1. Expert opinion on the diagnostic odyssey and management of late-onset Pompe disease: a neurologist's perspective

Abstract: This consensus statement by a panel of neurology experts aimed to provide a practical and implementable guidance document to assist clinicians with the best clinical practice in terms of diagnosis, treatment, and monitoring of late-onset Pompe disease (LOPD). The participating experts consider the clinical suspicion of LOPD by the physician to be of utmost importance in the prevention of diagnostic and therapeutic delay in LOPD patients. A diagnostic algorithm is proposed to facilitate the diagnosis of LOPD in patients presenting with unexplained proximal/axial weakness (with or without respiratory symptoms) or restrictive respiratory insufficiency with hyperCKemia and/or exercise intolerance as the red flag symptoms/signs that raise the index of suspicion for LOPD diagnosis. The diagnosis is based on the subsequent use of dried blood spot (DBS) assay, and the DBS assay can be confirmed by acid alpha-glucosidase (GAA) tissue analysis in leukocytes, fibroblasts, or muscle fibers and/or genetic mutation analysis. Accordingly, experts consider increased awareness among physicians about potential presenting characteristics with a high index of suspicion for LOPD to be crucial to suspect and consider LOPD in the differential diagnosis, while strongly suggesting the use of a diagnostic algorithm combined with DBS assay and confirmatory tests in the timely diagnosis of LOPD and implementation of best practice patterns.

2. Identifying a Cut-off Point for Timed Up and Go Test in Neuromuscular Diseases

Abstract: Objective: Balance problems and falls due to a progressive loss of muscle strength in neuromuscular diseases (NMD) negatively affect the activities of daily living and functional independence of patients. The timed up and go (TUG) test is an easy-to-apply and objective measure of balance. This study aimed to determine the cut-off value of the TUG test in adult individuals with NMD. Materials and Methods: This retrospective study included 102 patients. The receiver operating characteristic analysis was performed and cut-off, sensitivity, and specificity values were calculated in NMD to determine the diagnostic validity of the TUG test. Results: The mean ages of 55 patients with a fall history and 47 patients without a fall history were 34.40+-10.41 and 31.87+-10.11 years, respectively. The mean values of the TUG scores of these individuals were calculated as 11.79+-4.30 and 7.33+-1.51 s, respectively. The cut-off value of the TUG test in adult individuals with the NMD was determined as 8.9 s. Conclusion: The present study revealed that individuals with NMD who complete the TUG test over 8.9 s have a high risk of falling. Determining the risk of falling in NMD, which is one of the neurological patient groups where falling complaints are most common, is extremely important for patient safety and the effectiveness of rehabilitation.

Abstract: Trigeminal neuropathy is an infrequent condition, usually limited to the sensorial component of the nerve, and it is one of the most common neurologic manifestations of mixed connective tissue disease (MCTD). However, to the best of our knowledge, no cases have been reported with MCTD presenting as trigeminal sensory and motor neuropathy. Herein, we report a 28-year-old female with unilateral trigeminal sensory–motor neuropathy who presented with numbness over the distribution of the right trigeminal region and right masseter atrophy. We also reviewed the literature for trigeminal neuropathy associated with MCTD.

4. Comprehensive clinical, biochemical, radiological and genetic analysis of 28 Turkish cases with suspected metachromatic leukodystrophy and their relatives

Abstract: Metachromatic leukodystrophy (MLD) is a glycosphingolipid storage disease caused by deficiency of the lysosomal enzyme arylsulfatase A (ASA) or its activator protein saposin B. MLD can affect all age groups in severity varying from a severe fatal form to milder adult onset forms. Diagnosis is usually made by measuring leukocyte ASA activity. However, this test can give false negative or false positive laboratory results due to pseudodeficiency of ASA and saposin B deficiency, respectively. Therefore, we aimed to evaluate patients with suspected MLD in a Turkish population by comprehensive clinical, biochemical, radiological, and genetic analyses for molecular and phenotypic characterization. We analyzed 28 suspected MLD patients and 41 relatives from 24 families. ASA activity was found to be decreased in 21 of 28 patients. Sixteen patients were diagnosed as MLD (11 late infantile, 2 juvenile and 3 adult types), 2 MSD, 2 pseudodeficiency (PD) and the remaining 8 patients were diagnosed as having other leukodystrophies. Enzyme analysis showed that the age of onset of MLD did not correlate with residual ASA activity. Sequence analysis showed 11 mutations in ARSA, of which 4 were novel (p.Trp195GlyfsTer5, p.Gly298Asp, p.Arg301Leu, and p.Gly311Asp), and 2 mutations in SUMF1 causing multiple sulfatase deficiency, and confirmed the diagnosis of MLD in 2 presymptomatic relatives. All individuals with confirmed mutations had low ASA activity and urinary sulfatide excretion. Intra- and inter-familial variability was high for the same ARSA missense genotypes, indicating the contribution of other factors to disease expression. Imaging findings were evaluated through a modified brain MRI scoring system which indicated patients with protein-truncating mutations had more severe MRI findings and late-infantile disease onset. MRI findings were not specific for the diagnosis. Anti-sulfatide IgM was similar to control subjects, and IgG, elevated in multiple sulfatase deficiency. In conclusion, the knowledge on the biochemical, clinical and genetic basis of MLD was expanded, a modified diagnostic laboratory algorithm for MLD based on integrated evaluation of ASA activity, urinary sulfatide excretion and genetic tests was devised.

6. The altered expression of perineuronal net elements during neural differentiation

Abstract: Abstract Background Perineuronal nets (PNNs), which are localized around neurons during development, are specialized forms of neural extracellular matrix with neuroprotective and plasticity-regulating roles. Hyaluronan and proteoglycan link protein 1 (HAPLN1), tenascin-R (TNR) and aggrecan (ACAN) are key elements of PNNs. In diseases characterized by neuritogenesis defects, the expression of these proteins is known to be downregulated, suggesting that PNNs may have a role in neural differentiation. Methods In this study, the mRNA and protein levels of HAPLN1, TNR and ACAN were determined and compared at specific time points of neural differentiation. We used PC12 cells as the in vitro model because they reflect this developmental process. Results On day 7, the HAPLN1 mRNA level showed a 2.9-fold increase compared to the non-differentiated state. However, the cellular HAPLN1 protein level showed a decrease, indicating that the protein may have roles in neural differentiation, and may be secreted during the early period of differentiation. By contrast, TNR mRNA and protein levels remained unchanged, and the amount of cellular ACAN protein showed a 3.7-fold increase at day 7. These results suggest that ACAN may be secreted after day 7, possibly due to its large amount of post-translational modifications. Conclusions Our results provide preliminary data on the expression of PNN elements during neural differentiation. Further investigations will be performed on the role of these elements in neurological disease models.

7. Dysferlinopathy: A Case Report and Literature Update

Abstract: Dysferlinopathy is a rare autosomal recessive myopathy, resulting in the lack or absence of dysferlin production caused by mutations in the encoding gene. Dysferlin is a sarcolemmal membrane protein involved in the repair of membrane damage caused by calcium. There are four identified phenotypic dysferlinopathies, two of which are relatively frequently observed. Miyoshi myopathy and limb-girdle muscular dystrophy type 2B are frequently observed; the two very rare dysferlinopathies are distal anterior compartment myopathy and scapuloperoneal muscular dystrophy (observed in only one case). Serum CK levels are quite high, even in the pre-clinical period. A muscle biopsy typically shows dystrophic patterns, often accompanied with T-lymphocyte-based inflammatory changes. The clinical course of dysferlinopathy is usually much better than that of other recessive trait muscular dystrophies. Dysferlinopathies should be considered in the differential diagnosis of polymyositis to avoid unnecessary and potentially dangerous medications such as oral steroids or immunosuppressive therapies. Here we report the case a 21-year-old Syrian patient diagnosed with dysferlinopathy who has had serious CK elevations from the age of 1 and who had been diagnosed with polymyositis by a muscle biopsy 7 years ago and who therefore used steroids/azathioprine for the following 3 years.

Abstract: Objective: Hereditary neuropathy with liability to pressure palsy (HNPP) needs to be differentiated from entrapment neuropathies due to differences in the treatment management. Materials and Methods: Among 5075 patients with entrapment neuropathy, we retrospectively evaluated the neurophysiologic results of 20 patients with three or more entrapments. Results: Ten patients were diagnosed as having HNPP according to their genetic or nerve biopsy results; eight (80%) had bilateral Carpal tunnel syndrome, nine (90%) had bilateral ulnar entrapment neuropathy, eight (80%) had bilateral median and ulnar entrapment together; and three (30%) had one-sided peroneal neuropathy. Conclusion: Our data suggest that analyzing the neurophysiologic studies and keeping HNPP in mind are essential to characterize underdiagnosed patients with HNPP referred for entrapment neuropathy.

10. Early Diagnosis of Distal Peripheral Polyneuropathy Due to Glucose Metabolism Disorders via Intraepidermal Nerve Fiber Analysis

Abstract: OBJECTIVE: Polyneuropathy is the most frequent complication of diabetes mellitus (DM), which is a common disorder worldwide. Early diagnosis of this complication is critical for treatment planning. Recently, intra-epidermal nerve fiber (IENF) analysis has been used for the early diagnosis of diabetic neuropathy and was reported to be superior to both conventional fascicular nerve biopsy and other diagnostic tests. The present study aimed to determine the effect of early glucose metabolism disorders on the peripheral nerves using IENF analysis. METHODS: : This prospective study compared IENF nerve fiber density in 10 patients with early-stage DM and 12 patients with impaired glucose tolerance (IGT) to that of healthy controls in order to investigate fiber loss in the asymptomatic stage. RESULTS: Mean age of the DM and IGT groups was 53.4 ± 10.9 years and 50.8 ± 10.6 years, respectively. Neither the IGT nor DM patients had paresthesia. Neurological examination results of the patients were all in the normal range. Mean fiber density was 6.4 ± 2.7 mm-1 in the DM group, 7.5 ± 3.2 mm-1 in the IGT group, and 13.7 ± 2.7 mm-1 in the control group (p< 0.0001). Paired analysis of the groups showed a significant difference between the 2 patient groups and the control group. There was a statistically significant difference in mean nerve fiber density between the DM and control groups (p< 0.0001, Mann-Whitney U test), and between the IGT and control groups (p< 0.0001, Mann-Whitney U test); however, there wasn’t a significant difference between the DM and IGT groups (p= 0.5, Mann-Whitney U test). CONCLUSION: The results show that IENF density in the IGT and DM groups decreased in the asymptomatic stage of DM. This means that polyneuropathy can begin in the prediabetic period in patients with glucose metabolism disorders. This critical information highlighs the importance of the early diagnosis and treatment of glucose metabolism disorders in preventing neurological complications

Abstract: Amaç: İnsan immün yetmezlik virüsü, dünyadaki en önemli halk sağlığı sorunlarından biri olmaya devam etmektedir. Ruh ve beden sağlığı ile sosyal yaşam üzerinde önemli bir etkiye sahiptir. Hastalığın HİV'den AİDS'e ilerlemesinde erken tanı, düzenli muayene, takip ve antiretroviral tedavi önleyicidir. Bununla birlikte, damgalama ve ayrımcılık, HİV ve HİV ile ilgili komplikasyonlarla başa çıkmada başlıca engellerdir. Bu nedenle, ailelerde, sağlık bakım ortamlarında, topluluklarda ve işyerlerinde HİV pozitiflere yönelik damgalama ve ayrımcılık algısını anlamak çok önemlidir.Yöntem: Veri analizi için kuram temelli nitel analiz kullanılmıştır. Verilerin toplanmasında yarı yapılandırılmış bir anket ile amaçlı ve kartopu örnekleme yöntemi kullanılmıştır. Yöntem ve sonuçları raporlamak için Niteliksel Araştırma Raporlama için Konsolide Kriterler (COREQ) ilkeleri formu kullanılmıştır. Veri analizi için MAXQDA21 nitel programı kullanılmıştır.Bulgular: Araştırma, yaşları 23 ile 66 arasında değişen toplam 20 erkek ve 3 kadın katılımcıdan oluşmaktadır. Katılımcıların çoğunluğu homoseksüel erkeklerden oluşmaktadır (%69,5; n:16). Katılımcılardan biri hariç hepsinin cinsel ilişki yoluyla HIV'e yakalandığı görülmüştür. Katılımcıların neredeyse yarısı üniversite mezunudur; %13'ten biraz fazlası (n:3) iş güvencesine sahiptir. Veri analizi sonucunda damgalama ve ayrımcılık, ruhsal sağlık sorunlarını da kapsayan sonuçlar olmak üzere iki tema ve beş kategori ortaya çıkmıştır.Sonuç: Bu çalışmaya katılanların neredeyse tamamının yaşamın her seviyesinde damgalanma ve ayrımcılığa maruz kaldığı görülmüştür. It has an important influence on mental and physical health, and social life. Early detection, regular examination, follow-ups, and antiretroviral therapy are preventive in progressing the disease from HIV to AIDS. However, stigma and discrimination are major impediments to cope with HIV and HIV related complications. Thus, it is crucial to understand the perception of stigma and discrimination towards HIV-positive within families, health care settings, communities and workplaces. Method: A grounded theory-based qualitative analysis was used for data analysis. Purposive and snowball sampling method was used with a semi-structured questionnaire for data collection. The principles of the Consolidated Criteria for Reporting Qualitative Research (COREQ) were applied to report the methods and results. MAXQDA21 qualitative program was used for data analysis. Results: The study consisted of 20 male and 3 female participants aged between 23 and 66 years old. Majority of them were men who have sex with men (69.5%; n:16). All of them, except one, contracted HIV by sexual intercourse. Almost half of them were university graduates; just over 13%(n:3) of them had job security. Two themes and five categories emerged from the codes defined by the data analysis. These themes were as follows: stigma and discrimination; and outcomes including mental health problems.Conclusions: Almost all participants in this study experienced stigma and discrimination at all levels of life. Their coping strategies and interpretations of HIV/AIDS varied. Mental health problems were main outcome. Providing public engagement and increasing awareness/knowledge through social media, schools, universities, and formal and private communication resources may help them tackle negative life experiences, such as stigma and discrimination. It can be suggested that HIV, which is an important chronic infectious disease in terms of public health, should be included in the education curriculum of health departments.

Abstract: ObjectiveTo evaluate whether eculizumab helps patients with anti–acetylcholine receptor–positive (AChR+) refractory generalized myasthenia gravis (gMG) achieve the Myasthenia Gravis Foundation of America (MGFA) post-intervention status of minimal manifestations (MM), we assessed patients' status throughout REGAIN (Safety and Efficacy of Eculizumab in AChR+ Refractory Generalized Myasthenia Gravis) and its open-label extension.MethodsPatients who completed the REGAIN randomized controlled trial and continued into the open-label extension were included in this tertiary endpoint analysis. Patients were assessed for the MGFA post-intervention status of improved, unchanged, worse, MM, and pharmacologic remission at defined time points during REGAIN and through week 130 of the open-label study.ResultsA total of 117 patients completed REGAIN and continued into the open-label study (eculizumab/eculizumab: 56; placebo/eculizumab: 61). At week 26 of REGAIN, more eculizumab-treated patients than placebo-treated patients achieved a status of improved (60.7% vs 41.7%) or MM (25.0% vs 13.3%; common OR: 2.3; 95% CI: 1.1–4.5). After 130 weeks of eculizumab treatment, 88.0% of patients achieved improved status and 57.3% of patients achieved MM status. The safety profile of eculizumab was consistent with its known profile and no new safety signals were detected.ConclusionEculizumab led to rapid and sustained achievement of MM in patients with AChR+ refractory gMG. Abstract: The Ottoman Empire, the organization during the fall period up has not much importance to the production or the forest products forestry activities in the period and the production area only in rural areas of taxation means is seen as war and the palace meeting the needs of the vehicle. Reforms Dönemi'yl with natural forests and property values begin to understand the self-regulation by the Ottoman Empire for the forest, it has issued codes of law. Forestry General Directorate of Forestry and Forest School Provincial Organization of rich forests for timber sales through auctions on the other hand have started to show sensitivity to the protection of the established forest were carried out. The first efforts to protect forests and to regulate forestry activities in the Ottoman Empire has been Abdulmecit period. Forestry Directorate was established in 1839 Abdulmecit period. A forest protection staff in 1851 was prepared. Launched westernization attempts at each site with a wriggling mAlArA Tanzimat led teams in forestry. In the Ottoman Empire, the largest arrangements related to forestry activities during the period of Sultan Abdulaziz Forest Regulations 1870 has been removed. This regulation remained in practice until 1937. Especially the structure of the Constitutional period of Forestry Directorates after the Tanzimat period, the introduction of foreign forestry experts, their exploration and mapping work, the forest and the establishment of the Agricultural School of the administrative and legal arrangements for the Ottoman Forestry was given in the form of implementation in the Ottoman geography of these regulations. Organization of forest remaining in the background, our goal is to do research in this particularly Ottoman Economic History and bureaucratic structure, will reveal forestry activities.

Abstract: Metachromatic leukodystrophy (MLD) is a glycosphingolipid storage disease caused by deficiency of the lysosomal enzyme arylsulfatase A (ASA) or its activator protein saposin B. MLD can affect all age groups in severity varying from a severe fatal form to milder adult onset forms. Diagnosis is usually made by measuring leukocyte ASA activity. However, this test can give false negative or false positive laboratory results due to pseudodeficiency of ASA and saposin B deficiency, respectively. Therefore, we aimed to evaluate patients with suspected MLD in a Turkish population by comprehensive clinical, biochemical, radiological, and genetic analyses for molecular and phenotypic characterization. We analyzed 28 suspected MLD patients and 41 relatives from 24 families. ASA activity was found to be decreased in 21 of 28 patients. Sixteen patients were diagnosed as MLD (11 late infantile, 2 juvenile and 3 adult types), 2 MSD, 2 pseudodeficiency (PD) and the remaining 8 patients were diagnosed as having other leukodystrophies. Enzyme analysis showed that the age of onset of MLD did not correlate with residual ASA activity. Sequence analysis showed 11 mutations in ARSA, of which 4 were novel (p.Trp195GlyfsTer5, p.Gly298Asp, p.Arg301Leu, and p.Gly311Asp), and 2 mutations in SUMF1 causing multiple sulfatase deficiency, and confirmed the diagnosis of MLD in 2 presymptomatic relatives. All individuals with confirmed mutations had low ASA activity and urinary sulfatide excretion. Intra- and inter-familial variability was high for the same ARSA missense genotypes, indicating the contribution of other factors to disease expression. Imaging findings were evaluated through a modified brain MRI scoring system which indicated patients with protein-truncating mutations had more severe MRI findings and late-infantile disease onset. MRI findings were not specific for the diagnosis. Anti-sulfatide IgM was similar to control subjects, and IgG, elevated in multiple sulfatase deficiency. In conclusion, the knowledge on the biochemical, clinical and genetic basis of MLD was expanded, a modified diagnostic laboratory algorithm for MLD based on integrated evaluation of ASA activity, urinary sulfatide excretion and genetic tests was devised.

Abstract: Background: In the phase III eculizumab for refractory generalized myasthenia gravis REGAIN study [ClinicalTrials.gov identifier: NCT01997229] and its open-label extension (OLE) [ClinicalTrials.gov identifier: NCT02301624], patients with treatment-refractory antiacetylcholine receptor antibody-positive generalized myasthenia gravis had clinically meaningful improvements with eculizumab versus placebo. This subgroup analysis evaluated data from patients with a recent history of chronic intravenous immunoglobulin (IVIg) use before study entry. Methods: The subgroup comprised patients who had received IVIg at least four times in 1 year, with at least one IVIg treatment cycle during the 6 months before the first REGAIN study dose. Data from REGAIN and the OLE were analyzed. Response to eculizumab versus placebo was assessed using four validated, disease-specific measures. Incidences of exacerbations and safety endpoints were recorded. Results: The subgroup had similar patient and disease characteristics as the overall REGAIN population. Clinical assessments showed sustained eculizumab efficacy during REGAIN and the OLE over 18 months. Patients receiving placebo in REGAIN experienced rapid improvements in assessment scores when treated with eculizumab in the OLE. There was a lower rate of disease exacerbations with eculizumab than with placebo during REGAIN, and eculizumab was well tolerated. Conclusion: Eculizumab treatment, compared with placebo, results in meaningful clinical improvements and fewer disease exacerbations for patients who previously received chronic IVIg. Abstract: Istanbul sehri tarih boyunca cok sayida imparatorluk ve kulture baskentlik yapmistir. Bu vasiflarindan oturu hem konumuyla hem de sahip oldugu tarihi zenginlikleriyle arastirilmaya deger ozelligini daima korumustur. Bu dogrultuda Osmanli tarihiyle ilgili dogru bilgilere ulasilmasini saglayan birinci elden kaynak olan seriyye sicilleri hic suphesiz bu amac ugrunda en kayda deger kayitlardir. 199 Numarali Mahmud Pasa Mahkemesi Ser´iyye Sicili’nin transkripsiyon ve degerlendirilmesi sonucunda cok sayidaki davanin alacak verecek ile alakali oldugu tespit edilmistir. Bunun sebebi ise 17. yuzyillarda bir ikamet alani olan Mahmud Pasa Semti’nin 20. yuzyilda artik tamamen bir ticaret semti olmasidir. Bu calismamiz, H.1322-1324/M.1904-1907 yillari arasini kapsayan 199 Numarali Mahmud Pasa Mahkemesi Ser´iyye Sicili’nin transkripsiyonu ve degerlendirmesini icermekle birlikte ulastigimiz bilgiler isiginda Musluman ve gayrimuslim unsurlar arasinda ve diger azinliklar veya sadece Osmanli vatandaslarinin arasindaki alacak veya benzer davalarin hukuki boyutu uzerinde durulmustur.

Abstract: Background Complement is likely to have a role in refractory generalised myasthenia gravis, but no approved therapies specifically target this system. Results from a phase 2 study suggested that eculizumab, a terminal complement inhibitor, produced clinically meaningful improvements in patients with anti-acetylcholine receptor antibody-positive refractory generalised myasthenia gravis. We further assessed the efficacy and safety of eculizumab in this patient population in a phase 3 trial. Methods We did a phase 3, randomised, double-blind, placebo-controlled, multicentre study (REGAIN) in 76 hospitals and specialised clinics in 17 countries across North America, Latin America, Europe, and Asia. Eligible patients were aged at least 18 years, with a Myasthenia Gravis-Activities of Daily Living (MG-ADL) score of 6 or more, Myasthenia Gravis Foundation of America (MGFA) class II-IV disease, vaccination against Neisseria meningitides, and previous treatment with at least two immunosuppressive therapies or one immunosuppressive therapy and chronic intravenous immunoglobulin or plasma exchange for 12 months without symptom control. Patients with a history of thymoma or thymic neoplasms, thymectomy within 12 months before screening, or use of intravenous immunoglobulin or plasma exchange within 4 weeks before randomisation, or rituximab within 6 months before screening, were excluded. We randomly assigned participants (1:1) to either intravenous eculizumab or intravenous matched placebo for 26 weeks. Dosing for eculizumab was 900 mg on day 1 and at weeks 1, 2, and 3; 1200 mg at week 4; and 1200 mg given every second week thereafter as maintenance dosing. Randomisation was done centrally with an interactive voice or web-response system with patients stratified to one of four groups based on MGFA disease classification. Where possible, patients were maintained on existing myasthenia gravis therapies and rescue medication was allowed at the study physician's discretion. Patients, investigators, staff, and outcome assessors were masked to treatment assignment. The primary efficacy endpoint was the change from baseline to week 26 in MG-ADL total score measured by worst-rank ANCOVA. The efficacy population set was defined as all patients randomly assigned to treatment groups who received at least one dose of study drug, had a valid baseline MG-ADL assessment, and at least one post-baseline MG-ADL assessment. The safety analyses included all randomly assigned patients who received eculizumab or placebo. This trial is registered with ClinicalTrials.gov, number NCT01997229. Findings Between April 30, 2014, and Feb 19, 2016, we randomly assigned and treated 125 patients, 62 with eculizumab and 63 with placebo. The primary analysis showed no significant difference between eculizumab and placebo (least-squares mean rank 56·6 [SEM 4·5] vs 68·3 [4·5]; rank-based treatment difference -11·7, 95% CI -24·3 to 0·96; p=0·0698). No deaths or cases of meningococcal infection occurred during the study. The most common adverse events in both groups were headache and upper respiratory tract infection (ten [16%] for both events in the eculizumab group and 12 [19%] for both in the placebo group). Myasthenia gravis exacerbations were reported by six (10%) patients in the eculizumab group and 15 (24%) in the placebo group. Six (10%) patients in the eculizumab group and 12 (19%) in the placebo group required rescue therapy. Interpretation The change in the MG-ADL score was not statistically significant between eculizumab and placebo, as measured by the worst-rank analysis. Eculizumab was well tolerated. The use of a worst-rank analytical approach proved to be an important limitation of this study since the secondary and sensitivity analyses results were inconsistent with the primary endpoint result; further research into the role of complement is needed. The period of fountain water wasreplaced by a new period in which water was distributed to houses andworkplaces through taps. As the need for water increased day by day and the newsites, industrialization and irrigated farming became more and more widespread,it became compulsory to use the water systematically. In the last century ofOttoman Empire, most of services were given in exchange for privileges and itwas seen that Tohma water was also given for privilege in order to bring it instate economy in the period of constitutional monarchy.

27. Çocuklarda Okul Temelli Yalnızlık ve Yaşam Doyumu Arasındaki İlişkide Algılanan Stresin Aracı Rolü

Abstract: The aim of this study is to examine the mediating role of perceived stress among school-based loneliness and life satis-faction in children. In this direction, 199 (50.5%) girls and 193 (49%) boys were 392 grade 4 students. Data were obtained by using the School Based Loneliness Scale in Children, Life Satisfaction Scale for Children and Perceived Stress Child Form and Personal Information Form. In the analysis of the data ; Pearson analysis, Independent Groups T test, Tool variable analysis and Bootstrap Analysis were used. According to the findings, there was a negative correlation between life satisfaction and perceived stress; there was a negative corre-lation between school-based loneliness and life satisfaction scores, and a significant positive correlation between perceived stress and school-based loneliness. According to the results of the study, the partial mediating role of perceived stress between school-based loneliness and life satisfaction is statistically significant. In future researches, different variables can be handled in different age groups. Psychoeducation programs can be developed by researching students' school-based loneliness and possible sources of stress., Bu araştırmanın amacı çocuklarda okul temelli yalnızlık ve yaşam doyumu arasında algılanan stresin aracı rolünün incelenmesidir. Bu doğrultuda Kocaeli ili’nde yaşları 9-11 arasında değişen 199’u (%50,5) kız, 193’ü (%49) erkek toplam 394 4.sınıf öğrencisi katılmıştır. Veriler Çocuklarda Okul Temelli Yalnızlık Ölçeği, Çocuklar İçin Yaşam Doyumu Ölçeği, Algılanan Stres Ölçeği ve Kişisel Bilgi Formu kullanılarak elde edilmiştir. Verilerin analizinde Pearson analizi, Bağımsız Gruplar T testi, Aracı değişken analizleri ve Bootstrap Analizi kullanılmıştır. Elde edilen bulgulara göre yaşam doyumu ile algılanan stres arasında negatif yönde ; okul temelli yalnızlık ile yaşam doyumu puanları arasında negatif yönde ve algılanan stres ile okul temelli yalnızlık arasında pozitif yönde manidar bir ilişki bulunmuştur .Araştırma sonuçlarına göre okul temelli yalnızlık ile yaşam doyumu arasında algılan stresin kısmi aracılık rolünün istatistiksel olarak anlamlı olduğu görülmektedir. Bundan sonra yapılacak araştırmalarda farklı değişkenler, farklı yaş gruplarında ele alınabilir. Öğrencilerin okul temelli yalnızlık ve olası stres kaynakları araştırılarak psikoeğitim programları geliştirilebilir.

Abstract: The terminal complement inhibitor eculizumab was shown to improve myasthenia gravis-related symptoms in the 26-week, phase 3, randomized, double-blind, placebo-controlled REGAIN study (NCT01997229). In this 52-week sub-analysis of the open-label extension of REGAIN (NCT02301624), eculizumab's efficacy and safety were assessed in 11 Japanese and 88 Caucasian patients with anti-acetylcholine receptor antibody-positive refractory generalized myasthenia gravis. For patients who had received placebo during REGAIN, treatment with open-label eculizumab resulted in generally similar outcomes in the Japanese and Caucasian populations. Rapid improvements were maintained for 52 weeks, assessed by change in score from open-label extension baseline to week 52 (mean [standard error]) using the following scales (in Japanese and Caucasian patients, respectively): Myasthenia Gravis Activities of Daily Living (-2.4 [1.34] and - 3.3 [0.65]); Quantitative Myasthenia Gravis (-2.9 [1.98] and - 4.3 [0.79]); Myasthenia Gravis Composite (-4.5 [2.63] and - 4.9 [1.19]); and Myasthenia Gravis Quality of Life 15-item questionnaire (-8.6 [5.68] and - 6.5 [1.93]). Overall, the safety of eculizumab was consistent with its known safety profile. Abstract: Genel tarih kaynağı olan Zübdetü’l-Âsâr, Abdullah bin Muhammed bin Ali Nasrullâhî tarafından Türkçe (Çağatayca) yazılmış, 1525 yılında tamamlanmış ve Süyünç Hoca Han’ın oğlu olan Sultan Muhammed’e ithaf edilmiş bir eserdir. Bu eserde yazar, kendi dönemine kadar olan birinci el kaynakları kullanmış ayrıca gördüğü olayları anlatmış, çağdaşı kişilerden dinlediklerini de eserine aktarmıştır. Aslı kayıp olan eserin elimizde 3 nüshası bulunmaktadır. Eserin 4. nüshasına ait bilgi, Devin DeWeese tarafından ilim dünyasına A note on manuscripts of the Zubdat al-āthār adlı çalışmasında duyurulmuştur. Burada değerlendireceğimiz sözcükler devlet teşkilatına dair terimlerdir. Eserin tarihî bir kaynak olması sebebiyle tarihî olaylar anlatılırken kullanılan terimler de bize o döneme ait devlet teşkilatı hakkında bilgi vermektedir. Bu makale kapsamında özellikle Timurlulara ait devlet teşkilatıyla ilgili belli başlı terimler (ınaḳ, ḳorçı, tovaçı, bitikçi vb.) üzerinden dönemin devlet teşkilatı ortaya konmaya çalışılacaktır, Zubdat-al-āthār is a historical work which was written by Abdullah bin Muhammad bin Ali Nasrullāhī, in (Chagatai) Turkic, completed in 1525 and dedicated to Sultan Muhammad who was the son of Suyunch Khoja Khan. In the work the author mainly refers to primary sources to relate events which had occurred before his era. He also narrates some events that he personally witnessed and that he heard directly from eyewitnesses. The work’s original manuscript is missing; there are three copies which have survived. Information about a fourth copy of the work was announced to the scientific community by DeWeese in his work titled A note on manuscripts of the Zubdat al-āthār. As Zubdat-al-āthār is a chronicle, the author uses many governmental and administrative terms and titles which provide information about government organizations of the states of his period. In this study we will deal with the administrative terms that are attested in Zubdat-al-āthār with a focus on Timurid dynasty. Some of the terms we discuss in this paper are ınaḳ, ḳorçı, tovaçı, bitikçi.

Yordayıcıkorelasyonel modelin kullanıldığı bu araştırmada veri toplama aracı olarak10-16Yaş Çocuk ve Ergenler İçin Erken Dönem Uyumsuz Şema Ölçekler Takımı ve On-Maddeli Kişilik Ölçeği kullanılmıştır. Çalışmagrubundan toplanan veriler SPSS 21 programı ile analiz edilmiştir. Araştırmada çoklu doğrusal regresyon vepearson korelasyon analizi kullanılmıştır. Buaraştırmanın temel amacı, sağlamlık ve yaşam doyumunun sosyal medya bozukluğuüzerindeki etkisini incelemek ve nevrotiklik, olumsuz değerlilik ve deneyimeaçıklığın aracı rolünü belirlemektir Bu araştırma için seçilen değişkenler sonçalışmaların kuramsal açıklamalarına dayanmaktadır. Üniversite öğrencileri içinsosyal medya kullanım bozukluğunu ölçecek bir ölçek bulunmamasından dolayı,İngilizce dilinde mevcut bir ölçeğin güvenilirliği ve geçerlilik çalışmasıyapılmıştır. Daha sonra, bir yol analiz modeli oluşturularak bu model testedilmiştir. Modelin test edilmesinde kullanılan veri 9 fakültede öğrenim gören638 öğrencinin görüşlerine başvurularak toplanmıştır. Çalışma sonucunda eldeedilen bulgulara göre nevrotiklik, olumsuz değerlilik, dayanıklılık ve yaşamdoyumunun üniversite öğrencileri arasında sosyal medya bozukluğunu doğrudanetkilediğine dair kanıtlar oluşturduğu görülmüştür. Ek olarak, sonuçlar,nevrotikliğin ve negatif değerliliğin bazı durumlar için aracı bir roloynadığını göstermiştir. Bu sonuçlar araştırmacılara ve sağlık çalışanlarıtarafından sosyal medya bozukluklarının daha iyi anlaşılması sağlamıştır vesosyal medya bozukluğunun olumsuz etkilerini en aza indirmek için müdahalelergeliştirirken kişilik özelliklerinin önemini dikkate almayı vurgulamaktadır., Increaseduse of social media platforms that result in psychological and behavioralproblems suggest an effort to increase nomological network of social mediadisorder with psychological constructs. The main purpose of this research wasto test the effect of resilience and satisfaction with life on social mediadisorder and mediating role of the neuroticism, negative valence and opennessto experience. The variables selected for this research are based ontheoretical explanations of recent studies. Due to the lack of a scale tomeasure social media use disorder for university students, reliability andvalidy study of an existing scale available in English was performed primarily.Later, a path analytic model was constructed and tested. The model was tested witha sample of 638 students from 9 different faculties. The conclusions drawn fromthis study are substantial, in sum, this study generated evidence thatneuroticism, negative valence, resilience and life satisfaction have directinfluence on social media disorder among university students. In addition, theresults indicated that neuroticism and negative valence plays a mediating rolefor some circumstances. These conclusions offer researchers and healthprofessionals’ better understanding of social media disorders and stresses onthe importance of personality traits when developing interventions to minimizethe negative effects of social media disorder.

Denizden yüksekliği 1070metredir.Sultansuyu harası, güney ve güneydoğu illeri hayvanlarının onarılmasındave asil kanlı damızlık yetiştiriciliği yönünden son derece önemli bir konumdadır.Müessese 1928 yılında Yüksek Bakanlar Kurulu Kararı ile tekrar faaliyetegeçirilmiş, eski Sultansuyu Çiftliği arazisi ile binası ve binanın eklentileri harayadevr edilmiştir.Hara’nın tarihi süreç içerindeki temel görevi; Malatya’dan itibarenSuriye, Irak ve Kafkas hududuna kadar devam eden geniş bölgede bulunan yüzbinlerce at ile sayıları milyonlara çıkan sığırların ıslahı amacıyla gerekli olandamızlıkları yetiştirmekti. Önemli askeri ve iktisadi gayeleri gerçekleştirmek üzerekurulmuş olan bu Hara’nın, kısa bir zamanda kuruluşunu tamamlamak, faaliyetinitemin etmek ve aynı zamanda hazineye mümkün olduğu yük olmaması için, kendigeliri ile idaresi uygun görülmüştü. Abstract: Muscle weakness is a nonspecific finding of myopathy of any etiology that include iatrogenic, toxic, endocrinological, infectious, immunologic, and metabolic disorders. Among the metabolic myopathies glutaric aciduria type II (GAII) is an autosomal recessively inherited rare disorder of fatty acid and amino acid metabolisms. The late onset form is heterogeneous in terms of symptomatology and severity and for the cases that chronic manifestations of lipid storage myopathy are the only clues for the disease, differential diagnosis can be challenging. Here we report two cases of GAII: the first one was 18-year old boy who presented with proximal muscle weakness and in another center, he was diagnosed as polymyositis and treated with immunosuppressive therapies. He admitted to our clinic with ongoing muscle weakness and symptoms that were related to the side effects of immunosuppressive therapies. The second case was also presented with muscle weakness. For both cases, muscle biopsies and urinary organic acid analyses were consistent with the diagnosis of GAII. To differentiate inflammatory myositis from non-inflammatory myopathies; rheumatic symptoms, accompanying complaints of the patient and autoantibody positivity can be helpful. To our knowledge this is the first report to underline the differential diagnosis of inflammatory myopathies from metabolic myopathies.

Abstract: Hereditary spastic paraplegias (HSPs) are a group of genetic disorders resulting in pyramidal tract impairment, predominantly in lower limbs. KIF1C gene has recently been identified as one of the genetic causes of HSP and associated with pure or complicated HSP. We present three patients with complicated HSP from two unrelated families, who had early onset progressive cerebellar signs and developed pyramidal tract signs during follow-up. Whole exome sequencing in these patients followed by segregation analysis identified novel truncating KIFIC mutations (c.463C> T; p.R155* and c.2478delA; p.A1a828Argfs*13). Neuroimaging findings showed cerebral and upper cervical spinal atrophy, bilateral symmetrical pyramidal tract involvement, and focal cerebral white matter lesions. Patients with KIFIC mutations may present with cerebellar signs and pyramidal findings may emerge later, therefore complicated HSP should be considered in the differential diagnosis of unidentified cases with cerebellar dysfunction. Abstract: BackgroundCombined central and peripheral nervous system demyelination is a rare and poorly described phenomenon. Recently, anti-neurofascin antibodies were reported to be positive in 86% of these patients in a Japanese cohort. Yet, there seems to be a clinical, radiological, and serological heterogeneity among these patients. In this report, our aim is to describe characteristics of our patients with this entity and compare with others in the literature.MethodsWe report clinical, electrophysiological, radiological, and laboratory characteristics of five patients with both multiple sclerosis and chronic inflammatory demyelinating polyradiculoneuropathy from our institutional database containing 1890 MS patients.ResultsThree patients presented with extensive, active demyelination of both central nervous system and peripheral nervous system with hypertrophic peripheral nerves. Plexuses, trunks, division and cords were involved in the process. Oligoclonal band was negative. Conduction block was not detected. Corticosteroid treatment was not adequate. Others had a slowly progressive clinical course. Serum anti-neurofascin antibody was negative. Review of the literature revealed similar cases with active disease, early-onset hypertrophic peripheral nerves, and central demyelination, in addition to other cases with an insidious course.ConclusionsPatients with combined central and peripheral demyelination form a spectrum. Some patients may have an antibody-mediated syndrome with or without anti-neurofascin antibodies and others seem to represent a coincidence.

Abstract: The purpose of this study is to determine the predictive level of test anxiety and selective attention skills on test performances of adolescents, who are studying in public and private schools and took TEOG (Transition from Basic Education to Secondary Education) exam in 2015-2016. The sample of this study consists of 258 8th grade students in total, including 134 boys and 124 girls, between 13-14 ages. The data of the study were obtained from d2 Test of Attention, Test Anxiety Inventory and from TEOG exams. After examination of the results of the study, it has been indicated that there is a significant relationship between students’ performance on TEOG exams and selective attention skills. As the quality of student’s visual scanning accuracy, rule compliance and attention performance increase during the exam, test performance increases as well. Likewise, as the students’ negative thoughts about themselves and negative assessments about failing their exams increase, then test performance decreases., Bu araştırmanın amacı devlet ve özel okullarda öğrenim gören, 2015-2016 yılında TEOG (Temel eğitimden ortaöğretime geçiş) sınavına giren ergenlerin, sınav kaygısı ve seçici dikkat becerilerinin sınav performanslarını yordama düzeyinin belirlenmesidir. Araştırmanın örneklemini İstanbul ili Anadolu yakasında bulunan ortaokullar arasından tesadüfi örnekleme yöntemi ile belirlenen üç okulda öğrenim görmekte olan 13-14 yaş aralığındaki 134 erkek, 124 kız olmak üzere toplam 258 öğrenci oluşturmaktadır. Araştırmanın verileri d2 dikkat testi, Sınav Kaygısı Envanteri ve TEOG sınav sonuçları ile elde edilmiştir. Elde edilen sonuçlar incelendiğinde öğrencilerin TEOG performansı ile seçici dikkat becerileri arasında anlamlı ilişkinin olduğu görülmektedir. Öğrencilerin sınav esnasındaki görsel tarama doğruluğu, kurala uyumları ve dikkat performanslarının kalitesi arttıkça sınav performansları da artmaktadır. Yine aynı şekilde öğrencilerin kendileri hakkındaki olumsuz düşünceleri, sınav başarısızlığına ilişkin olumsuz değerlendirmeleri arttıkça sınav performansları azalmaktadır. Bununla birlikte öğrencilerin sınava ilişkin olumsuz değerlendirmeleri ve kuruntuları yoğunlaştıkça dikkat performanslarının kalitesinin de düştüğü belirlenmiştir.

Abstract: Spinal muscular atrophy (SMA) is one of the most common childhood onset neurodegenerative disorders in global health whereby novel biomarkers and therapeutic targets are sorely needed. SMA is an autosomal recessive genetic disorder resulting in degeneration of α-motor neurons in the brain stem and spinal cord that leads to mortality in infants worldwide. In majority of the patients, SMA is caused by homozygous deletion of the SMN1 gene. The clinical spectrum of the SMA displays, however, large person-to-person variations where the underlying mechanisms are poorly understood. We report in this study transcriptomics insights gleaned from patients with the severe type I (GM03813 and GM09677) and the mild type III. Pathway enrichment and functional analysis showed that especially extracellular matrix (ECM), synapse organization, and ECM receptor interaction pathways were affected. Among the neural ECM components, hyaluronan and proteoglycan link protein (HAPLN1), which is a key triggering molecule of the perineuronal net (PNN), was significantly downregulated in type I fibroblasts compared to type III. PNN is a specialized form of neural ECM around the neuronal cell bodies and dendrites in the central nervous system. In addition, we evaluated the PNN expression in vitro in a model established by SMN silencing in the PC12 rat pheochromocytoma cell line which can be differentiated into neurons with nerve growth factor treatment. In this neuronal in vitro model, we found that HAPLN1 showed a significant 50% decrease. Our results describe the association between PNN elements, especially HAPLN1, and SMA pathophysiology for the first time. These observations collectively inform future translational research on SMA for discovery of novel molecular targets for diagnostics and precision medicine innovation.

Abstract: Objective: ST6 beta-galactosamide α-2,6-sialyltranferase 1 (ST6GAL1), the major α2,6sialyltransferase responsible for the broad synthesis of glycoproteins and glycolipids, is another physiological substrate of Beta site APP-cleaving enzyme 1 (BACE1) other than amyloidbeta precursor protein (AbetaPP). We have previously shown that AbetaPP overexpression in C2C12 mouse myoblast cell line increased the expression and secretion of ST6GAL1 enzyme under in vitro conditions. Since the secretion of ST6GAL1 is known to be enhanced during inflammation, we investigated whether AbetaPP induced ST6GAL1 secretion from C2C12 cells affected proinflammatory cytokine production by J774 mouse macrophage cell line. Methods: J774 macrophage cells were cultured with conditioned medium derived from either AbetaPP or ST6GAL1 overexpressing C2C12 cells and analyzed for proinflammatory cytokine (tumor necrosis factor (TNF)-α, interleukin (IL)-1β and IL-6) expression by quantitative realtime PCR (qRT-PCR). Also, culture supernatants were analyzed for IL-1β release by ELISA. Results: The results of our study demonstrated that secretion of ST6GAL1 enzyme from either AbetaPP or ST6GAL1 overexpressing C2C12 cells induces the expression of TNF-α, IL-1β, IL-6 and the secretion of IL-1β by J774 macrophages. Conclusion: In our study, results were obtained pointing that secreted ST6GAL1 might have a potential role in the inflammation process observed in the muscle tissue. Our results should be confirmed under in vivo system.

Abstract: Trigeminal neuropathy is an infrequent condition, usually limited to the sensorial component of the nerve, and it is one of the most common neurologic manifestations of mixed connective tissue disease (MCTD). However, to the best of our knowledge, no cases have been reported with MCTD presenting as trigeminal sensory and motor neuropathy. Herein, we report a 28-year-old female with unilateral trigeminal sensory–motor neuropathy who presented with numbness over the distribution of the right trigeminal region and right masseter atrophy. We also reviewed the literature for trigeminal neuropathy associated with MCTD.

Abstract: Sensory neuronopathy is a well-established presentation in paraneoplastic neurological syndromes that is mostly associated with small cell lung cancer and anti-Hu antibodies. Motor neuronopathy, on the other hand, is an extremely rare observation in this syndrome. A 56-year-old man presented with asymmetric brachial diparesis and sensory ataxia. Electrophysiological studies revealed sensory ganglionopathy and progressive anterior horn degeneration in cervical segments. Small cell lung carcinoma with associated anti-Hu antibodies was later diagnosed. The patient did not improve despite the administration of steroids and chemotherapy. Paraneoplastic syndromes may exceptionally present with a bilateral arm weakness. Cases accompanied by sensory ganglionopathy should therefore be promptly investigated for any underlying malignancy.

Abstract: This study aimed to identify PYGM mutations in patients with McArdle disease from Turkey by next generation sequencing (NGS). Genomic DNA was extracted from the blood of the McArdle patients (n = 67) and unrelated healthy volunteers (n = 53). The PYGM gene was sequenced with NGS and the observed mutations were validated by direct Sanger sequencing. A diagnostic algorithm was developed for patients with suspected McArdle disease. A total of 16 deleterious PYGM mutations were identified, of which 5 were novel, including 1 splice-site donor, 1 frame-shift, and 3 non-synonymous variants. The p.Met1Val (27-patients/11-families) was the most common PYGM mutation, followed by p.Arg576* (6/4), c.1827+7A>G (5/4), c.772+2_3delTG (5/3), p.Phe710del (4/2), p.Lys754Asnfs (2/1), and p.Arg50* (1/1). A molecular diagnostic flowchart is proposed for the McArdle patients in Turkey, covering the 6 most common PYGM mutations found in Turkey as well as the most common mutation in Europe. The diagnostic algorithm may alleviate the need for muscle biopsies in 77.6% of future patients. A prevalence of any of the mutations to a geographical region in Turkey was not identified. Furthermore, the NGS approach to sequence the entire PYGM gene was successful in detecting a common missense mutation and discovering novel mutations in this population study. (C) 2017 Elsevier B.V. Abstract: Loanwords in the languages show the interaction between the different societies in the past. Therefore, a study of the Mongolian loanwords in Kipchak dictionaries will be helpful to understand the Turkish-Mongolian interaction. Although there are some studies about the Mongolian elements in the Turkish language, there is no mention of the Mongolian elements in the Kipchak dictionaries. This essay will discuss the Mongolian loanwords in Kipchak dictionaries, Dildeki alıntı sözcükler o dili kullanan milletin geçmişte iletişimde bulunduğu toplulukları gösterir. Bu bakımdan incelendiğinde Kıpçak sözlüklerindeki Moğolca alıntılar da Türk-Moğol ilişkisini yansıtması bakımından önem arz etmektedir. Türkçedeki Moğolca unsurlar zaman zaman incelenmiş olmasına rağmen Kıpçak sözlüklerindeki Moğolca alıntılar konusuna bir bütün olarak değinilmemiştir. Bu yazıda söz konusu eserlerdeki Moğolca olduğu düşünülen sözcükler üzerinde durulacaktır

Abstract: Bu araştırmanın amacı yaşlı bireylerin, bazı demografik değişkenlerle birlikte, yaşam doyumu, yaşamın anlamı ve yalnızlık düzeyleri arasındaki ilişkiyi incelemektir. Bu araştırmada, uygun / elverişli örnekleme yöntemiyle İstanbul ilinin Fatih ve Kağıthane semtlerinde yaşayan 60 yaş ve üzeri 96 katılımcıya (51 erkek, 45 kadın) "Yaşamın Anlamı Ölçeği", "Yaşam Doyum Ölçeği" ve "UCLA Yalnızlık Ölçeği" uygulanmıştır. Araştırmada yaşamın anlamı, yaşam doyumu ve yalnızlık düzeyleri puanları demografik değişkenlere göre farklılaşmamıştır (p>,05). Ancak yaşamın anlamı ve yaşam doyum düzeyleri arasında anlamlı bir ilişki bulunmuştur (r=,266; p,05). Abstract: Spinal Muscular Atrophy (SMA) is a neurodegenerative disease with autosomal recessive inheritance. Homozygous loss of exon 7 of the Survival of motor neuron 1 (SMN1) gene is the main cause of SMA. Although progressive muscle weakness and atrophy are common symptoms, disease severity varies from severe to mild. Type III is one of the milder and less frequent forms of SMA. In this study, we report molecular genetic characteristics of 24 Turkish type III SMA patients. Homozygous loss of SMN1 exon 7 and 8 was analysed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and multiplex ligation dependent probe amplification (MLPA). SMN2, homologue of SMN1, and Neuronal apoptosis inhibitory protein (NAIP) genes were also evaluated considering their influence on disease severity. We determined that male patients who were born in consanguineous families were predominant in our cohort and these patients mostly carry the homozygous loss of SMN1 exon 7 and 8 and four copies of SMN2 gene without NAIP deletions.
