Background. Acute adhesive small bowel obstruction (AASBO) is a common surgical emergency requiring immediate interventions. AASBO is a usual indication for both small bowel resection and adhesiolysis. Postoperative adhesions cause 60% of small bowel obstructions.Materials and methods. An analysis of treatment outcomes is presented for 197 acute adhesive small bowel obstruction patients; 63 patients had urgent laparotomy, and 134 were scheduled for laparoscopy. The examination included physical, laboratory, radiological methods (abdominal radiography, ultrasound, CT scan), laparoscopy and intra-abdominal pressure monitoring.Results and discussion. Of 134 patients scheduled for laparoscopic adhesiolysis, only 46.2% had laparoscopy completed, and 53.8% required conversion to laparotomy. The main rationale for conversion were massive adhesions, intraoperative haemodynamic instability, a need for intestinal decompression, as well as rare complications. Laparoscopic operations were reported with the lower vs. laparotomy rates of surgical complications (6.4 vs. 12.69%), mortality (6 vs. 6.3%), shorter hospital stays (6.5 vs. 12 days) and operation times (75 vs. 118 min, respectively).Conclusion. Laparoscopic surgery in acute adhesive small bowel obstruction was feasible in 31.47% patients and in 46.2% — after a diagnostic laparoscopy; however, a thorough patients selection for laparoscopic adhesiolysis is necessary. The first estimated should be the patient’s haemodynamic stability, the severities of condition, adhesions and comorbid cardiorespiratory pathology. Grade I—II adhesions are an indication for laparoscopic surgery.

The OBJECTIVE of the work was to study the incidence of general surgical diseases, their structure and individual issues in general surgical care to the population using the example of the region of the Russian Federation – the Republic of Bashkortostan.METHODS AND MATERIALS. The results of a retrospective analysis of the work of the surgical service of the region, the surgical departments of 54 medical organizations, the statistics of the medical information analytical center of the Ministry of Health of the Republic of Bashkortostan for 2008–2018 are presented. 87 484 inpatient, 71 036 outpatient surgical patients were analyzed.RESULTS. The incidence of general surgical pathology (incidence) was 2302.1 per 100 000 population, taking into account outpatient surgical patients – 4171.5/100 000, which is higher than the incidence of cancer, HIV infections, tuberculosis and is at the level of diseases of the circulatory system (4121/100 000). The incidence in its structure depends on the demographic indicators: in people over 80 years old, it is 2 times higher than in people of 20–55 years old, and in the structure of morbidity in the elderly and senile age, the leading role belongs to acute cholecystitis, acute mesenteric ischemia, and also to rupture aortic aneurysms. The issues of volume (number) of operations per 1 surgeon, the need to emergency general surgery with qualified personnel, the material base, the correspondence of the indicators of the surgical service to the indicators proposed by Lancet Сommission Global Surgery are considered.CONCLUSION. Despite the fact that general surgery is not a priority in the healthcare system, insufficient funding, the indicators of this service for 10 years has improved and, in many areas, corresponds to those in developed countries. It is necessary to pay more attention to financing, equipping general surgical departments, training specialists - general surgeons, creating a separate emergency general surgical care service.

Aim. Current clinical recommendations accentuate current methods for the diagnosis and treatment of irritable bowel syndrome (IBS).Key points. IBS is a functional bowel disorder manifested with recurrent, at least weekly, abdominal pain with the following attributes (any two leastwise): link to defecation, its frequency or stool shape. The symptoms are expected to persist for at minimum three months in a total six-month follow-up. Similar to other functional gastrointestinal (GI) disorders, IBS can be diagnosed basing on the patient symptoms compliance with Rome IV criteria, provided the absence of potentially symptom-causative organic GI diseases. Due to challenging differential diagnosis, IBS can be appropriately established per exclusionem, with pre-examination as follows: general and biochemical blood tests; tissue transglutaminase IgA/IgG antibody tests; thyroid hormones test; faecal occult blood test; hydrogen glucose/ lactulose breath test for bacterial overgrowth; stool test for enteric bacterial pathogens and Clostridium difficile A/B toxins; stool calprotectin test; abdominal ultrasound; OGDS, with biopsy as appropriate; colonoscopy with biopsy. The IBS sequence is typically wavelike, with alternating remissions and exacerbations often triggered by psychoemotional stress. Treatment of IBS patients includes dietary and lifestyle adjustments, various-class drug agents prescription and psychotherapeutic measures.Conclusion. Adherence to clinical recommendations can facilitate timely diagnosis and improve medical aid quality in patients with different clinical IBS variants.

Introduction. Treatment of post-radiation damage to the rectum is a complex medical problem. The search for new methods of treatment for this category of patients based on modern achievements in regenerative surgery is urgent.The aim of the study was to evaluate the immediate and long-term results of treatment of post-radiation lesions of the rectum using microfat grafting.Materials and methods. The study involved 66 patients — women aged 28 to 83 years inclusive with complications of radiation therapy (rectovaginal fistulas, radiation proctitis, radiation ulcers).Results and discussion. After the treatment, there is a tendency to improve the condition. So, if before the procedure the average sizes of fistulas and ulcers were 1.67 ± 0.54 cm in diameter, then after 3 months these indicators were 1.39 ± 0.54, and after 6 months — 1.1 ± 0.56 cm Complete epithelialization of the defect was observed in all patients. The period for which it was possible to achieve complete epithelialization varies from 6 months to 2 years. On average, to achieve complete epithelialization of post-radiation damage to the rectum, most patients required 3 injections of autologous adipose tissue. Also, all patients showed an improvement in elastometry indices over time. As a result of treatment, there is a tendency to improve the quality of life of patients.Conclutions. The study allows us to conclude about the effectiveness of the treatment, as well as that in the long term, the treatment will also be effective, the likelihood of relapse is minimal.

The article provides an analysis of the literature on the frequency and nature of postoperative complications in patients infected with a new coronavirus infection – COVID-19. Risk factors for the development of postoperative complications and mortality are considered; the presence of SARS-CoV-2 is a leading risk factor for these complications and mortality, significant risk factors are also patients age ≥70 years, male gender, cancer surgery, severity of the patient’s condition before surgery (3–5 points on the ASA scale), performance emergency operations, a significant amount of surgical interventions. Postoperative complications in patients with COVID-19 develop in 50 %, the mortality rate is 2 times higher compared with patients without coronavirus infection.

We presents the clinical case of splenosis of the abdominal cavity, simulating carcinomatosis. Radiation methods (ultrasound, computed tomography, positron emission tomography) revealed multiple soft tissue tumors in the right iliac region, pelvis, bed of the removed spleen. The patient underwent a splenectomy 23 years ago for a spleen injury. Laparoscopy was performed, tumor formations in the right iliac region and pelvis were removed. Histological examination confirmed the diagnosis of splenosis.

Introduction. Acute appendicitis takes the first place in incidence among acute surgical disorders of the abdominal cavity organs. While many issues of the diagnosis and treatment strategy of this disease have been studied in detail, the diagnosis of acute appendicitis remains primarily a matter of clinical presentation. National guidelines on acute appendicitis also recommend additional methods for a number of stipulated situations; these methods include the Alvarado score. The key goal of this study is to analyse the results of using the Alvarado score in the diagnosis of acute appendicitis and juxtaposing these results with the laparoscopic and pathomorphological data.Materials and methods. This non-randomised prospective study presents the analysis of examination and treatment of 4,941 patients with acute appendicitis from the two periods — from 2006 to 2010 and from 2011 to 2015 — which is before and after the Alvarado score was introduced into the examination protocol. The likelihood of acute appendicitis according to the Alvarado score is evaluated as follows. A score of one to four is interpreted as appendicitis is unlikely; a score of five or six — acute appendicitis is possible, further dynamic observation is required; a score of seven to ten indicates a most probable acute appendicitis.Results and discussion. The total sensitivity of the scoring system exceeded 90%; however, it did not result in a reduction of the number of diagnostic laparoscopies.Conclusion. The authors see as the upsides of the use of the Alvarado score the improvement of the diagnostic precision and the reduced incidence of catarrhal appendicitis (down to 0.1%) and gangrenous forms of appendicitis (from 14.6% to 8.9%) due to a shorter observation period. The authors explain the failure to achieve the result planned by the healthcare funding costs and the drive to obtain a more precise diagnosis.

This article presents results of surgical treatment of 2963 patients with cholelithiasis. The majority of them (71,8%) had an elective surgery, though 28,2% of patients underwent an emergency operation. A comparative assessment of obligate treatment criteria was made in 2 groups. A first group consisted of 803 patients and it was before introduction of Roman selection criteria used for surgical treatment of patients. A second group included 2963 patients in the period after introduction of selection criteria. Preoperative planning of surgery with evaluation of technical complexity of all stages of intervention with prognosis for possible complications should be the base of reasonable choice of surgical method of treatment of cholelithiasis and safety protection of surgery. The study completed and results obtained allowed decrease of the rate of postoperative complications to 1,4% and lethality - to 0,3%, respectively.

An analysis of the condition of emergency surgery for the patients with acute abdominal diseases was made in the period of 1972–2012. The dynamics of surgical beds provision and surgical personnel were taken into account. The authors investigated the rate of general and postoperative lethality, tendencies and regular occurrences and leading reasons of negative results in given diseases. Reserves of futher decrease of the lethality were considered and it was associated with shortening of hospitalization part later than 24 hours. The future improvement of intensive therapy and resuscitation, more wide application of minimally traumatic techniques of surgical treatment could give better results in this group of patients.

The article presents an analysis of modern methods of the measurement, including classical and original methods and devices developed by the authors. The results of examination and treatment were analyzed in 397 patients with acute obturation intestinal obstruction and acute destructive pancreatitis, who had the abdominal compartment syndrome from I to IV degree. The authors showed that on-time decompressive laparotomy allowed the decrease of lethality from 25% to 15-17,8%.

OBJECTIVE. The research assessed the efficacy of WHO checklist for prevention of avoidable complications in surgery. MATERIALS AND METHODS. The article presents the results of implementation of WHO recommendations (checklist of measures) for prevention of so-called avoidable complications in surgery during performance of 35300 operations in 3-year period. The authors included two additional issues in the checklist about prevention of thromboembolic complications and temperature control in the operating unit. RESULTS. The number of preventable complications decreased in clinic after the implementation of WHO recommendations. The rate of thromboembolic and different postoperative complications also reduced in comparison with other clinic, where these recommendations haven’t been applied. CONCLUSIONS. The introduction of principles of prevention of complications in surgery according to WHO recommendations allowed significant decrease of the number of avoidable complications.

Experimental investigation showed that through short-term after splenectomy the death rate of white rats was 16,25%. Morphological investigation indicates that animals have postinfectious hepatitis and the first stage of pulmonary tissue inflammation. Mesenteric lymphatic nodes as the organs of hemoand immune cytopoiesis appear to be under condition of pronounced functional tension in response to inflammation.

An analysis of application results of complex research methods of diagnostics of intraorganic hepatic and splenic hematomas was made. At the same time, options of these methods were used for determination of prescription of injury. The ultrasound, CT, MR-imaging, videolaparoscopy, angiography, Doppler ultrasonics, impedometry, biochemical, laboratory and cytological study of punctate sample from hematomas were applied for this purpose in 33 patients. According to authors, an evolution of hematomas happened in 3 stages, each of this stage was characterized by specified data associated with investigation results. The staging procedure of hematomas or their evolution allowed setting the prescription of injury.
