The article analyzes the results of the All-Russian competition for general practitioners, family doctors and internists held over five years by the National Medical Research Center for Therapy and Preventive Medicine with the support of the Russian Society for the Prevention of Non communicable Diseases.Aim. To determine the methodological characteristics of managing the academic competition as a type of informal education.Material and methods. To conduct the competition, practice-oriented tasks corresponding to professional tasks of a general practitioner were developed. In addition, we developed an original questionnaire to assess the impact of participation in the academic competition on the professional development. Family doctors and general practitioners with 1-10 years of experience, as well as internists, were included.Results. The methodological characteristics of the academic competition management as a type of informal education were determined: practice-oriented goals aimed at improving the knowledge, skills and abilities, competencies of the participants; the content of tasks based on the theory-practice relationship; compliance of tasks and real situations in the practice of a general practitioner; a systematic approach to competition management. Conclusion. The effectiveness of the academic competition as a type of informal education contributes to the professional development of participants, promotes the development of long-term attitudes towards improving professional knowledge, skills, abilities and competencies necessary for solving professional problems.

Aim. To assess the prognostic value of current scales and indices for risk stratification of any surgical and cardiovascular complications (CVC) in patients undergoing non-cardiac surgical interventions.Material and methods. This single-center cohort retrospective study was conducted in patients who underwent non-cardiac surgery in 2018 and 2020. Surgical postoperative complications (POCs) were assessed according to the Clavien-Dindo classification. CVCs included any cardio-vascular events (CVEs), major adverse cardiac events (MACE), ST-T abnormalities on the electrocardiogram (ECG), decompensated heart failure (HF), arrhythmias, episodes of hypotension or hypertension, delirium, bleeding, thromboembolic events (TEEs). Risk stratification of POCs was carried out using recommended prognostic scales and indices. Their prognostic significance was assessed using ROC analysis with assessment of the area under the curve (AUC).Results. POC risk stratification was performed in 2937 patients. In assessment of total risk of Clavien-Dindo surgical complications and any CVEs, none of the scales showed a predictive value of >0,7.Conclusion. Any CVE stratification requires improvement of current tools and development of novel prognostic tools.

One of the key tasks facing the medical community and health professionals is the identification of patients at high risk of hospitalization, including emergency one. Currently, various approaches are used to obtain information about certain predictors in a patient that are as­sociated with an increased hospitalization risk, but there is no consensus on which of them are most effective.Aim. To summarize data from studies that has analyzed the prognostic value of various predictors used in questionnaires or patient surveys associated with an increased hospitalization risk.Material and methods. We search for full-text articles in 8 Russian and foreign electronic databases (Scopus, PubMed, BMC Health Services Research, BMJ Research, Google Scholar, e-library, Oxford Academic, Wiley Online Library), published in the period from January 1, 1993 to January 1, 2023 and containing information about questionnaires on hospitalization risk prediction. The study was carried out in 4 following stages: 1 — search for articles using keywords in electronic databases; 2 — duplicate elimination; 3 — search for full-text versions of articles and their assessment for compliance with inclusion/ exclusion criteria; 4 — data analysis, systematization of hospitalization predictors used in questionnaires.Results. In total, out of 28133 initial publications, 13 full-text papers describing studies that fully met the inclusion/exclusion criteria were admitted to stage 4 and were included in the final analysis. These studies were devoted to the description of 11 questionnaires, their modification or testing. The majority of the articles found (53,8%) were conducted in the United States. When analyzing questionnaires to assess the hospitalization risk, among 61 identified predictors, the following were most often used In questionnaires: "age ≥75 years"; "≥1 hospitalization within one to two years preceding filling out the questionnaire"; "taking ≥5 prescription medications"; "male sex"; "history of diabetes", and "satisfactory/unsatisfactory self-perceived health status". A more detailed analysis of questionnaires to assess the risk of one-year hospitalization among people ≥65 years old found that the following predictors were most often used: "age ≥75 years", "≥1 hospitalization in the period from one to two years before filling out the questionnaire", and "taking ≥3 prescription medications".Conclusion. Evidence is summarized on predictors used in questionnaires on hospitalization risk assessment. Additional research is needed to validate established predictors in the Russian population, their cause-and-effect relationship, and to search for novel predictors influencing the hospitalization risk among the working age and post-working age population.

Aim. To determine differences in laboratory coagulation parameters using routine and integral monitoring methods in patients depending on anticoagulant therapy >12 months after pulmonary embolism (PE) with a high recurrence risk.Material and methods. The study included 72 patients who, according to indications, received long-term anticoagulant therapy >12 months due to a high risk of recurrent thromboembolism. During the follow-up period (15-20 months from the first pulmonary embolism episode), the plasma coagulation parameters were studied using a conventional panel and D-dimer assessment, as well as a thrombin generation test and thrombodynamics. Patients adherent to prolonged anticoagulation were included in group 1. During the collection period, 8 patients independently stopped taking anticoagulants, despite the high risk of recurrent pulmonary embolism (group 2).Results. There were no significant differences in coagulation parameters and D-dimer levels between the studied groups of patients. In the group of patients continuing to take anticoagulants, there was an increase in clot growth delay parameters in comparison with patients of group 2 according to the thrombodynamics results (p=0,046) and the thrombin generation test in platelet-poor plasma in comparison with reference values (p=0,001).Conclusion. Long-term anticoagulant therapy effectively prevents recurrent venous thromboembolism. In patients after primary pulmonary embolism who refuse anticoagulation, the clot growth delay values are shortened, which indicates an increased risk of recurrent venous thromboembolism. The most stable plasma coagulation parameters are observed in patients taking direct oral anticoagulants.

Aim. To evaluate any cardiovascular abnormalities in the postoperative period, their prevalence and impact on the course of postoperative period in patients after non-cardiac surgery, which underwent postoperative examinations of cardiovascular system.Material and methods. The study included 2937 patients. The assessed end points were postoperative cardiovascular events (CVEs). Along with major adverse cardiovascular events (MACE), we analyzed any cardiovascular abnormalities in the postoperative period as follows: electrocardiographic ST-T abnormalities, acute or decompensated heart failure (HF), arrhythmias, episodes of hypotension or hypertension, bleeding, thromboembolic events (TEEs), cerebrovascular events, postoperative delirium. Any CVEs constituted the any cardiovascular events' (ACVEs) group. Statistical analysis was carried out using the StatTech v program. 3.1.6 (OOO Stattekh, Russia).Results. Any postoperative complications according to the Clavien-Dindo classification were 54,7%, while grade 1 — 33,9%, grade 2 — 20,3%, grade 3 — 1 case, grade 4 — 0,4%, grade 5 — 0,1%. MACEs developed in 0,2% of cases. ACVEs amounted to 13,3%, including 2,3% — ST-T abnormalities, 6,3% — significant systolic blood pressure changes, 2,6% — arrhythmias, 0,7% — HF, 2,4% — bleeding, 1,1% — TEEs, 0,3% — delirium. In addition, 67 (17,5%) patients had ≥2 CVEs. Comparison of the length of hospital stay of patients with ACVEs, in particular with MACEs, ECG ST-T abnormalities, HF, arrhythmia, episodes of hypotension or hypertension, bleeding, TEEs, cerebrovascular events, delirium, with groups of patients without such complications revealed significant differences. ACVEs accounted for 1/4 of all postoperative complications and 2/3 of complications of grades 2-5.Conclusion. We expanded the concept of CVEs, united the totality of any cardiovascular abnormalities in the postoperative period, emphasized not only their clinical significance, but also the economic feasibility of taking into account ACVEs.

Group I (n=106) included persons who received a homologous revaccination with Gam-COVID-Vac, while group II (n=54) — heterologous revaccination with Gam-COVID-Vac and Covivac, group III (n=40) — homologous Covivac revaccination. Coronavirus disease 2019 (COVID-19) pandemic was not only a serious challenge for the healthcare system around the world, but also an incentive for intensive research and development for the introduction of innovative technologies and drugs, in particular vaccines against the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-COV-2). The first vaccination campaigns provided significant protection against severe disease course and hospitalization. However, new SARS-COV-2 variants require further comprehensive research and the introduction of booster vaccination. Booster vaccination is the most important tool for immunostimulation and increase of protection duration against the severe disease course. The study of booster vaccines, including BioNTech/Pfizer, Moderna, Oxford AstraZeneca, Sputnik V, Sinopharm и Covaxin vaccines, sheds light on their unique action mechanisms and contribution to long-term immunity. The analysis of clinical data demonstrates their effectiveness and safety. The review summarizes modern knowledge about booster vaccinations against the COVID-19 with focus on action mechanisms and efficiency. In addition, the immune system function in response to COVID-19 is considered, while the role of memory cells, antibody and cellular immunity reactions are emphasized.

The effectiveness of humoral and cell-mediated immunity as a result of primary vaccination for Severe Acute Respiratory Syndrome- related CoronaVirus 2 (SARS-CoV-2), as well as the effectiveness of revaccination, is an important research problem. Studying and selecting optimal revaccination regimens will allow for long-term protection against coronavirus disease 2019 (COVID-19).Aim. To study the severity of humoral and cell-mediated immune response in various (homo- and heterologous) SARS-CoV-2 revaccination regimens as part of a prospective observational study 18 and 24 months after primary vaccination.Material and methods. The study was carried out within the prospective registry SATURN, which included individuals who received various SARS-CoV-2 revaccination (homo- and heterologous regimens) based on a combination of two following vaccines: Gam- COVID-Vac and CoviVac. Depending on the chosen regimen, 3 following groups were formed: group I (n=106) — participants with a homologous Gam- COVID-Vac regimen at each stage of vaccination and revaccination; group II (n=54) — participants with a heterologous regimen of sequential administration of Gam- COVID-Vac and CoviVac at the stage of vaccination and revaccination; group III (n=40) — participants with a homologous CoviVac regimen at the stage of vaccination and revaccination. At the first visit, all participants underwent a medical history collection, examination, and identification of potential contraindications to vaccination. At each subsequent visit, the level of anti- SARS-CoV-2 S-glycoprotein IgG antibodies was additionally determined. At visits 1, 3 and 5, the activity of specifically sensitized T-lymphocytes to the surface and nuclear antigen of SARS-CoV-2 was assessed. The IgG concentration was analyzed using the Abbott Architect SARS-CoV-2 IgG reagent kit, while T-cell immunity was assessed using the T-Spot.COVID test system (Oxford Immunotec). Visit 1 corresponded to the 1st stage of primary vaccination, visit 2 — 2nd stage of primary vaccination, visit 3 (12 months after visit 1) — 1st stage of revaccination, visit 3 (21 days after visit 3) — 2nd stage of revaccination, visit 5 –18 months after primary vaccination, visit 6 — 24 months after primary vaccination.Results. The average level of anti- SARS-CoV-2 S-glycoprotein IgG antibodies in group I at visit 3 was 520 [478; 540] BAU/ml, in group II — 499 [199,5; 540] BAU/ml and in group III — 456 [389; 509,5] BAU/ml. The values were comparable to those obtained at visit 6. However, in group II, IgG level at visit 5 was significantly higher compared to visit 6 (p=0,001). The greatest decrease by visit 5 in the number of active T cells responding to SARS-CoV-2 Spike stimulation was recorded in group III (5,00 [0,50; 11,50] vs 1,00 [0,00; 5,50]; relative changes -80,0%; p=0,067). Also, among patients in group III, there was a significant decrease in the number of active T cells responding to stimulation with SARS-CoV-2 nucleocapsid (10,00 [3,00; 22,50] vs 1,00 [0,00; 11,50]; relative changes -90,0%; p=0,0160). Participants in groups I and II demonstrated stable results on visit 5, relative to visit 3. In all studied groups, no significant relationship was found between IgG levels to SARS-CoV-2 S-glycopeptide and the number of active T cells. Introduction. The development of drugs against SARS-CoV-2 continues to be crucial for reducing the spread of infection and associated mortality. The aim of the work is to study the neutralization of the SARS-CoV-2 virus with interferon gamma preparations in vitro. Materials and methods. The activity of recombinant human interferon gamma for intramuscular and subcutaneous administration of 500,000 IU and for intranasal administration of 100,000 IU against the SARS-CoV-2 virus in vitro was studied. The methodological approach of this study is based on the phenomenon of a decrease in the number of plaques formed under the action of a potential antiviral drug. Results. The antiviral activity of recombinant interferon gamma has been experimentally confirmed, both in preventive and therapeutic application schemes. The smallest number of plaques was observed with the preventive scheme of application of the tested object at concentrations of 1000 and 333 IU/ml. The semi-maximal effective concentration (EC50) with the prophylactic regimen was 24 IU/ml. Discussion. The preventive scheme of application of the tested object turned out to be more effective than therapeutic one, which is probably explained by the launch of the expression of various interferon-stimulated genes that affect to a greater extent the steps of virus entry into the cell and its reproduction. Conclusion. Further study of the effect of drugs based on recombinant interferon gamma on the reproduction of the SARS-CoV-2 virus for clinical use for prevention and treatment is highly relevant.

Recently, two following demographic trends have been clearly obser­ved in the world: population aging and an increase in the prevalence of obesity. In February 2022, the European Society for Clinical Nutrition and Metabolism (ESPEN) and the European Association for the Study of Obesity (EASO) published the first consensus guidelines for the definition and diagnosis of sarcopenic obesity, which characterizes by a combination of excess adipose tissue with a decrease in muscle mass, muscle strength and impaired muscle function. The purpose of this review is to describe the prevalence, the main etiopathogenetic factors of sarcopenic obesity, as well as currently accepted approaches to the diagnosis of this condition.

Polycythemia vera is not only a clonal disease that causes hematopoietic stem cells, but also a pathology that often leads to thrombotic complications. Thrombosis can have different localization and is clinically manifested by stroke, myocardial infarction, deep vein thrombosis of the lower extremities, pulmonary embolism, thrombosis of the veins of internal organs and other conditions. One of the most formidable thrombotic complications is acute cerebrovascular accident. The heterogeneity of the possible causes of acute cerebrovascular accident requires a careful approach to differential diagnosis for timely diagnosis and individual, pathogenetically grounded selection of means of long-term antithrombotic therapy. The presented clinical case of the development of cerebrovascular accident in a patient with polycythemia vera demonstrates the importance of an informal approach to diagnosis, as well as interdisciplinary interaction for finding the true cause of the development of acute cerebrovascular accident and the appointment of pathogenetically based treatment, aimed, among other things, at the prevention of repeated episodes of acute cerebrovascular accident and others. thrombotic complications.

Aim. To evaluate the prevalence of oral anticoagulant therapy at the outpatient and inpatient stages in patients hospitalized with ischemic stroke.Material and methods. This open observational prospective real-world study included 114 patients with atrial fibrillation (AF) hospitalized with confirmed stroke.Results. To compare the immunogenicity (humoral and cellular immunity) of various (heterologous and homologous) revaccination regimens against coronavirus disease 2019 (COVID-19) in a prospective observational study.Material and methods. In individuals aged ≥18 years, in the absence of contraindications to vaccination, two following types of vaccines were used for primary vaccination and revaccination: Gam-COVIDVac and CoviVac. The review presents up-to-date information on the physiological and pathophysiological aspects of short-term middle-altitude adaptation in healthy individuals and patients with various chronic diseases. Unlike acute mountain sickness, which develops going to ≥3000, the physiological aspects of human adaptation to 2000-2500 m remain insufficiently studied. However, these altitudes are the most visited among tourist groups and individually.

Aim. To study the features of the use of thrombolytic therapy (TLT) in normotensive patients with pulmonary embolism (PE) in real clinical practice in Russian hospitals.Material and Methods. From 04/1 5/2018 to 04/15/2019 patients hospitalized with a diagnosis of PE consistently were included in the Russian multicenter observational prospective register "SIRENA” (RusSIan REgistry of pulmoNAry embolism).Results. For 12 months in the registry was included 609 patients with a lifetime confirmed diagnosis of PE. TLT was performed in 152 patients with PE (25.0%), of which only 51 (33.8%) were indicated as "high risk" (shock or hypotension). In 101 not high risk patients, the indications for TLT were: severe shortness of breath/respiratory failure - 19 (18.8%), massive venous thrombosis - 7 (6.9%), signs of massive/submassive PE - 10 (9.9%), intermediate-high risk - 14 (13.9%), suspicion of acute coronary syndrome with ST segment elevation - 3 (2.9%), high pulmonary hypertension -2 (2.0%). The other 46 (45.5%) non-high-risk patients had no clear indication of the reasons for TLT in their medical history. To study the features of management of patients with not high-risk PE who received TLT (group 1), a selection of pairs of patients from the "SIRENA” registry, comparable in gender and age, in a ratio of 1:1 of patients with not high-risk PE who did not perform TLT (group 2). Hospital mortality was 4 (4%) patients in the TLT group and 6 (5.9%) patients in group 2 (р=0,748). The ongoing pandemic of coronavirus disease 2019 makes it important to study the immunogenicity, the duration of immune response, and the safety of existing vaccines.Aim. As part of a prospective observational study, to assess associations between levels of anti-Severe Acute Respiratory Syndrome CoronaVirus 2 (SARS-CoV-2) S protein IgG antibodies and thrombodynamics parameters in individuals vaccinated with Gam-COVID-Vac and CoviVac vaccines.Material and methods. The study included 137 people who completed the first 3 visits: 30 people received the Gam-COVID-Vac, 107 people — CoviVac. The study participants underwent venous blood sampling before the introduction of the 1st and 2nd vaccine doses, as well as 42 days after the administration of 1st dose in order to quantify the level of IgG antibodies. At each visit, plasma hemostasis parameters were analyzed using a thrombodynamics test.Results. During the follow-up period, there was a clear increase in the level of anti-SARS-CoV-2 S protein IgG antibodies in both groups. At the same time, this increase over time was significantly greater in Gam-COVID-Vac group. There was no correlation detected between thrombodynamics test results and the levels of anti-SARS-CoV-2 S protein IgG antibodies.Conclusion. The data obtained demonstrate the ability of both vaccines to stimulate the production of anti-SARS-CoV-2 antibodies. However, immune response to Gam-COVID-Vac is much higher. The lack of correlation between thrombodynamics and the level of specific antibodies suggests that vaccination for COVID-19 with Gam-COVIDVac and CoviVac does not cause changes in plasma hemostasis and does not increase the risk of thrombosis.

Preliminary data obtained in studies of properties of recombinant interferon gamma (IFN-γ) that involved patients with community-acquired pneumonia (including bacterial), acute respiratory viral infection (ARVI), influenza and new coronavirus infection have shown promising results. The purpose of the study was to assess the effect of subcutaneous administration of IFN-γ in patients with viral pneumonia on the changes of vital signs and the duration of hospital stay. Material and methods. An open-label, randomized, low-interventional study included patients with moderate new coronavirus infection COVID-19 over 18 years of age of both sexes. IFN-γ 500,000 IU was administered s/c, daily, once a day, during 5 days. Results. IFN-y in addition to complex therapy of the disease resulted in more favorable changes in the stabilization of vital signs, as well as in reduced length of fever and hospital stay by 2 days what allows suggesting a positive effect of this substance on the recovery processes in patients with moderate COVID-19. Special emphasis should be made to the fact that patients who received recombinant IFN- γ experienced no progression of respiratory failure and required no transfer to intensive care unit. Discussion. This study confirms earlier obtained data on the positive effect of IFN-y on the rate of clinical stabilization and recovery of patients with community-acquired pneumonia and viral infections. Presented results are limited to a small number of patients; further study of drug properties in post-marketing studies is required. Conclusion. Progress in the treatment of patients with moderate COVID-19 by adding recombinant IFN-γ to the complex therapy may reasonably expand the range of existing treatment options for this infection.

Aim. To evaluate the incidence and severity of acute kidney injury (AKI), as well as its ability to reclassify the risk of premature mortality and association with inhospital mortality in patients with pulmonary embolism (PE) in the Russian population.Material and methods. From April 2018 to April 2019, the SIRENA Russian Multicenter Registry included patients with PE, as well as the deceased, in whom the PE was detected by autopsy. AKI was diagnosed according to current KDIGO guidelines (2012). Creatinine calculated according to the MDRD equation and corresponding to a glomerular filtration rate of 75 ml/min/1,73 m2 (baseline) was taken as the initial one, with subsequent assessment relative to the parameter value upon admission. The risk stratification of early death was carried out in accordance with the current ESC clinical guidelines (2019).Results. A total of 604 patients with PE were examined (men — 293 (49%), women — 311 (51%), mean age — 64±15 years). AKI was diagnosed in 223 (37%) of them. Stage 1 AKI was detected in 146 (65%), 2 — in 55 (25%), 3 — in 22 (10%) patients. Prior chronic kidney disease was recorded in 61 (10%) patients. Aim. To study the blood coagulation status by various laboratory methods in patients after pulmonary embolism (PE) receiving long-term anticoagulant therapy.Material and methods. The blood of 23 patients with pulmonary embolism, who received long-term anticoagulant therapy, was studied. The study of coagulation profile, D-dimer, thrombodynamics, thromboelastography and thrombin generation test were carried out.Results. The thrombin generation test shows a significant increase in the time of its formation, while the maximum amount of thrombin formed is half that of the reference values. There is a slightly increased median fibrin clot growth rate in the thrombodynamics test — 30,4 gm/min with a normal coagulation rate of 20-29 gm/min. The result of thromboelastography also reflects the blood hypocoagulation, in terms of R, Angle a and CI.Conclusion. Integral methods for assessing the thrombotic readiness in combination with a routine coagulation panel demonstrate a complete picture of blood coagulation potential in patients after pulmonary embolism requiring long-term anticoagulant therapy.

To assess the prevalence, severity and prognostic value of renal dysfunction (RD) in patients with pulmonary embolism (PE) of the Russian population, as well as to determine the RD significance as a marker that improves the predictive ability of current risk stratification systems.Material and methods. From April 2018 to April 2019, patients hospitalized due to PE were sequentially included in the Russian multicenter observational prospective registry SIRENA. Aim. To study the association between a positive results of Heart-type fatty acid binding protein (h-FABP) test with the development of adverse outcomes during the year of observation in patients with non-ST elevation acute coronary syndrome (NSTE-ACS) depending on the type 2 diabetes mellitus (DM-2) presence.Material and methods. The study included 153 patients with NSTEACS hospitalized in the department of emergency cardiology in the period from March 2014 to January 2017. In addition to the standard examination, all patients underwent a qualitative analysis on the h-FABP in venous blood. Results. Kaplan–Meier estimator demonstrated a significant probability of a combined end point in patients with positive h-FABP, both with and without DM-2. H-FABP also acts as a marker for the development of adverse outcomes during the year after NSTE-ACS. Patients with adverse outcomes and type 2 diabetes mellitus had the highest frequency (100%) of positive test reactions.Conclusion. H-FABP positive test when admitted to hospital may be considered as a predictor of adverse long-term outcomes in patients with NSTE-ACS. That statistics is most significant for patients with DM-2.

Purpose: to identify the factors associated with the development of death events during the year follow-up after hospitalization for pulmonary embolism (PE). Materials and methods: 93 patients with PE discharged to the outpatient stage of observation were studied. 45 (61,6%) patients were female with an average age of 66 years. The examination of patients at the stage of inclusion in the study consisted of standard methods of examination for this pathology. The diagnosis was confirmed by multislice computed tomography. Follow-up was 12 months. Statistical analysis was performed using the MedCalc Version 16.2.1. Results: during the one-year follow-up period 62 (66,7%) patients with PE were alive but 11 patients (11,8%) died, and no information was obtained about 20 patients. The causes of death were as follows: the development of recurrent PE – 4 (36,4%) patients, cancer – 3 patients (27,3%), stroke – 2 (18,1%), one patient (9,1%) died due to severe heart failure and one – myocardial infarction. A comparative analysis in the groups of alive patients (n = 62) and patients with a fatal events (n = 11) showed that the dead patients were older (78 (68; 81) vs. 65 (49; 75) years; p = 0,003), had a higher PESI score (119,0 (99,7; 137,2) vs. 88,0 (68,0; 108,0); p = 0,016) and were less compliant to prolonged anticoagulant therapy during the one year of observation (45,5% of patients (n = 5) vs. 82,3% ( = 51); p = 0,015). The ROC curve determined that a high risk of death during the one year after PE is associated with age over 70 years (p = 0,0001) and more than 95 points by PESI in the hospital period (p = 0,0001). Conclusion: The death events were developed in 11,8% of cases in patients with pulmonary embolism during the first year of follow-up. The death outcomes were significantly associated with elderly age, intermediate and high risk by PESI in the hospital period and low compliance to anticoagulant therapy extended during the year after pulmonary embolism.

Aim. To assess the features of diagnosis, treatment and outcomes of pulmonary embolism (PE) in patients of Russian hospitals.Material and methods. The register included all hospitalized patients with PE identified by any diagnostic method. Duration of inclusion was 12 months. In-hospital period management was assessed. Information about the included patients was provided by 20 hospitals from 15 Russian cities.Results. For the period from April 15, 2018 to April 15, 2019, 609 patients were included in the register (women — 50,7%, mean age — 63,0±14,5 years, minimum-maximum — 19-94 years). Among the known risk factors for PE, the most common were lower limb varicose veins (31,4%), heart failure (23,3%), previous deep vein thrombosis (19,4%), cancer (17,1%). The median time from symptom onset to suspicion/confirmation of PE was 4 days (1-3 quartiles — 1-8 days). Shortness of breath, syncope/presyncope, chest pain/discomfort, cough, leg pain or lower extremity asymmetry and hemoptysis were noted in 88,7%, 30,0%, 29,8%, 17,3%, 9,4% and 8,5% of patients, respectively. Echocardiography was performed in 89,5%, and Doppler ultrasound of lower limb veins — in 85,9% of patients. Signs of venous thrombosis were found in 57,8% of patients. Computed tomographic (CT) pulmonary angiography was performed in 89,2% of patients, pulmonary scintigraphy and pulmonary angiography — 0,8% each.Conclusion. Symptoms and main risk factors for PE in Russian patients did not fundamentally differ from those previously known. There was good adherence to modern guidelines for the diagnosis of PE, but adherence to guidelines on drug therapy met requirements only in half of the cases. There was an excessive use of TLT, as well as the use of ineffective methods of administering and monitoring anticoagulant therapy.In total, imaging technologies were used in 90,6% of patients, and signs confirming PE were found in 92,7%. Thrombolytic therapy (TLT) was performed in 25,0%; 92,0% of patients received anticoagulants. At the same time, unfractionated heparin (UFH) was injected subcutaneously in 26,3% of cases, and in one third of patients the drug was injected in doses unadjusted by body weight. In 42,7% of patients received UFH, the target activated partial thromboplastin time was not achieved. With warfarin treatment, the target international normalized ratio was achieved in only 48,4% of patients. Inferior vena cava filter placement and thrombectomy were performed in 1,3% each. During hospitalization (median — 11 days), 9,9% of patients died.

Aim. To study the clinical course and management of patients with pulmonary embolism (PE) of various age groups hospitalized in a cardiology hospital.Material and methods. Aim. Evaluation of the association of cardiac-ankle vascular index and development of adverse outcomes in patients after non-ST elevation acute coronary syndrome (NSTEACS).Material and methods. To the registry of NSTEACS in 2009. Totally, 415 patients had been included. The group of main long term (5-year) follow up consisted of 385 persons. Measurement of cardiac-ankle vascular index (CAVI) was done with the VaSera-1000 (Japan) equipment. As the endpoints, death, non-fatal myocardial infarction, unstable angina hospitalization, ischemic stroke were taken. Combination of the outcomes was labeled as combinatory endpoint. Statistics was done with Statistica v.8.0, MedCalc v.16.2.1. For the risk evaluation, the odds ratio (OR) was calculated with 95% confidence interval (95% CI). Differences in the comparison groups were taken as significant with (р)8,55 was associated with adverse cardiovascular events in NSTEACS patients during 5 years follow-up, and CAVI ≥9,15 was associated with fatal outcomes.Conclusion. The following clinical and instrumental parameters are associated with adverse cardiovascular events in NSTEMI patients during 5 year follow-up: older age, anamnesis of myocardial infarction and coronary bypass surgery, ischemic ECG changes, decreased left ventricle ejection fraction, higher GRACE score at admission and CAVI >8,55 by sphygmomanometry. This makes possible to use these parameters as the predictors of long term adverse outcomes. Value of CAVI ≥9,15 can be used as a marker of general fatal outcomes in NSTEACS.

Lymphotoxin-α (LTA) is a major pro-inflammatory cytokine produced at the early stages of vascular inflammation, taking part in the formation of arterial atherosclerosis and development of coronary heart disease. Functional changes in the gene encoding LTA production may influence the development of coronary heart disease with unfavorable progression. However, studies for associations between rs1041981 (C-804A) LTA gene variant and development of acute cerebrovascular accidents, myocardial infarction, and severity of coronary atherosclerosis have yielded contradictory results. The purpose of our study was to investigate an association of rs1041981 gene LTA with risk of adverse events within five years of follow-up in the patients with acute coronary syndrome without ST elevation ST (nonST-ACS). 178 patients with nonST-ACS from the Kemerovo Cardiology Center Registry were included into the study. Genotyping of rs1041981 site variable LTA gene was performed by TaqMan technique using an “iCycler iQ” device (BIO-RAD, USA). Results: we have found that the A allele and A/A genotype polymorphism in LTA gene (rs1041981) have been associated with development of adverse cardiovascular events over five years of observation (respective p levels were 0.02 and 0.036). In patients with A/A genotype, the rs1041981 polymorphism in LTA gene was associated with 3.8-fold increase in adverse cardiovascular events, compared to patients having A/C or C/C genotype. Carriage of A allele in LTA gene (rs1041981) doubles the risk of adverse cardiovascular events in patients with nonST-ACS at long observation terms. By means of Kaplan-Meier method, we have determined that survival to the first endpoint occurred more often in carriers of the genotype A/A of LTA gene (rs1041981). The A/A genotype of LTA gene (rs1041981) proved to be more significant (p = 0.016) for development of adverse outcomes, when combining the patients with A/C and C/C genotypes. One may draw a conclusion that A allele and A/A genotype of rs1041981 LTA polymorphism is associated with development of adverse cardiovascular events during the five-year period following the index event in patients with nonST-ACS.

Aim. To define early echocardiographic markers of cardiovascular risk in women.Material and methods. In 76 females, of 1st degree of kinship, from 61 families, two groups were selected: 26 daughters, whose mothers have arterial hypertension (AH) — group 1; 41 daughters from normotensive mothers — group 2. Anamnesis was assessed, including registration of cardiovascular risk factors, heredity, anthropometry, bidimensional echocardiography (EchoCG), and tissue Doppler.Results. In women from group 1, waist circumference (WC) and waist/hip ratio were significantly higher than in group 2. In bidimensional EchoCG, structural and functional parameters of the left ventricle function (LV) were within the reference values, however left atrium volume index in group 1 was significantly higher in comparison with the daughters from normotensive mothers. Also, in the group 1, there were significant differences od tissue Doppler values that characterize diastolic LV function. Relation E/e’ on lateral and septal areas of the mitral valve did not reach pathological values in both groups, as the in groups almost healthy persons were included; in the group of daughters form hypertensive mothers though, these parameters were significantly higher than in the group 2.Conclusion. Early adverse factors of cardiovascular risk in women are inheritance for cardiovascular diseases and AH, markers of metabolic syndrome (increased WC and waist/hip ratio), and left atrium volume and E/e’ that represent diastolic function of the LV.

Aim. Evaluation of the association of variation sites G-1082A (rs3024491) and C-592A (rs1800872) gene IL10 with one year outcomes of ST elevation acute coronary syndrome (STEACS).Material and methods. Totally, 178 patients included, with STEACS in whom the polymorphisms C-592А (rs1800872) and G-1082А (rs3024491) of the gene IL10 were checked. Genotyping was done with TaqMan “iCycler iQ” (BIO-RAD, USA). To the control group 185 relatively healthy persons included living in Kemerovo city. In 93, the concentration of interleukin-10 (IL-10) was measured by the hard-phase immune-enzyme assay, by BIOSOURCE (Belgium). Reference values of IL-10 were set at 2,98 (1,75-4,31) pg/mL. At the one-year stage, endpoints developed in 42 (23,5%) patients, progressive angina — in 30 (16,8%); cardiovascular mortality for one year reached 1,1% (n=2), nonfatal myocardial infarction developed in 6 (3,4%), ischemic stroke — in 4 (3,2%) patients.Results. While comparing IL-10 concentrations in STEACS with the reference values in Kemerovo city inhabitants, relatively low concentration was revealed in STEACS patients (0,38 vs 2,98 pg/mL; p=0,001). Association analysis of the variation sites of IL-10 with adverse outcomes during one year showed that genotype А/С rs3024491 (G-1082А) of gene IL10 is associated with the development of adverse outcome after NSTEACS. Endowment analysis towards the endpoint during 12 months showed that the most adverse is heterozygous carriage of А/С rs3024491 (G-1082А) gene IL10 (р=0,048).Conclusion. In NSTEACS patients genotype С/С rs1800872 (С-592А) gene IL10 associated with lots of cardiovascular risk factors, but genotype А/А rs3024491 (G-1082А) on the contrary, shows associations with protective factors. Genotype A/C of polymorphic variant rs3024491(G-1082А) gene IL10 is associated with adverse yearly outcomes in STEACS patients.

Cardiovascular diseases (CVD) remain the main cause of mortality in the world. [1] At the same time, arterial hypertension (AH) is one of the most common CV pathologies, which, according to foreign researchers, affects about 30–45% of the general population [2] and about 40–47% of the population according to Russian studies. [3] The experts forecast that by 2030 the prevalence of hypertension will increase by approximately 10%. [4]

The study included 120 patients with acute coronary syndrome (ACS), which were divided: in two groups, respectively, with ACS-ST segment elevation (group 1, n = 80), and without this clinical sign (group 2, n = 40). The levels of interleukins (IL)-6, -8, -10, and tumor necrosis factor-α (TNFα) were determined by quantitative ELISA test on the 1st and 10th days of the study. In both groups, the IL-8 and TNFα values on day 1 were similar to the reference level, whereas IL-10 levels were significantly reduced as compared to the standard values. IL-6 level was increased in patients with ACS-ST elevation only in acute phase of ACS, while the patients from group 2 showed normal IL-6 values. On the day 10, TNFα levels in the both groups of patients were significantly increased as compared to the 1st day, whereas IL-6 and IL-10 levels were significantly decreased by the day 10 in both groups of patients. Differential dynamics of IL-8 was observed for these groups of the patients from the day 1 to day 10, i.e., the IL-8 concentration showed a tendency to increase (by 40%) for the group 1 and decrease by 54% in the group 2. In conclusion, the patient grouped by different clinical ACS variants exhibit a similar dynamics of cytokine concentrations, but an increase of IL-8 by the day 10 of the disease was seen only in patients with ACS segment elevation. It may be indicative of different intensity of the inflammatory response.

Abstract. The study included 154 cases of acute myocardial infarction (MI) with ST segment elevation. Cytokine levels were determined in all the patients at days +1 and +10 after hospital admission, using quantitative ELISA assays. On day +1, a more that 2-fold increase was revealed for TNFα and IL-1α, as compared with healthy individuals. On day +10, we found a more than twofold increase in TNFα and IL-8 in blood plasma, whereas IL-1α and IL-10 levels became significantly reduced by the day +10, as compared to median values at day +1 of the study. Moreover, dynamics of the cytokines was compared between clinical subgroups of MI patients classified by gender, age, or presence of early complications. It was noted that the patients with complicated course of myocardial infarction exhibited high levels of IL-1α both on the day 1, and on day +10 of the disease. Aim. To investigate the association between endothelial dysfunction markers and multifocal atherosclerosis in non-ST elevation acute coronary syndrome (NSTE- ACS). Material and methods. The study included 82 patients with NSTE-ACS, who underwent coronary angiography, colour duplex ultrasound of brachiocephalic arteries (BCA) and lower extremity arteries (LEA), and a complex assessment of endothelial function during their hospitalisation. Vasodilating endothelial function was assessed by endothelium-dependent vasodilatation (EDVD) in a reactive hyperemia test. Vasoconstrictory and adhesive endothelial function was assessed by the blood levels of endothlin-1, sP-selectin, and sE-selectin in a solid-phase enzyme immunoassay. Statistical analyses were performed using the STATISTICA 6.0 software package. Results. Isolated coronary artery (CA) atherosclerosis was registered in 46,3% of the patients, while isolated BCA atherosclerosis was observed in 9,7%, and the combination of CA and peripheral artery atherosclerosis was registered in 19,5%. No cases of isolated LEA atherosclerosis were observed. According to the visualisation results, 24,3% of the patients had no signs of atherosclerotic stenosis (>30%) in any vascular basin. The highest levels of endothelin-1 were registered in patients with isolated BCA atherosclerosis and with the combination of peripheral and CA atherosclerosis. No significant differences were observed between the groups by the blood levels of sP-selectin and sE-selectin; moreover, these levels were within the reference range. All groups demonstrated similar levels of vasodilating endothelial function. Conclusion. In NSTE-ACS patients, vasoconstrictory endothelial function was clearly associated with the atherosclerosis parameters. At the same time, there was no link between the atherosclerosis severity and localisation and the parameters of vasodilating and adhesive endothelial function. Endothelin-1, as a vasoconstriction marker, was most informative in patients with peripheral and multifocal atherosclerosis.

Aim. To assess the associations between inflammatory factors and anxiety (A) and depression (D) levels in patients with myocardial infarction (MI). Material and methods. The study included 100 MI patients, hospitalised with a diagnosis of Q-wave MI (mean age 62,0±1,3 years). The methods of psychosocial status assessment included Zung depression scale and SpielbergerKhanin personal and reactive anxiety scales. The inflammatory markers of interest included interleukins (IL) 1-beta, IL-6, IL-8, IL-10, and C-reactive protein (CRP). Results. D and A symptoms in the early post-MI stage were associated with higher risk of cardiovascular events in the following year. Among MI patients with comparable MI severity, D and A symptoms were linked to higher levels of pro-inflammatory cytokines IL-1-beta, IL-8, IL-8, and INF-gamma. Conclusion. In MI patients with D and A symptoms, one of the mechanisms of poor prognosis is an activation of subclinical inflammation.

Aim. To analyse retrospectively the prevalence and prognostic value of “intact” coronary arteries (CA) in patients with acute coronary syndrome (ACS). Material and methods. In 2006-2007, 913 urgent coronary angiographies were performed in ACS patients. The follow-up length was 12 months after hospital discharge. Results. No CA occlusion or stenosis was detected in 76 (8,3%) out of 913 ACS patients. The clinical group with “intact” CA mostly included individuals with non-Q myocardial infarction; the diagnosis was confirmed by biomarkers in 22%. The in-hospital and post-discharge prognosis was better, compared to the patients with atherosclerotic CA stenosis. In addition, gastro-intestinal pathology was more prevalent in individuals with “intact” CA. Conclusion. The study results confirm the possibility of coronary heart disease and ACS, in particular, in patients with “intact” CA. They also point to the importance of using all the available ACS criteria while identifying the causes of chest pain syndrome.
