The objective of the study is to determine the antibiotic resistance and biofilm formation of Klebsiella pneumoniae in different surfaces. In this study, 47 K. pneumoniae strains were procured from K.A.P. Viswanatham Government Medical College, Trichy, Tamil Nadu. All the isolates were reconfirmed through biochemical reactions. Maximum resistance was observed against Ampicillin, Cefepime, Cefotaxime, Co-trimoxazole, Aztreonam, and Imipenem. The lowest resistance was noticed against Cefuroxime. Among these 85% strains exhibited multidrug resistance with 13% was carbapenem and 98% ESBL resistant strains. The biofilm formation of all the strains in different surfaces revealed that stainless steel surface found to be adhered high number of cells than in other surfaces. Similarly, the biofilm formation of strains grown with glucose in tryptic soya broth (TSB) enhanced adherence ability. It is concluded that presence of glucose or any sugar substrates enhances the biofilm formation thereby developing high resistance against different antibiotics. This condition is detrimental to human health and causes considerable concern.

The goal of this research is on antibiotic resistance and biofilm formation of Escherichia coli on different surfaces. 37 E.coli isolates were obtained from K.A.P. Viswanatham Government Medical College, Tiruchirappalli, Tamil Nadu, India. Biochemical assays were used to re-confirm all the isolates. Ampicillin, Cefepime, Cefotaxime, Co-trimoxazole, Tetracycline and Levofloxacin showed substantial levels of resistance. Meropenem, Tigecycline, and Colistin showed the least amount of resistance. 75.6% of the E.coli strains were multidrug resistant (MDR). Biofilm formation of E.coli was higher in TSBG than in TSB in all (polystyrene, polypropylene, glass and stainless steel) surfaces. It is evident that the presence of glucose or any sugar substrate promotes biofilm development, resulting in notable antibiotic resistance. This situation is hazardous to human health.

Background: Rats fed diets deficient in choline develop hepatocellular carcinoma. Tumor DNA from these animals is characteristically hypomethylated, suggesting that disruption of the one-carbon metabolism pathway is an underlying mechanism for hepatocarcinogenesis. Prospective studies in humans on circulating choline and other one-carbon metabolites and hepatocellular carcinoma risk have been lacking.Methods: We prospectively examined the association between prediagnostic serum concentrations of one-carbon metabolites including betaine, choline, cystathionine, homocysteine, methionine, 5-methyltetrahydrofolate (5-MTHF), pyridoxal-5-phosphate (PLP, the bioactive form of vitamin B6) and S-adenosylmethionine (SAM), and risk of developing hepatocellular carcinoma based on a nested case–control study of 297 incident cases and 631 matched controls from a cohort of 18,244 men in Shanghai, China. Logistic regression methods were used to calculate ORs and 95% confidence intervals (CI) adjusted for established risk factors for hepatocellular carcinoma.Results: Serum choline and PLP were associated with statistically significant reduced risk of hepatocellular carcinoma, whereas serum cystathionine, methionine, and SAM were associated with increased hepatocellular carcinoma risk (all Ptrend < 0.05). The inverse associations for hepatocellular carcinoma risk with choline and PLP remained statistically significant after adjusting for all potential confounders. The multivariate-adjusted ORs (95% CIs) for the highest versus lowest quintiles of serum choline and PLP were 0.35 (0.16–0.78; P = 0.010) and 0.44 (0.25–0.78; P = 0.005), respectively. There were no associations for hepatocellular carcinoma risk with 5-MTHF, betaine, or homocysteine.Conclusion: The inverse associations between choline and vitamin B6 and the risk of hepatocellular carcinoma development are novel and warrant further investigation.Impact: Identifying new modifiable factors for hepatocellular carcinoma prevention is warranted. In this study, we aimed to fabricate nanostarch (NS) reinforced polyaniline/polyvinyl alcohol (PANI-PVA) bionanocomposite of polyaniline-polyvinyl alcohol-nanostarch for skin tissue engineering application. The microstructure of the bionanocomposite was analyzed using TEM and the particle size ranged from 35 to 37 nm. The swelling degree was increased with the increase in the concentration of nanostarch and displayed a greater hydrophilic surface leading to the increase in the absorption. The degradation index analysis was recorded with an increase in the weight loss of the scaffold which attributed to a strong hydrogen bond network. The bionanocomposites exhibited 70% to 82% of cytocompatibility towards L929 cells during the incubation period of day 1, day 4 and day 7. Hence, the novel bionanocomposite with the features of enhanced water-holding capacity, weight loss percentage and cytocompatibility made it suitable for the prospective use in skin tissue engineering applications. Flying Ad-hoc Networks (FANET) allow for an ad-hoc networking among Unmanned Aerial Vehicles (UAV), have recently gained popularity in a variety of military and non-militant applications. The existing work used the Glowworm Swarm Optimization (GSO) algorithm to create a self-organization depending on clustering technique for FANET. Owing to UAV increased mobility, network topology might vary over time, providing route discovery and maintenance is one of the most difficult tasks. And also, the network throughput is still more worsened by the network congestion. To solve this problem, the proposed work designed an energy efficient clustering and fuzzy-based path selection for FANET. In this work, initially, the clustering is performed using the UAV distance. For efficient communication and energy consumption, optimal selection of Cluster Head (CH) is performed by using Adaptive Mutation with Teaching-Learning-Based Optimization (AMTLBO) algorithm. To improve the optimal selection of CH nodes, the best fitness values are calculated. The fitness function depends on Link capacity, remaining energy and neighbor UAV distance. Next to that, nodes begin communications as well as transmit their information to their CH. Improved Fuzzy-based Routing (IFR) is introduced for improving the route discovery process. The goal is to find routes that have a high level of flying autonomy, minimal mobility, and a higher Received Signal Strength Indicator (RSSI). As a result, the energy usage of network is decreased, as well as the cluster’s lifespan is extended. Finally, an adaptive and reliable congestion detection mechanism is introduced to transmit the packets with congestion free path. The experimental result shows that the proposed AMTLBO system attains higher performance compared to the existing system in terms of energy usage, throughput, delay, overhead and packet delivery ratio.

Due to the workload and lack of a critical mass of trained operational researchers within their ranks, health systems and programmes may not be able to dedicate sufficient time to conducting operational research (OR). Hence, they may need the technical support of operational researchers from research/academic organisations. Additionally, there is a knowledge gap regarding implementing differentiated tuberculosis (TB) care in programme settings. In this 'how we did it' paper, we share our experience of implementing a differentiated TB care model along with an inbuilt OR component in Tamil Nadu, a southern state in India. This was a health system initiative through a collaboration of the State TB cell with the Indian Council of Medical Research institutes and the World Health Organisation country office in India. The learnings are in the form of eleven tips: four broad principles (OR on priority areas and make it a health system initiative, implement simple and holistic ideas, embed OR within routine programme settings, aim for long-term engagement), four related to strategic planning (big team of investigators, joint leadership, decentralised decision-making, working in advance) and three about implementation planning (conducting pilots, smart use of e-tools and operational research publications at frequent intervals). These may act as a guide for other Indian states, high TB burden countries that want to implement differentiated care, and for operational researchers in providing technical assistance for strengthening implementation and conducting OR in health systems and programmes (TB or other health programmes). Following these tips may increase the chances of i) an enriching engagement, ii) policy/practice change, and iii) sustainable implementation.

This process can be difficult to start and to implement effectively. Empowerment includes control over resources physical, human, intellectual and financial) and over ideology (beliefs, valuesand attitudes) It is not merely a feel of greater extrinsic control, but also grows intrinsic capacity, greater self-confidence and an internal transformation of ones consciousness that enables one to overcome external barriers to accessing resources or changing traditional ideology. &nbsp

Background Pain after anterior cruciate ligament reconstruction (ACLR) with autologous hamstring graft can be attributed to both arthroscopic surgery and the graft donor site. This study investigated whether donor site pain control was superior with the addition of either peri-hamstring injection or anterior division obturator nerve block in comparison with adductor canal block (ACB) alone. Methods Patients scheduled to undergo knee arthroscopy with ACLR using a graft from the ipsilateral hamstring were randomised to one of three groups. All patients received ACB and multimodal analgesia. Subjects in Group H received peri-hamstring local anaesthetic injection while subjects in Group O received an anterior division of the obturator nerve block, and subjects in Group C served as a control group (ACB alone). Results In 105 subjects undergoing ACLR, there was no significant difference between groups H, O, and C for the primary outcome of pain on movement as assessed by numerical rating scale (NRS) on knee flexion at 2 h after operation (P=0.11). There was no difference in NRS at any time point in the first 48 h after operation, nor was there a difference in oxycodone consumption between the three groups at 24 h (P=0.2). Worst knee pain was initially at the graft donor site in all three groups, which transitioned to anterior knee pain after 12 h. Conclusions The addition of ultrasound-guided peri-hamstring injection or anterior division of obturator nerve block to ACB did not result in a significant reduction in pain or opioid consumption after ACLR with ipsilateral hamstring graft.
