2. GLR in Colorectal Cancers: An Easily Accessible Prognostic Marker.

Objectives: Colorectal cancer remains a significant health concern, necessitating reliable prognostic indicators for effective management. This study explores the preoperative prognostic significance of the Glucose/Lymphocyte Ratio (GLR) in colorectal cancers. Methods: The study retrospectively analyzed records of patients who underwent surgery for elective colorectal cancers between January 1, 2013, and December 31, 2021, at the Koşuyolu Training and Research Hospital Gastroenterologic Surgery Department. Demographic, clinicopathological, and follow-up data were comprehensively assessed. A cutoff was established from GLR ratios and patients were divided into two groups for prognosis analysis. Results: The study enrolled 222 eligible patients, examining variables such as age, sex, ASA score, neoadjuvant treatment, lymphovascular and perineural invasion, tumor grade, TNM stage, and GLR. The groups consisted of 128 patients with low GLR and 94 patients with high GLR. Statistical analyses revealed relations between GLR levels (p ≤ 0.001) and various prognostic factors such as age (p = 0.034), Perineural Invasion (PNI) (p = 0.002), tumor grade (p = 0.017), TNM stage (p = 0.003), and surgery time (p = 0.029), individuals with GLR ≥ 3.04 were observed to show higher mortality rates (p = 0.001). Above GLR cutoff point of 3.04 patients showed better overall survival rates. All survival related parameters were related with prognosis in univariant Cox regression tests. In multivariant cox regression tests GLR ≥ 3.04 significantly increased mortality by 2.9 times. (p = 0.003). Conclusion: This study demonstrates that GLR, calculated from preoperative glucose and lymphocyte values serves as an independent prognostic factor in colorectal cancers. The findings suggest potential applications for GLR in survival analyses, with significant associations identified in age, PNI, tumor grade, TNM stage, and surgery time. İskemi reperfüzyon hasarı, karaciğer operasyonları ve nakillerinden sonraki işlev azalması ya da tamamıyla kaybının önemli sebeplerinden biri olarak gösterilmektedir. Sesamin, `sesamum indicum` (susam) bitkisinin tohumunda ve yağında bol miktarda bulunan ve bunlardan saflaştırılabilen lignanlardan biridir.Bölümümüzde planlanan ve Trakya Üniversitesi Deney Hayvanları Araştırma Laboratuvarı'nda gerçekleştirilen çalışmamızda; sesaminin ratlarda karaciğer iskemi reperfüzyon hasarında doku koruyucu ve antioksidan etkilerinin olup olmadığını, serumda ve dokuda biyokimyasal analizle ve histopatolojik olarak araştırmayı amaçladık.Çalışmamızda Trakya Üniversitesi Deney Hayvanları Araştırma Birimi'nde üretilmiş ve standart laboratuvar koşullarında tutulan ağırlığı 180 - 230 gram arasında değişen erişkin 24 adet Wistar Albino dişi rat kullanıldı. Sham grup (n= 6), hepatoduodenal ligamenti ortaya konan ancak klampe edilmeyen, iskemi-reperfüzyon grubu (n= 6): Pringle manevrası uygulanan, 45 dakika iskemi sonrası 3 saat reperfüze edilen; sesamin grubu (n= 6) orogastrik gavaj yoluyla 30 mg/kg sesamin verilen, sonrasında hepatoduodenal ligamenti ortaya konan ancak klampe edilmeyen; iskemi reperfüzyon + sesamin grubu (n= 6) 7 gün süreyle orogastrik gavaj yoluyla 30 mg/kg sesamin verilen, 7. günün sonunda Pringle manevrası uygulanan ve 45 dakika iskemi sonrası 3 saat reperfüze edilen grup. Serum ALT,AST, ALP,GGT,LDH, serum ve karaciğer MPO, TNF-α , TAS; TOS, OSI ve karaciğer MDA düzeyleri ölçüldü, histopatolojik analiz yapıldı ve karşılaştırıldı.Sesaminin karaciğer iskemi reperfüzyon hasarında doku hasarını arttırdığı ancak bunun serbest oksijen radikalleri üzerinden olmadığı görülmektedir. Sesaminin tek başına karaciğere toksik olmadığı bulunmuştur. İskemi reperfüzyon hasarında sesaminin doku koruyucu ve antioksidan etki göstermediği ortaya konmuştur. Ischemia reperfusion injury is one of the major causes of decreased function after liver operations and transplants. Sesamin is one of the main lignans in the sesame seed. The aim of this study was to invest the tissue protective and antioxidant effects of sesamin on liver ischemia reperfusion injury in rats.In this study, 24 adult Wistar Albino female rats which were produced in Experimental Animals Research Laboratory of Trakya University and weighted between 180-230 grams were randomly divided in to four groups: Sham group(n = 6), hepatoduodenal ligament was exposed but not clamped. Ischemia-reperfusion group (n = 6), 45 minutes of Pringle maneuver and 3 hours of reperfusion was performed. Sesamin group (n = 6) was given 30 mg / kg sesamin by orogastric gavage for 7 days, hepatoduodenal ligament was exposed but not clamped. Ischemia reperfusion + sesamine group (n = 6) was given 30 mg / kg sesamin by orogastric gavage for 7 days, followed by 45 minutes of Pringle maneuver and 3 hours of reperfusion. Plasma levels of ALT,AST, ALP,GGT,LDH, plasma and liver tissue levels of MPO, TNF-α , TAS; TOS, OSI and tissue levels of MDA were measured and also histopathological analysis was performed and compared.Plasma ALT, AST, GGT levels were significantly higher in ischemia-reperfusion+ sesamin group but TAS,TOS, OSI levels were not significantly different between groups. Sesamin increases tissue damage in liver ischemia reperfusion injury, but not through free oxygen radicals. Sesamin alone is not toxic to the liver. Sesamin does not have tissue protective and antioxidant effects in ischemia reperfusion injury. 54
