Introduction: Liver dystemperament is one of the main concepts of Iranian Traditional Medicine. However, the significance of its diagnostic criteria is not clear. The purpose of this study is to determine the major and minor diagnostic criteria of liver dystemperament in Iranian Traditional Medicine. Methods: This study is a qualitative study based on a hybrid model or a combination of theoretical and empirical studies. At the theoretical stage, 11 books of the main texts of Iranian Traditional Medicine were reviewed and scientific databases were searched by the keywords "liver" and "temperament" (mizaj) in Persian and English. Then, in order to examine the diagnostic method and the importance of each of the criteria, a semi-structured face-to-face interview was conducted with 16 traditional medicine specialists. The data were analyzed through conventional content analysis. Then, the significance and diagnostic priority of each of the symptoms were assessed through a discussion group featuring 10-12 professors. Results: In this study, six major criteria and six minor criteria were identified for hot and cold dystemperament and eight major and ten minor criteria were identified for wet and dry liver dystemperament. Conclusion: This study is the first study to assess the importance of symptoms from the viewpoint of traditional medicine specialists and can be considered as an important step forward in integrating the methods of traditional Iranian medicine in terms of diagnosis and treatment. Further studies, as well as validation of the proposed diagnostic indicators and tools for liver dystemperament, are recommended.

Background: The effect of kind of dialysis solution on the control of blood pressure and adequacy and efficacy of hemodialysis is a most debated and controversial issue.Methods: Twenty six chronic hemodialized patients in Ghaem hemodialysis center, Mashhad, Iran, in winter of the year 2004, enrolled this study for one month. The patients were dialyzed with bicarbonate buffer and were dialyzed in the next month with acetate. The kind of membranes and the dialysis machines were the same in both months. Systolic and diastolic pressures were measured before and after hemodialysis in at least three hemodialysis sessions, and he means recorded. The mean of the body weight were also recorded. In the same sessions of hemodialysis by measuring the urea, before and after hemodialysis, urea reduction ratio (URR), was also calculated. Statistical analysis was done with paired student t test for paired measurement, and independent t test for evaluating the effect of the kind of dialysate buffer on the measured parameters.Results: There were no significant statistical differences between acetate and bicarbonate dialysis solution buffers in the control of diastolic blood pressure. But use of acetate buffer resulted in more significant systolic blood pressure reduction compared with bicarbonate buffer (p=0.045). When adequacy of hemodialysis were compared between two kinds of buffers there were no significant statistical differences between them. When the effect of dialysate sodium concentration in the range utilized in two buffers, was evaluated on the systolic and diastolic pressure reduction, no significant differences, between two types of buffers attributable to the difference between dialysates sodium concentrations were found.Conclusion: Hemodialysis with both, acetate and bicarbonate buffers results in significant reductions in systolic and diastolic blood pressure, weight and urea ratio. Use of acetate buffer is more significantly associated with systolic blood pressure reduction compared to bicarbonate buffer (p=0.045), this effect is independent of the dialysate buffer sodium concentration.

BACKGROUND AND OBJECTIVE: COVID-19 started in December 2019 in Wuhan, China and quickly became a global pandemic. According to studies, traditional medicine interventions along with conventional medicine protocols have been reported to be effective in prevention and treatment. The aim of this study was to summarize the recommendations of Persian medicine based on evidence in the prevention and treatment of symptoms of acute respiratory syndrome. METHODS: In this review article, keywords related to the symptoms of COVID-19 in Persian medicine sources, review and items related to the prevention and treatment of acute respiratory syndrome were extracted. Individual items with the keywords "Influenza" or "SARS" or "MERS" or "COVID-19" or "Coronavirus" or "Antiviral" or "Anti-inflammatory" or "Immune system" or "Antioxidant" were searched in the databases SID, Pubmed, Scopus, Cochrane, Web of science, Science direct, and Google scholar, and finally those with scientific evidence were presented in the form of auxiliary instructions for prevention and treatment. FINDINGS: A significant part of the recommendations of Persian medicine to prevent or control the symptoms of acute respiratory syndrome has practical clinical evidence. These include social distance, avoiding heavy meals, and using herbal remedies according to the specific conditions of the patients. CONCLUSION: According to the results of this study, the use of simple strategies of Persian medicine in preventing and improving the symptoms of acute respiratory syndrome caused by COVID-19 along with common medicine protocols, can help control the epidemic conditions. Hybrid plasmonic waveguides (HPWGs) have received attention worldwide for many different kinds of applications, including on-chip polarization control, enhanced nonlinear optical effects, and biosensing. The HPWG sensor can achieve detection limit lower than possible with purely plasmonic sensors. It can be also used for obtaining additional information about complex biological samples. We analyze the effects of various parameters on such a sensor, optimize the sensor design, and predict the optimum performance achievable for an HPWG sensor in the Mach-Zehnder configuration. In this study we compare some demographic and epidemiological characteristics of hemodialysis and peritoneal dialysis patients in an attempt to analyze this issue."n"n Methods: The results of the research protocol "A survey of the epidemiological characteristics of chronic dialyzed patients in Khorassan province", was used to compare some demographic and epidemiological aspects of chronic hemodialysis and peritoneal dialysis patients in search of any statistically significant difference."n"n Results: A total of 836 chronic dialysis patients entered this study, 802 (95.9%) of which were under chronic hemodialysis, and 34 (4.1%) patients were undergoing peritoneal dialysis, with 339 (42.3%) and 12 (35.3%) being female, respectively. The mean age for each group was 47.57 and 46.75 years, respectively. The causes of end-stage renal disease in hemodialzed patients were most commonly hypertension (32.2%), followed by diabetes mellitus (22.3%), unknown causes (19.6%) and chronic glomeru-lonephritis (10.2%). In the peritoneal dialysis group, chronic glomerulonephritis (32.4%) was the most common reason for dialysis, followed by diabetes mellitus (26.5%) and unknown causes (11.8%). The mean serum hemoglobin creatinine, calcium, and phosphorus levels for hemodialysis patients were 10.42, 8.5, 8.93, and 6.03 mg/dL, respectively, and those for peritoneal dialysis patients were 11.28, 8.52, 9.41 and 5.40 mg/dL. There were no statistical differences between the median age, sexual distribution and mean levels of Hgb, Ca, phosphorus, creatinine, HCV antibody and HbsAg of the hemodialysis patients and those of the peritoneal dialysis patients."n"n Conclusions: The results of our study show that hemodialysis and peritoneal dialysis are similarly effective treatments. This study was conducted to assess the effect of oral calcitriol on glucose metabolism in patients on hemodialysis (HD). A total of 27 patients on HD at the Mashhad University of Medical Sciences, Iran, none of whom had received calcitriol or had history of diabetes, were selected. The patients were randomly divided into two groups; Group I: patients who received oral calcitriol for eight weeks and, Group II: patients who received placebo. In all cases, levels of fasting glucose, insulin, lipid profile, calcium, phosphorous, parathormone (PTH), HbA1C and blood sugar after administration of 75 grams of glucose, insulin resistance and beta cell function were measured, before and after the treatment period. The two sets of results were then compared with one another. In Group l patients, the levels of the parameters studied before and after the study period were as follows: blood sugar after 75 grams of glucose (88.67 ± 8.68 versus 99.83 ± 34.42 mg/dL, p = 0.045), HOMA-IR (2.05 ± 1.42 versus 2.42 ± 1.33, p = 0.035), HbA1C (5.99 ± 1.00 versus 6.14 ± 1.19, p = < 0.001), total cholesterol (153.3 ± 43.80 mg/dL versus 157.0 ± 52.62, p = 0.037) and triglycerides (175.30 ± 99.65 versus 214.9 ± 117.7 mg/dL, p = 0.036). Thus, there was a significant decrease after the study period. In Group II, fasting blood sugar (110.7 ± 26.12 versus 81.14 ± 13.31 mg/dL, p = 0.002), HbA1C (6.99 ± 1.44 versus 6.17 ± 1.66, p = 0.004) and HOMA-IR (5.85 ± 5.11 versus 3.20 ± 2.39, p = 0.036) significantly increased and beta cell function significantly decreased (149.5 ± 90.57 versus 355.7 ± 299.3, p = 0.032) after the study period. In conclusion, our results show that vitamin D has a significant influence on glucose metabolism. Similar studies on larger sample size are required to confirm this observation.

Background: Metabolic syndrome (MS) is a collection of known cardiovascular risk factors including hypertension, dyslipidemia, truncal obesity, and impaired glucose tolerance. Vitamin D is one of known lipophilic hormones that have major effects on the homeostasis of calcium and phosphorus. There are reports in general population showing low serum 25-OH D3 levels may be associated with higher prevalence of MS. Objective: As MS reduces graft survival after transplantation, this study aims to find relationship between vitamin D deficiency and metabolic syndrome on kidney transplant patients. Methods: We performed a cross-sectional study from October 2014 to Mach 2015, on 86 stable renal transplant recipients to detect relation between serum vitamin D level and MS. This syndrome was diagnosed according to the Asian National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATPIII) criteria. Patients with pervious history of diabetes mellitus, malabsorption syndrome, chronic diarrhea, and ileostomy, and those with recent use of oral or injection of vitamin D products, anti-convulsants, and alcohol were excluded from the study. Results: There were 43 patients in each group with and without MS. 30 of them (70%) had three diagnostic criteria of MS; 13 (30%) had four criteria. Totally, they were 48 (56%) males and 38 (44%) females. The mean age of those with MS was significantly higher than those without MS. There was no significant difference between mean serum creatinine and GFR between the two groups. Those with MS had higher weight and body mass index (BMI) (p<0.05) compared to those without MS. The prevalence of BMI >25 kg/m2 in MS group was 54% vs. 33% in non-MS group (p<0.05). In MS group 64.3% of patients had vitamin D deficiency, 21% had vitamin D insufficiency and 14% had sufficient level of vitamin D. The rates in non-MS group were 49%, 30%, and 21%, respectively. Although the prevalence were higher in MS group, the difference was not significant. Conclusion: It seems that MS are commonly associated with vitamin D deficiency in kidney transplant recipients and correction of this vitamin deficiency may prevent MS and its side effects and thus increase graft survival. Infectious diseases were one of the most important public health problems in Ardabil during the first Pahlavi period (1925-1941 AD). These diseases caused the illness and death of many people. The purpose of this study is to investigate the factors and consequences of the spread of infectious diseases in Ardabil during the first Pahlavi period. The research method is descriptive and historical-retrospective, which tries to answer the research questions using documentary and library sources. The findings show that malaria, trachoma, smallpox, tuberculosis, rinderpest, venereal diseases, and diphtheria were among the most common infectious diseases. Poverty, illiteracy, poor public health, lack of medical facilities, and the geographical location were the main factors in the spread of these diseases. Considering the critical nature of the issue, the first Pahlavi government tried to control and prevent diseases by carrying out measures such as public education, establishing health and treatment centers and reforming their structure, sending doctors and distributing medicine, and implementing quarantine and vaccination. The results showed that the efforts of the first Pahlavi government in fighting infectious diseases in Ardabil were relatively successful and reduced the prevalence of some diseases., (© 2024 The Author(s). Background: Introduction: Mizaj is the basis of attention to individual differences in Persian Medicine (PM). Regarding the importance of Mizaj for health preservation and treating diseases, it is necessary to achieve a standard tool for Mizaj identification. The purpose of this study was to design a standard self-reporting Mizaj identification questionnaire for elders., Methods: In this exploratory sequential study, criteria of Mizaj identification were extracted by reviewing PM literatures and interview with PM experts and elders. The primary questionnaire was designed and its validity and reliability were assessed, using weighted Kappa statistics, Pearson correlation coefficient (PCC) assessment, receiver operating characteristic (ROC) curve and determining the specificity and sensitivity of cut-off points., Results: Among the 101 items in the primary questionnaire, 73 items had acceptable reliability. The final 20-item questionnaire was obtained after the criterion validity and PCC assessment. The sensitivity and specificity of this questionnaire were 83% and 88% for warmness, 49% and 80% for moderate in warmness-coldness, 72% and 91% for coldness, 57% and 78% for wetness, 30% and 79% for moderate in wetness-dryness, and 81% and 67% for dryness, respectively., Conclusion: The standard Mizaj identification is recommended as a supplementary diagnostic tool for clinicians and researchers in PM. Background: The utilization of complementary and alternative medicine (CAM) is experiencing a global surge, accompanied by the adoption of national CAM policies in numerous countries. Traditional Persian medicine (TPM) is highly used as CAM in Iran, and the ongoing scientific evaluation of its interventions and the implementation of evidence-based medicine (EBM) encounters various barriers. Therefore, comprehending the characteristics and interactions of stakeholders is pivotal in advancing EBM within TPM policies. In this study, we utilized both classical stakeholder analysis and social network analysis to identify key stakeholders and potential communication patterns, thereby promoting EBM in TPM policy-making., Methods: A cross-sectional nationwide stakeholder analysis was conducted in 2023 using snowball sampling. The interviews were carried out using a customized version of the six building blocks of health. Data were collected through semi-structured interviews. Stakeholders were assessed based on five factors (power, interest, influence, position, and competency). The connections and structure of the network were analyzed using degree, betweenness, closeness centrality, and modularity index to detect clusters of smaller networks., Results: Among twenty-three identified stakeholders, the Ministry of Health and Medical Education (MOHME) and the Public were the most powerful and influential. The Iranian Academy of Medical Sciences was the most competent stakeholder. Social network analysis revealed a low density of connections among stakeholders. Pharmaceutical companies were identified as key connectors in the network, while the Public, supreme governmental bodies, and guilds acted as gatekeepers or brokers. The MOHME and Maraji were found to be high-ranking stakeholders based on four different centrality measures., Conclusion: This study identifies powerful stakeholders in the network and emphasizes the need to engage uninterested yet significant stakeholders. Recommendations include improving competence through education, strengthening international relations, and fostering stronger relationships. Background: Mizaj (Temperament) is a concept to express individual differences in Persian medicine and according to this theory, there is a relationship between Mizaj type and the abilities of different body organs. This cross-sectional study aimed to investigate the relationship between the type of Mizaj and the memory score (Quotient)., Methods: The target population was the 18 to 38 years old students of Babol University of Medical Sciences. Mojahedi's Mizaj questionnaire (MMQ) was used for determining the whole Mizaj. The physical Persian version of Wechsler Memory Scale III (WMS III) was used to assess memory score. The collected data were analyzed by SPSS Version 22 and the chi square (x2) and t-test were run and p- value 0.05 was considered as significant difference., Results: Forty-two of participants were females and 18 were males. The average age of them was 23.6 (21-27). The average of Memory Quotient (MQ) was 122.1 ± 5.7. The average of MQ in warm Mizaj was 125.46 ± 1.2 and in cold Mizaj was 118.79 ± 6.5. The difference between two groups is statistically significant (p< 0.001). The average of MQ in dry Mizaj was 124.16 ± 2.67 and in wet Mizaj was 118.40 ± 7.64. The difference between two groups is statistically significant (P= 0.005)., Conclusion: The results showed there are significant relationship between memory score and warm/cold Mizaj and dry /wet Mizaj. It means students with warm or dry Mizaj had better memory score than students with cold or wet Mizaj. This relation was also detected between subtypes of memory and Mizaj expect between working memory and dry/wet Mizaj. These results are in accordance with theories in PM which indicate people with warm Mizaj and dry Mizaj have better memory and people with cold Mizaj and wet Mizaj have weaker memory and are more at risk of memory dysfunction., Competing Interests: no conflict of interest., (© The Author(s).)

In a novel approach to layer-based holography, we propose a machine learning-assisted light sheet holography-an optimized holography technique which projects a target scene onto sheets of light along the longitudinal planes (i.e. planes perpendicular to the plane of the hologram). Using a convolutional neural network in conjunction with superposition of Bessel beams, we generate high-definition images which can be stacked in parallel onto longitudinal planes with very high fidelity. Our holography system provides high axial resolution and excellent control over the light intensity along the optical path, which is suitable for augmented reality and/or virtual reality applications.

Background: Mizaj is an individualized viewpoint in Persian Medicine (PM) that is used for the prevetion of diseases and also treatment. Evaluating Mizaj in the two domains of hotness-coldness, and wetness-dryness, 10 criteria have been introduced, most of them are qualitative. To achieve valid and reliable questionnaires, the weight of these criteria must be determined in assessing the Mizaj., Methods: In a cross-sectional study with Delphi method, 10 indices were extracted from PM references and sent to PM experts via e-mail. They were asked to score the weight of each index in determining the Mizaj from 0 to 10. The scores ranked and comparing previous preliminary studies, criteria of major and minor were proposed., Results: Out of 147 invited PM experts, 122 completed the tables. Based on scores, physical functions, physique, and responsiveness of organs obtained the highest scores in the field of hotness-coldness. In wetness-dryness muscle/fat mass and sleep/wakefulness received the highest scores from the viewpoint of experts., Conclusion: Physical functions, physique (Anthropometry), responsiveness of organs and psychic function can be used as major criteria in Mizaj assessment methods in the hotness-coldness field. Purpose: The authors developed nanostructured lipid carriers (NLCs) loaded with sirolimus (SRL) and cyclosporine (CsA) to improve their therapeutic efficacy in recurrent pregnancy loss (RPL) patients., Methods: Mono-delivery and co-delivery of SRL and CsA by NLCs (S-NLCs, C-NLCs, and S-C-NLCs) were developed. The MTT assay was used to study the optimum dose of formulations. PCR, Western blotting, and ELISA were also conducted., Results: Well-designed nanodrugs with a suitable size, zeta potential, desirable encapsulation efficiency drug loading, and cellular uptake confirmed optimum formulations. Based on cell viability, the amounts of SRL and CsA could be reduced greatly due to the co-delivery by NLCs. Following S-NLCs and C-NLCs interventions in T cells of patients with RPL and immune abnormality, a significant difference was observed in transcription factors and cytokine levels of Th1, Th17, and Tregs compared with healthy samples. Thus, a higher level of pro-inflammatory cytokines (IFN-γ, TNF-α, IL-17, and IL-21) and their regulators (T-bet and RORγt), as well as a lower level of an anti-inflammatory cytokine (IL-10) and its regulatory (Foxp3), were observed. Background: The process of diagnosis and treatment in Persian medicine (PM) are based on the concept of Mizaj (temperament). Among the indices of Mizaj determination, anthropometric indices are less influenceable regarding age change and other environmental factors. The purpose of this study was to investigate the relationship between anthropometric indices and Mizaj., Methods: Four PM experts determined the Mizaj of 121 participants. Those who had ≥70% agreement in their Mizaj determination by the experts were selected and their anthropometric indices were measured. The best cutoff point of each index and its relationship with the defined Mizaj were extracted using Receiver Operative Characteristic Curve and Binary Logistic Regression., Results: 52 out of 121 participants entered the main study. The warm-Mizaj people had larger dimensions in height, shoulder, chest, palm and sole width, and head height. Cold-Mizaj people had smaller dimensions in weight, height, shoulder, chest and head. High levels of BMI, chest depth and head dimensions had the highest correlation with the wet Mizaj and lower dimensions of these indices had the highest correlation with the dry Mizaj., Conclusion: Among the anthropometric indices, chest, palm, sole dimensions, head height and weight had the highest correlation with warmness/coldness and BMI, head width and chest dimensions had the highest correlation with wetness/dryness. The BMI which is more closely related to the soft tissue, only correlates with the wetness/dryness, while, bone dimensions are associated with warmness/coldness. We propose a dual-polarization optimization method for the bulk sensitivity of silicon-on-insulator (SOI) waveguides by defining a multi-objective function that accounts for the substrate leakage losses. The proposed optimization method was used to design micro-ring resonator bulk sensors with strip, slot, subwavelength grating, and subwavelength grating slot waveguides. The subwavelength grating slot waveguide has a bulk sensitivity of 520 nm/RIU and 325 nm/RIU for the TE and TM modes, respectively, both of which are higher than the bulk sensitivities of strip, slot, and subwavelength grating waveguides. Moreover, our Monte Carlo analysis shows that the subwavelength grating slot waveguide has the highest immunity to fabrication errors.
