1. Real-time correction of betatron tune ripples on a slowly extracted beam

Abstract

In the main ring (MR) of the Japan Proton Accelerator Research Complex (J-PARC), the power supplies of magnets induce large ripples in the betatron tunes. The tune ripples degrade the quality of the beam spill on the slowly extracted beam. To improve the spill quality, the J-PARC MR employed a feedback system using a beam spill monitor; however, the existing system could not correct the beam spills suitably, since the tune ripples were too large. We developed a new correction system using the information of magnet currents as an input of the correction. The new system consists of current monitors and a corrector quadrupole, and it allows us to correct the ripples in real time. Using this system, we successfully corrected the tune ripples for the first time in the J-PARC MR. The correction system improved the spill quality from 3% (without any correction) to 30% (with the new correction), whereas the quality using the present feedback system is 17.4%.

Abstract

The Tokai-to-Kamioka (T2K) experiment is a second generation long baseline neutrino oscillation experiment which aims at a sensitive search for the nu_e appearance. The main design features of the T2K near detectors located at 280 m from the target are presented. Scintillator counters developed for the T2K near detectors are described. Readout of the counters is provided via WLS fibers embedded into S-shape grooves in a scintillator and viewed from both ends by multi-pixel avalanche photodiodes operating in a limited Geiger mode. A desription, operational principles and the results of tests of photosensors with a sensitive area of 1.1 mm^2 are presented. A time resolution of 1.5 ns, a spatial resolution of 9.9-12.4 cm, and a MIP detection efficiency of more than 99% were obtained for scintillator detectors in a beam test., Comment: 23 pages, 20 figures, 2 tables

Abstract

We present measurements of nu_mu disappearance in K2K, the KEK to Kamioka long-baseline neutrino oscillation experiment. One hundred and twelve beam-originated neutrino events are observed in the fiducial volume of Super-Kamiokande with an expectation of 158.1^{+9.2}_{-8.6} events without oscillation. A distortion of the energy spectrum is also seen in 58 single-ring muon-like events with reconstructed energies. The probability that the observations are explained by the expectation for no neutrino oscillation is 0.0015% (4.3sigma). In a two flavor oscillation scenario, the allowed Delta m^2 region at sin^2(2theta) is between 1.9 and 3.5 x 10^{-3} eV^2 at the 90% C.L. with a best-fit value of 2.8 x 10^{-3} eV^2., Comment: 40 pages, 48 figures

Abstract

Precise measurement of neutrino beam direction and intensity was achieved based on a new concept with modularized neutrino detectors. INGRID (Interactive Neutrino GRID) is an on-axis near detector for the T2K long baseline neutrino oscillation experiment. INGRID consists of 16 identical modules arranged in horizontal and vertical arrays around the beam center. The module has a sandwich structure of iron target plates and scintillator trackers. INGRID directly monitors the muon neutrino beam profile center and intensity using the number of observed neutrino events in each module. The neutrino beam direction is measured with accuracy better than 0.4 mrad from the measured profile center. Abstract

In this paper, the authors propose a novel block feeding method for use in linear induction motors. The proposed method involves shifting of the excited section at every coil. Using the proposed method, the excited-section length can be shortened greatly compared with that in the ordinary method shifting at every block section. Thus, the power consumption becomes lower, improving efficiency. In addition, by short-circuiting the coils at both ends of the excited section during shifting, continuous power supply becomes possible, and thus electric transient phenomena are reduced. They confirm the validity of the method by finite element analysis and experiments.

Abstract

The charge exchange energy spectrum of beam ions is analyzed in order to study the fast ion confinement. The spectrum is observed to have a depletion, and the effects of the magnetic field, B0 and radial electric field, Er, induced losses on the depletion structures are discussed. Relaxation oscillations of the spectrum are found and a time varying Er is considered to interpret them. A reduction of the depletion structure is observed with electron cyclotron resonance heating (ECRH), and is discussed with the edge Er change and improved ion heating.

Abstract

This paper describes magnetic equilibrium diagnostics that are installed both inside and outside the vacuum vessel in the toroidal helical device Heliotron E. These magnetic diagnostics consist of Rogowski coils for plasma currents, a diamagnetic loop with compensation coils, a set of full flux loops, a set of partial saddle loops, modulated Rogowski coils with several harmonics, a 2D array of Bp coils and Rogowski coils for bus lines of helical, auxiliary vertical and toroidal coils. Up to now we have routinely obtained the diamagnetic stored energy, toroidal plasma currents, the dipole moment of the Pfirsch-Schluter current, the toroidal voltage, the finite β free-boundary plasma shift and poloidal fluxes. We will describe the signals of measured magnetic fluxes and give some interpretations of the finite β equilibrium.

Abstract

In the Heliotron E device, fast ion slowing-down processes (deceleration, parallel diffusion, and pitch angle scattering) and the particle loss process are investigated by the measurement temporal behaviour of charge-exchange energy spectra during neutral beam injection (NBI). Using electron cyclotron resonance heating (ECRH), two effects are studied with respect to: (1) extrapolation of slowing-down behaviour to a longer slowing-down time and (2) modification of the ion loss associated with the radial electric field. It is found that (1) there are discrepancies :in the theoretical value in the deceleration and the parallel diffusion processes in NBI + ECRH and ECRH plasmas, and that (2) the particle loss is reduced by the use of ECRH on the NBI plasma.

Abstract

An edge electron cyclotron heating (ECH) experiment has been performed on the Heliotron-E helical device using 35 GHz and 53 GHz gyrotrons. Transition phenomena have been observed during the 53 GHz edge ECH. The Hα emission and the outflux to the wall dropped, and then the core electron density increased by 20%–60%. The electron stored energy seems to be improved up to 20% after the transition. This phenomenon is found to depend strongly on the edge heating power, the target plasma density and the limiter position. A possible mechanism is discussed in terms of high energy electrons produced by the edge ECH.

Abstract

The SciBooNE and MiniBooNE collaborations report the results of a νμ disappearance search in the Δ'm2 region of 0.5-40 eV2. The neutrino rate as measured by the SciBooNE tracking detectors is used to constrain the rate at the MiniBooNE Cherenkov detector in the first joint analysis of data from both collaborations. Two separate analyses of the combined data samples set 90% confidence level (CL) limits on νμ disappearance in the 0.5-40 eV2 Δm2 region, with an improvement over previous experimental constraints between 10 and 30 eV2

Abstract

We report a measurement of muon-neutrino disappearance in the T2K experiment. The 295-km muon-neutrino beam from Tokai to Kamioka is the first implementation of the off-axis technique in a long-baseline neutrino oscillation experiment. With data corresponding to 1.43 × 10(20) protons on target, we observe 31 fully-contained single μ-like ring events in Super-Kamiokande, compared with an expectation of 104 ± 14(syst) events without neutrino oscillations. The best-fit point for two-flavor νμ → ντ oscillations is sin 2(2θ(23)) = 0.98 and |Δm(2)(32)| = 2.65 × 10(−3) eV2. The boundary of the 90% confidence region includes the points (sin2 (2θ(23)), |Δm(2)(32)|) = (1.0, 3.1 × 10(−3) eV2), (0.84, 2.65 × 10(−3) eV2) and (1.0, 2.2 × 10(−3) eV2)., Physical Review D, 85 (3), ISSN:1550-7998, ISSN:0556-2821, ISSN:1550-2368

Abstract

The SciBooNE Collaboration reports K+ production cross section and rate measurements using high energy daughter muon neutrino scattering data off the SciBar polystyrene (C8H8) target in the SciBooNE detector. The K+ mesons are produced by 8 GeV protons striking a beryllium target in Fermilab Booster Neutrino Beam line (BNB). Using observed neutrino and antineutrino events in SciBooNE, we measure d2σ/dpdΩ = (5.34 ±0.76) mb/(GeV/c x sr) for p + Be =K+ + X at mean K+ energy of 3.9 GeV and angle (with respect to the proton beam direction) of 3.7 degrees, corresponding to the selected K+ sample. Compared to Monte Carlo predictions using previous higher energy K+ production measurements, this measurement, which uses the NUANCE neutrino interaction generator, is consistent with a normalization factor of 0.85 ± 0.12. This agreement is evidence that the extrapolation of the higher energy K+ measurements to an 8 GeV beam energy using Feynman scaling is valid. This measurement reduces the error on the K+ production cross section from 40% to 14%.

Abstract

In this paper we report on the measurement of the rate of inclusive $\pi^0$ production induced by charged-current neutrino interactions in a C$_8$H$_8$ target at a mean energy of 1.3 GeV in the K2K near detector. Out of a sample of 11,606 charged current neutrino interactions, we select 479 $\pi^0$ events with two reconstructed photons. We find that the cross section for the inclusive $\pi^0$ production relative to the charged-current quasi-elastic cross section is $$\frac{\sigma_{CC\pi^0}}{\sigma_{CCQE}}=0.426\pm0.032(stat.)\pm0.035(syst.)$$ The energy dependent cross section ratio is also measured. Abstract

The T2K experiment is a long-baseline neutrino oscillation experiment. Its main goal is to measure the last unknown lepton sector mixing angle {\theta}_{13} by observing {\nu}_e appearance in a {\nu}_{\mu} beam. It also aims to make a precision measurement of the known oscillation parameters, {\Delta}m^{2}_{23} and sin^{2} 2{\theta}_{23}, via {\nu}_{\mu} disappearance studies. Other goals of the experiment include various neutrino cross section measurements and sterile neutrino searches. The experiment uses an intense proton beam generated by the J-PARC accelerator in Tokai, Japan, and is composed of a neutrino beamline, a near detector complex (ND280), and a far detector (Super-Kamiokande) located 295 km away from J-PARC. Abstract

The SciBooNE Collaboration reports inclusive neutral current neutral pion production by a muon neutrino beam on a polystyrene target (C8H8). We obtain (7.7 +/- 0.5(stat) +/- 0.5(sys)) X 10(-2) as the ratio of the neutral current neutral pion production to total charged current cross section; the mean energy of neutrinos producing detected neutral pions is 1.1 GeV. The result agrees with the Rein-Sehgal model implemented in our neutrino interaction simulation program with nuclear effects. The spectrum shape of the pi(0) momentum and angle agree with the model. We also measure the ratio of the neutral current coherent pion production to total charged current cross section to be (0.7 +/- 0.4) X 10(-2)., We acknowledge the Physics Department at Chonnam National University, Dongshin University, and Seoul National University for the loan of parts used in SciBar and the help in the assembly of SciBar. Abstract

The SciBooNE Collaboration reports a measurement of neutral current coherent pi(0) production on carbon by a muon neutrino beam with average energy 0.8 GeV. The separation of coherent from inclusive pi(0) production has been improved by detecting recoil protons from resonant pi(0) production. We measure the ratio of the neutral current coherent pi(0) production to total charged current cross sections to be 1.16 +/- 0.24) x 10(-2). The ratio of charged current coherent pi(+) to neutral current coherent pi(0) production is calculated to be 0.14(-0.28)(+0.30), using our published charged current coherent pion measurement., We acknowledge the Physics Department at Chonnam National University, Dongshin University, and Seoul National University for the loan of parts used in SciBar and the help in the assembly of SciBar. Abstract

Purpose To generate a transgenic (Tg) rabbit model of retinal degeneration and to characterize the pattern of degeneration by using histology and electrophysiology. Methods Rhodopsin Pro347Leu Tg rabbits were generated by BAC transgenesis. Tg rabbits were identified by Southern blot analysis, and the expression levels were measured by quantitative RT-PCR. Retinal histology was examined by light and electron microscopy and immunohistochemistry. Retinal function was assessed by full-field electroretinograms (ERGs). Results Six lines of Tg rabbits were generated, and two lines with higher levels of expression showed rod-dominant progressive retinal degeneration. Retinal histology indicated a marked regional variation in the loss of photoreceptors with the central retina more severely affected than the peripheral retina. The characteristics of the ERGs of transgenic rabbits indicated that the rod components of the ERGs were reduced to only 5% by 48 weeks, whereas the cone components remained at 35% in the wild-type at the same time point. The retinal ultrastructure of Tg rabbits showed a large number of small vesicles that accumulated in the extracellular space of the photoreceptors. Conclusions To the best of the authors' knowledge, this is the first rabbit model of progressive retinal degeneration. Because rabbits have large eyes and are easy to handle and breed, they will provide a useful animal model for the study of the pathophysiology of and new treatments for retinal degeneration.

Abstract

The SciBooNE Collaboration has performed a search for charged current coherent pion production from muon neutrinos scattering on carbon, \nu_\mu ^{12}C \to \mu^- ^{12}C \pi^+, with two distinct data samples. No evidence for coherent pion production is observed. We set 90% confidence level upper limits on the cross section ratio of charged current coherent pion production to the total charged current cross section at 0.67\times 10^{-2} at mean neutrino energy 1.1 GeV and 1.36\times 10^{-2} at mean neutrino energy 2.2 GeV., Comment: 18 pages, 16 figures, Minor revisions to match version accepted for publication in Physical Review D

Abstract

The atmospheric neutrino background for proton decay via p→e + π 0 in ring imaging water Cherenkov detectors is studied with an artificial accelerator neutrino beam for the first time. In total, 3.14 × 10 5 neutrino events corresponding to about 10 megaton-years of atmospheric neutrino interactions were collected by a 1000 ton water Cherenkov detector (KT). The KT charged-current single π 0 production data are well reproduced by simulation programs of neutrino and secondary hadronic interactions used in the Super-Kamiokande (SK) proton decay search. The obtained p→ e + π 0 background rate by the KT data for SK from the atmospheric neutrinos whose energies are below 3 GeV is 1.63 +0.42 -0.33 (stat) +0.45 -0.51 (syst)(megaton-year) -1 . This result is also relevant to possible future, megaton-scale water Cherenkov detectors.

Abstract

Abstract

This document presents the physics case for bringing SciBar, the fully active, finely segmented tracking detector at KEK, to the FNAL Booster Neutrino Beam (BNB) line. This unique opportunity arose with the termination of K2K beam operations in 2005. At that time, the SciBar detector became available for use in other neutrino beam lines, including the BNB, which has been providing neutrinos to the MiniBooNE experiment since late 2002. The physics that can be done with SciBar/BNB can be put into three categories, each involving several measurements. First are neutrino cross section measurements which are interesting in their own right, including analyses of multi-particle final states, with unprecedented statistics. Second are measurements of processes that represent the signal and primary background channels for the upcoming T2K experiment. Third are measurements which improve existing or planned MiniBooNE analyses and the understanding of the BNB, both in neutrino and antineutrino mode. For each of these proposed measurements, the SciBar/BNB combination presents a unique opportunity or will significantly improve upon current or near-future experiments for several reasons. First, the fine granularity of SciBar allows detailed reconstruction of final states not possible with the MiniBooNE detector. Additionally, the BNB neutrino energy spectrum is amore » close match to the expected T2K energy spectrum in a region where cross sections are expected to vary dramatically with energy. As a result, the SciBar/BNB combination will provide cross-section measurements in an energy range complementary to MINERvA and complete the knowledge of neutrino cross sections over the entire energy range of interest to the upcoming off-axis experiments.« less

Abstract

We have analyzed 105 autosomal polymorphic short tandem repeat (STR) loci for nine East and South-eastern Asian populations (two Japanese, five Han Chinese, Thai, and Burmese populations) and a Caucasian population using a multiplex PCR typing system. All the STR loci are genomewide tetranucleotide repeat markers of which the total number of observed alleles and the observed heterozygosity were 756 and 0.743, respectively, for Japanese populations. Phylogenetic analysis for these allele frequency data suggested that the Japanese populations are more closely related with southern Chinese populations than central and/or northern ones. STRUCTURE program analysis revealed the almost clearly divided and accountable population structure at K=2-6, that the two Japanese populations always formed one group separated from the other populations and never belong to different groups at Kor =3. Furthermore, our new allele frequency data for 91 loci were analyzed with those for 52 worldwide populations published by previous studies. Phylogenetic and multidimensional scaling (MDS) analyses indicated that Asian populations with large population size (six Han Chinese, three Japanese, two Southeast Asia) formed one distinct cluster and are closer to each other than other ethnic minorities in east and Southeast Asia. This pattern may be the caviar of comparing populations with greatly differing population sizes when STR loci were analyzed.

Abstract

Retinal ischemia-reperfusion injury induces apoptosis of retinal neurons. The purpose of this study was to examine the association of c-Jun, caspase-1, -2, and -3 immunoreactivities and neuronal apoptosis in the retinal ganglion cell layer (GCL) and to study the effects of intravitreal brain-derived neurotrophic factor (BDNF) on the expression of these gene products in a rat model of retinal ischemia-reperfusion injury.After 60 minutes of ischemia, eyes were enucleated after 3, 6, 12, 24, and 168 hours of reperfusion. The numbers of c-Jun-, caspase-1-, caspase-2-, caspase-3, and TdT-dUTP terminal nick-end labeling (TUNEL)-positive cells in the GCL were counted. Recombinant human BDNF (5 microg) or vehicle was injected intravitreally immediately after reperfusion. At 6, 24, and 168 hours, the numbers of immunoreactive cells in BDNF- and vehicle-treated groups were compared.Expression of c-Jun and caspase-2 was found in dying cells in flat-mounted retinas. The numbers of caspase-1- and caspase-3-positive cells were fewer than c-Jun- or caspase-2-positive cells. Cell death in the retinal GCL was suppressed by an intravitreal injection of BDNF. The numbers of TUNEL- and caspase-2-positive cells were lower in the BDNF-treated group at 6 hours after reperfusion (P0.01). The number of c-Jun-positive cells in the treated retinas was not altered by the treatment.Expression of c-Jun and caspase-2 is associated with neuronal cell apoptosis in the GCL. Suppression of caspase-2 expression may explain the neuroprotective effects of BDNF.

Abstract

The weak nucleon axial-vector form factor for quasielastic interactions is determined using neutrino interaction data from the K2K Scintillating Fiber detector in the neutrino beam at KEK. More than 12 000 events are analyzed, of which half are charged-current quasielastic interactions ${\ensuremath{\nu}}_{\ensuremath{\mu}}n\ensuremath{\rightarrow}{\ensuremath{\mu}}^{\ensuremath{-}}p$ occurring primarily in oxygen nuclei. We use a relativistic Fermi gas model for oxygen and assume the form factor is approximately a dipole with one parameter, the axial-vector mass ${M}_{A}$, and fit to the shape of the distribution of the square of the momentum transfer from the nucleon to the nucleus. Our best fit result for ${M}_{A}=1.20\ifmmode\pm\else\textpm\fi{}0.12\text{ }\text{ }\mathrm{GeV}$. Furthermore, this analysis includes updated vector form factors from recent electron scattering experiments and a discussion of the effects of the nucleon momentum on the shape of the fitted distributions.

Abstract

The decrease in nociceptive threshold has been demonstrated in rats exposed to 24 "C and cold environment (-3 "C) for alternate one hour periods from 10:00-17:00 hours and then kept with cold temperature from 17:00-10:00 hours in the following morning for several days. This model is referred to as the SART (Specific Alternation of Rhythm in Temperature) stress. In the present study, to verify the mechanisms underlying such hyperalgssia, antinociceptive effects of various neuroactive drugs were investigated with the intrathecal injections. The analgesic effects of intrathecal serotonin (5-HT, 30 and 100 nmol/rat), 1-(m-chlorophenyl)-biguanlde hydrochlorlde (mCPBG,10-10Omol/rat, a 5-HT3 agonist) and clonldine (30nmoVrat, an 0~ agonist) in SART stressed rats were significantly greater than in normal rats. (±)-8-hydroxy-2-(di-n-propylamino) tetralin hydrochloride (DPAT, a 5-HT~A agonist) and CGS12066B (a 5-HT,B agonist) produced hyperalgesic action in normal rats. An intrathecal a-methyl 5-HT (a 5-HT2 agonist), phenylephrine (an o¢, agonist), isoproteranol (a I~ agonist) and dopamine had no influence on the nociceptive threshold in both normal and SART stressed rats. On the other hand, the analgesic effects of intrathecal morphine and [D-Ala =, MetS] enkephalinamide (an opioidergic I~ agonist) or [D-Pen~,S]-enkephalin (a ~ agonist) were same degree between SART stressed and normal rats. These results suggest that SART stressed rat exhibit the hyperalgesis resulting from the decreased activltiss of descending monoaminergic pain inhibitory systems, probably mediated by either 5-HT~ serotonergic or o~ noradrenergic receptors in the spinal dorsal horn.

Abstract

Effect of Neurotropin on the delayed type hypersensitivity (DTH) response to sheep red blood cells was examined in mice. Neurotropin suppressed the high DTH responses in ddY and BALB/c mice, while it enhanced the low DTH response in C57BL/6 mice when administered after sensitization. In addition, Neurotropin exerted a suppressive effect on the enhanced DTH response by cyclophosphamide pretreatment in C57BL/6 mice, but had a restorative effect on the suppressed DTH response in ddY mice loaded with restraint stress.

Abstract

IgE-independent mediator release from basophils is considered an important event in inflammation, particularly in nonallergic immediate hypersensitivity and in allergic late-phase reactions. This study demonstrates that after exposure to IL-3, basophils release histamine and leukotrienes in response to the neutrophil-activating peptide NAF/NAP-1. Thus, the sequential action of two pure cytokines can promote basophils mediator release. In the presence of IL-3, NAF/NAP-1 functions like a "histamine-releasing factor" and may therefore not only induce cellular infiltration but also provoke symptoms of hypersensitivity reactions.

Abstract

Macular hole (MH) is a retinal break involving the fovea that causes impaired vision. Although advances in vitreoretinal surgical techniques achieve >90% MH closure rate, refractory cases still exist. For such cases, autologous retinal transplantation is an optional therapy showing good anatomic success, but visual improvement is limited and peripheral visual field defects are inevitable after graft harvesting. Here, using a non-human primate model, we evaluated whether human embryonic stem cell-derived retinal organoid (RO) sheet transplantation can be an effective option for treating MH. After transplantation, MH was successfully closed by continuous filling of the MH space with the RO sheet, resulting in improved visual function, although no host-graft synaptic connections were confirmed. Mild xeno-transplantation rejection was controlled by additional focal steroid injections and rod/cone photoreceptors developed in the graft. Abstract

Retinitis pigmentosa (RP) is the most common inherited retinal dystrophy and a major cause of blindness. RP is caused by several variants of multiple genes, and genetic diagnosis by identifying these variants is important for optimizing treatment and estimating patient prognosis. Next-generation sequencing (NGS), which is currently widely used for diagnosis, is considered useful but is known to have limitations in detecting copy number variations (CNVs). In this study, we re-evaluated CNVs in EYS, the main causative gene of RP, identified via NGS using multiplex ligation-dependent probe amplification (MLPA). CNVs were identified in NGS samples of eight patients. To identify potential CNVs, MLPA was also performed on samples from 42 patients who were undiagnosed by NGS but carried one of the five major pathogenic variants reported in Japanese EYS-RP cases. All suspected CNVs based on NGS data in the eight patients were confirmed via MLPA. CNVs were found in 2 of the 42 NGS-undiagnosed RP cases. Furthermore, results showed that 121 of the 661 patients with RP had EYS as the causative gene, and 8.3% (10/121 patients with EYS-RP) had CNVs. Although NGS using the CNV calling criteria utilized in this study failed to identify CNVs in two cases, no false-positive results were detected. 37. Mid-Term Outcomes of Aortic Valve Repair Without Aortic Root Replacement: A Single-Center Experience.

Abstract

Background Aortic valve (AV) repair is a challenging procedure due to its complexity, lower reproducibility, and steep learning curve. To examine its durability and validity, we investigated mid-term outcomes following AV repair without aortic root replacement. Methods Between March 2007 and May 2018, we retrospectively identified 14 patients who underwent AV repair without aortic root replacement at our institution. We investigated their baseline characteristics and postoperative outcomes, including the reoperation rate due to aortic regurgitation (AR) recurrence. Furthermore, we divided them into two groups: those who required reoperation due to AR recurrence (Group R) and those who did not require reoperation (Group F), and statistically compared them. Results The median age was 52.5 years (IQR: 42.0-60.8), with 11 male patients (78.6%). Eight patients (57.1%) had a bicuspid AV. Five cases (35.7%) underwent reoperation due to AR recurrence during a median follow-up period of 5.5 years. There were no significant differences in baseline characteristics between Group R (n=5, 35.7%) and Group F (n=9, 64.3%), including AR etiology, AV repair procedure, and intraoperative AR grade after the final declamp. All cases in Group R had at least mild to moderate AR on the echocardiogram before discharge. Regarding the AR grade before discharge, Group R had a significantly higher grade than Group F (p = 0.013). Conclusions The indication for AV repair for AR might need to be reassessed due to the considerable mid-term reoperation rate. 38. A case of surgical treatment for recurrence of right ventricular metastasis due to renal cell carcinoma after molecular targeted therapy.

Abstract

Background: Cardiac metastasis including the right ventricle from renal cell carcinoma is rare. No standard treatment for cardiac metastasis and recurrence in renal cell carcinoma has been established., Case Presentation: We present the case of a 61-year-old man who underwent the resection of recurrent right ventricular metastasis caused by renal cell carcinoma following molecular targeted therapy. The first cardiac operation was performed for right ventricular metastasis due to renal cell carcinoma. The patient had a good postoperative course. Two years after the first operation, however, follow-up computed tomography revealed the recurrence of the right ventricular tumor and metastases in both lungs. Molecular targeted therapy was carried out and effectively controlled the lung metastasis but the right ventricular lesion remained unchanged, leading to reoperation. The recurrent right ventricular tumor was completely resected through a redo median sternotomy assisted by cardiopulmonary bypass. The patient had an uneventful postoperative course and was discharged on the 13th postoperative day. Follow-ups at 2 years showed no cardiac recurrence., Conclusion: Surgical intervention was considered useful in managing the recurrence of right ventricular metastasis from renal cell carcinoma after molecular targeted therapy., (© 2024. Abstract

Memory T cells demonstrate superior in vivo persistence and antitumor efficacy. However, methods for manufacturing less differentiated T cells are not yet well-established. Here, we show that producing chimeric antigen receptor (CAR)-T cells using berbamine (BBM), a natural compound found in the Chinese herbal medicine Berberis amurensis, enhances the antitumor efficacy of CAR-T cells. BBM is identified through cell-based screening of chemical compounds using induced pluripotent stem cell-derived T cells, leading to improved viability with a memory T cell phenotype. Transcriptomics and metabolomics using stem cell memory T cells reveal that BBM broadly enhances lipid metabolism. Furthermore, the addition of BBM downregulates the phosphorylation of p38 mitogen-activated protein kinase and enhanced mitochondrial respiration. CD19-CAR-T cells cultured with BBM also extend the survival of leukaemia mouse models due to their superior in vivo persistence. Abstract

Clinical studies using suspensions or sheets of human pluripotent cell-derived retinal pigment epithelial cells (hiPSC-RPE) have been conducted globally for diseases such as age-related macular degeneration. Despite being minimally invasive, cell suspension transplantation faces challenges in targeted cell delivery and frequent cell leakage. Conversely, although the RPE sheet ensures targeted delivery with correct cell polarity, it requires invasive surgery, and graft preparation is time-consuming. We previously reported hiPSC-RPE strips as a form of quick cell aggregate that allows for reliable cell delivery to the target area with minimal invasiveness. In this study, we used a microsecond pulse laser to create a local RPE ablation model in cynomolgus monkey eyes. The hiPSC-RPE strips were transplanted into the RPE-ablated and intact sites. The hiPSC-RPE strip stably survived in all transplanted monkey eyes. The expansion area of the RPE from the engrafted strip was larger at the RPE injury site than at the intact site with no tumorigenic growth. Histological observation showed a monolayer expansion of the transplanted RPE cells with the expression of MERTK apically and collagen type 4 basally. Abstract

This study aimed to investigate how the extent and central/peripheral location of the residual visual field (VF) in patients with late-stage inherited retinal diseases (IRDs) are related to retinal sensitivity detected using full-field stimulus testing (FST). We reviewed the results of Goldmann perimetry and FST from the medical records of patients with IRDs whose VF represents central (within 10°) and/or peripheral islands, or undetectable. In total, 19 patients (19 eyes) were analyzed in this study. The median value of residual VF area was 1.38%. The median values of rod and cone sensitivities were  - 14.9 dB and 7.4 dB, respectively. Patients with only the peripheral island (- 33.9 dB) had better median rod sensitivity than other groups (only central,  - 18.9 dB; both,  - 3.6 dB). VF area significantly correlated with rod sensitivity (r = - 0.943, p = 0.005) in patients with only peripheral island, but not with cone sensitivity. Peripheral VF islands were significant contributors to FST results, especially rod sensitivity. With reduced or loss of central vision, the extent of residual peripheral VF significantly affected rod sensitivity, suggesting that FST can be useful in quantitatively estimating the overall remaining vision in patients with late-stage IRD., (© 2024. Objective: Cystoid macular edema (CME) in retinitis pigmentosa (RP) is an important complication causing visual dysfunction. We investigated the effect of CME on photoreceptors in RP patients with previous or current CME, using an adaptive optics (AO) fundus camera., Methods: We retrospectively observed the CME and ellipsoid zone (EZ) length (average of horizontal and vertical sections) by optical coherence tomography. The density and regularity of the arrangement of photoreceptor cells (Voronoi analysis) were examined at four points around 1.5° from superior to inferior and temporal to nasal. We also performed a multivariate analysis using CME duration, central macular thickness and transversal length of CME., Results: We evaluated 18 patients with previous or current CME (18 eyes; age, 48.7 ± 15.6 years) and 24 patients without previous or current CME (24 eyes; age, 46.0 ± 14.5 years). There were no significant differences in age, logMAR visual acuity, or EZ length. In groups with and without CME, cell density was 11967 ± 3148 and 16239 ± 2935 cells/mm2, and sequence regularity was 85.5 ± 3.4% and 88.5 ± 2.8%, respectively; both parameters were significantly different. The correlation between photoreceptor density and age was more negative in group with CME. The CME group tended toward greater reductions in duration of CME., Conclusion: Complications of CME in RP patients may lead to a decrease in photoreceptor density and regularity. 43. Performance of ChatGPT in Board Examinations for Specialists in the Japanese Ophthalmology Society.

Abstract

We investigated the potential of ChatGPT in the ophthalmological field in the Japanese language using board examinations for specialists in the Japanese Ophthalmology Society. We tested GPT-3.5 and GPT-4-based ChatGPT on five sets of past board examination problems in July 2023. Japanese text was used as the prompt adopting two strategies: zero- and few-shot prompting. We compared the correct answer rate of ChatGPT with that of actual examinees, and the performance characteristics in 10 subspecialties were assessed. ChatGPT-3.5 and ChatGPT-4 correctly answered 112 (22.4%) and 229 (45.8%) out of 500 questions with simple zero-shot prompting, respectively, and ChatGPT-4 correctly answered 231 (46.2%) questions with few-shot prompting. The correct answer rates of ChatGPT-3.5 were approximately two to three times lower than those of the actual examinees for each examination set (p = 0.001). However, the correct answer rates for ChatGPT-4 were close to approximately 70% of those of the examinees. ChatGPT-4 had the highest correct answer rate (71.4% with zero-shot prompting and 61.9% with few-shot prompting) in "blepharoplasty, orbit, and ocular oncology," and the lowest answer rate (30.0% with zero-shot prompting and 23.3% with few-shot prompting) in "pediatric ophthalmology." 44. Detailed Evaluation of Chromatic Pupillometry and Full-Field Stimulus Testing to Assess Ultralow Vision in Retinitis Pigmentosa.

Abstract

Purpose: Novel therapeutic options, such as regenerative medicine and gene therapy, are now emerging as viable treatment options for patients with severe visual impairments, such as retinitis pigmentosa (RP). Gradable assessment of patients' visual function is essential to consider treatment options and to evaluate treatment outcomes; however, evaluation of visual function in patients with advanced low vision is often challenging because of patients' poor and sometimes unpredictable responses. In this study, we attempted to accurately assess visual capabilities and disease stage in patients with RP with a visual acuity (VA) of ≤ 0.01., Design: Retrospective analysis of visual function indicators, including VA, retinal thickness, full-field stimulus testing (FST), and chromatic pupillometry., Subjects: Overall, 43 patients (84 eyes) with advanced RP with a VA of ≤ 0.01 visited Kobe City Eye Hospital from 2019 to 2021., Methods: Hierarchical (multilevel) Bayesian modeling was used to estimate individual eye's pupil response and FST threshold, taking into account the ambiguity and randomness often observed in patients with ultralow vision. Using the estimated ability obtained from each test, the correlation between each test and retinal thickness was further analyzed to make a comprehensive assessment of the data., Main Outcome Measures: Visual acuity, retinal thickness, FST threshold, and pupil diameter change to different light stimuli., Results: Full-field stimulus testing and pupillometry measurements were moderately correlated with VA but exhibited a wide range of values within the same VA groups. Full-field stimulus testing was not correlated with central retinal thickness at counting fingers/hand motion VA range and seemed to reflect overall remaining photoreceptor function, including peripheral retina. Objective: To predict the visual prognosis of cataract surgery in patients with retinitis pigmentosa by measuring ellipsoid zone (EZ) width using spectral-domain optical coherence tomography., Methods: This retrospective study included patients with retinitis pigmentosa who underwent uncomplicated cataract surgery between December 2017 and June 2020. Preoperative best-corrected visual acuity (BCVA) and the best postoperative BCVA during follow-up were collected. EZ width was measured on preoperative cross-sectional optical coherence tomography images along the horizontal/vertical meridian through the fovea., Results: Thirty-eight eyes of 38 patients (22 female; mean [±standard deviation] age, 62.1 ± 11.8 years) were included. The median preoperative logarithm of the minimum angle of resolution BCVA of 0.52 (range, 0.00-3.00) significantly improved to 0.07 (range, -0.18-3.00) after surgery (P < 0.001). On preoperative spectral-domain optical coherence tomography images, the median horizontal, vertical, and average EZ widths were 783 (range, 0-9837), 761 (range, 0-10 250), and 769 (range, 0-10 043) μm, respectively. Postoperative BCVA significantly correlated with the horizontal (r = -0.784, P < 0.001), vertical (r = -0.777, P < 0.001), and average EZ widths (r = -0.777, P < 0.001). The area under the receiver operating characteristic curve for the ability of the horizontal, vertical, and average EZ widths to discriminate eyes with and without postoperative BCVA ≤ 0.3 was 0.971, 0.960, and 0.963, respectively, with best cut-off values of 513, 608, and 515 μm, respectively., Conclusions: EZ width measurement can help predict the visual prognosis of cataract surgery in patients with retinitis pigmentosa. Transplantation of embryonic/induced pluripotent stem cell-derived retina (ESC/iPSC-retina) restores host retinal ganglion cell light responses in end-stage retinal degeneration models with host-graft synapse formation. We studied the immunological features of iPSC-retina transplantation using major histocompatibility complex (MHC)-homozygote monkey iPSC-retinas in monkeys with laser-induced retinal degeneration in MHC-matched and -mismatched transplantation. MHC-mismatched transplantation without immune suppression showed no evident clinical signs of rejection and histologically showed graft maturation without lymphocytic infiltration, although immunological tests using peripheral blood monocytes suggested subclinical rejection in three of four MHC-mismatched monkeys. Although extensive photoreceptor rosette formation was observed on histology, evaluation of functional integration using mouse models such as mouse ESC-retina (C57BL/6) transplanted into rd1(C3H/HeJ, MHC-mismatched model) elicited light responses in the host retinal ganglion cells after transplantation but with less responsiveness than that in rd1-2J mice (C57BL/6, MHC-matched model). This study aimed to evaluate the distribution of retinal pigment epithelium (RPE) melanin in patients with retinitis pigmentosa (RP) using entropy measurements by custom-made polarization-sensitive optical coherence tomography (PS-OCT) images, and compare entropy with the intensity of short-wavelength (SW) and near-infrared (NIR) autofluorescence (AF). We retrospectively reviewed the retinal images, including PS-OCT, SW-AF, and NIR-AF of patients with RP who had a hyperautofluorescent ring on AF. A total of 12 eyes of 12 patients (8 women and 4 men; mean age: 37.9 years) were included. There was a strong positive correlation between entropy value and NIR-AF intensity (r = 0.626, p < 0.001), and there was a very weak negative correlation between entropy value and SW-AF (r = - 0.197, p = 0.001). The mean values of the entropy in the foveal, temporal (2 mm from the fovea), and nasal (2 mm from the fovea) sections were 0.41 (± 0.09), 0.29 (± 0.08), and 0.26 (± 0.08), respectively. The entropy was significantly higher in the foveal section than in the temporal and nasal sections (p = 0.002 and p = 0.003, respectively). There was no significant difference between the entropies values for the temporal and nasal sections (p = 0.157). Age, logMAR best-corrected visual acuity, ellipsoid zone width, and central retinal thickness were not correlated with foveal entropy. 48. One-year outcomes and predictable factors for microhook ab interno trabeculotomy.

Aim: To investigate the one-year outcomes and factors that influence the results of microhook ab interno trabeculotomy (µLOT)., Methods: The medical records of consecutive patients with open angle glaucoma who underwent µLOT (including combination of µLOT and cataract surgery) between February 2018 and July 2019 were retrospectively reviewed. Surgical success was defined as the following: an intraocular pressure (IOP)≤21 mm Hg or IOP≤preoperative IOP with a reduced number of glaucoma eye drops, without additional glaucoma surgery, and assessed using Kaplan-Meier survival analysis. A multivariate Cox proportional-hazards regression model was used to investigate the factors associated with surgical failure., Results: The 59 eyes of 59 patients comprising 28 eyes with primary open angle glaucoma (POAG) and 31 with secondary open angle glaucoma (SOAG) were included. The mean IOP and number of glaucoma eye drops significantly decreased from 25.3±7.2 mm Hg and 3.9±1.1, preoperatively to 16.1±4.4 mm Hg ( P <0.01) and 2.1±1.8 ( P <0.01), respectively, 12mo postoperatively, with a cumulative success rate of 63.1%. The one-year success rate was significantly higher in POAG eyes than in SOAG eyes (80.0% vs 48.0%; P =0.011, log-rank test). Multivariate analyses revealed SOAG [ P =0.017, adjusted hazard ratio (aHR): 3.468, 95%CI: 1.246-9.654] and the postoperative IOP spike (IOP>25 mm Hg within 2wk post-surgery; P <0.001, aHR: 5.382, 95%CI: 2.113-13.707) as independent factors associated with surgical failure., Conclusion: The µLOT is a good treatment option for POAG eyes. Purpose: To compare the accuracy of intraoperative aberrometry (IA) for predicting postoperative refraction between eyes with emmetropia and myopia targets., Patients and Methods: This retrospective analysis included patients with axial myopia (axial length ≥ 25.0 mm) who underwent uncomplicated phacoemulsification cataract surgery with IA to achieve emmetropia (plano to -0.5 D) or intentional myopia (-2.5 D to -5.0 D). Preoperative ocular biometry was performed in all eyes using an IOLMaster. Refractive prediction errors in IA were compared between eyes with emmetropia and myopia targets. Refractive prediction errors in IA for both groups were also compared with those predicted by intraocular lens power calculation formulas including the SRK/T, Holladay 1, Hoffer Q, Holladay 2, Haigis, and Barrett Universal II formulas., Results: Thirty-nine eyes of 39 patients with a target of emmetropia and 22 eyes of 22 patients with a target of intentional myopia were included in the final analysis. The mean numerical error was significantly different from zero (myopic trend) in myopia-targeted eyes (-0.37 ± 0.54 D, one-sample t -test, P = 0.004, 95% confidence interval: -0.61 to -0.14), while it was close to zero in emmetropia-targeted eyes. The mean absolute error was significantly smaller in emmetropia-targeted eyes (0.28 ± 0.27 D) than in myopia-targeted eyes (0.51 ± 0.41 D, P = 0.01). IA was revealed as the most accurate method for predicting postoperative refraction in eyes with emmetropia target, whereas Barrett Universal II formula was found to be the most accurate for eyes with myopia target., Conclusion: In patients with axial myopia, the performance of IA was altered when targeting intentional myopia compared with emmetropia. Abstract

A 67-year-old man had left upper lung cancer with invasion into the descending aorta. He underwent pre-emptive thoracic endovascular aortic repair using a Valiant Navion followed by left lung upper lobectomy with resection of the aortic wall. Because of continuous bleeding, he underwent re-thoracotomy. Since the surgically resected aortic wall was largely cleaved, bleeding around the stent-graft that herniated into the left pleural cavity was observed. Re-thoracic endovascular aortic repair using a GORE TAG was immediately performed to prevent further stent-graft herniation and impending lethal haemorrhage. It may be necessary to consider reinforcement of the resected aortic wall to prevent thoracic endovascular aortic repair-related complications., (© The Author(s) 2021.
