Alternating Hemiplegia of Childhood (AHC) is a rare neurological disease characterized by early-onset recurrent paroxysmal events and persistent neurological deficits. TBC1D24 gene variants have been associated with a phenotypic spectrum having epilepsy as the main clinical manifestation. Herein, we report the case of a child affected by developmental delay, polymorphic seizures, and nonepileptic episodes characterized by hemiplegia or bilateral plegia, pallor, hypotonia, and dystonic postures without loss of consciousness that resolved with sleep. Noteworthy, the patient fulfills all the diagnostic criteria for AHC. An epilepsy gene panel revealed a novel TBC1D24 mutation. This variant may be considered a PM5, according to the American College of Medical Genetics and Genomics guidelines. TBC1D24 gene variants are associated with various clinical features, and increasing data confirms the association with permanent and paroxysmal movement disorders. Our report suggests that the TBC1D24 molecular analysis could be considered in the diagnostic workup of AHC patients.

Damage to the cerebellum during posterior fossa surgery can lead to ataxia and in paediatric cases, the risk of cerebellar mutism syndrome. Animal electrophysiological and human imaging studies have shown compartmentalisation of sensorimotor and cognitive functions within the cerebellum. In the present study, electrophysiological monitoring of sensory and motor pathways was carried out to assess the location of limb sensorimotor representation within the human cerebellum, as a potential approach for real time assessment of neurophysiological integrity to reduce the incidence of cerebellar surgical morbidities.Thirteen adult and paediatric patients undergoing posterior fossa surgery were recruited. For sensory mapping (n=8), electrical stimulation was applied to the median nerves, the posterior tibial nerves, or proximal and distal limb muscles and evoked field potential responses were sought on the cerebellar surface. For motor mapping (n=5), electrical stimulation was applied to the surface of the cerebellum and evoked EMG responses were sought in facial and limb muscles.Evoked potentials on the cerebellar surface were found in two patients (25% of cases). In one patient, the evoked response was located on the surface of the right inferior posterior cerebellum in response to stimulation of the right leg. In the second patient, stimulation of the extensor digitorum muscle in the left forearm evoked a response on the surface of the left inferior posterior lobe. In the motor mapping cases no evoked EMG responses could be found.Intraoperative electrophysiological mapping, therefore, indicates it is possible to record evoked potentials on the surface of the human cerebellum in response to peripheral stimulation.

Stroke is among the top 10 causes of death in children and survivors carry resulting disabilities for decades, at substantial cost to themselves and their families. Children are not currently able to access reperfusion therapies, due to limited evidence supporting safety and efficacy and long diagnostic delays. The Australian Clinical Consensus Guideline for the Diagnosis and Acute Management of Childhood Stroke was developed to minimize unwarranted variations in care and document best evidence on the risk factors, etiologies, and conditions mimicking stroke that differ from adults. Clinical questions were formulated to inform systematic database searches from 2007 to 2017, limited to English and pediatric studies. SIGN methodology and the National Health and Medical Research Council system were used to screen and classify the evidence. The Grades of Recommendation, Assessment, Development, and Evaluation system (GRADE) was used to grade evidence as strong or weak. The Guideline provides more than 60 evidence-based recommendations to assist prehospital and acute care clinicians in the rapid identification of childhood stroke, choice of initial investigation, to confirm diagnosis, determine etiology, selection of the most appropriate interventions to salvage brain at risk, and prevent recurrence. Recommendations include advice regarding the management of intracranial pressure and congenital heart disease. Implementation of the Guideline will require reorganization of prehospital and emergency care systems, including the development of regional stroke networks, pediatric Code Stroke, rapid magnetic resonance imaging and accreditation of primary pediatric stroke centers with the capacity to offer reperfusion therapies. The Guideline will allow auditing to benchmark timelines of care, access to acute interventions, and outcomes. It will also facilitate the development of an Australian childhood stroke registry, with data linkage to international registries, to allow for accurate data collection on stroke incidence, treatment, and outcomes.

Forest management activities have the potential to significantly modify large wood loads in riparian areas, and thus the availability of material for recruitment into the stream channel, hence it is important that forest management practices take account of the role wood supplied from riparian areas play in the development of channel structure and sediment retention capacity. In this study the cellular landscape and river reach process model CAESAR LISFLOOD (C-L) was used to simulate the effect of in-channel obstructions created by large wood on the distribution of erosion and deposition within a small headwater channel and suspended sediment yield at the catchment outlet. The process model was parameterised and calibrated against an observed discharge and suspended sediment record. Following calibration, 12 discharge and large wood obstruction scenarios were carried out to model erosion and suspended sediment transport over a timber harvesting cycle. The inclusion of obstructions into the model runs to simulate in-channel LW pieces substantially reduced suspended sediment yields in all three discharge scenarios and increased sediment storage along the channel. The modelled location of channel bed degradation and aggradation generally coincided with the pattern of sediment storage along the channel bed demonstrating that C_L was predicting the pattern of erosion and deposition in the channel reasonably well. Modelled erosion rates were extremely low for the respective discharge and large wood model scenarios, with the highest rates ranging from 0.02 t ha−1 year−1 to 0.1 t ha−1 year−1 for the no large wood model over the 22-year period. These findings reinforce the importance of maintaining sufficient volume of standing wood in buffer strips alongside small headwater channels to ensure ongoing recruitment of large wood to the channel network in forests managed for timber production.

City planning draws out relationships between land use patterns, housing form and density, transport, energy provision and other infrastructures. While commonly struggling with the task, a city’s planning system could be one of the key integrating forces for tackling the sustainability challenge. As it happens, these particular systems are quite regularly subject to coordinated and relatively well resourced reform efforts, usually not generated from a sustainability viewpoint. This chapter raises the question of whether, and how, transition management ideas might more directly enter and influence this “turbulent” reform space. Sydney provides the case-setting for the introduction of transition management, and related conceptions, to a recent major planning system reform project for that city. Working from a theoretical meshing of planning, relational-institutional and transitions studies, a new process model for planning system reform projects is introduced: the Planning System Transition (PST) framework. This framework’s development and testing, through empirical research, is explained. This chapter suggests positive prospects for transition management in city planning reform, but also highlights the critical requirement for contextualisation with practical and scholarly planning insights, as exemplified in the PST framework.

Background: Vincristine is a chemotherapeutic agent frequently used in the treatment of childhood cancer. Peripheral neuropathy is an important side effect of Vincristine. We assessed the incidence, severity and type of peripheral neuropathy in children on Vincristine using clinical parameters and nerve conduction studies. Methods : Children with cancer treated with Vincristine were enrolled. Symptoms and signs of motor, sensory and autonomic peripheral neuropathy at baseline and at 3, 6 and 12 month follow up were documented. A modified total neuropathy score grading was undertaken. Nerve conduction studies (NCS) were undertaken and vitamin E levels measured at the time of these assessments. Results: 28 patients were enrolled over 12 months. 93% had evidence of neuropathy clinically and/or electrophysiologically. Weakness, pain and constipation were the most common symptoms and the most common sign was loss of ankle reflexes. The peripheral neuropathy was mild in most cases. Plasma levels of vitamin E remained in the normal range, however they dropped significantly from the baseline value. Conclusions: Peripheral neuropathy assessed clinically and electrophysiologically, occurs in the majority of children on Vincristine treatment. The neuropathy is sensory-motor and generally mild. © 2018 Elsevier Ltd Sydney has emerged as a major global city in the 21st century. We review the “global city thesis”, which dominates urban scholarship and practice, and ask whether it adequately captures the Sydney experience. Although the global city thesis is a useful analytical construct for policy makers and scholars, we argue that it does not adequately chart Sydney's rise as a leading global city and its current problems. The global city thesis ignores the political institutions and processes that shape and direct the global city. The City of Sydney is a small area of the city-region, accounting for only about four percent of the metropolis. Sydney lacks a metropolitan or regional government and has few regional collaborative processes or platforms. Instead the global city strategy of Sydney is shaped and directed by the New South Wales state government. This is contrary to the political decentralisation and devolution trends heralded by international actors such as the Organisation for Economic Co-operation and Development. Sydney also illustrates the dilemma of global cities in that those members at the top of the knowledge economy are highly rewarded and those in the middle and lower strata face difficulty maintaining or improving their situations. Local governments lack the capacity to act independently and the state and federal governments are unwilling to address serious urban problems associated with globalisation, such as public transit or housing. Given the governments' embrace of neoliberalism, the global city vision advanced by leaders is threatened. There is little concrete policy offered by any level of government to address the crisis.

With a high prevalence of well-established risk dementia risk factors (e.g. tobacco, alcohol use, hypertension, and diabetes), there is a dire need for data on prevalence and correlates of cognitive impairment and related service utilization among older Korean Americans. Memory and Aging study of Koreans (MASK) is a two-stage epidemiological survey with two-step sampling strategy to obtain a representative sample. We screened 1,118 older Korean Americans (mean age: 70.7, female: 67.2%) with a Korean version of the Mini-Mental State Exam, and 118 persons sampled from two levels of performance on the MMSE underwent the second stage clinical evaluation that included the Consortium to Establish a Registry for Alzheimer’s Disease (CERAD). Preliminary analysis estimated that community prevalence of mild cognitive impairment in this population was 28.4% (CI: 18.2%-41.5%) and dementia was 7.2% (3.8% - 13.2%). Correlates of cognitive impairment, and potential barriers to the services will be discussed during this presentation.

All hospitalizations during the study period with primary diagnosis of hip fracture were identified. Age- and sex-specific hip fracture rates were calculated for nineteen 5-year age groups and four 5-year calendar periods, resulting in 20 birth cohorts. The effect of age, calendar period, and birth cohort on hip fracture rates was assessed using age-period-cohort models as proposed by Clayton and Schiffers. Introduction L’Agence de la santé publique du Canada, en collaboration avec 12 experts dans le domaine de la santé des os et de l’ostéoporose de partout au Canada, a sélectionné, par un processus de consensus formel, un ensemble d’indicateurs de base pour la surveillance de l’ostéoporose en santé publique. Méthodologie Une revue de la littérature a permis d’identifier des indicateurs potentiels, qui ont été par la suite regroupés dans un cadre d’indicateurs propre à l’ostéoporose. Un sondage a ensuite été élaboré pour connaître l’opinion des experts sur l’importance de ces indicateurs pour la santé publique. Les indicateurs ayant obtenu un score inférieur à 3 sur une échelle de Likert allant de 1 (faible importance) à 5 (importance élevée) ont été exclus de tout examen subséquent. Par la suite, l’importance des autres indicateurs pour la santé publique a été soumise à un vote majoritaire au cours d’une réunion en face-à-face. Résultats La littérature a permis de repérer 111 indicateurs, dont 88 ont été sélectionnés grâce au sondage en vue d’un examen approfondi. Lors de la réunion en face-à-face, 39 indicateurs ont été estimés importants du point de vue de la santé publique par au moins la moitié des experts. Conclusion Cet ensemble d’indicateurs de base servira à orienter la création de nouvelles sources de données ainsi que l’intégration, l’analyse et l’interprétation de données existantes sous la forme de produits de surveillance à des fins d’intervention dans le domaine de la santé publique.

Introduction The Public Health Agency of Canada, in collaboration with bone health and osteoporosis experts from across Canada (n = 12), selected a core set of indicators for the public health surveillance of osteoporosis using a formal consensus process. Methods A literature review identified candidate indicators that were subsequently categorized into an osteoporosis-specific indicator framework. A survey was then administered to obtain expert opinion on the indicators’ public health importance. Indicators that scored less than 3 on a Likert scale of 1 (low) to 5 (high) were excluded from further consideration. Subsequently, a majority vote on the remaining indicators’ level of public health importance was sought during a face-to-face meeting. Results The literature yielded 111 indicators, and 88 were selected for further consideration via the survey. At the face-to-face meeting, more than half the experts considered 39 indicators to be important from the public health perspective. Conclusion This core set of indicators will serve to inform the development of new data sources and the integration, analysis and interpretation of existing data into surveillance products for the purpose of public health action.

We identified hospitalizations throughout Canada during 2000–2005 in which the most responsible diagnosis was a proximal femoral fracture. Use of the US fracture risk assessment tool (FRAX) would be inappropriate for Canada as it would overestimate fracture risk in Canadian women and older men. It is recommended that the WHO fracture risk assessment tool should be calibrated to the target population. We identified hospitalizations for women and men throughout Canada during the study period 2000–2005 in which the most responsible diagnosis was a proximal femoral fracture (147,982 hip fractures). Age-standardized hip fracture rates were compared between Canadian provinces, and national rates were compared with those reported for the USA and Germany. There were relatively small differences in hip fracture rates between provinces, and most did not differ appreciably from the Canadian average. Hip fracture rates for women in Canada in 2001 were substantially lower than in the USA (population-weighted rate ratio 0.70) and were also lower than in Germany for 2004 (population-weighted rate ratio 0.74). Overall hip fracture rates for Canadian women were found to be substantially lower than those for the USA and Germany. This study underscores the importance of assessing country-specific fracture patterns prior to adopting an existing FRAX tool.

Shared services arrangements in the Australian third sector are becoming more common. Notably, however, there is a lack of information to guide nonprofit organisations through the development and engagement of shared structures. This article reviews the lessons that have been recorded from the public and private sectors with respect to the engagement of shared services arrangements. Additionally this article explores the different types of shared services structures that can be adopted. Overall, this article highlights the need for further research and analysis of issues relating to shared services arrangements in order to assist the increasing number of Australian nonprofit organisations engaging these collective arrangements and structures.

Neurophysiological techniques have a long history of clinical and research use in the setting of Multiple Sclerosis. With the output of evoked potential and electroencephalographic recordings being a direct consequence of the underlying functional integrity and activity of the nervous system it is unsurprising there is currently a resurgent interest in their possible use as biomarkers of disease related dysfunction. Herein we discuss the promise and possible pitfalls associated with their use in translational research of MS therapeutics. The numerous advantages of these methods are touched upon as demonstrated by their application to date. A number of obstacles which must be overcome prior to their successful widespread implementation are also outlined and readily achievable solutions discussed.

There is increasing pressure being placed on government agencies both in Australia and internationally to act in a more collaborative, integrated manner. Community and welfare programs are being conducted in new “shared” or “whole‐of‐government” ways, which pose a challenge to traditional models of public sector reporting. Existing accountability mechanisms are designed for vertical accountability relationships, and these are inadequate for horizontal or “networked” accountability across government agencies. This paper uses the case of the Community Renewal Program based in the Queensland state government department to illustrate the problems which arise when reporting on “shared” programs. The paper offers a different approach to improve reporting and accountability for shared programs.

DnaJ is a molecular chaperone and the prototypical member of the J-protein family. J proteins are defined by the presence of a J domain that can regulate the activity of 70-kDa heat-shock proteins. Sequence analysis on the genome of Saccharomyces cerevisiae has revealed 22 proteins that establish four distinguishing structural features of the J domain: predicted helicity in segments I–IV, precisely placed interhelical contact residues, a lysine-rich surface on helix II and placement of the diagnostic sequence HPD between the predicted helices II and III. We suggest that this definition of the J-protein family could be used for other genome-wide studies. In addition, three J-like proteins were identified in yeast that contain regions closely resembling a J domain, but in which the HPD motif is non-conservatively replaced. We suggest that J-like proteins might function to regulate the activity of bona fide J proteins during protein translocation, assembly and disassembly.

TGF-β family members play a relevant role in tumorigenic processes, including hepatocellular carcinoma (HCC), but a specific implication of the Bone Morphogenetic Protein (BMP) subfamily is still unknown. Although originally isolated from fetal liver, little is known about BMP9, a BMP family member, and its role in liver physiology and pathology. Our results show that BMP9 promotes growth in HCC cells, but not in immortalized human hepatocytes. In the liver cancer cell line HepG2, BMP9 triggers Smad1,5,8 phosphorylation and inhibitor of DNA binding 1 (Id1) expression up- regulation. Importantly, by using chemical inhibitors, ligand trap and gene silencing approaches we demonstrate that HepG2 cells autocrinely produce BMP9 that supports their proliferation and anchorage independent growth. Additionally, our data reveal that in HepG2 cells BMP9 triggers cell cycle progression, and strikingly, completely abolishes the increase in the percentage of apoptotic cells induced by long-term incubation in low serum. Collectively, our data unveil a dual role for BMP9, both promoting a proliferative response and exerting a remarkable anti-apoptotic function in HepG2 cells, which result in a robust BMP9 effect on liver cancer cell growth. Finally, we show that BMP9 expression is increased in 40% of human HCC tissues compared with normal human liver as revealed by immunohistochemistry analysis, suggesting that BMP9 signaling may be relevant during hepatocarcinogenesis in vivo. Our findings provide new clues for a better understanding of BMPs contribution, and in particular BMP9, in HCC pathogenesis that may result in the development of effective and targeted therapeutic interventions.

Age-standardized rates of hip fracture in Canada declined during the period 1985 to 2005. We investigated whether this incidence pattern is explained by period effects, cohort effects, or both. Cohorts born before 1950 had a higher risk of hip fracture, whereas those born after 1954 had a lower risk. After adjusting for age and constant annual linear change (drift term common to both period and cohort effects), we observed a significant nonlinear birth cohort effect for males (p = 0.0126) but not for females (p = 0.9960). In contrast, the nonlinear period effect, after adjustment for age and drift term, was significant for females (p = 0.0373) but not for males (p = 0.2515). For males, we observed no additional nonlinear period effect after adjusting for age and birth cohort, whereas for females, we observed no additional nonlinear birth cohort effect after adjusting for age and period. Although hip fracture rates decreased in both sexes, different factors may explain these changes. In addition to the constant annual linear decrease, nonlinear birth cohort effects were identified for males, and calendar period effects were identified for females as possible explanations.

Infection by Leishmania and Trypanosoma causes severe disease and can be fatal. The reduced effectiveness of current treatments is largely due to drug resistance, hence the urgent need to develop new drugs, preferably against novel targets. We have recently identified a mitochondrial membrane-anchored protein, designated MIX, which occurs exclusively in these parasites and is essential for virulence. We have determined the crystal structure of Leishmania major MIX to a resolution of 2.4 A. MIX forms an all α-helical fold comprising seven α-helices that fold into a single domain. The distribution of helices is similar to a number of scaffold proteins, namely HEAT repeats, 14-3-3, and tetratricopeptide repeat proteins, suggesting that MIX mediates protein-protein interactions. Accordingly, using copurification and mass spectroscopy we were able to identify several proteins that may interact with MIX in vivo. Being parasite specific, MIX is a promising new drug target and, thus, the structure and potential interacting partners provide a basis for structure-guided drug discovery.

An attempt is made to solve the problem of the loss caused in a Gaussian beam by an off-axis circular aperture. Although in general the problem cannot be solved in closed form, three special cases are considered. If the offset is zero, the limit of the radial integration coincides with the radius of the circular aperture; in this case, the integration over the angular variable can be performed in closed form. In a second special case, an analytical expression can be obtained when the offset is small. A third special case occurs when the offset is large and equals the aperture radius.

Laboratory measurements of mercury oxidation during selective catalytic reduction (SCR) of nitric oxide, simulation of pilot-scale measurements of mercury oxidation and adsorption by unburned carbon and fly ash, and synthesis of new materials for simultaneous oxidation and adsorption of mercury, were performed in support of the development of technology for control of mercury emissions from coal-fired boilers and furnaces. Conversion of gas-phase mercury from the elemental state to water-soluble oxidized form (HgCl{sub 2}) enables removal of mercury during wet flue gas desulfurization. The increase in mercury oxidation in a monolithic V{sub 2}O{sub 5}-WO{sub 3}/TiO{sub 2} SCR catalyst with increasing HCl at low levels of HCl (< 10 ppmv) and decrease in mercury oxidation with increasing NH{sub 3}/NO ratio during SCR were consistent with results of previous work by others. The most significant finding of the present work was the inhibition of mercury oxidation in the presence of CO during SCR of NO at low levels of HCl. In the presence of 2 ppmv HCl, expected in combustion products from some Powder River Basin coals, an increase in CO from 0 to 50 ppmv reduced the extent of mercury oxidation from 24 {+-} 3 to 1 {+-} 4%. Further increase in CO to 100 ppmv completely suppressed mercury oxidation. In the presence of 11-12 ppmv HCl, increasing CO from 0 to {approx}120 ppmv reduced mercury oxidation from {approx}70% to 50%. Conversion of SO{sub 2} to sulfate also decreased with increasing NH{sub 3}/NO ratio, but the effects of HCl and CO in flue gas on SO{sub 2} oxidation were unclear. Oxidation and adsorption of mercury by unburned carbon and fly ash enables mercury removal in a particulate control device. A chemical kinetic mechanism consisting of nine homogeneous and heterogeneous reactions for mercury oxidation and removal was developed to interpret pilot-scale measurements of mercury oxidation and adsorption by unburned carbon and fly ash in experiments at pilot scale, burning bituminous coals (Gale, 2006) and blends of bituminous coals with Powder River Basin coal (Gale, 2005). The removal of mercury by fly ash and unburned carbon in the flue gas from combustion of the bituminous coals and blends was reproduced with satisfactory accuracy by the model. The enhancement of mercury capture in the presence of calcium (Gale, 2005) explained a synergistic effect of blending on mercury removal across the baghouse. The extent of mercury oxidation, on the other hand, was not so well described by the simulation, because of oversensitivity of the oxidation process in the model to the concentration of unburned carbon. Combined catalysts and sorbents for oxidation and removal of mercury from flue gas at low temperature were based on surfactant-templated silicas containing a transition metal and an organic functional group. The presence of both metal ions and organic groups within the pore structure of the materials is expected to impart to them the ability to simultaneously oxidize elemental mercury and adsorb the resulting oxidized mercury. Twelve mesoporous organosilicate catalysts/sorbents were synthesized, with and without metals (manganese, titanium, vanadium) and organic functional groups (aminopropyl, chloropropyl, mercaptopropyl). Measurement of mercury oxidation and adsorption by the candidate materials remains for future work.

The mitochondrion is an essential cellular compartment in eukaryotes. The mitochondrial proteins Tom20 and Tom22 are receptors that ensure recognition and binding of proteins imported for mitochondrial biogenesis. Comparison of the sequence for the Tom20 and Tom22 subunits in the yeasts Saccharomyces cerevisiae and Saccharomyces castellii, show a rare case of domain stealing, where in Saccharomyces castellii Tom22 has lost an acidic domain, and Tom20 has gained one. This example of domain stealing is a snapshot of evolution in action and provides excellent evidence that Tom20 and Tom22 are subunits of a single, composite receptor that binds precursor proteins for import into mitochondria.

One of the thorniest issues in the relationship between doctors and patients has been how to deal with a dispute. The rhetoric of the past few governments, both Conservative and Labour, has emphasised the need to treat patients as consumers of services provided by the NHS. Patients need to be “at the centre” of all arrangements for planning, delivery, and accountability. At the same time, the private healthcare system in Britain has grown by aggressively marketing its service as modern and responsive to patients' needs as “customers,” and patients are becoming used to making choices both about who provides their services and the nature of the treatment they are to have. The growth in consumerism has been accompanied by an explosion in information systems, most notably the internet, which have made patients more knowledgeable customers when it comes to disease, treatments, and patients' rights. Recently, information on the performance of hospitals has become available through private initiatives such as the Good Hospital Guide ,1 and the Department of Health plans to publish more detailed information on the performance of individual consultants. When there is a dispute, however, the relationship between doctors and patients can still seem rather paternalistic. Feedback from community health councils, which support people with complaints, suggests that many patients still get removed from general practitioners' lists for no other reason than having dared to complain about their doctor's service or attitude.
