Objective: Immature platelet counts (IPC) may prove useful in guiding platelet transfusion management in preterm neonates. However, the relationship between IPCs and thrombopoietin (Tpo) concentrations has not been evaluated in preterm neonates. Methods: Prospective cohort study in thrombocytopenic (n = 31) and non-thrombocytopenic very low birth weight (VLBW) infants (n = 38), and healthy term neonates (controls; n = 41). Absolute platelet counts (APCs), IPCs, and Tpo concentrations were assessed by a fully-automated hematological analyzer (IPC, APC) and by ELISA (Tpo concentrations) in parallel on day 1 of life (d1), d3, and d7. Results: In healthy term neonates, APCs remained stable between d1 and d3. In non-thrombocytopenic VLBW infants, APCs increased from d1 to d7, while in the thrombocytopenia group, APCs declined from d1 to d3, before they slightly increased again by d7. Median IPCs were similar in healthy term vs. non-thrombocytopenic VLBW infants and remained stable between d1 and d3 (p > 0.05). Notably, IPCs significantly increased between d3 and d7 in both non-thrombocytopenic and thrombocytopenic VLBW infants. However, in thrombocytopenic VLBW infants, IPC values were significantly lower at each time point as compared to non-thrombocytopenic VLBWs (p < 0.001). In each subgroup, Tpo concentrations increased from d1 to d3. The median Tpo concentrations were significantly higher in thrombocytopenic as compared to non-thrombocytopenic VLBW infants at d3 (p = 0.01) and d7 (p = 0.002). Discussion: Term infants, thrombocytopenic, and non-thrombocytopenic preterm infants display similar developmental changes in indices of megakaryopoietic activity. In thrombocytopenic preterm infants, however, the responsive increases in Tpo and immature platelets appear to be developmentally limited.

Einleitung: Dynamische Fluoreszenz-Flowzytometrie Parameter des erweiterten Differentialblutbildes helfen bei der Differentialdiagnostik verschiedener Anämieformen, bei der Unterscheidung von Inflammation und Infektion, bei der Differenzierung zwischen bakteriellen und viralen Infekten und zur Echtzeit- Theragnostik und Steuerung einer anti-infektiven Chemotherapie beim septischen Patienten. Ebenso können diese zellulären Parameter zur Prognoseabschätzung und Mortalitätsprädiktion, sowohl beim Neugeborenen als auch bei erwachsenen Patienten, eingesetzt werden. Methodik: Untersucht wurden Kohorten von Patienten mit verschiedenen Anämieformen im Vergleich zu „Blut-gesunden“ Patienten. Für Ret-He und Delta-He wurden Referenzbereiche der einzelnen Patientenpopulationen ermittelt und kinetische Verläufe im 9-feldrigen Hämaplot dargestellt. Des Weiteren wurden in einer klinischen Studie mit Intensivpatienten der aus mehreren zellulären Parametern bestehende ICIS-Score (Intensive Care Infection Score) erhoben und mit Proteinbiomarkern (CRP und PCT) zur infektionsmedizinischen Diagnostik und Therapiesteuerung verglichen. Abschließend wurde der prädiktive Wert von Erythroblasten-Konzentrationen bei VLBW (very low birth weight) Neugeborenen hinsichtlich Mortalität und Outcome analysiert. Ergebnisse: Die Parameter Ret-He und Delta-He ermöglichten eine Differenzierung verschiedener Anämieformen. Die beiden Marker unterschieden sich signifikant zwischen „Blut-gesunden“ Erwachsenen und Neugeborenen, sowie zwischen Patienten mit Eisenmangelanämie (IDA), chronischer Anämie (ACD), Hämoglobinopathie/Thalassämie und waren geeignet, den Zustand vor Therapiebeginn und unter Therapie zu beschreiben. Der Hämaplot ermöglichte unter anderem das Monitoring von Parameterkinetiken von Anämiepatienten unter laufender Therapie. Durch den ICIS-Score und seine zugrunde liegenden Parameter konnte eine Unterscheidung getroffen werden zwischen Inflammation und Infektion, zwischen bakterieller und viraler Infektion und es konnte die Effektivität einer anti-infektiven Chemotherapie kontrolliert sowie die Prognose abgeschätzt werden. Eine Erythroblastenkonzentration von > 2/nL stellte den besten cut-off Wert zur Mortalitätsprädiktion mit einer Sensitivität von 85% und einer Spezifität von 75% bei Neugeborenen dar. Schlussfolgerungen: Zelluläre Marker des erweiterten Differentialblutbildes eigenen sich für eine schnelle, standardisierte, rund um die Uhr verfügbare Form der Analytik zur Differentialdiagnostik von Anämien, zur Echtzeit- Theragnostik bei SIRS und Sepsis in Intensivpatienten sowie zur Prognoseabschätzung und zur Mortalitätsprädiktion bei Neugeborenen., Background: Dynamic fluorescence-flowzytometric parameters of the extended full blood count help with the differential diagnosis of different types of anemia, distinguish patients with and without infection, differentiate between bacterial and viral infections and particularly serve for realtime theragnostics and guidance of anti-infective chemotherapy in sepsis. Additionally, these cellular parameters can be employed for prognosis assessment and mortality prediction in newborns as well as in adult patients. Methods: Cohorts of patients with different types of anemia as well as „blood- healthy“ newborns and adults were compared to each other. Reference values were established for Ret-He and Delta-He for the different patient populations and kinetics of those parameters were depicted in the 9-field hemaplot for single patients under therapy. In addition, the ICIS Score (Intensive Care Infection Score), consisting of a couple of cellular parameters, was tested in a clinical study and was compared to protein biomarkers (CRP und PCT) for infectious medical diagnostics and therapeutic guidance in ICU patients. Finally, the predictive value of NRBC-concentrations (erythroblasts) was determined for VLBW (very low birth weight) newborns regarding mortality and outcome. Results: By use of the parameters Ret-He and Delta-He differentiating between different types of anemia was possible. Both markers significantly differentiated between „blood-healthy“ adults and newborns, as well as patients suffering from iron deficiency anemia (IDA) from chronic anemia (ACD), hemoglobinopathy/thalassemia and their respective states without or under therapy. The hemaplot allowed for the kinetic tracking of Ret-He and Delta-He of anemic patients under therapy. The ICIS-score and its parameters made it possible to distinguish between inflammation and infection, between bacterial and viral infection, enabled monitoring of the effectivity of an anti-infective chemotherapy as well as assessing the patient‘s prognosis. NRBC-concentrations of > 2/nL proved to be the best cut-off value for mortality prediction with a sensitivity of 85% and a specificity of 75% in newborns. Conclusions: Cellular markers of the extended full blood count are a form of analytics which is fast, standardized and available at a 24/7 basis for the differential diagnosis of anemia, realtime-theragnostics in patients with SIRS and sepsis as well as for prognosis assessment and mortality prediction in newborns.

Die automatisierte Blutbildanalytik durch den Einsatz der Flowzytometrie hat über die letzten Jahrzehnte das manuelle Differentialblutbild abgelöst, wenn auch noch nicht in allen Bereichen vollständig ersetzt. Die Entwicklungsfortschritte, insbesondere in der Fluoreszenz-Flowzytometrie, erlauben die Quantifizierung von Zell-Subgruppen sämtlicher Zellreihen bzw. der Erfassung ihrer physiologischen (Aktivierungs-) Zustände aus geringsten Probenmengen. Der neue dynamische Parameter IPF zur Quantifizierung der unreifen Plättchenpopulation zeigt Potential zur Differenzierung der Pathophysiologie (Publikation 2) einer Thrombozytopenie, ermöglicht die Differentialdiagnose von Erkrankungen, die sonst bloß durch die invasive Maßnahme einer Knochenmarkspunktion möglich gewesen wäre (Publikation 3) und erlaubt ein verbessertes Therapiemonitoring bei Thrombozytentransfusionen (Publikation 1). Unreife Vorstufen von Retikulozyten (IRF) und deren Hämoglobingehalt (Ret-Hb) im Vergleich zu reifen Erythrozyten (delta-Hb) sind nicht nur Marker zur Differentialdiagnose von Anämien, sondern können zum Therapiemonitoring von Intensivpatienten (Spies und Weimann, Manuskript in Vorbereitung) herangezogen werden. Ein diagnostischer Mehrwert bei der Untersuchung einer sehr milden inflammatorischen Situation am Beispiel eines Marathonlaufs (Publikation 5) oder bei dem äußerst komplexen pathobiochemischen Zustand nach Nierentransplantation (Publikation 4) konnte allerdings nicht gezeigt werden. Durch eine Automatisierung von analytischen Prozessen kann eine Beschleunigung, Objektivierung und Standardisierung von Methoden implementiert werden. Publikation 6 zeigte den erfolgreichen Ersatz der manuellen mikroskopischen Methode der semiquantitativen Beschreibung der toxischen Granulation von neutrophilen Granulozyten durch den vollautomatisierten GI-Index mittels Fluoreszenz-Flowzytometrie, einem Parameter, der bereits Eingang in die Routineanalytik des Differentialblutbildes gefunden hat., The automated hematological analysis employing flowcytometry has replaced the manual diffential blood count over the past decades in most fields. The developmental progress especially in the field of fluorescence-flowcytometry allows for the quantification of cellular subgroups of all cell lines in the peripheral blood, respectively determining their physiological state (of activation) from rather little amounts of sample. The new dymanic paramter IPF for quantifying the immature platetlet population shows potential for differentiating the patophysiological state (publication No. 2) of a thrombozytopenia and thus enables to state a differential diagnosis of diseases which otherwise could have only be classified by invasive procedures like bone marrow aspiration (publication No. 3). Even an optimized transfusion regime for administering platelet pools might be possible by using IPF (publication No. 1). Immature precursor cells of reticulocytes (IRF) and their content of hemoglobin (Ret-He) in contrast to mature erythrocytes (delta-Hb) are not only markers for anemia diagnostics, but can be employed for monitoring ICU patients as well. An additional diagnostic value when investigating a model of very mild inflammation (i.e. marathon run, publication No.5) or when analyzing the truly complex pathobiochemic situation after a kidney transplant could not be shown (publication No. 4). By automation of the analytical processes acceleration, objectivization and standardization can be achieved. Publication No. 6 shows the successful substitution of the manual microscopic method describing semiquantitatively the toxic granulation of neutrophils by the fully automated GI-index using the fluorescence-flowcytometry technique. This parameter has meanwhile become part of the routine analytical laboratory sprectrum.
