The activity of the 2018–2019 eruption of Kuchinoerabujima Volcano in Japan changed from continuous ejection of ash-laden plumes between October 21 and the middle of December, to intermittent explosive activity accompanied by several pyroclastic density currents until January 2019. To understand the behaviors of magma and hydrothermal fluid that controlled the eruptive sequence, we carried out component analysis, X-ray diffractometry, and leachate analysis for ash samples. The proportion of non-altered volcanic ash particles is ~ 15% in the earlier phase, then it decreased to less than 10% in the later explosive phase. Accordingly, the mineral assemblage of the volcanic ash samples changed from plagioclase-dominant to sulfate minerals-dominant. Concentration of SO42− and Cl/SO4 values of the ash-leachates decreased toward the later activity. These results indicate that the proportion of fresh volcanic rocks decreased and sulfuric acid fluid-derived sulfate minerals increased toward the later activities. Consequently, the 2018–2019 eruption at Kuchinoerabujima Volcano changed from magmatic activity to phreatomagmatic activity. Weak glowing of the crater was observed during the magmatic activity, indicating the volcanic conduit was hot enough to dry up the sub-volcanic hydrothermal system. The following phreatomagmatic activity indicates that the hydrothermal fluid recharged after the magmatic eruption phase. Recharge of the hydrothermal fluid likely caused the variation of the eruption style, and is a process that may control the evolution of hazards during future eruption scenarios at similar active volcanoes in Japan and worldwide. PURPOSE: Intramuscular myxoma (IM) is a rare benign myxoid tumor that may be challenging to differentiate from sarcoma in small amounts of biopsied material. Although IM appears to be well-circumscribed macroscopically, it infiltrates the adjacent edematous muscle microscopically. The recommended treatment is resection, but there is controversy with regard to the appropriate surgical margin. This study aimed to clarify which surgical procedure that should be applied when the preoperative diagnosis is IM and how to manage treatment if the postoperative diagnosis turns out to be a sarcoma. METHODS: We retrospectively examined 55 IM patients treated from January 1982 to December 2014. Patient characteristics, tumor location, tumor size, radiograph, preoperative and postoperative pathological reports, surgical techniques, treatment outcome, and complications were reviewed. The patients were followed up on for at least 5 years. All patients were confirmed not to have Mazabraud syndrome. RESULTS: In the 55 IM patients examined, the mean patient age was 48 years and most were female. The most common tumor locations were in the muscles of the thighs (47%) and buttocks (20%). The mean tumor diameter was 5 cm. Wide resection and marginal resection were performed in 24 and 31 patients, respectively. The mean follow-up duration was 19 years. No local recurrence, malignant transformation, or complications were observed. CONCLUSIONS: Marginal resection is suitable in patients whose preoperative diagnosis is IM, as it is able to prevent local recurrence and allows for the preservation of muscle and muscle fascia. If the postoperative diagnosis turns out to be myxoid sarcoma, minimum surgical contamination makes additional wide resection less invasive.

Background Extraskeletal myxoid chondrosarcoma (EMC) is a rare malignant soft-tissue tumor and often shows extracompartmental tumoral invasion. The aim of our study was to investigate the clinical features, especially extracompartmental tumoral invasion (ETI) of EMC. Patients and methods A total of 35 operative patients diagnosed with EMC were enrolled in this study from January 1980 to March 2018 in the Cancer Institute Hospital of The Japanese Foundation for Cancer Research. The operative procedure was principally wide excision. Univariate analysis assessed how clinicopathological factors (e.g. age, gender, tumor site, tumor size, histopathological grade, surgical margin, metastasis before operation, barrier invasion, local recurrence, metastasis after operation) influenced patient prognosis. We assessed how clinicopathological factors influenced ETI of EMC. Results Among 35 patients, 10 patients showed ETI. The average follow-up was 5.57 (range=0.2-20 years). The 5- and 10-year overall survival was 91.3% and 71.2%, respectively. The 5- and 10-year overall survival of patients with M0 disease was 96.1% and 73.2%, respectively, while both were 75.0% for those with M1 disease, respectively. The patients with distant metastasis at first visit tended to have a poor prognosis (p=0.07). It is notable that all of the 10 patients with ETI had distant metastasis after surgery. Conclusion Patients with distant metastasis at first visit tended to have a poor prognosis. ETI of EMC induced distant metastasis after surgery. Patients with ETI of EMC should, therefore, be carefully monitored over a prolonged period.

Simple Summary Second-line systematic therapy options for soft-tissue sarcoma (STS) have remained unchanged for decades due to the rarity and various histological types associated with STS. Challenges with molecular-targeted treatments for STS led to the approval of pazopanib and its wide use for STS. However, predictive markers of pazopanib treatment for STS have not been identified. Baseline neutrophil-to-lymphocyte ratio (NLR) is known as a candidate biomarker for several cancers. In this retrospective study, we investigated the use of the NLR as a predictor for the efficacy and prognosis of pazopanib in patients with STS. Our findings could be useful for the development of biomarker-targeted therapies for STS. Abstract Pazopanib with trabectedin and eribulin is widely used to treat soft-tissue sarcoma (STS). We have shown that baseline neutrophil-to-lymphocyte ratio (NLR) may predict the efficacy and patient prognosis of eribulin. Changes in NLR, but not baseline NLR, can predict patient prognosis of trabectedin. However, prognostic factors of pazopanib for STS have not been identified. We present a retrospective analysis of 141 patients treated with pazopanib for recurrent or metastatic non-round cell STS. Univariate and multivariate analyses were performed to determine the predictive factors of durable clinical benefit (DCB), overall survival (OS), and progression-free survival. L-sarcoma histology (odds ratio [OR] = 0.31, 95% CI = 0.12–0.79; p = 0.014) and pre-treatment NLR < 3.0 (OR = 2.03, 95% CI = 1.02–6.67; p = 0.045) were independent predictive factors of DCB. Pre-treatment NLR < 3.0 (hazard ratio [HR] = 0.55, 95% CI = 0.36–0.84; p = 0.0057), liposarcoma histology (HR = 1.78, 95% CI = 1.09–2.91; p = 0.022), primary extremity site (HR = 0.48, 95% CI = 0.31–0.75; p = 0.0010), ECOG PS ≥ 1 (HR = 1.62, 95% CI = 1.08–2.42; p = 0.019), and CRP < 0.3 (HR = 0.52, 95% CI = 0.33–0.82; p = 0.0050) were independent predictive factors of OS. These findings indicate that baseline NLR predicts the efficacy and patient prognosis of pazopanib for STS.

Fatty acid esters of hydroxy fatty acids (FAHFAs) are a new class of endogenous lipids with interesting physiological functions in mammals. Despite their structural diversity and links with nuclear factor erythroid 2-related factor 2 (NRF2) biosynthesis, FAHFAs are less explored as NRF2 activators. Herein, we examined for the first time the synthetic docosahexaenoic acid esters of 12-hydroxy stearic acid (12-DHAHSA) or oleic acid (12-DHAHOA) against NRF2 activation in cultured human hepatoma-derived cells (C3A). The effect of DHA-derived FAHFAs on lipid metabolism was explored by the nontargeted lipidomic analysis using liquid chromatography-mass spectrometry. Furthermore, their action on lipid droplet (LD) oxidation was investigated by the fluorescence imaging technique. The DHA-derived FAHFAs showed less cytotoxicity compared to their native fatty acids and activated the NRF2 in a dose-dependent pattern. Treatment of 12-DHAHOA with C3A cells upregulated the cellular triacylglycerol levels by 17-fold compared to the untreated group. Fluorescence imaging analysis also revealed the suppression of the degree of LDs oxidation upon treatment with 12-DHAHSA. Overall, these results suggest that DHA-derived FAHFAs as novel and potent activators of NRF2 with plausible antioxidant function.

Background: Extra-abdominal desmoid tumors often occur in the necks, shoulder, chest wall, back, arm, buttock, thigh and leg. Multicentric extra-abdominal desmoids are rather rare and seem to have other clinical features. The aim of our study was to investigate clinical features, especially multicentric occurrence of extra-abdominal desmoid tumors. Patients and Methods: A total of 135 patients diagnosed with extra-abdominal desmoid were enrolled in this study from January 2005 to December 2019 at the Cancer Institute Hospital of The Japanese Foundation for Cancer Research. The operative procedure was principally wide excision. The clinicopathological factors [e.g., age, gender, pain, restriction of range of motion (ROM), tumor site, tumor size, surgical margin, multicentric occurrence, local recurrence, tumoral regression] were collected and assessed by univariate analysis. We assessed how multicentric occurrence influenced clinicopathological factors of desmoid tumors. Results: The median follow-up was 39.9 months (range=0.29-259 months). Among 135 patients, 20 had multicentric occurrence. Multicentric extra-abdominal desmoids occurred in the neck in six cases, shoulder in four, chest wall in three, back in three, thigh in two and leg in two. In the case of multicentric occurrence on thighs and legs, tumors arose not in the anterior compartment but in the posterior compartment. Univariate analysis showed association of multicentric extra-abdominal desmoids with high local recurrence (p=0.0003), restriction of ROM (p=0.0012) and tumor size larger than 5 cm (p=0.04) but surgical margins were not correlated with local recurrence (p=0.37). Conclusion: Surgery should be performed in those who have severe pain or restriction of ROM. A 'Wait and see' policy is a first-line management, especially for those with multicentric extra-abdominal desmoids.

Background Recently, the number of osteosarcomas in middle-aged and older patients has demonstrated an increasing trend; moreover, their results are comparatively worse than those of young patients. In Europe and the USA, the prognosis for osteosarcoma in middle-aged and older patients has improved with adjuvant chemotherapy. In Japan, however, the prognosis has remained poor. Materials and Methods We retrospectively analyzed the outcomes of osteosarcoma, especially in regards to preoperative chemotherapy, from January 1980 to July 2014. A total of 29 patients with high-grade osteosarcoma between the age of 40 and 65 years were included. We included patients without distant metastasis and with primary lesions that were deemed resectable. The mean age was 52.8 years (range 41–65 years), and the mean follow-up period was 103.2 months (range 5–314 months). Results Adjuvant chemotherapy was administered to 27 of 29 patients (93%), and 8 of 15 cases (53%) were able to undergo preoperative chemotherapy as planned, including CDDP. A major complication of chemotherapy was acute kidney injury due to CDDP (26%). The 5-year OS and 5-year EFS were 64.9% and 57.1%, respectively. After 2006, a policy to prioritize the resection of the primary lesion was implemented, and if the primary lesion was deemed resectable, preoperative chemotherapy was either not administered or administered for only a short duration. The 5-year OS after 2006 improved to 78.8%. Conclusions This study shows that administration of high-dose intensity preoperative chemotherapy was difficult in middle-aged and older patients due to their high rate of acute kidney injury by CDDP. For cases of osteosarcoma in middle-aged and older patients, if the primary lesion is resectable, preoperative chemotherapy should be minimized to prioritize the resection of the primary lesion. It was considered that, with appropriate measures to prevent complications, adjuvant chemotherapy may lead to improved prognosis. Level of evidence V.

BACKGROUND The role of neoadjuvant chemotherapy for localized synovial sarcoma is still controversial. This study aimed to explore the influence of neoadjuvant chemotherapy combined with surgery in localized synovial sarcoma through analysis of our hospital's patient records. PATIENTS AND METHODS A total of 122 patients diagnosed with synovial sarcoma were enrolled in this study from January 1980 to December 2016 at the Cancer Institute Hospital of The Japanese Foundation for Cancer Research. The impact of neoadjuvant chemotherapy on overall survival was assessed to show how clinicopathological factors (e.g. age, tumor size, treatment, dose intensity, pathological pattern and histological grading) influenced patient prognosis. RESULTS Among 106 patients, 76 (71.7%) received neoadjuvant chemotherapy and 30 (28.3%) did not. The median follow-up was 39.2 (range=12-286) months. Aluminum-phosphate-sulfates (APS) of the alunite supergroup occur in igneous rocks within zones of advanced argillic and silicic alteration in porphyry and epithermal ore environments. In this study we report on the presence of woodhouseite-rich APS in ash from the 27 September 2014 hydrothermal eruption of Ontake volcano. Scanning electron microscope coupled with energy dispersive X-ray spectrometer (SEM-EDS) and field emission (FE)-SEM-EDS observations show two types of occurrence of woodhouseite: (a) as cores within chemically zoned alunite-APS crystals (Zoned-alunite-woodhouseite-APS), and (b) as a coherent single-phase mineral in micro-veinlets intergrown with similar micro-veinlets of silica minerals (Micro-wormy-vein woodhouseite-APS). The genetic environment of APS minerals at Ontake volcano is that of a highly acidic hydrothermal system existing beneath the volcano summit, formed by condensation in magmatic steam and/or ground waters of sulfur-rich magmatic volatiles exsolved from the magma chamber beneath Mt. Ontake. Under these conditions, an advanced argillic alteration assemblage forms, which is composed of silica, pyrophyllite, alunite and kaolinite/dickite, plus APS, among other minerals. The discovery of woodhouseite in the volcanic ash of the Ontake 2014 hydrothermal eruption represents the first reported presence of APS within an active volcano. Other volcanoes in Japan and elsewhere with similar phreatic eruptions ejecting altered ash fragments will likely contain APS minerals derived from magmatic-hydrothermal systems within the subvolcanic environment. The presence of APS minerals within the advanced argillic zone below the summit vent of Ontake volcano, together with the prior documentation of phyllic and potassically altered ash fragments, provides evidence for the existence within an active volcano in Japan of an alteration column comparable to that of porphyry copper systems globally.

Synovial sarcoma is a malignant tumor, it accounts for approximately 5–10% of the soft tissue sarcoma, and mainly develops in the para-articular regions in adolescents and young adults. We reported a case of synovial sarcoma extending into the vastus intermedius muscle beyond the intermuscular septum which is considered to be the greatest barrier to ensuring a safety margin in musculoskeletal surgery. Preoperative sequential MRI showed the intermuscular-septum line and direct tumoral invasion beyond the intermuscular septum. A histopathological examination showed the direct invasion to the intermuscular septum. Synovial sarcoma seldom shows such direct invasive pattern to barriers, much less the robust barrier like an intermuscular septum. To our knowledge, no previous study has reported such unique extension of a synovial sarcoma beyond the intermuscular septum. Surgeons should be aware of the potential vulnerability of the intermuscular septum.

Combined limb salvage surgery and radiation therapy has been gaining traction for the treatment of soft tissue sarcomas of the foot in recent years. However, limb salvage surgery without radiation therapy is more desirable. In this study, we obtained good results from limb salvage surgery without radiation therapy in soft tissue sarcoma of the foot. Twenty patients histologically diagnosed with soft tissue sarcoma who underwent limb salvage surgery, including ray amputation, were evaluated. Their mean age was 39 years. The mean follow-up period was 94.5 months (6–355 months). The histological diagnoses included 9 cases of synovial sarcoma. Treatment, local recurrence rate, survival rate, complication, and ADL were evaluated. Surgical procedures consisted of ray amputation in 5 cases and limb salvage surgery in 15. Of these 15 cases, 14 (93%) required reconstructive surgery for skin defects. One patient (5%) exhibited recurrence. The 5-year overall survival rate was 94.7%, and the 5-year local progression-free survival rate was 92.9%. Early postoperative complications were found in 5 of 20 cases (25%), comprising flap failure in 4 cases and infection in 1 case; however, all patients eventually demonstrated good functional outcomes. In this study, the recurrence rate was 5% for patients who underwent limb salvage surgery without radiation therapy, and we obtained good local control. Since the anatomical structures of the foot act as barriers to tumor progression, the use of a method of evaluating resection margins to determine the extent of resection enabled successful surgery, not combined with radiation, without the recurrence of sarcomas.

MicroRNA (miRNA) can function as tumor suppressors or oncogenes, and also as potential specific cancer biomarkers; however, there are few published studies on miRNA in synovial sarcomas, and their function remains unclear. We transfected the OncomiR miRNA Precursor Virus Library into synovial sarcoma Fuji cells followed by a colony formation assay to identify miRNAs to confer an aggressive tumorigenicity, and identified miR-17-5p from the large colonies. MiR-17 was found to be induced by a chimeric oncoprotein SS18-SSX specific for synovial sarcoma, and all examined cases of human synovial sarcoma expressed miR-17, even at high levels in several cases. Overexpression of miR-17 in synovial sarcoma cells, Fuji and HS-SYII, increased colony forming ability in addition to cell growth, but not cell motility and invasion. Tumor volume formed in mice in vivo was significantly increased by miR-17 overexpression with a marked increase of MIB-1 index. According to PicTar and Miranda algorithms, which predicted CDKN1A (p21) as a putative target of miR-17, a luciferase assay was performed and revealed that miR-17 directly targets the 3′-UTR of p21 mRNA. Indeed, p21 protein level was remarkably decreased by miR-17 overexpression in a p53-independent manner. It is noteworthy that miR-17 succeeded in suppressing doxorubicin-evoked higher expression of p21 and conferred the drug resistance. Meanwhile, introduction of anti-miR-17 in Fuji and HS-SYII cells significantly decreased cell growth, consistent with rescued expression of p21. Taken together, miR-17 promotes the tumor growth of synovial sarcomas by post-transcriptional suppression of p21, which may be amenable to innovative therapeutic targeting in synovial sarcoma.

Volcanic fans and volcaniclastic aprons (ring plains) developed at the foot of a volcano preserve the products of high-discharge, sediment-laden lahar flows that originate from a volcano. Geological and geomorphological features of a volcanic fan are important for discussing the flow characteristics and history of far-reaching flows such as lahars. The present study focuses on volcanic-fan deposits at the northern base of Chokai Volcano in northeast Japan. The authors conducted geomorphological analysis, excavation survey at the distal part of the volcanic fan, sedimentary facies analysis, radiocarbon dating, and historical document analysis to understand the flow characteristics and history of far-reaching lahars in relation to changes in the depositional processes and their triggering volcanic activity. Geological observation by manual trenching and coring revealed that the volcanic fan is composed mainly of a series of lahar deposits formed by at least five large lahar events. The 14C ages of the lahar deposits and their correlation with historical documents indicate that the large lahars travelled down the volcanic fan at: the 2nd–5th century BCE (L1); 2nd to 4th century BCE (L2); 5th to 7th century CE (L3); 871 CE (L4); and 1801 CE (L5) after the Kisakata debris avalanche (2.5 ka). According to the stratigraphy, ratio of magmatic fragments and hydrothermally altered material content in the deposits, and correlation with historical documents, these lahar events can be classified into three types in terms of their material source: (1) a cohesive debris flow sourced by the debris avalanche deposit (L1); (2) a non-cohesive lahar by a magmatic eruption (L2, L3, L4, and L5–1 subunit); and (3) a cohesive debris flow by a phreatic eruption (L5–2 subunit). The facies analysis revealed that the different flow behaviors of lahars reflect their sourced volcanic activity. The oldest lahar (L1), incorporating debris avalanche deposit, flowed as hazardous cohesive debris flow; lahars after magmatic eruptions (L2, L3, L4, and L5–1 subunit) tends to be streamflow through the flow transformation of sandy debris flow; and a cohesive lahar (L5–2 subunit) by a phreatic eruption tends to be of small scale. These results indicate that the characteristics of a lahar depend largely on causal volcanic activity.

The volcanic ash of the eruption on September 27, 2014 at Ontake volcano consists mostly of altered rock fragments. The ash contains partly altered volcanic rock fragments consisting of primary igneous minerals (plagioclase, orthopyroxene, titanomagnetite, and feldspars) and volcanic glass accompanied by alteration minerals to some extents, and contains no juvenile fragments. These features indicate that the eruption was a non-juvenile hydrothermal eruption that was derived from the hydrothermal system developed under the crater. The major minerals derived from hydrothermal alteration zones are silica mineral, kaolin-group mineral, smectite, pyrophyllite, muscovite, alunite, anhydrite, gypsum, pyrite, K-feldspar, albite, and rutile. Minor chlorite, biotite, and garnet are accompanied. Five types of alteration mineral associations are identified from observations on individual ash particles: silica–pyrite, silica–pyrite ± alunite ± kaolin, silica–pyrophyllite–pyrite, silica–muscovite ± chlorite, and silica–K-feldspar ± albite ± garnet ± biotite. The associations indicate development of advanced argillic, sericite, and potassic alteration zones under the crater. Occurrence of anhydrite veinlet and the set of alteration zones indicate hydrothermal alteration zones similar to late-stage porphyry copper systems. Comparing the mineral associations with the geologic model of the late-stage porphyry copper systems, the source depths of mineral associations are estimated to range from near surface to >2 km. The depths of advanced argillic alteration, sericite, and potassic zones are 0 to ~2, ~1.5 to ~2, and >2 km, respectively.

Creep rupture life in the time range of 105 h and longer has been evaluated on the basis of creep rupture data available on modified 9Cr–1Mo (Gr.91) steel. The steel is strengthened by a tempered martensite lath structure, which degrades by means of strain induced recovery in short-term creep (Region H). The degradation leads to creep rupture. In long-term creep (Region L), static recovery of the martensite lath structure takes place in addition to the strain induced recovery, resulting in premature creep rupture and the consequent decrease in stress exponent n for rupture life. The premature creep rupture always occurs in Gr.91 steel, but the activation energy QL for rupture life in region L is usually close to the value QH in Region H in the steel. In such a case, long-term rupture life can be predicted correctly by simple extrapolation of short-term data. In some heats of Gr.91 steel, however, another region with low values of n and Q, namely Region L2, appears in the time range longer than 104 h. Simple extrapolation of short-term data results in overestimation of rupture life in Region L2. The static recovery and the consequent premature creep rupture are controlled by migration of sub-boundaries stabilized by precipitates. M23C6 as well as MX precipitates in Gr.91 steel have higher thermal stability than those in other high Cr ferritic steels, resulting in the later appearance or absence of Regions L and L2 in the steel.

Cavitation erosion often causes the leakage of water in piping systems of industrial plants. In this study, cavitation erosion tests were carried out for the pipe steel specimens fabricated from carbon steel pipes for pressure service, alloy steel pipes and stainless steel pipes. Total 15 pipe materials were tested in a stationary specimen test method using a vibratory apparatus specified by ASTM G32-03 and cavitation erosion were evaluated for each material. It was found that the erosion resistance could be estimated precisely by material hardness.

Cavitation erosion often causes the leakage of water in piping systems of industrial plants. Cavitation erosion tests were carried out for S15C carbon steel equivalent to pipe steel STPG370 in a stationary specimen test method using a vibratory apparatus specified by ASTM G32-03. Another test was performed using a cavitating liquid jet method according to ASTM G134-95 to simulate the flow condition. It was found that the maximum depth of erosion (MaxDE) increases with exposure time with a power of about 0.5 which is different from the ordinary power of 1.0. The distribution of the maximum depth of erosion pits was obtained by the extreme value analysis (Gumbel distribution) at every exposure time.
