Autismikirjon häiriöön voi liittyä erityispiirteitä, jotka voivat alentaa kynnystä väkivaltaiselle käyttäytymiselle ja väkivaltarikoksille sekä myös rikoksen uhriksi joutumiselle. Väkivaltarikollisuus ei tutkimusten valossa ole autismikirjon potilailla muuta väestöä yleisempää. Toisaalta vankipopulaatioon ja oikeuspsykiatriseen hoitoon kertyy autismikirjon potilaita jonkin verran enemmän, kuin mikä heidän osuutensa on väestössä. Potilaiden psyykkisen hyvinvoinnin kohentaminen ja rikosten uusimisen vähentäminen edellyttävät nimenomaan tälle potilasryhmälle suunniteltuja interventioita. Suomessa on kehitetty kehityksellisen neuropsykiatrisen diagnoosin saaneiden vankien kuntoutuksen mallia.

Two themes have puzzled the research on developmental and learning disorders for decades. First, some of the risk and protective factors behind developmental challenges are suggested to be shared and some to be specific for a given condition. Second, language-based learning difficulties like dyslexia are suggested to result from or correlate with also nonlinguistic aspects of information processing. In the current study, we investigated how adults with developmental dyslexia and ADHD as well as healthy controls cluster across various dimensions designed to tap the prominent nonlinguistic theories of dyslexia. Participants were 18–55-year-old adults with dyslexia (n = 36), ADHD (n = 22), and controls (n = 35). Nonlinguistic theories investigated with experimental designs included temporal processing impairment, abnormal cerebellar functioning, procedural learning difficulties as well as visual processing and attention deficits. Latent profile analysis (LPA) was used to investigate the emerging groups and patterns of results across these experimental designs. LPA suggested three groups: 1) a large group with average performance in the experimental designs, 2) participants predominantly from the clinical groups but with enhanced conditioning learning, and 3) participants predominantly from the dyslexia group with temporal processing as well as visual processing and attention deficits. Despite the presence of these distinct patterns, participants did not cluster very well based on their original status, nor did the LPA groups differ in their dyslexia or ADHD-related neuropsychological profiles. Remarkably, the LPA groups did differ in their intelligence. These results highlight the continuous and overlapping nature of the observed difficulties and support the multiple deficit model of developmental disorders, which suggests shared risk factors for developmental challenges. It also appears that some of the risk factors suggested by the prominent nonlinguistic theories of dyslexia relate to the general level of functioning in tests of intelligence. peerReviewed

We examined the emotional and psychophysiological underpinnings of social interaction in the context of autism spectrum disorder, more specifically, involving participants diagnosed with Asperger syndrome (AS). We recorded participants’ autonomic nervous system (ANS) activation (electrodermal activity, heart rate, and heart rate variability) and facial muscle activation during conversations in two different types of male dyads: (1) ten dyads where one participant has been diagnosed with AS (AS/NT dyads) and (2) nine dyads where both participants are neurotypical (NT/NT dyads). Afterwards, three independent raters assessed continuously each participant’s affiliative and dominant behaviors during the first and last 10 minutes of the conversations. The relationship between the assessed data and ANS responses was examined. We found that, in the NT/NT dyads, a high level of affiliation displayed by the conversational partner calms down the participant when they are actively dominating the interaction. In contrast,when the participants themselves expressed affiliation, their psychophysiological responses indicated increase in arousal, which suggests that the giving of affiliation is physiologically “hard work.” The affiliation-related ANS responses were similar in those NT participants whose conversational partner had AS, while some differences in facial muscle activation did occur in comparison to NT/NT dyads. In the AS participants, in contrast, a high level of affiliation provided by the conversational partner was associated with increase in arousal, suggesting heightened alertness and stress. As for their own affiliative behavior, the AS participants exhibited similar indicators of alertness and stress as the NT participants, but only when their own level of dominance was low. Our results increase understanding of how individuals with AS experience social interaction at the physiological level, and how this experience differs from that in NT individuals. Moreover, our results confirm and further specify our earlier results, where we proposed that affiliation involves the type of “sharing of the burden” that also reverberates in the participants’ bodies.

Objective. Attention-deficit hyperactivity disorder (ADHD) is a developmental disorder characterized by a long-standing pattern of impulsive behavior, hyperkinesia, and inattention. Psychostimulants, for example, methylphenidate, are the treatment of choice for ADHD both in children, adolescents, and adults. Method. The effect of methylphenidate on sleep structure is not well known. We studied the effect of long-acting methylphenidate on sleep in adult ADHD patients, in a naturalistic treatment setting, using actigraphic and polysomnographic recordings. Results. One of our patients experienced manic episode after starting methylphenidate. A wrist-worn accelerometer recording demonstrated a decrease in the duration and quality of sleep. After discontinuation of methylphenidate treatment, the patient's symptoms subsided and there was no need for hospital admission. Actigraphic recording showed a decrease in the amount and quality of the patient's sleep as triggering factor for the manic symptoms. Conclusions. Disruptions of the sleep-wake cycle are probably important etiologic factors in mood disorders, especially bipolar disorder. The changes in length and quality of sleep described in this case report bear close resemblance to those of patients with a manic episode, although these symptoms were induced by methylphenidate.

Attention deficit hyperactivity disorder (ADHD) is a common psychiatric condition, characterized by the core symptoms of inattention, hyperactivity and impulsivity. In previous studies, ADHD has been associated with psychosocial difficulties in parenting and attachment relationships. However, little is known about the sensitivity and attachment strategies of adults with ADHD and that of their children. This thesis consists of three internationally published peer-reviewed articles (Studies I-III). Study I explored the self-protective strategies of adults with ADHD and their histories of dangers and traumas, as presented in retrospect. In Study I, nine adults with the ADHD diagnosis were interviewed using the modified Adult Attachment Interview (the DMM AAI). Studies II and III examined the self-protective strategies of parents with ADHD and the sensitivity they displayed in dyadic interaction with their children. The parents were interviewed with the DMM AAI. Parental sensitivity was assessed using the CARE-Index. Additionally, Study III explored the self-protective strategies of parents with ADHD as well as those of their children as mediated by parental sensitivity. The self-protective strategies of the children were assessed with the Strange Situation Procedure (SSP) or the Preschool Assessment of Attachment (PAA). In Studies II and III, six parents with the ADHD diagnosis and their children, aged between 7 and 36 months, participated. One parent took part with her both children. In all sub-studies, three subgroups were formed on the basis of risk as indicated by Crittenden’s gradient of transformation of information. Study I showed a variety of the self-protective strategies of adults with ADHD in combination with unresolved traumas and losses. In addition, the respondents described in the AAI triangulation in their family of origin. However, the adults with ADHD were not able to evaluate and analyze the impact of the triangulated family system on their own development. Instead, they blamed themselves for the intersubjective problems in their childhood families and considered the punishments of their parents as legitimate, caused mainly by themselves. Studies II and III confirmed the results of Study I about the variety and complexity of the self-protective strategies of parents with the ADHD diagnosis. Results also indicated that unresolved traumas and losses may decrease these parents’ sensitivity and impair their ability to engage in mutual regulation of arousal and emotion with their children. The parents’ own needs for self-protection impaired their ability to protect their children and decreased the clarity of their communication. The children’s self-protective strategies matched those of their parents in regard to the degree of distortion of information as mediated by parental sensitivity. In conclusion, recognizing the variety of self-protective strategies, disorientation at times modifying, and unresolved traumas interrupting the strategic functioning of the individuals with ADHD, can contribute to the tailoring of individualized psychological treatment. DMM-oriented family functional formulations, based on the assessment of the self-protective strategies of each family member would make possible to plan a treatment adapted to the unique family needs, and also to screen early risk. Aktiivisuuden ja tarkkaavuuden häiriö (ADHD) on yleinen psykiatrinen häiriö, jonka ydinoireita ovat tarkkaamattomuus, yliaktiivisuus ja impulsiivisuus. Aiemmissa tutkimuksissa ADHD on yhdistetty psykososiaalisiin vaikeuksiin vanhemmuudessa ja kiintymyssuhteissa. ADHD diagnoosin saaneiden aikuisten ja heidän lastensa kiintymyssuhdestrategioista sekä ADHD diagnoosin saaneiden aikuisten sensitiivisyydestä vuorovaikutuksessa lastensa kanssa tiedetään kuitenkin hyvin vähän. Väitöskirja koostuu kolmesta vertaisarvioidusta, kansainvälisesti julkaistusta artikkelista (osatutkimukset I-III). Osatutkimus I tarkasteli ADHD diagnoosin saaneiden aikuisten itseä suojaavia kiintymyssuhdestrategioita sekä heidän kokemiaan vaaroja ja traumoja. Osatutkimuksen I aineisto koostui yhdeksän aikuisen modifioidusta Adult Attachment Interview- haastattelusta (DMM-AAI). Osatutkimukset II ja III tarkastelivat ADHD diagnoosin saaneiden vanhempien itseä suojaavia strategioita DMM-AAI- haastattelulla ja tutkivat vanhempien sensitiivisyyttä vuorovaikutuksessa lastensa kanssa CARE-Index menetelmällä. Osatutkimuksessa III kartoitettiin lisäksi tutkimukseen osallistuneiden ADHD diagnoosin saaneiden vanhempien lasten itseä suojaavia strategioita Strange Situation Procedure (SSP) ja Preschool Assessment of Attachment (PAA) menetelmillä. Osatutkimuksiin II ja III osallistui kuusi vanhempaa 7-36 kk ikäisten lastensa kanssa. Yksi vanhemmista osallistui tutkimukseen molempien lastensa kanssa. Jokaisessa osatutkimuksessa tutkittavat erottuivat kolmeksi eriasteisen riskin ryhmäksi Crittendenin kuvaaman tiedonkäsittelyn vääristymisen asteen mukaisesti. Ensimmäinen osatutkimus osoitti, että tutkimukseen osallistuneiden ADHD diagnoosin saaneiden aikuisten itseä suojaavat strategiat ovat hyvin monimuotoisia. AAI-haastatteluissa esiin nousivat myös näiden aikuisten omat käsittelemättömät trauma- ja menetyskokemukset. Lisäksi he kuvasivat lapsuuden perheisiinsä liittynyttä triangulaatiota.Tutkittavat eivät kuitenkaan pystyneet arvioimaan ja analysoimaan sitä, kuinka triangulaatio heidän perhesysteemissään oli vaikuttanut heidän omaan kehitykseensä. Päinvastoin, he syyttivät itseään lapsuudenperheidensä intersubjektiivisista ongelmista ja pitivät kokemiaan rangaistuksia oikeutettuina ja pääasiassa itse aiheutettuina. Toinen ja kolmas osatutkimus vahvistivat ensimmäisessä osatutkimuksessa havaittua tietoa siitä, että tutkimukseen osallistuneiden ADHD diagnoosin saaneiden vanhempien itseä suojaavat strategiat ovat kompleksisia ja monimuotoisia. Tulokset viittasivat myös siihen, että vanhempien omat käsittelemättömät trauma- ja menetyskokemukset saattavat heikentää näiden vanhempien sensitiivisyyttä sekä vaikuttaa vanhempien kykyyn auttaa lapsiaan viritystason ja tunnetilan säätelyssä. Tutkimukseen osallistuneiden vanhempien tarve suojella itseään heikensi heidän kykyään suojella lastaan ja vähensi heidän viestinnän selkeyttään. Lasten itseä suojaavien strategioiden kompleksisuus oli linjassa vanhempien strategioiden kanssa sensitiivisyyden toimiessa välittäjänä. Yhteenvetona todetaan, että ADHD diagnoosin saaneiden henkilöiden itseä suojaavien strategioiden monimuotoisuus sekä disorientaatio strategioita muuntavana ja käsittelemättömät traumakokemukset yksilön strategista toimintaa haittaavina tekijöinä olisi tärkeää huomioida yksilöllisten interventioiden suunnittelussa. DMM-mallin mukainen kaikkien perheenjäsenten itseä suojaavien strategioiden arviointi mahdollistaisi kunkin perheen yksilöllisten tarpeiden ja varhaisten riskien huomioinnin.

The onset of schizophrenia before the age of 18 is commonly categorized as early onset schizophrenia. It accounts for less than 4% of all cases of schizophrenia, the prevalence of which is about 1% worldwide. Premorbid abnormalities seem to be very typical, and prognosis is often poor. Besides the negative and positive symptoms, severe neurocognitive deficits also occur. Schizophrenia and pervasive developmental disorders share a common history. From 1970 onwards these disorders have been regarded as separate conditions. These disorders have different ages of onset as well as developmental courses. Nevertheless, the clinical symptoms as well as the results of genetic and neuroimaging studies overlap. The participants in this thesis were adolescent schizophrenia patients (n = 18; 7 males, 11 females). The mean age of the participants at recruitment was 15.6 years (studies I-III) and at the time of psychological assessment 16.2 years (study IV). Study IV also included 15 adolescents with PDD without psychotic disorder (n = 15; 7 males, 8 females). Their mean age at recruitment was 16.1 years. All participants had a primary IQ over 70. Study I of this thesis assessed the possible comorbidity of early onset schizophrenia and pervasive developmental disorders. We found that a total of 44% of adolescents with schizophrenia had some pervasive developmental disorder already in childhood. However, most of the adolescents had a correct comorbid diagnosis until the onset of psychotic symptoms and the diagnosis of schizophrenia. Study II evaluated the number and nature of catatonic features among adolescents with schizophrenia. A further comparison was made of the nature and numbers of those features between adolescents suffering from schizophrenia alone and those schizophrenia patients with comorbid pervasive developmental disorder. All adolescents with schizophrenia presented many lifetime catatonic features. Adolescents with schizophrenia and comorbid pervasive developmental disorder had an earlier onset of catatonic features, more catatonic features and a greater variety of catatonic features compared to schizophrenia patients without comorbidity. Study III aimed to assess the developmental skills of adolescents with schizophrenia. All of the adolescents with schizophrenia exhibited some developmental delays. Adolescents with schizophrenia and comorbid pervasive developmental disorders exhibited more delays in developmental skills than did those schizophrenia adolescents without comorbidity. Study IV investigated neurocognition and social cognition between adolescents suffering from schizophrenia alone, those schizophrenia patients with comorbid pervasive developmental disorder and adolescents with pervasive developmental disorder only. The profiles of the neurocognitive abilities and disabilities between these three groups differed. The adolescents with schizophrenia and comorbid pervasive developmental disorder did not have more severe problems than the other two groups. The comorbid group displayed a combination of the visual strengths those are typical for PDDs and the deficits in processing speed associated with schizophrenia. These study findings showed that among adolescents with early onset schizophrenia is a subgroup of adolescents with comorbid pervasive developmental disorder. The symptoms of these adolescents seem to be more severe, and this subgroup should be taken into account in psychiatric services and rehabilitation. Skitsofrenian esiintyvyys on maailmanlaajuisesti noin 1 %. Ennen 18 ikävuotta puhkeavaa skitsofreniaa kutsutaan varhain alkavaksi skitsofreniaksi. Sen esiintyvyydeksi on arvioitu alle 4 % kaikista skitsofreniatapauksista. Varhain puhkeavalle skitsofrenialle tyypillisiä piirteitä ovat kehitykselliset poikkeavuudet ennen psykoosioireiden puhkeamista sekä huono ennuste. Negatiivisten ja positiivisten oireiden lisäksi sairauteen liittyy merkittäviä neurokognitiivisia poikkeavuuksia. Skitsofrenialla ja laaja-alaisilla kehityshäiriöillä on pitkä yhteinen historia. Vasta 1970-luvulta lähtien nämä häiriöt on virallisissa diagnoosijärjestelmissä erotettu toisistaan. Oirekuvien puhkeamisikä ja taudin kehityskulku poikkeavat toisistaan, mutta sekä kliinisessä oirekuvassa että geeni- että neurokuvantamistutkimusten tuloksissa esiintyy paljon päällekkäisyyttä. Tämän väitöskirjan tutkimuskohteen muodostivat skitsofreniaan sairastuneet nuoret (n = 18; 7 poikaa, 11 tyttöä), joiden keski-ikä oli 15.6 vuotta (Osajulkaisuissa I-III) ja psykologisen tutkimuksen aikaan 16.2 vuotta (Osajulkaisu IV). Osajulkaisussa IV oli mukana myös 15 nuorta, joilla oli diagnosoitu laaja-alainen kehityshäiriö, mutta ei psykoosisairautta (n = 15; 7 poikaa, 8 tyttöä). Heidän keski-ikänsä oli 16.1 vuotta. Kaikkien tutkittavien primaari älykkyysosamäärä oli yli 70. Väitöskirjan ensimmäisessä osatyössä selvitettiin, mikä on varhain alkavan skitsofrenian ja laaja-alaisten kehityshäiriöiden yhteissairastavuus. Tutkimuksessamme 44%:lla nuorista skitsofreniapotilaista esiintyi samanaikainen laaja-alainen kehityshäiriö. Kuitenkin suurimmalle osalle nuorista kyseinen diagnoosi asetettiin vasta skitsofrenian puhkeamisen yhteydessä. Toinen osatyö selvitti skitsofreniaa sairastavien nuorten katatonisten oireiden määrää ja laatua. Lisäksi vertailtiin katatonisten oireiden määrää ja laatua niillä nuorilla, jotka sairastivat ainoastaan skitsofreniaa ja niillä, jotka sairastivat sekä skitsofreniaa että samanaikaista laaja-alaista kehityshäiriötä. Skitsofreniaan sairastuneilla nuorilla esiintyi runsaasti katatonisia oireita. Nuorilla, joilla oli sekä skitsofrenia että laaja-alainen kehityshäiriö, katatonisia piirteitä esiintyi nuoremmalla iällä, oireita oli enemmän ja ne olivat laaja-alaisempia verrattuna nuoriin, joilla sairastivat pelkästään skitsofreniaa. Kolmas osatyö keskittyi tutkimaan skitsofreniaan sairastuneiden nuorten kehityksellisiä taitoja. Kaikilla skitsofreniaan sairastuneilla nuorilla oli havaittavissa viiveitä kehityksellisissä taidoissa. Ne nuoret, jotka sairastivat sekä skitsofreniaa että samanaikaista laaja-alaista kehityshäiriötä omasivat enemmän viiveitä kehityksellisissä taidoissaan kuin ne nuoret, jotka sairastivat ainoastaan skitsofreniaa. Neljännessä osatyössä tutkimuksen kohteena oli neurokognitio sekä sosiaalinen kognitio. Vertailuryhmät muodostivat nuoret, jotka sairastivat skitsofreniaa, nuoret, jotka sairastivat sekä skitsofreniaa että laaja-alaista kehityshäiriötä sekä nuoret, joilla oli diagnosoitu laaja-alainen kehityshäiriö ilman skitsofreniaa. Ryhmien neurokognitiiviset suoritusprofiilit erosivat toisistaan. Ongelmat eivät olleet laaja-alaisempia niillä nuorilla, jotka sairastivat sekä skitsofreniaa että laaja-alaista kehityshäiriötä. Heidän suoriutumisessaan näkyi laaja-alaisille kehityshäiriöille tyypillinen visuaalinen vahvuus ja toisaalta skitsofreniaan liitetty prosessointinopeuden heikkous. Nämä tulokset osoittivat, että varhain alkavaan skitsofreniaan sairastuneiden nuorten joukossa on alaryhmä, jolla on samanaikainen laaja-alainen kehityshäiriö. Tämän alaryhmän oirekuva näyttäytyy vaikeampana ja tämä ryhmä tulisi huomioida psykiatrisissa hoito- ja kuntoutuspalveluissa.
