Patients with moderate to severe acute respiratory distress syndrome (ARDS) have been recommended to receive prone position ventilation (PPV). However, the dynamic changes in respiratory mechanics during PPV and their relationship with the prognosis have not been sufficiently evaluated. In addition, the impact of using neuromuscular blocking agents (NMBAs) during PPV on respiratory mechanics is not clear enough. Thus, the study aims to investigate the above-mentioned issues.A prospective cohort study was conducted on 22 patients with moderate to severe ARDS who received PPV in the intensive care unit (ICU) of the First Affiliated Hospital of Guangzhou Medical University. A multifunctional gastric tube was used to measure the patients' respiratory mechanics during supine position ventilation (SPV), early PPV (PPV within 4 h of initiation), and middle/late PPV (more than 6 h after the initiation of PPV). Longitudinal data were analyzed with generalized estimating equations (GEE).Compared with SPV, the esophageal pressure swings (ΔPes) measured during the PPV was significantly higher (SPV 7.46During the transition from SPV to early PPV and then to middle/late PPV, the ΔPes in the PPV were greater than SPV and it fluctuated within a normal range while oxygenation improved significantly in all patients. The Ppeak, ΔPes and RR in the death group were significantly higher than survival group. When NMBAs were used, the ΔPes, inspiratory transpulmonary pressure (PLei), driving pressure (DP) and DPL were significantly decreased, suggesting that the rational combination of NMBAs and PPV may exert a synergistic protective effect on the lungs.

The rupture process of large earthquakes is generally complex and contains multiple sub-faults planes with different focal mechanisms. The focal mechanisms inversion of these sub-faults by applying the Multi-Point-Source Faulting Representations (MPSFR) are essential for seismic stress analysis and earthquake disaster assessment. The MPSFR method is time-consuming and often with unstable results. In this study, we develop an Image Segmentation and Iterative Inversion (ISII) approach to calculate the MPSFR for large earthquakes by inverting near-field strong motion data. This new approach analyzes the rupture image of the earthquake and divides the entire rupture surface into several sub-rupture segments as a point source in the MPSFR. We approach the ISII model to the 2010 El-Mayor Cucapah (EMC) earthquake and the 2016 Kaikoura earthquake, respectively. In the EMC earthquake, the overall misfit was reduced from 0.58 (earthquake rupture model with the same focal mechanism) to 0.47 (IISI model with four different focal mechanisms). In the Kaikoura earthquake, the overall misfit was reduced from 0.67 to 0.55. The rupture process inverted by the ISII model is consistent with the joint multi-method inversion and the operation process is high efficiency. The test results indicate the ISII model can accurately and quickly invert the complex earthquakes rupture process and provide valuable information for earthquake disaster assessment.

In this research, we designed and implemented a small, convenient, and non-invasive paper-based microfluidic sweat sensor that can simultaneously detect multiple key biomarkers in human sweat. The origami structure of the chip includes colorimetric and electrochemical sensing regions. Different colorimetric sensing regions are modified with specific chromogenic reagents to selectively identify glucose, lactate, uric acid, and magnesium ions in sweat, as well as its pH value. The regions of electrochemical sensing identify cortisol in sweat by molecular imprinting. The entire chip is composed of hydrophilic- and hydrophobically-treated filter paper, and 3D microfluidic channels are constructed by using folding paper. The thread-based channels formed after hydrophilic and hydrophobic modifications are used to control the rate of flow of sweat, which in turn can be used to control the sequence of reactions in the differently colored regions of development to ensure that signals of the best color can be captured simultaneously in the colorimetric sensing regions. Finally, the results of on-body experiments verified the reliability of the proposed sweat sensor and its potential for the non-invasive identification of a variety of sweat biomarkers.

In this study, systematic pharmacology and bioinformatic approaches were employed to identify the potential targets of Polygonum cuspidatum (PC) for treating heart failure (HF). The active ingredients of PC were screened by using the TCMSP database, and HF-related genes were identified in the GEO database. Then, the herb-HF targeted-gene networks were constructed using Cytoscape software. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) functional analyses were performed to obtain the enriched molecular pathways associated with the pathogenesis of HF. Finally, in vitro experiment was performed to evidence network pharmacology analysis. 170 intersection genes were obtained, and key genes (FOXO3, NFKB1, and TNF) were identified. Besides, GO and KEGG findings indicated that PC treatment of HF was achieved via regulating apoptosis, IL-17 signaling pathway, TNF signaling pathway, response to oxidative stress, and response to reactive oxygen species. And cell experiment revealed that PC could decrease the expression of NFKB1 and TNF and increase the expression of FOXO3, SOD1, and GPX1 in H9C2 cells. These findings showed that the therapeutic mechanism of PC in the treatment of HF may be associated with the regulation of inflammation-related and oxidative stress-related genes.

Blumea balsamifera (L.) DC., a medicinal plant with high economic value in the Asteraceae family, is widely distributed in China and Southeast Asia. However, studies on the population structure or phylogenetic relationships with other related species are rare owing to the lack of genome information. In this study, through high-throughput sequencing, we found that the chloroplast genome of B. balsamifera was 151,170 bp in length, with a pair of inverted repeat regions (IRa and IRb) comprising 24,982 bp, a large single-copy (LSC) region comprising 82,740 bp, and a small single-copy (SSC) region comprising 18,466 bp. A total of 130 genes were identified in the chloroplast genome of B. balsamifera, including 85 protein-coding, 37 transfer RNA, and 8 ribosomal RNA genes; furthermore, sequence analysis identified 53 simple sequence repeats. Whole chloroplast genome comparison indicated that the inverted regions (IR) were more conserved than large single-copy and SSC regions. Phylogenetic analysis showed that B. balsamifera is closely related to Pluchea indica. Conclusively, the chloroplast genome of B. balsamifera was helpful for species identification and analysis of the genetic diversity and evolution in the genus Blumea and family Asteraceae.

Background: Prone position ventilation (PPV) is an important strategy for patients with severe acute respiratory distress syndrome (ARDS). This prospective study investigated the use of electromyography of the diaphragm (EMGdi) for monitoring respiratory drive in patients with moderate to severe ARDS during long-term PPV Methods: An integrated nostril-gastric feeding tube containing an esophageal electrode and balloon was placed in 14 patients with severe ARDS prior to PPV. EMGdi and trans-pulmonary pressure (∆PL) data were collected before PPV (baseline), every 2 h during PPV, and 2 h after the restoration of supine position ventilation (post-2 h SPV). Results: In ARDS patients, the static compliance of the chest wall was significantly decreased after PPV. EMGdi levels were slightly lower in the early, middle, and late stages of PPV compared with baseline. Patients who received neuromuscular blocker experienced a greater drop in EMGdi from baseline than those who did not. Conclusions: For ARDS patients, EMGdi was slightly decreased after prolonged PPV. This is contrary to the change in diaphragm electromyography during normal body position changes. Monitoring EMGdi regularly during PPV in ARDS patients is feasible and can be used as a reference for lung protective ventilation strategies.

BACKGROUND: Patients with difficult weaning who undergo mechanical ventilation are more likely to be at risk of reintubation and the sequential use of oxygen therapy after extubation is a concern for clinicians. Therefore, the aim of the present study was to compare the effects of transnasal high-flow nasal cannula (HFNC) oxygen therapy and non-invasive positive-pressure ventilation (NIV) on respiratory mechanics in patients with difficult weaning. METHODS: The present study was a single-center, retrospective, observational study. Twenty-nine patients with difficult weaning off invasive mechanical ventilation from the Department of Critical Care Medicine, The First Affiliated Hospital of Guangzhou Medical University, from December 2018 to April 2021, were included. Within 48 h after extubation, alternate respiratory support with HFNC and NIV was provided. Relevant indicators were recorded after each support mode had been maintained for at least 60 min. These included esophageal pressure (Pes), gastric pressure (Pga), transdiaphragmatic pressure (Pdi), pressure-time product of Pes (PTPes), pressure-time product of Pga (PTPga), pressure-time product of Pdi (PTPdi), ratio of the PTPdi to the PTPes (PTPdi/PTPes), and ratio of the Pes to the Pdi (Pes/Pdi), diaphragmatic electromyogram (EMGdi), percentage of esophageal pressure coefficient of variation (CVes%),diaphragmatic electromyogram coefficient of variation (CVEMG),inspiratory time (Ti), expiratory time (Te) and respiratory cycle time (Ttot). RESULTS: Of the 29 patients included, 22 were males and 7 were females [age: 63.97±15.34 years, Acute Physiological and Chronic Health Estimation II (APACHE II) score: 18.00±5.63]. With the increase of glass bridges or glass walkways in the scenic area, the safety protection provided by railings cannot be neglected. However, there is a difference between the demands for a horizontal load on the top of railings in the present design specifications. Especially, it does not give specific advice on the horizontal load on the top of railings of glass bridges or glass roads in the scenic area. In this study, experimental research is conducted on railing columns based on the investigation of common railings of glass bridges or glass roads in the scenic area. The type of railing columns includes finished products and field production. The number of railing columns is 12. The displacement and strain results of the railing columns under different loads are obtained. The rigidity and strength are analyzed by numerical simulation combined with test results. A reasonable horizontal load on the top of the railings of a glass bridge or glass road is advised. These results provide a reference for the design, inspection, and evaluation of railings of glass bridges or gallery roads in the scenic area.

Background This study examined how rhizosphere fungi influence the accumulation of chemical components in fruits of a small population species of Cinnamomum migao. Results Ascomycota and Basidiomycota were dominant in the rhizosphere fungal community of C. migao. Pestalotiopsis and Gibellulopsis were associated with α-Terpineol and sabinene content, and Gibellulopsis was associated with crude fat and carbohydrate content. There were significant differences in rhizosphere fungal populations between watersheds, and there was no obvious change between fruiting periods. Gibberella, Ilyonectria, Micropsalliota, and Geminibasidium promoted sabinene accumulation, and Clitocybula promoted α-Terpineol accumulation. Conclusion The climate-related differentiation of rhizosphere fungal communities in watershed areas is the main driver of the chemical composition of C. migao fruit. The control of the production of biologically active compounds by the rhizosphere fungal community provides new opportunities to increase the industrial and medicinal value of the fruit of C. migao.

BACKGROUND: Interferon (IFN) is widely used in clinical practice and nebulization inhalation is one of the commonly used routes of administration. However, nebulization drugs such as interferon-α (IFN-α) with large molecular weights may deposit in the membrane of the breathing filters, causing its resistance to gradually increase. Thus, our study explores the effect of IFN-α and other nebulization drugs on the resistance of breathing circuit filters under invasive mechanical ventilation. METHODS: We divided 96 breathing filters into eight groups. The baseline group was not treated while the blank group was installed but were not nebulized. The remaining groups received jet nebulized or vibrating nebulized with either normal saline, Combivent, Amphotericin B, or IFN-α at a frequency of once every 12 hours separately and were removed from the breathing circuit after 24 hours. The resistance of the filter of each group was then measured and statistical comparisons were made. RESULTS: Filter resistance of the IFN-α jet nebulization group was greater than that of the other groups, and there were statistical differences except for the Amphotericin B jet nebulization group. Comparison of the resistance [cmH(2)O/(L·s)] of the IFN-α jet nebulization group vs. the baseline group showed 2.56 (2.40, 2.68) vs. 2.26 (2.03, 2.40), P=0.037; of the IFN-α jet nebulization group vs. the blank group showed 2.56 (2.40, 2.68) vs. 2.11 (1.98, 2.27), P=0.003; of the IFN-α jet nebulization group vs. the normal saline group: 2.56 (2.40, 2.68) vs. 2.16 (2.08, 2.32), P=0.023; of the IFN-α jet nebulization group vs. the Combivent jet nebulization group: 2.56 (2.40, 2.68) vs. 2.18 (2.14, 2.27), P=0.018; and of the IFN-α jet nebulization group vs. the Amphotericin B jet nebulization group: 2.56 (2.40, 2.68) vs. 2.33 (2.05, 2.45), P=0.221. The effect of jet nebulization and vibrating mesh nebulization on the resistance of breathing filters showed no significant statistical difference. CONCLUSIONS: Jet nebulization with IFN-α significantly increased the resistance of the breathing filter within 24 hours and there was no significant difference in filter resistance between jet nebulization and vibrating mesh nebulization of IFN-α or Amphotericin B.

Sabia parviflora Wall. ex Roxb., an evergreen climbing woody vine, is a Chinese herbal medicine commonly used by ethnic minorities in some areas of China. In this study, the chloroplast genome of S. parviflora was sequenced for the first time. Its genome is 162,054 bp in length with 38.6% of GC content. The genome consists of a large single copy (LSC) region of 90,001 bp, a small single copy (SSC) region of 18,887 bp, and two inverted repeat (IRa and IRb) regions of 26,583 bp each. A total of 130 genes were annotated, including 85 protein-coding genes, 37 tRNA genes, and 8 rRNA genes. Phylogenetic analysis was conducted by nine species from order Proteales, which demonstrated a close relationship between the family Sabiaceae and Nelumbonaceae.

Introduction Prone position ventilation (PPV) has been shown to improve oxygenation and decrease pulmonary vascular resistance and mortality in patients with severe acute respiratory distress syndrome (ARDS). Whether these benefits of PPV occur similarly in acute exacerbations of interstitial lung disease (ILD) is not clear. We retrospectively explored the use of PPV in acute exacerbation with ILD versus those with severe acute respiratory distress syndrome (severe ARDS). Methods Retrospective study of the application of PPV in 17 patients with acute exacerbations of ILD and in 19 patients with severe ARDS. Pre- and post-PPV hemodynamic parameters, respiratory mechanics, prognostic indicators, complications and mortality rates at 28, 60 and 90 days were compared. A new flavonol glycoside named Sabiapside A (1), along with four known compounds, quercetin-3-O-gentiobioside (2), camellianoside (3), isobariclisin-3-O- rutinoside (4), tsubakioside A (5), was isolated from Sabia parviflora. Their structures were elucidated by extensive spectroscopic analysis including MS, UV, IR and NMR data. The antioxidant activities of these glycosides evaluated by ABTS+ and DPPH radical scavenging reaction was higher than that of vitamin C used as the reference antioxidant.

Background: Weaning from mechanical ventilation (MV) is an important and universal issue in critically ill patients, with no consensus that predicts weaning success. Twitch tracheal airway pressure (TwPtr) may be a more objective indicator of diaphragm function. The present study evaluated TwPtr relative to negative inspiratory force (NIF) or Medical Research Council (MRC) score, for predicting success of weaning from MV. Methods: From December 2015 to March 2017, 62 patients were included who received invasive MV >48 hours and then underwent a test for spontaneous breathing. The NIF and MRC score were sequentially determined. The TwPtr measurement was performed via magnetic stimulation of the neck phrenic nerve. Results: Weaning success was achieved by 54 patients (87.1%), including 30, 12, and 12 patients who experienced simple, difficult, and prolonged weaning, respectively. The areas under receiver operating characteristic curves (AUCs) for NIF, MRC score, and TwPtr were 0.778, 0.560, and 0.792. When TwPtr was combined with NIF, the sensitivity and specificity of weaning success were 96.3% and 75.0%, and AUC was 0.807. In the weaning success group, after correction for age, the AUC of TwPtr was 0.878, which differentiated the simple weaning from non-simple group. Conclusions: TwPtr was superior to either NIF or MRC score for differentiating patients in the simple weaning group from those patients who experienced difficult, prolonged, or failed weaning. TwPtr combined with NIF may be used to predict weaning success better than the MRC score alone.

Infectious diseases caused by antibiotic multidrug-resistant microorganisms are major causes of morbidity and mortality in humans. Hence, there is an urgent need to search for new antimicrobial agents. Initially known as a defensive effector in the innate immunity of certain organs of the human body, human goose-type lysozyme 2 (hLysG2) has been shown to possess therapeutically useful potential against multidrug-resistant microorganisms. Developing a novel strategy for large-scale production that provides high yields of this protein with high purity, quality, and potency is critical for pharmaceutical applications. To overcome the issues related to prokaryotic expression, here we report the production of recombinant hLysG2 (rhLysG2) using the methylotrophic yeast Pichia pastoris as expression host. The strong inducible alcoholoxidase 1 (AOX1) promoter was used to drive expression of the optimized hLysG2 gene. Under the optimal expression conditions, the lytic activity of rhLysG2 reached 113 U/mL of culture supernatant in shake flask cultivation and this was increased to 2084 U/mL in fed-batch fermentation. Using chitin affinity chromatography and size-exclusion chromatography, rhLysG2 was produced with a yield of 137 mg/L, purity of > 99%, molecular weight of 21,504.6 Da, and specific activity of 13,500 U/mg. In vitro assays indicated that rhLysG2 possessed muramidase activity, isopeptidase activity, and free radical scavenging activity. This report describes an efficient strategy for the production of biologically active rhLysG2 in P. pastoris on a large scale with a high yield, which provides a solid foundation for possible future pharmaceutical applications.

Aims & Backgrounds Although inflammatory bowel diseases (IBD) are emerging and increasing in China, epidemiologic data are rarely available. This study was to investigate the epidemiological and clinical characteristics of IBD in Northern China. Methods This is a prospective, population-based study of incidence of IBD in Daqing,Heilongjiang province of Northern China from March 1, 2012 to February 28, 2013. All incident patients with IBD were clinically identified by IBD specialist group from five main General Hospitals covering the healthcare service for 1,343,364 residents in the urban areas of Daqing. IBD cases included in this study were followed-up for three months for diagnosis confirmation. Results A total of 27 new IBD cases including 25 cases of ulcerative colitis (UC) and 2 cases of Crohn's disease (CD) were identified. The population at risk was 1,343,364 person years. Age-adjusted incidence for total IBD, CD and UC were 1.77, 0.13, and 1.64 per 100,000population, respectively. A male predominance was found in CD patients (male to female ratio was 2∶0). In contrast, no obvious gender predominance was found in UC patients (male to female ratio was 1∶1.1). CD patients were diagnosed at an average age of 39.5 years. The main disease phenotypes of UC were distal colitis with a 24% of proctitis and 56% of left-sided colitis. The mean diagnostic age of UC patients was 48.9 years. Conclusions This is the first report on the incidence of IBD in the Northern Chinese population. A lower incidence of IBD, similar male predominance for CD, similar disease phenotype of UC, and lower disease activity was observed in Daqing compared to that in Southern China.

Background Selaginellaceae is a family of nonseed plants with special evolutionary significance. Plants of the family Selaginellaceae are similarly shaped and easily confused, complicating identification via traditional methods. This study explored, for the first time, the use of the DNA barcode ITS2 to identify medicinal plants of the Selaginellaceae family. Methodology/Principal Findings In our study, 103 samples were collected from the main distribution areas in China; these samples represented 34 species and contained almost all of the medicinal plants of Selaginellaceae. The ITS2 region of the genome was amplified from these samples and sequenced using universal primers and reaction conditions. The success rates of the PCR amplification and sequencing were 100%. There was significant divergence between the interspecific and intraspecific genetic distances of the ITS2 regions, while the presence of a barcoding gap was obvious. Using the BLAST1 and nearest distance methods, our results proved that the ITS2 regions could successfully identify the species of all Selaginellaceae samples examined. In addition, the secondary structures of ITS2 in the helical regions displayed clear differences in stem loop number, size, position, and screw angle among the medicinal plants of Selaginellaceae. Furthermore, cluster analysis using the ITS2 barcode supported the relationship between the species of Selaginellaceae established by traditional morphological methods. Conclusion The ITS2 barcode can effectively identify medicinal plants of Selaginellaceae. The results provide a scientific basis for the precise identification of plants of the family Selaginellaceae and the reasonable development of these resources. This study may broaden the application of DNA barcoding in the medicinal plant field and benefit phylogenetic investigations.

Mn3O4-anchored reduced graphene oxide (Mn3O4/RGO) nanocomposites have been successfully synthesized via a facile, effective, energy-saving technique. The surface morphology and structure of the Mn3O4/RGO composites were characterized by X-ray diffraction (XRD), X-ray photoelectron spectroscopy (XPS), Fourier transform infrared (FTIR), high resolution transmission electron microscopy (TEM), scanning electron microscopy (SEM), energy dispersive X-ray spectroscopy (EDS), Brunauer–Emmett–Teller (BET) surface area and Barrett–Joyner–Halenda porosity (BJH). The composite material was applied as the counter electrode (CE) for dye-sensitized solar cells (DSSCs). Electrochemical impedance spectra (EIS), cyclic voltammetry (CV) and Tafel polarization measurements revealed a better electrocatalytic performance for the reduction of triiodide in Mn3O4/RGO based DSSCs, compared with the devices with reduced graphene oxide (RGO) CEs. The Mn3O4/RGO improves the fill factor by 56% and the power conversion efficiency of this kind of DSSC reaches 5.90%, which is much higher than the DSSC assembled from RGO CEs. A Pt counter electrode as a reference was also tested.

It is well known that three-dimensional (3D) cultures can more accurately represent physiological cell responses, and that two-dimensional (2D) in vitro cell culture systems fall short in predicting tumor cell behavior in vivo. Major shortcomings of 2D cell culture systems include altered cell morphologies, reduced aggression, and an inaccurate representation of the dynamic 3D cellular environment experienced by cells in vivo. These 2D cell culture obstacles become even more deleterious to research when performing drug screenings or studying disease pathogenesis. Hence, cancer research demands a more biologically relevant in vitro model system that can improve drug discovery success rates to combat rising rates of cancer and other diseases. Moreover, an accurate and reproducible in vitro model will accelerate the drug discovery process and significantly reduce development costs. To this end, we have engineered a novel transparent 3D polystyrene (PS) scaffold to meet the emerging demand. We hypothesize that these 3D PS scaffolds are a superior 3D in vitro model for tumor pathogenesis research and drug discovery and development. To test our hypothesis we cultured and observe the growth of two cancer cell lines, human epithelial MCF-7 cells and HepG2 hepatocarcinoma cells, on both 2D PS-tissue culture plates (TCP) and 3D PS scaffolds (3D Insert™-PS). Observation by an inverted light microscope indicated that MCF-7 and HepG2 cells cultured on the 3D PS scaffolds formed cell aggregates along scaffold fibers and within pores, whereas cells cultured on the 2D PS-TCPs adopted a phenotype characteristically seen in cells growing in flat monolayers. Proliferation time-course assays performed with MCF-7 and HepG2 cells demonstrated that growth rates and metabolic activities of cells cultured on 3D scaffolds were significantly enhanced at numerous time points when compared with cells cultured on 2D TCPs, as determined by Alamar blue and MTT assays. These data suggest that 3D Insert™-PS scaffolds provide an improved in vitro culture environment. The susceptibility of both 2D and 3D cultured MCF-7 and HepG2 cells to anticancer drugs was assessed by tamoxifen (TAM) [10-6 M and 10-5 M] and methotrexate (MTX) [25 µM and 100 µM] treatments, respectively. The cellular cytotoxic response to TAM and MTX was measured by MTT and LDH assays and was found to be significantly lower in MCF-7 and HepG2 cultured on 3D scaffolds compared with cells cultured in 2D TCPs. Taken together, these data indicate that cancer cells cultured on 3D Insert™-PS scaffolds are more robust and resistant to anticancer drug treatments than cells cultured on traditional 2D TCPs. Furthermore, 3D Insert™-PS scaffolds do not have the same limitations encountered with traditional 2D cell culture. In conclusion, using 3D Insert™-PS scaffolds as in vitro 3D tumor models will provide a superior 3D cell culture environment for cancer research and the evaluation of new anticancer drugs.
