The adsorption process of inorganic arsenic (As) plays an important role in its mobility, bioavailability, and toxicity in the river environment. In this work, the adsorption of dissolved arsenite (As(III)) and arsenate (As(V)) by microplastics (MPs) pellets (polystyrene (PS) and low-density polyethylene (LDPE)), river sediment, and their mixture were investigated to assess the adsorption affinities and mechanism. The adsorption kinetics showed slow and mild rising zones from the natural behavior of the chemical adsorption. The results indicated that both MP characteristics and water properties played a significant role in the adsorption behavior of inorganic As species. The As adsorption equilibrium was modeled well by both Langmuir and Freundlich isotherms and partly fitted with the Sips model suggesting that both mono-layer and multi-layer adsorption occurred during adsorption The spontaneous adsorption process for both As(III) and As(V) was evidenced by the adsorption thermodynamics. The maximum adsorption capacities of As(III) and As(V) reached 143.3 mg/kg and 109.8 mg/kg on PS in deionized water, which were higher than those on sediment-PS mixture (119.3 mg/kg, 99.2 mg/kg), which were all lower than on sediment alone (263.3 mg/kg, 398.7 mg/kg). The Fourier transform infrared spectroscopy analysis identified that As(III) and As(V) interaction with sediment surface functional groups was the main adsorption mechanism from surface complexation and coordination. Two functional groups of polystyrene (-NH2, -OH) were mainly involved in the adsorption of inorganic As species on PS, while -COO- and -OH functional groups contributed to the adsorption mechanism of inorganic As species on LDPE. This letter proposes improved pilot designs to enhance the connectivity of multicarrier massive MIMO systems. Exploiting the fact that orthogonal frequency-division multiplexing (OFDM) training symbols end with zero samples, the proposed pilot designs reduce pilot overhead by truncating the zero samples and/or intentionally using them for a guard interval without affecting the channel estimation quality. The proposed designs allow a massive MIMO system either to serve more users with a fixed amount of training time, or requires less training time for a fixed number of users. Numerical results obtained for a cell-free massive MIMO system (as an application use case) demonstrate significant improvements in connectivity and spectral efficiency by the proposed designs over the conventional designs.

This paper studies the uplink (UL) transmission of a cell-free massive multiple-input multiple-output (massive MIMO) system in which multiple base stations (or access points), each equipped with a multiple-antenna array and connected to a central processing unit (CPU) over a backhaul network, simultaneously serve multiple users in a cell-free service area. The paper focuses on the non-orthogonal multiple access (NOMA) approach for sharing pilot sequences among users. Unlike the conventional cell-free massive MIMO-NOMA systems in which the UL signals from different access points are equally combined over the backhaul network, this paper develops an optimal backhaul combining (OBC) method to maximize the UL signal-to-interference-plus-noise ratio (SINR). With the proposed OBC method, a closed-form SINR expression is derived under Rayleigh fading and used to formulate a max-min quality-of-service (QoS) power control problem to further enhance the system performance. Numerical results corroborate the analysis and demonstrate that the proposed optimal backhaul combining method clearly outperforms equal-gain combining.

This paper proposes a novel successive interference cancellation method to enhance the ergodic spectral efficiency of a cell-free massive multiple-input multiple-output (MIMO) system with non-orthogonal multiple-access (NOMA). Unlike the majority of existing research works on performance evaluation of NOMA, which assume perfect channel state information and perfect data detection for successive interference cancellation, we take into account the effect of practical (hence imperfect) successive interference cancellation (SIC). We show that the received signal at the backhaul network of a cell-free massive MIMO-NOMA system can be effectively treated as a signal received over an AWGN channel. As a result, a discrete joint distribution between the interfering signal and its detected version can be analytically found, from which an adaptive SIC scheme is proposed to improve performance of interference cancellation.

The paper focuses on the non-orthogonal multiple access (NOMA) approach for sharing pilot sequences among users. Unlike the conventional cell-free massive MIMO-NOMA systems in which the UL signals from different access points are equally combined over the backhaul network, this paper first develops an optimal backhaul combining (OBC) method to maximize the UL signal-to-interference-plus-noise ratio (SINR). It is shown that, by using OBC, the correlated interference can be effectively mitigated if the number of users assigned to each pilot sequence is less than or equal to the number of base stations (BSs). As a result, the cell-free massive MIMO-NOMA system with OBC can enjoy unlimited performance when the number of antennas at each BS tends to infinity. A closed-form SINR expression is derived under Rayleigh fading and used to formulate a max-min quality-of-service (QoS) power control problem to further enhance the system performance. To deal with the NP-hardness of the concerned optimization problem, a successive inner approximation technique is applied to convert the original problem into a series of convex optimizations, which can be solved iteratively. In addition, a user grouping algorithm is also developed and shown to be better than random user grouping and a grouping method recently proposed in the literature. Numerical results are presented to corroborate the analysis and demonstrate the superiority of the proposed optimal backhaul combining over both equal-gain backhaul combining and zero-forcing backhaul combining.

Over recent years, human autoantibodies targeting myelin oligodendrocyte glycoprotein (MOG Ab) have been associated with monophasic and relapsing central nervous system demyelination involving the optic nerves, spinal cord, and brain. While the clinical relevance of MOG Ab detection is becoming increasingly clear as therapeutic and prognostic differences from multiple sclerosis are acknowledged, an in-depth characterization of human MOG Ab is required to answer key challenges in patient diagnosis, treatment, and prognosis. Herein, we investigated the epitope, binding sensitivity, and affinity of MOG Ab in a cohort of 139 and 148 MOG antibody-seropositive children and adults (n = 287 patients at baseline, 130 longitudinal samples, and 22 cerebrospinal fluid samples). MOG extracellular domain was also immobilized to determine the affinity of MOG Ab. MOG Ab response was of immunoglobulin G1 isotype, and was of peripheral rather than intrathecal origin. High affinity MOG Ab were detected in 15% paediatric and 18% adult sera. More than 75% of paediatric and adult MOG Ab targeted a dominant extracellular antigenic region around Proline42. MOG Ab titers fluctuated over the progression of disease, but affinity and reactivity to Proline42 remained stable. Adults with a relapsing course intrinsically presented with a reduced immunoreactivity to Proline42 and had a more diverse MOG Ab response, a feature that may be harnessed for predicting relapse. Higher titers of MOG Ab were observed in more severe phenotypes and during active disease, supporting the pathogenic role of MOG Ab. Loss of MOG Ab seropositivity was observed upon conformational changes to MOG, and this greatly impacted the sensitivity of the detection of relapsing disorders, largely considered as more severe. Careful consideration of the binding characteristics of autoantigens should be taken into account when detecting disease-relevant autoantibodies.

Investigation into the macromolecular structure-Activity relationship of synthetic antimicrobial polymers has been gaining scientific interest due to the possibility of discovering new alternatives for combating the increase of multidrug resistance in bacteria. Recently, we reported the development of new antimicrobial polymers in the form of amphiphilic ternary copolymers that consist of low-fouling (oligoethylene glycol), cationic and hydrophobic side chains. The combination of these three main functional groups is crucial in endowing the polymers with high antimicrobial potency against Gram-negative pathogens and low cytotoxicity. Following on from our previous study, we herein present a systematic assessment on the effects of the polymer chain length and architecture (i.e., random vs. block copolymers and linear vs. hyperbranched) on the antimicrobial activity and hemocompatibility of antimicrobial ternary copolymers. The polymer chain length in random copolymers slightly affects the antimicrobial activity where longer chains are marginally more bacteriostatic against Pseudomonas aeruginosa and Escherichia coli. In terms of hemocompatibility, polymers with shorter chains are more prone to hemagglutination. Interestingly, when the hydrophilic and hydrophobic segments are separated into diblock copolymers, the antimicrobial activity is lost, possibly due to the stable core-shell architecture. The hyperbranched structure which consists of 2-ethylhexyl groups as hydrophobic side-chains yields the best overall biological properties, having similar antimicrobial activity (MIC = 64 μg mL-1) and >4-fold increase in HC50 compared to the linear random copolymers (HC50 > 10000 μg mL-1) with no hemagglutination. In this study, an antimicrobial platform in the form of nitric oxide (NO) gas-releasing polydopamine (PDA)-coated iron oxide nanoparticles (IONPs) is developed for combating bacterial biofilms. NO is bound to the PDA-coated IONPs via the reaction between NO and the secondary amine moieties on PDA to form N-diazeniumdiolate (NONOate) functionality. To impart colloidal stability to the nanoparticles in aqueous solutions (e.g., phosphate buffered saline (PBS) and bacteria cell culture media M9), a polymer bearing hydrophilic and amine pendant groups, P(OEGMA)-b-P(ABA), is synthesized via reversible addition-fragmentation chain transfer (RAFT) polymerization and is subsequently grafted onto the PDA-coated IONPs by employing the Schiff base/Michael addition reaction between o-quinone and a primary amine. These nanoparticles are able to effectively disperse Pseudomonas aeruginosa biofilms (up to 79% dispersal) at submicromolar NO concentrations. In addition, the nanoparticles demonstrate excellent bactericidal activity toward P. aeruginosa planktonic and biofilm cells (up to 5-log10 reduction).

The nitric oxide (NO) molecule has gained increasing attention in biological applications to combat biofilm-associated bacterial infections. However, limited NO loading, relatively short half-lives of low molecular weight NO donor compounds, and difficulties in targeted delivery of NO have hindered their practical clinical administration. To overcome these drawbacks, the combination of NO and scaffolds based on biocompatible polymers is an effective way towards realizing the practical utility of NO in biomedical applications. In this regard, the present overview highlights the recent developments in NO-releasing polymeric biomaterials for antimicrobial applications, focusing on antibiofilm treatments and the challenges that need to be overcome.

Synthetic polymers have shown promise in combating multidrug-resistant bacteria. However, the biological effects of sequence control in synthetic antimicrobial polymers are currently not well understood. As such, we investigate the antimicrobial effects of monomer distribution within linear high-order quasi-block copolymers consisting of aminoethyl, phenylethyl, and hydroxyethyl acrylamides made in a one-pot synthesis approach via photoinduced electron transfer–reversible addition–fragmentation chain transfer polymerisation (PET-RAFT). Through different combinations of monomer/polymer block order, antimicrobial and haemolytic activities are tuneable in a manner comparable to antimicrobial peptides.

OBJECTIVE: We characterised the clinical course, treatment and outcomes in 59 patients with relapsing myelin oligodendrocyte glycoprotein (MOG) antibody-associated demyelination. METHODS: We evaluated clinical phenotypes, annualised relapse rates (ARR) prior and on immunotherapy and Expanded Disability Status Scale (EDSS), in 218 demyelinating episodes from 33 paediatric and 26 adult patients. RESULTS: The most common initial presentation in the cohort was optic neuritis (ON) in 54% (bilateral (BON) 32%, unilateral (UON) 22%), followed by acute disseminated encephalomyelitis (ADEM) (20%), which occurred exclusively in children. ON was the dominant phenotype (UON 35%, BON 19%) of all clinical episodes. 109/226 (48%) MRIs had no brain lesions. Patients were steroid responsive, but 70% of episodes treated with oral prednisone relapsed, particularly at doses <10 mg daily or within 2 months of cessation. Immunotherapy, including maintenance prednisone (P=0.0004), intravenous immunoglobulin, rituximab and mycophenolate, all reduced median ARRs on-treatment. Treatment failure rates were lower in patients on maintenance steroids (5%) compared with non-steroidal maintenance immunotherapy (38%) (P=0.016). 58% of patients experienced residual disability (average follow-up 61 months, visual loss in 24%). Patients with ON were less likely to have sustained disability defined by a final EDSS of ≥2 (OR 0.15, P=0.032), while those who had any myelitis were more likely to have sustained residual deficits (OR 3.56, P=0.077). CONCLUSION: Relapsing MOG antibody-associated demyelination is strongly associated with ON across all age groups and ADEM in children. Patients are highly responsive to steroids, but vulnerable to relapse on steroid reduction and cessation.

Infections caused by multidrug-resistant bacteria are on the rise and, therefore, new antimicrobial agents are required to prevent the onset of a postantibiotic era. In this study, we develop new antimicrobial compounds in the form of single-chain polymeric nanoparticles (SCPNs) that exhibit excellent antimicrobial activity against Gram-negative bacteria (e.g., Pseudomonas aeruginosa) at micromolar concentrations (e.g., 1.4 μM) and remarkably kill ≥99.99% of both planktonic cells and biofilm within an hour. Linear random copolymers, which comprise oligoethylene glycol (OEG), hydrophobic, and amine groups, undergo self-folding in aqueous systems due to intramolecular hydrophobic interactions to yield these SCPNs. By systematically varying the hydrophobicity of the polymer, we can tune the extent of cell membrane wall disruption, which in turn governs the antimicrobial activity and rate of resistance acquisition in bacteria. We also show that the incorporation of OEG groups into the polymer design is essential in preventing complexation with proteins in biological medium, thereby maintaining the antimicrobial efficacy of the compound even in in vivo mimicking conditions. In comparison to the last-resort antibiotic colistin, our lead agents have a higher therapeutic index (by ca. 2-3 times) and hence better biocompatibility. We believe that the SCPNs developed here have potential for clinical applications and the information pertaining to their structure-activity relationship will be valuable toward the general design of synthetic antimicrobial (macro)molecules.

The rise of hospital-acquired infections, also known as nosocomial infections, is a growing concern in intensive healthcare, causing the death of hundreds of thousands of patients and costing billions of dollars worldwide every year. In addition, a decrease in the effectiveness of antibiotics caused by the emergence of drug resistance in pathogens living in biofilm communities poses a significant threat to our health system. The development of new therapeutic agents is urgently needed to overcome this challenge. We have developed new dual action polymeric nanoparticles capable of storing nitric oxide, which can provoke dispersal of biofilms into an antibiotic susceptible planktonic form, together with the aminoglycoside gentamicin, capable of killing the bacteria. The novelty of this work lies in the attachment of NO-releasing moiety to an existing clinically used drug, gentamicin. The nanoparticles were found to release both agents simultaneously and demonstrated synergistic effects, reducing the viability of Pseudomonas aeruginosa biofilm and planktonic cultures by more than 90% and 95%, respectively, while treatments with antibiotic or nitric oxide alone resulted in less than 20% decrease in biofilm viability.

The dispersal phase that completes the biofilm lifecycle is of particular interest for its potential to remove recalcitrant, antimicrobial tolerant biofilm infections. Here we found that temperature is a cue for biofilm dispersal and a rise by 5 °C or more can induce the detachment of Pseudomonas aeruginosa biofilms. Temperature upshifts were found to decrease biofilm biomass and increase the number of viable freely suspended cells. The dispersal response appeared to involve the secondary messenger cyclic di-GMP, which is central to a genetic network governing motile to sessile transitions in bacteria. Furthermore, we used poly((oligo(ethylene glycol) methyl ether acrylate)-block-poly(monoacryloxy ethyl phosphate)-stabilized iron oxide nanoparticles (POEGA-b-PMAEP@IONPs) to induce local hyperthermia in established biofilms upon exposure to a magnetic field. POEGA-b-PMAEP@IONPs were non-toxic to bacteria and when heated induced the detachment of biofilm cells. Finally, combined treatments of POEGA-b-PMAEP@IONPs and the antibiotic gentamicin reduced by 2-log the number of colony-forming units in both biofilm and planktonic phases after 20 min, which represent a 3.2- and 4.1-fold increase in the efficacy against planktonic and biofilm cells, respectively, compared to gentamicin alone. The use of iron oxide nanoparticles to disperse biofilms may find broad applications across a range of clinical and industrial settings.

BACKGROUND: Better understanding of the trends and disparities in health at old age in terms of life expectancy will help to provide appropriate responses to the growing needs of health and social care for the older population in the context of limited resources. As a result of rapid economic, demographic and epidemiological changes, the number of people aged 60 and over in Vietnam is increasing rapidly, from 6.7% in 1979 to 9.2% in 2006. Life expectancy at birth has increased but not much are known about changes in old ages. This study assesses the trends and socioeconomic inequalities in RLE at age 60 in a rural area in an effort to highlight this vulnerable group and to anticipate their future health and social needs. METHODS: An abridged life table adjusted for small area data was used to estimate cohort life expectancies at old age and the corresponding 95% confidence intervals from longitudinal data collected by FilaBavi DSS during 1999-2006, which covered 7,668 people at age 60+ with 43,272 person-years, out of a total of 64,053 people with 388,278 person-years. Differences in life expectancy were examined according to socioeconomic factors, including socio-demographic characteristics, wealth, poverty and living arrangements. RESULTS: Life expectancies at age 60 have increased by approximately one year from the period 1999-2002 to 2003-2006. The increases are observed in both sexes, but are significant among females and relate to improvements among those who belong to the middle and upper household wealth quintiles. However, life expectancy tends to decrease in the most vulnerable groups. There is a wide gap in life expectancy according to poverty status and living arrangements, and the gap by poverty status has widened over the study period. The gender gap in life expectancy is consistent across all socioeconomic groups and tends to be wider amongst the more disadvantaged population. Population ageing is rapidly becoming a global issue and will have a major impact on health policies and programmes. The World Health Organization's Study on global AGEing and adult health (SAGE) aims to address the gap in reliable data and scientific knowledge on ageing and health in low- and middle-income countries. SAGE is a longitudinal study with nationally representative samples of persons aged 50+ years in China, Ghana, India, Mexico, Russia and South Africa, with a smaller sample of adults aged 18-49 years in each country for comparisons. Instruments are compatible with other large high-income country longitudinal ageing studies. Wave 1 was conducted during 2007-2010 and included a total of 34 124 respondents aged 50+ and 8340 aged 18-49. In four countries, a subsample consisting of 8160 respondents participated in Wave 1 and the 2002/04 World Health Survey (referred to as SAGE Wave 0). Wave 2 data collection will start in 2012/13, following up all Wave 1 respondents. Wave 3 is planned for 2014/15.
