Abstract

BACKGROUND: Reducing hospital readmission offer potential benefits for patients, providers, payers, and policymakers to improve quality of healthcare, reduce cost, and improve patient experience. We investigated effectiveness of remdesivir in reducing 30-day coronavirus disease 2019 (COVID-19)-related readmission during the Omicron era, including older adults and those with underlying immunocompromising conditions. METHODS: This retrospective study utilized the US PINC AI Healthcare Database to identify adult patients discharged alive from an index COVID-19 hospitalization between December 2021 and February 2024. Odds of 30-day COVID-19-related readmission to the same hospital were compared between patients who received remdesivir vs those who did not, after balancing characteristics of the two groups using inverse probability of treatment weighting (IPTW). Analyses were stratified by maximum supplemental oxygen requirement during index hospitalization. RESULTS: Of 326 033 patients hospitalized for COVID-19 during study period, 210 586 patients met the eligibility criteria. Of these, 109 551 (52%) patients were treated with remdesivir. After IPTW, lower odds of 30-day COVID-19-related readmission were observed in patients who received remdesivir vs those who did not, in the overall population (3.3% vs 4.2%, respectively; odds ratio [95% confidence interval {CI}]: 0.78 [.75-.80]), elderly population (3.7% vs 4.7%, respectively; 0.78 [.75-.81]), and those with underlying immunocompromising conditions (5.3% vs 6.2%, respectively; 0.86 [.80-.92]). These results were consistent irrespective of supplemental oxygen requirements. CONCLUSIONS: Treating patients hospitalized for COVID-19 with remdesivir was associated with a significantly lower likelihood of 30-day COVID-19-related readmission across all patients discharged alive from the initial COVID-19 hospitalization, including older adults and those with underlying immunocompromising conditions.

Abstract

Advanced experiments using THz pump and X-ray probe pulses at modern free-electron lasers (FELs) like the European X-ray FEL require a frequency-tunable, high-power, narrow-band THz source maintaining the repetition rate and pulse structure of the X-ray pulses. This paper reports the first results from a THz source, that is based on a single-pass high-gain THz FEL operating with a central wavelength of 100 micrometers. The THz FEL prototype is currently in operation at the Photo Injector Test facility at DESY in Zeuthen (PITZ) and uses the same type of electron source as the European XFEL photo injector. A self-amplified spontaneous emission (SASE) FEL was envisioned as the main mechanism for generating the THz pulses. Although the THz FEL at PITZ is supposed to use the same mechanism as at X-ray facilities, it cannot be considered as a simple scaling of the radiation wavelength because there is a large difference in the number of electrons per radiation wavelength, which is five orders of magnitude higher for the THz case. The bunching factor arising from the electron beam current profile contributes strongly to the initial spontaneous emission starting the FEL process. Proof-of-principle experiments were done at PITZ using an LCLS-I undulator to generate the first high-power, high-repetition-rate single-pass THz FEL radiation. Electron bunches with a beam energy of ~17 MeV and a bunch charge of up to several nC are used to generate THz pulses with a pulse energy of several tens of microjoules. For example, for an electron beam with a charge of ~2.4 nC, more than 100 microjoules were generated at a central wavelength of 100 micrometers. The narrowband spectrum was also demonstrated by spectral measurements. 3. Treatment of patients hospitalized for COVID-19 with remdesivir is associated with lower likelihood of 30-day readmission: a retrospective observational study.

Abstract

Aim: This observational study investigated the association between remdesivir treatment during hospitalization for COVID-19 and 30-day COVID-19-related and all-cause readmission across different variants time periods. Patients & methods: Hospitalization records for adult patients discharged from a COVID-19 hospitalization between 1 May 2020 to 30 April 2022 were extracted from the US PINC AI Healthcare Database. Likelihood of 30-day readmission was compared among remdesivir-treated and nonremdesivir-treated patients using multivariable logistic regression models adjusted for age, corticosteroid treatment, Charlson comorbidity index and intensive care unit stay during the COVID-19 hospitalization. Analyses were stratified by maximum supplemental oxygen requirement and variant time period (pre-Delta, Delta and Omicron). Results: Of the 440,601 patients discharged alive after a COVID-19 hospitalization, 248,785 (56.5%) patients received remdesivir. Overall, remdesivir patients had a 30-day COVID-19-related readmission rate of 3.0% and all-cause readmission rate of 6.3% compared with 5.4% and 9.1%, respectively, for patients who did not receive remdesivir during their COVID-19 hospitalization. After adjusting for demographics and clinical characteristics, remdesivir treatment was associated with significantly lower odds of 30-day COVID-19-related readmission (odds ratio 0.60 [95% confidence interval: 0.58-0.62]), and all-cause readmission (0.73 [0.72-0.75]). Significantly lower odds of 30-day readmission in remdesivir-treated patients was observed across all variant time periods. Conclusion: Treating patients hospitalized for COVID-19 with remdesivir is associated with a statistically significant reduction in 30-day COVID-19-related and all-cause readmission across variant time periods. These findings indicate that the clinical benefit of remdesivir may extend beyond the COVID-19 hospitalization.

Abstract

The development of algorithms for remote sensing of water quality (RSWQ) requires a large amount of in situ data to account for the bio-geo-optical diversity of inland and coastal waters. The GLObal Reflectance community dataset for Imaging and optical sensing of Aquatic environments (GLORIA) includes 7,572 curated hyperspectral remote sensing reflectance measurements at 1 nm intervals within the 350 to 900 nm wavelength range. In addition, at least one co-located water quality measurement of chlorophyll a, total suspended solids, absorption by dissolved substances, and Secchi depth, is provided. The data were contributed by researchers affiliated with 59 institutions worldwide and come from 450 different water bodies, making GLORIA the de-facto state of knowledge of in situ coastal and inland aquatic optical diversity. Each measurement is documented with comprehensive methodological details, allowing users to evaluate fitness-for-purpose, and providing a reference for practitioners planning similar measurements. We provide open and free access to this dataset with the goal of enabling scientific and technological advancement towards operational regional and global RSWQ monitoring.

Abstract

Photoinjectors are the main high brightness electron sources for X-ray free electron lasers (XFEL). Photoinjector emittance reduction is one of the key knobs for improving XFEL lasing, so precise emittance measurement is critical. It's well known that rms emittance is very sensitive to low intensity tails of particle distributions in the phase space, whose measurement depend on the signal to noise ratio (SNR) and image processing procedures. Such sensitivities make the interpretations of beam transverse brightness challenging, leading to different emittance definitions to reduce the impact of tail particles. In this paper, transverse phase space is analyzed in action and phase coordinates for both analytical models and experiments, which give a more intuitive way to calculate the beam core brightness.

Abstract

Slice energy spread is one of the key parameters in free electron laser optimizations, but its accurate measurement is not straightforward. Two recent studies from high energy ($>$100 MeV) photoinjectors at SwissFEL and European XFEL have reported much higher slice energy spread than expected at their XFEL working points (200 - 250 pC). In this paper, a new method for measuring slice energy spread at a lower beam energy ($\sim$20 MeV) is proposed and demonstrated at the PhotoInjector Test facility at DESY Zeuthen (PITZ), and the results for 250 pC and 500 pC are much lower than those measured at high energy injectors.

Abstract

An essential part for the accurate classification of electrocardiogram (ECG) signals is the extraction of informative yet general features, which are able to discriminate diseases. Cardiovascular abnormalities manifest themselves in features on different time scales: small scale morphological features, such as missing P-waves, as well as rhythmical features apparent on heart rate scales. For this reason we incorporate a variant of the complex wavelet transform, called a scatter transform, in a deep residual neural network (ResNet). The former has the advantage of being derived from theory, making it well behaved under certain transformations of the input. The latter has proven useful in ECG classification, allowing feature extraction and classification to be learned in an end-to-end manner. Through the incorporation of trainable layers in between scatter transforms, the model gains the ability to combine information from different channels, yielding more informative features for the classification task and adapting them to the specific domain. For evaluation, we submitted our model in the official phase in the PhysioNet/Computing in Cardiology Challenge 2020. Our (Team Triage) approach achieved a challenge validation score of 0.640, and full test score of 0.485, placing us 4th out of 41 in the official ranking.

Abstract

Over the last decades, photoreceptive proteins were extensively studied with biophysical methods to gain a fundamental understanding of their working mechanisms and further guide the development of optogenetic tools from them. Time-resolved infrared (IR) spectroscopy is one of the key methods to access their functional non-equilibrium processes with high temporal resolution, but has the major drawback that experimental data is usually highly complex. Linking the spectral response to specific molecular events is a major obstacle. Here, we investigate a cyanobacteriochrome (CBCR) photoreceptor with a combined approach of transient absorption spectroscopy in the Visible and IR spectral regions. We obtain kinetic information in both spectral regions by analysis with two different fitting methods, global multiexponential fitting and lifetime analysis. We investigate the ground state dynamics that follow photoexcitation in both directions of the bi-stable photocycle (Pr* and Pg*) in the nanosecond and microsecond time regime. We find two ground state intermediates associated with the decay of Pr* and four with Pg* and report the macroscopic time constants of their interconversions. One of these processes is assigned to a structural change in the protein backbone. At later delay times, the spectroscopic responses are stretched out in time, an effect that can be better described by the lifetime analysis than the global fit. Abstract

In state of the art photoinjector electron sources, thermal emittance from photoemission dominates the final injector emittance. Therefore, low thermal emittance cathode developments and diagnostics are very important. Conventional thermal emittance measurements for the high gradient gun are time-consuming and thus thermal emittance is not measured as frequently as quantum efficiency during the lifetime of photocathodes, although both are important properties for the photoinjector optimizations. In this paper, a single shot measurement of photoemission transverse momentum, i.e., thermal emittance per rms laser spot size, is proposed for photocathode RF guns. By tuning the gun solenoid focusing, the electrons transverse momenta at the cathode are imaged to a downstream screen, which enables a single shot measurement of both the rms value and the detailed spectra of the photoelectrons transverse momenta. Abstract

Chronic HIV infection may exacerbate atherosclerotic vascular disease, which at advanced stages presents as necrotic plaques rich in crystalline cholesterol. Such lesions can catastrophically rupture precipitating myocardial infarct and stroke, now important causes of mortality in those living with HIV. However, in this population little is known about plaque structure relative to crystalline content and its chemical composition. Here, we first interrogated plaque crystal structure and composition in atherosclerotic SIV-infected macaques using non-linear optical microscopy. By stimulated Raman scattering and second harmonic generation approaches both amorphous and crystalline plaque lipid was detected and the crystal spectral profile indicated a cholesterol ester (CE) dominated composition. Versus controls, SIV+ samples had a greater number of cholesterol crystals (CCs), with the difference, in part, accounted for by crystals of a smaller length. Given the ester finding, we profiled HIV+ plaques and also observed a CE crystalline spectral signature. We further profiled plaques from Ldlr-/- mice fed a high fat diet, and likewise, found CE-dominate crystals. Finally, macrophage exposure to CCs or AcLDL induced auto-fluorescent puncta that co-stained with the LC3B autophagy sensor. In aggregate, we show that atheromatous plaques from mice, macaques and humans, display necrotic cores dominated by esterified CCs, and that plaque macrophages may induce autophagic vesicle formation upon encountering CCs. These findings help inform our knowledge of plaque core lipid evolution and how the process may incite systemic inflammation.

Abstract

A non-resonant streaming instability driven by cosmic-ray currents, also called Bell's instability, is proposed as a candidate for providing the required magnetic turbulence of efficient diffusive shock accelerations. To demonstrate the saturation level and mechanism of the non-resonant streaming instability in a laboratory environment, we attempt to develop an experiment at the Photo Injector Test Facility at DESY, Zeuthen site (PITZ). As an electron beam is used to replace the proton beam to carry the cosmic-ray current in our experiment, the polarization of the non-resonant streaming instability will be modified from the left-handed (LH) mode to the right-handed (RH) mode. The theoretical instability analysis shows that the growth rate of this RH non-resonant mode may be smaller than it of the LH resonant mode. However the LH resonant mode can be ignored in our experiment while the expected wavelength is longer than the used plasma cell. The results of PIC simulations will also support this contention and the occurrence of non-resonant streaming instability in our experiment.

Abstract

In state of the art photoinjector electron sources, thermal emittance from photoemission dominates the final injector emittance. Therefore, low thermal emittance cathode developments and diagnostics are very important. Conventional thermal emittance measurements for the high gradient gun are time-consuming and thus thermal emittance is not measured as frequently as quantum efficiency during the lifetime of photocathodes, although both are important properties for the photoinjector optimizations. In this paper, a single shot measurement of photoemission transverse momentum, i.e., thermal emittance per rms laser spot size, is proposed for photocathode rf guns. By tuning the gun solenoid focusing, the electrons' transverse momenta at the cathode are imaged to a downstream screen, which enables a single shot measurement of both the rms value and the detailed spectra of the photoelectrons' transverse momenta. Both simulations and proof of principle experiments are reported.

Abstract

Temporally-modulated electron beams have a wide array of applications ranging from the generation of coherently-enhanced electromagnetic radiation to the resonant excitation of electromagnetic wakefields in advanced-accelerator concepts. Likewise producing low-energy ultrashort microbunches could be useful for ultra-fast electron diffraction and new accelerator-based light-source concepts. In this Letter we propose and experimentally demonstrate a passive microbunching technique capable of forming a picosecond bunch train at $\sim 6$~MeV. The method relies on the excitation of electromagnetic wakefields as the beam propagates through a dielectric-lined waveguide. Owing to the non-ultrarelativistic nature of the beam, the induced energy modulation eventually converts into a density modulation as the beam travels in a following free-space drift. The modulated beam is further accelerated to $\sim20$~MeV while preserving the imparted density modulation.

Abstract

We report on an approach to produce quasi continuous-wave (cw) electron beams with an average beam current of milliamperes and a mean beam energy of a few MeV in a pulsed RF injector. Potential applications are in the planned laboratory astrophysics programs at DESY. The beam generation is based on field emission from a specially designed metallic field emitter. A quasi cw beam profile is formed over subsequent RF cycles at the resonance frequency of the gun cavity. This is realized by debunching in a cut disk structure accelerating cavity (booster) downstream of the gun. The peak and average beam currents can be tuned in beam dynamics simulations by adjusting operation conditions of the booster cavity. Optimization of the transverse beam size at specific positions (e.g., entrance of the plasma experiment) is performed by applying magnetic focusing fields provided by solenoids along the beam line. In this paper, the design of a microtip field emitter is introduced and characterized in electromagnetic field simulations in the gun cavity. A series of particle tracking simulations are conducted for multi-parametric optimization of the parameters of the produced quasi cw electron beams. The obtained results will be presented and discussed. In addition, measurements of the parasitic field emission (PFE) current (dark current) in the PITZ gun will be exemplarily shown to distinguish its order of magnitude from the produced beam current by the designed field emitter., Comment: 23 pages, 17 figures

Abstract

Particle-beam-driven plasma wakefield acceleration (PWFA) enables various novel high-gradient techniques for powering future compact light-source and high-energy physics applications. Here, a driving particle bunch excites a wakefield response in a plasma medium, which may rapidly accelerate a trailing witness beam. In this Letter, we present the measurement of ratios of acceleration of the witness bunch to deceleration of the driver bunch, the so-called transformer ratio, significantly exceeding the fundamental theoretical and thus far experimental limit of 2 in a PWFA. An electron bunch with ramped current profile was utilized to accelerate a witness bunch with a transformer ratio of 4.6_{-0.7}^{+2.2} in a plasma with length ∼10  cm, also demonstrating stable transport of driver bunches with lengths on the order of the plasma wavelength.

Abstract

This paper discusses the behavior of electron bunch charge produced in an L-band normal conducting radio frequency cavity (RF gun) from Cs2Te photocathodes illuminated with ps-long UV laser pulses when the laser transverse distribution consists of a flat-top core with Gaussian-like decaying halo. The produced charge shows a linear dependence at low laser pulse energies as expected in the quantum efficiency limited emission regime, while its dependence on laser pulse energy is observed to be much weaker for higher values, due to space charge limited emission. However, direct plug-in of experimental parameters into the space charge tracking code ASTRA yields lower output charge in the space charge limited regime compared to measured values. The rate of increase of the produced charge at high laser pulse energies close to the space charge limited emission regime seems to be proportional to the amount of halo present in the radial laser profile since the charge from the core has saturated already. By utilizing core + halo particle distributions based on measured radial laser profiles, ASTRA simulations and semi-analytical emission models reproduce the behavior of the measured charge for a wide range of RF gun and laser operational parameters within the measurement uncertainties., Comment: 15 pages, 16 figures, 2 tables

Abstract

In this paper we define Clinical Data Intelligence as the analysis of data generated in the clinical routine with the goal of improving patient care. We define a science of a Clinical Data Intelligence as a data analysis that permits the derivation of scientific, i.e., generalizable and reliable results. We argue that a science of a Clinical Data Intelligence is sensible in the context of a Big Data analysis, i.e., with data from many patients and with complete patient information. We discuss that Clinical Data Intelligence requires the joint efforts of knowledge engineering, information extraction (from textual and other unstructured data), and statistics and statistical machine learning. Abstract

This book examines tribally controlled Indian colleges established since the early 1960s and provides perspectives on their educational philosophy, history, and present status. Chapter 1 is an overview of four centuries of abortive efforts by churches and the federal government to provide higher education for American Indians, including profiles of specific missionary schools, factors in the failure of missionary education, the unusual success of Choctaw Academy, features of Carlisle Indian School and other federally funded off-reservation boarding schools, and the effects on Indian higher education of the General Allotment Act of 1887 and termination policies of the 1950s. Chapter 2 discusses the birth of tribally controlled community colleges amidst the social activism of the 1960s, and presents profiles of 19 such institutions. Chapter 3 summarizes and analyzes information on the tribally controlled colleges: student characteristics, enrollments, faculty, administrators, physical facilities, student services, college objectives, external influences, funding, and accreditation. Chapter 4 outlines other higher education opportunities for American Indians and Alaska Natives: federally controlled two-year colleges, special programs for the preparation of Indian professionals, colleges or universities with high Native American enrollments and degrees earned, and Indian Studies programs. Chapter 5 summarizes the history and current status of Indian higher education. This book contains approximately 300 references and an index. (SV)

Abstract

Background: The SARS-CoV-2 pandemic is still ongoing and a major challenge for health care services worldwide. In the first WSES COVID-19 emergency surgery survey, a strong negative impact on emergency surgery (ES) had been described already early in the pandemic situation. However, the knowledge is limited about current effects of the pandemic on patient flow through emergency rooms, daily routine and decision making in ES as well as their changes over time during the last two pandemic years. This second WSES COVID-19 emergency surgery survey investigates the impact of the SARS-CoV-2 pandemic on ES during the course of the pandemic. Methods: A web survey had been distributed to medical specialists in ES during a four-week period from January 2022, investigating the impact of the pandemic on patients and septic diseases both requiring ES, structural problems due to the pandemic and time-to-intervention in ES routine. Results: 367 collaborators from 59 countries responded to the survey. The majority indicated that the pandemic still significantly impacts on treatment and outcome of surgical emergency patients (83.1% and 78.5%, respectively). As reasons, the collaborators reported decreased case load in ES (44.7%), but patients presenting with more prolonged and severe diseases, especially concerning perforated appendicitis (62.1%) and diverticulitis (57.5%). Otherwise, approximately 50% of the participants still observe a delay in time-to-intervention in ES compared with the situation before the pandemic. Relevant causes leading to enlarged time-to-intervention in ES during the pandemic are persistent problems with in-hospital logistics, lacks in medical staff as well as operating room and intensive care capacities during the pandemic. This leads not only to the need for triage or transferring of ES patients to other hospitals, reported by 64.0% and 48.8% of the collaborators, respectively, but also to paradigm shifts in treatment modalities to non-operative approach
