Wstęp. Rejestr Kliniczny Fundacji ORKIESTRA był badaniem prospektywnym, wieloośrodkowym, obserwacyjnym, postmarketingowym, w którym badano stosowanie pompy insulinowej z systemem ciągłego monitorowania glikemii (CGM) lub bez CGM [tj. pompy wzbogaconej o system ciągłego monitorowania glikemii (SAP) lub pompy zintegrowanej z systemem ciągłego monitorowania glikemii (SIP)] u kobiet z cukrzycą typu 1 (T1DM). Materiał i metody. Rekrutacja kobiet do badania była prowadzona w 24 ośrodkach w Polsce, w których zbierano dane dotyczące dawkowania insuliny i obserwacji przez okres do 22 miesięcy (tj. do 12 miesięcy przed koncepcją, w trakcie ciąży i 6 tygodni po porodzie). Kobiety, które były już w ciąży, zostały włączone do 16. tygodnia ciąży. Badane wyniki obejmowały stężenie HbA1c przed ciążą i w czasie ciąży oraz poważne działania niepożądane (np. ciężka hipoglikemia, kwasica ketonowa, poronienie i hospitalizacja z powodu jakichkolwiek krwawień lub objawów sugerujących przedwczesny poród). Rejestrowano także rutynowe dane kliniczne, takie jak masa ciała kobiet ciężarnych, wskaźnik masy ciała (BMI) oraz dzienne zużycie insuliny. Urządzenia podające insulinę wykorzystane w badaniu to pompa insulinowa MiniMed™ Paradigm™ REAL-Time z funkcją ciągłego monitorowania glikemii (CGM) (za pomocą sensora MiniMed Sof-sensor™) lub bez CGM oraz MiniMed Paradigm Veo™ z funkcją CGM (za pomocą sensora Enlite™). Wyniki. Rekrutację do badania rozpoczęto w maju 2013 roku, a ostatnia pacjentka ukończyła badanie w sierpniu 2017 roku. Wnioski. Rejestr Kliniczny Fundacji ORKIESTRA pozwoli ocenić wyniki zautomatyzowanej podaży insuliny u kobiet w ciąży z T1DM przy użyciu pomp insulinowych z funkcją ciągłego monitorowania glikemii lub bez takiej funkcji. Background. The ORCHESTRA FOUNDATION Registry study was a prospective, multicenter, observational, post-market study investigating the use of an insulin pump with or without continuous glucose monitoring (CGM) [i.e., sensor-augmented pump (SAP) or sensor-integrated insulin pump (SIP)]; before, during, and after pregnancy, in women with type 1 diabetes mellitus (T1DM). Methods. Study participants enrolled in 24 centers, in Poland, and contributed intake and follow-up data for up to 22 months (i.e., up to 12 months pre-conception, throughout pregnancy, and 6 weeks after delivery). Participants who were already pregnant were enrolled up to the 16th week of pregnancy. Investigated outcomes included HbA1c before and during pregnancy, and serious adverse events (e.g., severe hypoglycemia, diabetic ketoacidosis, miscarriage, and hospitalization due to any bleeding or any symptoms suggesting premature delivery). Routine clinical data including maternal weight, body mass index, and daily insulin use were also recorded. The insulin delivery devices used in the study were the MiniMed™ Paradigm™ REAL-Time insulin pump with CGM (via the MiniMed Sof-sensor™ sensor) or without CGM, and the MiniMed Paradigm Veo™ with CGM (via the Enlite™ sensor). Results. Study enrollment began in May 2013 and the last patient completed the study in August 2017. Conclusions. The ORCHESTRA FOUNDATION Registry study provides an opportunity to assess the effects of automated insulin delivery in pregnant women with T1DM using insulin pumps with or without continuous glucose monitoring.

Background: ARETAEUS 1 study showed that a great majority of patients with type 2 dia­betes mellitus (T2DM) of short duration did not meet all of the treatment goals. Since then the treatment goals in T2DM have been changed. The aim of the ARETAEUS 2-Grupa Study was to assess cardiovascular (CV) risk management and meeting treatment goals in the population of T2DM of more than 10-year duration. Methods: ARETAEUS2-Grupa was a cross-sectional questionnaire-based study conducted in Poland in 2012. Randomly selected physicians recruited 1,740 patients with T2DM diag­nosed more than 10 years before the study. Results: Lipid treatment goals were met respectively: for total cholesterol in 34.5% of all patients, triglycerides in 53.8%, low density lipoprotein cholesterol (LDL-C) in 26.5% and high density lipoprotein cholesterol (HDL-C) in 38.2%. Most of patients with and without coronary artery disease were receiving aspirin (90.3% and 60%, respectively) and statins (84.4% and 67.7%, respectively). The current blood pressure (BP) goal (140/90 mm Hg) was met in 43.5% of pa­tients and the previous goal (< 130/80 mm Hg) in 12.4%. The patients were mainly treated with ≥ 3 antihypertensive drugs. All treatment goals (for HbA1c, BP and LDL-C) were reached only by 8.2% of patients, any two goals by 26.3% of patients, one goal by 39.8% of patients, none by 25.6% of patients. Conclusions: The new less restrictive treatment goals are reached more frequently but still much is to be done in the field of clinical practice guidelines implementation and CV prevention in T2DM population.

Background and aims A healthy diet has been inversely associated with endothelial dysfunction (ED) and low-grade inflammation (LGI). We investigated the association between nutrient consumption and biomarkers of ED and LGI in type 1 diabetes. Methods and results We investigated 491 individuals. Nutrient consumption and lifestyle risk factors were measured in 1989 and 1997. Biomarkers of ED (von Willebrand factor, soluble vascular cell adhesion molecule-1 and soluble endothelial selectin) and LGI (C-reactive protein, interleukin 6 and tumour necrosis factor α) were measured in 1997 and averaged into Z-scores. The nutrient residual method was used to adjust individual nutrient intake for energy intake. Data were analysed with generalised estimation equations. We report increments/decrements in nutrient consumption, averaged over time, per +1 standard deviation (SD) of 1997 ED or LGI Z-scores, after adjustment for sex, age, duration of diabetes, investigation centre, body mass index, energy intake, smoking behaviour, alcohol consumption, and each of the other nutrients. One SD elevation in ED Z-score was associated with a diet lower in fibre [β(95%CI);−0.09(−0.18;−0.004)], polyunsaturated fat [−0.18(−0.31;−0.05)] and vegetable protein [−0.10(−0.20;−0.001)]. For the LGI Z-score results showed associations with fibre [−0.09(−0.17;−0.01)], polyunsaturated fat [−0.14(−0.24;−0.03)] and cholesterol [0.10(0.01; 0.18)]. Conclusion In type 1 diabetes, consumption of less fibre, polyunsaturated fat and vegetable protein, and more cholesterol over the study period was associated with more ED and LGI. Following dietary guidelines in type 1 diabetes may reduce cardiovascular disease risk by favourably affecting ED and LGI.

Background/objectives - Individuals with type 1 diabetes have a high risk of developing cardiovascular diseases, and it has been reported that they consume a high atherogenic diet. We examined how nutrient intake and adherence to current European nutritional recommendations evolved in a large cohort of European individuals with type 1 diabetes over a period of 7 years. Subjects/methods - We analysed data from the EURODIAB Prospective Complications Study, a European multicentre prospective cohort study. Standardized 3-day dietary records were employed in individuals with type 1 diabetes. One thousand one hundred and two patients (553 men, 549 women, baseline age 33 ± 10 years, duration 15 ± 9 years) had complete nutritional data available at baseline and after 7 years. We calculated mean differences in reported nutrients over time and adjusted these for age, gender, HbA1c and BMI with ANOVA models. Results - Compared to baseline, there were minor changes in nutrients. Reported protein (-0.35 % energy (en), fat (-1.07 % en), saturated fat (-0.25 % en) and cholesterol (-7.42 mg/1000 kcal) intakes were lower, whereas carbohydrate (+1.23 % en) and fibre (+0.46 g/1000 kcal) intakes were higher at the 7-year follow-up. European recommendations for adequate nutrient intakes were followed in individuals with type 1 diabetes for protein (76 % at baseline and 78 % at follow-up), moderately for fat (34, 40 %), carbohydrate (34, 41 %) and cholesterol (39, 47 %), but poorly for fibre (1.4, 2.4 %) and saturated fat (11, 13 %). Conclusion - European individuals with type 1 diabetes consume a high atherogenic diet as few patients met recommendations for dietary fibre and saturated fat. This study showed minor changes in dietary nutrients and energy intakes over a period of 7 years. Nutrition education needs particular focus on strategies to increase dietary fibre and reduce saturated fat to exploit their potential benefit.

Mieszanki insulinowe odgrywają ważną rolę w leczeniu cukrzycy typu 2 po wystąpieniu nieskuteczności leków doustnych. Przy rozpoczynaniu leczenia insuliną, po krótkim okresie postępowania skojarzonego lekami doustnymi i insuliną NPH, często konieczne jest przejście do stosowania mieszanek. Akceptowane jest także rozpoczynanie insulinoterapii od stosowania mieszanek dwa razy dziennie. Nie wykazano istotnych różnic w efektach leczenia między mieszankami insulin ludzkich i analogowych. Przy wprowadzeniu mieszanek insulin ludzkich obserwowano poprawę jakości życia. W niektórych sytuacjach bardziej przydatne są mieszanki insulin ludzkich niż analogowych, na przykład przy gastroparezie albo u chorych preferujących dodatkowe przekąski między posiłkami. Premixed insulins have a very important role in the therapy of type 2 diabetes after fail of the oral treatment. In initiation of insulin therapy, after short period of combined therapy with oral drugs and NPH insulin, introduction of premixed insulins is necessary. Often the initiation of insulin therapy with two daily injections of premixed insulins is also accepted. There are no serious differences in effects of therapy between human and analog premixed insulins. After introduction of human premixed insulins the improvement in quality of life was observed. In some circumstances there is superiority of human premixed insulins in comparison to analog premixed insulins, eg. in gastroparesis or in patients preferring snacks between main meals.

W pracy opisano budowę i działanie akarbozy, jak i jej wpływ na glikemię poposiłkową. Akarboza jest ważnym lekiem hamującym powstanie jawnej cukrzycy i zmniejszającym ryzyko sercowo-naczyniowe u osób ze stanem przedcukrzycowym. Zarówno działanie, jak i charakter skutków ubocznych akarbozy jest bardzo zbliżony do leków inkretynowych. Być może mechanizm działania tego leku częściowo wiąże się z osią jelitowo-trzustkową. Głównymi beneficjentami akarbozy są pacjenci z cukrzycą typu 2 z wysoką glikemią poposiłkową i osoby ze stanem przedcukrzycowym. Podsumowując, akarboza może być przepisywana na każdym etapie leczenia cukrzycy typu 2. The structure and mechanism of action of acarbose as well as ist role in normalization of postprandial hiperglycaemia is described. Acarbose is very important drug in reduction of risk of development of overt diabetes and in reduction of cardiovascular risk among persons with prediabetes state. The action as well as character of additional effects of acarbose is very similar to its described by incretins. Perhaps the mechanism of action of acarbose is partially connected with its effect on intestine-pancreatic axis. The beneficients of acarbose are patients with type 2 diabetes with high postprandial glycaemia as well persons with prediabetes. In summary, acarbose can be prescribed in every stage of treatment of type 2 diabetes.

Metformina ma znaczenie na wszystkich etapach leczenia cukrzycy typu 2. Wśród mechanizmów jej działania istotne miejsce zajmuje mechanizm inkretynowy. Działanie inkretynowe metforminy najprawdopodobniej polega na redukcji oporności komórki beta na GLP-1, co jest defektem uwarunkowanym genetycznie. The role of metformin is very important on every stage of therapy of type 2 diabetes. Among mechanismas of its action important is inkretin-like effect. This effect bases on reduction of the resistance of beta cell against GLP-1, what is probably determined genetically.

Neuropatia autonomiczna przewodu pokarmowego jest jednym z poważnych powikłań cukrzycy wpływających na stopień wyrównania choroby. Opóźnienie opróżniania żołądkowego zaburza relacje między absorpcją substratów pokarmowych a czasem działania insuliny. Opóźnienie szczytu działania insuliny, charakterystyczne dla insuliny ludzkiej, w porównaniu z analogami szybkodziałającymi, może być korzystne w odtworzeniu właściwych relacji czasowych między wchłanianiem pokarmu a działaniem hipoglikemicznym insuliny. Gastrointestinal autonomic neuropathy is one of the serious complications of diabetes influencing the degree of compensation of disease. Delay of stomach emptying disregulates relations between absorption of energy fuels and time of insulin action. Some delay of the peak of insulin action characteristic for human insulin in comparison with short acting insulin analogs may be beneficial in restoring proper relation in timing of meal absorption and hypoglycaemic effect of injected insulin.

W algorytmie terapii hiperglikemii, opublikowanym jako konsensus ADA/EASD, akarboza jest wymieniona jako dodatkowy lek. Obecnie ten algorytm jest krytykowany i wymaga modyfikacji. W zaleceniach PTD akarbozę wymienia się na II etapie leczenia. Jest ona bezpieczną i skuteczną opcją w indywidualizacji terapii cukrzycy typu 2. In algorithm of management of hyperglicaemia published as ADA/EASD consensus acarbose is mentioned as additional therapy. This algorithm is now criticized and needs modifications. In Polish recommendations acarbose is located on second step of therapy. It is safe and effective alternative in individualization of type 2 diabetes treatment.

W niniejszej pracy przedstawiono znaczenie glikemii poposiłkowej, zwłaszcza w wywoływaniu powikłań cukrzycy. Wskazano na rolę interwencji terapeutycznej, podkreślając udział analogów insuliny, glinidów, akarbozy i leków osi inkretynowej. W kontroli glikemii poposiłkowej szczególnie ważna jest samokontrola, zwłaszcza w użyciu glukometrów wskazujących na tę formę glikemii i jej związek z rodzajem przyjmowanych posiłków. The importance of postprandial glycaemia especially in generation of long-term complications of diabetes was presented. Among therapeutic possibilities the role of insulin analogs, glinids, acarbose and drugs of so called incretin axis was described. The self-monitoring of glycaemia is very important in monitoring of postprandial glycaemia with special role of glucometers indicating the link between hiperglycaemia and composition of meals.

The prevalence of type 2 diabetes in Poland is estimated at 1.6-3.7%. Type 2 diabetes is diagnosed in about 90-95% of the cases worldwide. The prognosis alarms, that number of type 2 diabetes might reach even 300 million patients in the year 2025. Hospital admissions among diabetic patients exceed three folds admissions in non-diabetic patients. About 70-80% incidences of death in diabetic patients results from the cardio-vascular complications. These decrease life expectancy by 4-10 years. The total direct medical costs of type 2 diabetes in the year 2002 in Poland reached 2.6 billion PLN which constituted 8% of the total health care spending. The epidemic characteristic of the disease justify inclusion of the type 2 diabetes management in the health care policy priorities. The access of innovations in type 2 diabetes treatment in Poland is limited. The treatment goals determined by the Polish Society of Diabetology could only be executed with the use of all the available therapeutic options including also innovative drugs. The optimal glicemic control and maintenance of effectiveness in type 2 diabetes patients still constitutes the main therapeutic concern. Despite the currently used oral anti-diabetics and increasing administration of novel insulin therapies, there is plenty of scope for improvement with the use of innovative drugs acting through the mechanism close to the physiological reactions. The DPP-4 inhibitors constitute new therapeutic class of oral anti-diabetics effective in terms of gilcemic control and well tolerated. Moreover, DPP-4 have neutral influence on the body weight and their use is associated with very low risk of hypogycemy. Therefore DPP-4 give an opportunity to improve type 2 diabetes management, particularly in combination with metformine. As a result of their therapeutic added value, DPP-4 have been included into therapeutic guidelines of Polish Society of Diabetology issued in the year 2009. Results of the pharmacoeconomics evaluations, proved, that use of the DPP-4 (sitagliptine) as ad on therapy to metformine is cost-effective as compare to sulfonylourea addition. The calculated cost per QALY amounted to 6.7 thousand PLN in Polish setting. The increase of funds on innovative anti-diabetic drugs could constitute more profitable strategy than much more costly treatment of the future complications related to type 2 diabetes. Chorobowość cukrzycy typu 2 w Polsce oceniono na 1,6–3,7%. Około 90–95% wszystkich chorych na świecie stanowią pacjenci z cukrzycą typu 2, a prognozy na 2025 rok wskazują, że liczba chorych może sięgnąć 300 mln osób. Chorzy na cukrzycę są 3-krotnie częściej hospitalizowani niż osoby bez cukrzycy. Około 70–80% chorych na cukrzycę umiera wskutek powikłań sercowo-naczyniowych, co prowadzi do skrócenia życia o 4-10 lat. Całkowite bezpośrednie koszty medyczne cukrzycy typu 2 w Polsce w 2002 roku oszacowano na 2,6 mld zł, co stanowiło 8% wydatków poniesionych na opiekę zdrowotną. Epidemia cukrzycy typu 2 uzasadnia umieszczenie profilaktyki i leczenia cukrzycy wśród priorytetów polityki zdrowotnej. W Polsce pacjenci mają ograniczony dostęp do nowoczesnych terapii i procedur stosowanych w leczeniu cukrzycy. Cele leczenia cukrzycy określone przez Polskie Towarzystwo Diabetologiczne (PTD) mogą być realizowane tylko na podstawie wykorzystania wszystkich dostępnych metod terapeutycznych, w tym innowacyjnych. Ze względu na naturalny przebieg choroby uzyskanie oraz utrzymanie optymalnej kontroli glikemii nadal pozostaje trudnym wyzwaniem. Mimo możliwości stosowania wielu doustnych leków i nowych schematów insulinoterapii, zwraca się uwagę na nowe klasy preparatów przeciwcukrzycowych, jak najbardziej zbliżonych w swym działaniu do fizjologicznego ideału. Inhibitory DPP-IV należą do najnowszych leków doustnych stosowanych w leczeniu cukrzycy typu 2, skutecznie obniżających glikemię, przy dobrej tolerancji, neutralnym wpływie na masę ciała oraz bardzo małym ryzyku wystąpienia hipoglikemii. Stanowią jedną z najbardziej atrakcyjnych metod poprawy kontroli cukrzycy typu 2, zwłaszcza w skojarzeniu z metforminą, dlatego znalazły swoje miejsce w zaleceniach klinicznych PTD na 2009 rok. Wyniki analiz farmakoekonomicznych wskazują również, że dołączenie inhibitora DPP-4 (sitagliptyny) do metforminy jest efektywną kosztowo opcją wobec stosowania pochodnych sulfonylomocznika, przy koszcie 6,7 tys. zł za jednostkę QALY. Nakłady na poprawę kontroli cukrzycy typu 2 przy zastosowaniu innowacyjnych leków mogą stanowić bardziej opłacalną strategię niż znacznie bardziej kosztowna terapia odległych powikłań.

W niniejszym artykule podsumowano założenia, projekt i wyniki badania ADVANCE - jak dotąd największej próby dotyczącej leczenia cukrzycy typu 2. Miało ono dwa główne ramiona badawcze - jedno na temat oceny efektów kontroli ciśnienia tętniczego w leczeniu opartym na stosowaniu Noliprelu (rezultaty tej części badania zaprezentowano w 2007 roku), drugie, szczegółowo opisane w tej pracy, odnoszące się do oceny związku między intensywną kontrolą glikemii (HbA1c £ 6,5%) opartą na Diaprelu MR a częstością mikro- i makronaczyniowych punktów końcowych (wyniki ogłoszono 6 czerwca 2008 r. na Kongresie ADA oraz opublikowano w NEJM). Glikemiczne ramię badania zaprojektowano jako prospektywne, randomizowane otwarte, z zaślepionymi punktami końcowymi. Pacjentów przydzielano losowo do jednej z dwóch grup badawczych. W pierwszej grupie stosowano intensywną wieloczynnikową terapię opartą na Diaprelu MR, mającą na celu osiągnięcie stężenia HbA1c wynoszącego 6,5% lub mniej. W drugiej grupie pacjentów leczono standardowo, zgodnie z obowiązującymi aktualnie regułami opieki diabetologicznej. Po średnio 5-letniej obserwacji wartość HbA1c w grupie intensywnie leczonej była istotnie niższa niż w grupie poddanej konwencjonalnej terapii (odpowiednio: 6,5% vs. 7,3%; p < 0,001). W grupie leczonej intensywnie (HbA1c £ 6,5%) 70% chorych przyjmowało Diaprel MR w dawce 120 mg (4 tabl.). Intensywne leczenie cukrzycy, oparte na stosowaniu preparatu Diaprel MR, wiązało się z 10-procentowym zmniejszeniem względnego ryzyka rozwoju wszystkich naczyniowych powikłań cukrzycy (p = = 0,01), w tym z redukcją ryzyka rozwoju powikłań mikronaczyniowych o 14% (p = 0,01). Intensywna terapia cukrzycy okazała się szczególnie efektywna w prewencji pojawienia się/progresji nefropatii cukrzycowej (zmniejszenie ryzyka o 21%; p = 0,006), w tym prewencji rozwoju makroalbuminurii (zmniejszenie ryzyka o 30%; p < 0,001). W przeciwieństwie do badania ACCORD, w badaniu ADVANCE wykazano pozytywny trend w redukcji śmiertelności z przyczyn sercowo-naczyniowych (12%) w grupie intensywnie leczonej, mimo że uzyskano porównywalne wartości HbA1c. The article summarizes rationale, design and results of ADVANCE - the biggest ever study on type 2 diabetes. The study had two treatment arms, one aimed on control of blood pressure with the usage of Noliprel as basic drug (this arm of the study has already been summarized and the results were published in 2007), the other, discussed in details in this article, aimed on assesment of the relationship between tight glicemic control, as well as micro- and macrovascular outcomes (the results were presented on June 6, 2008 at the ADA congress and published in NEJM). The glycemic arm of the study was designed as Prospective Randomized Open with Blinded End-points (PROBE) trial. Patients were randomly allocated to intensive glucose control group receiving multifactorial intervention and pharmacotherapy based on Diaprel MR aiming at achieving target HbA1c of 6.5% or less or to the standard treatment group. Intensive treatment of diabetes, based on Diaprel MR 120 mg (4 tabs.) daily, reduced the combined incidence of macro- and microvascular events by 10% (p = 0.01); the risk reduction of microvascular events was reduced by 14% (p = 0.01). Intensive diabetes treatment was particularly effective in prevention of the onset/progression of diabetic nephropathy (relative risk reduction by 21%; p = 0.006), with 30% of risk reduction of the onset of macroalbuminuria (p < 0.001). Contrary to the results of the ACCORD study, the ADVANCE study showed positive trend towards a reduction in cardiovascular mortality (12%) in the intensive treatment group, despite achieving comparable levels of HbA1c.

WSTĘP. W literaturze znana jest rola ultrasonografii w diagnostyce osteoporozy miejscowej i w badaniach przesiewowych. Ponieważ brakuje obecnie danych o roli markerów resorpcji i tworzenia w ocenie dynamiki zmian kostnych w różnych postaciach zespołu stopy cukrzycowej, podjęto pracę, której celem jest ocena markerów kostnych w różnych postaciach zespołu stopy cukrzycowej w przewidywaniu postępu zmian kostnych. MATERIAŁ I METODY. Badaniem objęto 55 chorych na cukrzycę typu 1 (n = 7) i na cukrzycę typu 2 (n = 48), leczonych metodą intensywnej insulinoterapii. Pacjentów podzielono na 4 grupy: grupa 1 — z przewlekłą neuroosteoartropatią Charcota , grupa 2 — z przewlekłą neuroosteoartropatią Charcota z owrzodzeniem, grupa 3 — z ostrą neuroosteoartropatią Charcota oraz grupa 4 – z przewlekłym owrzodzeniem. W grupie 3 monitorowano markery co miesiąc — w pozostałych jednorazowo. Oznaczono następujące markery tworzenia kości — osteokalcynę i frakcję kostną fosfatazy zasadowej (krew), markery resorpcji — N-końcowy telopeptyd kolagenu typu 1 w moczu (NTX). Markery oznaczano metodą immunoenzymatyczną ELISA. Masę kostną oceniono metodą ultradźwiękową z użyciem aparatu Achilles. Oceniono przewlekłe powikłania cukrzycy. WYNIKI. W badaniu uwzględniono następujące powikłania przewlekłe w różnych grupach stopy cukrzycowej: grupa 1 (n = 10): retinopatia nieproliferacyjna — 7, proliferacyjna — 2, nefropatia wczesna — 3, zaawansowana — 0, neuropatia obwodowa — 9, autonomiczna — 1; grupa 2 (n = 18): retinopatia nieproliferacyjna — 7, proliferacyjna — 2, nefropatia wczesna — 2, zaawansowana — 1, neuropatia obwodowa — 10, autonomiczna — 1; grupa 3 (n = 5): retinopatia nieproliferacyjna — 2, nefropatia wczesna — 1, zaawansowana — 1, neuropatia obwodowa — 3, autonomiczna — 0; grupa 4 (n = 17): retinopatia nieproliferacyjna — 8, proliferacyjna — 8, nefropatia wczesna — 6, zaawansowana — 5, neuropatia obwodowa — 16, autonomiczna — 3. Średni czas trwania cukrzycy wynosił: grupa 1 — 15,8 ± ± 9,04 roku; grupa 2 — 12,6 ± 10,4 roku; grupa 3 — 10 ± 6,16 roku; grupa 4 — 12,8 ± 7,78 roku. Średnie stężenie HbA1c było równe: grupa 1 — 6,9 ± 0,78%; grupa 2 — 8,1 ± 1,42%; grupa 3 — 7,75 ± 2,89%; grupa 4 — 8,1 ± 1,11%. Średnia wartość T-score wynosiła odpowiednio: –0,81; –1,94; 0,52; –1,70. Różnice w średnich stężeniach osteokalcyny (p < 0,05) były równe: grupa 1 — 7,16 ± 2,06 ng/ml (n = 10); grupa 2 — 10,3 ± 7,6 ng/ml (n = 18); grupa 3 — 9,7 ± ± 3,05 ng/ml (n = 5); grupa 4 — 6,46 ± 1,27 ng/ml (n = 16). Różnice w średnich stężeniach izoenzymu fosfatazy alkalicznej wynosiły: grupa 1 — 30,16 ± ± 19,2 j./l (n = 11); grupa 2 — 74,8 ± 35,7 j./l (n = 10); grupa 3 — 37,4 j./l ± 0,08 (n = 3); grupa 4 — 20,31 ± ± 7,24 j./l (n = 17). Różnice w średnich stężeniach NTX były równe: grupa 1 — 56,0 ± 22,9 ng/ml (n = 11); grupa 2 — 74,8 ± 35,7 ng/ml (n = 10); grupa 3 — 95,2 ± 33,8 ng/ml (n = 4); grupa 4 — 64,8 ± 31,5 ng/ml (n = 17). WNIOSKI. 1. Stwierdzenie najniższego stężenia markerów tworzenia kostnego i podwyższonego stężenia markerów resorpcji w grupie 2 i 4 przy najniższej masie kostnej (–1,94, –1,7 T-score) wskazuje na przewagę resorpcji nad tworzeniem w przewlekłym owrzodzeniu z neuroosteopatią Charcota i bez niej. 2. Najwyższe stężenie markerów resorpcji kości (NTX 95,2 ng/ml) i podwyższone stężenie markerów tworzenia (fosfataza alkaliczna) przy prawidłowej masie kostnej sugerują ich przydatność w ocenie wzrostu obrotu kostnego (głównie resorpcji) poprzedzającej pojawienie się zmian kostnych. INTRODUCTION. The role of ultrasonographic examination in diagnostics of local osteoporosis and screening is widely confirmed. There is a lack of actual data concerning the role of resorption and creation markers in evaluation of bone changes dynamics in different forms of diabetic foot. Thus, bone markers in different forms of diabetic food were measured to predict a progress of bone changes. MATERIAL AND METHODS. The research included 55 patients with type 1 diabetes (n = 7) and type 2 diabetes (n = 48) treated with intensive insulin therapy. Patients were divided into 4 groups: group 1 — with chronic Charcot neuroosteoarthropathy, group 2 — with chronic Charcot neuroosteoarthropathy with ulceration, group 3 — with acute Charcot neuroosteoarthropathy and group 4 — with chronic ulceration. In group 3 bone turnover markers were monitored each month, in groups 1, 2 and 4 only once. Following markers of bone formation — osteocalcin and bone alkaline phosphatase (blood), as well as marker of bone resorption — NTX, were measured. Markers were estimated by means of immunoenzymatic ELISA method. Bone mass was assessed by ultrasound method using an Achilles device. Chronic complications of diabetes were also evaluated. RESULTS. In the study following chronic complications of diabetes in different groups of diabetic foot were evaluated: group 1 (n = 10): non-proliferative retinopathy — 7, proliferative retinopathy — 2, early-onset nephropathy — 3 and advanced nephropathy — 0, peripheral neuropathy — 9, autonomic neuropathy — 1; group 2 (n = 18): non-proliferative retinopathy — 7, proliferative retinopathy — 2, early-onset nephropathy — 2 and advanced nephropathy — 1, peripheral neuropathy — 10, autonomic neuropathy — 1; group 3 (n = 5): non-proliferative retinopathy — 2, early-onset nephropathy — 1 and advanced nephropathy — 1, peripheral neuropathy — 3, autonomic neuropathy — 0; group 4 (n = 17): non-proliferative retinopathy — 8, proliferative retinopathy — 8, early-onset nephropathy — 6 and advanced nephropathy — 5, peripheral neuropathy — 16, autonomic neuropathy — 3. Average time of diabetes: group 1 — 15.8 ± ± 9.0 years; group 2 — 12.6 ± 10.4 years; group 3 — 10 ± 6.16 years; group 4 — 12.8 ± 7.78 years. Average HbA1c level: group 1 — 6.9 ± 0.78%; group 2 — 8.1 ± ± 1.42%; group 3 — 7.75 ± 2.89%; group 4 — 8.1 ± ± 1.11%. Average T-score: –0.81, –1.94, 0.52, and –1.70 respectively. Differences in the mean osteocalcin levels (p < 0.05) were as follows: group 1 — 7.16 ± 2.06 ng/mL (n = 10); group 2 — 10.3 ± ± 7.6 ng/mL (n = 18); group 3 — 9.7 ± 3.05 ng/mL (n = 5); group 4 — 6.46 ± 1.27 ng/mL (n = 16). Differences in the mean bone alkaline phosphatase values were as follows: group 1 — 30.16 ± 19.2 U/L (n = 11); group 2 — 74.8 ± 35.7 U/L (n = 10); group 3 — 37.4 ± ± 0.08 U/L (n = 3); group 4 — 20.31 ± 7.24 U/L (n = 17). Differences in the mean NTX levels were as follows: group 1 — 56.0 ± 22.9 ng/mL (n = 11); group 2 — 74.8 ± 35.7 ng/mL (n = 10); group 3 — 95.2 ± 33.8 ng/mL (n = 4); group 4 — 64.8 ± ± 31.5 ng/mL (n = 17). CONCLUSIONS. 1. The lowest level of bone formation markers and increased level of bone resorption markers in group 2 and 4 with the lowest value of bone mass (–1.94, –1.7 T-score) indicates the advantage of resorption over formation process in chronic ulceration with or without Charcot neuroosteoarthropathy. 2. The highest level of bone resorption marker (NTX = 95.2 ng/mL) and increased level of bone formation marker (alkaline phosphatase) with normal bone mass, suggest their usefulness in evaluation of increase in bone turnover (mainly resorption) prior to development of bone changes.

Celem analizy było porównanie jakości życia i satysfakcji z terapii przez pacjentów z rozpoznaniem cukrzycy typu 1 i 2, leczonych za pomocą insuliny aspart lub dwufazowej insuliny aspart w stosunku 30/70, z chorymi, u których zastosowano terapię insuliną ludzką. Analizę jakości życia i satysfakcji z leczenia w cukrzycy typu 1 przeprowadzono na podstawie badań włączonych do systematycznego przeglądu oceniającego efektywność kliniczną insuliny aspart i dwufazowej insuliny aspart w stosunku 30/70, w porównaniu z insuliną ludzką. W wyniku systematycznego przeszukania baz danych, w ramach przeglądu systematycznego odnaleziono 4 badania kliniczne RCT (randomized controlled trial, badanie randomizowane), open-label (badanie otwarte). W celu uzupełnienia tego przeglądu, aby poszerzyć wiedzę o inne stosowane schematy leczenia, do analizy dołączono wyniki badania porównującego omawiane punkty końcowe u pacjentów leczonych insuliną aspart w połączeniu z insuliną protaminową Hadegorna (NPH) z osobami, u których stosowano krótkodziałającą insulinę ludzką, również w połączeniu z NPH. W ocenie satysfakcji z leczenia pacjentów z rozpoznaniem cukrzycy typu 2 posłużono się wynikami badania obserwacyjnego, w którym porównywano wymienione punkty końcowe u osób leczonych dwufazową insuliną aspart w stosunku 30/70 z kilkoma schematami wcześniej zastosowanego leczenia. Wymienione dwa badania wykraczały poza kryteria włączenia ustalone dla przeprowadzonego przeglądu systematycznego. Dzięki rozszerzonym kryteriom włączenia dla potrzeb tej analizy możliwe było pełniejsze porównanie zarówno jakości życia, jak i satysfakcji z leczenia osiąganych w terapii insuliną aspart lub dwufazową insuliną aspart w stosunku 30/70, w porównaniu z insuliną ludzką w różnych schematach leczenia. Satysfakcję z leczenia oceniano za pomocą skali DTSQ, podskali DSQOLS, natomiast jakość życia - za pomocą specyficznych kwestionariuszy: DSQOLS, DHP, QLsc. Wyniki wszystkich badań odnalezionych w ramach przeglądu systematycznego dowodzą osiągnięcia wyższej jakości życia oraz lepszej satysfakcji z leczenia insuliną aspart w porównaniu z terapią insuliną ludzką w cukrzycy typu 1. Satysfakcja z leczenia insuliną aspart w połączeniu z NPH, w porównaniu z terapią insuliną ludzką z NPH, jest również lepiej oceniana przez pacjentów. Chorzy ocenili korzystniej także terapię dwufazową insuliną aspart, podawaną za pomocą wstrzykiwacza (pen), niż leczenie stosowane przed włączeniem do badania zarówno w odniesieniu do wyniku całkowitego, jak i do części dotyczących wygody, elastyczności, skuteczności klinicznej, jakości życia oraz ogólnych preferencji dotyczących terapii. The aim of this analysis was to compare quality of life and treatment satisfaction of patients with diagnosed type 1 or 2 diabetes mellitus treated with insulin aspart or biphasic insulin aspart 30/70 with those of patients, to whom therapy with human insulin was administered. Quality of life and treatment satisfaction analysis for type 1 diabetes mellitus was based on a systematic review, in which efficacy and safety of insulin aspart and biphasic insulin aspart 30/70 vs. human insulin was evaluated. In addition, database search within a systematic review revealed 4 randomized, open-label clinical trials. In order to supplement the review mentioned above and extend the analysis to other treatment protocols, the results of a study, in which the same endpoints were assessed in patients, in whom insulin aspart in combination with Hagedorn protamin insulin (NPH) was used, vs. patients treated with short-acting human insulin, also in combination with NPH, were taken into account in the analysis. In assessment of treatment satisfaction of patients with type 2 diabetes mellitus the results of another observational study were taken into account, in which the endpoints mentioned above were evaluated in patients treated with biphasic insulin aspart 30/70 as compared with several previously used treatments. The two studies mentioned above exceeded the inclusion criteria assumed for the systematic review. Extension of the inclusion criteria for this analysis made it possible to compare both quality of life and treatment satisfaction achieved with therapy with insulin aspart or biphasic insulin aspart 30/70 as compared to human insulin in different treatment protocols and in a more complete way. Treatment satisfaction was assessed using DTSQ scale/DSQOLS subscale and quality of life with specific questionnaires: DSQOLS, DHP, QLsc. Results of all the studies identified within the systematic review demonstrated higher quality of life and treatment satisfaction achieved with insulin aspart as compared to human insulin in type 1 diabetes mellitus. Treatment satisfaction for insulin aspart in combination with NPH was also higher as compared to human insulin in combination with NPH. Therapy with biphasic insulin aspart administered by means of a pen was also evaluated higher by the patients than the treatment used prior to enrollment; this was true for total score as well as for the parts related to convenience, flexibility, efficacy, quality of life and general preferences with respect to treatment.
