The scarcity of high-purity limestone subjects the cement industry to consider alternative supplementary cementitious materials for cement production. This research aimed to investigate the mechanical properties and the hydration kinetics of cement pastes in the presence of high levels and different fineness of dolomite, especially the formation of thaumasite. A dolomite filler with 20% magnesium oxide, with average diameters of 10 µm, 6 µm and 4 µm was used as a partial replacement for Portland cement at 0%, 15% and 35% in cement pastes cured at 5 ºC and 20 ºC. The results of compressive strength showed similarities in all pastes analyzed, but the fineness of the dolomite showed a significant difference between the strengths obtained. The isothermal calorimetry indicated that the presence of dolomite had a nucleation and dilution effect. The thermogravimetry revealed that dependent of the substitution levels, the pastes with dolomite presented significant differences in the amount of ettringite and carbonate phases. The mineralogical analysis detected the presence of thaumasite only at the temperature of 20 ºC, which this mineral can also form at higher temperatures than those reported in the literature. Finally, we conclude positively about the possibility of using dolomite in cementitious matrices at levels up to 35%, regardless of fineness and curing temperature. Resumo A escassez de calcário de alta pureza sujeita a indústria cimenteira a considerar materiais cimentícios suplementares alternativos para a produção de cimentos. Esta pesquisa buscou investigar a cinética de hidratação e as propriedades de pastas cimentícias na presença de elevados teores e diferentes finuras de dolomita, em especial à formação de taumasita. Adotou-se um fíler dolomítico com 20% de óxido de magnésio, de diâmetros médios de 10 µm, 6 µm e 4 µm como substituto parcial do cimento Portland nos teores 0%, 15% e 35% em pastas cimentícias curadas a 5 ºC e 20 ºC. Os resultados de resistência à compressão apresentaram semelhanças em todas as pastas analisadas, mas a finura da dolomita mostrou uma diferença significativa entre as médias de resistências obtidas. A calorimetria isotérmica apontou que a presença de dolomita apresentou efeito de nucleação e diluição. A termogravimetria revelou que dependente dos teores de substituição, as pastas com dolomita apresentaram diferenças significativas na quantidade de fases de etringita e carbonatos. A análise mineralógica detectou a presença de taumasita apenas na temperatura de 20 ºC, em que esse mineral também pode se formar em temperaturas mais elevadas do que as temperaturas relatadas na literatura. Por fim, conclui-se positivamente sobre a possibilidade de utilização de dolomita em matrizes cimentícias em teores de até 35%, independente da finura e da temperatura de cura.

BACKGROUND: Patients hospitalized with COVID-19 suffer thrombotic complications. Risk factors for poor outcomes are shared with coronary artery disease.OBJECTIVES: To investigate the efficacy of an acute coronary syndrome regimen in patients hospitalized with COVID-19 and coronary disease risk factors.METHODS: A randomized controlled, open-label trial across acute hospitals (United Kingdom and Brazil) added aspirin, clopidogrel, low-dose rivaroxaban, atorvastatin, and omeprazole to standard care for 28 days. Primary efficacy and safety outcomes were 30-day mortality and bleeding. The key secondary outcome was a daily clinical status (at home, in hospital, on intensive therapy unit admission, or death).RESULTS: Three hundred twenty patients from 9 centers were randomized. The trial terminated early due to low recruitment. At 30 days, there was no significant difference in mortality (intervention vs control, 11.5% vs 15%; unadjusted odds ratio [OR], 0.73; 95% CI, 0.38-1.41; p = .355). Significant bleeds were infrequent and were not significantly different between the arms (intervention vs control, 1.9% vs 1.9%; p > .999). Using a Bayesian Markov longitudinal ordinal model, it was 93% probable that intervention arm participants were more likely to transition to a better clinical state each day (OR, 1.46; 95% credible interval [CrI], 0.88-2.37; Pr [beta > 0], 93%; adjusted OR, 1.50; 95% CrI, 0.91-2.45; Pr [beta > 0], 95%) and median time to discharge to home was 2 days shorter (95% CrI, -4 to 0; 2% probability that it was worse).CONCLUSION: Acute coronary syndrome treatment regimen was associated with a reduction in the length of hospital stay without an excess in major bleeding. A larger trial is needed to evaluate mortality.

The cutaneous mastocytosis is characterized by infiltration and proliferation of mast cells in the skin with no evidence of extracutaneous involvement. The clinical findings most frequently found are brownish macules or papules slightly elevated, frequently localized in the limbs, chest and abdomen. Mutations in the c-kit receptor (CD117) are frequent and provide the pathophysiology basis of the disease. These mutations allow autophosphorylation of the c-kit receptor and subsequent degranulation of mast cell precipitated with a variety of stimuli, releasing local or systemic cell mediators capable of causing symptoms. Skin lesions occur in both the systemic and the cutaneous form of the disease, though the latter is more frequently seen in children, while most adults with mastocytosis present signs and symptoms of systemic involvement. Although this clinical pattern, we report a rare case of adult-onset cutaneous mastocytosis in a masculine patient of 32 years old. Clinical presentation featured hyperpigmented macules with brownish coloration diffused through the chest and limbs. Skin and bone marrow biopsies and laboratory testing were made and strengthened the diagnosis of cutaneous mastocytosis, a clinical presentation with little similar case reports.

It hinges on visual development of the interface and business logic but also on easy integration with data stores and services while delivering robust applications that scale.Data integration increasingly means accessing a variety of NoSQL stores. Unfortunately, the diversity of data and processing models, that make them useful in the first place, is difficult to reconcile with the simplification of abstractions exposed to developers in a low-code platform. Moreover, NoSQL data stores also rely on a variety of general purpose and custom scripting languages as their main interfaces.In this paper we report on building a polyglot data access layer for the OutSystems Platform that uses SQL with optional embedded script snippets to bridge the gap between low-code and full access to NoSQL stores.

Estes materiais devem ser compostos principalmente por óxidos de silício, alumínio e ferro, que, em fases amorfas, proporcionarão a reação pozolânica. O objetivo deste trabalho foi estudar a reatividade do óxido de ferro e sua influência na reação pozolânica da metacaulinita. Foi realizada calcinação de caulim com elevada pureza para obtenção de um metacaulim com desidroxilação de 97,17%. O estudo foi conduzido em pasta de cimento branco, por ser isento de ferro, com substituições de 25% da massa de cimento por composições dos materiais testados. Os teores de óxido de ferro empregados em relação à massa total foram de 1,5%, 3%, 4,5%, 6%, 9% e 25%. A fim de comparar com pastas com metacaulim e sem óxido de ferro, mas que tivessem o mesmo consumo de cimento, foram produzidas amostras com composições de metacaulim e pó de quartzo (considerado inerte nesta pesquisa) para avaliação da hidratação aos 3, 7, 28 e 63 dias. As amostras produzidas com 25% de metacaulim obtiveram índice de desempenho mecânico aos 63 dias de 141,17% em relação à amostra padrão com 100% de cimento, enquanto as amostras com 25% de óxido de ferro e 25% de pó de quartzo obtiveram índices de 82,11 e 83,86 % respectivamente; enquanto as amostras com composições de metacaulim com pó de quartzo ou óxido de ferro apresentaram elevação nos índices à medida que o teor de metacaulim aumentou, não sendo observada diferença significativa quando compara-se pastas com mesmo teor de óxido de ferro e pó de quartzo. Houve redução do teor de portlandita e crescimento na formação dos demais produtos hidratados com o aumento do teor de metacaulim nas composições, verificados pelas áreas de curvas de DTG e pelo aumento da água combinada; sendo proposto ainda um método de cálculo de água combinada efetiva (sem portlandita). Destaca-se que nos difratogramas de DRX não foi possível identificar a formação de produtos hidratados contendo ferro, inferindo-se que o óxido permaneceu inerte durante o processo de hidratação do cimento e durante a reação pozolânica da metacaulinita. Diante do estudo realizado, compreende-se que o único óxido de ferro presente em argilas calcinadas é a hematita, observada em análise de DRX, que não possui atividade pozolânica e não participa da hidratação do cimento, não podendo ser classificado como óxido pozolânico. Pozzolans are materials that react with calcium hydroxide in the presence of water, forming secondary hydrated products and promoting gains in strength and durability in cementitious products. These materials must be composed mainly of silicon, aluminum and iron oxides, which, in amorphous phases, will provide the pozzolanic reaction. The objective of this work was to study the reactivity of iron oxide and its influence on the pozzolanic reaction of metakaolinite. Kaolin, with high purity, was calcined to obtain metakaolin with 97,17% dehydroxylation. The study was conducted in white cement paste, as it is Fe-free, with 25% replacement of the cement mass by compositions of the materials tested. The iron oxide contents used in relation to the total mass were 1,5%, 3%, 4,5%, 6%, 9% and 25%. To compare pastes with metakaolin and without iron oxide, but which had the same cement consumption, samples were produced with compositions of metakaolin and quartz powder (considered inert in this research) to evaluate hydration at 3, 7, 28 and 63 days. The samples produced with 25% metakaolin had a mechanical performance index at 63 days of 141,17% compared to the standard sample with 100% cement, while the samples with 25% iron oxide and 25% quartz powder obtained indexes of 82,11 and 83,86% respectively; while the samples with compositions of metakaolin with quartz powder or iron oxide showed an increase in the indexes as the metakaolin content increased, with no significant difference being observed when comparing pastes with the same iron oxide content and quartz powder. There was a reduction in the content of portlandite and growth in the formation of other hydrated products with the increase in the content of metakaolin in the compositions, verified by the areas of DTG curves and by the increase in the combined water; a method for calculating effective combined water (without portlandite) is also proposed. It is noteworthy that in XRD diffractograms it was not possible to identify the formation of hydrated products containing iron, implying that the oxide remained inert during the cement hydration process and during the pozzolanic reaction of metakaolinite. In view of the study carried out, it is understood that the only iron oxide present in calcined clays is hematite, observed in XRD analysis, which does not have pozzolanic activity and does not participate in the hydration of cement, and cannot be classified as pozzolanic oxide.

Background Studies have demonstrated an association between anti-TNF/immunomodulator agents used in inflammatory bowel disease (IBD) and impaired hepatitis B virus (HBV) vaccine immunogenicity, but little data exist on whether specific medication types affect protective HBsAb titers. Our aim was to analyze this association. Methods This is a retrospective cohort study. Inclusion criteria: age ≥18, diagnosis of Crohn's disease (CD) or ulcerative colitis (UC), previous HBV vaccination series and/or ≥1 positive HBsAb, and record of IBD therapy in 6 months before titer level. Patients were stratified based upon medication exposures: anti-TNF, immunomodulator, combination anti-TNF and immunomodulatory, and a reference arm. Titer levels following vaccination and specific medication types given in the 6 months before titer were recorded. Seroprotection was defined as HBsAb ≥10 IU/l and ≥100 IU/l. Results The study cohort (N = 391) was 70.8% white, 51.4% female and 64.2% had CD and 35.8% had UC. The mean age was 45.8 years. A significantly lower percentage of patients exposed to anti-TNF, immunomodulator or dual therapy had titers ≥10 (P < 0.01). Regarding specific medications, only patients exposed to infliximab (P < 0.01) were less likely to have titer levels ≥10, after controlling for other medication exposures, age at titer level, and interval time between vaccination/titer level. This was not found for patients exposed to adalimumab, methotrexate, 6-mercaptopurine, or azathioprine. Conclusions Patients exposed to infliximab were significantly less likely to have protective HBsAb titer levels following vaccination, a trend not seen in patients on adalimumab. Efforts to vaccinate IBD patients against HBV before use of immunomodulators and anti-TNFs, infliximab specifically, and screen periodically thereafter must be reinforced.

Historicamente, a investigação criminal foi evoluindo desde a criação da Guarda Nacional Republicana, acompanhando a complexidade e exigências crescentes da sociedade, mas mantendo sempre a lógica de proximidade com o cidadão e procurando responder aos problemas criminais locais. Neste sentido, apesar da evolução do quadro legal, no que diz respeito à organização da investigação criminal e ao sistema de segurança interna, a Guarda procurou através da evolução do funcionamento dos Núcleos de Investigação Criminal adaptar a sua resposta, nos termos previstos na lei. Desta forma, é analisada a atuação dos Núcleos de Investigação Criminal da Guarda Nacional Republicana no âmbito da resposta à criminalidade local. Para efetuar esta análise foi utilizado primeiramente o método histórico, materializado através de uma revisão de literatura. De seguida, foi utilizado o método inquisitivo, operacionalizado através de entrevistas a entidades da Guarda Nacional Republicana e interlocutores locais. Destarte, foi possível concluir que a atividade dos Núcleos de Investigação Criminal é fundamental no âmbito do combate à criminalidade local, na medida em que, ao nível do seu Destacamento Territorial, são que tem o conhecimento mais próximo da população onde, conseguem perceber o movimento desta assim como têm a capacidade de, localmente, perceber quais são os focos de criminalidade e de os colmatar. Nesta senda, estes núcleos, contribuem substancialmente para um aumento do sentimento de segurança das populações locais através dos resultados obtidos nas suas investigações. Todavia, é possível aumentar a eficiência destes, nomeadamente, através do aproveitamento e canalização para a atividade de Investigação Criminal das viaturas apreendidas e revertidas a favor do Estado. Atendendo à constante evolução tecnológica existente nos dias de hoje, há a necessidade de uma constante atualização quer de meios técnicos, quer ao nível da formação destes militares, sob pena de estes ficarem desatualizados num curto espaço de tempo e, por conseguinte, diminuir a capacidade de resposta face a certos tipos de criminalidade por parte dos Núcleos de Investigação Criminal.

Software’s structure profoundly affects its development and maintenance costs. Poor software’s structure may lead to well-known design flaws, such as large modules or long methods. A possible ap- proach to reduce a module’s complexity is the Extract Method refactor- ing technique. This technique allows the decomposition of a large and complex method into smaller and simpler ones, while reducing the orig- inal method’s size and improving its readability and comprehension. The OutSystems platform is a low-code platform that allows the de- velopment of web and mobile applications that rely on a set of visual Domain-Specific Languages (DSLs). Even low-code languages when im- properly used can lead to software that has maintenance issues like long methods. Thus, the purpose of this paper is to present the research and devel- opment done to provide the OutSystems platform with a tool that au- tomatically suggests Extract Method refactoring opportunities. The re- search combines program slicing techniques with code complexity metrics to calculate the best refactoring opportunities that preserve programs’ functionality. Pediatric management of complicated pleural effusion (CPE) remains controversial. Different approaches include antibiotics and chest tube drainage alone or the use of fibrinolitics, videothorascoscopy (VTC) and surgical decortication through thoracotomy.The aim of the present study was to review, evaluate and update technical approach to CPE. We retrospectively reviewed the clinical files of children admitted to the Pediatric Respiratory Ward between 1992 and 2003 with the diagnosis of CPE. Twenty-five patients were included [15 male (60%)]. Mean (±SD) age was 37,4 (±37,0) months. Bacteria were identified in 17/25 (68%) [S. aureus in 6/17 (35%), St. pneumoniae in 5/17 (29%)], 16/17 (94%)in the pleural fluid. Twenty-five children were treated with antibiotics and thoracocentesis (100%). Chest tube drainage was required in 22/25 (88%) with mean (±DP) duration of 14,2 (±7,8) days. Fibrinolitics were employed in 1 only case and surgical decortication in 11/25 (44%). One patient (4%) was submitted to primary VTC. Median length of stay was 30,4 (±15,1) days and no deaths were recorded. Center skills in CPE management are critical on the choice of the technique and the timing of approach. This seems to influence immediate prognosis. Resumo: A abordagem do derrame pleural parapneumónico complicado, em idade pediátrica, permanece controversa. As opções terapêuticas incluem antibioticoterapia e drenagem pleural contínua, instilação intrapleural de fibrinolíticos, videotoracoscopia e toracotomia com descorticação. O objectivo deste estudo foi rever, avaliar e actualizar a abordagem ao derrame pleural complicado. Procedeu-se à revisão retrospectiva dos processos clínicos das crianças internadas na UPP por derrame pleural complicado entre 1992 e 2003. Foram incluídos 25 doentes, com idade média (±DP): 37,4 (± 37,0) meses, sendo 15/25 (60%) do sexo masculino. A identificação do agente foi possível em 17/25 (68%) casos [S. aureus 6/17 (35%), St. pneumoniae 5/17 (29%)], no líquido pleural em 16/17 (94%) casos. Todos os doentes realizaram toracocentese e antibioticoterapia sistémica. A drenagem pleural contínua foi instituída em 22/25 (88%) casos com duração média (±DP): 14,2 (± 7,8) dias; em 1 caso houve instilação de fibrinolíticos intrapleurais e em 11/25 (44%) realizou-se toracotomia com descorticação. Um doente foi submetido a videotoracoscopia primária. A duração média de internamento (±DP) foi de 30,4 (± 15,1) dias e não ocorreram óbitos. A experiência do centro é determinante na abordagem escolhida e na rapidez de actuação. Provavelmente ambas influenciam o prognóstico imediato. Dissertação de Mestrado apresentada ao ISPA - Instituto Universitário Constatou-se na revisão de literatura uma tendência crescente de programas de arte-terapia na saúde mental e uma escassez de estudos que investiguem o impacto que o teatro, em particular, tem na vida e no processo de Recovery dos utentes de serviços de reabilitação psicossocial. Propus-me investigar a experiência subjetiva, de utentes e técnicos, acerca da implementação de um grupo de teatro no contexto sócio-ocupacional do G.I.R.A. (Grupo de Intervenção e Reabilitação Ativa). Uma amostra composta por 6 participantes (4 utentes da G.I.R.A. diagnosticados com doença mental, 2 técnicos que dinamizaram as sessões) que pertenceram ao grupo de teatro do “Girarte”, com os quais realizei 6 entrevistas fenomenológicas semi-diretivas, tendo concluído a existência de impacto na vida e no Recovery Psicológico e Social dos participantes. ABSTRACT: I noted, in the literature review a growing trend of art therapy programs in mental health and a paucity of studies to investigate the impact that theater, in particular, has in life and in the Recovery process of the users in psychosocial rehabilitation services. Therefore I investigated the subjective experience of users and technicians, about the implementation of a theater group in the socio-occupational context of G.I.R.A. (Group of Intervention and Active Rehabilitation ). For this I used a sample of 6 participants (4 users of G.I.R.A. diagnosed with mental illness and 2 technicians that directed the sessions) that belonged to the theater group "Girarte", to this end, I conducted 6 phenomenological interviews semidirective, and I concluded that there was an impact in the life and in the Psychological and Social Recovery of the participants.
