Introduction: The evaluation of peripheral perfusion is essential for the management of diabetic patients, in order to detect the risks of amputations and the evolution of wound healing.Objective: To identify the etiological factors and clinical manifestations that characterize peripheral vascular impairment in diabetic patients.Methodology: An integrative review was carried out in May 2019 using the following databases BDENF, CINAHL, LILACS, MEDLINE and PUBMED with the following descriptors in English and Portuguese: diabetic angiopathies, diabetes mellitus, diabetic foot. The sample consisted of 18 articles. The most frequent etiological factors were Peripheral Arterial Disease (PAD), duration of diabetes, peripheral neuropathy, advanced age and smoking. Regarding clinical manifestations, they presented a higher frequency of low Ankle-brachial Index (ITB), low Halluxbrachial Index (IHB), absence of peripheral pulses and delayed healing.Conclusion: PAD was more evident and considered the main cause of vascular involvement, it was found that ABI is not a safe technique to assess peripheral perfusion in diabetic patients and it must be performed in conjunction with IHB, the most reliable alternative. Therefore, it is essential for health professionals to track the etiological factors and clinical manifestations in order to provide targeted care to this at-risk population. Introdução: A avaliação da perfusão periférica é fundamental para o manejo de pacientes diabéticos, com o intuito de detectar os riscos de amputações e a evolução da cicatrização de feridas.Objetivo: Identificar os fatores etiológicos e manifestações clínicas que caracterizam o comprometimento vascular periférico em pacientes diabéticos.Metodologia: Desenvolveu-se uma revisão integrativa em maio de 2019 utilizando as seguintes bases de dados BDENF, CINAHL, LILACS, MEDLINE e PUBMED com os descritores angiopatias diabéticas, diabetes mellitus, pé diabético nos idiomas inglês e português. A amostra foi constituída por 18 artigos. Os fatores etiológicos mais frequentes foram Doença arterial periférica (DAP), tempo de evolução do diabetes, neuropatia periférica, idade avançada e tabagismo. Sobre as manifestações clínicas, apresentaram maior frequência baixo Índice Tornozelo-braquial (ITB), baixo Índice Hálux-braquial (IHB), ausência de pulsos periféricos e retardo na cicatrização.Conclusão: A DAP foi mais evidente e considerada a principal causa de acometimento vascular, verificou-se que o ITB não é uma técnica segura para avaliar a perfusão periférica em pacientes diabéticos e esta deve ser realizada em concomitância ao IHB, alternativa mais confiável. Portanto, é fundamental os profissionais de saúde rastrearem os fatores etiológicos e manifestações clínicas para realizarem uma atenção direcionada para essa população de risco.
