Introduction: Anatomical femoral tunnel placement is critical for anterior cruciate ligament reconstruction (ACLR). Tunnel placement may vary with different surgical techniques. The aim of this study was to compare the accuracy of femoral tunnel placement between the Anteromedial (AM) and Anterolateral (AL) visualisation portals on post-operative CT scans among a cohort of ACLR patients. Materials and methods: This cross-sectional study was conducted from January 2018 to March 2020 after obtaining ethics clearance. Patients who went for arthroscopic ACLR in our institute were divided into an AM (group 1) and an AL (group 2) based on the visualisation portal for creating the femoral tunnel and a 3D CT scan was done. The femoral tunnel position was calculated in deep to shallow and high to low direction using the Bernard Hertel grid. Femoral tunnel angle was measured in the 2D coronal image. Statistical analysis was done with the data collected. Results: Fifty patients with an average age of 26.36 (18-55) years ±7.216 SD were enrolled in the study. - Abstract
OBJECTIVES: To evaluate the association between the pre-extubation sum of eye and motor components of the Glasgow Coma Score (GCS-EM) and odds of extubation failure in patients with acute brain injury being liberated from mechanical ventilation. DESIGN: Secondary analysis of a prospective, multicenter observational study (ClinicalTrials.gov identifier NCT03400904). SETTING: Sixty-three hospital sites worldwide, with patient recruitment from January 2018 to November 2020. PATIENTS: One thousand one hundred fifty-two critically ill patients with acute brain injury, with a median age of 54 years, of whom 783 (68.0%) were male, 559 (48.5%) had traumatic brain injury, and 905 (78.6%) had a GCS-EM greater than 8 before extubation (scores range from 2 to 10). INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: GCS-EM was computed in intubated patients on the day of extubation. The main outcome was extubation failure, defined as unplanned reintubation within 5 days of extubation. Analyses used multilevel logistic regression with adjustment for patient characteristics and a random intercept for hospital site. In the primary analysis, GCS-EM was not associated with extubation failure (odds ratio, 1.07 per additional point; 95% CI, 0.87-1.31). Findings were consistent in sensitivity analyses that: 1) used different adjustment covariates, 2) included a verbal estimate to derive an overall GCS, 3) accounted for missing data, 4) considered a 2-day time interval to define extubation failure, 5) accounted for competing risks, and 6) used a propensity score-based model. There was no association between GCS-EM and extubation outcome in subgroups defined by brain injury diagnosis or age. CONCLUSIONS: In this large, contemporary, multicenter cohort of patients with acute brain injury, we found no association between the GCS-EM and odds of extubation failure. Elliptic‐curve cryptography (ECC) is a popular technique of public‐key cryptography used for secure communications in Internet of Things (IoT) applications. In this study, the implementation and analyze the Elliptic Curve Digital Signature Algorithm (ECDSA) with NIST‐256 on MicroBlaze processors based on RISC‐V architecture. This implementation aims to enhance secure communication and data exchange between IoT devices and also provides a detailed account of our ECDSA implementation on a RISC‐V processor and evaluates its performance, achieving a reduction of 11.8% in execution time compared to existing techniques. The implementation takes 483 clock cycles and utilizes approximately 4096 bytes of memory. These results demonstrate that ECDSA can be efficiently implemented on RISC‐V processors, making it a promising option for secure communication in IoT applications. Furthermore, the study highlights specific security considerations critical to implementing ECC on RISC‐V processors, ensuring robust and reliable security in IoT environments. [ABSTRACT FROM AUTHOR]

- Abstract
Purpose: The use of arterial partial pressure of carbon dioxide (PaCO2) as a target intervention to manage elevated intracranial pressure (ICP) and its effect on clinical outcomes remain unclear. We aimed to describe targets for PaCO2 in acute brain injured (ABI) patients and assess the occurrence of abnormal PaCO2 values during the first week in the intensive care unit (ICU). The secondary aim was to assess the association of PaCO2 with in-hospital mortality. Methods: We carried out a secondary analysis of a multicenter prospective observational study involving adult invasively ventilated patients with traumatic brain injury (TBI), subarachnoid hemorrhage (SAH), intracranial hemorrhage (ICH), or ischemic stroke (IS). PaCO2 was collected on day 1, 3, and 7 from ICU admission. Normocapnia was defined as PaCO2 > 35 and to 45 mmHg; mild hypocapnia as 32–35 mmHg; severe hypocapnia as 26–31 mmHg, forced hypocapnia as < 26 mmHg, and hypercapnia as > 45 mmHg. Results: 1476 patients (65.9% male, mean age 52 ± 18 years) were included. On ICU admission, 804 (54.5%) patients were normocapnic (incidence 1.37 episodes per person/day during ICU stay), and 125 (8.5%) and 334 (22.6%) were mild or severe hypocapnic (0.52 and 0.25 episodes/day). Forced hypocapnia and hypercapnia were used in 40 (2.7%) and 173 (11.7%) patients. PaCO2 had a U-shape relationship with in-hospital mortality with only severe hypocapnia and hypercapnia being associated with increased probability of in-hospital mortality (omnibus p value = 0.0009). Important differences were observed across different subgroups of ABI patients. Conclusions: Normocapnia and mild hypocapnia are common in ABI patients and do not affect patients' outcome. Extreme derangements of PaCO2 values were significantly associated with increased in-hospital mortality.

Mutational robustness is the extent to which an organism has evolved to withstand the effects of deleterious mutations. We explored the extent of mutational robustness in the budding yeast by genome wide dosage suppressor analysis of 53 conditional lethal mutations in cell division cycle and RNA synthesis related genes, revealing 660 suppressor interactions of which 642 are novel. This collection has several distinctive features, including high co-occurrence of mutant-suppressor pairs within protein modules, highly correlated functions between the pairs, and higher diversity of functions among the co-suppressors than previously observed. Dosage suppression of essential genes encoding RNA polymerase subunits and chromosome cohesion complex suggest a surprising degree of functional plasticity of macromolecular complexes and the existence od degenerate pathways for circumventing potentially lethal mutations. The utility of dosage-suppressor networks is illustrated by the discovery of a novel connection between chromosome cohesion-condensation pathways involving homologous recombination, and Huntington's disease., Comment: 42 pages, 2 tables, 6 Figures. - Abstract
Background: There is insufficient evidence to guide ventilatory targets in acute brain injury (ABI). Recent studies have shown associations between mechanical power (MP) and mortality in critical care populations. We aimed to describe MP in ventilated patients with ABI, and evaluate associations between MP and clinical outcomes. Methods: In this preplanned, secondary analysis of a prospective, multi-center, observational cohort study (ENIO, NCT03400904), we included adult patients with ABI (Glasgow Coma Scale ≤ 12 before intubation) who required mechanical ventilation (MV) ≥ 24 h. Using multivariable log binomial regressions, we separately assessed associations between MP on hospital day (HD)1, HD3, HD7 and clinical outcomes: hospital mortality, need for reintubation, tracheostomy placement, and development of acute respiratory distress syndrome (ARDS). Results: We included 1217 patients (mean age 51.2 years [SD 18.1], 66% male, mean body mass index [BMI] 26.3 [SD 5.18]) hospitalized at 62 intensive care units in 18 countries. Hospital mortality was 11% (n = 139), 44% (n = 536) were extubated by HD7 of which 20% (107/536) required reintubation, 28% (n = 340) underwent tracheostomy placement, and 9% (n = 114) developed ARDS. The median MP on HD1, HD3, and HD7 was 11.9 J/min [IQR 9.2–15.1], 13 J/min [IQR 10–17], and 14 J/min [IQR 11–20], respectively. MP was overall higher in patients with ARDS, especially those with higher ARDS severity. After controlling for same-day pressure of arterial oxygen/fraction of inspired oxygen (P/F ratio), BMI, and neurological severity, MP at HD1, HD3, and HD7 was independently associated with hospital mortality, reintubation and tracheostomy placement. The adjusted relative risk (aRR) was greater at higher MP, and strongest for: mortality on HD1 (compared to the HD1 median MP 11.9 J/min, aRR at 17 J/min was 1.22, 95% CI 1.14–1.30) and HD3 (1.38, 95% CI 1.23–1.53), reintubation on HD1 (1.64; 95% CI 1.57–1.72), and tracheostomy on HD7 (1

- Abstract
Background: In acute brain injury (ABI), the effects of hypoxemia as a potential cause of secondary brain damage and poor outcome are well documented, whereas the impact of hyperoxemia is unclear. The primary aim of this study was to assess the episodes of hypoxemia and hyperoxemia in patients with ABI during the intensive care unit (ICU) stay and to determine their association with in-hospital mortality. The secondary aim was to identify the optimal thresholds of arterial partial pressure of oxygen (PaO2) predicting in-hospital mortality. Methods: We conducted a secondary analysis of a prospective multicenter observational cohort study. Adult patients with ABI (traumatic brain injury, subarachnoid aneurysmal hemorrhage, intracranial hemorrhage, ischemic stroke) with available data on PaO2 during the ICU stay were included. Hypoxemia was defined as PaO2 < 80 mm Hg, normoxemia was defined as PaO2 between 80 and 120 mm Hg, mild/moderate hyperoxemia was defined as PaO2 between 121 and 299 mm Hg, and severe hyperoxemia was defined as PaO2 levels ≥ 300 mm Hg. Results: A total of 1,407 patients were included in this study. The mean age was 52 (±18) years, and 929 (66%) were male. Over the ICU stay, the fractions of patients in the study cohort who had at least one episode of hypoxemia, mild/moderate hyperoxemia, and severe hyperoxemia were 31.3%, 53.0%, and 1.7%, respectively. PaO2 values below 92 mm Hg and above 156 mm Hg were associated with an increased probability of in-hospital mortality. Differences were observed among subgroups of patients with ABI, with consistent effects only seen in patients without traumatic brain injury. Conclusions: In patients with ABI, hypoxemia and mild/moderate hyperoxemia were relatively frequent. Hypoxemia and hyperoxemia during ICU stay may influence in-hospital mortality. However, the small number of oxygen values collected represents a major limitation of the study.

Background: A solitary nodule is a goiter which, on clinical examination appears to be a single nodule in one lobe of the thyroid with no palpable abnormality elsewhere in the gland. The incidence of solitary nodule in general population in South India is 9%. This study aims to assess the presence of malignancy in patients presenting with solitary thyroid nodule and to evaluate and compare results of various investigative modalities used to diagnose a solitary thyroid nodule and the outcome of management modalities in solitary thyroid nodule. Methods: The present study on clinicopathological correlation and management of solitary thyroid nodule has been conducted on cases admitted and managed in the Department of Surgery at KIMS medical college, Hubli. Prospective analysis of 50 cases of solitary nodule thyroid in the specified period was done. These cases were selected by random sampling method and studied in detail clinically and recorded as per the proforma. Routine investigations and specific investigations including FNAC of the nodule, Thyroid profile, IDL, Plain X-ray neck, USG neck were done in all cases. Special investigations like radio-isotope scanning were not performed as the facilities were not available. All the patients were managed by surgery and diagnosis was confirmed by histo- pathological examination. The patients were grouped according to different variables like age, sex, size of the nodule, site of the nodule, functional thyroid status, FNAC reports and histo-pathological examination reports, then analyzed and compared with the previous similar studies conducted elsewhere. Results: Total of 50 cases of solitary nodule of thyroid studied. The mean age of presentation is 38.5 years. Cases in 3rd to 5th decades constitutes 72% of the cases studied. Out of 50 cases studied 46 were females and 4 were males, and the ratio comes to M:F = 1 : 11.5. Out of 50 cases along with swelling in front of the neck, 3 cases had pain, 3 cases had discomfort .....

- Abstract
Purpose: Neurocritical care patients receive prolonged invasive mechanical ventilation (IMV), but there is poor specific information in this high-risk population about the liberation strategies of invasive mechanical ventilation. Methods: ENIO (NCT03400904) is an international, prospective observational study, in 73 intensive care units (ICUs) in 18 countries from 2018 to 2020. Neurocritical care patients with a Glasgow Coma Score (GCS) ≤ 12, receiving IMV ≥ 24 h, undergoing extubation attempt or tracheostomy were included. The primary endpoint was extubation failure by day 5. An extubation success prediction score was created, with 2/3 of patients randomly allocated to the training cohort and 1/3 to the validation cohort. Secondary endpoints were the duration of IMV and in-ICU mortality. Results: 1512 patients were included. Among the 1193 (78.9%) patients who underwent an extubation attempt, 231 (19.4%) failures were recorded. The score for successful extubation prediction retained 20 variables as independent predictors. The area under the curve (AUC) in the training cohort was 0.79 95% confidence interval (CI95) [0.71–0.87] and 0.71 CI95 [0.61–0.81] in the validation cohort. Patients with extubation failure displayed a longer IMV duration (14 [7–21] vs 6 [3–11] days) and a higher in-ICU mortality rate (8.7% vs 2.4%). Three hundred and nineteen (21.1%) patients underwent tracheostomy without extubation attempt. Patients with direct tracheostomy displayed a longer duration of IMV and higher in-ICU mortality than patients with an extubation attempt (success and failure). Conclusions: In neurocritical care patients, extubation failure is high and is associated with unfavourable outcomes. A score could predict extubation success in multiple settings. However, it will be mandatory to validate our findings in another prospective independent cohort.

20. Medication adherence and its association with socioeconomic factors and quality of life in patients with rheumatoid arthritis.

Background: Medication adherence to prescribed medications in chronic diseases like rheumatoid arthritis (RA) is influenced by social and economic factors, health-care team/system factors, characteristic of the disease, disease therapies, and physician-related and patient-related factors. Socioeconomic factors of the patients play a major role in adherence to long-term therapies. In view of lack of consistent findings and limited studies, this study aimed to study the socioeconomic factors and quality of life (QOL) and their association with medication adherence in RA. Materials and Methods: This is a prospective observational study carried out in a tertiary care hospital from February 2021 to June 2021. The Compliance-Questionnaire-Rheumatology 19 was used to assess adherence with scores ≥80% considered adherent and others nonadherent. QOL was assessed by the WHOQOL-BREF Questionnaire, and socioeconomic factors were determined using the Modified Kuppuswamy's Socioeconomic Status scale 2020. Results: Of the 135 patients (108 females and 27 males) in the study, adherence was observed only in 19.3% (26) and the remaining 80.7% (109) were nonadherent. There was a significant difference (P = 0.013) in the number of medications used among both the groups. Among the socioeconomic factors, though there is difference in occupation of both the groups, it was found be not significant (P = 0.058). There was no association of adherence with any of the four domains of QOL. Conclusion: There was no association of medication adherence with socioeconomic factors and QOL in RA. A global event such as the COVID-19 crisis presents new, often unexpected responses that are fascinating to investigate from both, scientific and social standpoints. Despite several documented similarities, the Coronavirus pandemic is clearly distinct from the 1918 flu pandemic in terms of our exponentially increased, almost instantaneous ability to access/share information, offering an unprecedented opportunity to visualise rippling effects of global events across space and time. Personal devices provide “big data” on people’s movement, the environment and economic trends, while access to the unprecedented flurry in scientific publications and media posts provides a measure of the response of the educated world to the crisis. Most bibliometric (co-authorship, co-citation, or bibliographic coupling) analyses ignore the time dimension, but COVID-19 has made it possible to perform a detailed temporal investigation into the pandemic. Here, we report a comprehensive network analysis based on more than 20000 published documents on viral epidemics, authored by over 75,000 individuals from 140 nations in the past one year of the crisis. In contrast to the 1918 flu pandemic, access to published data over the past two decades enabled a comparison of publishing trends between the ongoing COVID-19 pandemic and those of the 2003 SARS epidemic, to study changes in thematic foci and societal pressures dictating research over the course of a crisis.

The influence of Calcination temperature on the physical properties of Ni0.6Zn0.4Fe2O4 ferrite nanoparticles were investigated. These ferrite nanoparticles have been synthesized by sol-gel auto combustion method using citric acid as fuel agent at different calcination temperatures (4000C, 5000C and 6000C). The Morphological investigation, average crystallite size and microstructure of the material were examined by using X-ray diffraction (XRD) and confirmed by field emission scanning electron microscope (FESEM) and FTIR spectra. The Effects of calcination temperature on the dielectric and magnetic properties were calculated by using LCR meter and vibrating sample magnetometer (VSM). The XRD result shows a single-phase cubic spinel structure with average crystallite size increases from 27 to 29.5 nm, with an increase of temperature. The highest saturation magnetization was found at a calcination temperature 6000C with value 80.39 emu/g, and the value coercive field (Hc) was inverse with the crystallite size.

Under organic management condition, it is presumed that the nutrients applied to preceding crops exhibits the considerable effect on succeeding crops, hence a fixed plot field experiment was conducted during three years (2011 to 2014) at Research Farm, National Organic Farming Research Institute (NOFRI), Tadong, Gangtok to assess the residual effect of mulching and vermicompost on growth, productivity, profitability and energetics of popcorn (Zea mays everta) under toria (Brassica campestris)-popcorn cropping system. The results revealed that the mulching materials applied to the preceding crops exhibited significant residual effect on growth, yield attributes and grain yield of succeeding popcorn over non-mulching (bare soil). However, among the mulching materials, rice straw mulch @ 5 t/ha exerted maximum residual effect and resulted in higher cob length (16.0 cm), cob girth (12.0 cm), grain yield (1.93 ha), stover yield (3.07 t/ha), net returns (72.8 × 103 rs.`/ha), B:C ratio (2.87) and energy use efficiency (20.62%) of popcorn. With regards to the vermicompost levels, residual effect of vermicompost @ 1.5 t/ha recorded the maximum grain yield (1.9 t/ha), net returns (71.3× 103 rs.`/ha), B:C ratio (2.81) and energy use efficiency (20.34%) of the succeeding popcorn under toria-popcorn cropping system.
