Background: Left ventricular assist device (LVAD) candidates are at increased risk of immune dysregulation and infectious complications. To attenuate the elevated proinflammatory cytokine levels and associated adverse clinical outcomes, it has been postulated that extracorporeal blood purification could improve the overall survival rate and morbidity of patients undergoing LVAD implantation. Methods: We retrospectively reviewed prospectively collected data of 15 patients who underwent LVAD implantation at our center between January 2021 and March 2022. Of these, 15 (100%) who received HeartMate 3™ (St. Jude Medical, Abbott, MN, USA) device were eligible. Intraoperatively, patients were single randomized 1:1:1 to three groups: group 1, patients who received Cytosorb therapy (n = 5; installed in the CPB circuit); group 2, patients who received Jafron HA330 (n = 5; installed in the CPB circuit); and control group 3, patients who did not receive filter (n = 5; usual care, neither Cytosorb nor Jafron during CPB). Baseline patient characteristics and intraoperative data were compared between the groups. Blood sample analyses were performed to assess the levels of inflammatory markers (IL-1, 6, 8; CRP, Leukocyte, Lactate, PCT, NT-proBNP, TNF-α) in both preoperative and postoperative data. Results: Baseline patient characteristics were similar in all three groups. We found that IL1α; IL 6; IL8; Lactatedehydrogenase, PCT, pro-BNP, CRP; Leukocyte, and TNFα levels significantly increased with LVAD implantation and that neither Cytosorb nor Jafron influenced this response. In-hospital mortality and overall survival during follow-up were similar among the groups. Conclusion: Our preliminary results showed that hemoadsorption therapy using Cytosorb or Jafron hemoadsorption (HA) 330 may not be clinically beneficial for patients with advanced heart failure undergoing LVAD implantation. Large prospective studies are needed to evaluate the potential role of HA therapy in improving outcomes in patients undergoing LVAD implantation.

The relevance of the study is caused by the necessity to form a plan for the development of secondary vocational education that provides training of the most popular and promising specialties and working professions in accordance with international standards and advanced technologies on the basis of the leading professional educational organizations. The goal of this article is to identify the main motives and meanings of applicants' choices of popular and prospective specialties of secondary vocational education and working professions during the psychological, pedagogical and sociological research to paint a sociological portrait of students of vocational education and specialties from the TOP-50 list. The goals of the research are to carry out a comparative analysis of: 1) professional self-determination, 2) value orientations, 3) attitudes toward the teaching and upbringing, 4) life plans of applicants and students. The leading method in investigating this problem is the method of questioning. In our study we surveyed the applicants and students of specialties from the TOP-50 list. Results of the research: In order to study certain aspects of the sociological portrait of students through the method of questioning of students from the TOP-50 specialties we compiled a sociological questionnaire that included four blocks. The questionnaire for students included 28 questions and the questionnaire for applicants included 33 open and closed questions with the use of different scales with the possibility of selecting several of the listed options and the fields for answers. The results of the questioning of applicants and students are presented; the similarities and differences of the key motives, life plans, values of applicants and students who have chosen perspective professions of secondary vocational education are identified and summarized.

The cathode material of Na2/3Ni1/3Mn2/3O2 composition (NNMO) was obtained by the carbonate coprecipitation followed by solid-phase reaction and characterized by XRD analysis, ICP-MS, and electron and impedance spectroscopy. NNMO crystallizes in a P2-type layered hexagonal structure (sp. gr. P63/mmc), consists of spherical agglomerates of ~ 1–3 microns in size, and forms from the plate-like primary grains. The NNMO ionic conductivity value at room temperature was 1.8*10−4 and 1.3*10−4 S cm−1 when measured perpendicularly and parallel to the compression axis, respectively. The estimated in dc mode values of electronic conductivity were by 2–3 orders of magnitude less than ionic conductivity. The obtained materials were tested as cathodes in sodium-ion battery cells versus sodium metal. The discharge capacity of NNMO was 160 mAh g−1 and 86 mAh g−1 in the potential range of 1.5–4.0 V and 2.3–4.0 V, respectively (20 mA g−1). The work carried out a geoecological assessment of the acoustic pollution of the city of Shelekhov, based on the zoning of the territory according to the noise level. Field surveys were carried out and the number of vehicles registered on the city's road network was calculated. With the help of sound level meters, instrumental measurements were made in the daytime and at night, and the daily course of the distribution of the noise level on the main and secondary roads was plotted. In order to identify the propagation range of noise generated by motor vehicles, the sound pressure levels in the city were calculated for daytime and nighttime using the Ecologist Noise program. The results obtained were compared with the established sanitary standards. The result of acoustic monitoring was the creation of maps of the sound pressure level of the territory. Based on noise pollution maps, a set of measures is proposed to prevent and reduce noise impact on the urban environment.

The article considers the question of narrative unity of «canonical» classic autobiography of Austrian literature of the 20th century. T. Bernhard presented tales «An Indication of the Cause» (Die Ursache: Eine Andeutung, 1975), «The Cellar» (Der Keller: Eine Entziehung, 1976), «The Breath» (Der Atem: Eine Entscheidung 1978), «The Cold» (Die K lte: Eine Isolation, 1981) and «A Child» (Ein Kind, 1982). Fictional autobiography is nonuniqueness in narrative terms. The manner of narration in the novel «A Child», dedicated to early childhood of the writer, is significantly different from the manner of the narrative of the first four novels based on the events of his youth. The first four novels of the pentolog share a common plot scheme and the availability of copyright of meta comment where in rhetorical manner the author talks about the truth and falsehood in an autobiographical narrative. The fifth story does not fit the scheme of the plot of the first four, is devoid of metanarrative level. Reflection of the author of the autobiographical narrative moves to plot level (a fictional episode of the bike ride, for which can be traced several references), change the nature of the characters and the narrative style: the story of his own life events conducted in more direct manner, different from the stylized narrative of the first stories.

This paper is devoted to the study of the construction and application of an artificial neural network for calculating the doses of fertilizer application at the planned yield level in the conditions of grey forest soils of the Republic of Tatarstan. When using mineral fertilizers for crops, it is crucial to comply with the norms and doses of the introduced substances. An overabundance of fertilizer leads to residual accumulation in the soil. Lack of quantity of applied doses affects the quality of the crop, as well as the fertility and ecological situation of agricultural lands. High modern technology and information tools allow solving this problem.

The precise balance of Th1, Th2, and Th17 cytokines is a key factor in successful pregnancy and normal embryonic development. However, to date, not all humoral factors that regulate and influence physiological pregnancy have been completely studied. Our data here pointed out cyclophilin A (CypA) as the adverse pro-inflammatory factor negatively affecting fetal development and associated with pregnancy complications. In different mouse models in vivo, we demonstrated dramatic embryotoxicity and teratogenicity of increased CypA levels during pregnancy. Using generated transgenic models, we showed that CypA overexpression in fetal tissues induced the death of all transgenic fetuses and complete miscarriage. Administration of recombinant human CypA in a high dose to pregnant females during fetal organogenesis (6.5–11.5 dpc) exhibited teratogenic effects, causing severe defects in the brain and bone development that could lead to malformations and postnatal behavioral and cognitive disorders in the offspring. Embryotoxic and teratogenic effects could be mediated by CypA-induced up-regulation of M1 macrophage polarization via activation of the STAT1/3 signaling pathways. Here, we propose secreted CypA as a novel marker of complicated pregnancy and a therapeutic target for the correction of pregnancy complications. [ABSTRACT FROM AUTHOR]
