1. INVESTIGATION OF AWARENESS OF ROTAVIRUS INFECTION AND VACCINATION IN PARENTS OF INFANTS LESS THAN ONE YEAR-A QUESTIONNAIRE STUDY FROM TURKEY

Abstract

Introduction: Rotavirus infection is one of the most common causes of infantile diarrhea, with significance in developing countries where it accounts for 15–30% of the mortality rate. Vaccination is an effective way to prevent disease and mortality. Therefore, parents' knowledge about Rotavirus infection and vaccination is critical in increasing vaccination rates. We aimed to investigate parental awareness about Rotavirus infection and vaccine, evaluate attitudes towards Rotavirus vaccination, and determine factors affecting acceptance and hesitations about immunization. Methods: This survey included 1856 participants: parents who had a child aged under one year of age. A self-explanatory questionnaire consisting of 13 questions was composed to determine parental awareness and behavior regarding Rotavirus infection and the vaccine. Results: More than half (n=541, 58.30%) of the parents knew that diarrhea could be a fatal disease, less than half (n=354, 38.15%) knew that RV could cause diarrhea. The parental knowledge rates about the Rotavirus infection and the vaccine were positively correlated with parents' education level and monthly family income. Less than half (41.59%) of the parents were found to have vaccinated their children for Rotavirus infection. Vaccination rates increased in parallel with the increase in income and education level of parents. The primary reasons for nonvaccination were lack of knowledge (44.83%) and the fear of possible side effects (14.94%). Conclusion: The present survey demonstrates that parents' knowledge of rotavirus vaccination is not adequate, and the rate of rotavirus vaccination is even low. The present study underlines that health providers are critical insufficient society awareness and acceptance of Rotavirus immunization.

Abstract

Amaç: Düzce Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Yenidoğan Yoğun Bakım Ünitesinde takip edilen prematüre olguların Uluslararası Çocuk Gelişimi İzleme ve Destekleme Rehberi'nin (GİDR) Türk çocuklarında standardize edilmiş formu ile nörogelişim basamaklarının incelenmesi ve demografik özelliklerinin değerlendirilmesidir.Metot: Düzce Üniversitesi Tıp Fakültesi Hastanesi Yenidoğan Yoğun Bakım Ünitesinde Ocak 2015-Aralık 2016 tarihleri arasında takip edilen 37. gestasyon haftası altı doğan prematüre hastalar takip dosyaları ile retrospektif olarak incelendi. Hastalar Hastanemize davet edilerek detaylı nörolojik muayeneleri yapıldı ve nörogelişimsel gelişimleri Uluslararası GİDR ile değerlendirildi. Bütün veriler bilgisayar ortamında SPSS 15.0 (SPSS, Inc., Chicago, Illinois, USA) istatistik programı kullanılarak incelendi. Tüm veriler için öncelikle tanımlayıcı istatistikler uygulandı. GİDR'ye göre kalma üzerine etkili olan klinik ve demografik veriler lojistik regresyon analizi ile değerlendirildi.Bulgular: Çalışmaya 241 prematüre infant dahil edildi. 28-32 hafta arası 24 (%10) (ortalama gebelik haftası 30,35±7,8), 32-37 hafta arası 217 (%90) (ortalama gebelik haftası 34,32±7,8) preterm yenidoğan mevcuttu. Her iki grup arasında GİDR geçme yönünden anlamlı fark saptanmadı. Gebelik haftasının küçük olması, Apgar skorunun 1. ve 5. dakika düşük olması, erkek cinsiyet olmak, sepsis, respiratuar distres sendromu, intrakranial kanama, bronkopulmoner displazi, indirekt hiperbilirubinemi varlığı, venöz kateter kullanımı, sürfaktan kullanımı, nazal CPAP, hood içi ve küvöz içi oksijen kullanımının GİDR'ye göre kalan infantlarda anlamlı olarak daha yüksek olduğunu gözlendi.Sonuç: Preterm yenidoğanlarda nörogelişimin gecikmesinde en çok etkili olan faktörün 6 kat risk artışı ile intrakranial kanama olduğunu; ardından bronkopulmoner displazi (5 kat), sepsis (4 kat) risk artışı yarattığını, respiratuar distres sendromunun (1 kat) risk artışı oluşturduğunu gözlemledik.Anahtar Sözcük: Çocuk Gelişimi İzleme ve Destekleme Rehberi, GİDR, Nörogelişim, Prematür, Yenidoğan. Aim: Investigation of neurodevelopment and demographic findings of premature infants who were followed up in the Intensive Care Unit of Düzce University, Faculty of Medicine, Department of Pediatrics, by the standardized form of the International Guide for Monitoring Child Development (GMCD) on Turkish children.Methods: Premature patients born under the 37th gestational week followed in The Newborn Intensive Care Unit of Düzce University Medical Faculty Hospital between January 2015 and December 2016 were retrospectively investigated through follow-up files. The patients were invited to Our Hospital for detailed neurological examinations and their neurodevelopmental processes were evaluated by the GMCD. All data were evaluated by using SPSS 15.0 (SPSS, Inc., Chicago, Illinois, USA) statistical program and descriptive statistics were primarily applied for all data. Clinical and demographic data affecting the failing at guide were evaluated by logistic regression analysis.Results: 241 infants were included in this study, There were 24 (10%) (mean gestational age 30.35±7,8) preterm newborns between 28-32 weeks of gestation and 217 (90%) (mean gestational age 34.32±7.8) preterm newborns were present between 32-37 weeks, there was no significant difference between the two groups from the aspect of success at GMCD. We have observed that small gestational age, low APGAR score at 5 minutes, male gender, sepsis, intracranial hemorrhages, bronchopulmonary dysplasia, presence of indirect hyperbilirubinemia, use of venous catheter, respiratory distress syndrome, surfactant therapy, nasal CPAP, usage of oxygen inside incubator and oxygen therapy in hood were significantly higher at infants failed at GMCD. Conclusion: We observed that the most effective factors in delaying neurodevelopment at preterm newborns were intracranial hemorrhage with 6-fold increased risk, followed by bronchopulmonary dysplasia (5-fold), sepsis (4-fold) and respiratory distress syndrome (1 fold).Keywords: GMCD, International Guide for Monitoring Child Development, Neurodevelopment, Newborn, Premature. 83
