Abstract

Background: Metastatic melanoma stands out as the most lethal form of skin cancer because of its high propensity to spread and its remarkable resistance to treatment methods. Methods: In this review article, we address the incidence of melanoma worldwide and its staging phases. We thoroughly investigate the different melanomas and their associated risk factors. In addition, we underscore the principal therapeutic goals and pharmacological methods that are currently used in the treatment of melanoma. Results: The implementation of targeted therapies has contributed to improving the approach to patients. However, because of the emergence of resistance early in treatment, overall survival and progression-free periods continue to be limited. Conclusions: We provide new insights into plant serine protease inhibitor therapeutics, supporting high-throughput drug screening soon, and seeking a complementary approach to explain crucial mechanisms associated with melanoma. Abstract

Background: SARS‐CoV2 virus, responsible for the COVID‐19 pandemic, has four structural proteins and 16 nonstructural proteins. S‐protein is one of the structural proteins exposed on the virus surface and is the main target for producing neutralizing antibodies and vaccines. The S‐protein forms a trimer that can bind the angiotensin‐converting enzyme 2 (ACE2) through its receptor binding domain (RBD) for cell entry. Aims: The goal of this study was to express in HEK293 cells a new RBD recombinant protein in a constitutive and stable manner in order to use it as an alternative immunogen and diagnostic tool for COVID‐19. Materials & Methods: The protein was designed to contain an immunoglobulin signal sequence, an explanded C‐terminal section of the RBD, a region responsible for the bacteriophage T4 trimerization inducer, and six histidines in the pCDNA‐3.1 plasmid. Following transformation, the cells were selected with geneticin‐G418 and purified from serum‐fre culture supernatants using Ni2+‐agarand size exclusion chromatography. The protein was structurally identified by cross‐linking and circular dichroism experiments, and utilized to immunize mice in conjuction with AS03 or alum adjuvants. The mice sera were examined for antibody recognition, receptor‐binding inhibition, and virus neutralization, while spleens were evaluated for γ‐interferon production in the presence of RBD. Results: The protein released in the culture supernatant of cells, and exhibited a molecular mass of 135 kDa with a secondary structure like the monomeric and trimeric RBD. After purification, it formed a multimeric structure comprising trimers and hexamers, which were able to bind the ACE2 receptor. It generated high antibody titers in mice when combined with AS03 adjuvant (up to 1:50,000). The sera were capable of inhibiting binding of biotin‐labeled ACE2 to the virus S1 subunit and could neutralize the entry of the Wuhan virus strain into cells at dilutions up to 1:2000. It produced specific IFN‐γ producing cells in immunized mouse splenocytes. Discussion: Our data describe a new RBD containing protein, forming trimers and hexamers, which are able to induce a protective humoral and cellular response against SARS‐CoV2. Abstract

The peptide derived from E. contortisiliquum trypsin inhibitor (Pep-3-EcTI), peptide derived from kallikrein inhibitor isolated from B. bauhinioides (Pep-BbKI), and B. rufa peptide modified from B. bauhinioides (Pep-BrTI) peptides exhibit anti-inflammatory and antioxidant activities, suggesting their potential for treating asthma--chronic obstructive pulmonary disease (COPD) overlap (ACO). We compared the effects of these peptides with dexamethasone (DX) treatment in an ACO model. In this study, 11 groups of male BALB/c mice were pre-treated under different conditions, including sensitization with intraperitoneal injection and inhalation of ovalbumin (OVA), intratracheal instillation of porcine pancreatic elastase (ELA), sensitization with intraperitoneal injection, and various combinations of peptide treatments with Pep-3-EcTI, Pep-BbKI, Pep-BrTI, dexamethasone, and non-treated controls (SAL-saline). Respiratory system resistance, airway resistance, lung tissue resistance, exhaled nitric oxide, linear mean intercept, immune cell counts in the bronchoalveolar lavage fluid, cytokine expression, extracellular matrix remodeling, and oxidative stress in the airways and alveolar septa were evaluated on day 28. Results showed increased respiratory parameters, inflammatory markers, and tissue remodeling in the ACO group compared to controls. Treatment with the peptides or DX attenuated or reversed these responses, with the peptides showing effectiveness in controlling hyperresponsiveness, inflammation, remodeling, and oxidative stress markers. These peptides demonstrated an efficacy comparable to that of corticosteroids in the ACO model. However, this study highlights the need for further research to assess their safety, mechanisms of action, and potential translation to clinical studies before considering these peptides for human use. Abstract

Helminth infections, mainly by gastrointestinal nematodes (GIN), are one of the main concerns for animal health, welfare and productivity in grazing ruminant livestock worldwide. The use of a sensitive, precise, accurate, low-cost, and easy-to-perform copromicroscopic technique is of pivotal importance to perform reliable fecal egg count (FEC) and fecal egg count reduction test (FECRT), in order to determine the need of anthelmintic treatment, but also anthelmintic efficacy or resistance. This approach is fundamental to a correct and efficient control of GIN. Unfortunately, in worldwide ruminant farm practice, repeated anthelmintic treatments are carried out, without prior diagnosis of infection, contributing to the spread of Anthelmintic Resistance (AR). Tackling this phenomenon, improving mainly the GIN diagnosis and AR status in farm animals, is a priority of the European COST Action “COMBAR—COMBatting Anthelmintic Resistance in Ruminants” and of the STAR-IDAZ International Research Consortium on Animal Health. One of the specific objectives of the COMBAR Working Group 1 (WG1) is to conduct an European market analysis of new diagnostics and develop a business plan for commercial test introduction, leveraging technical know-how of participants. Since the Mini-FLOTAC in combination with the Fill-FLOTAC may be considered a good candidate for a standardized FEC and FECRT in the laboratory, as well as directly in the field, the aim of this study was to conduct SWOT (Strength—Weaknesses—Opportunities—Threats) and PESTEL (Political, Economic, Social, Technological, Environmental, and Legal) analyses of these tools in 20 European countries involved in the COMBAR WG1, in order to identify the opportunities, barriers, and challenges that might affect the Mini-FLOTAC and Fill-FLOTAC commercialization in Europe.

Abstract

This is a descriptive, cross-sectional survey which aims to evaluate the characteristics of the productive processes of meals in the implementation of NutriSus in four municipalities of Rio Grande do Norte. For the data collection, interviews and visits were carried out to the daycare centers and to the Technical Manager of each participating municipality, to make the observation and the approach of questions to evaluate how NutriSus was implemented, as well as the recordings of the interviews and transcription of the obtained audios. These data made possible the knowledge about the strategies used in each city for the implementation of the Program, in order to know if it was necessary to carry out new menu planning, implementation or adjustments in some preparations, besides verifying how is the acceptability. To achieve the expected result of the Program, it is necessary to follow the recommended administration schedule: two months of supplementation with a three or four-month interval. In the visited municipalities, it was identified that the implementation of the strategy occurred only as a pilot project, happening in some months of 2014 or 2015, not completing the cycle, without progression in 2016 and with a limited number of children. This discontinuity was due to the insufficient number of sachets sent to the municipalities by the Ministério da Saúde (Ministry of Health). It is concluded that nationalized programs that involve the three governmental spheres in their moment of implementation, such as Nutrisus, require a consistent planning, with the participation of the involved actors, which encompasses the factors necessary for its correct execution and full coverage of the target population. Berdasarkan hasil observasi yang dilakukan oleh penulis berkaitan dengan pelaksanaan posyandu lansia di wilayah kerja Puskesmas Nunpene, kegiatan Posyandu Lansia belum terlaksana dengan baik, hal ini disebabkan karena minat, kualitas kader serta anggaran kegiatan Program Posyandu Lansia juga masih terbatas. Penelitian ini bertujuan untuk mengetahui bagaimana pelaksanaan posyandu lansia, kelengkapan saran dan prasarana, ketersediaan anggaran serta kulitas dan kuantitas Sumber Daya Manusia yaitu Tenaga Kesehatan dan Kader di Puskesmas Nunpene. Penelitian ini menggunakan jenis penelitian kualitatif deskriptif, dengan teknik pengumpulan data menggunakan wawancara, pengamatan dan dokumentasi, dengan jumlah sampel sebanyak 25 orang. Hasil penelitian ini menunjukkan bahwa sarana dan prasarana posyandu lansia diwilayah kerja Puskesmas Nunpene sudah tersedia dengan lengkap sedangkan minat lansia dalam pelaksanaan program posyandu lansia masih rendah, anggaran untuk posyandu lansia di wilayah kerja Puskesmas Nunpene juga belum tercukupi dan belum ada dana khusus untuk pelaksanaan program posyandu, serta kualitas dan kuantitas sumber daya manusia dalam hal ini kualitas tenaga pembantu seperti kader masih perlu ditingkatkan. Posyandu lansia bisa berjalan dengan baik jika minat lansia dalam mengunjungi posyandu tinggi, terdapat anggaran yang cukup serta ketersedian tenaga kesehatan dan kader dengan kualitas dan jumlah yang memadai. Petugas Kesehatan dan kader diharapkan lebih aktif melakukan kunjungan rumah pada lansia yang mempunyai keterbatasan gerak.

50. Femoral neck fracture: the reliability of radiologic classifications

Abstract

Background Femoral neck fractures (FNF) are one of the most common injury in the elderly. A valid radiographic classification system is mandatory to perform the correct treatment and to allow surgeons to facilitate communication. This study aims to evaluate reliability of 2018 AO/OTA Classification, AO/OTA simplified and Garden classification. Methods Six Orthopaedic surgeons, divided in three groups based on trauma experience, evaluated 150 blinded antero-posterior and latero-lateral radiography of FNF using Garden classification, 2018 AO/OTA and simplified AO/OTA classification. One month later, the radiographs were renumbered and then each observer performed a second evaluation of the radiographs. The Kappa statistical analysis was used to determine the reliability of the classifications. Cohen’s Kappa was calculated to determine intra and inter observer reliability. Fleiss’ Kappa was used to determine multi-rater agreement. Results The k values of interobserver reliability for Garden classification was from 0,28 to 0,73 with an average of 0,49. AO classification showed reliability from 0,2 to 0,42, with average of 0,30. Simplified AO/OTA classification showed a reliability from 0,38 to 0,58 with an average of 0,48. The values of intra observer reliability for Garden classification was from 0,48 to 0,79 with an average of 0,63. AO classification showed reliability from 0,2 to 0,64 with an average of 0,5. Simplified AO/OTA classification showed a reliability from 0,4 to 0,75 with an average of 0,61. Conclusion The revised 2018 AO/OTA classification simplified the previous classification of intracapsular fracture but remain unreliable with only fair interobserver reliability. The simplified AO/OTA classification show a reliability similar to Garden classification, with a moderate interobserver reliability. The experience of the surgeons seems not to improve reliability. No classification has been shown to be superior in terms of reliability.
