We report myocardial perfusion SPECT pattern in four subsequent patients with rate dependent left bundle branch block (LBBB). Three females and one male (aged 48, 51, 63 and 67 years) were studied. None of the patients had history of typical chest pain and all suffered from atypical chest pain or dyspnea on exertion. All patients were tested for baseline and serial heart rate, blood pressure, and electrocardiogram recordings. The exercise treadmill tests (ETT) were carried out under the strict supervision of a cardiologist, a nuclear medicine physician and close availability of an expert cardio-pulmonary resuscitation team and cardiac care unit within just few seconds. Maximal stress test (at least 85% of calculated heart rate, following development of LBBB) was achieved in all four patients according to standard Bruce protocol. No adverse cardiac events were noted and all ETT stress protocols terminated completely and safely. Myocardial perfusion SPECT imaging showed no evidence of reversible perfusion defects. The only patient with past history of exercise induced LBBB showed nonreversible perfusion defects in the septal and anteroseptal regions and mild LV cavity dilatation. The limited number of patients enrolled in our study does not allow us to draw a definite conclusion. Despite the presence of false-positive defects in myocardial perfusion SPECT in patients with sustained LBBB, such a finding is not a consistent finding in patients with rate dependent or exercised-induced LBBB, unlike that which we expected to see. Introduction: The aim of this study was to evaluate the effect of successful kidney transplantation (KT) on myocardial perfusion, and, left ventricular function by both qualitative (visual) interpretation and semiquantitative parameters, using myocardial perfusion scintigraphy with gated-single photon emission computed tomography (gated-SPECT) in patients suffering from end-stage renal disease. Methods: From a total of 38 patients who were candidates of KT, twenty-six patients (16 female, 10 male, mean age: 47.5 yr, range: 24-64 yr) who had successful KT were included. Myocardial perfusion scintigraphy was performed by Gated Single Photon Emission Computed Tomography (Gated-SPECT) method, before and after surgery (mean: 24 months). Perfusion and function status was evaluated by qualitative and semiquantitative parameters. Results: Our data showed qualitative evidence of perfusion and functional abnormality in pre-transplant scans as follows: Abnormal perfusion in left anterior descending (LAD), left circumflex (LCX) and right coronary artery (RCA) territories in 42.5%, 53.8% and 65.4% of cases, respectively; dilation in 57.7% and inhomogenity of uptake in 53.8% of cases. However no statistically significant change was noted after transplantation, i.e. p value for all semiquantitative values including summed stress score (SSS), summed rest score (SRS) and summed difference score (SDS), summed motion score (SMS), summed thickening score (STS), ejection fraction (EF); end diastolic volume (EDV), end systolic volume (ESV), and stroke volume (SV) was greater than 0.05. Conclusion: Renal transplantation may not have considerable long term effect on myocardial perfusion and function in patients with chronic renal failure. The authors reflect on the appropriateness of referrals for single-photon emission computed tomography myocardial perfusion imaging (SPECT-MPI) in Iran, which is one of the most widely used procedures of nuclear medicine to detect perfusion abnormalities. The authors say that they conducted a study to describe the ordering practices of physicians in Iran. They add that there was no vital difference between governmental and private medicine centers in regard to the level of appropriateness.

The authors discuss the past, present, and future trends of scientific journalism on nuclear medicine in Iran. They explore the history of publication of scientific issues starting in 1961. They cite that scientific publication began increasing in the 2000s when nuclear medicine was made an independent medical specialty in the country. They note that the nuclear medicine community should attempt to connect with other disciplines.

Objective: It has been postulated that if patients fail to achieve their age-predicted target heart rate (THR: 85% of predicted maximal HR), the electrocardiographic changes following exercise treadmill test (ETT) and also scintigraphic results of this stress protocol for myocardial perfusion imaging (MPI) are unreliable. We decided to assess the ability of submaximal ETT in provoking ischemia as determined by MPI. Methods: One hundred and nine patients (60 female, 49 male, mean age: 50.8±11.4, range: 32-76 years), were prospectively assessed with MPI after stress protocol of ETT. Forty-nine patients failed to attain THR. Myocardial perfusion imaging was performed based on the 1-day protocol, using Technetium-99m sestamibi (99mTc-MIBI). Results: Sixty patients attained THR, of which 28 patients (46.7%) had normal myocardial perfusion pattern and 32 patients (53.3%) had scintigraphic evidence of ischemia. The remaining 49 patients had submaximal ETT, of which 15 patients (30.6%) had normal results and 34 patients (69.4%) had scintigraphic evidence of ischemia. Although ischemia was more common in those patients with submaximal ETT, however the difference was not statistically significant (p=0.088). Conclusion: Ischemia is more common in patients who had performed a sub-maximal ETT. It could be a logical hypothesis that ischemic heart disease may limit the ability of patients to complete ETT and therefore, it would not be judged that submaximal ETT is always insufficient for MPI. In these settings, in patients who fail to attain the THR, it would be possible to proceed to the stress MPI test. [ABSTRACT FROM AUTHOR]

A 65 year-old overweight man with a history of prostate cancer was referred to our nuclear medicine department for bone scanning. Anterior projection images showed two small foci of increased radiotracer uptake corresponding to the anterior arcs of the right and left sixth ribs, which were interpreted as suspicious for metastatic involvement. Eight months later the patient was referred for follow-up bone scan. In the follow-up scan, those two foci of abnormal radiotracer activity were outside the limits of the bony structures of the chest. In fact, those foci changed their position and were due to radiotracer uptake by the enlarged breasts of this gentelman (Gynecomastia). Previously, it has been just one report concerning radiotracer uptake in the breasts of a man. Based on our case report, this abnormal finding is not exclusively observed in women and it can be also seen in men who suffer from gynecomastia. Physical examination in these settings can be extremely helpful. Oblique, lateral and SPECT (Single Photon Emission Tomography) views can also confirm the extraskeletal origin of radiotracer uptake. An abstract of the article "The First Experience of Stem Cell Labeling in Iran Using 111In- Oxine," by Ali Gholamrezanezhad and colleagues is presented.

Objective: To evaluate the epidemiology of pubertal development of girls in a developing community., Methods: The cross-sectional study was undertaken in July 2005, on 788 healthy 7-16 years old girls in Gorgan, the most populous city of Golestan province in northern Iran. Physical examination, was performed and height, weight and body mass index were noted. Pubertal stages were assessed by clinical examination according to the methods of Marshall and Tanner, while the mean menarcheal age was calculated by using the status quo method. The data was treated with analysis of variance Pearson's linear regression and SPSS version 16.0., Results: The mean age, weight, height and BMI +/- standard deviation of the studied girls were 10.32 +/- 2.48 years, 35.07 +/- 11.30 kg, 140.11 +/- 14.62 cm, and 17.34 +/- 3.55 kg/m2 respectively. Menarche had occurred with the mean menarcheal age being 12.15 +/- 1.11 years. The mean age at the onset of breast development was 10.55 +/- 1.57 years. There was no significant association between mean height, weight and BMI and the menarcheal age and pubertal stage., Conclusion: Based on the epidemiology of pubertal development of girls in the study, there seems to be a case for creating particular guidelines for pubertal development assessment for each type of society in addition to the standard international guidelines in this regard.

Multi-drug resistance (MDR) is a major challenge in the treatment of multiple myeloma (MM). There is low sensitivity of technetium-99m methoxy isobutyl isonitrile ((99m)Tc-MIBI) whole body scan (WBS) in the detection of active MM lesions, because (99m)Tc-MIBI is washed out from malignant cells in the presence of P-glycoprotein (PGP). The objective of the present cohort study was to evaluate of (99m)Tc-MIBI WBS in the prediction of MDR in MM patients during a course of one year follow up. Thirty four patients with MM (25 male, 9 female of mean age 54.12+/-11.46 years) entered the study. Thirteen patients had no previous history of treatment and 21 had a history of previous chemo-radiotherapy. The diagnosis and staging of the disease were based upon routine laboratory and clinical criteria like bone marrow plasma cell count, serum M component, calcium, albumin, beta(2)-microglobulin. Measurements of PGP and WBS using (99m)Tc-MIBI were performed before initialization of treatment and the response to treatment was assessed one year later. The baseline (99m)Tc-MIBI WBS were considered positive for the detection of active lesions when at least one area of non-physiologic increased activity was noted. The follow up (99m)Tc-MIBI WBS was positive for MDR when the patient had active disease but normal WBS. Our results showed that for WBS, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy in active state for the diagnosis of MDR were 38.9%, 62.5%, 70%, 31.2% and 46.1%, respectively. Also the above values for the detection of MDR, using PGP values were 50%, 50%, 69.2%, 30.8% and 50%, respectively. The relative risk of resistant to multiple regimens of chemotherapy after one year follow up in patients with negative to patients with positive (99m)Tc-MIBI WBS was 1.02 (0.60-1.72). In conclusion, we found a low sensitivity of WBS and of PGP in the detection of MDR in patients with active MM. However, both WBS and PGP have 70% and 69% positive predictive value for MDR.

Objective: To develop a system to score disease activity based on clinical manifestations in patients with active ulcerative colitis and to assess the relationship of this score with endoscopic disease severity as assessed by colonoscopy., Methods: In a pilot study of 43 patients, nine clinical variables were examined by univariate analysis. Six factors that correlated with disease severity included age, well-being, defecation frequency, bloody stool, extraintestinal manifestations (Ext) and hemoglobin (Hb). Multivariate analysis resulted in the generation of a new activity index for the prediction of disease severity. Validation of this index was performed on 88 patients with definite diagnosis of ulcerative coli-tis based on previous histological examination., Results: The activity index is derived as follows: (18 x bloody stool) + (20 x defecation frequency) + (5 x Ext) + (15 x Hb) - (well-being x 17) + (age x 2) + 158. Values below 280, between 280 and 310 and above 310 corresponded to mild, moderate and severe disease, respectively. The model classified disease severity correctly in 71% of cases., Conclusion: This activity index may be useful for noninvasive evaluation of disease severity in patients with ulcerative colitis.

A 32-year-old man with a history of painful swelling of the right ankle underwent bone scintigraphy, which showed increased uptake in the right ankle and also unexpected diffuse uptake throughout the right hemithorax. A single photon emission tomography scan performed after the intravenous injection of 740 MBq of technetium-99m methylene-diphosphonate ((99m)Tc-MDP) showed abnormal uptake throughout the right lung. Computed tomography (CT) revealed a large mass in the right lower lobe. CT-guided biopsy of this mass led to a diagnosis of metastatic Ewing's sarcoma. Although lung uptake on bone scans has been noted in various occasions (such as: pulmonary alveolar microlithiasis, Pneumocystis carinii pneumonia, and various tumoral lesions), increased uptake of (99m)Tc-MDP in lung metastases of Ewing's sarcoma has not been reported according to our knowledge until now. We report such a case.
