This study aimed to compare hip and knee biomechanics during sidestep cutting on the operated and nonoperated sides in individuals with anterior cruciate ligament reconstruction (ACLR), and in an uninjured control group. Twenty male basketball athletes, 10 individuals with ACLR and 10 controls, were recruited. Hip and knee joint angles and angular velocities were investigated with a three-dimensional motion analysis system, and ground reaction forces (GRF) along with moments were collected during the deceleration phase of the stance limb during sidestep cutting maneuvers. We found significantly higher peak hip flexion, hip internal rotation angular velocities, and peak thigh angular velocity in the sagittal plane in the ACLR group. In addition, the peak vertical GRF and peak posterior GRF of the ACLR group were significantly higher than those of the control group. Univariate analyses indicated that the posterior GRF of the non-operated side was significantly higher than in the matched operated side in the control group. The operated and non-operated sides in male basketball athletes with ACLR showed alterations in hip and knee biomechanics compared with a control group, especially in the sagittal plane. Therefore, the emphasis of neuromuscular control training for the hip and the knee in basketball players with ACLR is required. [ABSTRACT FROM AUTHOR]

It is well-established that processes involving changing direction or turning in which either or both standing and walking turns are utilized involve coordination of the whole-body and stepping characteristics. However, the turn context and whole-body coordination have not been fully explored during different turning amplitudes. For these reasons, this present study aimed to determine the effects of turning amplitude on whole-body coordination. The findings from this study can be utilized to inform the rationale behind fall prevention factors and to help design an exercise strategy to address issues related to amplitude of turning in older adults. Twenty healthy older and twenty healthy younger adults were asked to complete standing turns on level ground using three randomly selected amplitudes, 90°, 135° and 180°, at their self-selected turn speed. Turning kinematics and stepping variables were recorded using Inertial Measurement Units. Analysis of the data was carried out using Mixed Model Analysis of Variance with two factors (2 groups × 3 turning amplitudes) and further post hoc pairwise analysis to examine differences between factors. There were significant interaction effects (p < 0.05) between the groups and turning amplitudes for step duration and turn speed. Further analysis using Repeated Measure Analysis of Variance tests determined a main effect of amplitude on step duration and turn speed within each group. Furthermore, post hoc pairwise comparisons revealed that the step duration and turn speed increased significantly (p < 0.001) with all increases in turning amplitude in both groups. In addition, significant main effects for group and amplitudes were seen for onset latency of movement for the head, thorax, pelvis, and feet, and for peak head–thorax and peak head–pelvis angular separations and stepping characteristics, which all increased with turn amplitude and showed differences between groups. These results suggest that large amplitude turns result in a change in turning and stepping kinematics. Therefore, when assessing the turning characteristics of older adults or those in frail populations, the turning amplitude should be taken into account during turning, and could be gradually increased to challenge motor control as part of exercise falls prevention strategies. Background Excessive knee valgus has been strongly suggested as a contributing key factor for anterior cruciate ligament (ACL) injuries. Three-dimensional (3D) motion analysis is considered the "gold standard" to assess joint kinematics, however, this is difficult for on-field assessments and for clinical setting. Purpose To investigate the concurrent validity of 2D measurements of knee valgus angle during cutting in different directions and to explore intra-rater and inter-rater reliability of the 2D measurements. Study Design Descriptive laboratory study Method Seven recreational soccer players participated in this study. Participants performed three trials of cutting maneuvers in three different directions (30°, 60°, and 90°) with the dominant leg. Cutting maneuvers were recorded simultaneously with a video camera and a ViconTM motion capture system. Knee valgus angle from 2D and 3D measurements at initial contact and at peak vertical ground reaction force (vGRF) were extracted. The Pearson's correlation was used to explore the relationship between the 2D and 3D measurements, and reliability of the 2D measurements were performed using intraclass correlation coefficients (ICC). Result Significant correlations between 2D and 3D knee valgus measurements were noted for 60° (r = 0.45) and 90° (r = 0.77) cutting maneuvers at initial contact. At peak vGRF, significant correlations between 2D and 3D knee valgus measurements were noted for 30°, 60°, and 90° cutting maneuvers (r=0.45, r=0.74, r=0.78), respectively. Good-to-excellent intra-rater and inter-rater reliability of the 2D knee valgus measurements was observed during cutting in all directions (ICCs: 0.821-0.997). Conclusion Moderate-to-strong correlation between 2D and 3D knee valgus measurements during 60°-90° cutting maneuvers, and good-to-excellent intra-rater and excellent inter-rater reliability for the 2D measurements in the present study supports the use of 2D knee valgus measurements in the evaluation of targeted interventions, although the limitations of examining cutting maneuvers using 2D measurement in complex movement still need to be considered. Background: Running is a cyclic movement requiring bilateral symmetry between the lower limbs to reduce injury risk. The assessment of side-to-side differences is often performed to detect functional deficits. Objectives: The purpose was to study side-to-side differences using clinical and running performance assessments in healthy runners (HR), runners with a history of lower limb injuries (RHI), and runners with a current lower limb injury (RLI). Methods: Forty-three runners were recruited, with 14 participants being allocated to the HR group, 13 to the RHI group, and 16 to the RLI group. Peak vertical ground reaction force (GRF), midfoot pressure, foot rotation, and gait variability were recorded using a Zebris FDM-T treadmill analysis system. Participants were also assessed using the navicular drop test. Dependent t-tests were used to determine if any differences existed between the lower limbs within each group. One-way ANOVAs were then used to investigate the side-to-side differences between the three groups. Results: Significant differences were seen in navicular drop height between lower limbs within both the HR (P = 0.02) and RHI (P = 0.009) groups, and side-to-side differences in foot rotation were greatest in the RLI group (~ 34%) compared to both the RHI (~ 30.5%) and HR (~ 24%) groups. The lateral variability of the center of pressure was greatest in the RLI group (37.1 mm) compared to the RHI (28.9 mm) and HR (22.2 mm) groups. Conclusions: Variability of butterfly center of pressure diagram may help identify runners at a greater risk of lower limb injury. Side-to-side differences should be expected to progressively decrease from the injured stage, through the recovery and return to sport phases. Target goals of less than 34% side-to-side difference for foot rotation and 37.1 mm for the lateral center of pressure variability may be used to help the decision-making process when considering a return to running practice.
