1. 2D Skyrmion topological charge of spin textures with arbitrary boundary conditions: a two-component spinorial BEC as a case study

Abstract

We derive the most general expression for the Skyrmion topological charge for a two-dimensional spin texture, valid for any type of boundary conditions or for any arbitrary spatial region within the texture. It reduces to the usual one $Q = 1/4\pi \iint \vec f \cdot \left(\partial_x \vec f \times \partial_y \vec f\right)$ for the appropriate boundary conditions, with $\vec f$ the spin texture. The general expression resembles the Gauss-Bonet theorem for the Euler-Poincar\'e characteristic of a 2D surface, but it has definite differences, responsible for the assignment of the proper signs of the Skyrmion charges. Additionally, we show that the charge of a single Skyrmion is the product of the value of the normal component of the spin texture at the singularity times the Index or winding number of the transverse texture, a generalization of a Poincar\'e theorem. We illustrate our general results analyzing in detail a two-component spinor Bose-Einstein condensate in 2D in the presence of an external magnetic field, via the Gross-Pitaevskii equation. The condensate spin textures present Skyrmions singularities at the spatial locations where the transverse magnetic field vanishes. We show that the ensuing superfluid vortices and Skyrmions have the same value for their corresponding topological charges, in turn due to the structure of the magnetic field., Comment: 32 pages, 15 figures

Abstract

We show that the long-distance behavior of the two-body density correlation functions and the Cooper-pair probability density of a balanced mixture of a two-component Fermi gas at $T = 0$, is universal along the BEC-BCS crossover. Our result is demonstrated by numerically solving the mean-field BCS model for different finite short-range atomic interaction potentials. We find an analytic expression for the correlation length in terms of the chemical potential and the energy gap at zero momentum., Comment: 6 pages, 3 figures

Abstract

We study the onset of dissipation in an atomic Josephson junction between Fermi superfluids in the molecular Bose-Einstein condensation limit of strong attraction. Our simulations identify the critical population imbalance and the maximum Josephson current delimiting dissipationless and dissipative transport, in quantitative agreement with recent experiments. We unambiguously link dissipation to vortex ring nucleation and dynamics, demonstrating that quantum phase slips are responsible for the observed resistive current. Our work directly connects microscopic features with macroscopic dissipative transport, providing a comprehensive description of vortex ring dynamics in three-dimensional inhomogeneous constricted superfluids at zero and finite temperatures., Comment: To be published in PRL

5. Coherent and dissipative transport in a Josephson junction between fermionic superfluids of $^6$Li atoms

Abstract

Quantum systems out of equilibrium offer the possibility of understanding intriguing and challenging problems in modern physics. Studying transport properties is not only valuable to unveil fundamental properties of quantum matter but it is also an excellent tool for developing new quantum devices which inherently employ quantum-mechanical effects. In this contribution, we present our experimental studies on quantum transport using ultracold Fermi gases of $^6$Li atoms. We realize the analogous of a Josephson junction by bisecting fermionic superfluids by a thin optical barrier. We observe coherent dynamics in both the population and in the relative phase between the two reservoirs. For critical parameters, the superfluid dynamics exhibits both coherent and resistive flow due to phase-slippage events manifesting as vortices propagating into the bulk. We uncover also a regime of strong dissipation where the junction operation is irreversibly affected by vortex proliferation. Abstract

La ricerca si propone di indagare la possibile funzione educativa dei materiali audiovisivi d’animazione e, al contempo, visto il loro ruolo attivo nel produrre significati e percezioni ambientali, incentivarne l’uso come strumento educativo-didattico. I presupposti teorici da cui si muove la ricerca sono quelli dell’ecocriticism (Glotfelty, 1996), linea di pensiero che dimostra come i prodotti culturali di massa (Garrard, 2010; dell’Agnese, 2011) hanno un ruolo attivo nella produzione di idee sull’ambiente e conseguentemente nel condizionare i comportamenti umani nei confronti dello stesso. La crisi ambientale attuale impone un nuovo atteggiamento nei confronti dell’ambiente che ci circonda, richiede dunque un’azione di educazione ambientale e di promozione della consapevolezza di come le idee sull’ambiente derivino anche dai prodotti culturali di massa. La ricerca è stata costruita nell’ambito del PRIN Greening the Visual: an Environmental Atlas of Italian Landscape (2017-0422), che si propone di indagare il modo in cui le rappresentazioni visuali influenzano le idee ambientali della comunità andando a costruire un atlante visuale on-line dei paesaggi italiani. Il progetto Prin in questione ha coinvolto tre unità di lavoro, in particolare l’unità di ricerca dell’Università degli Studi di Milano-Bicocca, a cui afferisce il suddetto progetto di dottorato, si è dedicata all’analisi delle rappresentazioni relative al paesaggio costiero e marino. I materiali d’animazione sui quali si è lavorato hanno dunque riguardato lo spazio pelagico, attualmente al centro dell’attenzione mediatica e scientifica grazie alla proclamazione da parte delle Nazioni Unite della Decade of Ocean Science for Sustainable Development (2021-2030). Attenzione derivata dal riconoscimento, da parte della comunità scientifica internazionale, del grande ruolo rivestito dallo spazio marino nel mantenimento dell’equilibrio planetario così come lo conosciamo oggi. Una delle principali agenzie, The research aims to investigate the possible educational function of audiovisual animation materials and, at the same time, given their active role in producing environmental meanings and perceptions, to encourage their use as an educational-didactic tool. The theoretical assumptions from which the research moves are those of ecocriticism (Glotfelty, 1996), a line of thought that demonstrates how mass cultural products (Garrard, 2010; dell'Agnese, 2011) play an active role in the production of ideas about the environment and consequently in conditioning human behaviour towards it. The current environmental crisis imposes a new attitude towards the environment that surrounds us, and therefore requires environmental education and the promotion of awareness of how ideas about the environment also derive from mass cultural products. The research was carried out as part of the project Greening the Visual: an Environmental Atlas of Italian Landscapes (PRIN, 2017-0422), which aims to investigate the way in which visual representations influence the community's environmental ideas by constructing an online visual atlas of Italian landscapes that constitutes a repertoire of the ways in which Italian landscapes have been represented and thus communicated. The Prin project in question involved three work units. In particular, the research unit of the University of Milan-Bicocca, to which the PhD project in question relates, dedicated itself to the analysis of representations of the coastal and marine landscape. The animation materials on which the project worked therefore concerned the pelagic space, currently at the centre of media and scientific attention thanks to the proclamation by the United Nations of the Decade of Ocean Science for Sustainable Development (2021-2030). 8. Corti di mare. Ricerca-azione nella scuola primaria

Abstract

Un ruolo importante nella costruzione dell’immagine ambientale italiana lo svolgono i film d’animazione. Il presente contributo analizza le idee ambientali che emergono da alcuni cortometraggi ambientati in località marine e costiere così da identificarne temi, artifici narrativi e meccanismi discorsivi. Il loro riconoscimento è stato alla base del metodo con cui si sono scelti dei corti d’animazione utilizzati per svolgere attività educative e formative rivolte ad alunni e docenti della scuola primaria. La particolare efficacia del linguaggio dei cartoons ha stimolato il coinvolgimento emotivo degli alunni, capace di promuovere l’appropriazione affettiva degli spazi marini e di portare a una maggiore consapevolezza del ruolo che ciascuno può avere nella salvaguardia dell’ambiente marino. I risultati delle attività di ricerca-azione hanno confermato che l’utilizzo degli strumenti visuali è efficace per esplorare il paesaggio marino e la nostra relazione con esso., Animated films have an important role in the construction of the Italian environmental image. This paper analyses the environmental ideas emerging from some short films, set in marine and coastal sites, in order to identify their themes, narrative artifices and discursive mechanisms. Their recognition formed the basis of the method used to select short animated films used to carry out educational and training activities aimed at primary school students and teachers. The particular power of cartoons’ language stimulated emotional engagement in the young students, promoting affective appropriation of marine spaces and increasing in them the consciousness of their role in preserving the marine environment. The results of the action research activities confirmed that the use of visual tools is efficacious in terms of exploring the seascape and our relationship with it.

9. Josephson effect in fermionic superfluids across the BEC-BCS crossover

Abstract

We report on the observation of the Josephson effect between two strongly interacting fermionic superfluids coupled through a thin tunneling barrier. We prove that the relative population and phase are canonically conjugate dynamical variables, coherently oscillating throughout the entire crossover from molecular Bose-Einstein condensates (BEC) to Bardeen-Cooper-Schrieffer (BCS) superfluids. We measure the plasma frequency and we extract the Josephson coupling energy, both exhibiting a non-monotonic behavior with a maximum near the crossover regime. We also observe the transition from coherent to dissipative dynamics, which we directly ascribe to the propagation of vortices through the superfluid bulk. Our results highlight the robust nature of resonant superfluids, opening the door to the study of the dynamics of superfluid Fermi systems in the presence of strong correlations and fluctuations., Comment: 12 pages, 11 Figures

Some of our participants showed depressive and anxious attitudes even in the absence of real pictures of depression and anxiety. This phenomenon, combined with mechanisms of emulation and conditioning, can trigger a vicious cycle within interpersonal relationships and promote the administration of unnecessary treatments. Various approaches have been proposed to help populations suffering from psychosocial problems induced by the Covid-19 pandemic, but there is an objective difficulty in treating a large population.Methods: To contain and reduce this widespread psychosocial unease, in this study we used two radio electric asymmetric conveyer (REAC) technology neuromodulation treatments, neuro postural optimization (NPO) and neuropsychophysical optimization–cervicobrachial (NPPO-CB), aimed at optimizing an individual’s response to the effects of environmental stressors. These treatments are quick and easy to administer; therefore, they can be administered to a large cohort of participants in a short time. To evaluate the effects of the REAC NPO and NPPO-CB treatments, the DASS-21 psychometric test was used because it has already been used to test depression, anxiety, and stress during the Covid-19 pandemic.Results: The results of the study confirm the usefulness of REAC NPO and NPPO-CB treatments in helping participants to have better coping strategies for the environmental pressures and reduce the neuropsychological and behavioral effects induced by the Covid-19 pandemic.Discussion: The results obtained in this study are consistent with previous clinical studies confirming the usefulness of the treatments to face neuropsychological and behavioral effects induced by exposome pressure.Keywords: coronavirus, behavioral disorders, noninvasive neurostimulation, DASS-21

Time of duration of the disorder was less than two years in half of the clinical sample, and more than 10 years in the other half. The following instruments were used to measure neuropsychological performance: the Matrix Test, the Stroop task, the Trail-Making Test, the Tower of London Test, the Posner Spatial Task, the Rey’s Complex Figure, the Wechsler Vocabulary Test, and the Hayling Completion Test.Results: It showed that persons with long-term ED presented more impaired neuropsychological profiles, but not in all areas. In contrast, the short-term ED group presented similar profiles to the control groups.Discussion: This study highlights the positive association between cognitive impairment and time of evolution of ED, above all in perceptual measures and non-verbal memory. Considering the effect of the evolution of ED cognitive performance (especially in long-term patients) may further our understanding of the development of the disorder and the factors that may favor its persistence.Keywords: eating disorder, cognitive impairment, long-duration ED

A settecento anni dalla morte di Dante, si è sviluppata un'ampia riflessione sul personaggio e sulla sua opera, in particolare sul valore che il testo conserva nell'attualità. Il presente contributo si è concentrato sul potenziale didattico del testo e delle illustrazioni dei luoghi della Divina Commedia per avvicinare i bambini alla comprensione del valore simbolico e paradigmatico del paesaggio. The Geography of Dante explained to children. – During the 7th century since Dante’s death, a broad reflection on the character and his work has developed, particularly on the value the text retains in the present day. This contribution has focused on the didactic potential that the text and the illustrations of the places of the Divine Comedy can have in order to introduce children to an understanding of the symbolic and paradigmatic value of the landscape.

Nel presente contributo si utilizzano le pagine letterarie di Bernard Moitessier per analizzare la sua esperienza dell’ambiente, in particolare di quello marino e, più in dettaglio, con riferimento alla sua componente sonora. Si è voluto verificare se fosse possibile mettere in relazione la metodologia d’analisi testuale proposta da Maria de Fanis (2001) con le categorie di analisi del paesaggio sonoro proposte da Murray Schafer (1985) non solo per descrivere l’ambiente, ma, soprattutto, per far emergere come il paesaggio sonoro possa diventare la metafora delle complesse relazioni tra gli esseri umani e l’ambiente (Lando, 1993, p. 10). Il testo prescelto è La lunga rotta (1972) di Bernard Moitessier (1925-1994), un navigatore dalle notevoli capacità di scrittura, particolarmente attento alla componente sensoriale della sua esperienza del mare. L’autore sottopone gli spazi marini ad uno sguardo (e all’ascolto) peculiare che trasforma l’ambiente che lo circonda nel simbolo dei propri sentimenti, del senso della vita, sua e dell’intero genere umano e del rapporto tra quest’ultimo e il pianeta in cui vive. In this contribution we use the literary pages to analyse the experience of the environment, in particular the marine environment and, more in detail, with reference to its sound component. We wanted to see if it was possible to relate the textual analysis methodology proposed by Maria de Fanis (2001) with the categories of soundscape analysis proposed by Murray Schafer (1985) not only to describe the environment, but above all, to show how the soundscape can become a metaphor for the complex relationships between human beings and the environment (Lando, 1993, p. 10). The chosen text is La lunga rotta (1972) by Bernard Moitessier (1925-1994), a navigator with remarkable writing skills, particularly attentive to the sensory component of his experience of the sea. The author subjects the marine spaces to a peculiar look (and sound) that transforms the environment that surrounds him into a symbol of his own feelings, of the meaning of life, his and the whole human race and the relationship between the latter and the planet in which he lives. Dans cette contribution, les pages littéraires sont utilisées pour analyser l’expérience de l’environnement, en particulier l’environnement marin et, plus en détail, en référence à sa composante sonore. Nous avons voulu voir s’il était possible de relier la méthodologie d’analyse textuelle proposée par Maria de Fanis (2001) aux catégories d’analyse du paysage sonore proposées par Murray Schafer (1985) non seulement pour décrire l’environnement, mais surtout pour montrer comment le paysage sonore peut devenir une métaphore des relations complexes entre les êtres humains et l’environnement (Lando, 1993, p. 10). Abstract

Geography and “the visual” are an inseparable duo: a powerful relationship that elicits research and papers in both geographic research and academic geography. Starting from the commentary on Giorda’s book, this short essay proposes a general reflection on the function that visual teaching aids (maps, images) have in producing and reproducing geographical stereotypes and imagery. The commentary specifically focuses on the value that work such as Giorda’s has for those involved in research in the field of geography in education today.

Abstract

NAVIGATOR is an Italian regional project boosting precision medicine in oncology with the aim of making it more predictive, preventive, and personalised by advancing translational research based on quantitative imaging and integrative omics analyses. The project’s goal is to develop an open imaging biobank for the collection and preservation of a large amount of standardised imaging multimodal datasets, including computed tomography, magnetic resonance imaging, and positron emission tomography data, together with the corresponding patient-related and omics-related relevant information extracted from regional healthcare services using an adapted privacy-preserving model. The project is based on an open-source imaging biobank and an open-science oriented virtual research environment (VRE). Available integrative omics and multi-imaging data of three use cases (prostate cancer, rectal cancer, and gastric cancer) will be collected. All data confined in NAVIGATOR (i.e., standard and novel imaging biomarkers, non-imaging data, health agency data) will be used to create a digital patient model, to support the reliable prediction of the disease phenotype and risk stratification. The VRE that relies on a well-established infrastructure, called D4Science.org, will further provide a multiset infrastructure for processing the integrative omics data, extracting specific radiomic signatures, and for identification and testing of novel imaging biomarkers through big data analytics and artificial intelligence.

Il seguente contributo vuole, in linea con il programma del «Decennio delle scienze del mare per lo sviluppo sostenibile» (2021-2030) e più in generale con l’Ocean Literacy e quindi con lo sforzo conoscitivo e divulgativo riferito all’ambiente marino, proporre l’uso di materiali cinematografici per promuovere l’educazione ambientale marina. Intende altresì riflettere sul ruolo che il visuale, in particolare il cinema d’animazione, può giocare nella didattica della geografia suggerendo alcuni esempi su come utilizzare il potere descrittivo ed evocativo dei film d’animazione in attività di discussione e riflessione con studenti e insegnanti di scuola primaria.

Il seguente contributo descrive una ricerca di dottorato, costruita all’interno del Prin Greening the Visual: an Environmental Atlas of Italian Landscapes2, che intende indagare il ruolo del cinema d’animazione, in particolare dei corti d’animazione, nel plasmare le idee ambientali e di come esse possono essere utilizzate per promuovere un’educazione ambientale marina e l’adozione di un atteggiamento di cittadinanza oceanica all’interno del contesto di scuola primaria. Il progetto di ricerca è volto contemporaneamente a verificare da un lato, il grado di consapevolezza degli insegnanti del ruolo attivo dei materiali d’animazione nel costruire l’idea di natura e del rapporto tra quest’ultima e gli esseri umani, dall’altro, a costruire un’attività di ricerca-azione in grado di rafforzare la consapevolezza dell’importanza degli ambienti naturali e della necessità di prendersene cura utilizzando come strumento educativo le fonti visive: nel contempo, rendere coscienti alunni e insegnanti che le fonti visive hanno un ruolo attivo nella costruzione della nostra idea di natura e di come sollecitino, plasmino e riorientino la percezione di essa. L’attività didattica e di ricerca si concentra in particolare sull’ambiente marino.

I libri di testo hanno un ruolo fondamentale nella trasmissione dei saperi a scuola. In occasione del decennio ONU dedicato alle scienze del mare si è voluto analizzare come i libri di testo per la scuola primaria affrontino questo spazio. La visione del mare che è veicolata da questi strumenti è fortemente terracentrica, il mare è prevalentemente descritto come il limite della terra ferma. Dal punto di vista iconografico si riscontrano delle contraddizioni tra testo e immagini, inoltre il mare è prevalentemente rappresentato sulla falsa riga della sua immagine turistica.

Abstract

Children embrace the sea through Miyazaki’s narration. The contribution below aims to reflect on the role that cinematographic materials can play in the educational context and their validity in promoting knowledge and critical reflection on marine spaces in students, encouraging the adoption of an attitude of Ocean Citizenship. In particular, animated films were considered, a cultural product, popular and appreciated by young students, capable of appealing to the affective-emotional channel, using a language that is easy to understand, which encourages identification with the protagonists, allowing students to relate to the spaces represented through cognitive and affective intelligence. The example on which we will focus will be the animated feature film Ponyo on the Cliff by director Hayao Miyazaki.

Abstract

PRIMAGE is a European Commission-financed project dealing with medical imaging and artificial intelligence aiming to create an imaging biobank in oncology. The project includes a task dedicated to the interoperability between imaging and standard biobanks. We aim at linking Digital imaging and Communications in Medicine (DICOM) metadata to the Minimum Information About BIobank data Sharing (MIABIS) standard of biobanking. A very first integration model based on the fusion of the two existing standards, MIABIS and DICOM, has been developed. The fundamental method was that of expanding the MIABIS core to the imaging field, adding DICOM metadata derived from CT scans of 18 paediatric patients with neuroblastoma. The model was developed with the relational database management system Structured Query Language. The integration data model has been built as an Entity Relationship Diagram, commonly used to organise data within databases. Five additional entities have been linked to the “Image Collection” subcategory in order to include the imaging metadata more specific to the particular type of data: Body Part Examined, Modality Information, Dataset Type, Image Analysis, and Registration Parameters. The model is a starting point for the expansion of MIABIS with further DICOM metadata, enabling the inclusion of imaging data in biorepositories.

Abstract

Background & Aims: The ANSWER study reported that long-term albumin administration in patients with cirrhosis and uncomplicated ascites improves survival. During treatment, serum albumin increased within a month and remained stable thereafter. In this post hoc analysis, we aimed to determine whether on-treatment serum albumin levels could guide therapy. Methods: Logistic regression was used to assess the association between baseline serum albumin and mortality, as well as to determine on-treatment factors associated with mortality and to predict the achievement of a given on-treatment serum albumin level. Survival was assessed by Kaplan-Meier estimates and second-order polynomial regression. Patients whose on-treatment serum albumin remained below normal were compared with a subset of patients from the control arm matched by principal score. Results: Baseline serum albumin was closely associated with 18-month mortality in untreated patients; albumin treatment almost effaced this relationship. On-treatment serum albumin and MELD-Na at month 1 were the sole independent variables associated with mortality. Second-order polynomial regression revealed that survival improved in parallel with increased 1-month on-treatment serum albumin. Kaplan-Meier estimations showed that any value of 1-month on-treatment serum albumin (0.1 g/dl intervals) in the range 2.5–4.5 g/dl discriminated patient survival. In the normal range of serum albumin, the best discriminant value was 4.0 g/dl. Compared to untreated patients, survival even improved in patients whose on-treatment serum albumin remained below normal. Conclusion: Baseline serum albumin per se should not guide the decision to start albumin therapy. Conversely, 1-month on-treatment serum albumin levels are strongly associated with outcomes and could guide the use of albumin – 4.0 g/dl being the target threshold. Abstract

This article's main contributions are twofold: 1) to demonstrate how to apply the general European Union's High-Level Expert Group's (EU HLEG) guidelines for trustworthy AI in practice for the domain of healthcare and 2) to investigate the research question of what does "trustworthy AI" mean at the time of the COVID-19 pandemic. To this end, we present the results of a post-hoc self-assessment to evaluate the trustworthiness of an AI system for predicting a multiregional score conveying the degree of lung compromise in COVID-19 patients, developed and verified by an interdisciplinary team with members from academia, public hospitals, and industry in time of pandemic. The AI system aims to help radiologists to estimate and communicate the severity of damage in a patient's lung from Chest X-rays. It has been experimentally deployed in the radiology department of the ASST Spedali Civili clinic in Brescia, Italy, since December 2020 during pandemic time. The methodology we have applied for our post-hoc assessment, called Z-Inspection®, uses sociotechnical scenarios to identify ethical, technical, and domain-specific issues in the use of the AI system in the context of the pandemic.

Abstract

Topological gauge theories provide powerful effective descriptions of certain strongly correlated systems, a prime example being the Chern-Simons gauge theory of fractional quantum Hall states. Engineering topological gauge theories in controlled quantum systems is of both conceptual and practical importance, as it would provide access to systems with exotic excitations such as anyons without the need for strong correlations. Here, we discuss a scheme to engineer the chiral BF theory, a minimal model of a topological gauge theory corresponding to a one-dimensional reduction of the Chern-Simons theory, with ultracold atoms. Using the local conservation laws of the theory, we encode its quantum Hamitonian into an ultracold quantum gas with chiral interactions. Building on a seminal proposal by Edmonds et al. (Phys. Rev. Lett. 110, 085301 (2013)), we show how to implement it in a Raman-coupled Bose-Einstein condensate with imbalanced scattering lengths, as we have recently realized experimentally (Fr\"olian et al., Nature 608, 293 (2022)). We discuss the properties of the chiral condensate from a gauge theory perspective, and assess the validity of the effective quantum description for accessible experimental parameters via numerical simulations. Abstract

Prematurity (all birth that occurs before the 37th week of gestation, World Health Organization) represents an important perinatal health problem as it is one of the main causes of neonatal morbidity and death. In the past decades, the gradual increase in survival of preterm birthsleaded clinician to face preemies smaller for weight and gestational age (GA), at high risk for negative development sequences. Indeed, the immaturity of the biological systems, joined with the complications and the treatment needed for survival, have a relevant impact on infant, exposing him to the risk of developmental delays. They also more frequently present neuropsychological problems, such as language and attention deficits, disorders in perception, visual-motor integration and learning.

Abstract

Objectives: Multidisciplinary tumour boards (MTBs) play an increasingly important role in managing cancer patients from diagnosis to treatment. However, many problems arise around the organisation of MTBs, both in terms of organisation-administration and time management. In this context, the European Society of Oncologic Imaging (ESOI) conducted a survey among its members, aimed at assessing the quality and amount of involvement of radiologists in MTBs, their role in it and related issues. Methods: All members were invited to fill in a questionnaire consisting of 15 questions with both open and multiple-choice answers. Simple descriptive analyses and graphs were performed. Results: A total of 292 ESOI members in full standing for the year 2018 joined the survey. Most respondents (89%) declared to attend MT-Bs, but only 114 respondents (43.9%) review over 70% of exams prior to MTB meetings, mainly due to lack of time due to a busy schedule for imaging and reporting (46.6%). Perceived benefits (i.e. surgical and histological feedback (86.9%), improved knowledge of cancer treatment (82.7%) and better interaction between radiologists and referring clinicians for discussing rare cases (56.9%)) and issues (i.e. attending MTB meetings during regular working hours (71.9%) and lack of accreditation with continuing medical education (CME) (85%)) are reported. Conclusions: Despite the value and benefits of radiologists’ participation in MTBs, issues like improper preparation due to a busy schedule and no counterpart in CME accreditation require efforts to improve the role of radiologists for a better patient care. Abstract

Radiomics is a process of extraction and analysis of quantitative features from diagnostic images. Liquid biopsy is a test done on a sample of blood to look for cancer cells or for pieces of tumourigenic DNA circulating in the blood. Radiomics and liquid biopsy have great potential in oncology, since both are minimally invasive, easy to perform, and can be repeated in patient follow-up visits, enabling the extraction of valuable information regarding tumour type, aggressiveness, progression, and response to treatment. Both methods are in their infancy, with major evidence of application in lung and gastrointestinal cancer, while still undergoing evaluation in other cancer types. In this paper, the main oncologic applications of radiomics and liquid biopsy are reviewed, and a synergistic approach incorporating both tests for cancer diagnosis and follow-up is discussed within the context of systems medicine. Abstract

Background: Evidence is scarce on the efficacy of long-term human albumin (HA) administration in patients with decompensated cirrhosis. The human Albumin for the treatmeNt of aScites in patients With hEpatic ciRrhosis (ANSWER) study was designed to clarify this issue. Methods: We did an investigator-initiated multicentre randomised, parallel, open-label, pragmatic trial in 33 academic and non-academic Italian hospitals. We randomly assigned patients with cirrhosis and uncomplicated ascites who were treated with anti-aldosteronic drugs (≥200 mg/day) and furosemide (≥25 mg/day) to receive either standard medical treatment (SMT) or SMT plus HA (40 g twice weekly for 2 weeks, and then 40 g weekly) for up to 18 months. The primary endpoint was 18-month mortality, evaluated as difference of events and analysis of survival time in patients included in the modified intention-to-treat and per-protocol populations. This study is registered with EudraCT, number 2008–000625–19, and ClinicalTrials.gov, number NCT01288794. Findings: From April 2, 2011, to May 27, 2015, 440 patients were randomly assigned and 431 were included in the modified intention-to-treat analysis. 38 of 218 patients died in the SMT plus HA group and 46 of 213 in the SMT group. Overall 18-month survival was significantly higher in the SMT plus HA than in the SMT group (Kaplan-Meier estimates 77% vs 66%; p=0·028), resulting in a 38% reduction in the mortality hazard ratio (0·62 [95% CI 0·40–0·95]). 46 (22%) patients in the SMT group and 49 (22%) in the SMT plus HA group had grade 3–4 non-liver related adverse events. Interpretation: In this trial, long-term HA administration prolongs overall survival and might act as a disease modifying treatment in patients with decompensated cirrhosis. Abstract

Mobile devices (smartphones, tablets, etc.) have become key methods of communication, data access and data sharing for the population in the past decade. The technological capabilities of these devices have expanded very rapidly; for example, their in-built cameras have largely replaced conventional cameras. Their processing power is often sufficient to handle the large data sets of radiology studies and to manipulate images and studies directly on hand-held devices. Thus, they can be used to transmit and view radiology studies, often in locations remote from the source of the imaging data. They are not recommended for primary interpretation of radiology studies, but they facilitate sharing of studies for second opinions, viewing of studies and reports by clinicians at the bedside, etc. Other potential applications include remote participation in educational activity (e.g. webinars) and consultation of online educational content, e-books, journals and reference sources. Social-networking applications can be used for exchanging professional information and teaching. Users of mobile device must be aware of the vulnerabilities and dangers of their use, in particular regarding the potential for inappropriate sharing of confidential patient information, and must take appropriate steps to protect confidential data. Key Points • Mobile devices have revolutionized communication in the past decade, and are now ubiquitous. • Mobile devices have sufficient processing power to manipulate and display large data sets of radiological images. • Mobile devices allow transmission & sharing of radiologic studies for purposes of second opinions, bedside review of images, teaching, etc. • Mobile devices are currently not recommended as tools for primary interpretation of radiologic studies. • The use of mobile devices for image and data transmission carries risks, especially regarding confidentiality, which must be considered.

Abstract

Background and aims the Barcelona Clinic Liver Cancer intermediate stage (BCLC-B) of hepatocellular carcinoma (HCC) includes extremely heterogeneous patients in terms of tumor burden and liver function. Transarterial-chemoembolization (TACE) is the first-line treatment for these patients although it may be risky/useless for someone, while others could undergo curative treatments. This study assesses the treatment type performed in a large cohort of BCLC-B patients and its outcome. Methods retrospective analysis of 485 consecutive BCLC-B patients from the ITA.LI.CA database diagnosed with naive HCC after 1999. Patients were stratified by treatment. Results 29 patients (6%) were lost to follow-up before receiving treatment. Treatment distribution was: TACE (233, 51.1%), curative treatments (145 patients, 31.8%), sorafenib (18, 3.9%), other (39, 8.5%), best supportive care (BSC) (21, 4.6%). Median survival (95% CI) was 45 months (37.4-52.7) for curative treatments, 30 (24.7-35.3) for TACE, 14 (10.5-17.5) for sorafenib, 14 (5.2-22.7) for other treatments and 10 (6.0-14.2) for BSC (p

Abstract

IntroductionPreterm and small for gestational age (SGA) infants are at increased risk of poor growth, disability and delayed development. While growing up they are also at increased risk of obesity, diabetes and later heart disease. The risk of such adverse outcomes may be altered by how preterm and SGA infants are fed after birth. Faltering postnatal growth is common due to failure to achieve recommended high energy and protein intakes, and thus preterm and SGA infants are often provided with supplemental nutrition soon after birth. Enhanced nutrition has been associated with improved early growth and better cognitive development. However, limited evidence suggests that faster growth may increase the risk for later adiposity, metabolic and cardiovascular disease, and that such risks may differ between girls and boys.Methods and analysisWe will search Ovid MEDLINE, Embase, Cochrane CENTRAL, Cochrane Database of Systematic Reviews, controlled-trials.com, ClinicalTrials.gov and anzctr.org.au for randomised trials that studied the effects of macronutrient supplements for preterm and SGA infants on (i) developmental and metabolic and (ii) growth outcomes after hospital discharge. The outcomes will be (i) cognitive impairment and metabolic risk (co-primary) and (ii) body mass index. Individual participant data (IPD) from all available trials will be included using an intention-to-treat approach. A one-stage procedure for IPD meta-analysis (MA) will be used, accounting for clustering of participants within studies. Exploratory subgroup analyses will further investigate sources of heterogeneity, including sex and size of infants, different timing, duration and type of supplements.Ethics and disseminationThis IPD-MA is approved by the University of Auckland Human Participants Ethics Committee (reference number: 019874). Individual studies have approval from relevant local ethics committees. Results will be disseminated in a peer-reviewed journal and presented at international conferences.PROSPERO registration numberCRD42017072683
