1. Hierarchy of coupled Burgers-like equations induced by conditional symmetries

Abstract

It is known that $Q$-conditional symmetries of the classical Burgers' equation express in terms of three functions satisfying a coupled system of Burgers-like equations. The search of conditional symmetries of this system leads to a system of five coupled Burgers-like equations. Iterating the procedure, an infinite hierarchy of systems made of an odd number of coupled Burgers-like equations is obtained. Moreover, starting from a pair of Burgers-like equations, a similar hierarchy of systems made of an even number of coupled Burgers-like equations arises; it is also proved that the two hierarchies may be unified. Writing a generic element of this hierarchy as a matrix Burgers' equation, the existence of the matrix Hopf-Cole transformation allows for its linearization and the determination of its solution. Finally, it is shown that each element of the hierarchy possesses a five-dimensional Lie algebra of classical point symmetries. Abstract

This paper concerns with the modeling of the spreading of information through a complex, multi-layered network, where the information is transferred from an initial transmitter to a final receiver. The mathematical model is deduced within the framework of operatorial methods, according to the formal mathematical apparatus typical of quantum mechanics. Two different approaches are considered: one based on the ($H,\rho$)-induced dynamics, and one on the Gorini-Kossakowski-Sudarshan-Lindblad (GKSL) equation. 3. $(H,\rho)$--induced dynamics and large time behaviors

Abstract

In some recent papers, the so called $(H,\rho)$-induced dynamics of a system $\mathcal{S}$ whose time evolution is deduced adopting an operatorial approach, borrowed in part from quantum mechanics, has been introduced. Here, $H$ is the Hamiltonian for $\mathcal{S}$, while $\rho$ is a certain rule applied periodically (or not) on $\mathcal{S}$. The analysis carried on throughout this paper shows that, replacing the Heisenberg dynamics with the $(H,\rho)$-induced one, we obtain a simple, and somehow natural, way to prove that some relevant dynamical variables of $\mathcal{S}$ may converge, for large $t$, to certain asymptotic values. This can not be so, for finite dimensional systems, if no rule is considered. In this case, in fact, any Heisenberg dynamics implemented by a suitable hermitian operator $H$ can only give an oscillating behavior. We prove our claims both analytically and numerically for a simple system with two degrees of freedom, and then we apply our general scheme to a model describing a biological system of bacteria living in a two-dimensional lattice, where two different choices of the rule are considered.

4. $(H,\rho)$-induced dynamics and the quantum game of life

Abstract

We propose an extended version of quantum dynamics for a certain system S, whose evolution is ruled by a Hamiltonian $H$, its initial conditions, and a suitable set $\rho$ of {\em rules}, acting repeatedly on S. The resulting dynamics is not necessarily periodic or quasi-periodic, as one could imagine for conservative systems with a finite number of degrees of freedom. In fact, it may have quite different behaviors depending on the explicit forms of $H$, $\rho$ as well as on the initial conditions. After a general discussion on this $(H,\rho)$-{\em induced dynamics}, we apply our general ideas to extend the classical game of life, and we analyze several aspects of this extension.

5. Dynamics of closed ecosystems described by operators

Abstract

We adopt the so--called \emph{occupation number representation}, originally used in quantum mechanics and recently adopted in the description of several classical systems, in the analysis of the dynamics of some models of closed ecosystems. In particular, we discuss two linear models, for which the solution can be found analytically, and a nonlinear system, for which we produce numerical results. We also discuss how a damping effect could be {\em effectively} implemented in the model.

Abstract

Prognostic assessment in patients with HCC remains an extremely difficult clinical task due to the complexity of this cancer where tumour characteristics interact with degree of liver dysfunction, patient general health status, and a large span of available treatment options. Several prognostic systems have been proposed in the last three decades, both from the Asian and European/North American countries. Prognostic scores, such as the CLIP score and the recent MESH score, have been generated on a solid statistical basis from real life population data, while staging systems, such as the BCLC scheme and the recent CNLC classification, have been created by experts according to recent HCC prognostic evidences from the literature. A third category includes combined prognostic systems that can be used both as prognostic scores and staging systems. A recent example is the ITA.LI.CA prognostic system including either a prognostic score and a simplified staging system. This review focuses first on an overview of the main prognostic systems for HCC classified according to the above three categories, and, second, on a comprehensive description of the methodology required for a correct comparison between different systems in terms of prognostic performance. In this second section the main studies in the literature comparing different prognostic systems are described in detail. Lastly, a formal comparison between the last prognostic systems proposed for each of the above three categories is performed using a large Italian database including 6882 HCC patients in order to concretely apply the comparison rules previously described.

Abstract

Background: During the first coronavirus disease 2019 (COVID-19) pandemic wave, an unprecedented number of patients with respiratory failure due to a new, highly contagious virus needed hospitalization and intensive care unit (ICU) admission. The aim of the present study was to describe the communication and visiting policies of Italian intensive care units (ICUs) during the first COVID-19 pandemic wave and national lockdown and compare these data with prepandemic conditions. Methods: A national web-based survey was conducted among 290 Italian hospitals. Each ICU (active between February 24 and May 31, 2020) was encouraged to complete an individual questionnaire inquiring the hospital/ICU structure/organization, communication/visiting habits and the role of clinical psychology prior to, and during the first COVID-19 pandemic wave. Results: Two hundred and nine ICUs from 154 hospitals (53% of the contacted hospitals) completed the survey (202 adult and 7 pediatric ICUs). Among adult ICUs, 60% were dedicated to COVID-19 patients, 21% were dedicated to patients without COVID-19 and 19% were dedicated to both categories (Mixed). A total of 11,102 adult patients were admitted to the participating ICUs during the study period and only approximately 6% of patients received at least one visit. Communication with family members was guaranteed daily through an increased use of electronic devices and was preferentially addressed to the same family member. Compared to the prepandemic period, clinical psychologists supported physicians more often regarding communication with family members. Fewer patients received at least one visit from family members in COVID and mixed-ICUs than in non-COVID ICUs, l (0 [0-6]%, 0 [0-4]% and 11 [2-25]%, respectively, p < 0.001). Abstract

Background and aims: In patients with hepatocellular carcinoma (HCC), macrovascular invasion (MaVI) limits treatment options and decreases survival. Detailed data on the relationship between MaVI extension and patients' characteristics, and its impact on patients' outcome are limited. We evaluated the prevalence and extension of MaVI in a large cohort of consecutive HCC patients, analysing its association with liver disease and tumour characteristics, as well as with treatments performed and patients' survival. Methods: We analysed data of 4774 patients diagnosed with HCC recorded in the Italian Liver Cancer (ITA.LI.CA) database (2008-2018). Recursive partition analysis (RPA) was performed to evaluate interactions between MaVI, clinical variables and treatment, exploring the inter-relationship determining overall survival. Results: MaVI prevalence was 11.1%, and median survival of these patients was 6.0 months (95% CI, 5.1-7.1). MaVI was associated with younger age at diagnosis, presence of symptoms, worse Performance Status (PS) and liver function, high alphafetoprotein levels and large HCCs. MaVI extension was associated with worse PS, ascites and greater impairment in liver function. RPA identified patients' categories with different treatment indications and survival, ranging from 2.4 months in those with PS > 1 and ascites, regardless of MaVI extension (receiving best supportive care in 90.3% of cases), to 14.1 months in patients with PS 0-1, no ascites and Vp1-Vp2 MaVI (treated with surgery in 19.1% of cases). Conclusions: MaVI presence and extension, together with PS and ascites, significantly affect patients' survival and treatment selection. The decision tree based on these parameters may help assess patients' prognosis and inform therapeutic decisions.

Abstract

Background and aims: Epidemiology of hepatocellular carcinoma (HCC) is changing in most areas of the world. This study aimed at updating the changing scenario of aetiology, clinical presentation, management and prognosis of HCC in Italy during the last 15 years. Methods: Retrospective analysis of the Italian Liver Cancer (ITA.LI.CA) database included 6034 HCC patients managed in 23 centres from 2004 to 2018. Patients were divided into three groups according to the date of cancer diagnosis (2004-2008, 2009-2013 and 2014-2018). Results: The main results were: (i) a progressive patient ageing; (ii) a progressive increase of non-viral cases and, particularly, of ‘metabolic’ and ‘metabolic + alcohol’ HCCs; (iii) a slightly decline of cases diagnosed under surveillance, but with an incremental use of the semiannual schedule; (iv) a favourable cancer stage migration; (v) an increased use of radiofrequency ablation to the detriment of percutaneous ethanol injection; (vi) improved outcomes of ablative and transarterial treatments; (vii) an improved overall survival (adjusted for the lead time in surveyed patients) in the last calendar period, particularly in viral patients; (viii) a large gap between the number of potential candidates (according to oncologic criteria and age) to liver transplant and that of transplanted patients. Conclusions: During the last 15 years several aspects of HCC scenario have changed, as well as its management. The improvement in patient survival observed in the last period was likely because of a larger use of thermal ablation with respect to the less effective alcohol injection and to an improved management of intermediate stage patients.

Abstract

Background: Hepatocellular carcinoma prognosis depends on both liver and tumor determinants, especially on maximum tumor diameter, multifocality, and presence of portal vein thrombosis, despite apparently complete tumor removal by resection or liver transplantation. Aims: To examine parameters of hepatocellular carcinoma aggressiveness as tumor size increases. Methods: A large hepatocellular carcinoma database was examined for trends in serum alpha-fetoprotein and the percentage of patients with macroscopic portal vein thrombosis or tumor multifocality. Results: A total of 13,016 hepatocellular carcinoma patients were identified having full tumor and survival data. Of these, 76.56% were male and 23.44% were female, with a median age of 64.4 years. We found that as the maximum tumor diameter increased, there was a significant trend for increased alpha-fetoprotein levels (P<0.001) and an increased percentage of patients with either portal vein thrombosis or tumor multifocality, each P<0.0001. Furthermore, the increases of both alpha-fetoprotein and portal vein thrombosis were proportionately greater than the related maximum tumor diameter increases. These trends of increased alpha-fetoprotein, portal vein thrombosis, and multifocality with increasing maximum tumor diameter had non-linear patterns. Within alpha-fetoprotein and multifocality trends, there were identifiable sub-trends associated with specific maximum tumor diameter ranges. Conclusions: The greater fold-increases in alpha-fetoprotein and portal vein thrombosis compared with increases in maximum tumor diameter imply that hepatocellular carcinoma characteristics may change with increasing size to a more aggressive phenotype, suggesting that follow-up tumor sampling might be useful, in addition to baseline tumor sampling, for optimal therapeutic choices to be made.

Abstract

BACKGROUND & AIMS: According to the Barcelona Clinic Liver Cancer (BCLC) staging system, monofocal hepatocellular carcinoma (HCC) is classified as early (BCLC A) irrespective of its size, even though controversies still exist regarding staging and treatment of large tumours. We aimed at evaluating the appropriate staging and treatment for large (>5 cm) monofocal (HCC). METHODS: From the Italian Liver Cancer database, we selected 924 patients with small early monofocal HCC (2-5 cm; SEM-HCC), 163 patients with larger tumours (>5 cm; LEM-HCC) and 1048 intermediate stage patients (BCLC B). RESULTS: LEM-HCC patients had a worse overall survival (OS) than SEM-HCC (31.0 vs 49.0 months; P < .0001), and this was confirmed at multivariate analysis (HR 1.63, 95% CI 1.29-2.05; P < .0001). The small difference in OS between LEM-HCC and BCLC B patients (31.0 vs 27.0 months; P = .03) disappeared in the multivariate model (HR 0.98, 95% CI 0.77-1.25; P = .89). In all monofocal tumours, treatment was the strongest independent predictor of survival, with a progressively decreasing survival benefit moving from "curative" to "palliative" therapies. The survival of resected patients with LEM-HCC was significantly shorter than that of SEM-HCC (44.0 vs 78.0 months; P = .002), but liver resection provided the highest survival benefit in both groups compared to other treatments. CONCLUSIONS: Monofocal HCC larger than 5 cm should not be staged as BCLC A and either a different staging system or a different subgrouping of patients (e.g. BCLC AB) should be used. Liver resection, if feasible, remains the recommended treatment for all these patients.

Abstract

The ability to evaluate liver stiffness non-invasively in clinical practice by measuring transient elastography using FibroScan® has resulted in considerable interest and enthusiasm. A core study group, organized by the Italian Association for the Study of the Liver, has assessed the usefulness of FibroScan® in the diagnosis and management of liver disease in clinical practice. The group concluded that FibroScan® is a valuable, non-invasive technique and have developed a consensus report form for registering transient elastography results. In this article, we report the findings of the study group.

Abstract

The automatic identification system (AIS), a ship reporting system originally designed for collision avoidance, is becoming a cornerstone of maritime situational awareness. The recent increase of terrestrial networks and satellite constellations of receivers is providing global tracking data that enable a wide spectrum of applications beyond collision avoidance. Nevertheless, AIS suffers the lack of security measures that makes it prone to receiving positions that are unintentionally incorrect, jammed or deliberately falsified. In this study, the authors' analyse a solution to the problem of AIS data verification that can be implemented within a generic networks of ground AIS base stations with no need for additional sensors or technologies. The proposed approach combines a classic radio-localisation method based on time difference of arrival with an extended Kalman filter designed to track vessels in geodetic coordinates. The approach is validated using anonymised real AIS data collected by multiple base stations that partly share coverage areas. The results show a deviation between the estimated origin of detected signals and the broadcast position data in the order of hundreds of metres, therefore demonstrating the operational potential of the methodology.

Abstract

Treatment with peginterferon α-2a (PegIFN) for 48 weeks is the standard of care for selected HBeAg-negative patients chronically infected with hepatitis B virus (HBV), but with limited treatment efficacy. A study was undertaken to investigate whether treatment extension to 96 weeks improves the outcome in this patient population.128 HBeAg-negative patients (120 genotype D) were randomised to weekly 180 μg PegIFN for 48 weeks (group A, n=51), 180 μg PegIFN for 48 weeks followed by 135 μg weekly for an additional 48 weeks (group B, n=52) or 180 μg PegIFN plus lamivudine (100 mg/day) for 48 weeks then 135 μg PegIFN for 48 weeks (group C, n=25). Endpoints were alanine aminotransferase normalisation plus HBV DNA3400 IU/ml (primary), HBV DNA2000 IU/ml and HBsAg clearance at 48 weeks after treatment.Forty-eight weeks after treatment, six patients in group A and 13 in group B achieved alanine aminotransferase normalisation plus HBV DNA3400 IU/ml (11.8% vs 25.0%, p=0.08), 6 vs 15 patients had HBV DNA2000 IU/ml (11.8% vs 28.8%, p=0.03), 0 vs 3 achieved HBsAg clearance (0% vs 5.8%, p=0.24) and 0 vs 5 had HBsAg10 IU/ml (0% vs 9.6%, p=0.06). While extended PegIFN treatment was the strongest independent predictor of response, the combination with lamivudine did not improve responses. Discontinuation rates were similar among the groups (19.6%, 23.1%, 32.0%, p=0.81) and were mostly due to PegIFN-related adverse events.In HBeAg-negative genotype D patients with chronic hepatitis B, PegIFN treatment for 96 weeks was well tolerated and the post-treatment virological response improved significantly compared with 48 weeks of treatment.http://ClinicalTrials.gov registration number: NCT01095835.

Abstract

In this letter, we apply a Bernoulli Filter for moving target detection and tracking using real Multiple Input Multiple Output radar data in a challenging environment. The interest in MIMO radars has been growing since they can provide high performance at lower cost than conventional antenna arrays. The Bernoulli Filter performs tracking using the Track-Before-Detect approach, which is required to achieve satisfactory tracking performance in low signal-to-noise scenarios. The algorithm is validated with real MIMO data of a walking person.

Abstract

We study the geometry of differential equations determined uniquely by their point symmetries, that we call Lie remarkable. We determine necessary and sufficient conditions for a differential equation to be Lie remarkable. Furthermore, we see how, in some cases, Lie remarkability is related to the existence of invariant solutions. We apply our results to minimal submanifold equations and to Monge–Ampère equations in two independent variables of various orders.

Abstract

Bronchial carcinoid is the most frequent cause of Cushing's syndrome due to ectopic ACTH production. The authors report a case of bronchial carcinoid which diagnosis was difficult because of the presence of pulmonary mycosis, that determined a hypercorticosuprarenalism. Medical treatment with octreotide, ketoconazolo and mitotane was useless, and bilateral suprarenalectomy was performed. A scintigraphy with raced somatostatin revealed a left lung area capting radiation. A CT scan of the thorax revealed a lesion of the lingula and the patient underwent an atypical lung resection with complete solution of the symptom. The problems of diagnosis and treatment of neuroendocrine tumors of the lung are discussed and the importance of SSA in the diagnostic procedure is pointed out.
