Abstract

The Future of Work (FoW) is witnessing an evolution where AI systems (broadly machines or businesses) are used to the benefit of humans. Work here refers to all forms of paid and unpaid labor in both physical and virtual workplaces and that is enabled by AI systems. This covers crowdsourcing platforms such as Amazon Mechanical Turk, online labor marketplaces such as TaskRabbit and Qapa, but also regular jobs in physical workplaces. Bringing humans back to the frontier of FoW will increase their trust in AI systems and shift their perception to use them as a source of self-improvement, ensure better work performance, and positively shape social and economic outcomes of a society and a nation. To enable that, physical and virtual workplaces will need to capture human traits, behavior, evolving needs, and provide jobs to all. Attitudes, values, opinions regarding the processes and policies will need to be assessed and considered in the design of FoW ecosystems.

Abstract

Background: The incidence of ocular candidiasis (OC) in patients with candidemia varies across different reports, and the issue of whether routine ophthalmoscopy improves outcomes has been raised. This study investigated the incidence of OC and evaluate whether the extent of OC impacts the clinical outcomes. Methods: This retrospective study included non-neutropenic patients with candidemia who underwent treatment at one of 15 medical centers between 2010 and 2016. Chorioretinitis without other possible causes for the ocular lesions and endophthalmitis was classified as a probable OC. If signs of chorioretinitis were observed in patients with a systemic disease that causes similar ocular lesions, they were classified as a possible OC. Results: In total, 781 of 1089 patients with candidemia underwent an ophthalmic examination. The prevalence of OC was 19.5%. The time from the collection of a positive blood culture to the initial ophthalmic examination was 5.0 ± 3.9 days in patients with OC. The leading isolate was Candida albicans (77.9%). Possible OC was associated with unsuccessful treatments (resolution of ocular findings) (odds ratio: 0.354, 95% confidence interval: 0.141–0.887), indicating an overdiagnosis in patients with a possible OC. If these patients were excluded, the incidence fell to 12.8%. Endophthalmitis and/or macular involvement, both of which require aggressive therapy, were detected in 43.1% of patients; a significantly higher incidence of visual symptoms was observed in these patients. Conclusion: Even when early routine ophthalmic examinations were performed, a high incidence of advanced ocular lesions was observed. These results suggest that routine ophthalmic examinations are still warranted in patients with candidemia.

9. Dynamic modelling of cold-hardiness in tea buds by imitating past temperature memory.

Abstract

Background and Aims Most perennial plants memorize cold stress for a certain period and retrieve the memories for cold acclimation and deacclimation, which leads to seasonal changes in cold-hardiness. Therefore, a model for evaluating cold stress memories is required for predicting cold-hardiness and for future frost risk assessments under warming climates. In this study we develop a new dynamic model of cold-hardiness by introducing a function imitating past temperature memory in the processes of cold acclimation and deacclimation. Methods We formulated the past temperature memory for plants using thermal time weighted by a forgetting function, and thereby proposed a dynamic model of cold-hardiness. We used the buds of tea plants (Camellia sinensis) from two cultivars, 'Yabukita' and 'Yutakamidori', to calibrate and validate this model based on 10 years of observed cold-hardiness data. Key Results The model captured more than 90 % of the observed variation in cold-hardiness and predicted accurate values for both cultivars, with root mean square errors of ~1.0 °C. The optimized forgetting function indicated that the tea buds memorized both short-term (recent days) and long-term (previous months) temperatures. The memories can drive short-term processes such as increasing/decreasing the content of carbohydrates, proteins and antioxidants in the buds, as well as long-term processes such as determining the bud phenological stage, both of which vary with cold-hardiness. Conclusions The use of a forgetting function is an effective means of understanding temperature memories in plants and will aid in developing reliable predictions of cold-hardiness for various plant species under global climate warming. 10. Usefulness of Serum C-reactive Protein in the Management of Adult Community-acquired Pneumonia

Abstract

C-Reactive protein (CRP) is widely used as a marker of infection, but there is insufficient evidence as to its usefulness in patients with community-acquired pneumonia (CAP). In the present study, we investigated the clinical usefulness of CRP in a retrospective study of 242 patients aged 14 years who were hospitalized with CAP. Patients were classified into three groups according to the number of days between disease onset and the initial measurement of CRP as follows: Group 1, 0-1 day; Group 2, 2-4 days; Group 3, 5 days. Patients in Groups 2 and 3, who had more severe pneumonia, had higher CRP levels. Over time, CRP levels decreased in the responders in Groups 2 and 3; specifically, in Group 2, median CRP levels on Days 0, 3, and 7 were 9.85, 5.33, and 0.81mg/dL, respectively, compared with 9.99, 4.29, and 0.70mg/dL, respectively, in Group 3. In patients not responding to initial treatment, median CRP levels increased from Day 0 to Day 3 (4.32 vs. 11.70mg/dL, respectively). In all non-responders, CRP levels on Day 3 were>50% of levels on Day 0. In conclusion, when measured approximately 48 h after disease onset, CRP is useful for evaluating the severity of pneumonia and predicting the response to treatment. A good clinical outcome is likely when CRP levels on Day 3 are 50% of those on admission.

14. Evaluation of joint thickness in the hip before and after osteotomy using three-dimensional modelling

Abstract

Joint thickness was assessed using three-dimensional modelling techniques before and after the Bombelli intertrochanteric valgus-extension osteotomy. Before operation, the average thickness of the entire hemispherical surface was 2.3 mm, which was significantly thinner than that of the controls. After operation, 11 of 12 patients showed an average increase in thickness to 3 mm, nearly equal to that of the controls, and this was accompanied by clinical improvement. Abstract

Effects of geometrical configurations of welds on stress and strain distributions are investigated experimentally and theoretically. Three types of weld configurations are analyzed under axial and bending loads. Analyses by using a threedimensional finite element method are performed, and the results are compared with the experimental ones with good accuracy. Stress and strain concentration factors are also investigated, and the applicability of Neuber's equation is examined.

Abstract

Techniques and setups for testing full-size non-sway structural subassemblages with laterally braced and unbraced columns are described. The height of the sub-assemblages varied from approximately 20 to 33 feet and the width 20 to 37 feet. The columns were subjected to axial forces applied by a universal testing machine and also bending moments transmitted from the adjoining beams. These beams were loaded by two quarter-point loads through the use of gravity load simulators. Depending on the locations of the beams, the columns in the subassemblage were bent either in single curvature or in double curvature. In-plane deflections of the columns were recorded electronically while the out-of-plane deformations of the unbraced columns were recorded by means of depth gages. Electrical resistance gages were mounted on the beams and columns in a unique arrangement so as to allow various types of strains to be analyzed separately. These strains are: normal strain due to axial force, normal strain due to strong axis bending, normal strain due to weak axis bending, normal strain due to warping of section and St. Venant shearing strain. 26. Management of large and giant pituitary adenomas with suprasellar extensions.

Abstract

Objective and Background: We evaluated the feasibility and effectiveness of transsphenoidal surgery for large and giant pituitary adenomas with suprasellar extensions as these tumors have been therapeutic challenge., Subjects and Methods: We retrospectively analyzed 50 cases with 56 surgeries in patients with pituitary adenomas that were surgically treated between January 2005 and January 2010 at Fujita Health University. Among those cases, 39 cases were large or giant pituitary adenomas including 11 cases of giant adenomas., Results: 37 cases 41 approaches were transsphenoidal, 2 approaches were transcranial, and in 1 case transcranial approach following transsphenoidal surgery was performed. The most frequent preoperative symptoms were visual impairment and visual field defect (28 cases, 75.6%), and improvement of visual function after surgery was observed in 18 cases, 64%. As endocrinological results, among the 11 cases of functioning adenomas, improvement of endocrinological examination was observed in 10 cases, and normalization of the hormonal examination and complete remission was seen in 7 cases which was 64%., Conclusions: Transsphenoidal approach is safe and effective procedure even in large or giant pituitary adenomas, because it allows rapid and appropriate decompression of the optic nerves and chiasm with low morbidity rates. Transcranial approaches were indicated only in irregular shaped adenomas or eccentric extensions that could not be reached through the transsphenoidal route.

Abstract

Airway and vascular constrictions are expected problems for lung cancer patients. The clinical course of a patient in whom stents were inserted in 3 constriction regions is reported. A 72-year-old man with advanced primary lung cancer (squamous cell carcinoma stage IIIB) developed suffocating constriction of the trachea and showed superior vena cava (SVC) syndrome. Self-expandable metal stents were inserted into the trachea, SVC and left subclavian vein and the patient was treated by radiotherapy and weekly irinotecan. Within 5 months of treatment, complete local control was achieved and there was no reconstriction. Both endobronchial and endovascular stenting prior to antitumor therapy may be beneficial in the treatment of locally advanced cases of lung cancer.
