Abstract

Until recently, it has been generally held that stable angina pectoris (SAP) primarily reflects the presence of epicardial coronary artery stenoses due to atheromatous plaque(s), while acute myocardial infarction (AMI) results from thrombus formation on ruptured plaques. This concept is now challenged, especially by results of the ORBITA and ISCHEMIA trials, which showed that angioplasty/stenting does not substantially relieve SAP symptoms or prevent AMI or death in such patients. These disappointing outcomes serve to redirect attention towards anomalies of small coronary physiology. Recent studies suggest that coronary microvasculature is often both structurally and physiologically abnormal irrespective of the presence or absence of large coronary artery stenoses. Structural remodelling of the coronary microvasculature appears to be induced primarily by inflammation initiated by mast cell, platelet, and neutrophil activation, leading to erosion of the endothelial glycocalyx. This leads to the disruption of laminar flow and the facilitation of endothelial platelet interaction. Glycocalyx shedding has been implicated in the pathophysiology of coronary artery spasm, cardiovascular ageing, AMI, and viral vasculitis. Physiological dysfunction is closely linked to structural remodelling and occurs in most patients with myocardial ischemia, irrespective of the presence or absence of large-vessel stenoses. Dysfunction includes the impairment of platelet and vascular responsiveness to autocidal coronary vasodilators, such as nitric oxide, prostacyclin, and hydrogen sulphide, and predisposes both to coronary vasoconstriction and to a propensity for microthrombus formation. These findings emphasise the need for new directions in medical therapeutics for patients with SAP, as well as a wide range of other cardiovascular disorders. [ABSTRACT FROM AUTHOR]

Abstract

The liabilities and their enforcement, which are carried out by the law institutions in the country, India, in a particular system as compared to the other systems, which would cost fewer resources, latter is said to be more efficient in terms of the economic aspects. The key issue here is to achieve the objective of curbing the breach of laws by way of prevention. The objective achieved by minimal resources calls for an economically better position for the society in terms of the amounts that we pay to run the Nation, which are eventually used by the lawmakers to enact those laws that apply to the society and help in the prevention of crimes or in the other way, enforcement of such laws decreases the percentage of commission of crimes eventually boosting the economy on the whole at the macro level, which will help outreach to every citizen of the country in respect of the economic aspects, for example, the purchasing power, tax rates, GDP, share prices etc. Efficiency in terms of the economic aspects is a major concern that is to be maintained for the society as far as the criminal acts are concerned. The liabilities that are given in terms of the breach of laws or violation of the same, could be either of civil or criminal nature. The balance in the economy can be achieved by curbing the crimes and after all, prevention is better than cure. Justification of the losses is more difficult, but preventing the crimes is an effective and easier remedy. Similarly, the Government needed to take urgent action to ensure the protection of the Nation's BPL population and most vulnerable people in the context of COVID-19 confinement, the steps taken were not sufficient. Here, it is equally true that people of near relationship were found as predators, who try to spoil the future of an innocent child for their illicit and immoral sexual pleasures. In other words, risk from known persons is bigger than the risk from the strangers. There are many reasons for the increasing rate of this despicable act, yet presence of obscene materials, more particularly, child pornographic materials on the internet is one of the most prominent reasons of it. Free and easy availability of Internet facility has boosted this social evil and transformed it into a big industry known as 'Porn Industry'. Abstract

BACKGROUND: Patients on dual antiplatelet therapy following percutaneous coronary intervention often have indications for concurrent oral anticoagulation or triple antithrombotic therapy (TT). Although TT may decrease ischemic complications, it may confer increased bleeding risk. HYPOTHESIS: We hypothesize that the use of ticagrelor in TT is associated with higher risk of complications; accordingly, we sought to determine predictors of complications in patients on TT. METHODS: Patients discharged on TT after percutaneous coronary intervention were followed prospectively for 12 months. The primary endpoint was a composite of ischemic (death, myocardial infarction, stroke) and major bleeding complications or net adverse clinical event (NACE). A major secondary endpoint was BARC (Bleeding Academic Research Consortium) types 2, 3, or 5 bleeding. Outcomes were compared between ticagrelor‐ and clopidogrel‐treated patients. Multivariable analyses were performed to elucidate predictors of complications. RESULTS: Twenty‐seven of 152 patients discharged on TT were on ticagrelor. NACE occurred in 52% of patients and BARC 2, 3, or 5 bleeding occurred in 18%. There was no difference in the primary or secondary outcome between ticagrelor vs clopidogrel subgroup. On logistic regressions, use of TT in patients with acute coronary syndrome (P = 0.002) and bridging in with ticagrelor (P = 0.02) were associated with increased NACE. Low estimated glomerular filtration rate was an independent predictor of bleeding (P = 0.03). CONCLUSIONS: The risk of bleeding and ischemic complications among patients on TT is similar between those on ticagrelor and clopidogrel. However, caution with use of bridging anticoagulation should be taken when using ticagrelor.

BACKGROUND: Takotsubo cardiomyopathy (TTC) is increasingly well‐recognized as a cause of chest‐pain syndromes, especially in aging females. The most common complications of TTC occur in the first 24 hours post onset of symptoms and include shock and/or arrhythmias. HYPOTHESIS: We tested the hypothesis that the severity of early hypotension in TTC reflects the extent of myocardial involvement and dysfunction. METHODS: In 80 consecutive TTC patients, correlates of blood pressure on the day of admission were sought via univariate followed by multivariate analysis.
