Abstract

The DIII-D tokamak has elucidated crucial physics and developed projectable solutions for ITER and fusion power plants in the key areas of core performance, boundary heat and particle transport, and integrated scenario operation, with closing the core-edge integration knowledge gap being the overarching mission. New experimental validation of high-fidelity, multi-channel, non-linear gyrokinetic turbulent transport models for ITER provides strong confidence it will achieve Q ⩾ 10 operation. Experiments identify options for easing H-mode access in hydrogen, and give new insight into the isotopic dependence of transport and confinement. Analysis of 2,1 islands in unoptimized low-torque IBS demonstration discharges suggests their onset time occurs randomly in the constant β phase, most often triggered by non-linear 3-wave coupling, thus identifying an NTM seeding mechanism to avoid. Pure deuterium SPI for disruption mitigation is shown to provide favorable slow cooling, but poor core assimilation, suggesting paths for improved SPI on ITER. At the boundary, measured neutral density and ionization source fluxes are strongly poloidally asymmetric, implying a 2D treatment is needed to model pedestal fuelling. Detailed measurements of pedestal and SOL quantities and impurity charge state radiation in detached divertors has validated edge fluid modelling and new self-consistent ‘pedestal-to-divertor’ integrated modeling that can be used to optimize reactors. New feedback adaptive ELM control minimizes confinement reduction, and RMP ELM suppression with sustained high core performance was obtained for the first time with the outer strike point in a W-coated, compact and unpumped small-angle slot divertor. Advances have been made in integrated operational scenarios for ITER and power plants. Wide pedestal intrinsically ELM-free QH-modes are produced with more reactor-relevant conditions, Low torque IBS with W-equivalent radiators can exhibit predator-prey oscillations in T _e and radiation which need control. High- β _P scenarios with q _min > 2, q _95 –7.9, β _N > 4, β _T –3.3% and H _98y2 > 1.5 are sustained with high density ( $\bar n$ = 7E19 m ^−3 , f _G –1) for 6 τ _E , improving confidence in steady-state tokamak reactors. Diverted NT plasmas achieve high core performance with a non-ELMing edge, offering a possible highly attractive core-edge integration solution for reactors.

Abstract

The distributions of random variables are of interest in many areas of science. In this paper, ascertaining on the importance of multi-hop transmission in contemporary wireless communications systems operating over fading channels in the presence of cochannel interference, the probability density functions (PDFs) of minimum of arbitrary number of ratios of Rayleigh, Rician, Nakagami-m, Weibull and α-µ random variables are derived. These expressions can be used to study the outage probability as an important multi-hop system performance measure. Various numerical results complement the proposed mathematical analysis.

Abstract

Aim of the study was to determine whether the TNF-a levels, proximal inflammatory mediator, in septic patients presenting to the emergency department (ED) and admitted to the intensive care unit (ICU) are associated with progression to severe sepsis, septic shock or death. Material and methods. A retrospective observational study was performed on a sample of one hundred adult subjects presenting to the ED with systemic inflammatory response syndrome of 2 etiologies: presumed (and later confirmed in the ICU and/or operating room) severe acute pancreatitis or generalized peritonitis. Blood TNF-a samples measurements were taken shortly after ED admission. TNF-a was measured by commercial ELISA test in plasma. Results. 4. Offsetting Voltage-Dependent Kv1.5 Channel Opening Through Charged Residue Substitutions on Top of the First Transmembrane Segment

Abstract

Background: Voltage-dependent K+ (Kv) channels exist as tetramers of subunits consisting of six transmembrane segments (S1-S6). The S1 through S4 segments assemble into a voltage-sensing domain (VSD) that detects the membrane electric field. Therefore, the S4 is rich in positively charged amino acid residues forming the main component of the VSD. Upon membrane de- or repolarization, these positive charges, termed gating charges, cross the electric field, causing the VSD to change conformation. The S1-S3 segments surround the S4 and shape the electric field sensed by these gating charges.Materials and Methods: In the Shaker-type hKv1.5 channel residues C268 to R279 at the extracellular end of S1 and beginning of the S1-S2 linker were substituted by either a positive (lysine), a negative (glutamic acid), or a neutral (glutamine) residue. The charge substitution or charge introduction mutants were functionally analyzed by expressing the channels in Ltk- cells and performing patch-clamp current recordings.Results: At positions C268, L269, T271, and P273, a charge introduction was not tolerated, yielding no current expression. On the contrary, mutants D277K and R279E yielded channels with gating properties similar to wild type (WT) hKv1.5. The mutants E270Q, L272E, L272K, and F275K shifted the voltage dependence of channel activation toward more positive potentials compared to hKv1.5. However, this shift was accompanied by a shallower slope factor and altered activation and/or deactivation kinetics. Interestingly, at positions E274, R276, and E278, a charge substitution shifted the voltage dependence of channel opening without affecting channel kinetics, that is, exerting a surface charge effect.Conclusion: The cluster of charged residues at the top of S1 and the beginning of the S1-S2 linker are sufficiently close to the S4 and offset (polarize) the electric field sensed by the S4, contributing to the voltage dependence of channel activation.

5. Quantum size effects in ultra-thin YBa2Cu3O7 − x films

Abstract

Abstract The d-wave symmetry of the order parameter with zero energy gap in nodal directions stands in the way of using high-temperature superconductors for quantum applications. We investigate the symmetry of the order parameter in ultra-thin YBa2Cu3O7 − x (YBCO) films by measuring the electrical transport properties of nanowires aligned at different angles relative to the main crystallographic axes. The anisotropy of the nanowire critical current in the nodal and antinodal directions reduces with the decrease in the film thickness. The Andreev reflection spectroscopy of the nanoconstrictions shows the presence of a thickness-dependent energy gap that does not exist in bulk YBCO. We find that the thickness-dependent energy gap appears due to the quantum size effects in ultra-thin YBCO films that open the energy gap along the entire Fermi surface. The fully gapped state of the ultra-thin YBCO films makes them a promising platform for quantum applications, including quantum computing and quantum communications.

Abstract

BACKGROUND: The prevalence of atrial fibrillation(AF) and diabetes mellitus is rising to epidemic proportions. We aimed to assess the impact of diabetes on the management and outcomes of patients with AF.METHODS: The EORP-AF General Long-Term Registry is a prospective, observational registry from 250 centres across 27 European countries. Outcomes of interest were as follows: i)rhythm control interventions; ii)quality of life; iii)healthcare resource utilisation; and iv)major adverse events.RESULTS: Of 11,028 patients with AF, the median age was 71 (63-77) years and 2537 (23.0%) had diabetes. Median follow-up was 24 months. Diabetes was related to increased use of anticoagulation but less rhythm control interventions. Using multivariable analysis, at 2-year follow-up, patients with diabetes were associated with greater levels of anxiety (p = 0.038) compared to those without diabetes. Overall, diabetes was associated with worse health during follow-up, as indicated by Health Utility Score and Visual Analogue Scale. Healthcare resource utilisation was greater with diabetes in terms of length of hospital stay (8.1 (±8.2) vs. 6.1 (±6.7) days); cardiology and internal medicine/general practitioner visits; and emergency room admissions. Diabetes was an independent risk factor of major adverse cardiovascular event (MACE; HR 1.26 [95% CI, 1.04-1.52]), all-cause mortality (HR 1.28 [95% CI, 1.08-1.52]), and cardiovascular mortality (HR 1.41 [95% CI, 1.09-1.83]).CONCLUSION: In this contemporary AF cohort, diabetes was present in 1 in 4 patients and it served as an independent risk factor for reduced quality of life, greater healthcare resource utilisation and excess MACE, all-cause mortality and cardiovascular mortality. There was increased use of anticoagulation therapy in diabetes but with less rhythm control interventions.

Abstract

In automatic speech recognition systems, the training data used for system development and the data actually obtained from the users of the system sometimes significantly differ in practice. However, other, more similar data may be available. Transfer learning can help to exploit such similar data for training in order to boost the automatic speech recognizer's performance for a certain domain. This paper presents a few applications of transfer learning in the context of speech recognition, specifically for the Serbian language. Several methods are proposed, with the goal of optimizing system performance on a specific part of the existing speech database for Serbian, or in a noisy environment. The experimental results evaluated on a test set from the desired domain show significant improvement in both word error rate and character error rate.

Publisher Copyright: © 2022 IAEA, Vienna. DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stability limits and regulate β N in the ITER baseline scenario, based on plasma response to probing 3D fields, was demonstrated. Finally, innovative tokamak operation using a negative triangularity shape showed many attractive features for future pilot plant operation.

Abstract

BACKGROUND: Cardiac troponins (cTn) have been reported to be predictors for adverse outcomes in atrial fibrillation (AF), patients, but their actual use is still unclear.AIM: To assess the factors associated with cTn testing in routine practice and evaluate the association with outcomes.METHODS: Patients enrolled in the ESC-EHRA EORP-AF General Long-Term Registry were stratified into 3 groups according to cTn levels as (i) cTn not tested, (ii) cTn in range (≤99th percentile), (iii) cTn elevated (>99th percentile). The composite outcome of any thromboembolism /any acute coronary syndrome/cardiovascular (CV) death, defined as Major Adverse Cardiovascular Events (MACE) and all-cause death were the main endpoints.RESULTS: Among 10 445 AF patients (median age 71 years, 40.3% females) cTn were tested in 2834 (27.1%). cTn was elevated in 904/2834 (31.9%) and in-range in 1930/2834 (68.1%) patients. Female sex, in-hospital enrollment, first-detected AF, CV risk factors, history of coronary artery disease, and atypical AF symptoms were independently associated with cTn testing. Elevated cTn were independently associated with a higher risk for MACE (Model 1, hazard ratio [HR] 1.74, 95% confidence interval [CI] 1.40-2.16, Model 2, HR 1.62, 95% CI 1.28-2.05; Model 3 HR 1.76, 95% CI 1.37-2.26) and all-cause death (Model 1, HR 1.45, 95% CI 1.21-1.74; Model 2, HR 1.36, 95% CI 1.12-1.66; Model 3, HR 1.38, 95% CI 1.12-1.71).CONCLUSIONS: Elevated cTn levels were associated with an increased risk of all-cause mortality and adverse CV events. Clinical factors that might enhance the need to rule out CAD were associated with cTn testing.

10. Late Clinical and Hemodynamic Outcomes in patients with degenerated bioprosthetic aortic valves undergoing transcatheter valve-in-valve implantation

Abstract

Background Clinical studies have shown promising early outcomes for valve-in-valve transcatheter aortic valve implantation (ViV-TAVI); however, the late outcomes of this procedure remain under-investigated. Purpose We performed the present analysis to assess the late clinical and hemodynamic outcomes of ViV-TAVI in patients with degenerated bioprosthetic aortic valves. Methods A comprehensive chart review was performed for eligible patients to retrieve data on procedural characteristics, admission details following the procedure, and echocardiographic parameters. Clinical outcomes included all-cause mortality, heart failure hospitalization and structural valve deterioration (SVD), as defined by VARC-II criteria, up to 5 years of follow-up. To assess the trends in mean and peak transvalvular gradients, data from the follow-up echocardiographic reports were analyzed using Syngo Dynamics imaging software. Results A total of 188 patients were included with a mean age of 75.8±10.4 years. Balloon- and self-expandable valves were used in 155 (82.4%) and 33 (17.6%) patients, respectively. At 30 days, 3 (1.6%) patients died and 8 (4.2%) required hospitalization for heart failure, while at 5 years, both events were recorded in 29 (15.4%) and 37 (19.7) patients, respectively. Kaplan-Meier survival analysis showed that patients with smaller surgical valves (internal diameter ≤21 mm) had a significantly higher mortality rate (log-rank p=0.021) than those with larger valves; however, no significant difference (log-rank p=0.59) was detected between different transcatheter valves (self vs. balloon-expandable). Three patients underwent re-intervention, performed via a transcatheter approach. Further, assessment of follow-up echocardiographic reports revealed 9 (4.8%) cases of SVD, as well as stable mean (16.3±6.9 at discharge and 16.9±11.3) and peak (30.3±12.1 at discharge and 30.7±18.4 at 5 years) transvalvular gradients. No difference (p>0.05) was observed based on transcatheter valve type or surgical valve internal diameter in terms of mean and peak transvalvular gradients throughout the follow-up period. Conclusion The present study showed good clinical outcomes among patients undergoing VIV-TAVI, with stable VIV performance over a five-year period. Future long-term studies are warranted to analyze the predictors of outcomes following ViV-TAVI and explore the role of this treatment option in the life-long management of aortic stenosis. Abstract

Relativistic electron (RE) beams at high current density (low safety factor, q ( a )) yet very low free-electron density accessed with D-2 secondary injection in the DIII-D and JET tokamak are found to exhibit large-scale MHD instabilities that benignly terminate the RE beam. In JET, this technique has enabled termination of MA-level RE currents without measurable first-wall heating. This scenario thus offers an unexpected alternate pathway to achieve RE mitigation without collisional dissipation. Benign termination is explained by two synergistic effects. First, during the MHD-driven RE loss events both experiment and MHD orbit-loss modeling supports a significant increase in the wetted area of the RE loss. Second, as previously identified at JET and DIII-D, the fast kink loss timescale precludes RE beam regeneration and the resulting dangerous conversion of magnetic to RE kinetic energy. During the termination, the RE kinetic energy is lost to the wall, but the current fully transfers to the cold bulk thus enabling benign Ohmic dissipation of the magnetic energy on longer timescales via a conventional current quench. Hydrogenic (D-2) secondary injection is found to be the only injected species that enables access to the benign termination. D-2 injection: (1) facilitates access to low q ( a ) in existing devices (via reduced collisionality & resistivity), (2) minimizes the RE avalanche by 'purging' the high-Z atoms from the RE beam, (3) drives recombination of the background plasma, reducing the density and Alfven time, thus accelerating the MHD growth. This phenomenon is found to be accessible when crossing the low q ( a ) stability boundary with rising current, falling toroidal field, or contracting minor radius-the latter being the expected scenario for vertically unstable RE beams in ITER. While unexpected, this path scales favorably to fusion-grade tokamaks and offers a novel RE mitigation scenario in principle accessible with the day-one disruption mitigation system of ITER.

Abstract

Within the EUROfusion MST1 work package, a series of experiments has been conducted on AUG and TCV devices to disentangle the role of plasma fueling and plasma shape for the onset of small ELM regimes. On both devices, small ELM regimes with high confinement are achieved if and only if two conditions are fulfilled at the same time. Firstly, the plasma density at the separatrix must be large enough (), leading to a pressure profile flattening at the separatrix, which stabilizes type-I ELMs. Secondly, the magnetic configuration has to be close to a double null (DN), leading to a reduction of the magnetic shear in the extreme vicinity of the separatrix. Abstract

An experiment was performed in the DIII-D lower divertor to validate numerical SOL tungsten (W) impurity erosion and redeposition simulations against experimental data. The net and gross erosion of W were calculated as a function of the voltage (or bias) applied to the exposed material. Five samples were inserted into the DIII-D lower divertor using the Divertor Material Evaluation System (DiMES) manipulator and exposed to constant L-mode attached plasma conditions. Each sample was partially coated with W. During plasma shots, samples were biased with respect to the machine vessel ground, ranging from −60 V to 25 V. The ERO2.0 code was used to numerically simulate the experiment aiming to compare the numerical results with experimental measures. A good agreement is found between estimated and measured tungsten erosion at least for negative biases.

Abstract

In this paper control-oriented dynamical models of current, horizontal and vertical displacement for Runaway Electrons (RE) beam in the plateau phase are presented. The dynamical models were obtained through gray-box identification techniques on experimental data collected on post-disruptive RE beam shots in the Frascati Tokamak Upgrade (FTU). Based on the identified models, the design of simple robust controllers to improve RE beam mitigation strategies is proposed and simulations results are provided. © 2018 Elsevier B.V.

Abstract

ABSTRACT The research was conducted during the 2018/19 hunting year at three hunting grounds: "Barajevska reka" - "Takovo" and "Jadar". The average density of Roe deer in hunting grounds varied from 48.67 to 74 individuals per 1,000 ha of hunting-productive area. However, when observing the number of individuals per 1,000 ha of total hunting area, the density varies from 16.07 individuals to 34.72 individuals. The determined gender ratio at the hunting ground "Jadar" was (M: F = 1: 0.93). At the hunting ground "Takovo", the gender ratio was (M: F = 1: 1.22), while at the hunting ground "Barajevska reka" the ratio was (M: F = 1: 1.59). The average fertility of Roe deer was 1.67 embryos per individual, or 1.75 embryos per pregnant female. Depending on the study area, the real growth varied from 0.54 to 0.73 fawns per female. Loss in Roe deer are caused mostly by biotic factors. The determined average age of shot individuals varied depending on the hunting ground from 3.53 to 5.24 years. The current state of Roe deer populations in Serbia varies between analyzed hunting grounds, but the overall situation is quite unsatisfactory, especially in terms of density, gender ratio and age structure of culled individuals.

Abstract

Special issue featuring the invited talks from the 43rd EPS Conference on Plasma Physics, Leuven, 4-8 July, 2016 We present an overview of FTU experiments on runaway electron (RE) generation and control carried out through a comprehensive set of real-time (RT) diagnostics/control systems and newly installed RE diagnostics. An RE imaging spectrometer system detects visible and infrared synchrotron radiation. A Cherenkov probe measures RE escaping the plasma. A gamma camera provides hard x-ray radial profiles from RE bremsstrahlung interactions in the plasma. Experiments on the onset and suppression of RE show that the threshold electric field for RE generation is larger than that expected according to a purely collisional theory, but consistent with an increase due to synchrotron radiation losses. This might imply a lower density to be targeted with massive gas injection for RE suppression in ITER. Experiments on active control of disruption-generated RE have been performed through feedback on poloidal coils by implementing an RT boundary-reconstruction algorithm evaluated on magnetic moments. Abstract

Experiments have been carried out to evaluate the threshold electric field for runaway generation during the flat-top phase of ohmic discharges in the Frascati Tokamak Upgrade tokamak. An investigation of the conditions for runaway electron generation and suppression has been performed for a wide range of plasma parameter values. The measured threshold electric field is found to be significantly larger (similar to 2 - 5 times) than predicted by the relativistic collissional theory of runaway generation, E-R = n(e) e(3) ln Lambda/4 pi e(0)(2) m(e) c(2), and can be explained to a great extent by an increase of the critical electric field due to the effect of the electron synchrotron radiation losses. These findings are consistent with the results of an ITPA joint experiment to study the onset, growth, and decay of relativistic runaway electrons [Granetz et al., Phys. Plasmas 21, 072506 (2014)]. Confirmation of these results for disruptions with high electric field might imply significantly lower requirements on electron densities for suppression and prevention of runaway formation in ITER. Abstract

The aim of this study was to assess whether the Crno jezero (Black Lake) could be designated as a site possessing specific reference conditions of a glacial lake in the Dinaric Western Balkan ecoregion. The results of a Lake Habitat Survey (LHS), analysis of macrophytes and a basic water quality assessment indicate that the lake is in a near pristine state, particularly with regard to its hydromorphological status, and that it fulfills the requirements of High Ecological Status (HES), as set by the Water Framework Directive. However, to confirm these preliminary findings, an integrated assessment of the ecological and chemical status, using other biological quality elements and a full set of physico-chemical parameters, is necessary.

Abstract

In this study, we present evidence that red blood cell (RBC) membrane p68 in the Belgrade (b/b) rat is similar if not identical to rat serum albumin (RSA). Structural homology between RSA and the RBC p68 has been determined by a variety of biochemical and immunological criteria. This albumin-like protein is a normal constituent of rat RBC and it is partially exported by exosomes during erythroid differentiation. Abstract

Purpose To create a model of the topography of the Henle Fibers (HF) connecting photoreceptor cell bodies of the human fovea to the inner retina based on information available in recent imaging and histological studies. Detailed knowledge of the HF is of importance for analysis of connectivity between the outer and inner retina and for evaluation of radial displacement of retinal neurons from their synaptically connected photoreceptors. Methods Reported data of the Henle Fiber Layer (HFL) thickness (HFLt) at different eccentricities along the horizontal meridian of the human fovea from studies with optical coherence tomography (OCT) and histology was used. The radial displacement caused by HF was calculated from data of total displacement within the retina, corrected by data of other displacements obtained from Sjostrand et al( unpublished). Results A comparison of OCT and histology data showed a similar profile of HFLt vs eccentricity and similar HF topography. The angle of the Henle fibers vs. the external limiting membrane (ELM) was low centrally ranging from a few degrees (deg) to approximately 6 deg at a cone eccentricity of 0.9 mm.Thereafter the angle slowly increased with increasing eccentricity to an angle of 9-10 deg at cone eccentricity of 1.8 mm . The locations of the maxima of HFLt and projected HF length were at a cone eccentricity of approximately 0.5 mm. A model was constructed displaying the characteristics of the topography of HFL. Conclusion Published data of the topography of HFL fits with a model where the the maximal HFLt is attained at a cone eccentricity where the fiber angle vs ELM still is of low degree and the HF length reaches a maximum.

Purpose To present a model useful for analysis of images of central retina based on functional connectivity between outer retina with its photoreceptors and retinal ganglion cells (RGC) of the inner retina. In addition, to evaluate the nomenclature and to propose clinically relevant definitions of the fovea and surrounding zones based on neural connectivity, function and development. Methods Published measurements of radial displacement of inner retina relative to outer retina were used to determine the area of RGC corresponding to central photoreceptors. A model of central retina with displaced outer and inner retinal zones was constructed based on data from histological reports and data from recent studies with non-invasive imaging studies, the convention of the ETDRS study, and psychophysical studies. Results A model is presented where functional relationships within the retina are based on neural connectivity, i.e the radial displacement of RGC and synaptically connected cones. The statement by S. Polyak (1941) that "the entire inner foveal excavation corresponds to a small central region" of photoreceptors is confirmed. The model indicates that the template for analysis should be separate for outer and inner retina in order to mirror corresponding areas and functional relationships. Based on the model we propose a template relevant for clinical analysis of images of the outer retina mainly based on anatomical nomenclature and partly adapted according to the ETDRS convention. Conclusion A model of the central retina based on neural connectivity is recommended as a frame for analysis of corresponding but displaced zones of the central retina.Templates for analysis of zones of cones and RGC are proposed.

Abstract

In this study we analyse the quantity and the quality of the use of computers in mathematics teaching in schools (both primary and secondary) in Serbia, observing two samples. The data for the first research was obtained through a sample consisting of 142 experienced teachers. By conducting a survey we determined the current situation regarding personal and school use of computers, and also examined whether or not computer usage was affected by some non-attitudinal (e.g., demographic) and attitudinal variables. The second research included 123 beginner teachers, applicants for the professional state teacher license. The collected data was based on our estimates of the applicants’ preparation for the practical part of the licensing examination. We examined the impact of some demographic and academic factors on the quality of computer usage. In both researches, the data collected was coded and analysed using the SPSS 13.0 statistical program (cross tabulation and appropriate tests for comparing groups). The aims of our researches were to determine the current situation and give some possible suggestions for improving computer use, U ovom radu analiziramo kvantitetu i kvalitetu uporabe računala u poučavanju matematike u Srbiji (u osnovnim i srednjim školama) promatrajući dva uzorka. Provedbom istraživanja ustanovili smo trenutnu situaciju s obzirom na privatnu uporabu računala i uporabu računala u školi, kao i utječu li na uporabu računala i neke objektivne (npr. demografija) i subjektivne varijable. Drugo istraživanje je provedeno na uzorku od 123 nastavnička pripravnika (bez profesionalne licence). Prikupljeni podaci su temeljeni na našim procjenama o pripravi praktičnog dijela stručnog ispita pripravnika. Istražili smo utjecaj nekih demografskih i akademskih čimbenika na kakvoću uporabe računala. U oba istraživanja, podaci su obrađeni uporabom statističkog programa SPSS 13.0 (unakrsno tabeliranje i odgovarajući testovi za uspoređivanje skupina). Cilj istraživanja je bio utvrditi trenutnu situaciju i predložiti moguća poboljšanja uporabe računala.

Purpose A knowledge of connectivity from the central photoreceptors and their axons, via the bipolar cells to the retinal ganglion cells (RGC), is required to understand the consequences of lesions in different locations of the human fovea. The purpose was to create a model of the connectivity within the fovea based on information available in recent non-invasive studies of the normal fovea with optical coherence tomography (OCT) and adaptive optics (AO). Methods OCT data from the study by Lujan et al. (2011) of the thickness of the Henle fiber layer (HFL) at various eccentricities was used in combination with data of cone density and visual resolution obtained from AO studies. An angle of 6 degrees was assumed between the cone axons and the external limiting membrane, at eccentricities between 0.2 and 1 mm, to calculate the lateral displacement from the cone photoreceptor cell body to the cone pedicle. Cone density and visual resolution was obtained from published AO studies (Wolfing et al., 2006; Rossi & Roorda, 2010). Results The thickness (HFLt) versus eccentricity along the horizontal meridian was derived from Lujan et al. (2011) and used to calculate the lateral displacement of cone pedicles. Zones in the cone pedicle layer and RGC layer connected to corresponding regions of the cone mosaic (0-0.2 mm, 0.2-0.4 mm, 0.4-0.6 mm) were calculated and related to AO estimates of visual resolution. Conclusion Our model may be useful to visualize functional connectivity and to predict the effect of lesions within the fovea. A maximal effect of lesions in the bipolar and RGC layers, due to the displacement caused by the cone axons, may be anticipated at an eccentricity of about 0.5 mm or 2 degrees.

Abstract

Tremor constitutes the most common movement disorder; in fact 14.5% of population between 50 to 89 years old suffers from it. Moreover, 65% of patients with upper limb tremor report disability when performing their activities of daily living (ADL). Unfortunately, 25% of patients do not respond to drugs or neurosurgery. In this regard, TREMOR project proposes functional compensation of upper limb tremors with a soft wearable robot that applies biomechanical loads through functional electrical stimulation (FES) of muscles. This wearable robot is driven by a Brain Neural Computer Interface (BNCI). This paper presents a multimodal BCI to assess generation, transmission and execution of both volitional and tremorous movements based on electroencephalography (EEG), electromyography (EMG) and inertial sensors (IMUs). These signals are combined to obtain: 1) the intention to perform a voluntary movement from cortical activity (EEG), 2) tremor onset, and an estimation of tremor frequency from muscle activation (EMG), and 3) instantaneous tremor amplitude and frequency from kinematic measurements (IMUs). Integration of this information will provide control signals to drive the FES-based wearable robot.

Abstract

Since the 2012 IAEA-FEC Conference, FTU operations have been largely devoted to runaway electrons generation and control, to the exploitation of the 140GHz electron cyclotron (EC) system and to liquid metal limiter elements. Experiments on runaway electrons have shown that the measured threshold electric field for their generation is larger than predicted by collisional theory and can be justified considering synchrotron radiation losses. A new runaway electrons control algorithm was developed and tested in presence of a runaway current plateau, allowing to minimize the interactions with plasma facing components and safely shut down the discharges. The experimental sessions with 140GHz EC system have been mainly devoted to experiments on real-time control of magnetohydrodynamic (MHD) instabilities using the new EC launcher with fast steering capability. Experiments with central EC injection have shown the onset of 3/2 and 2/1 tearing modes, while EC assisted breakdown experiments have been focused on ITER start-up issues, exploring the polarization conversion at reflection from inner wall and the capability to assure plasma start-up even in presence of a large stray magnetic field. A new actively cooled lithium limiter has been installed and tested. The lithium limiter was inserted in the scrape-off layer, without any damage to the limiter surface. First elongated FTU plasmas with EC additional heating were obtained with the new cooled limiter. Density peaking and controlled MHD activity driven by neon injection were investigated at different plasma parameters. A full real-time algorithm for disruption prediction, based on MHD activity signals from Mirnov coils, was developed exploiting a large database of disruptions. Reciprocating Langmuir probes were used to measure the heat flux e-folding length in the scrape-off layer, with the plasma kept to lay on thea internal limiter to resemble the ITER start-up phase. New diagnostics were successfully installed and tested, as a diamond probe to detect Cherenkov radiation produced by fast electrons and a gamma camera for runaway electrons studies. Laser induced breakdown spectroscopy measurements were performed under vacuum and with toroidal magnetic field, so demonstrating their capability to provide useful information on the surface elemental composition and fuel retention in present and future tokamaks, such as ITER. © 2015 EURATOM.

Aim of the study was to determine whether the TNF-a levels, proximal inflammatory mediator, in septic patients presenting to the emergency department (ED) and admitted to the intensive care unit (ICU) are associated with progression to severe sepsis, septic shock or death. Material and methods. A retrospective observational study was performed on a sample of one hundred adult subjects presenting to the ED with systemic inflammatory response syndrome of 2 etiologies: presumed (and later confirmed in the ICU and/or operating room) severe acute pancreatitis or generalized peritonitis. Abstract

Percutaneous transluminal septal myocardial ablation (PTSMA) is becoming more and more significant in the therapy of symptomatic patients with hypertrophic obstructive cardiomyopathy (HOCM). We report on the first successful PTSMA done in Yugoslavia in a 65-year old female patient with HOCM and in NYHA functional class III. The procedure was performed with infection of a relatively small amount of absolute alcohol (3 ml) in the first septal branch of LAD, and short duration of balloon inflation (3 minutes), with reduction of the left ventricular outflow tract gradient at rest from 88 mm Hg to 11 mmHg. The patient's in-hospital course was uneventful and has improved to NYHA functional class I.

Abstract

To estimate the occurrence of congenital cataract in a Nordic country.In 1980, we constructed a database in the Department of Ophthalmology, Göteborg University, containing basic data from all cases in western Sweden diagnosed with congenital cataract. By collecting and processing these data from 1980 onwards, we hoped to improve the management of congenital cataract treatment and to optimize the outcome of the treatment.In this study, the incidence of congenital cataract in the four western counties of Sweden was evaluated. The occurrence rate of all cases with congenital cataract during the study period was 36 cases per 100,000. The occurrence rate for dense bilateral and all unilateral cases were both 14 per 10,0000 each.There were no time-related changes in incidence of congenital cataract from 1980 until today, although there was a large variation in the yearly incidence.

Abstract

In our department we prefer laparoscopic treatment ectopic pregnancy. In 1999 and 2000 years we were laparoscopically treated 70% ectopic pregnancy. For promtly and safely diagnosis we use clinical, ultrasound and beta choriongonadotropine hormon examination. In this period we treated 26 women with diagnosis of ectopic pregnancy. In 96% patients ultrasound examination was made. Ultrasound examination showed in 88% women adnexal masses or free fluid in Douglas space. In most cases ectopic pregnancy was in right tube (69%). Ultrasound evaluation in correlation with beta choriongonadotropin hormone creates early and safely diagnosis ectopic pregnancy and provides laparoscopic approach. Laparoscopic approach in treatment ectopic pregnancy is method of choice.

Abstract

The identification and characterization of pathological tremor are necessary for the development of techniques for tremor suppression, for example, based on functional electrical stimulation. For this purpose, the amplitude and phase characteristics of the tremor signal should be estimated by effective detection techniques, either from the kinematics or from muscle recordings. This paper presents an approach for the estimation of the characteristics of pathological tremor from the surface electromyogram (EMG) signal based on the iterated Hilbert transform (IHT). It is shown that the IHT allows an asymptotically exact modeling of the tremor and the voluntary activity components in the surface EMG, and an effective demodulation of the pathological tremor parameters. The method was tested on signals generated by a recent model for tremor generation as well as experimentally recorded from patients affected by pathological tremor. The results showed the ability of the proposed approach to demodulate effectively the tremor amplitude (average correlation with imposed amplitude: R^2=0.52), the frequency (root mean square error in frequency estimation: 2.6 Hz), and phase, as well as the degree of voluntary activity (correlation with simulated inertial load: R^2=0.62). The application of the method to the experimental data indicated that the estimated tremor component closely resembles inertial measurements of limb movement (peak cross correlation across four patients: 0.62\pm 0.15). Compared to the performance of empirical mode decomposition, the proposed method proved to be more accurate for tremor characterization without a priori knowledge of the tremor characteristics. Abstract

A series of plasmids was constructed to study the effect of two enhancers, the simian virus 40 72-base-pair repeat and the Harvey sarcoma virus 73-base-pair repeat, on the mouse beta maj-globin promoter. These plasmids contain the mouse beta maj-globin promoter linked to the Escherichia coli galK gene, thus allowing galactokinase enzyme activity to be used as a measure of promoter function. In CV-1 (primate) cells, it was found that an enhancer is required for optimal promoter activity and that the simian virus 40 (primate) enhancer increases galactokinase fourfold more than the Harvey sarcoma virus (mouse) enhancer. In L (mouse) cells, however, the Harvey sarcoma virus enhancer is 1.3-fold stronger than the simian virus 40 enhancer. These data support the hypothesis that enhancer activity can be species specific. Furthermore, when both enhancers are present on the same plasmid, their effect is additive on the beta-globin promoter whether the plasmid is in CV-1 cells or L cells.

The service life of rails depends especially on the operational loads and speeds on the railway lines, as well as on the rail maintenance policy. There are multiple aspects of rail maintenance optimization. The following article considers optimisation of rail maintenance activities relating to rail head surface condition in order to reduce corrective work on track geometry, as well as to minimize the overall costs for track maintenance. The objects of this research are issues and challenges related to rail and track maintenance in curves on modern conventional railway lines for mixed traffic.

Abstract

Rail defects due to rolling contact fatigue (RCF) threaten the traffic safety around the world. That hazard is more distinct on railways without adequate maintenance strategy. Realization of interoperability of European railway network demands from every infrastructure manager to have a maintenance plan for the infrastructure subsystem. Besides that, this plan includes rail inspection and strategy against RCF defects. This paper emphasizes the importance of rail inspection and early detection of RCF because the most of RCF crack should be removed in rail grinding campaigns (preventive, cyclical and corrective activities) during the whole rail service life.

Abstract

The interaction of enhancers with different promoters was studied by measuring the influence of two enhancers (from simian virus 40 and from Harvey sarcoma virus) on the activity of expression vectors that are identical except for their promoter region. The promoters examined were from the simian virus 40 early region, with or without its own 72-base-pair repeat, and the mouse beta major-globin gene. It is clear that the promoter acted upon strongly influences the level of activity of an enhancer.

Abstract

A portable Runaway Electron Imaging and Spectrometry System (REIS) was developed in ENEA-Frascati to measure synchrotron radiationspectra from in-flight runaway electrons in tokamaks. The REIS is a wide-angle optical system collecting simultaneously visible and infraredemission spectra using an incoherent bundle of fibers, in a spectral range that spans from 500 nm to 2500 nm, and visible images using a CCDcolor microcamera at a rate of 25 frames/s. The REIS system is supervised and managed using a dedicated LabVIEW program to acquire datasimultaneously from three spectrometers every 20 ms (configurable down to 10 ms). An overview of the REIS architecture and acquisitionsystem and resulting experimental data obtained in FTU are presented and discussed in this paper. Abstract

Purpose: It is well established that children born very preterm are at increased risk for ocular abnormalities including altered foveal morphology. However, little is known about how children born moderate-to-late preterm (MLP), gestational age 32 + 0-36 + 6 weeks, are affected later in life. This study investigates foveal avascular zone (FAZ) area, vascular density (VD), central foveal thickness (CFT) and ganglion cell layer thickness (GCL++) of adolescents born MLP without history of retinopathy of prematurity and compare the results with full-term controls., Methods: In a prospective population-based cohort study, 50 adolescents (26 girls; mean age 16.5 years) born MLP were examined with optical coherence tomography (OCT) and OCT angiography (OCT-A). FAZ and VD were obtained from OCT-A scans and adjusted for ocular magnification. CFT and GCL++ were obtained from the OCT scans. The results from the MLP individuals were compared with the results from 49 healthy full-term controls (29 girls; mean age 16.7 years)., Results: The results showed statistically significant differences in FAZ area (0.22 vs 0.28 mm 2 ; p = 0.0032) and CFT (198.1 vs 187.1 μm; p = 0.0010) between the MLP group and controls. Strong correlations between FAZ area and CFT (r = -0.773, p < 0.0001) and GCL++ (r = -0.924, p < 0.0001) were found in the MLP group. There were no differences in VD., Conclusions: Our results show that adolescents born MLP have a smaller FAZ area and an increased CFT compared with full-term controls. These results indicate that adolescents born MLP have similar parafoveal changes as children born extreme or very preterm., (© 2024 The Authors. Abstract

Background/Objectives: Cervical dystonia (CD) is a condition characterized by involuntary activity of cervical muscles, which is often accompanied by various non-motor symptoms. Recent studies indicate impaired saccadic eye movements in CD. Local administration of botulinum toxin type A (BoNT/A), which causes temporary paralysis of the injected muscle, is the first-line treatment of focal dystonia, including CD. To our knowledge, concurrent observation of the effect of BoNT/A on smooth eye movements, voluntary saccades, memory-guided saccades, and antisaccades in CD has not yet been explored. The aim of this study was to assess the effect of BoNT/A on eye movements and non-motor symptoms in patients with CD, which, when altered, could imply a central effect of BoNT/A. Methods: Thirty patients with CD performed smooth pursuit, prosaccadic expression, memory-guided saccades, and antisaccade tasks; eye movements were recorded by an eye tracker. Motor and non-motor symptoms, including depression, anxiety, pain, disability, and cognitive changes prior to and after BoNT/A administration, were also evaluated. Results: The number of correct onward counts ( p < 0.001), overall correct memory-guided saccades count ( p = 0.005), motor symptoms ( p = 0.001), and non-motor symptoms, i.e., anxiety ( p = 0.04), depression ( p = 0.02), and cognition ( p < 0.001) markedly improved after BoNT/A administration. Conclusions: Memory-guided saccades, depression, and anxiety improve after BoNT/A in CD.

50. Case report: Rapid-onset parkinsonism after a hornet sting.

Abstract

Neurological manifestations with basal ganglia involvement following Hymenoptera stings are rare and clinically ill-defined conditions. We present a patient with acute parkinsonism non-responsive to levodopa, who developed striatal lesions after a hornet sting. We report his response to immunomodulatory treatment and subsequent clinical and brain magnetic resonance imaging (MRI) follow-up. We also searched the literature for patients with acute extrapyramidal syndromes following an insect sting. Fourteen cases have been published; 12 of them are reviewed here. The majority of cases presented with symmetric akinetic syndrome with axial rigidity and/or gait impairment. Six patients were treated with levodopa and only two of these had a modest response to therapy. Brain MRI/computed tomography scan revealed lesions of the basal ganglia, which resulted in fatal outcome in four patients, whereas only one achieved complete recovery. Clinicians should be aware of this rare but devastating cause of acute-onset parkinsonism and specific clinical presentation of this condition, and should consider prompt and prolonged immunomodulatory treatment to prevent irreversible basal ganglia damage., Competing Interests: The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest.
