Abstract

Differentiated thyroid cancers (DTC) are slow- growing tumors with very low disease-specific mortality rates for local−regional disease (5-y survival: 99.9% for localized disease, 98.3% for regional metastatic disease). However, distant metastatic disease is associated with significantly worse prognosis (5-y survival: 54.9%) (1). Standard-of-care management for DTC includes risk-adapted surgery, postoperative 131I therapy, and thyroid hormone therapy. In uncommon cases of radioiodine-refractory tumors, additional therapy may include reoperative surgical intervention, external radiotherapy, and interventional radiology for treatment of locoregional metastases and multikinase or tyrosine kinase inhibitors for treatment of distant metastatic disease.

Abstract

Introduction Scintigraphic evaluation of the thyroid gland enables determination of the iodine-123 iodide or the Tc-99m-pertechnetate uptake and distribution and remains the most accurate method for the diagnosis and quantification of thyroid autonomy and the detection of ectopic thyroid tissue. In addition, thyroid scintigraphy and radioiodine uptake test are useful to discriminate hyperthyroidism from destructive thyrotoxicosis and iodine-induced hyperthyroidism, respectively. Methods Several radiopharmaceuticals are available to help in differentiating benign from malignant cytologically indeterminate thyroid nodules and for supporting clinical decision-making. This joint practice guideline/procedure standard from the European Association of Nuclear Medicine (EANM) and the Society of Nuclear Medicine and Molecular Imaging (SNMMI) provides recommendations based on the available evidence in the literature. Conclusion The purpose of this practice guideline/procedure standard is to assist imaging specialists and clinicians in recommending, performing, and interpreting the results of thyroid scintigraphy (including positron emission tomography) with various radiopharmaceuticals and radioiodine uptake test in patients with different thyroid diseases.

24. Urinary fibrinopeptide A levels in ischemic heart disease

Abstract

Because acute coronary thrombosis can cause unstable coronary artery disease, fibrinopeptide A, a reliable marker of coagulation activity, may play a role in the evaluation of unstable Ischemic syndromes. A new method of fibrinopeptide A sampling, spot urine normalized to urinary creatinine, was evaluated in patients with stable and unstable angina pectoris and acute myocardial infarction. Serial samples were obtained to characterize the changes in urinary fibrinopeptide A levels over time in patients with Ischemic heart disease.
