in rural areas [confidence interval (CI) 0.508-67.567]; 6.6 times in houses with cracked soil (CI 0.555-81.994), 6 times in houses located near caneys (CI 0.820-44.781), and 6.16 times with accumulated objects (CI 1.542-39.238)., Conclusion: Chagas disease is a complex problem that requires control based on the vector's elimination or surveillance, which implies identifying species and their distribution, generating alerts, knowledge, and awareness in the population. Background: The outbreak of coronavirus disease 2019 (COVID-19) began in December 2019 and continues to spread worldwide. Rapid and accurate identification of suspected cases is critical in slowing spread of the virus that causes the disease. We aimed to highlight discrepancies in the various criteria used by international agencies and highly impacted individual countries around the world., Methods: We reviewed the criteria for identifying a suspected case of COVID-19 used by two international public health agencies and 10 countries across Asia, Europe, and North America. The criteria included information on the clinical causes of illness and epidemiological risk factors. Non-English language guidelines were translated into English by a co-author who is fluent in that particular language., Results: Although most criteria are modifications of World Health Organization recommendations, the specific clinical features and epidemiological risks for triggering evaluation of patients with suspected COVID-19 differed widely among countries. The rationale for these differences may be related to each country's resources, politics, experience with previous outbreaks or pandemics, health insurance system, COVID-19 outbreak severity, and other undetermined factors., Conclusion: We found no consensus regarding the best diagnostic criteria for identifying a suspected case of COVID-19.

Background: The marketing of products of animal origin for human consumption, are of great importance, since its variety and features associated with the production, may not only reflect the hygienic sanitary conditions but also their level of epidemiological risk, in this sense, cultivation and sale of trout (Oncorhynchus mykiss) has massed in several regions of the country, and although they represent an opportunity for growth and economic and social development there is a place not calculated risk that several producers that supply local markets, are not related in the resulting records of food producers and do so without the proper registration. Objectives: To determine the microbiological analysis health hygienic status of trout (Oncorhynchus mykiss) marketed retail, in the city of Pamplona, Norte de Santander. Methods: For this purpose, were processed twenty-two 22 samples, which were analyzed according to the microbiological aspects contained in the Colombian technical standard 1443 / 2009 for e. coli, S. aureus coagulase positive, Salmonella spp. and Vibrio cholerae; the analyses were developed according to the horizontal methods for the study of microorganisms using solid media, supplemented by techniques of isolation and biochemical identification. Finally, he joined with a statistical study by calculating normal distribution, in order to determine the likelihood of potential risk of ETAs. Getting that trout according to their organoleptic characteristics proved to be an acceptable product without signs of alteration, modification or apparent decomposition, microbiologically was determined according to the standard, only 13.6% of samples complies with the established requirements; the greatest indicator of rejection is to e. coli with a 72.7%, while for Vibrio cholerae and Salmonella spp isolation frequencies were calculated between 9.09% and 22.7% respectively; for S. aureus population values were acceptable. Conclusions: There is a low degree of hygienic-sanitary compliance in this type of food, which can be linked to different causes, not identified in this study, but that determination by probabilistic microbiological criteria, the possibility of risk associated with the consumption was estimated at 42, 46% for E. coli, 38.59%, S. aureus; 7.21% salmonella spp and 0.09% V. cholerae.
