4. Intranasal Ectopic Tooth in Adult and Pediatric Patients: A Report of Two Cases

The aim of this study was to report two rare cases of ectopic tooth into the nasal cavity. The first case reports a 32-year-old female patient with the main complaint of having a tooth inside her nose. According to her, this condition causes pain and sporadic nosebleed. The patient had a facial trauma when she was 6 years old. The second case refers to an 8-year-old girl with left incomplete unilateral cleft lip and palate. The main complaint was left-sided nasal obstruction by a white hard mass. The treatment for both cases was surgical removal of the ectopic erupted tooth under general anesthesia. In conclusion, we can state that the surgical removal of intranasal tooth is a safe procedure and improves patient’s quality of life.

This study aims to describe a method for controlling the sterilization of materials used by dentistry students and to compare its efficiency against the former control method. A software was developed by the Information Technology Management Department at the Positivo University, motivated by an internal demand to improve the sterilizing department control. The new method allows the placement of labels with bar codes on the external surface of the students’ boxes, controls the entire entrance-and-exit flow of materials and provides greater safety and agility for the sterilization process. Furthermore, the system allows professors to access the list of materials sterilized by students, and the date, time, and number of the autoclave in which the material was sterilized. Questionnaires including subjective questions about the new and former method of control were administered to final-year dentistry students to evaluate their satisfaction toward the methods. Introduction. Class II division I malocclusions are the most common dentofacial deformities seen in clinical practice. Severe cases or cases in which growth has ceased may require full correction combining orthodontic and surgical treatment. We report a case of a total mandibular subapical alveolar osteotomy, performed to correct a class II division I dentofacial deformity. Case Report. A 19-year-old female patient was referred to the oral and maxillofacial surgery department at the Federal University of Paraná with chin aesthetic complaints as well as class II malocclusion. The proposed treatment was total mandibular subapical alveolar osteotomy, retaining the chin position and eliminating the need for genioplasty, since, although the patient presented with a class II dentofacial deformity, the chin was well positioned. Under general anesthesia, a “V-shaped” incision was conducted from the right retromolar region to the left retromolar region. A ring of cortical bone was removed around the mental foramen, with the aim to create a space around the mental nerve. Fixation was conducted with plates and screws of the 2.0 system. The patient on six-year follow-up showed osteotomy stability, a better overall occlusion, and outcome satisfaction.

Há uma escassez de estudos longitudinais sobre o tema com critérios diagnósticos adequados e amostras significativas. O objetivo deste estudo foi avaliar longitudinalmente o efeito da cirurgia ortognática nos sinais e sintomas de DTM em indivíduos com perfil facial III. Materiais e Métodos: Foi realizado um estudo observacional prospectivo e longitudinal com indivíduos apresentando padrão de perfil facial III, que foram submetidos a cirurgia ortognática no Serviço de Cirurgia e Traumatologia Buco-maxilo-faciais da Universidade Federal do Paraná. Foram selecionados todos os indivíduos que apresentavam deficiência anteroposterior de maxila e/ou excesso anteroposterior de mandíbula, submetidos às técnicas de Osteotomia Sagital dos Ramos Mandibulares e/ou Osteotomia de Le Fort I, que aceitaram participar da pesquisa. O diagnóstico de DTM foi realizado de acordo com o eixo 1 do questionário da RDC-TMD por um examinador treinado, em três períodos distintos: pré-operatório de uma semana (T0), pós-operatório de seis meses (T1) e com 1 ano de pós-operatório (T2). Também foram avaliadas variáveis dos indivíduos (idade, gênero, raça, assimetria e discrepância maxilo-mandibular) e da cirurgia (segmentos operados e tipo de fixação utilizada na mandíbula). As variáveis de DTM foram classificadas de acordo com a manifestação ou não dos sinais e sintomas e análises bivariadas foram realizadas para verificar sua associação com as variáveis independentes. Foi assumido como significante um valor de p menor que 0,05 (IC 95%) para os testes estatísticos. Resultados: No período pré-operatório, a dor miofascial foi mais prevalente no gênero feminino (p=0,036) e nos indivíduos não caucasianos (p=0,043). Introduction: The rehabilitation of atrophic anterior maxilla can be done by different techniques. Among the procedures for bone augmentation, we can use block grafting, guided bone regeneration, and split crest technique (SCT). SCT consists in bone crest osteotomy, followed by manual/mechanical expansion up to the splitting of the buccal plate from the lingual/palatal plate through greenstick fracture. SCT advantage is the possibility of simultaneously installing a dental implant. However, SCT planning should consider the remaining bone width and the the flap type to obtain success. Objective: To report a case of implant-supported rehabilitation of an atrophic anterior maxilla using the split crest technique with insertion of 4 immediate implants, showing the effectiveness of the technique. Introduction: Osteonecrosis of the jaw is associated with defects in vascularization and with the use of oral bisphosphonates. Osseous exposition and infection may occur. Recommended treatment is variable, from antibiotic medication, bony decortication to resections of the mandible in severe cases. Reconstruction of mandible, in cases of resections is essential for maintaining esthetic profile and adequate form and function. Objective: To report a case about the dangers of oral bisphosphonates in association with invasive procedures such as dental implants Case report: Female patient, 64 years-old, with osteonecrosis of the jaw caused by use of oral bisphosphonates after rehabilitation with dental implants. She had an edentulous and atrophic mandible and poor healthy. After diagnosis, partial resection of the jaw was performed together with the reconstruction with titanium plate, with no success. Then, iliac bone graft fixed by plates and screws was attempted, again with no success. Conclusion: Despite of the small number of cases of osteonecrosis associated with oral bisphosphonate reported in the literature, a simple implant surgery could result in adverse consequences if the use of this medication were overlooked in the anamnesis. Agência 1 O objetivo deste estudo foi avaliar os fatores de risco clínicos, psicossociais e genéticos para depressão em indivíduos com deformidades dentofaciais (DFD) que necessitam de cirurgia ortognática. Um estudo transversal foi realizado em 143 indivíduos com má oclusão esquelética de classes II e III que necessitavam de cirurgia ortognática. Todos os pacientes foram diagnosticados para depressão, dor crônica, somatização e disfunção temporomandibular (DTM) de acordo com o Critério Diagnóstico em Pesquisa para DTM - Research Diagnostic Criteria for Temporomandibular Disorders (RDC / TMD). O DNA foi obtido de células bucais. Polimorfismos genéticos em DRD2 (rs6275 e rs6276) e ANKK1 (rs1800497) foram genotipados usando PCR em tempo real. Os dados foram analisados no programa SPSS (IBM, USA) v.23.0 e um nível de significância de 0,05 foi considerado. A prevalência de depressão foi maior em pacientes do sexo feminino e com excesso vertical de maxila (p < 0,05). O diagnóstico de depressão foi associado a dor crônica, somatização e dor miofascial (p 0,05). Concluímos que o gênero feminino, excesso vertical de maxila, dor crônica, somatização e dor miofascial são fatores de risco para depressão em pacientes com deformidade dentofacial que necessitam de cirurgia ortognática. The aim of this study was to evaluate clinical, psychosocial and genetic risk factors for depression in individuals with dentofacial deformities (DFD) who require orthognathic surgery. A transversal study was performed in 143 individuals with Class II and III skeletal malocclusion requiring orthognathic surgery. All patients were diagnosed for depression, chronic pain, somatization and temporomandibular disorders (TMD) according to the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). The DNA was obtained from buccal cells. Genetic polymorphisms in DRD2 (rs6275 and rs6276) and ANKK1 (rs1800497) were genotyped using real-time PCR. Data were analyzed using the SPSS program (IBM, USA) v.23.0 and a significance level of 0.05 was considered. The prevalence of depression was higher in female patients and with vertical excess of the maxilla (p 0.05). We concluded that the female gender, vertical excess of the maxilla, chronic pain, somatization and myofascial pain are risk factors for depression in patients with dentofacial deformity who require orthognathic surgery.

20. Medidas dentárias em pacientes com fissura labiopalatina: estudo caso controle

Introdução: Indivíduos com fissura labiopalatina (FLP), podem apresentar anomalias dentárias importantes. Objetivo: Comparar as medidas dentárias mesiodistais (MD) e vestíbulo-palatinas/linguais (VP e VL) em pacientes adultos com FLP e pacientes sem FLP. Materiais e métodos: A amostra desse estudo foi constituída de 146 indivíduos adultos, de ambos os sexos, sendo 73 do grupo caso (com FLP) e 73 do grupo controle (sem FLP). Dados clínicos como sexo e idade foram coletados. Para o grupo caso foi avaliado ainda o tipo da fissura. Além disso foi realizada a avaliação de modelos de gesso para a obtenção das medidas dentárias por examinadores calibrados. Foi considerada a máxima medida mesiodistal (MD) e vestibulopalatina (VP) ou vestíbulo lingual (VL) de todos os dentes permanentes. Os resultados obtidos foram submetidos à análise estatística, com nível de significância de 0,05. Resultados: Na arcada superior, os incisivos centrais (IC) se apresentaram menores no grupo caso para as medidas MD e VP quando comparados ao grupo controle (p < 0,05). Já os dentes incisivos laterais (IL) e caninos (C) se apresentaram menores apenas na distância VP (p < 0,05). Em relação ao tipo de fissura, o dente 11 foi menor nos pacientes que apresentam FLP do que pacientes somente com fissura labial (p < 0,05). Já na arcada inferior, houve diferenças apenas nas medidas VL entre os grupos, os IC e IL apresentaram medidas menores nos indivíduos com FLP, enquanto os dentes 36 e 44 se apresentaram maiores (p < 0,05). Conclusão: Indivíduos com FLP apresentam tamanhos diferentes de determinados dentes quando comparados a indivíduos sem FLP. Adicionalmente, o tipo de fissura está associado com o tamanho dos incisivos centrais superiores na maxila. Introduction: Individuals with cleft lip and palate (CLP) may present important dental anomalies. Objective: This study aims to compare mesiodistal (MD) and vestibulopalatal/lingual (VP/L) dental measurements in adult patients with CLP and patients without CLP. Materials and methods: The sample of this study consisted of 146 adult individuals, of both sexes, in which 73 of them were from the case group (with CLP) and 73 were from the control group (without CLP). Clinical data such as sex and age were collected. For the case group, the type of cleft was also evaluated. In addition, plaster models were evaluated to obtain dental measurements by calibrated examiners. The maximum mesiodistal (MD) and vestibulo-palatal/lingual (VP/L) measurement of all permanent teeth was considered. The results obtained were submitted to statistical analysis, with a significance level of 0.05. Results: In the upper arch, the central incisors (CI) were smaller in the case group for the MD and VP measures when compared to the control group (p < 0.05). The lateral incisors (LI) and canine (C) teeth were smaller only in the VP distance (p < 0.05). Regarding the type of cleft, tooth 11 was smaller in patients with cleft lip and palate than in patients with cleft lip only (p < 0.05). In the lower arch, there were differences only in the VL measurements between the groups, the CI and LI presented smaller measurements in individuals with CLP, while teeth 36 and 44 were larger (p < 0.05). Conclusion: Individuals with CLP have certain teeth in different sizes when compared to individuals without CLP. Additionally, the type of cleft is associated with the size of the upper maxillary central incisors.

As deformidades dentofaciais se caracterizam por alterações no desenvolvimento craniofacial que afetam a parte esquelética e dentária do indivíduo, muitas vezes causando comprometimento funcional e estético. O desenvolvimento craniofacial é influenciado por diversos fatores, sejam eles genéticos ou ambientais. A literatura tem associado polimorfismos genéticos com o crescimento facial, inclusive polimorfismos dos genes dos receptores de estrogênio 1 (ESR1) e receptores de estrogênio 2 (ESR2). O objetivo deste trabalho foi avaliar possíveis associações entre polimorfismos dos genes ESR1 (rs2234693 e rs9340799) e ESR2 (rs1256049 e rs4986938) e medidas cefalométricas em indivíduos com deformidades dentofaciais. Foram selecionados indivíduos em preparo pré-operatório para cirurgia ortognática na Universidade Federal do Paraná e na Universidade Positivo. As medidas cefalométricas de todos os pacientes foram obtidas por meio de traçados cefalométricos realizados a partir da telerradiografia de perfil no software Dolphin 2D. Para a análise genética, o DNA dos pacientes foram extraídos de células epiteliais da mucosa bucal e foram genotipados através da técnica de PCR em tempo real (Applied Biosystems 7500 Real-Time PCR System), com o uso da tecnologia TaqMan™ (Applied Biosystems). Os dados encontrados foram submetidos à análise estatística por meio dos testes de Kolmogorv-Smirnov, MannWhitney e Kruskall-Wallis, utilizando o software IBM SPSS versão 24.0. Foi considerado nível de significância de 0,05. O polimorfismo rs1256049 mostrou associação com as medidas cefalométricas Altura Facial Posterior (S-GO) (p = 0,038) e Altura Facial Anterior (N-Me) (p = 0,039). E o polimorfismo rs9340799 mostrou associação com a medida cefalométrica Ângulo Goníaco Superior (Ar-Go-N) (p = 0,024). As demais medidas não tiveram associação com os polimorfismos estudados. Dentofacial deformities are characterized by changes in craniofacial development that affect the skeleton and dental part of the individual, often causing functional and aesthetic impairment. Craniofacial development is influenced by several factors, whether genetic or environmental. The literature has associated genetic polymorphisms with facial growth, including polymorphisms of the Estrogen Receptor 1 (ESR1) and Estrogen Receptor 2 (ESR2) genes. The objective of this work was to evaluate possible associations between polymorphisms of the ESR1 (rs2234693 and rs9340799) and ESR2 (rs1256049 and rs4986938) genes and cephalometric measurements in individuals with dentofacial deformities. Individuals in preoperative preparation for orthognathic surgery were selected at Federal University of Paraná and Positivo University. The cephalometric measurements of all patients were obtained by means of cephalometric tracings performed from the profile teleradiography in the Dolphin 2D software. For genetic analysis, patients' DNA was extracted from oral mucosa epithelial cells and was genotyped using the real-time PCR technique (Applied Biosystems 7500 Real-Time PCR System), using TaqMan ™ technology (Applied Biosystems). The data found were submitted to statistical analysis, through the Kolmogorv-Smirnov, MannWhitney and Kruskall-Wallis tests, using the IBM SPSS software version 24.0. It was considered a significance level of 0.05. The rs1256049 polymorphism showed an association with the cephalometric measurements Posterior Facial Height (S-GO) (p = 0.038) and Anterior Facial Height (N-Me) (p = 0.039). And the rs9340799 polymorphism showed an association with the Upper Gonial Angle (Ar-Go-N) cephalometric measure (p = 0.024). The other measures were not associated with the studied polymorphisms.

Introdução: A cirurgia ortognática é uma modalidade de tratamento bem estabelecida, que gera um efeito positivo na qualidade de vida (QV) associada a saúde bucal. Objetivo: Avaliar a associação entre a evolução da percepção da QV relacionada a saúde bucal após a cirurgia ortognática com polimorfismos genéticos. Materiais e métodos: Foram incluídos nesse estudo 112 pacientes adultos, de ambos os sexos, com deformidade dentofacial (DDF) que foram submetidos a cirurgia ortognática. Todos os pacientes foram avaliados no pré-operatório e no pósoperatório de 6 meses da cirurgia. Dados clínicos como sexo e perfil facial foram coletados. Além disso foi aplicado o Oral Health Impact Profile (OHIP-14) e realizado a coleta de células da mucosa bucal para extração do DNA. Os genes, COMT (rs165656 e rs174675), HTR2A (rs6313 e rs4941573) e MTNR1A (rs13140012, rs6553010 e rs6847693) foram genotipados por técnica de PCR em tempo real para os respectivos polimorfismos. Os resultados obtidos foram submetidos à análise estatística, com nível de significância de 0,05. Resultados: Quando comparado a evolução do OHIP-14 com polimorfismos genéticos, houve associação do COMT com o domínio de incapacidade psicológica no polimorfismo rs165656 (p = 0,042), indicando que indivíduos do genótipo CC e GC apresentam piora na evolução da QV associada a saúde bucal quando comparados a indivíduos GG. Além disso, houve associação do gene HTR2A no polimorfismo rs6313 e rs4941573 com o domínio de incapacidade social (p = 0,019; p = 0,038, respectivamente). Houve associação entre o MTNR1A e o domínio de desvantagem social do rs165656 (p = 0,042), apontando que esse gene pode estar associado a uma piora da percepção da QV associada a saúde bucal após a cirurgia ortognática. Conclusão: Fatores genéticos estão envolvidos na QV associada a saúde bucal. Os genes COMT, HTR2A e MTNR1A estão associados a percepção da QV associada a saúde bucal nos domínios referentes a incapacidade psicológica, incapacidade social e desvantagem social. Introduction: Orthognathic surgery is a well-established treatment modality that has a positive effect on quality of life associated with oral health (OHRQoL) Objective: To evaluate the association between the improvement in the perception of OHRQoL after orthognathic surgery and genetic polymorphisms. Materials and methods: This study included 112 adult patients, of both sexes, with dentofacial deformity (DFD) who underwent orthognathic surgery. All patients were assessed preoperatively and 6 months after surgery. Clinical data such as sex and facial profile were collected. In addition, the Oral Health Impact Profile (OHIP-14) was applied and oral mucosa cells were collected for DNA extraction. The COMT (rs165656 and rs174675), HTR2A (rs6313 and rs4941573) and MTNR1A (rs13140012, rs6553010 and rs6847693) genes were genotyped with real-time PCR for the respective polymorphisms. The results obtained were submitted to statistical analysis, with a significance level of 0.05. Results: When comparing the evolution of OHRQoL with genetic polymorphisms, there was an association of COMT with the psychological disability domain in the rs165656 polymorphism (p = 0.042), indicating that individuals of the CC and GC genotype have worse OHRQoL evolution associated with oral health when compared to GG individuals. Besides that, of the HTR2A gene in the rs6313 and rs4941573 polymorphism with the social disability domain (p < 0.019; p < 0.038 respectively). There was an association between MTNR1A and the social handicap domain in the recessive G model of rs165656 (p < 0.042). Conclusion: Genetic factors are involved on the OHRQoL. The COMT, HTR2A and MTNR1A genes are associated with the perception of OHRQoL in the domains related to psychological disability, social disability and social handicap.

23. Variation of Tooth Crown Size in Cleft Lip and Palate Patients.

Aim: This study aims to compare the mesiodistal (MD) and buccolingual (BL) tooth crown size (TCS) of adult patients with cleft lip and palate (CL/P) and patients without CL/P., Materials and Methods: The sample of this study consisted of 146 adult patients, of both genders, of which 73 were included in the case group (with CL/P) and 73 were included in the control group (without CL/P). Data regarding gender and age and cleft type were collected. In addition, dental models were evaluated to obtain the TCS in the maximum distance of the MD and BL dimensions of all erupted permanent teeth (except third molars). The results were submitted to statistical analysis with a significance level of 0.05., Results: In the upper arch, the central incisors (CI) were smaller in the case group for the MD and BL dimensions ( p < 0.05). The lateral incisors (LI) and canine (C) were smaller only in the BL width ( p < 0.05) and the second molars (SM), were smaller only in the MD dimensions. In the lower arch, there were significant differences only in the BL width between groups, the CI and LI presented smaller measurements in CL/P patients, while the left first molar (FM) and right first premolar (FPM) were larger ( p < 0.05) than in patients without CL/P., Conclusion: Patients with CL/P have different sizes in certain teeth compared to patients without CL/P., Clinical Relevance: Cleft lip and palate patients usually present important dental anomalies; thereby, the knowledge about trends in tooth size variations in CL/P patients can aid in dental and orthodontic treatment planning to obtain a stable, functional, and esthetic occlusion.
