Musculoskeletal disorders are prevalent among agricultural workers, increasing the risk of work-related diseases due to manual labor, repetitive tasks, and prolonged postures. This study evaluates the association between physical behaviors during work and non-work, measured objectively, and musculoskeletal pain in the neck, upper back, and lower back in viticulture workers. A cross-sectional quantitative study was conducted with 75 viticulturists of both sexes aged 18 years or older. An accelerometer measured physical behaviors (lying down, sitting, standing, moving, walking, and sleeping) during work and non-work periods. Pain intensity was quantified using a 0–10 scale and categorized as "Low" and "High" pain intensity. Binary logistic regression tested the association between pain and time spent on physical behaviors. Results indicated a high prevalence of pain: 46.7% cervical, 52% upper back, and 60% lower back. Standing was the most common behavior during work, while lying and sitting were predominant during non-work. An increased sleeping time was associated with a decreased probability of experiencing high-intensity neck pain. Increased time spent lying down during non-work hours was associated with an increased probability of experiencing high-intensity upper back pain. No physical behavior was associated with high-intensity lower back pain. In conclusion, sedentary behaviors worsen upper back pain, and sleep reduces neck pain in viticulture workers. - Abstract
Nuestro proyecto APS tiene como objetivo la prevención y concienciación a la población sobre la problemática de la hipertensión arterial, dada su elevada prevalencia y su impacto sobre la salud y el bienestar. A través de charlas y talleres, nuestros estudiantes de ciencias de la Salud de la UCM tutorizados por profesores, llevan a cabo demostraciones para que los participantes aprendan a medir correctamente su presión arterial en casa y entender su significado, alertándoles de cuándo deben acudir al médico. Esta herramienta formativa es una manera eficaz para detectar la hipertensión arterial de forma precoz a personas que desconocen que la padecen o ayudar a las personas hipertensas a controlar mejor su enfermedad. De esta manera, nuestros estudiantes de los grados de ciencias de la salud de la UCM, a la vez que se forman como futuros profesionales de la salud, realizan un servicio a la sociedad que tiene como objetivo prioritario luchar contra la hipertensión arterial y salvar vidas. También participamos en Ferias y Campañas de la Salud que organizan la UCM y los Ayuntamientos de los municipios de Alcobendas, Alcorcón, Coslada y La Cabrera de Madrid. Contamos con el apoyo y la colaboración del Dr. Orduñez, Líder de la estrategia HEARTS en las Américas de la OPS/OMS, la Sociedad Española de Hipertensión (SEH-LELHA), Sociedad Española de Ciencias Fisiológicas (SECF), la Sociedad Española de Atención a las Personas con Enfermedades Crónicas (SEAPEC), el Colegio Oficial de Farmacéuticos de Madrid (COFM), el Colegio Oficial de Enfermería de Madrid (CODEM) y los Ayuntamientos de Alcobendas, Alcorcón, Coslada y La Cabrera de la Madrid.

- Abstract
El software Complete Anatomy (CA) permite a los usuarios investigar los detalles más minuciosos de la anatomía humana en 3D. Bases Anatómicas y Fisiológicas del Cuerpo Humano I (BANYFIS I) forma parte de las asignaturas de primer curso de la carrera de Farmacia de la Universidad Complutense de Madrid (UCM). Los profesores de las lecciones prácticas involucrados en las clases de BANYFIS I desarrollaron varios cursos de CA para 2 de las 5 lecciones prácticas que se realizan. Además, se elaboraron y entregaron a los estudiantes instrucciones para la instalación y el uso de CA antes de dichas lecciones prácticas. El objetivo de este estudio fue evaluar si los cursos fueron útiles en los procesos de aprendizaje de los estudiantes. Además, evaluamos la satisfacción de los estudiantes sobre el uso de CA y las instrucciones proporcionadas. Existen 5 lecciones diferentes durante las prácticas, 4 de ellas incluyen características anatómicas. Creamos 2 cursos diferentes con información anatómica para las estructuras musculares y óseas, y lecciones del sistema renal. Los otros 2 sistemas, cardiovascular y respiratorio, se utilizaron como controles. Por lo tanto, los estudiantes tuvieron acceso a CA y a los cursos antes, durante y después de las lecciones prácticas. Para evaluar la satisfacción de los estudiantes con el software CA y los cursos diseñados a medida, se les entregó una encuesta de 16 preguntas al final de la última práctica. Debían responder a cada ítem de forma numérica, considerando una escala Likert de cinco puntos de 0 a 4, siendo 0 “totalmente en desacuerdo” y 4 “totalmente de acuerdo”. Se recopilaron, compilaron y analizaron datos de 120 estudiantes. La mayoría de los estudiantes (82,3%, teniendo en cuenta las dos respuestas principales de la encuesta) mostró que el uso de CA mejoraba su comprensión de la información y que tener cursos era un beneficio para su motivación y aptitud para las lecciones prácticas (70%). - Abstract
Obesity is related to vascular dysfunction through inflammation and oxidative stress and it has been identified as a risk factor for chronic renal disease. In the present study, we assessed the specific relationships among reactive oxygen species (ROS), cyclooxygenase2 (COX-2), and endothelial dysfunction in renal interlobar arteries from a genetic model of obesity/insulin resistance, the obese Zucker rats (OZR). Relaxations to acetylcholine(ACh) were significantly reduced in renal arteries from OZR compared to their counterpart, the lean Zucker rat (LZR), suggesting endothelial dysfunction. Blockade of COX with indomethacin and with the selective blocker of COX-2 restored the relaxations to ACh in obese rats. Selective blockade of the TXA2/PGH2 (TP) receptor enhanced ACh relaxations only in OZR, while inhibition of the prostacyclin (PGI2) receptor (IP) enhanced basal tone and inhibited ACh vasodilator responses only in LZR. Basal production of superoxide was increased in arteries of OZR and involved NADPH and xanthine oxidase activation and NOS uncoupling. Under conditions of NOS blockade, ACh induced vasoconstriction and increased ROS generation that were augmented in arteries from OZRandbluntedbyCOX-2 inhibition and by the ROS scavenger tempol. Hydrogen peroxide (H2O2) evoked both endothelium-and vascular smooth muscle (VSM)-dependent contractions, as well as ROS generation that was reduced by COX-2 inhibition.In addition, COX-2 expression was enhanced in both VSM and endothelium of renal arteries from OZR. - Abstract
El Proyecto Stop Hipertensión con ApS/UCM pretende aportar nuestro granito de arena a la lucha contra la hipertensión arterial (HTA), utilizando una herramienta educativa de aprendizaje en servicio (ApS), que permita a los estudiantes de los Grados en ciencias de la salud de la UCM aprender a la vez que realizan un servicio público. La hipertensión arterial (HTA) es un problema de salud pública y un importante factor de riesgo de padecer enfermedades cardiovasculares (ECV). Su diagnóstico está determinado por la medida de la presión arterial (PA) por lo que es de extrema importancia asegurar una medida fiable y válida. La automedida de la PA (AMPA) consiste en la medida de la PA por el propio paciente o un familiar, habitualmente en su domicilio, y es una herramienta útil para el diagnóstico y control de la HTA. Pero ¿sabemos medirnos de forma correcta la PA? ¿Usamos bien los tensiómetros? ¿Sabemos interpretar los registros de la PA y cuándo consultar al médico? Realizar AMPA correctamente no es fácil y exige un esfuerzo extraordinario de recursos humanos y económicos por parte de los profesionales de Atención Primaria. Por ello son necesarias nuevas estrategias para capacitar a la población en AMPA. Con este Proyecto nos proponemos luchar contra la HTA a través de la metodología educativa de aprendizaje-servicio que combina objetivos académicos con el servicio comunitario. El alumnado de los grados de Ciencias de la Salud de la UCM forman a la población en la correcta AMPA, asesorándoles sobre el uso correcto de los tensiómetros, enseñándoles a interpretar los resultados y alertándoles de cuándo acudir al médico. En el curso 2022-2023, el servicio público se ha dirigido principalmente al colectivo de mayores por ser especialmente vulnerables a padecer ECV en los Ayuntamientos de Coslada, Alcobendas y Alcorcón, y en las residencias Afanias y Neurovida, y a la población general que acuda a la Facultad durante la Semana de la Ciencia CAM/UCM 2022, a la Feria de la Sal

- Abstract
En las últimas décadas, la educación universitaria ha evolucionado hacia un enfoque constructivista en consonancia con las recomendaciones del Espacio Europeo de Educación Superior (EEES). En este paradigma, los estudiantes asumen un papel activo en el proceso de enseñanza-aprendizaje, mientras los profesores actúan como facilitadores. Las metodologías constructivistas fomentan el desarrollo tanto individual como grupal de competencias específicas y genéricas, al tiempo que permiten la inclusión de agentes de evaluación formativa para estimular la crítica y la autocrítica del alumno en su desempeño. En este contexto, surge la necesidad de aplicar el constructivismo a la evaluación, involucrando al estudiante en su propio proceso de evaluación. La autoevaluación y la coevaluación emergen como alternativas concretas para lograrlo. La autoevaluación implica que el estudiante analice y valore de manera sistemática su trabajo durante el proceso de aprendizaje para mejorar resultados y fomentar la autocrítica. Por otro lado, la coevaluación es una evaluación entre compañeros que permite valorar la implicación y actitud de los miembros del grupo, estimulando el aprendizaje colectivo. Las Tecnologías de la Información y Comunicación (TIC) juegan un papel importante en la educación y en la evaluación de los alumnos, diferenciándose de las prácticas tradicionales. La implementación de TIC no solo desarrolla habilidades en el proceso enseñanza-aprendizaje, sino también favorece la autoevaluación y la coevaluación. Con base en este enfoque, se presenta un proyecto de innovación docente en la asignatura de Fisiopatología para estudiantes de Farmacia. 2023 Acuerdos transformativos CRUE, Abnormal endothelin-1 (ET-1) activity is involved in the pathogenesis of vascular diseases such as essential and pulmonary arterial hypertension, coronary artery disease, and cerebrovascular disease, blockade of ET receptors having shown efficacy in clinical assays and experimental models of hypertension. Augmented Ca2+ influx and changes in Ca2+ sensitization associated with arterial vasoconstriction underlie increased systemic vascular resistance in hypertension. Since peripheral resistance arteries play a key role in blood pressure regulation, we aimed to determine here the specific Ca2+ signaling mechanisms linked to the ET receptor-mediated vasoconstriction in resistance arteries and their selective regulation by protein kinase C (PKC), Rho kinase (RhoK), the phosphatidylinositol 3-kinase (PI3K) and the mitogen-activated protein kinase (MAPK). ET-1-induced contraction was mediated by the endothelin ETA receptor with a minor contribution of vascular smooth muscle (VSM) endothelin ETB receptors. ET receptor activation elicited Ca2+ mobilization from intracellular stores, extracellular Ca2+ influx and Ca2+ sensitization associated with contraction in resistance arteries. Vasoconstriction induced by ET-1 was largely dependent on activation of canonical transient receptor potential channel 3 (TRPC3) and extracellular Ca2+ influx through nifedipine-sensitive voltage-dependent Ca2+ channels. PI3K inhibition reduced intracellular Ca2+ mobilization and Ca2+ entry without altering vasoconstriction elicited by ET-1, while PKC has dual opposite actions by enhancing Ca2+ influx associated with contraction, and by inhibiting Ca2+ release from intracellular stores. RhoK was a major determinant of the enhanced sensitivity of the contractile filaments underlying ET-1 vasoconstriction, with also a modulatory positive action on Ca2+ influx and intracellular Ca2+ release. Objective: To know the prevalence of visual impairment in children from pre-selected public schools, as well as to analyze and intervene in it. Methods: The study was conducted in four stages. During the first and second ones, school professionals and medical students were trained in visual screening to perform the visual acuity of children in the first years of Elementary School. Children with altered acuity received ophthalmologic referral for diagnosis and intervention. Results: Of the 450 children screened, 64 (14.22%) were referred for complete ophthalmologic examination, of whom 52 (81.25%) attended the consultation. Of these, 37 (71.1%) had refractive errors as the main cause of visual impairment. Nineteen (51.35%) were diagnosed with myopia, 9 (17.3%) with hyperopia, and 9 (17.3%) with astigmatism, who were referred to ophthalmologic follow-up. Conclusion: The prevalence of refractive errors in the child population of municipal public schools was the main cause of visual impairment. [ABSTRACT FROM AUTHOR]

Introduction: Head and neck cancer is an umbrella term for tumor manifestations across the head and neck regions, including the oral cavity, pharynx (including the naso, oro, and hypopharynx), larynx, and sinuses. Treatment options for head and neck cancer include surgery, radiation therapy, chemotherapy, and immunotherapy, with specific treatment plans depending upon individual tumor location and staging, together with overall patient health status. Furthermore, definitive chemoradiotherapy (CRT) has emerged as a highly effective therapeutic option for locoregional advanced head and neck squamous cell cancer. However, such therapy has also been linked to the development of spondylodiscitis. Spondylodiscitis consists of an infection starting at the vertebral endplates and spreading into the intervertebral discs, typically manifesting in adults. Case Presentation and Conclusion: This case report describes our clinical team's experience in managing three separate cases of spondylodiscitis following CRT for head and neck tumors that presented at our clinic for diagnosis and treatment in order to identify predisposing factors that underlie the link between CRT and spondylodiscitis. O tromboembolismo venoso é uma condição que pode ser fracionada em trombose venosa profunda e tromboembolismo pulmonar. Estas compõe este espectro a qual proporcionalmente ao estado hemodinâmico, cursando não obrigatoriamente com o trio típico, exemplificada pela dispneia, dor torácica pleurítica e hemoptise, sendo a dispneia o mais clássico. Estima-se que a minoria dos portadores desta condição são devidamente identificados e tratados, devido o enquadramento isolado não propiciar confirmação ou exclusão patológica. Objetivos: Descrever o quadro clínico do paciente portador de tromboembolismo venoso, no intuito de favorecer o diagnóstico e manejo precoce e principalmente o bom prognóstico do mesmo. Trata-se de uma revisão bibliográfica de literatura fundamentada nas plataformas do SciELO, PubMed, Lilacs e Google Scholar no período de janeiro a março de 2023, utilizando se os seguintes descritores tromboembolismo venoso; embolia pulmonar. Resultados e Discussão: O trombobolismo venoso é uma condição de elevada morbidade e complicações, associado ao fato de que este pode cursar de modo silencioso e culminar em deteriorações e até ao óbito.

Background: Type 2 Diabetes (T2D) diagnosis is based solely on glycaemia, even though it is an endpoint of numerous dysmetabolic pathways. Type 2 Diabetes complexity is challenging in a real-world scenario; thus, dissecting T2D heterogeneity is a priority. Cluster analysis, which identifies natural clusters within multidimensional data based on similarity measures, poses a promising tool to unravel Diabetes complexity. Methods: In this review, we scrutinize and integrate the results obtained in most of the works up to date on cluster analysis and T2D. Results: To correctly stratify subjects and to differentiate and individualize a preventive or therapeutic approach to Diabetes management, cluster analysis should be informed with more parameters than the traditional ones, such as etiological factors, pathophysiological mechanisms, other dysmetabolic co-morbidities, and biochemical factors, that is the millieu. Ultimately, the above-mentioned factors may impact on Diabetes and its complications. Lastly, we propose another theoretical model, which we named the Integrative Model. We differentiate three types of components: etiological factors, mechanisms and millieu. Each component encompasses several factors to be projected in separate 2D planes allowing an holistic interpretation of the individual pathology. Conclusion: Fully profiling the individuals, considering genomic and environmental factors, and exposure time, will allow the drive to precision medicine and prevention of complications. publishersversion published

RESUMO: A prevalência alarmante de doenças crónicas não comunicáveis e, em particular, da diabetes continua a aumentar. A International Diabetes Federation (IDF) estimou um aumento de 50% da prevalência da diabetes até 2040. Desde a descoberta da insulina e a disponibilização de várias classes de medicamentos antidiabéticos orais, o impacto da diabetes deve-se principalmente ao aparecimento de complicações crónicas. As complicações da diabetes é um dos principais problemas de saúde da população, apesar de todo o investimento feito anualmente, contribuindo para a ameaça da sustentabilidade dos cuidados de saúde. Nos últimos 15 anos, o aumento dos custos dos cuidados de saúde atribuíveis à diabetes atingiu 315%. É importante notar que a diabetes tem características comuns com outras doenças crónicas não comunicáveis e que também partilham fatores de risco (hipertensão, dislipidemia e obesidade). A diabetes é definida como uma alteração no metabolismo ou ação da insulina que cursa com o aumento dos valores de glicose no sangue. Assim, o diagnóstico é baseado em valores de corte de glicémia definidos operacionalmente. No entanto, há pessoas com diabetes que nunca desenvolvem complicações enquanto as pessoas sem diabetes podem já mostrá-las. Um dos fatores que tem sido apontado para explicar o cenário preocupante acima mencionado é que a diabetes é uma condição heterogénea. É cada vez mais reconhecido que é possível que existam vários subtipos desta doença, para além dos classicamente identificados. Mas, além disso, podem existir outros fatores que contribuem para a heterogeneidade da diabetes: 1) a insulina tem várias ações no nosso organismo que intervêm no metabolismo de outros substratos, impactando assim em fatores de risco da diabetes e das suas complicações para além da glicemia; 2) o metabolismo da glicose ocorre de forma integrada com outros mecanismos, bem como com o metabolismo de outros substratos, tais como os lípidos. Assim, poder-se-á perguntar: a diabetes é apenas uma questão de hiperglicemia? Resolver a heterogeneidade da diabetes e identificar subgrupos para os quais as intervenções de cuidados de saúde podem ser definidas de uma forma direcionada e específica é um enorme desafio. Nos últimos anos, o conceito de medicina de precisão traz a oportunidade de dar o tratamento certo à pessoa certa no momento certo. O recente desenvolvimento do poder e memória computacionais, e de algoritmos matemáticos que podem resolver problemas que envolvem a análise de grandes quantidades de dados, está a ajudar a tornar a medicina de precisão uma realidade. Uma abordagem baseada em dados tem a vantagem de nos permitir olhar para estes problemas sem pressupostos e, assim, podemos ter como objetivo compreender melhor naturalmente estas condições. Neste trabalho aplicamos a metodologia de Systems Medicine à análise de coortes detalhadamente fenotipados, a fim de estratificar a população no que diz respeito a doenças crónicas não comunicáveis, e mais especificamente à diabetes e aos seus fatores de risco, no contexto de uma abordagem de medicina de precisão. Desta forma, pode ser construído um modelo de diagnóstico e/ou abordagem integrado que tenha aplicação na prática clínica corrente e apoie a inversão do impacto que as doenças crónicas não comunicáveis atualmente têm sobre as pessoas afetadas, as suas famílias e, em última análise, sobre a sociedade. Em primeiro lugar, colocamos a hipótese de que os mecanismos fisiopatológicos das doenças metabólicas determinam quantitativamente parâmetros clínicos, relacionados com a glicose, insulina e metabolismo dos ácidos gordos livres, explicando perfis dismetabólicos distintos. Utilizando a análise de clusters, procurámos estratificar a glucose no sangue contextualizada na multidimensionalidade metabólica e extrair conhecimentos fisiopatológicos do perfil metabólico. Realizámos uma análise de clusters para estratificar 974 indivíduos (coorte PREVADIAB2) com normoglicemia, pré-diabetes, ou diabetes não tratada. O algoritmo foi informado por idade, antropometria, e parâmetros metabólicos (glucose, insulina, peptídeo C, e ácidos gordos livres durante o teste de tolerância à glucose oral. Para a definição do perfil dos clusters, utilizámos adicionalmente índices de mecanismos (dis)metabólicos. Encontrámos uma heterogeneidade proeminente dentro de dois clusters, representando principalmente o normometabolismo (Cluster-I) ou a resistência à insulina e a doença de fígado gordo não alcoólico (Cluster-II). Isto foi ilustrado por subclusters que mostraram uma prevalência semelhante de NAFLD, mas diferenciados por glicemia, FFA e taxa de filtração glomerular (GFR) (Cluster-II). Os subclusters com glicemia e FFA semelhantes mostraram uma clearance e secreção de insulina distintos (Cluster-I). É importante notar que este trabalho revelou que a heterogeneidade da diabete tipo 2 (T2D) pode ser capturada por um perfil metabólico detalhado bem como pelos mecanismos que lhe estão subjacentes, a que no conjunto chamamos pegada metabólica (metabolic footprint). Considerando que a insulina também afeta o metabolismo lipídico, e que a glicose e o metabolismo lipídico estão altamente interligados, colocámos então a hipótese de que os indivíduos estratificados pela resistência à insulina, secreção de insulina e clearance da insulina, três mecanismos fisiopatológicos chave na diabetes, têm perfis distintos de glicose e lipídos no sangue. O nosso objetivo foi avaliar o impacto dos mecanismos associados às condições dismetabólicas tanto na glicémia, como no metabolismo lipídico. Para este fim, realizámos uma análise de clusters com a coorte PREVADIAB, informada por índices dos mecanismos-chave acima mencionados. Adicionalmente, os clusters foram perfilados também com outros índices, incluindo índices de resistência à insulina específicos de órgão, bem como várias espécies de lípidos. Verificámos que sujeitos homogéneos nos mecanismos têm perfis de glicose distintos, mas também de lípidos. Estes perfis podem explicar melhor as complicações da diabetes. Verificámos adicionalmente que homens e mulheres tinham padrões de clusters semelhantes, no entanto, os centróides dos clusters eram diferentes. Isto poderia sugerir e explicar riscos distintos para a diabetes e complicações da diabetes de ambos os sexos. No entanto, pode também sugerir que se tenha de abordar as mulheres e os homens separadamente, considerando diferentes valores de corte para a doença metabólica. Estas hipóteses devem ser avaliadas em trabalhos futuros. Finalmente, o nosso objetivo foi prever mutações genéticas através do conhecimento de poucos parâmetros do meio metabólico, utilizando modelos de Machine Learning (ML). Colocámos a hipótese de que os modelos de ML podem superar o Dutch Lipid Score, o padrão de ouro para o rastreio de casos de hipercolesterolémia familiar, para detetar mutações genéticas causadoras desta condição. Utilizámos 3 algoritmos de ML informados por idade, LDL-c e TG e comparámo-los com os resultados obtidos utilizando o Dutch Lipid Score. Os nossos resultados mostraram que, considerando a idade e alguns elementos do meio metabólico, é possível prever uma mutação genética, sugerindo que podemos prever um fator etiológico (mutação genética), conhecendo o meio metabólico. Os resultados deste trabalho sugerem que a diabetes é uma condição heterogénea. No entanto, deve ser abordada num contexto mais amplo, considerando outros fatores patológicos que estão intimamente ligados aos níveis glicémicos e que são também afetados pela deficiência absoluta ou relativa de insulina. Propomos o Modelo Integrativo para a abordagem da diabetes. Este modelo permite não só abordar a heterogeneidade da condição, mas também contextualiza a diabetes, ou hiperglicemia resultante da alteração do metabolismo da insulina, numa perspetiva mais ampla do dismetabolismo. Considera, juntamente com a glicemia, outros fatores que também podem ser afetados e contribuir para o aparecimento da diabetes e complicações da diabetes. Inspirados pelo Modelo de Paleta, propomos que devem ser considerados 3 planos multidimensionais (etiologia; milieu; e mecanismos) ao longo do tempo. Sugerimos que é possível prever onde uma pessoa se encontra em cada um dos planos mencionados, conhecendo a sua posição nos outros. No seu conjunto, estes planos colocam o indivíduo num caminho para o dismetabolismo, e permitirão prever as complicações da diabetes. Este modelo permitirá identificar as intervenções mais adequadas para cada pessoa, ou seja, numa abordagem de medicina de precisão. The alarming prevalence of non-communicable diseases (NCDs), and particularly diabetes prevalence, continues to increase. International Diabetes Federation estimated an increase of 50% of diabetes prevalence by 2040. Since the discovery of insulin and the availability of various classes of oral antidiabetic drugs, the impact of diabetes is mainly due to the onset of its chronic complications. The latter remains one of the populations' major public health problems despite all the investment made each year, contributing to the threat of healthcare sustainability. In the last 15 years, the increase in health care costs attributable to diabetes reached 315%. Importantly, diabetes has common features with other NCDs and they also share risk factors (dyslipidemia, hypertension, obesity). Diabetes is defined as an alteration in the metabolism or action of insulin that progresses with increased blood glucose values. Thus, the diagnosis is based on operationally defined blood glucose cut-offs. Nevertheless, there are people with diabetes who never develop complications while people without diabetes may already have them. One of the factors that has been pointed out to explain the above-mentioned worrying scenario is diabetes heterogeneity. It is increasingly recognized that there are several subtypes of this condition in addition to the classically recognized ones. Still, there may be other factors contributing to this situation: 1) insulin has several actions in our body intervening in the metabolism of other substrates, thus impacting on diabetes complications risk factors other than glycemia; 2) glucose metabolism occurs in an integrated manner with other mechanisms as well as the metabolism of other substrates, such as lipids. Therefore, one might ask, is diabetes just about hyperglycemia? Resolving the heterogeneity of diabetes and identifying subgroups for which health care interventions can be defined in a targeted and specific manner is an enormous challenge. In recent years the concept of precision medicine has raised the opportunity to give the right medicine to the right person at the right time. The recent development of memory computing power and mathematical algorithms that can solve problems involving the analysis of large amounts of data is helping to make precision medicine a reality. A data-driven approach has the advantage of allowing us to look at these problems without a pre-assumption and thus we can aim to better understand these conditions. In this work we apply System Medicine methodology to analyze cohorts based on detailed phenotyping, to stratify the population with respect to NCDs, more specifically diabetes and their risk factors, to be useful to precision medicine approaches. In this way, a model of integrated diagnosis and approach can be built that has application in current clinical practice and supports reversing the impact that currently NCD’s have on the affected persons, their families and ultimately in society. We first hypothesize that the pathophysiological mechanisms of metabolic diseases quantitatively determine clinical parameters related to glucose, insulin, and free fatty acid metabolism, explaining distinct dysmetabolic profiles. We aimed at stratifying blood glucose within the metabolic multidimensionality, using cluster analysis, and extract pathophysiological insights from the metabolic profile. We performed cluster analysis to stratify 974 subjects (PREVADIAB2 cohort) with normoglycemia, pre-diabetes, or untreated diabetes. The algorithm was informed by age, anthropometry, and metabolic means (glucose, insulin, C-peptide, and free fatty acid (FFA) during oral glucose tolerance test (OGTT)). For cluster profiling, we additionally used indices of metabolic mechanisms. We found prominent heterogeneity within two optimal clusters, mainly representing normometabolism (Cluster-I) or insulin resistance and non-alcoholic fatty liver disease (NAFLD) (Cluster-II), with higher granularity. This was illustrated by subclusters showing a similar prevalence of NAFLD but differentiated by glycemia, FFA, and glomerular filtration rate (GFR) (Cluster-II). Subclusters with similar glycemia and FFA showed dissimilar insulin clearance and secretion (Cluster-I). Importantly, this work revealed that type 2 diabetes (T2D) heterogeneity can be captured by a thorough metabolic milieu and mechanisms profiling the metabolic footprint. Considering that insulin also affects lipid metabolism, and that glucose and lipid metabolism are highly interconnected, we hypothesized that subjects stratified by three key pathophysiological mechanisms of diabetes (insulin resistance, insulin secretion, and insulin clearance) have distinct glucose and lipid profiles. We aimed to assess the impact of mechanisms associated with dysmetabolic conditions on both glucose and lipid metabolism. To this end, we performed a cluster analysis with the PREVADIAB cohort, informed by surrogate indexes of the abovementioned key mechanisms and profiled by other organ-specific indices of insulin resistance, as well as several key lipid species. We found that subjects homogenous in their mechanism have distinct glucose but also lipid profiles. Together these profiles might better explain diabetes's complications. Interestingly, we additionally found that men and women had similar clusters patterns; nonetheless the cluster centroids were different. This might represent a distinct risk for diabetes and diabetes complications of both genders. Nonetheless, it might additionally suggest that one might have to approach women and men separately, considering different cut-offs for the disease. These hypotheses should be evaluated in future work. Finally, we aimed to predict genetic mutations by knowing few parameters of the metabolic milieu using Machine Learning (ML) models. We hypothesize that ML models can outperform the Dutch Lipid Score, the gold standard for screening familial hypercholesterolemia (FH) cases, to detect genetic mutations of familial hypercholesterolemia. We used 3 ML algorithms informed by age, low density lipoprotein cholesterol (LDL-c), high density lipoprotein cholesterol (HDL-c) and triglycerides (TG) and compared them with the results obtained using the Dutch Lipid Score. Our results showed that by considering age and some elements of the milieu it is possible to predict a gene mutation, suggesting that we can predict an etiological factor (gene mutation) by knowing the metabolic milieu. Altogether this work suggests that diabetes is an heterogenous condition. Nonetheless it should be approached in a broader context, considering other pathological factors that are intimately connected with glycemic levels and are also impacted by insulin absolute or relative deficiency. We propose the Integrative Model for approaching diabetes. This model allows not only to address the heterogeneity of the condition but furthermore it contextualizes diabetes, or hyperglycemia resulting from altered insulin metabolism, in a broader perspective of dysmetabolism. It considers, along with glycemia, other factors that may also be affected and contribute to the onset of diabetes and diabetes complications. Inspired by the Palette Model, we propose that three multidimensional planes (etiology, milieu, and mechanisms) should be considered through time. We propose that we can predict where a person is in each of the planes by knowing the others, and that they ultimately place a subject on a path to dysmetabolism, and will allow to predict diabetes complications, and identify the most appropriate interventions for each person. In conclusion the Integrative Model will allow a precision medicine approach to diabetes and NCDs.

Mutations in genes coding for proteasome subunits and/or proteasome assembly helpers typically cause recurring autoinflammation referred to as chronic atypical neutrophilic dermatosis with lipodystrophy and elevated temperatures (CANDLE) or proteasome-associated autoinflammatory syndrome (PRAAS). Patients with CANDLE/PRAAS present with mostly chronically elevated type I interferon scores that emerge as a consequence of increased proteotoxic stress by mechanisms that are not fully understood. Here, we report on five unrelated patients with CANDLE/ PRAAS carrying novel inherited proteasome missense and/or nonsense variants. Four patients were compound heterozygous for novel pathogenic variants in the known CANDLE/PRAAS associated genes, PSMB8 and PSMB10, whereas one patient showed additive loss-of-function mutations in PSMB8. Variants in two previously not associated proteasome genes, PSMA5 and PSMC5, were found in a patient who also carried the PSMB8 founder mutation, p.T75M. All newly identified mutations substantially impact the steady-state expression of the affected proteasome subunits and/or their incorporation into mature 26S proteasomes. Our observations expand the spectrum of PRAAS-associated genetic variants and improve a molecular diagnosis and genetic counseling of patients with sterile autoinflammation. [ABSTRACT FROM AUTHOR]

Ester hydrolysis is of wide biomedical interest, ranging from the environmentally friendly synthesis of pharmaceuticals to the development of biomaterials. Existing peptide-based catalysts exhibit low catalytic efficiency compared to natural enzymes, which is due to the conformational heterogeneity of peptides. Moreover, the correlation between the primary sequence and the catalytic function is not yet well understood. First, we statistically analyzed 22 enzymes that use unique triad mechanisms to perform ester hydrolysis (EC 3.1). We extended this analysis to 974 homologues to verify the identified microenvironment motifs. The goal was to identify sequence-level patterns that will better inform the creation of short peptides that contain important information for catalysis based on the catalytic triad, oxyanion holes, and microenvironments of triad residues. The results showed highly conserved catalytic sites with distinct positional patterns, chemical microenvironments conducive to catalysis, and revealed variations in the composition of catalytic sites that could be useful for the design of minimalistic catalysts. Linear and cyclic peptides were then synthesized and their ability to catalyze pNPA hydrolysis was tested. Control over the catalytic performance of peptides was achieved through modifications at the sequence level and by varying the chemical environment.

As a global pandemic is endangering our health system, there is a need for novel therapeutic and diagnostic tools. Peptide nanomaterials have emerged as a possible solution as they are able to mimic the viral morphology and/or functionality through self-assembly. We have analyzed the SARS- CoV-2 spike (S) protein interaction with the ACE2 and NRP1 receptors that enable viral entry into cells and designed three amphiphilic peptide sequences derived from the S-protein receptor binding domain (RBD)1, 2. Herein we show the results for only one, FFLYYKK. The design was based on the idea that such peptides can self- assemble into higher order spherical aggregates resembling the supramolecular architecture of SARS-CoV-2 and therefore used as safe platforms to study viral infections.

Kidney function in metabolism is often underestimated. Although the word “clearance” is associated to “degradation”, at nephron level, proper balance between what is truly degraded and what is redirected to de novo utilization is crucial for the maintenance of electrolytic and acid–basic balance and energy conservation. Insulin is probably one of the best examples of how diverse and heterogeneous kidney response can be. Kidney has a primary role in the degradation of insulin released in the bloodstream, but it is also incredibly susceptible to insulin action throughout the nephron. Fluctuations in insulin levels during fast and fed state add another layer of complexity in the understanding of kidney fine-tuning. This review aims at revisiting renal insulin actions and clearance and to address the association of kidney dysmetabolism with hyperinsulinemia and insulin resistance, both highly prevalent phenomena in modern society. publishersversion published

This report aims to present and analyse the activities undertaken at Porto Editora publishing house, in the context of the curricular internship which took place between November 2019 and March 2020, within the scope of the Masters Degree in Publishing Studies at the University of Aveiro. This report also explores the evolution of Coolbooks, an imprint of Porto Editora, and includes a reflection on the publishing decision process, as well as the editing process. - Abstract
CRUE-CSIC (Acuerdos Transformativos 2021), Background and Purpose Obesity is a risk factor for the development of chronic kidney disease independent of diabetes, hypertension and other co-morbidities. Obesity-associated nephropathy is linked to dysregulation of the cell energy sensor AMP-activated protein kinase (AMPK). We aimed here to assess whether impairment of AMPK activity may cause renal arterial dysfunction in obesity and to evaluate the therapeutic potential of activating renal AMPK. Experimental Approach Effects of the AMPK activator A769662 were assessed on intrarenal arteries isolated from ob/ob mice and obese Zucker rats and then mounted in microvascular myographs. Superoxide and hydrogen peroxide production were measured by chemiluminescence and fluorescence, respectively, and protein expression was analysed by western blotting. Key Results Endothelium-dependent vasodilation and PI3K/Akt/eNOS pathway were impaired in preglomerular arteries from genetically obese rats and mice, along with impaired arterial AMPK activity and blunted relaxations induced by the AMPK activator A769662. Acute ex vivo exposure to A769662 restored endothelial function and enhanced activity of PI3K/Akt/eNOS pathway in obese rats, whereas in vivo treatment with A769662 improved metabolic state and ameliorated endothelial dysfunction, reduced inflammatory markers and vascular oxidative stress in renal arteries and restored redox balance in renal cortex of obese mice. Conclusion and Implications These results demonstrate that AMPK dysregulation underlies obesity-associated kidney vascular dysfunction and activation of AMPK improves metabolic state, protects renal endothelial function and exerts potent vascular antioxidant and anti-inflammatory effects. - Abstract
Grafting is an ancient plant propagation technique widely used in horticultural crops, particularly in fruit trees. However, the involvement of two different species in grafting may lead to lack of affinity and severe disorders between the graft components, known as graft-incompatibility. This complex agronomic trait is traditionally classified into two categories: “localized” (weak graft unions with breaks in cambial and vascular continuity at the graft interface and absence of visual symptoms in scion leaves and shoots) and “translocated” (degeneration of the sieve tubes and phloem companion cells at the graft interface causing translocation problems in neighboring tissues, and reddening/yellowing of scion leaves). Over the decades, more attention has been given to the different mechanisms underlying the “localized” type of graft-incompatibility; whereas the phenylpropanoid-derived compounds and the differential gene expression associated with the “translocated” graft-incompatibility remain unstudied. Therefore, the aim of this study was to shed light on the biochemical and molecular mechanisms involved in the typical “translocated” graft-incompatibility of peach/plum graft-combinations. In this study, the “Summergrand” (SG) nectarine cultivar was budded on two plum rootstocks: “Adara” and “Damas GF 1869”. “Translocated” symptoms of incompatibility were shown and biochemically characterized in the case of “SG/Damas GF 1869” graft-combination, 3 years after grafting. Non-structural carbohydrates (soluble sugars and starch), phenolic compounds and antioxidant activity, were significantly enhanced in the incompatible graft-combination scion. Similarly, the enzymatic activities of the antioxidant enzyme peroxidase, the phenylalanine ammonia-lyase (PAL) and polyphenol oxidase involved in the phenylpropanoid pathway were significantly affected by the incompatible rootstock “Damas GF 1869”, inducing higher activities in the scion than those induced by the compatible root
