10995/27938 Мы изучили степень нарушенности луговых ценозов лесопарковой зоны г. Екатеринбурга и национального парка «Оленьи ручьи», на которых произрастает Одуванчик лекарственный – Taraxacumofficinale. Это позволит определить, где можно организовать заготовку этих растений для животных, сбор сырья для медицинских и фармацевтических целей с учётом применения нанотехнологий. Нанокапсулы сухого экстракта Одуванчика лекарственного получают в промышленности, медицине и фармацевтике методом осаждения нерастворителем с применением бутилхлорида в качестве осадителя. Abstract

10995/27938 Косаревский ручей является правым притоком реки Исети, протекает на территории Нижнеисетского лесопарка. Сохранение видового разнообразия на Среднем Урале невозможно без всестороннего изучения флоры и фауны. Этот ручей мало изучен, но изучение ручья позволит выделить бассейн реки Исети, и это важно для его использования жителями микрорайонов Химмаш и Нижнеисетск, для создания мест отдыха детей и подростков города Екатеринбурга. Поэтому целью нашей работы является – изучение, определение географического положения Косаревского ручья, расположенного на территории Нижнеисетского лесопарка, составление комплексной характеристики ручья и выявление ее живописной привлекательности для создания детского познавательно-рекреационного центра. Kosarevsky creek is the right tributary of the Iseti River, which flows in the territory of the Lower Iset Forest Park. Preservation of species diversity in the Middle Urals is impossible without a comprehensive study of flora and fauna. This stream is little studied, but the study of the stream will allow to distinguish the basin of the river Iseti, and it is important for its use by residents of the districts of Himmash and Nizhny setsetsk, to create recreational places for children and adolescents of the city of Yekaterinburg. Therefore, the purpose of our work is to study, determine the geographical location of the Kosarevsky creek, located on the territory of the Nizhny Iset Forest Park, to compile a comprehensive characteristic of the stream and to identify its picturesque attractiveness for the creation of a children’s educational and recreational center.

Abstract

We conducted an international study of idiopathic pulmonary fibrosis (IPF) diagnosis among a large group of physicians and compared their diagnostic performance to a panel of IPF experts. A total of 1141 respiratory physicians and 34 IPF experts participated. Participants evaluated 60 cases of interstitial lung disease (ILD) without interdisciplinary consultation. Diagnostic agreement was measured using the weighted kappa coefficient (κw). Prognostic discrimination between IPF and other ILDs was used to validate diagnostic accuracy for first-choice diagnoses of IPF and were compared using the C-index. A total of 404 physicians completed the study. Agreement for IPF diagnosis was higher among expert physicians (κw=0.65, IQR 0.53–0.72, p20 years of experience (C-index=0.72, IQR 0.0–0.73, p=0.229) and non-university hospital physicians with more than 20 years of experience, attending weekly MDT meetings (C-index=0.72, IQR 0.70–0.72, p=0.052), did not differ significantly (p=0.229 and p=0.052 respectively) from the expert panel (C-index=0.74 IQR 0.72–0.75). Experienced respiratory physicians at university-based institutions diagnose IPF with similar prognostic accuracy to IPF experts. Abstract

The aim of the work is to develop criteria for predicting the functional outcome of the acute period of recurrent cerebral ischemic hemispheric stroke (RCIHS) based on a comprehensive clinical and paraclinical examinations.Materials and methods. An open, comparative, prospective, cohort study of 111 patients (49 (44.1 %) women and 62 (55.9 %) men, age 72.0 (64.0; 77.0) years) with acute RCIHS was performed. All patients underwent a detailed clinical and neurological examination using National Institutes of Health Stroke Scale, mRS (modifid Rankin scale), computed tomography of the brain,hematological and biochemical investigation.Results. Based on a univariate logistic regression analysis, it was found that the functional outcome of the acute RCIHS isinflenced by the following parameters: age of patient (OR 1.06, 95 % CI 1.00–1.11, P = 0.0284), contralateral localization of RCIHS (OR 2.86 95 % CI 1.05–7.79, P = 0.0391), the baseline NIHSS score (OR 1.76. 95 % CI 1.38–2.26; P < 0. 0001), the infarct volume (OR 1.03, 95 % CI 1.00–1.05, P = 0.0221). It was determined that localization of RCIHS in the contralateral hemisphere was associated with the unfavorable functional outcome on the 21 st day of the disease (χ² = 4.44, P = 0.0352). The independent inflence of the initial level of glycemia on the functional outcome of the acute period of RCIHS in patients without diabetes (OR 2.39 95 % CI 1.32–4.33, P = 0.0042) was revealed.Conclusions. The independent predictor of the unfavorable functional outcome of the acute period of the RCIHS (mRS ≥3 at 21st day of disease) was score by NIHSS >10 at baseline (sensitivity 73.3 %, specifiity 90.5 %). The mathematical model, which included the baseline levels of neurological defiit was developed and it could determine the prognosis of the functional outcome of the acute period of RCIHS with an accuracy of 87.5 % (AUC = 0.92, P ˂ 0.05)., Цель работы – разработка критериев прогнозирования функционального исхода острого периода повторного мозговогоишемического полушарного инсульта (ПМИПИ) на основании комплексного клинико-параклинического исследованияМатериалы и методы. Незалежним предиктором несприятливого функціонального результату гострого періоду ПМІПІ (mRS ≥3 на 21 добу) є значення сумарного бала за NIHSS >10 на 1 добу захворювання (чутливість 73,3 %, специфічність 90,5 %). Abstract

Objective: to assess the favorable and unfavorable types of lung tissue fibrotic changes in patients with interstitial lung diseases (ILD) detected by high-resolution computed tomography (HRCT).Material and methods. The results of examinations were analyzed in 385 patients: 181 with respiratory organ sarcoidosis, 130 with fibrosing alveolitis, 36 with histiocytosis X, and 38 with lymphangioleiomyomatosis. All the patients underwent HRCT; the data were compared with the results of comprehensive functional study of external respiration (CFSER), histological examination (in 70.1%), and the pattern of the disease.Results. Comparison of the clinical and functional course of ILD with the types of lung tissue fibrotic changes detected by HRCT and morphological examination showed that the favorable types of pulmonary fibrosis included stringy central and peripheral interstitial fibrotic changes and the atelectatic type of fibrosis, the occurrence of which failed to affect the development of obvious perfusion and diffusion disorders and to give rise to respiratory failure. The unfavorable types of pulmonary fibrosis included the peripheral pulmonary interstitial fibrotic changes (acinar fibrosis, honeycomb lung), which led to restrictive changes and perfusion disorders, which were accompanied by significant respiratory failure, decreases in quality of life and survival, as well as fibrotic changes in the walls of long-lasting air-containing cysts and a fibrotic cavity mass that resulted in complications (pulmonary hemorrhage, pneumothorax, and pneumomediastinum).

35. Alien Species in the Flora of the Middle Volga Region

Abstract

The article presents information on the diversity and structure of the alien flora of the Middle Volga region on the border between the steppe and forest-steppe zones of the European botanical and geographical province. In the studied area, we identified 440 alien species of plants (24.6% of the total floristic diversity), including 59 invasive and potentially invasive species, the dispersal of which has already caused environmental consequences or could become a threat in the future. According to the degree of naturalization, alien plant species among the regional flora are represented by 146 ephemerophytes (33.2%), 122 colonophytes (27.7%), 151 epecophytes (34.3%) and 21 agriophytes (4.8%). As verified via the main method of skidding, alien species are distributed almost equally – 228 xenophytes (51.8%) and 212 ergasiophygophytes (48.2%). It was revealed that of the 150 species of alien plants, there are only 84 of the most common European species in the Middle Volga region, most of which do not show high cenotic activity. The obtained results actualize the development of a system of ecological and economic assessment of the consequences of introducing alien plant species into regional flora.

Abstract

4 locations of a rare endemic species Cephalaria litvinovii Bobr. (Dipsacaceae) are known in the Penza region at the present time. It occurs in the Kolyshlei, Mokshan and Maloserdobinsky districts of the Penza region. Three finds were made in the Volga basin, one in the Don basin. Flora and vegetation of each of these sites were analyzed, which allowed to clarify the ecological features of the species. With a view of protection in the Penza region it is recommended to create two new specially protected natural territories: Mayskoye in Maloserdobinsky («May Reserve of Cephalaria litvinovii») and near the station Scriabin in the Kolyshleysky Districts («Scriabin's Reserve Cephalaria litvinovii»).

Abstract

Information on the structure and dynamics of the zonal Central Russian meadow steppes is presented in terms of the example of the Privolzhskaya forest-steppe state nature reserve (Penza region, Russia). The cenotic diversity of the herbaceous vegetation of the reserve is represented by 91 associations, of which 48 refer to steppes and 43 to steppe meadows. It has been revealed that the vegetation of the studied placer sites under the influence of an absolutely reserved regime undergoes mesophyticization and silvatization, which are more intensively manifested in the forest-steppe landscapes of the secondary moraine plains (areas of the Poperechenskaya steppe and Ostrovtsovskaya forest-steppe reserve) than in the eroded denudational plains of forest-steppe landscapes (areas of the Kuncherovskaya steppe and Ostrovtsovskaya forest-steppe reserves). The study of the dynamics of zonal meadow steppes in the Volga Uplands testifies to the inevitability of their transformation in modern climatic conditions into meadow, shrubby and even forest communities.

49. Clinical and brain bioelectric features in patients with acute recurrent cerebral hemispheric ischemic stroke depending on acute disease period outcome

Abstract

Objective — to study clinical‑neurophysiological correlations in patients with recurrent cerebral hemispheric ischemic stroke (RCHIS) depending on the outcome of the acute period of the disease.Methods and subjects. Complex clinical‑neurological and computer‑electroencephalographic examination of 41 patients (22 (53.6 %) men and 19 (46.4 %) women, mean age 71 (68 — 72)) with acute RCHIS was performed. All patients underwent neurological examination at the first day of the disease by National Institute of Health Stroke Scale (NIHSS) and by Modified Rankin Scale (mRS) at 21st day. The computed tomography as well as electroencephalography of the brain was performed at baseline.Results. The patient did not differ in gender structure, lateralization and localization of RCHIS. The tendency of higher values of the relative spectral power of d‑rhythm, q‑rhythm, DAR and TAR ratio in the affected hemisphere among all cohort of patients was revealed. It was established that patients with an unfavorable outcome of the acute RCHIS at onset of the disease were characterized by higher levels of relative spectral power of d‑rhythm in both hemispheres of the brain. Patients with favorable functional outcome of RCHIS are characterized by dominance of blo‑rhythm and b‑rhythm in the affected hemisphere as well as by dominance of alo‑rhythm and a‑rhythm in the intact hemisphere.Conclusions. It was found that functional outcome of acute RCHIS depend on the following parameters: absolute spectral power of d‑rhythm, relative spectral power of d‑rhythm, relative spectral power of q‑rhythm in affected hemisphere and relative spectral power d‑rhythm in the intact hemisphere and as well as DAR ration in affected and intact hemispheres., Цель — изучить клинико‑нейрофизиологические корреляции у больных повторным мозговым полушарным ишемическим инсультом (ПМПИИ) в зависимости от исхода в острый период заболевания.Материалы и методы. Проведено комплексное клинико‑неврологическое и компьютерно‑электроэнцефалографическое исследование 41 пациента (22 (53,6 %) мужчины и 19 (46,4 %) женщин, средний возраст — 71 (68 — 72) год) в острый период ПМПИИ. Всем пациентам выполнено неврологическое обследование в 1‑е сутки заболевания с использованием NIHSS (National Institute of Health Stroke Scale) и модифицированной шкалы Ренкина на 21‑е сутки, компьютерная томография и электроэнцефалография головного мозга.Результаты. Группы пациентов с благоприятным и неблагоприятным исходом острого периода ПМПИИ статистически значимо не отличались по гендерной структуре, латерализации и локализации ПМПИИ. Выявлена тенденция к более высоким значениям относительной спектральной мощности (ОСМ) d‑ритма и q‑ритма и коэффициентов DАR и TAR в пораженном полушарии в общей когорте больных. Установлено, что больные с неблагоприятным исходом острого периода ПМПИИ в дебюте заболевания имели более высокие значения ОСМ d‑ритма в обоих полушариях головного мозга, тогда как для больных с благоприятным исходом ПМПИИ более характерным было преобладание в пораженном полушарии ОСМ blo‑ритма и b‑ритма, а в интактном — ОСМ alo‑ритма и a‑ритма.Выводы. На функциональный исход острого периода ПМПИИ влияла величина абсолютной спектральной мощности d‑ритма в пораженном полушарии, ОСМ d‑ритма в пораженном полушарии, ОСМ d‑ритма в интактном полушарии, коэффициента DАR в пораженном и интактном полушарии., Мета — вивчити клініко‑нейрофізіологічні кореляції у хворих із повторним мозковим півкульовим ішемічним інсультом (ПМПІІ) залежно від наслідку в гострий період захворювання.Матеріали і методи. Проведено комплексне клініко‑неврологічне і комп’ютерно‑електро­енцефало­графічне дослідження 41 пацієнта (22 (53,6 %) чоловіки і 19 (46,4 %) жінок, середній вік — 71 (68 — 72) рік) у гострий період ПМПІІ. Виявлено тенденцію до вищих значень відносної спектральної потужності (ВСП) d‑ритму і q‑ритму та коефіцієнтів DАR і TAR в ураженій півкулі у загальній когорті хворих. Установлено, що хворі з несприятливим наслідком гострого періоду ПМПІІ в дебюті захворювання мали більші значення ВСП d‑ритму в обох півкулях головного мозку, тоді як для хворих зі сприятливим наслідком ПМПІІ характернішим було переважання в ураженій півкулі ВСП blo‑ритму та b‑ритму, а в інтактній — ВСП alo‑ритму та a‑ритму.Висновки. На функціональний наслідок гострого періоду ПМПІІ впливала величина абсолютної спектральної потужності d‑ритму в ураженій півкулі, ВСП d‑ритму в ураженій півкулі, ВСП d‑ритму в інтактній півкулі, коефіцієнт DАR в ураженій та інтактній півкулі.

50. Optimization of treatment for patients with recurrent cerebral hemispheric ischemic stroke in an acute period

Abstract

Objective — to optimize treatment in patients with recurrent cerebral hemispheric ischemic stroke (RCHIS) by comparing the effectiveness of mononeuroprotective, combined neuroprotective and neuro­metabolic as well as basic therapy in the dynamics of an acute period of the disease.Methods and subjects. An open, comparative, cohort, prospective study that included 86 patients aged 50 to 80 years with RCHIS was conducted. All patients were randomized by sex, age, stroke subtype, lesion focus and concomitant pathology. The first observation group included 41 patients who received citicoline in addition to basic therapy, the second group consisted of 26 patients taking combination of citicoline and Actovegin drugs, and the third group consisted of 19 patients who received only basic therapy (antiplatelet, hypotensive drugs, anticoagulants, statin drugs and symptomatic therapy according to indications). On the 1st and on the 13th day of the acute period, all patients underwent a detailed study of the clinical­neurological status with the assessment of the severity of stroke by stroke scale NIH (National Institutes of Health). Cognitive profile was evaluated by scales: MMSE (Mini­Mental State Examination, MoCA (Montreal Cognitive Assessment) and FAB (Frontal Assessment Battery), On the 21st day of the acute period of RCHIS, the degree of disability was assessed according to the modified Rankin scale (mRS).Results. In the group of patients taking combination therapy with citicoline and Actovegin was observed a statistically significant difference in the mean score on the NIH, MoCA and MMSE scales in comparison with patients taking mononeuroprotective and basic therapy. The average score by mRS in the patients of the second observation group was 2.27 ± 0.15, which associated with favorable functional outcome of the acute period of the disease.Conclusions. В первой группе (n = 41) пациенты получали получали дополнительно к базисной терапии цитиколин, во второй группе (n = 26) — комбинацию препаратов цитиколин и Актовегин, в третьей группе (n = 19) — только базисную терапию (антиагреганты, гипотензивные препараты, антикоагулянты, липидоснижающие препараты, симптоматическая терапия по показаниям). В 1-е и на 13-е сутки острого периода ПМПИИ у всех пациентов проводили детальное изучение клинико-неврологического статуса с оценкой степени тяжести инсульта по шкале инсульта NIH (National Institutes of Health) и когнитивного профиля с помощью шкал MMSE (Mini-Mental State Examination), MoCA (Montreal Cognitive Assessment) и FAB (Frontal Assessment Battery). На 21-е сутки острого периода оценивали степень инвалидизации по модифицированной шкале Рэнкина.Результаты. У пациентов 2-й группы отмечены статистически значимые различия среднего суммарного балла по шкале инсульта NIH, шкалам МоСА и ММSE по сравнению с пациентами 1-й и 3-й групп. Средний балл по модифицированной шкале Рэнкина у пациентов 2-й группы составил 2,27 ± 0,15, что соответствовало благоприятному функциональному исходу острого периода заболевания.Выводы. Комбинированное действие препаратов цитиколин и Актовегин способствовало более активному регрессу неврологического и когнитивного дефицита, улучшению клинико­социального исхода острого периода заболевания и доказало свое превосходство над мононейропротекторной и базисной терапие, Мета — оптимізувати лікувальні заходи у пацієнтів з повторним мозковим півкульним ішемічним інсультом (ПМПІІ) шляхом порівняння ефективності мононейропротекторної, комбінованої нейропротекторної, нейрометаболічної та базисної терапії в динаміці гострого періоду захворювання.Матеріали і методи. На 21-шу добу оцінювали ступінь інвалідизації за модифікованою шкалою Ренкіна.Результати. У пацієнтів 2-ї групи зафіксовано статистично значущу різницю середнього сумарного бала за шкалою інсульту NIH, за шкалами МоСА і ММSE порівняно з пацієнтами 1-ї та 3-ї груп. Середній бал за модифікованою шкалою Ренкіна у пацієнтів 2-ї групи становив 2,27 ± 0,15, що відповідало сприятливому функціональному наслідку гострого періоду захворювання.Висновки. Комбінована дія препаратів цитиколін і Актовегін сприяла активнішому регресу неврологічного та когнітивного дефіциту і поліпшенню клініко­соціального наслідку гострого періоду захворювання та довела свою перевагу перед мононейропротекторною і базисною терапією.
