Abstract: Indirect Drive Inertial Confinement Fusion Experiments on the National Ignition Facility (NIF) have achieved a burning plasma state with neutron yields exceeding 170 kJ, roughly 3 times the prior record and a necessary stage for igniting plasmas. The results are achieved despite multiple sources of degradations that lead to high variability in performance. Results shown here, for the first time, include an empirical correction factor for mode-2 asymmetry in the burning plasma regime in addition to previously determined corrections for radiative mix and mode-1. Analysis shows that including these three corrections alone accounts for the measured fusion performance variability in the two highest performing experimental campaigns on the NIF to within error. Here we quantify the performance sensitivity to mode-2 symmetry in the burning plasma regime and apply the results, in the form of an empirical correction to a 1D performance model. Furthermore, we find the sensitivity to mode-2 determined through a series of integrated 2D radiation hydrodynamic simulations to be consistent with the experimentally determined sensitivity only when including alpha-heating.

Abstract: An experimental program is currently underway at the National Ignition Facility (NIF) to compress deuterium and tritium (DT) fuel to densities and temperatures sufficient to achieve fusion and energy gain. The primary approach being investigated is indirect drive inertial confinement fusion (ICF), where a high-Z radiation cavity (a hohlraum) is heated by lasers, converting the incident energy into x-ray radiation which in turn drives the DT fuel filled capsule causing it to implode. Previous experiments reported DT fuel gain exceeding unity [O.A. Hurricane et al., Nature 506, 343 (2014)] and then exceeding the kinetic energy of the imploding fuel [S. Le Pape et al., Phys. Rev. Lett. 120, 245003 (2018)]. We report on recent experiments that have achieved record fusion neutron yields on NIF, greater than 100 kJ with momentary fusion powers exceeding 1PW, and have for the first time entered the burning plasma regime where fusion alpha-heating of the fuel exceeds the energy delivered to the fuel via compression. This was accomplished by increasing the size of the high-density carbon (HDC) capsule, increasing energy coupling, while controlling symmetry and implosion design parameters. Two tactics were successful in controlling the radiation flux symmetry and therefore the implosion symmetry: transferring energy between laser cones via plasma waves, and changing the shape of the hohlraum. In conducting these experiments, we controlled for known sources of degradation. Herein we show how these experiments were performed to produce record performance, and demonstrate the data fidelity leading us to conclude that these shots have entered the burning plasma regime.

Non-invasive ventilation may have deleterious effects, especially in patients exerting strong inspiratory efforts, and no current recommendations support its use in this setting. Protective non-invasive ventilation using higher levels of positive-end expiratory pressure, more prolonged sessions and other interfaces such as the helmet may have beneficial physiological effects leading to it being proposed as alternative to high-flow nasal oxygen in acute hypoxemic respiratory failure. By contrast, non-invasive ventilation is the first-line strategy of oxygenation in patients with acute exacerbation of chronic lung disease, while high-flow nasal oxygen could be an alternative to non-invasive ventilation after partial reversal of respiratory acidosis. Questions remain about the target populations and non-invasive oxygen strategy representing the best alternative to standard oxygen in acute hypoxemic respiratory failure. Abstract: Creating non-equilibrium states of matter with highly unequal electron and lattice temperatures allows unsurpassed insight into the dynamic coupling between electrons and ions through time-resolved energy relaxation measurements. Recent studies on low-temperature laser-heated graphite suggest a complex energy exchange when compared to other materials. To avoid problems related to surface preparation, crystal quality and poor understanding of the energy deposition and transport mechanisms, we apply a different energy deposition mechanism, via laser-accelerated protons, to isochorically and non-radiatively heat macroscopic graphite samples up to temperatures close to the melting threshold. Using time-resolved x ray diffraction, we show clear evidence of a very small electron-ion energy transfer, yielding approximately three times longer relaxation times than previously reported. Abstract: Background & Aims: Autoimmune hepatitis (AIH) has been well characterised and codified through the development of diagnostic criteria. These criteria have been adapted and simplified and are widely used in clinical practice. However, there is a need to update and precisely define the criteria for both treatment response and treatment. Methods: A systematic review was performed and a modified Delphi consensus process was used to identify and redefine the response criteria in autoimmune hepatitis. Results: The consensus process initiated by the International Autoimmune Hepatitis Group proposes that the term ‘complete biochemical response’ defined as ‘normalization of serum transaminases and IgG below the upper limit of normal’ be adopted to include a time point at 6 months after initiation of treatment. An insufficient response by 6 months was a failure to meet the above definition. Non-response was defined as ‘<50% decrease of serum transaminases within 4 weeks after initiation of treatment’. Remission is defined as liver histology with a Hepatitis Activity Index <4/18. Intolerance to treatment was agreed to stand for ‘any adverse event possibly related to treatment leading to potential drug discontinuation’. Conclusions: These definitions provide a simple and reproducible framework to define treatment response and non-response, irrespective of the therapeutic intervention. A consensus on endpoints is urgently required to set a global standard for the reporting of study results and to enable inter-study comparisons. Abstract: This report describes the Key Comparison of the Consultative Committee for Photometry and Radiometry CCPR-K1.a.2017 for Spectral Irradiance in the wavelength range of 250 nm to 2500 nm. Tungsten quartz halogen lamps of the FEL type (1000 W) were used as artefacts. Each participant used its own set of lamps. The star scheme of measurements was applied with the sequence participant - pilot - participant. Twelve National Metrology Institutes participated in the comparison. Each participant's measurement data were based on a totally independent spectral irradiance scale realisation. VNIIOFI (the Russian Federation) served as a pilot. The measurements were carried out from 2017 - 2020. The analysis of the comparison was performed following the step-by-step approach described in Appendix B of the Guidelines for CCPR Key Comparison Report Preparation (CCPR-G2 Rev.4). The key comparison reference values (KCRV) were calculated independently at each wavelength as the weighted means with cut-off. The measurement data of all participants were used for the KCRV excluding just a few outliers in the wavelength range of 1700 nm to 2500 nm. The relative uncertainties of the KCRV were minimal, 0.08 %, at the wavelengths of 900 nm to 1500 nm and increasing towards the limits of the spectral range up to 0.28 % at 250 nm and 0.20 % at 2500 nm. Consistency checks were satisfied at 17 of the 28 measured wavelengths, but failed at 11 wavelengths, mostly in the infrared range. For the latter wavelengths the Mandel-Paule method was applied resulting in an additional uncertainty that varied depending on wavelength from 0.11 % to 0.89 %. In general, the measurement uncertainties submitted by the participants in this comparison were less than those submitted in the previous CCPR-K1.a comparison. This is evidenced by the values of the cut-off, which had decreased at all wavelengths by a factor of 1.2 to 3.0 depending on wavelength. Abstract: Background Currently, the standard therapy for autoimmune hepatitis (AIH) consists of a combination of prednisolone and azathioprine. However, 15% of patients are intolerant to azathioprine which necessitates cessation of azathioprine or changes in therapy. In addition, not all patients achieve complete biochemical response (CR). Uncontrolled data indicate that mycophenolate mofetil (MMF) can induce CR in a majority of patients. Better understanding of first-line treatment and robust evidence from randomised clinical trials are needed. The aim of this study was to explore the potential benefits of MMF as compared to azathioprine, both combined with prednisolone, as induction therapy in a randomised controlled trial in patients with treatment-naive AIH. Methods CAMARO is a randomised (1:1), open-label, parallel-group, multicentre superiority trial. All patients with AIH are screened for eligibility. Seventy adult patients with AIH from fourteen centres in the Netherlands and Belgium will be randomised to receive MMF or azathioprine. Both treatment arms will start with prednisolone as induction therapy. The primary outcome is biochemical remission, defined as serum levels of alanine aminotransferase and immunoglobulin G below the upper limit of normal. Secondary outcomes include safety and tolerability of MMF and azathioprine, time to remission, changes in Model For End-Stage Liver Disease (MELD)-score, adverse events, and aspects of quality of life. The study period will last for 24 weeks. Discussion The CAMARO trial investigates whether treatment with MMF and prednisolone increases the proportion of patients in remission compared with azathioprine and prednisolone as the current standard treatment strategy. Abstract: Tuberculosis is a global public health problem with 10 million people developing an active form each year. The exact incidence of the pelvic form of tuberculosis is not known due to under-reporting of asymptomatic cases, vague symptomatology and lack of reliable diagnosis. We report a case of ovarian tuberculosis mimicking cancer and peritoneal carcinosis. The case involved a 15-year-old girl, hospitalized for severe pelvic pain and secondary amenorrhea. The examination showed an altered general state, a fever, a sensitive distended abdomen. The biology showed an elevation of Ca125 to 357 IU/ml. The CT scan showed a latero-uterine mass taking contrast in a heterogeneous way with an irregular and nodular thickening of the peritoneum making evoke a peritoneal carcinosis. Exploratory laparotomy found a bilateral ovarian mass with ileo-ileal, ileo-parietal and ileo-ovarian adhesions. Biopsy of the mass and the peritoneum showed a caseo-follicular gigantocellular epitheloid granulomatosis.

16. Developing Mathematical Thinking and Self-Regulated Learning: A Teaching Experiment in a Seventh-Grade Mathematics Classroom

Abstract: Mathematics educators have found sociocultural models of teaching and learning to be powerful in their ability to describe and support the pursuit of instruction based on recent standards documents (e.g., National Council of Teachers of Mathematics [NCTM], 1989, 2000). These models of instruction, however, have been criticized for their lack of explicitness. Detailed descriptions of cognitive processes within self-regulated learning (SRL) and attribution theories (e.g., Borkowski et al., 1988; Butler, 2002; Zimmerman, 2000) lend support for and provide examples of explicit instruction embedded within sociocultural models of mathematics instruction. Self-regulated learners are active participants in their own learning, are able to select from a repertoire of strategies and to monitor their progress in using these strategies toward a goal. In this article, we describe the collaborative efforts of a seventh-grade mathematics teacher and a university faculty member to develop students' mathematical thinking and self-regulation within one middle school classroom. We argue that implementing mathematics instruction consonant with NCTM (1989, 2000) standards documents makes the development of self-regulated learners possible and is at the same time dependent upon some degree of self-regulation within the community of practice. Several factors are crucial to this development: multiple representations and rich mathematical tasks; classroom discourse; environmental scaffolding of strategic behavior; and varying needs for explicitness and support. This work constitutes beginning efforts to describe contexts that support the development of mathematical thinking and SRL in the middle school mathematics classroom.

Abstract: For more than half a century, researchers around the world have been engaged in attempts to achieve fusion ignition as a proof of principle of various fusion concepts. Following the Lawson criterion, an ignited plasma is one where the fusion heating power is high enough to overcome all the physical processes that cool the fusion plasma, creating a positive thermodynamic feedback loop with rapidly increasing temperature. In inertially confined fusion, ignition is a state where the fusion plasma can begin "burn propagation" into surrounding cold fuel, enabling the possibility of high energy gain. While "scientific breakeven" (i.e., unity target gain) has not yet been achieved (here target gain is 0.72, 1.37 MJ of fusion for 1.92 MJ of laser energy), this Letter reports the first controlled fusion experiment, using laser indirect drive, on the National Ignition Facility to produce capsule gain (here 5.8) and reach ignition by nine different formulations of the Lawson criterion.

Abstract: Item does not contain fulltext, BACKGROUND & AIMS: Autoimmune hepatitis (AIH) has been well characterised and codified through the development of diagnostic criteria. These criteria have been adapted and simplified and are widely used in clinical practice. However, there is a need to update and precisely define the criteria for both treatment response and treatment. METHODS: A systematic review was performed and a modified Delphi consensus process was used to identify and redefine the response criteria in autoimmune hepatitis. RESULTS: The consensus process initiated by the International Autoimmune Hepatitis Group proposes that the term 'complete biochemical response' defined as 'normalization of serum transaminases and IgG below the upper limit of normal' be adopted to include a time point at 6 months after initiation of treatment. An insufficient response by 6 months was a failure to meet the above definition. Non-response was defined as '<50% decrease of serum transaminases within 4 weeks after initiation of treatment'. Remission is defined as liver histology with a Hepatitis Activity Index <4/18. Intolerance to treatment was agreed to stand for 'any adverse event possibly related to treatment leading to potential drug discontinuation'. CONCLUSIONS: These definitions provide a simple and reproducible framework to define treatment response and non-response, irrespective of the therapeutic intervention. A consensus on endpoints is urgently required to set a global standard for the reporting of study results and to enable inter-study comparisons. Abstract: Contains fulltext : 252186.pdf (Publisher’s version ) (Open Access), BACKGROUND AND AIMS: Detection of autoantibodies is a mainstay of diagnosing autoimmune hepatitis (AIH). However, conventional autoantibodies for the workup of AIH lack either sensitivity or specificity, leading to substantial diagnostic uncertainty. We aimed to identify more accurate serological markers of AIH with a protein macroarray. APPROACH AND RESULTS: During the search for more-precise autoantibodies to distinguish AIH from non-AIH liver diseases (non-AIH-LD), IgG antibodies with binding capacities to many human and foreign proteins were identified with a protein macroarray and confirmed with solid-phase ELISAs in AIH patients. Subsequently, polyreactive IgG (pIgG) was exemplarily quantified by reactivity against human huntingtin-interacting protein 1-related protein in bovine serum albumin blocked ELISA (HIP1R/BSA). The diagnostic fidelity of HIP1R/BSA binding pIgG to diagnose AIH was assessed in a retrospective training, a retrospective multicenter validation, and a prospective validation cohort in cryoconserved samples from 1,568 adults from 10 centers from eight countries. Reactivity against HIP1R/BSA had a 25% and 14% higher specificity to diagnose AIH than conventional antinuclear and antismooth muscle antibodies, a significantly higher sensitivity than liver kidney microsomal antibodies and antisoluble liver antigen/liver pancreas antigen, and a 12%-20% higher accuracy than conventional autoantibodies. Importantly, HIP1R/BSA reactivity was present in up to 88% of patients with seronegative AIH and in up to 71% of AIH patients with normal IgG levels. Under therapy, pIgG returns to background levels of non-AIH-LD. CONCLUSIONS: pIgG could be used as a promising marker to improve the diagnostic workup of liver diseases with a higher specificity for AIH compared to conventional autoantibodies and a utility in autoantibody-negative AIH. Likewise, pIgG could be a major source of assay interference in untreated AIH.

Abstract: In this presentation we provide an overview of the commissioning and first user experiment of ReLaX in combination with the XFEL beam. We will introduce the setup and laser parameters. We will show the established standard setup combining small-angle x-ray scattering (SAXS), phase-contrast imaging (PCI) and spectroscopy techniques. Finally we will show examples of the data obtained in the interaction of the ReLaX laser with targets of interested for the short-pulse laser community.

Abstract: The High Energy Density (HED) instrument at the European XFEL provides a platform to study hot and warm dense matter. The Helmholtz International Beamline for Extreme Fields (HiBEF) is a User Consortium supplying HED with two laser systems (the high-intensity ReLaX laser, by Amplitude Technologies, and the high-energy Dipole-100X laser, by STFC), Diamond Anvil Cells setup and high-pulsed magnetic fields. These tools in combination with the XFEL beam enable the investigation of relativistic laser plasmas, strong-field QED phenomena, high-pressure astro- and planetary physics as well as magnetic phenomena in condensed matter. The successful commissioning of the ultra-short pulse high-intensity ReLaX laser, provides new unique opportunities in the plasma and high-field physics fields [1]. ReLaX is a double CPA Ti:Sa laser capable of delivering up to 300 TW pulses on target. In the first commissioning phase, 100 TW pulses were used, reaching intensities up to 1020 W/cm2. Small-Angle X-Ray Scattering (SAXS) without the need of a beamstop was first commissioned at HED in September 2019. Two highly annealed pyrolytic graphite (HAPG) crystals were used to reflect the SAXS photons onto a detector while allowing the main XFEL beam to go through [2]. In April and May 2021, Small Angle X-Ray Scattering and Phase Contrast Imaging (PCI) were simultaneously demonstrated in pump-probe experiments at HED in a community experiment involving 15 institutions from all over the world. In this talk we will present the preliminary results of this community experiment probing ultrafast phenomena in a wide array of target configurations: hole boring in wires, shockwave generation in CH blocks, buried heating of a wire inside a CH medium, foam ionization and collective effects in heated foils. Abstract: Background and aims: Autoimmune hepatitis (AIH) has been well characterised and codified through the development of diagnostic criteria. These criteria have been adapted and simplified and are widely used in clinical practice. However, there is a need to update and precisely define the criteria for both treatment response and treatment. Methods: a systematic review was performed and a modified Delphi consensus process was used to identify and redefine the response criteria in autoimmune hepatitis. Results: the consensus process initiated by the International Autoimmune Hepatitis Group proposes that the term 'complete biochemical response' defined as 'normalization of serum transaminases and IgG below the upper limit of normal' be adopted to include a time point at 6 months after initiation of treatment. An insufficient response by 6 months was a failure to meet the above definition. Non-response was defined as '<50% decrease of serum transaminases within 4 weeks after initiation of treatment'. Remission is defined as liver histology with a Hepatitis Activity Index <4/18. Intolerance to treatment was agreed to stand for 'any adverse event possibly related to treatment leading to potential drug discontinuation'. Conclusions: these definitions provide a simple and reproducible framework to define treatment response and non-response, irrespective of the therapeutic intervention. A consensus on endpoints is urgently required to set a global standard for the reporting of study results and to enable inter-study comparisons. Abstract: Ignition is needed to make fusion energy a viable alternative energy source, but has yet to be achieved (1). A key step on the way to ignition is to have the energy generated through fusion reactions in an inertially confined fusion plasma exceed the amount of energy deposited into the deuterium--tritium fusion fuel and hotspot during the implosion process, resulting in a fuel gain greater than unity. Here we report the achievement of fusion fuel gains exceeding unity on the US National Ignition Facility using a 'high-foot' implosion method (2,3), which is a manipulation of the laser pulse shape in a way that reduces instability in the implosion. These experiments show an order-of-magnitude improvement in yield performance over past deuterium--tritium implosion experiments. Abstract: Purpose: To study and compare the efficacy of different therapeutic options for the treatment of central serous chorioretinopathy (CSCR). Methods: This is a nonrandomized, international multicentre study on 1719 patients (1861 eyes) diagnosed with CSCR, from 63 centres (24 countries). Reported data included different methods of treatment and both results of diagnostic examinations [fluorescein angiography and/or optical coherent tomography (OCT)] and best-corrected visual acuity (BCVA) before and after therapy. The duration of observation had a mean of 11 months but was extended in a minority of cases up to 7 years. The aim of this study is to evaluate the efficacy of the different therapeutic options of CSCR in terms of both visual (BCVA) and anatomic (OCT) improvement. Results: One thousand seven hundred nineteen patients (1861 eyes) diagnosed with CSCR were included. Treatments performed were nonsteroidal anti-inflammatory eye drops, laser photocoagulation, micropulse diode laser photocoagulation, photodynamic therapy (PDT; Standard PDT, Reduced-dose PDT, Reduced-fluence PDT), intravitreal (IVT) antivascular endothelial growth factor injection (VEGF), observation and other treatments. The list of the OTHERS included both combinations of the main proposed treatments or a variety of other treatments such as eplerenone, spironolactone, acetazolamide, beta-blockers, anti-anxiety drugs, aspirin, folic acid, methotrexate, statins, vitis vinifera extract medication and pars plana vitrectomy. The majority of the patients were men with a prevalence of 77%. The odds ratio (OR) showed a partial or complete resolution of fluid on OCT with any treatment as compared with observation. In univariate analysis, the anatomical result (improvement in subretinal fluid using OCT at 1 month) was favoured by age 500 μm (p = 0.03). The OR for obtaining partial or complete resolution showed that anti-VEGF and eyedrops were not statistically significant; whereas PDT (8.5), thermal laser (11.3) and micropulse laser (8.9) lead to better anatomical results with less variability. However, up to 20% of patients experience either insufficient response or intolerance on first-line therapy. Calcineurin inhibitors (CNIs) are frequently used when first-line therapy fails. Although a number of studies report on efficacy, less is known on the patient trajectory before switch to CNIs. Our aim was to describe the road toward CNI therapy in AIH patients. METHODS: Patients with an AIH diagnosis who used CNIs as either second- or third-line treatment were included in the study. Reason for switch to CNI was assessed as either an insufficient response or intolerance to prior therapy. Efficacy was assessed by normalization of transaminases at last moment of follow-up. RESULTS: Final analysis included 20 patients who were treated with CNIs. Ten patients were treated with tacrolimus and ten patients received cyclosporine. In patients who used CNI treatment as third-line therapy (n = 13), duration of first-line therapy was almost twice as long as duration of second-line therapy (2.58 years vs. 1.33 years; P = 0.67). Patients treated with tacrolimus had relatively high trough levels (7.6 ng/mL) and more (minor) adverse events. Fifty-five percent of patients had normalization of transaminases at last moment of follow-up. CONCLUSION: CNI treatment in AIH as second- or third-line therapy is effective in ~50% of patients. The trajectory before switch varies considerably between patients.

Abstract: Contains fulltext : 219998.pdf (Publisher’s version ) (Closed access), BACKGROUND & AIMS: Changes in serum levels of transaminases immediately after initiation of treatment for autoimmune hepatitis (AIH) might be associated with biochemical markers of remission and liver-related events. We assessed the outcomes of patients with vs without rapid response to treatment of AIH in a large international cohort. METHODS: We performed a retrospective cohort study, collecting data from 2 independent cohorts of adults with AIH from 12 centers in 7 countries in Europe. We collected information on patient demographics; serologic, histologic, and biochemical analyses; and treatment. We used a receiver operating characteristic curve and Youden index to calculate the optimal percentage decrease in level of aspartate aminotransferase (AST) after 8 weeks of treatment that associated with normalization of transaminase levels after 26 weeks of treatment with predniso(lo)ne (primary outcome) in the first (discovery) cohort (n = 370). We evaluated the results in the second (validation) cohort (n = 370). Secondary outcomes were liver-related death or transplantation. We performed univariate and multivariable logistic and Cox regression with correction for confounders. RESULTS: A significant decrease in level of AST after 8 weeks of treatment was significantly associated with normalization of transaminase levels at 26 and 52 weeks (P < .001); a decrease of more than 80% in level of AST was associated with optimal normalization. In both cohorts, rapid responders (≥80% decrease in level of AST after 8 weeks) were more likely to achieve normalization of transaminases at 26 and 52 weeks when compared to non-rapid responders. Rapid responders in the discovery cohort had lower risk of liver-related death or transplantation (adjusted hazard ratio 0.18; 95% CI 0.05-0.63; P = .007), although this was not confirmed in the validation cohort. Abstract: Contains fulltext : 225262.pdf (Publisher’s version ) (Open Access), BACKGROUND: Guidelines regarding treatment for autoimmune hepatitis (AIH) favour two strategies for azathioprine (AZA) introduction: concurrent with steroids at induction or delayed by 2-4 weeks. The safety and efficacy of both strategies have been unexplored. METHODS: We established a cohort of 900 AIH patients from 12 centres in 7 European countries. There were 631 patients who used AZA as part of the therapeutic regimen. We distinguished two groups: patients with early AZA (<2 weeks) or delayed AZA initiation (≥2 weeks). Primary outcome was discontinuation of AZA in the first year of treatment. Cox regression and propensity score matching was performed to determine difference in outcomes between groups. RESULTS: Patients with early AZA initiation had significantly lower transaminases and bilirubin at baseline. Discontinuation rates of AZA did not differ between early and delayed starters (16.6% vs 14.2%), which did not reach statistical significance (hazard ratio 0.97, 95% confidence interval 0.61-1.55, P = .90). Stratification according to baseline disease activity or propensity score matching did not alter the results. Main reason for AZA discontinuation was intolerance to treatment (14.0% vs 13.2%, P = .78) with nausea and vomiting as main side effects. AIH remission rates were comparable among groups. CONCLUSION: The discontinuation rate of AZA in AIH treatment is ~15% in the first year of treatment. Early or delayed AZA initiation does not differ in remission and discontinuation rates in AIH induction therapy. Our data suggest that either strategy may be used as part of AIH treatment.

A subgroup of patients with AIH has IgG within the normal range despite evidence of clinical disease activity. The clinical significance of this biomarker has not been explored. METHODS: In a European multicentre study we compared biochemical, clinical and histological features from patients with AIH and normal IgG-values at diagnosis to an age- and sex-matched control group of patients with typical AIH presenting with elevated IgG. Data were assessed at diagnosis, after 12 months of therapy and at last follow-up. RESULTS: Out of 1,318 patients with AIH, 130 (10%) had normal IgG at presentation. Histological and biochemical parameters at diagnosis, as well as treatment response, showed no difference between groups. Stable remission off treatment was achieved more commonly in the normal IgG group than in the typical AIH group (24 vs. 8%; p = 0.0012). Patients of the control group not only had higher IgG levels (29.5 ± 5.8 vs. 12.5 ± 3.2 g/L; p <0.0001), but also a higher IgG/IgA ratio (9.3 ± 6.9 vs. 5.4 ± 2.4; p <0.0001) at diagnosis. The IgG/IgA ratio only declined in patients with typical AIH and was no longer different between groups after 12 months (6.3 ± 4.3 vs. 5.5 ± 2.2; p = 0.1), indicating a selective increase of IgG in typical AIH and its suppression by immunosuppression. Autoantibody titres were higher in the typical AIH group, but not when controlled for IgG levels. CONCLUSIONS: Compared to AIH with typical biochemical features, patients with normal IgG levels at diagnosis (i) show similar biochemical, serological and histological features and comparable treatment response, (ii) appear to lack the selective elevation of serum IgG levels observed in typical active AIH disease, (iii) may represent a subgroup with a higher chance of successful drug withdrawal. Abstract: Ignition implosions on the National Ignition Facility (NIF) [Lindl et al., Phys. Plasmas 11, 339 (2004)] are driven with a very carefully tailored sequence of four shock waves that must be timed to very high precision in order to keep the fuel on a low adiabat. The first series of precision tuning experiments on NIF have been performed. These experiments use optical diagnostics to directly measure the strength and timing of all four shocks inside the hohlraum-driven, cryogenic deuterium-filled capsule interior. The results of these experiments are presented demonstrating a significant decrease in the fuel adiabat over previously un-tuned implosions. The impact of the improved adiabat on fuel compression is confirmed in related deuterium-tritium (DT) layered capsule implosions by measurement of fuel areal density (ρR), which show the highest fuel compression (ρR ∼ 1.0 g/cm2) measured to date.

Abstract: An experimental campaign to tune the initial shock compression sequence of capsule implosions on the National Ignition Facility (NIF) was initiated in late 2010. The experiments use a NIF ignition-scale hohlraum and capsule that employs a re-entrant cone to provide optical access to the shocks as they propagate in the liquid deuterium-filled capsule interior. The strength and timing of the shock sequence is diagnosed with velocity interferometry that provides target performance data used to set the pulse shape for ignition capsule implosions that follow. From the start, these measurements yielded significant new information on target performance, leading to improvements in the target design. We describe the results and interpretation of the initial tuning experiments.

Abstract: Directly laser driven and X-radiation driven DT filled capsules differ in the relationship between neutron and X-ray images. Shot N110217, a directly driven DT-filled glass micro-balloon provided the first neutron images at the National Ignition Facility. As seen in implosions on the Omega laser, the neutron image can be enclosed inside time integrated X-ray images. HYDRA simulations show the X-ray image is dominated by emission from the hot glass shell while the neutron image arises from the DT fuel it encloses. In the absence of mix or jetting, X-ray images of a cryogenically layered THD fuel capsule should be dominated by emission from the hydrogen rather than the cooler plastic shell that is separated from the hot core by cold DT fuel. This cool, dense DT, invisible in X-ray emission, shows itself by scattering hot core neutrons. Germanium X-ray emission spectra and Ross pair filtered X-ray energy resolved images suggest that germanium doped plastic emits in the torus shaped hot spot, probably reducing the neutron yield.

Abstract: In France, the National Strategy for Ecological Transition towards Sustainable Development (SNTEDD) aims to give everyone the power to act for a new model of society more sober and combining economic progress, ecological and human. The sharing and accessibility of knowledge about environmental health is therefore a crucial step in enabling citizens to better understand and act sustainably on a daily basis; to reduce the risks of exposure to persistent pollutants such as (eco) toxic metals. However, innovative digital teaching tools such as MOOCs make it possible to massively inform the general public because they are accessible: frequent free of charge, attractive content and ergonomic use. In addition, they promote the consideration of the otherness of learners. That is why this communication concerns the creation of the MOOC "TEAM" entitled "Acting together to reduce the presence of toxic metals in our plate-Environmental transfers of metallic contaminants" intended for everyone, from high school, and totally accessible according to the European criteria of the Directive (EU) 2016/2102. Indeed, the French Sign Language (LSF), subtitles and a voiceover are included in the videos, for efficient training respectively of deaf or hard of hearing learners through a very visual learning, or visually impaired people. In addition, the LSF based on the visual and taking into account the 3 dimensions of the space facilitates the understanding of complex and / or abstract processes. In addition, a work was done to take into account the visually impaired and blind people with a work on the contrasts, the sharing of scripts, as well as a restitution of the information essential to the understanding of the oral course. Active pedagogy is implemented because it promotes the engagement, reflection and critical thinking of learners. Scientific concepts are thus approached from concrete examples of cases of pollution described in the media; learners are led to seek information and develop actions related to their daily practices of gardening, DIY, consumption ... The precise objectives of MOOC-TEAM developed by an interdisciplinary teaching team and multi actors (teachers, researchers, educational engineers, Koena company) are: 1-Creation of widely accessible educational resources on persistent metallic contaminants, while promoting interdisciplinary research work: continuum training-research-development and popular science. 2-Openness to society and voluntary inclusion of the deaf and visually impaired in the French education system for a more creative and supportive society, enhancing the value of otherness. 3-Pedagogical experimentation by example and the use of the LSF and visual aids to improve the learning of all on the complex subject of topical environmental health and ecological transition., En France, la Stratégie Nationale de Transition Ecologique vers un Développement Durable (SNTEDD) ambitionne de donner à chacun le pouvoir d'agir pour un nouveau modèle de société plus sobre et qui associe progrès économique, écologique et humain. Le partage et l'accessibilité des savoirs sur la santé environnementale est donc une étape cruciale pour permettre aux citoyens de mieux comprendre et agir durablement au quotidien ; pour réduire les risques d'exposition aux polluants persistants tels les métaux (éco)toxiques. Or, les outils pédagogiques numériques innovants tels que les MOOC permettent d'informer massivement le grand public car ils sont accessibles : gratuité fréquente, contenus attractifs et utilisation ergonomique. De plus, ils favorisent la prise en compte de l'altérité des apprenants. C'est pourquoi, cette communication concerne la création du MOOC « TEAM » intitulé « Agir ensemble pour réduire la présence de métaux toxiques dans notre assiette-Transferts environnementaux des contaminants métalliques » destiné à tous, à partir du lycée, et totalement accessible selon les critères européens de la directive (UE) 2016/2102. En effet, la langue des signes française (LSF), des sous-titres et une voix off sont inclus dans les vidéos, pour une formation efficiente respectivement des apprenants sourds ou malentendants par un apprentissage très visuel, ou encore des personnes malvoyantes. De plus, la LSF basée sur le visuel et la prise en compte des 3 dimensions de l'espace facilite la compréhension des processus complexes et/ou abstraits. Par ailleurs, un travail a été fait pour la prise en compte des personnes malvoyantes et aveugles avec un travail sur les contrastes, le partage des scripts, ainsi qu'une restitution des informations essentielles à la compréhension du cours à l'oral. La pédagogie active est mise en oeuvre car elle favorise l'engagement, la réflexion et l'esprit critique des apprenants. Les concepts scientifiques sont ainsi abordés à partir d'exemples concrets de cas de pollutions décrits dans les médias ; les apprenants sont amenés à rechercher des informations et à développer des actions en lien avec leurs pratiques quotidiennes de jardinage, bricolage, consommation…Les objectifs précis du MOOC-TEAM développé par une équipe pédagogique interdisciplinaire et multi acteurs (enseignants, chercheurs, ingénieurs pédagogiques, entreprise Koena) sont les suivants : 1-Création de ressources pédagogiques largement accessibles sur les contaminants métalliques persistants, tout en valorisant des travaux de recherche interdisciplinaires : continuum formation-recherche-développement et vulgarisation scientifique. 2-Ouverture à la société et inclusion volontaire des sourds et malvoyants dans le système éducatif français pour une société plus créative et solidaire, valorisant d'avantage l'altérité. 3-Expérimentation pédagogique par l'exemple et l'utilisation de la LSF et de supports visuels pour améliorer l'apprentissage de tous sur le sujet complexe d'actualité de la santé environnementale et de la transition écologique.

44. Prevalence of molecular markers of Plasmodium falciparum drug resistance in Dakar, Senegal

Abstract: Abstract Background As a result of the widespread resistance to chloroquine and sulphadoxine-pyrimethamine, artemisinin-based combination therapy (ACT) (including artemether-lumefantrine and artesunate-amodiaquine) has been recommended as a first-line anti-malarial regimen in Senegal since 2006. Intermittent preventive treatments with anti-malarial drugs based on sulphadoxine-pyrimethamine are also given to children or pregnant women once per month during the transmission season. Since 2006, there have been very few reports on the susceptibility of Plasmodium falciparum to anti-malarial drugs. To estimate the prevalence of resistance to several anti-malarial drugs since the introduction of the widespread use of ACT, the presence of molecular markers associated with resistance to chloroquine and sulphadoxine-pyrimethamine was assessed in local isolates at the military hospital of Dakar. Methods The prevalence of genetic polymorphisms in genes associated with anti-malarial drug resistance, i.e., Pfcrt, Pfdhfr, Pfdhps and Pfmdr1, and the copy number of Pfmdr1 were evaluated for a panel of 174 isolates collected from patients recruited at the military hospital of Dakar from 14 October 2009 to 19 January 2010. Results The Pfcrt 76T mutation was identified in 37.2% of the samples. The Pfmdr1 86Y and 184F mutations were found in 16.6% and 67.6% of the tested samples, respectively. Twenty-eight of the 29 isolates with the 86Y mutation were also mutated at codon 184. Only one isolate (0.6%) had two copies of Pfmdr1. The Pfdhfr 108N/T, 51I and 59R mutations were identified in 82.4%, 83.5% and 74.1% of the samples, respectively. The double mutant (108N and 51I) was detected in 83.5% of the isolates, and the triple mutant (108N, 51I and 59R) was detected in 75.3%. The Pfdhps 437G, 436F/A and 613S mutations were found in 40.2%, 35.1% and 1.8% of the samples, respectively. There was no double mutant (437G and 540E) or no quintuple mutant (Pfdhfr 108N, 51I and 59R and Pfdhps 437G and 540E). The prevalence of the quadruple mutant (Pfdhfr 108N, 51I and 59R and Pfdhps 437G) was 36.5%. Conclusions Since 2004, the prevalence of chloroquine resistance had decreased. The prevalence of isolates with high-level pyrimethamine resistance is 83.5%. The prevalence of isolates resistant to sulphadoxine is 40.2%. However, no quintuple mutant (Pfdhfr 108N, 51I and 59R and Pfdhps 437G and 540E), which is associated with a high level of sulphadoxine-pyrimethamine resistance, has been identified to date. The resistance to amodiaquine remains moderate.

Abstract: Zusammenfassung: Akteure nehmen in Interaktionen immer wieder soziale Kategorisierungen von anderen Personen vor und aktivieren die entsprechenden Stereotypen über diese Kategorien. Sowohl in der Sozialpsychologie als auch in der Soziologie fokussiert allerdings die Forschung auf die Untersuchung askriptiver Sozialkategorien, während Konzepte wie Klasse und Schicht weitgehend ausgeblendet bleiben. In diesem Aufsatz wird argumentiert, dass diese Kategorien in der Bevölkerung weithin bekannt und mental zugänglich sind sowie in entsprechenden Situationen auch aktiviert werden. Damit sind sie auch für die Orientierung in alltäglichen Interaktionssituationen relevant. Auf der Grundlage einer Onlineumfrage wird demonstriert, dass die Stereotypkontraste zwischen verschiedenen sozialen Schichten vor allem durch sozioökonomische Merkmale sowie durch hochkulturelle Lebensstilmerkmale bestimmt werden, während Persönlichkeitsmerkmale eine geringere Bedeutung aufweisen

46. Ex vivo susceptibility of Plasmodium falciparum isolates from Dakar, Senegal, to seven standard anti-malarial drugs

Abstract: Abstract Background As a result of widespread chloroquine and sulphadoxine-pyrimethamine resistance, artemisinin-based combination therapy (ACT) (which includes artemether-lumefantrine and artesunate-amodiaquine) has been recommended as a first-line anti-malarial regimen in Senegal since 2006. Since then, there have been very few reports on the ex vivo susceptibility of Plasmodium falciparum to anti-malarial drugs. To examine whether parasite susceptibility has been affected by the widespread use of ACT, the ex vivo susceptibility of local isolates was assessed at the military hospital of Dakar. Methods The ex vivo susceptibility of 93 P. falciparum isolates from Dakar was successfully determined using the Plasmodium lactate dehydrogenase (pLDH) ELISA for the following drugs: chloroquine (CQ), quinine (QN), mefloquine (MQ), monodesethylamodiaquine (MDAQ), lumefantrine (LMF), dihydroartemisinin (DHA) and doxycycline (DOX). Results After transformation of the isolate IC50 in ratio of IC50 according to the susceptibility of the 3D7 reference strain (isolate IC50/3D7 IC50), the prevalence of the in vitro resistant isolates with reduced susceptibility was 50% for MQ, 22% for CQ, 12% for DOX, 6% for both QN and MDAQ and 1% for the drugs LMF and DHA. The highest significant positive correlations were shown between responses to CQ and MDAQ (r = 0.569; P < 0.0001), LMF and QN (r = 0.511; P < 0.0001), LMF and DHA (r = 0.428; P = 0.0001), LMF and MQ (r = 0.413; P = 0.0002), QN and DHA (r = 0.402; P = 0.0003) and QN and MQ (r = 0.421; P = 0.0001). Conclusions The introduction of ACT in 2002 has not induced a decrease in P. falciparum susceptibility to the drugs DHA, MDAQ and LMF, which are common ACT components. However, the prevalence of P. falciparum isolates with reduced susceptibility has increased for both MQ and DOX. Taken together, these data suggest that intensive surveillance of the P. falciparum in vitro susceptibility to anti-malarial drugs in Senegal is required.

Abstract: Contains fulltext : 203198.pdf (publisher's version ) (Closed access), OBJECTIVES: Serrated polyposis syndrome (SPS) is a relatively new and under-recognized colorectal cancer (CRC) predisposition syndrome. Previous studies have reported miss-rates of SPS diagnosis varying from 40 to 82%. As SPS patients and their first-degree relatives have an increased risk of CRC, early recognition is important. We aimed to determine the miss-rate of SPS and to determine the reasons for missed diagnosis. PATIENTS AND METHODS: We retrospectively identified all patients diagnosed with at least one colorectal polyp or carcinoma detected at our tertiary referral center between January 1986 and July 2013 using the nationwide pathology registry. On the basis of cumulative polyp count with size and location, SPS patients were identified. We checked whether the SPS diagnosis was made in the medical files and, if not, what might have been the reason for missing the diagnosis. RESULTS: We randomly assessed 5000 patients, of whom 25 patients fulfilled the WHO criteria for SPS. In six patients, no previous SPS diagnosis had been made, leading to a miss-rate of 24.0% (95% confidence interval: 7.3-40.7). The reasons for missed diagnosis were polyps removed before establishment of the WHO criteria, unavailable pathology reports, and failure to apply the criteria by the clinician. CONCLUSION: The miss-rate for the diagnosis of SPS is considerable, even during longer follow-up with repeated colonoscopies. A preventable reason for missing SPS cases is failure to apply the WHO criteria. Awareness of this CRC predisposition syndrome needs to be raised to decrease the miss-rate of SPS.

49. Clinical management of autoimmune hepatitis

Abstract: Contains fulltext : 215399.pdf (publisher's version ) (Open Access), Autoimmune hepatitis is a rare and chronic liver disease that is characterised by increased serum transaminases and immunoglobulin G, inflammatory liver histology and presence of circulating autoantibodies. An autoimmune hepatitis diagnosis justifies life-long treatment in most patients in order to prevent development of cirrhosis and end-stage liver disease. The cornerstone of treatment is steroid induction therapy followed by maintenance therapy with azathioprine, which is effective in most cases. For patients who do not respond to standard treatment, second-line treatment with other immunosuppressants can be effective. Treatment should be aimed at biochemical remission of the disease, which is defined as normalization of transaminases and immunoglobulin G. Patients should be monitored intensively during the first months of treatment in order to monitor side-effects, assess symptoms and individualise treatment. Specialist consultation should be sought in difficult-to-treat patients. Future studies and networking initiatives should result in optimization of current treatment strategies in autoimmune hepatitis.
