Public perceptions of general practice and the allocation of funds I WRITE in response to the paper, ‘How much of Australia’s health expenditure is allocated to general practice and primary healthcare?’, by Wright et al. This research concluded that 6.8% of the total healthcare spend in Australia went on non-referred services (general practices). My hypothesis was that GP funding is lower than the general public’s perception. To investigate this, I commissioned Roy Morgan to survey a representative sample of 1000 Australian adults with three questions about their perception of general practice care and funding in 2021. The first question asked, ‘how important do you think GPs (general practitioners, sometimes referred to as family doctors) are for the Australian healthcare system?’. On a Likert scale where 10 is very important, 90.6% of respondents rated GPs as ≥8 out of 10. These results suggest that the public hold general practice in high regard.

Sulfur (S) is an essential macronutrient and important component of the earth’s crust, and its cycling has critical impacts on trace metal mobility, water quality, and human health. Pyrite weathering is the primary pathway by which sulfur enters surface waters. However, biogeochemical cycling of sulfur in soils and the river corridor mediates sulfate exports. In this study, we identified the major forms of sulfur across multiple compartments and scales in a pristine mountainous watershed, including shale bedrock weathering profiles, hillslope soils, and alluvial floodplain sediments, in order to provide insight into biogeochemical sulfur cycling in a hydrologically variable alpine system. X-ray absorption near-edge spectroscopy (XANES) analysis of shale weathering profiles showed clear evidence of pyrite oxidation to sulfate, with large accumulations of intermediate S(0) (20%–53%). Micro-scale XANES showed evidence of reprecipitation of pyrite at fracture surfaces within the permanently saturated zone. Organic sulfur dominated S speciation in shallow hillslope soil and floodplain sediment, with little evidence of reduced inorganic S. However, mackinawite formation, representing active sulfate reduction, was observed in saturated oxbow sediments and saturated weathered shale underlying floodplain sediments. Further evidence of sulfate reduction from aqueous sulfur isotopic analysis was observed in shallow groundwater transects across an Fe-reducing meander, whereas increases in pore water sulfate concentrations implied sulfur oxidation at other locations. The data present an integrated picture of sulfur cycling in a shale-dominated watershed, where riverine sulfate exports are mediated by biological cycling, particularly in redox-stratified and temporally dynamic hyporheic zone sediments.

We photoexcite high-energy holon-doublon pairs as a way to alter the magnetic free energy landscape and resulting phase diagram of the frustrated honeycomb magnet ??-RuCl3. The pair recombination through multimagnon emission is tracked through the time evolution of the magnetooptical response originating from ??-RuCl3???s competing zigzag spin-ordered ground state. A small holon-doublon density suffices to reach a spin-disordered state. The phase transition is described within a dynamical Ginzburg-Landau framework, corroborating the quasistationary nature of the transient phase. Our work suggests a new route to reach a nontrivial spin-disordered state in Kitaev-like magnets.

Op de bijzonder goed bezochte, online NHI-dag van 21 januari 2021 hing er één ding duidelijk in de lucht: enthousiasme over recente ontwikkelingen en de snelheid waarmee die elkaar opvolgen. Maar misschien nog wel belangrijker is de constatering dat we steeds meer gegevens, modellen en scripts met elkaar delen. Water houdt zich niet aan grenzen, dus moet er worden samengewerkt. En dat lukt: NHI is echt een gezamenlijk project van alle (semi-)overheden in het waterdomein.

The COVID-19 pandemic has resulted in multiple changes in the delivery of general practice services. In response to the threat of the pandemic and in order to keep their businesses safe and viable, general practices have rapidly moved to new models of care, embraced Medicare-funded telehealth and responded to uncertain availability of personal protective equipment with innovation. These changes have shown the adaptability of general practice, helped keep patients and practice staff safe, and undoubtedly reduced community transmission and mortality. The pandemic, and the response to it, has emphasised the potential dangers of existing fragmentation within the Australian health system, and is affecting the viability of general practice. These impacts on primary care highlight the need for improved integration of health services, should inform future pandemic planning, and guide the development of Australia's long-term national health plan.

Binnen het Horizon 2020 onderzoeksprogramma IMPREX is een risicobenadering voor droogte ontwikkeld, met als doel beleidsmakers inzicht te geven in het huidige en toekomstige droogterisico en het bepalen van de baten van zoetwatermaatregelen. In de droogterisicobenadering wordt rekening gehouden met de variabiliteit van droogtecondities en de potentiele negatieve gevolgen van droogte voor de maatschappij. Niet de kans op droogte maar de kans op gevolgen wordt bepaald, en leidt tot een droogterisicoprofiel per gebied en gebruiksfunctie. De methode is toegepast in twee voorbeeldgebieden (Berkel en Amsterdam-Rijnkanaal/Noordzeekanaal). In beide voorbeelden week de perceptie van gevolgen van droogte af van de bevindingen. Deels is het effect van droogte kleiner dan verwacht en deels wijkt het effect van maatregelen af van de verwachting. De risicobenadering stelt eisen aan de te gebruiken modellen. Ze moeten voldoende gedetailleerd zijn en het effect van maatregelen kunnen simuleren. Niet voor elke functie zijn de effecten al uit te drukken in monetaire waarden, wat vergelijken van de functies lastig maakt. We bevelen aan in te zetten op doorontwikkeling van effectmodules en deze te valideren aan de droogte van 2018.We constateren dat de stapsgewijze aanpak loont. Er ontstaat een gezamenlijk inzicht in en begrip van de functies die risico lopen en de grootteorde van het risico. De risicobenadering wordt gezien als een geschikte methode om besluiten over waterbeschikbaarheid en –verdeling objectief te onderbouwen.

Item does not contain fulltext, Primitive neuroectodermal tumors of the central nervous system (CNS-PNETs) are highly aggressive, poorly differentiated embryonal tumors occurring predominantly in young children but also affecting adolescents and adults. Herein, we demonstrate that a significant proportion of institutionally diagnosed CNS-PNETs display molecular profiles indistinguishable from those of various other well-defined CNS tumor entities, facilitating diagnosis and appropriate therapy for patients with these tumors. From the remaining fraction of CNS-PNETs, we identify four new CNS tumor entities, each associated with a recurrent genetic alteration and distinct histopathological and clinical features. These new molecular entities, designated "CNS neuroblastoma with FOXR2 activation (CNS NB-FOXR2)," "CNS Ewing sarcoma family tumor with CIC alteration (CNS EFT-CIC)," "CNS high-grade neuroepithelial tumor with MN1 alteration (CNS HGNET-MN1)," and "CNS high-grade neuroepithelial tumor with BCOR alteration (CNS HGNET-BCOR)," will enable meaningful clinical trials and the development of therapeutic strategies for patients affected by poorly differentiated CNS tumors.

The systemic and resistant nature of metastatic neuroblastoma renders it largely incurable with current multimodal treatment. Clinical progression stems mainly from the increasing burden of metastatic colonization. Therapeutically inhibiting the migration-invasion-metastasis cascade would be of great benefit, but the mechanisms driving this cycle are as yet poorly understood. In-depth transcriptome analyses and ChIP-qPCR identified the cell surface glycoprotein, CD9, as a major downstream player and direct target of the recently described GRHL1 tumor suppressor. CD9 is known to block or facilitate cancer cell motility and metastasis dependent upon entity. High-level CD9 expression in primary neuroblastomas correlated with patient survival and established markers for favorable disease. Low-level CD9 expression was an independent risk factor for adverse outcome. MYCN and HDAC5 colocalized to the CD9 promoter and repressed transcription. CD9 expression diminished with progressive tumor development in the TH-MYCN transgenic mouse model for neuroblastoma, and CD9 expression in neuroblastic tumors was far below that in ganglia from wildtype mice. Primary neuroblastomas lacking MYCN amplifications displayed differential CD9 promoter methylation in methyl-CpG-binding domain sequencing analyses, and highlevel methylation was associated with advanced stage disease, supporting epigenetic regulation. Inducing CD9 expression in a SH-EP cell model inhibited migration and invasion in Boyden chamber assays. Enforced CD9 expression in neuroblastoma cells transplanted onto chicken chorioallantoic membranes strongly reduced metastasis to embryonic bone marrow. Combined treatment of neuroblastoma cells with HDAC/ DNA methyltransferase inhibitors synergistically induced CD9 expression despite hypoxic, metabolic or cytotoxic stress. Our results show CD9 is a critical and indirectly druggable suppressor of the invasion-metastasis cycle in neuroblastoma.

Nieuw is dat de meetreeksen van De Bilt voor zowel neerslag als verdamping zijn gecorrigeerd voor de klimaattrend. Hierdoor geven de nieuwe statistieken een beter beeld van het klimaat van nu (rond 2014). Dit beeld bevestigt wat we in de praktijk al ervaren: extreme neerslaggebeurtenissen vinden vaker plaats en de hoeveelheden neerslag liggen hoger (orde 10 tot 15 procent, afhankelijk van seizoen, duur en herhalingstijd). De nieuwe statistieken zijn ook bepaald voor het klimaat rond 2030, 2050 en 2085 op basis van de zogenoemde KNMI’14-klimaatscenario’s.

In de projecten IMPREX en Instrumentarium Afwegingen – Economie wordt gewerkt aan een integrale, kwantitatieve, risicobenadering voor droogte en verzilting, die afwegingen bij het bepalen van maatregelen ondersteunt. De case ARK-NZK is een van de drie casestudies in het project IMPREX, en een van de twee casestudies in het project Instrumentarium Afwegingen – Economie. Hoofddoel van de casestudie ARK-NZK is gedefinieerd als ‘Bijdrage aan de beslissingsondersteunende informatie voor afspraken over de Waterbeschikbaarheid en Slim Water Management (SWM) in de vorm van de kans op droogte en verzilting, de gevolgen van droogte en verzilting en het gecombineerde risico’ (zie Plan van aanpak IMPREX). Uitvoering van de casestudie is gestart met een inventarisatie van bestaande gegevens, modellen, kennis en rapporten. Op 10 mei 2016 is vervolgens met de betrokken waterbeheerders een workshop gehouden. Doel van de workshop was om duidelijk te maken wat de belangrijkste droogterisico’s in het gebied zijn en welke kennisvragen beantwoord moeten worden om een risicobenadering toe te kunnen passen. Dit als basis voor voorliggend actieplan, waarin de kennisvragen worden geprioriteerd en de concrete werkzaamheden in 2016 binnen de casestudie worden beschreven.
