Abstract

Knee subchondroplasty (SCP) is one of the most novel minimally invasive methods for treating bone marrow lesions. The literature suggests that it is safe, with few complications and good outcomes. However, no studies have documented its usage for managing large subchondral bone cysts. This article outlines a case report and details the pearls and pitfalls of SCP in treating large subchondral bone cysts. Our patient underwent arthroscopic debridement with medial femoral condyle SCP. Mild posterior extravasation of synthetic bone substitute was observed on Postoperative Day 1, which was immediately rectified on revision arthroscopy. Gradual escalation of weight bearing and good pain relief were subsequently achieved, and the patient has remained complication-free after two years. No further extravasation were observed on repeat radiography. SCP is a feasible temporising measure that may help to delay the need for bone allograft or immediate knee arthroplasty in younger patients while retaining function and delaying loss of productivity. 14. Violent and Nonviolent Youth Offenders

Abstract

Youth violence is a costly social problem. This study compared the risk and needs of nonviolent youth offenders, with those who had committed violent offenses only (violent only) and those who had committed violent and nonviolent offenses (violent plus) to determine whether violent youth were a different “type” from nonviolent youth. The case files of 3,744 youth offenders (3,327 males and 417 females, between 12 and 18 years old) were retrospectively coded, before official recidivism records were obtained. Multivariate analysis of variance (MANOVA), χ2, and Cox regressions were conducted. Violent-plus youth were younger; higher in their total risk and all criminogenic needs; more likely to have several noncriminogenic needs; and at higher risk of any reoffending, violent reoffending, and nonviolent reoffending than nonviolent youth. Violent-only youth had the same total risk and risk of general and violent recidivism as nonviolent offenders but presented different criminogenic and noncriminogenic needs and risk of nonviolent recidivism. Compared to violent-only youth, violent-plus youth were younger, had higher total risk and criminogenic needs on five domains, were more likely to have several noncriminogenic needs, and were at higher risk of all types of reoffending (except sexual reoffending), suggesting subtypes of violent youth offenders. The implication is that nonviolent and violent youth offenders require different dosage and types of intervention.

15. Long-Term Results of Minimally Invasive Transforaminal Lumbar Interbody Fusion in Elderly Patients: A 5-Year Follow-Up Study

Abstract

Study Design Retrospective review of prospective data.Objectives Minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) may be beneficial for elderly patients due to decreased surgical morbidity and faster postoperative recovery. This study compared the clinical and radiological outcomes of MIS-TLIF in elderly patients and younger controls at minimum 5-year follow-up.Methods There were 120 patients who underwent single-level MIS-TLIF for degenerative spondylolisthesis. Elderly patients (≥70 years; n = 30) and controls (<70 years; n = 90) were matched 1:3 for demographics, comorbidities and preoperative patient-reported outcomes (PROs). The Oswestry Disability Index (ODI), 36-Item Short-Form Physical (SF-36 PCS) and Mental Component Summary (SF-36 MCS), Visual Analogue Scale (VAS) back pain, and VAS leg pain were compared at 6 months, 2 years and 5 years. Radiographic fusion, adjacent segment degeneration (ASD) and revision rates were assessed at mean 7.2 ± 2.0 years.Results Elderly patients had longer length of stay (4.7 ± 5.8 vs 3.3 ± 1.4 days, P= .035) and more readmissions (10% vs 1%, P= .019), but there was no difference in operative time, transfusions, complications or discharge disposition. All PROs were comparable at 5 years and satisfaction rates were similar (93% elderly vs 91% controls, P= .703). The rates of radiographic fusion in the control group and elderly group were similar (94% vs 97%, P= .605), as were the rates of ASD (40% vs 33%, P= .503). There were 3 revisions (3.3%) in the control group (2 for ASD, 1 for screw loosening) but none in the elderly group (P= .311).Conclusions Elderly patients undergoing MIS-TLIF achieved similar improvements in pain, disability and quality of life that were sustained at 5 years. 16. An examination of the relationship between Alexithymia and health anxiety

Abstract

A theoretical basis exists on which alexithymia may be associated with health anxiety (G. J. Taylor, Bagby, & Parker, 1997). However, despite this link, there has been little research into a theorized relationship between alexithymia and health anxiety, and even among such studies, there is no consensus on whether there is a relationship between alexithymia and health anxiety. The present series of studies firstly explored whether alexithymia and health anxiety are related constructs, and secondly examined in-depth the nature of any such relationship. Studies 1 and 2 explored whether alexithymia and health anxiety are related. Study 1 employed the 20-item Toronto Alexithymia Scale (TAS-20) and the Short Health Anxiety Inventory (SHAI) as measures of alexithymia and health anxiety respectively, while Study 2 utilized the Bermond-Vorst Alexithymia Questionnaire (BVAQ) to ascertain if the results from Study 1 could be replicated with a different measure of alexithymia. After controlling for depression and anxiety, both studies found alexithymia to be related to health anxiety, albeit only for certain domains of alexithymia – greater difficulty identifying feelings was associated with greater levels of health anxiety; however, engaging in externally oriented thinking, reduced analysis of feelings, and reduced levels of emotionalizing were all inversely related to health anxiety. Furthermore, these relationships varied by the severity of alexithymia and gender, with stronger associations observed with increasing alexithymic severity. Study 3 then examined if the relationship between alexithymia and health anxiety was mediated by an external variable – anxiety sensitivity. Study 3a again utilized the TAS-20 while Study 3b employed the BVAQ. Both dimensions dealing with concerns about the physical and cognitive consequences of emotional arousal were found to mediate the relationship between having difficulty identifying feelings and health anxiety.
