Aims: Patients affected by functional mitral regurgitation represent an increasingly high‐risk population. Edge‐to‐edge mitral valve repair (TEER) has emerged as a promising treatment option for these patients. However, there is limited research on the comparative outcomes of TEER versus surgical mitral valve repair (SMVr). This study seeks to compare the demographics, complications, and outcomes of TEER and SMVr based on a real‐world analysis of the National Inpatient Sample (NIS) database. Methods and Results: In the NIS database, from the years 2016 to 2018, a total of 6233 and 2524 patients who underwent SMVr and TEER were selected, respectively. The mean ages of the patients were 65.68 years (SMVr) and 78.40 years (TEER) (p < 0.01). The mortality rate of patients who received SMVr was similar to that of patients who were treated with TEER (1.7% vs. 1.9%, p = 0.603). Patients who underwent SMVr more likely suffered from perioperative complications including cardiogenic shock (2.3% vs. 0.4%, p < 0.001), cardiac arrest (1.7% vs. 1.1%, p = 0.025), and cerebrovascular infarction (0.9% vs. 0.4%, p = 0.013). The average length of hospital stay was longer (8.59 vs. 4.13 days, p < 0.001) for SMVr compared to TEER; however, the average cost of treatment was higher ($218 728.25 vs. $215 071.74, p = 0.031) for TEER compared to SMVr. Multiple logistic regression analysis showed that SMVr was associated with worse adjusted cardiogenic shock (OR, 7.347 [95% CI, 3.574−15.105]; p < 0.01) and acute kidney injury (OR, 2.793 [95% CI, 2.356−3.311]; p < 0.01). Conclusion: Patients who underwent TEER demonstrated a notable decrease in postoperative complications and a shorter hospitalization period when compared to those who underwent SMVr. The aim. To analyze the perioperative course of cardiac surgery patients with coronary artery disease (CAD) and heart valve dysfunction. Materials and methods. Retrospective analysis of 41 consecutive cardiac surgery patients (mean age 61.6 ± 7.8 years) with CAD and heart valve dysfunction who were operated on and discharged from the National Amosov Institute of Cardiovascular Surgery of the National Academy of Medical Sciences of Ukraine. All the patients underwent standard clinical and laboratory examinations, coronary angiography, on-pump cardiosurgical intervention. Results. The patients were divided into three subgroups: patients with post-infarction left ventricular aneurysm (LVA) combined with insufficiency of atrioventricular valves, n = 15 (36.6%); those with ischemic mitral insufficiency (MI), n = 11 (26.8%); and patients with CAD combined with aortic valve disease, n = 15 (36.6%). During echocardiographic study, decreased left ventricular ejection fraction was found in patients with LVA and with ischemic MI: 33 ± 6.4% and 39.4 ± 7.8%, respectively, and in patients with aortic valve disease, left ventricular ejection fraction averaged 48.2 ± 9.9% (p < 0.0001). According to the EuroSCORE II scale, the average risk was 11.8 ± 7.3%. All the patients were discussed by the heart team to choose the optimal method of cardiosurgical correction. In order to reduce the ischemic time, off-pump coronary bypass surgery was performed at the first stage, and the second stage was on-pump correction of valvular dysfunction. The average number of grafts was 2.8 ± 1.4, but the largest number of grafts was in the subgroup of ischemic MI: 4 ± 0.9 (p < 0.005). Mitral valve replacement in patients with LVA was performed through LVA in 12 patients. Acute heart failure, acute renal failure often developed in patients after correction of ischemic MI (p < 0.005). On the 1st day after the operation, there was an increase in average glucose values from 6.6 ± 2.8 to 10.3 ± 2.9 mmol/L (p < 0.0001), a decrease in glomerular filtration rate from 72.1 ± 15.3 to 57 ± 18.8 mL/min/1.73 m2 (p = 0.0001), the above indicators returned to the initial level at discharge (p < 0.0001). Conclusions. Off-pump myocardial revascularization in patients with CAD and dysfunction of the heart valves can significantly reduce the ischemic time. The method of mitral valve replacement through LVA developed in the department allowed to reduce the perfusion time (p = 0.0023) and the duration of the operation (p < 0.005). Preoperative discussion of examination data of patients by the heart team, thorough preparation for cardiac surgery, timely response to changes in laboratory parameters with correction of drug treatment avoid complications.

Even though left ventricular ejection fraction was preserved, the patient improved, when we provided him with optimal medical heart failure with reduced ejection fraction treatment. In our case report, we intend to highlight the benefits of optimized medical therapy in a patient with heart failure with preserved ejection fraction, due to mitral valve regurgitation leading to a hugely dilated left atrium.

Background There is little evidence about the prognostic role of mitral regurgitation (MR) in patients with low‐flow, low‐gradient aortic stenosis undergoing transcatheter aortic valve replacement (TAVR). The aim of this study was to assess the prevalence and outcome implications of MR severity in patients with low‐flow, low‐gradient aortic stenosis undergoing TAVR, and to evaluate whether MR improvement after TAVR could influence clinical outcome. Methods and Results This study included consecutive patients with low‐flow, low‐gradient aortic stenosis undergoing TAVR at 2 Italian high‐volume centers. The study population was categorized according to the baseline MR severity and to the presence of MR improvement at discharge. The primary outcome was the composite of all‐cause death and hospitalization for worsening heart failure up to 1 year. The study included 268 patients; 57 (21%) patients showed MR >2+. Patients with MR >2+ showed a lower 1‐year survival free from the primary outcome (P2+ was an independent predictor of the primary outcome (P2+, MR improvement was reported in 24 (44%) cases after TAVR. The persistence of MR was associated with a significantly reduced survival free from the primary outcome, all‐cause death, and heart failure hospitalization up to 1 year. Conclusions In this study, the presence of moderately severe to severe MR in patients with low‐flow, low‐gradient aortic stenosis undergoing TAVR portends a worse clinical outcome at 1 year. TAVR may improve MR severity in nearly half of the patients, resulting in a potential outcome benefit after discharge.

IntroductionMitral Regurgitation (MR) has a strong impact on quality of life and on mid-term survival. Transcatheter mitral valve replacement (TMVR) is rapidly expanding and a growing number of studies have been published recently.MethodsA systematic review of studies reporting on clinical data for patients with symptomatic severe MR undergoing TMVR was performed. Early- and mid-term outcomes (clinical and echocardiographic) were evaluated. Overall weighted means and rates were calculated. Risk ratios or mean differences were calculated for pre- and post-procedural comparisons.ResultsA total of 12 studies and 347 patients who underwent TMVR with devices clinically available or under clinical evaluation were included. Nonbacterial thrombotic endocarditis (NBTE) is a form of endocarditis associated with malignancy or autoimmune disorders. Diagnosis remains a challenge as patients are often asymptomatic up to embolic events or rarely, valve dysfunction. We report a case of NBTE with uncommon clinical presentation and identified with multimodal echocardiography. An 82‑year‑old man presented to our outpatient clinic reporting dyspnea. Past medical history included hypertension, diabetes, kidney disease, and unprovoked deep‑vein thrombosis. On physical examination, he was apyretic, mildly hypotensive, and hypoxemic, had a systolic murmur and lower limbs edema. Transthoracic echocardiography revealed severe mitral regurgitation due to verrucous thickening of the free margin of both leaflets, increased pulmonary pressure, and dilated inferior vena cava. Multiple blood cultures were negative. Transesophageal echocardiography confirmed “thrombotic” thickening of mitral leaflets. Nuclear investigations were highly suggestive of multi‑metastatic pulmonary cancer. We did not further proceed with the diagnostic workup and prescribed palliative care. Lesions seen on echocardiography were suggestive of NBTE: they involved both sides of mitral leaflets, close to the edges, had irregular shape and echo density, a broad base, and no independent motion. Criteria for infective endocarditis were not met and the final diagnosis was paraneoplastic NBTE due to underlying lung cancer. We remark the lack of definitive recommendations about the treatment of NBTE and the only role of anticoagulation to prevent systemic embolism. We have reported a case of NBTE presenting with atypical symptoms and likely related to the prothrombotic state induced by underlying lung cancer. Resumen: El uso de catéter percutáneo para la implantación de dispositivo es una de las técnicas más eficaces en la actualidad para el cierre de comunicación interauricular. En ocasiones puede presentarse mal posicionamiento o causar lesiones a estructuras adyacentes. Por otro lado, se han descrito varias causas de estenosis de vena pulmonar, una de ellas la iatrogenia durante procedimientos quirúrgicos cardiacos. Presentamos el caso de una paciente mujer de 41 años con antecedente de cierre de comunicación interauricular, quien presento síntomas de dolor torácico tipo punzada, palpitaciones y deterioro de la clase funcional. Al examen físico estaba sin deterioro hemodinámico, con hallazgo de soplo sistólico en foco pulmonar. Por antecedente se realizó ecocardiograma, identificándose insuficiencia mitral severa, tricúspidea moderada y comunicación interauricular residual 11 años después del cierre del defecto. Se realizó cirugía para el cierre de la comunicación residual y plastia mitral y tricúspidea, sin embargo, durante el postoperatorio presentó estenosis de la vena pulmonar derecha, por lo cual fue nuevamente intervenida para la liberación de venas pulmonares con adecuada evolución clínica a un año del seguimiento. Abstract: The use of a percutaneous catheter for device implantation is currently one of the most effective techniques for atrial septal defect closure. Occasionally, it may be poorly positioned or cause lesions to adjacent structures. Furthermore, several causes of pulmonary vein stenosis have been described, one of which being iatrogenic during cardiac surgical procedures. We present the case of a 41-year-old female patient with a history of atrial septal defect closure who presented with symptoms of stabbing chest pain, palpitations and functional class impairment.Physical examination showed no haemodynamic deterioration with systolic murmur in the pulmonary focus. Echocardiogram was performed identifying severe mitral regurgitation, moderate tricuspid regurgitation and residual atrial septal defect 11 years after defect closure. Surgery was performed to close the residual communication and mitral and tricuspid plasty. However, during the postoperative period the patient presented stenosis of the right pulmonary vein and was operated again to free the pulmonary veins with adequate clinical evolution at one year of follow-up.

Haemodynamic changes in mitral regurgitation underlie triggering of patient’s symptoms and development of heart failure. A number of endovascular methods for the correction of mitral regurgitation successfully counteract pathological hemodynamics and thus manage to improve both the manifestations of heart failure and the patient's symptoms. This article is a review of the physiology of the mitral valve and the changes that occur in the presence of mitral regurgitation. The peculiarities of the hemodynamics of the left atrium in the conditions of mitral regurgitation and after some types of transcatheter treatment – edge-to-edge correction of the valve with Mitraclip and the implantation of an occluder in paraprosthetic mitral regurgitation are considered. The change in left atrial parameters immediately after correction of the defect is of value both for the assessment of immediate procedural success and for the patient's prognosis.

In recent years, new interest is growing in the left atrium (LA). LA functional analysis and measurement have an essential role in cardiac function evaluation. Left atrial size and function are key elements during the noninvasive analysis of diastolic function in several heart diseases. The LA represents a "neuroendocrine organ" with high sensitivity to the nervous, endocrine, and immune systems. New insights highlight the importance of left atrial structural, contractile, and/or electrophysiological changes, introducing the concept of "atrial cardiomyopathy", which is closely linked to underlying heart disease, arrhythmias, and conditions such as aging. The diagnostic algorithm for atrial cardiomyopathy should follow a stepwise approach, combining risk factors, clinical characteristics, and imaging. Constant advances in imaging techniques offer superb opportunities for a comprehensive evaluation of LA function, underlying specific mechanisms, and patterns of progression. In this literature review, we aim to suggest a practical, stepwise algorithm with integrative multimodality imaging and a clinical approach for LA geometry and functional analysis. This integrates diastolic flow analysis with LA remodelling by the application of traditional and new diagnostic imaging techniques in several clinical settings such as heart failure (HF), atrial fibrillation (AF), coronary artery disease (CAD), and mitral regurgitation (MR). Atrial shunt devices are being implanted with increasing frequency, and there are potential concerns for subsequent procedures using transseptal access. A 79-year-old woman presented with progressive dyspnea of multifactorial etiology with already implanted atrial shunt device. Background: The use of thiazide diuretics is recommended in the American College of Veterinary Internal Medicine guidelines for advanced heart failure due to mitral insufficiency (MI) in dogs. However, there are no large-scale reports of the use of thiazide diuretics in dogs with advanced heart failure. Aim: This retrospective study evaluated the therapeutic effect of concomitant hydrochlorothiazide (HTCZ) with loop diuretics in dogs with heart failure. Methods: The study included 14 dogs diagnosed with advanced pulmonary edema with MI at two facilities. In all cases, high-dose loop diuretics (torsemide; 0.78 – 4 mg/kg/day) did not improve pulmonary edema. The results of the echocardiography and renal function tests before and after the administration of HTCZ (0.2–0.84 mg/kg/day) in addition to torsemide were statistically compared. Results: The echocardiographic data demonstrated significant improvement in relation to cardiac stress; left atrium to the aorta ratio, normalized left ventricular internal dimension in diastole, and E wave velocity (m/s) after HTCZ administration. However, blood urea nitrogen and creatinine levels increased, and potassium levels decreased, indicating a decline in renal function following HTCZ administration. Conclusion: This study suggests that the administration of HTCZ in combination with loop diuretics may be beneficial during advanced heart failure due to MI in dogs. The results can also be extended to patients who are resistant to loop diuretics, resulting in the improvement of cardiac function. However, as the combination of HTCZ and loop diuretics can deteriorate renal function, caution should be exercised prior to making recommendations regarding its use, and renal function should be monitored.

We report on a 72 years old male patient with recurrent heart failure hospitalizations caused by severe mitral regurgitation due to severe restriction of the posterior mitral leaflet treated with the transfemoral mitral valve replacement (TMVR) system Cardiovalve. Immediate interventional success was obtained resulting in a quick mobilization and discharge.

Over the last two decades, transcatheter devices have been developed to repair or replace diseased mitral valves (MV). Transcatheter mitral valve repair (TMVr) devices have been proven to be efficient and safe, but many anatomical structures are not compatible with these technologies. The most significant advantage of transcatheter mitral valve replacement (TMVR) over transcatheter repair is the greater and more reliable reduction in mitral regurgitation. However, there are also potential disadvantages. Mitral valve reconstruction has become a priority option for mitral regurgitation correction, due to the documented advantages, comparing with valve replacement, in terms of long-term survival, absence of valve-related side effects, and preservation of left ventricle function. Improvement of mitral valve reconstruction techniques has made almost all lesion variants (more than 95 %) suitable for reconstruction, with a 15-year freedom from reoperations in 90 % of all operated patients. Aim. To study clinical manifestations and results of reconstructive surgeries on the mitral valve in patients with primary and secondary mitral regurgitation. Materials and methods. The study is based on the analysis of clinical data of 218 patients with mitral valve insufficiency who were treated in National M. Amosov Institute of Cardiovascular Surgery of the National Academy of Medical Sciences of Ukraine from 01.01.2010 to 01.01.2015. The average age of patients was 52.8 ± 13.0 years (16.0–78.0). The ratio of male to female patients was 151 (69.3 %) and 67 (30.7 %) cases, respectively. Acute occurrence of mitral regurgitation was observed in 10 (4.6 %) cases. Results. According to the clinical and histological causes of mitral regurgitation, we systematized and conducted a comparative analysis between groups of patients with primary (n = 174) and secondary (n = 44) mitral regurgitation. The group with primary mitral regurgitation (n = 174) in the early postoperative period was characterized by averagely shorter duration of the total time of artificial ventilation compared with the secondary genesis of mitral regurgitation. Thus, in the group of patients with secondary mitral regurgitation significantly (P = 0.003) more often there were signs of acute cardiac insufficiency. Hospital mortality for the whole group of patients was 5 (2.3 %) cases. In the group with secondary mitral regurgitation, hospital mortality was significantly higher and amounted to – 6.8 % (3 cases in 44 patients), against 1.2 % (2 cases in 174 patients) in primary mitral regurgitation. Conclusions. Primary mitral regurgitation occurs due to a violation of the anatomy of the valvular apparatus, accompanied by increased mobility of the valves (prolapse) and regurgitation. Hospital mortality in primary mitral regurgitation was 1.2 %. Survival after 6 months and by the end of 1 and 5 years was 98.8 %, 98.2 % and 97.0 %, respectively. Secondary mitral regurgitation is caused by the initial dilatation of the left ventricle with preserved anatomy of the valvular apparatus. Hospital mortality was 6.8 %. Survival rate after 6 months, by the end of 1, 3 and 5 years, was 90.9 %, 86.3 %, 77.9 % and 74.0 % respectively.

Resumen: Objetivo: Revisar las características demográficas, clínicas y ecocardiográficas, así como el abordaje quirúrgico de pacientes con fiebre reumática menores de 18 años, durante un periodo de diez años. Materiales y métodos: Estudio descriptivo, retrospectivo, en el cual se incluyeron pacientes menores de 18 años, con diagnóstico de fiebre reumática entre los años 2006 a 2016. Resultados: Se incluyeron 22 niños con edades entre los 10 a los 14 años, de los cuales 59% (13/22) eran mujeres; 13,6% (3/22) provenían de Arauca y el 18,1% (4/22) eran indígenas. Se diagnosticó carditis en el 63,6% (14/22) seguido de poliartralgia y fiebre en un 59% (13/22) y 54% (12/22) respectivamente; así mismo, se detectó corea de Sydenham y eritema marginado 4,5% (1/22). Se determinó compromiso severo de válvula mitral en el 72,7% (16/22). 15 pacientes fueron llevados a cirugía; se realizó reemplazo valvular mitral en 13,3% (2/15), plastia mitral y tricúspide en el 40% (6/15), plastia mitral, tricúspide y aórtica en el 6,66%(1/15) y reemplazo aórtico en el 5,2% (1/15). Un paciente requirió oxigenación por membrana extracorpórea. La mortalidad fue del 0% (0/22). Conclusiones: Se observó que la fiebre reumática ocasionó grave compromiso cardiaco, que requirió un alto grado de intervención quirúrgica. Las poblaciones rurales e indígenas se beneficiarían notablemente de una prevención primordial, primaria y secundaria, al igual que de un diagnóstico y tratamiento oportunos. Materials and methods: A descriptive, retrospective study was conducted that included patients less than 18 years-old diagnosed with rheumatic fever between the years 2006 to 2016. Results: The study included a total of 22 children with ages between 10 and 14 years, of whom 59% were female. It was recorded that 13.6% (3/22) were from Arauca Department (Colombia), and 18.1% (4/22) were indigenous. Carditis was diagnosed in 63.6% (14/22), followed by polyarthralgia and fever in 59% (13/22) and 54% (12/22), respectively. Sydenham's chorea and marginal erythema was also detected in 4.5% (1/22). The severity of mitral valve involvement was determined in 72.7% (16/22). Of the 15 patients that had surgery, 13.3% (2/15) had a mitral valve replacement, mitral and tricuspid repair in 40% (6/15), mitral, tricuspid, and aortic repair in 6.66%, respectively (1/15), and aortic replacement in 5.2% (1/15). One patient required extracorporeal membrane oxygenation. There was no mortality. Conclusions: It was observed that rheumatic fever led to serious cardiac compromise that required a high rate of surgical interventions. Rural and indigenous population would notably benefit from basic, primary, and secondary prevention, as well as a timely diagnosis and treatment.

Abstract Background The etiology of Takayasu arteritis (TA) is unknown; however, a possible relationship between streptococcal infection and TA has been proposed. This study aimed to identify the clinical features and cardiac valvular involvement in untreated TA patients with an elevated antistreptolysin O (ASO) titer. Methods In this retrospective study, the clinical characteristics and features of valvular involvement were compared in TA patients with or without an elevated ASO titer. Results Of the 74 untreated TA patients, 13 patients were found have elevated ASO titers (17.6%). Mitral insufficiency was the most common in patients with elevated ASO (69.2%, 9/13), followed by aortic valve insufficiency (46.2%, 5/13) and tricuspid insufficiency (46.2%, 5/13), which were no significantly different than that in normal ASO group. The proportions of moderate to severe mitral (30.8% vs 1.6%, p = 0.000) and tricuspid valve (15.4% vs 1.64%, p = 0.023) insufficiency in the ASO positive group were significantly higher than those in the ASO negative group. The odds of mitral regurgitation in patients with elevated ASO titers were 3.9 times higher than those in the group with normal ASO titers (p = 0.053, OR = 3.929, 95% confidence interval [CI]: 0.983–15.694). Furthermore, the risk of moderate to severe mitral insufficiency in patients with elevated ASO titers was 41.6 times higher than that in patients with normal ASO titers (p = 0.002, OR = 41.600, 95% CI: 3.867–447.559). Conclusions An increase in ASO titer is related to valvular involvement in TA and is closely linked to mitral insufficiency.

In recent years, transcatheter treatment of mitral regurgitation (MR) has been extensively explored and has shown promising results. After transcatheter mitral valve repair (TMVr) with MitraClip has become a routine strategy in patients with symptomatic degenerate MR at prohibitive surgical risk, the benefits of TMVr are now being examined in patients with functional MR (FMR). However, the contradictory observations in COAPT and MITRA-FR clinical trials so far have suggested that only patients with severe FMR disproportionate to left ventricular disorder may benefit from this interventional therapy. Nevertheless, more evidence is required to support this hypothesis. In addition, various new devices have been developed, including two original designs from China Mainland, which have displayed excellent performance. Several percutaneous (femoral vein.transseptal) valves used in transcatheter mitral valve replacement have been developed and have also shown good feasibility. Furthermore, hybrid techniques in MR therapies have been emerging, which may reduce residual MR and improve the treatment effects.

Resumen: La pérdida de la geometría ventricular es la principal causa de progresión de la insuficiencia cardiaca y de riesgo en la morbimortalidad en pacientes con cardiomiopatía dilatada. La insuficiencia mitral ocurre secundariamente a la dilatación anular, a la geometría ventricular izquierda alterada y a la disfunción del músculo papilar. La cirugía de restauración ventricular (CRV) se introdujo como una opción terapéutica con la hipótesis que mejora el funcionamiento mitral al reducir los volúmenes del ventrículo izquierdo, las distancias de los músculos papilares y restaurar la geometría cardiaca. Este trabajo presenta el caso clínico de una paciente con insuficiencia cardiaca de etiología isquémica necrótica, que fue intervenida quirúrgicamente y se le realizó restauración ventricular, células madre y matriz de colágeno biodegradable. En el seguimiento a los dos años, la paciente no necesito más ingresos por descompensación, se observó que la clase funcional (CF) (NYHA) mejoró de CF III a I. La fracción de eyección del ventrículo izquierdo (FEVI) se incrementó de 20 a 27%, la insuficiencia mitral se redujo de grado IV a II, presentando estudios complementarios con mejora en el ejercicio. Los resultados de este caso sugieren que la técnica de restauración ventricular basada en el modelo de la banda miocárdica se aproxima a la anatomía y fisiología cardiaca normal, corrigiendo la insuficiencia mitral isquémica sin asociar cirugía valvular. Abstract: Loss of ventricular geometry is the main cause of progression of heart failure and risk of mortality in patients with dilated cardiomyopathies. Mitral regurgitation occurs secondary to annular dilatation, altered left ventricular geometry and papillary muscle dysfunction. Ventricular restoration surgery was introduced as a therapeutic option with the hypothesis that improves mitral functioning by reducing left ventricular volumes, papillary muscle distances and restoring cardiac geometry. This work presents a clinical case of a patient with heart failure of necrotic ischemic etiology, who underwent surgery and had ventricular restoration, stem cells and biodegradable collagen matrix. At the two-year follow-up, the patient did not need further hospitalizations due to decompensation, it was observed that the functional class (NYHA) improved from FC III to I. On the other hand, the ejection fraction improved from 20% to 27%, Mitral Insufficiency was reduced from grade IV to grade II presenting complementary studies with improvement in exercise. The results of this case suggest that the miocardial band and the restoration technique based on this model approximate normal cardiac anatomy and physiology, correcting ischemic mitral regurgitation without associating valvular surgery.

In this paper we represent the experience of the UCCC of the Ministry of Health of Ukraine in the surgical treatment of patients with mitral insufficiency caused by the prolapse of the mitral valve (MV) cusps. The aim. To analyze our own experience in reconstructive interventions on MV in patients with mitral valve prolapse. Materials and methods. From January 2011 to December 2017, 146 patients with severe mitral insufficiency underwent surgical interventions on the basis of the UCCC due to the prolapse of the mitral valve cusps. Results. MV repair was final in 144 (98.6 %) patients. At the time of discharge, 33 (23.9 %) patients had no residual MV insufficiency. Minimum (trivial) MV insufficiency was found in 97 (66.4 %) patients, mild MV insufficiency in 15 (10.3 %), and moderate in 1 (0.7 %) patient. There were no patients with severe MV insufficiency at the time of discharge. Therefore, mitral valve repair was successful in 145 (99.3 %) patients. Freedom from moderate and severe MV insufficiency in the remote postoperative period decreased and amounted to 95.4 %. We analyzed 8 patients who had reoperation or showed progression of residual mitral insufficiency. All of them had satisfactory result at the time of discharge from the hospital, but had unsatisfactory result at the follow-up. The reason for the unsatisfactory result was the use of artificial chords and suture annuloplication. Conclusion. Successful mitral valve repair is possible in most cases of severe mitral valve regurgitation caused by MV prolapse. A good immediate postoperative result does not mean long-term valve competency. Suture annuloplasty is a strong risk factor for the progression of MV insufficiency in the postoperative period. Resection of the posterior cusp in complex MV repair has positive impact on the long-term prognosis.
