- Abstract
The innate immune system is rapidly activated during myocardial infarction and blockade of extracellular complement system reduces infarct size. Intracellular complement, however, appears to be closely linked to metabolic pathways and its role in ischemia-reperfusion injury is unknown and may be different from complement activation in the circulation. The purpose of the present study was to investigate the role of intracellular complement in isolated, retrogradely buffer-perfused hearts and cardiac cells from adult male wild type mice (WT) and from adult male mice with knockout of complement component 3 (C3KO). Main findings: (i) Intracellular C3 protein was expressed in isolated cardiomyocytes and in whole hearts, (ii) after ischemia-reperfusion injury, C3KO hearts had larger infarct size (32 +/- 9% in C3KO vs. 22 +/- 7% in WT; p=0.008) and impaired post-ischemic relaxation compared to WT hearts, (iii) C3KO cardiomyocytes had lower basal oxidative respiration compared to WT cardiomyocytes, (iv) blocking mTOR decreased Akt phosphorylation in WT, but not in C3KO cardiomyocytes, (v) after ischemia, WT hearts had higher levels of ATP, but lower levels of both reduced and oxidized nicotinamide adenine dinucleotide (NADH and NAD+, respectively) compared to C3KO hearts. Conclusion: intracellular C3 protected the heart against ischemia-reperfusion injury, possibly due to its role in metabolic pathways important for energy production and cell survival.

Background The immunogenicity and safety following standard two-dose SARS-CoV-2 vaccination in patients with immune-mediated inflammatory diseases (IMIDs) are not well characterised, and data on third dose vaccination in this patient group are currently lacking. Methods This prospective, observational cohort study included adult patients on immunosuppressive therapy for Crohn’s disease (CD), ulcerative colitis (UC), rheumatoid arthritis (RA), spondyloarthritis (SpA), psoriatic arthritis (PsA), and healthy controls receiving standard two-dose SARS CoV-2 vaccination. Patients with a weak serologic response ( Results A total of 1641 patients (280 CD, 195 UC, 566 RA, 305 SpA, 295 PsA, median age 52 [IQR 40–63], 899 [55%] women), and 1114 healthy controls (median age 43 [IQR 32–55], 854 [77%] women), were included in the study. After standard SARS-CoV-2 two dose vaccination, 1504 (91%) patients compared to 1096 (98%) healthy controls (p Conclusion Response rate as well as anti-RBD levels were lower in IMID patients than healthy controls following standard vaccination. Third dose vaccination in serologically weak responders was safe and resulted in a response in most patients. Our data facilitate identification of patient groups at risk of an attenuated vaccine response eligible for post-vaccination serological monitoring. The data also support a third vaccine dose following standard SARS-CoV-2 vaccination to weak-responding IMID-patients.

- Abstract
Exposure of rats to hyperoxia before organ harvesting protected their isolated hearts against global ischaemia-reperfusion injury in a previous study. The present study investigates whether hyperoxia influences vasomotor function and regional ischaemia of the heart. Isolated rings of the thoracic aorta were obtained from rats immediately or 24 h after in vivo exposure to 60 min of hyperoxia (>95% O2), and the in vitro dose-response to phenylephrine (PHE), prostaglandin F2alpha (PGF2alpha) and endothelin-1 (ET-1), acetylcholine (Ach) and sodium nitroprusside (SNP) was assessed. Hyperoxia in vivo increased the relaxation of aortic rings to Ach and SNP, while it delayed contraction to PHE. The effect was more evident when the vessels were harvested immediately rather than 24 h after hyperoxic exposure. In separate experiments rat hearts were isolated immediately after hyperoxia, buffer-perfused, and subjected to 30 min of regional ischaemia and reperfused for 120 min. Infarct size was determined by triphenyl tetrazolium chloride staining. Hyperoxia significantly reduced infarct size. In normoxic controls 23.0 +/- 8.3% of the area at risk was infarcted, while in hyperoxic animals infarct size was 14.8 +/- 5.6% of the area at risk (P = 0.012). Exposure of rats to hyperoxia modifies the vasomotor response of isolated aortic rings, and reduces the infarct size of isolated rat heart. These novel aspects of hyperoxic treatment require further studies to explore the potential of its clinical application.

- Abstract
Here we report the generation of monoclonal antibodies (mAb) LOV3 and LOV8 to a 110-130-kDa membrane glycoprotein expressed by rat NK cells. This NK surface molecule was identified by eucaryotic expression cloning as the structural orthologue of the phagocytosis-stimulating receptor for complement factor C1q and mannose-binding lectin on human macrophages, C1qRp. Rat C1qRp is a monomeric type I integral membrane protein consisting of 643 amino acids with an N-terminal lectin-like domain, five epidermal growth factor-like domains, a transmembrane domain and a 45-residue cytoplasmic domain. It is encoded by a single gene on rat chromosome 3q41-q42 and is 67% and 87.5% identical at the amino acid level to human and mouse C1qRp, respectively. Rat C1qRp is expressed by resting and by activated NK cells, on subpopulations of NKR-P1(+) T cells (NK/T cells), dendritic cells, macrophages and granulocytes, but not by B cells or NKR-P1(-) T cells. Expression of this innate immune receptor is therefore not restricted to hematopoietic cells of the myeloid lineage, but is also expressed on subsets of cells of lymphoid origin. 1The effects on pulmonary prostaglandin synthesis of pulmonary hypertension and oedema have been studied.2Seventeen isolated lungs of rabbit, cat and guinea‐pig were perfused with plasma, whole blood or Krebs Ringer solution in a recirculating system.3The venous effluent from the lungs superfused (10 ml/min) a series of smooth muscle tissues sensitive to prostaglandins and thromboxane A2; these were: rat stomach strip, rat colon, chick rectum and rabbit aorta.4Left atrial pressure was increased by between 10 and 30 mmHg for periods of 3 to 59 minutes. Gross alveolar oedema eventually developed in all experiments.5Neither pulmonary hypertension nor subsequent oedema caused release of prostaglandins into the venous effluent.

40. Genes in two MHC class I regions control recognition of a single rat NK cell allodeterminant.

We have previously presented evidence suggesting that the non-classical class I region of the rat MHC, RT1.C, encodes polymorphic molecules which induce the cytolytic activity of alloreactive NK cells. Those studies used target cells from a panel of MHC congenic rat strains possessing recombined portions of the RT1a, RT1l and RT1u MHC haplotypes. We have now examined in addition a set of rat strains bearing MHC haplotypes recombinant between RT1av1 and RT1c, and a more complex picture of the MHC control of rat NK alloreactivity has emerged. The expression of a major NK allodeterminant [the allogeneic lymphocyte cytotoxicity (ALC) determinant 2 or ALC-2, defined operationally using cold-target inhibition assays], appears to be under the control of both the RT1.C and the classical class I RT1.A region. Similarly, the alloreactive repertoires of NK cells from these recombinant strains are influenced by elements encoded within these two MHC class I regions. We present a model in which the classical class I autoantigen RT1.Ac exhibits dominant inhibition of NK cytotoxicity specific for the stimulatory determinant ALC-2 shared by the nonclassical class I molecules RT1.Cav1, RT1.Ca and RT1.Cc, and also prevents the deletion of NK cells of this specificity during their development.

The present study examines the expression and involvement of cAMP-dependent protein kinase (PKA) isozymes in cAMP-induced inhibition of natural killer (NK) cell-mediated cytotoxicity. Rat interleukin-2-activated NK cells express the PKA alpha-isoforms RIalpha, RIIalpha, and Calpha and contain both PKA type I and type II. Prostaglandin E2, forskolin, and cAMP analogs all inhibit NK cell lysis of major histocompatibility complex class I mismatched allogeneic lymphocytes as well as of standard tumor target cells. Specific involvement of PKA in the cAMP-induced inhibition of NK cell cytotoxicity is demonstrated by the ability of a cAMP antagonist, (Rp)-8-Br-adenosine 3',5'-cyclic monophosphorothioate, to reverse the inhibitory effect of complementary cAMP agonist (Sp)-8-Br-adenosine 3',5'-cyclic monophosphorothioate. Furthermore, the use of cAMP analog pairs selective for either PKA isozyme (PKA type I or PKA type II), shows a preferential involvement of the PKA type I isozyme, indicating that PKA type I is necessary and sufficient to completely abolish killer activatory signaling leading to NK cell cytotoxicity. Finally, combined treatment with phorbol ester and ionomycin maintains NK cell cytotoxicity and eliminates the cAMP-mediated inhibition, demonstrating that protein kinase C and Ca2+-dependent events stimulate the cytolytic activity of NK cells at a site distal to the site of cAMP/PKA action.

42. Cloning, functional activities and in vivo tissue distribution of rat NKR-P1+ TCR alpha beta + cells.

We have successfully cloned nine NKR-P1+ TCR alpha beta + cells from PVG rat spleens, utilizing murine macrophage inflammatory protein-1 alpha (MIP-1 alpha) and IL-2. These clones are either double negative (DN, CD4-CD8-), which included clones 3.31, 3.71, 4.19, 4.59 and 4.65, or single positive (SP, CD4+CD8-), which included clones 1.64, 3.8, 3.76 and 3.78. No CD8+ clone was recovered. All nine clones are restricted in terms of their expression of the V beta antigens, since they express V beta 8.2 but not V beta 8.5, V beta 10 or V beta 16. These clones are agranular and they fall to generate NK or LAK activity upon incubation with IL-2, IL-12 or their combination. On the basis of their production of intracellular cytokines they can be divided into three categories: (I) SP clones (1.64, 3.8, 3.76 and 3.78) do not produce IL-2 or IL-4, but produce IFN-gamma and IL-12, and they vary in their production of IL-1, RANTES or tumor necrosis factor (TNF)-alpha; (II) DN clones 4.59 and 4.65 produce IL-1 alpha and IFN-gamma only, and fall to produce other cytokines; and (III) DN clones 3.31, 3.71 and 4.19 produce IL-1 alpha, IL-1 beta, IL-2, IL-12, IFN-gamma, RANTES and TNF-alpha. From all the clones examined only DN clones 3.31 and to a lesser degree 4.19 produce IL-4. In vivo tissue localization of clones 3.8, 3.31 and 4.59 shows that these cells distribute into the liver and bone marrow 24 h post i.v. administration. Their accumulation in the liver and bone marrow along with their ability to secrete various cytokines suggest that these cells may influence the generation, differentiation or apoptosis of immune or hematopoietic cells.

- Abstract
Objective: Simultaneous delivery of antegrade and retrograde cardioplegia may provide a more homogeneous distribution of cardioplegic solution. It may, however, increase myocardial edema and postcardioplegic myocardial injury. The purpose of this study was to compare simultaneous antegrade-retrograde cardioplegia with antegrade cardioplegia. Methods: After 30 minutes of warm, ''unprotected,'' global ischemia, pigs were given warm, continuous blood cardioplegia for 45 minutes (antegrade group, n = 8 and simultaneous antegrade-retrograde group, n = 9). All pigs were weaned from cardiopulmonary bypass 45 to 60 minutes after aortic unclamping. Indices of left ventricular function were measured after another 30 minutes with the conductance catheter technique and pressure-volume loops. Results: Global left ventricular function, evaluated by preload recruitable stroke work, decreased from baseline values of 126 (102 to 150) (mean [90% confidence limits]) (antegrade) and 122 (116 to 127) erg/ml x 10^3 (simultaneous) to 75 (61 to 89) (p = 0.004) and 95 (79 to 112) erg/ml x 10^3 (p = 0.02), respectively. End-diastolic pressure-volume relation increased from 0.25 (0.21 to 0.28) (antegrade) and 0.30 (0.25 to 0.35) mm Hg/ml (simultaneous) to 0.60 (0.41 to 0.79) (p = 0.009) and 0.53 (0.35 to 0.71) mm Hg/ml (p = 0.02), respectively. The time constant of left ventricular pressure relaxation was unchanged. No intergroup difference was observed in preload recruitable stroke work, preload recruitable stroke work area, end-diastolic pressure volume relation, or stiffness constant. Plasma levels of troponin T increased without any difference between groups. Myocardial water content was increased in the simultaneous group (81.1% [80.7% to 81.5%]) versus the antegrade group (80.1% [79.6% to 80.7%], p = 0.01). Conclusion: Despite a small increase in myocardial water content induced by simultaneous blood cardioplegia, no impairment of postcardioplegic cardiac function was observed compared with antegrade cardioplegia. Purpose: Heart failure is a growing problem worldwide. Cardiac remodeling, which features aseptic inflammation, metalloproteinase (MMP) activation and induction of matricellular proteins, contributes to heart failure progression after myocardial infarction (MI). The ShcA gene encodes three isoforms: p46-, p52- and p66ShcA, which are crucial for embryonic development of the heart. The p66ShcA isoform regulates mitochondrial respiration and oxidative stress. p66ShcA deletion protects against acute myocardial ischemia ex vivo. We studied the role of p66ShcA in chronic cardiac ischemia and cardiac remodeling in vivo. We aimed to find whether (a) p66ShcA expression was induced by MI, (b) p66ShcA was involved in cardiac remodeling and (c) to elucidate underlying mechanisms with focus on MMP and inflammation.Materials and Methods: MI was induced in vivo via occlusion of the left descending coronary artery in C57Bl/6 male mice. Cardiac expression of p66ShcA in wild type mice (WT) was evaluated serially postinfarction (n=105). The functional effects of p66ShcA were evaluated in knockout mice (p66KO) compared to WT (n=72). Hearts from WT and p66KO were harvested three days postinfarction to evaluate infarct size, expression of MMP and matricellular proteins. Simultaneously, spleens were collected for splenocytes isolation (n=5 in each group). Interleukins expression in the splenocytes was assessed by flow cytometry. Six weeks after MI, survival, heart function and cardiac fibrosis were evaluated (n=19 per group). Mouse embryonic fibroblasts with p66ShcA knockdown were used to study MMP-2 expression.Results: p66ShcA expression increased transiently during the first weeks postinfarction. p66KO mice had increased survival after MI (p=0,005) and reduced occurrence of heart rupture (p=0,04). Three days postinfarction, cardiac MMP-2 and matricellular proteins, such as connective tissue growth factor and thrombospondin-1, were down-regulated in the hearts of p66KO compared to WT mice. After MI, p66KO splenocytes had a lower expression of IL-17 and IL-4 than WT mice. Six weeks after MI, p66KO mice had attenuated both fibrosis and left ventricular dilatation. At the same time, p66ShcA depletion in vitro attenuated activation of MMP-2.Conclusions: The p66ShcA isoform is involved in adverse cardiac remodeling after myocardial infarction. Deletion of p66ShcA prevented cardiac rupture and improved survival. The underlying mechanism was partly elucidated: p66ShcA activates MMP-2 and regulates expression of IL-4 and IL-17. Purpose: Bone morphogenetic protein-2 (BMP2) is known for its wide role in embryonic heart development, encompassing the differentiation of cardiomyocytes, vascularization, valvulogenesis and connective tissue formation. However, BMP2 function in the adult heart is unknown. Recent work indicates that other BMP family members may be involved in myocardial remodeling. We hypothesized that BMP2 is expressed during ischemic cardiac injury and in heart failure. Furthermore we wanted to study which cells in the heart express BMP2 and what stimuli may induce this expression.Methods: Left ventricular biopsies were sampled from explanted hearts of patients with end-stage dilated cardiomyopathy and ischemic heart disease, from hearts of patients undergoing coronary artery bypass grafting and from donor hearts. Mice were subjected to infarction by permanent coronary artery ligation or sham operated, and hearts were sampled serially thereafter. Other hearts were dissected into ventricles and atria, as well as used for cell isolation and culture. Human umbilical vein endothelial cells (HUVEC) were subjected to mechanical stretch, hypoxia/reoxygenation and oxidative stress by means of hydrogen peroxide. Expression of BMP2 and some of its downstream targets was assessed using qPCR and Western blot.Results: BMP2 mRNA was upregulated in biopsies of coronary artery bypass grafting patients and in infarcted mouse hearts, but not in end-stage heart failure. Examination of its cellular source in untreated heart revealed that BMP2 protein expression was confined to cardiac myocytes, rather than to cardiac fibroblasts or endothelial cells, with the left ventricle displaying higher BMP2 expression than the atria. HUVEC had similar levels of BMP2 expression as cardiomyocytes. To investigate the potential mechanism of BMP2 activation, we subjected HUVEC to different modalities of cellular stress. Mechanical stress, but not hypoxia/reoxygenation or hydrogen peroxide induced BMP2 expression, as well expression of its downstream target genes sprouty-1 and beta-catenin. Conversely, oxidative stress strongly inhibited expression of sprouty-1 and BMP signaling co-factor thrombospondin-1.Conclusion: BMP2 is induced in biopsies from CABG patients and in postinfarction mouse hearts. Activation of BMP2 and its targets was also observed in HUVECs subjected to mechanical stress, but not in hypoxia/reoxygenation or oxidative stress. 50. Postoperative mediastinitis in cardiac surgery — microbiology and pathogenesis

Objective: During 1992–2000, postoperative mediastinitis developed after 126 (1.32%) of 9557 consecutive cardiac surgery procedures. The study was done to describe the variation in clinical characteristics and microbiological etiology in mediastinitis. Methods: The records of 126 cases of postoperative mediastinitis were reviewed. Results: The median time from operation to the development of mediastinitis was 7 days. Sternal dehiscence was seen in 86 patients (68%). Coagulase negative staphylococci (CNS) were isolated in 46% of the cases with a verified microbiological etiology, Staphylococcus aureus in 26% and gram-negative bacteria in 18%. CNS were more frequently isolated in patients with sternal dehiscence (44/80, 55%) than in patients with stable sternum (10/38, 26%) (
P=0.003 ). However, S. aureus was more frequent in patients with stable sternum (18/38, 47%) than in patients with sternal dehiscence (13/80, 16%) (P<0.001 ). High body mass index was associated with coagulase negative staphylococci (P<0.001 ) and with sternal dehiscence (P=0.008 ). Chronic obstructive pulmonary disease was also associated with sternal dehiscence (P<0.001 ) and with coagulase negative staphylococci (P=0.04 ). Patients who had been reoperated before onset of mediastinitis tended to have an increased risk for a gram-negative etiology (32 vs. 15% in patients not reoperated,P=0.06 ). The overall 90-day all cause mortality in patients with mediastinitis was 19%. High age, need for reoperation before mediastinitis, and a long primary operation time was associated with increased mortality (P=0.02 ,P=0.007 andP=0.001 , respectively). No specific bacterial etiology was associated with increased mortality nor was the presence of bacteriemia. Conclusions: Three different types of postoperative mediastinitis can be distinguished: (1) mediastinitis associated with obesity, chronic obstructive pulmonary disease, and sternal dehiscence, typically caused by coagulase negative staphylococci; (2) mediastinitis following peroperative contamination of the mediastinal space, often caused by S. aureus, and (3) mediastinitis mainly caused by spread from concomitant infections in other sites during the postoperative period, often caused by gram negative rods. The proposed classification of mediastinitis into three groups with different pathogenic mechanisms may be useful in understanding which prophylactic counter measures have the potentials to be effective in a given situation.
