Giriş : İnme yaşlanan toplumlarda sosyoekonomik önemi gittikçe artan bir hastalıktır. Dünya Sağlık Örgütü'nün (DSÖ) verilerinde 1990 yılında inme dünya genelinde ikinci, gelişmiş ülkelerde üçüncü ölüm nedeni olarak gösterilmiştir. Sakatlık ve işgücü kaybının ise birinci nedenidir. Son dönemlerde inme ile ilişkili mortalite oranı azalırken inme insidansında yükselme olmuş ve dizabilite ile yaşayan inme geçirmiş kişi sayısı artmıştır. Bu artış, inme geçiren kişinin kendisi, ailesi, toplum ve sağlık sektörü için büyük emosyonel ve sosyoekonomik sonuçlar doğurmaktadır. Ayrıca son dönemlerde inme ile ilişkili hospitalizasyon oranlarında artış olduğu izlenmiştir. Hospitalizasyon oranında artış ile birlikte hastanede kalış süresinde ve mortalite oranlarındaki düşüş korumanın iyileşmesi değil, inmenin daha iyi tedavisi olduğu görüşünü desteklemektedir. İskemik inme hastasında erken kan akımı sağlanmasının uzun dönem morbidite üzerine etkili olduğu çalışmalarla kanıtlanmıştır. Bu nedenle, çalışmaların çoğu rekanalizasyon ve reperfüzyon odaklıdır. Amaç : Çalışmanın amacı, hastane öncesi ve hastane içi faktörlerin, akut iskemik inme hastalarında prognozu ne şekilde etkilediğini bulmaktır.Materyel ve metod : Araştırmamız hastane öncesi ve hastane içi faktörlerin prognoz üzerine etkisini belirleyen kohort çalışmasıdır. Araştırmaya; Haziran 2015 – Ağustos 2015 tarihleri arasında semptom başlangıcının ilk 24 saatinde Dokuz Eylül Üniversitesi Tıp Fakültesi acil servisine başvuran, klinik, laboratuvar bulgular ve radyolojik tanı yöntemleriyle (bilgisayarlı tomografi (BT) ve/veya manyetik rezonans görüntülemesinde (MRG)) akut iskemik inme tanısı alan 18 yaşından büyük 80 hasta alındı. Dışlama ölçütü, dizabiliteye neden olabilecek başka bir komorbit durum olarak kabul edildi. Hastalar inmeyi takiben ilk 24 saat, 3.gün, taburculuk sırasında ve 3.ayda rutin İnme Poliklinik kontrolünde değerlendirildi.Bulgular ve Sonuç: Çalışmamızda 75 yaş üzeri hastalarda prognoz daha kötü saptanırken, vücut kitle indeksi ˃25 ve sigara kullanım öyküsü olan hastalarda prognoz daha iyiydi. 3.ayda kötü prognoza sahip hasta oranı tüm populasyonda % 27.5; 3.ay mortalite oranı ise % 18.75 olarak saptandı. 270 dakikanın altında başvuran hastalarda prognoz, 270 dakikanın üzerinde başvuran hastalara kıyasla daha iyiyken, trombolitik tedavi uygulanan hastalarda 3.ayda iyi fonksiyonel sonuç, uygulanmayanlara göre farklı bulunmadı. Duysal semptomla başlayan hastalarda semptom-kapı süresinin anlamlı derecede uzun olduğu, motor semptomların ise semptom – kapı zamanını kısalttığı görüldü. Hastane içi faktörlerden kapı – beyin görüntülemesi ve kapı – anjiografi süreleri semptom – kapı zamanı 270 dakikanın altında olanlarda daha düşük bulundu. Ayrıca, yatış süresi uzun olan, ilk gün ortalama kan şekeri 120 mg/dL, vücut sıcaklığı 36.7 C º'nin üzerinde olan hastaların prognozu kötü seyretti. Sonuç olarak, bu çalışmada, akut iskemik inme hastalarında hastane öncesi faktörlerden inmenin duysal semptomla başlaması prognozu kötü yönde etkilerken, uyarıcı motor semptomun prognozu iyi yönde etkilediği görüldü. Hastane içi faktörlerden ise yatış süresinin uzun olması, kan şekeri kontrolünün sağlanamaması ve yüksek vücut sıcaklığı kötü prognozla ilişkili saptandı. Background: Stroke is a disease that becomes increasingly important socioeconomically in aging societies. According to the data of World Heath Organization (WHO, 1990), stroke is the second leading cause of death worldwide, third leading cause in developed countries. In addition, stroke is an important cause of long term disability. Increase in the incidence of stroke with decreased stroke-related mortality caused high prevalence of stroke and disability, with the consequences of serious emotional and socioeconomic problems for the stroke patients, care givers, families, and the health sector. In addition, it was observed that there is an increase in stroke-related hospitalization rates nowadays. There has been an increased rate in hospitalization, where the rates of mortality an in-hospital periods have been decreased. This supports the concept that the treatment of acute stroke stroke is more effective than the preventive measures. Administration of intravenous recombinant tissue-type plasminogen activator as soon as possible to eligible patients is the basis of the stroke treatment. Providing blood flow as soon as possible is effective on long term morbidity in patient with ischemic stroke. Therefore, most of trials have been focused on recanalization and reperfusion.Purpose: The goal of the study is to explore how prehospital and inhospital factors influence the prognosis of patients with acute ischemic stroke.Materials and Method: Our study is a cohort study to determine the impact of prehospital and inhospital factors on prognosis. 80 patients over the of age 18 were included, admitted to Medical Faculty of Dokuz Eylul University, Emergency Department within the first 24 hours of the symptoms. Diagnosis of acute ischemic stroke was based on clinical, biochemical and diagnostic imaging (Computed Tomography (CT) and/or Magnetic Resonance Imaging (MRI)) findings. Patients were included between June 2015 and August 2015. Another comorbid state is considered as an exclusion criteria. Patients have been evaluated within the first 24 hours, third day, during discharge and after third month.Results and conclusion: The prognosis was poorer in patients above age 75. Patients with Body Mass Index (BMI) above 25 and with smoking history had better prognosis. Rate of patients with poor prognosis was 27.5% in all population, with the mortality rate of 18.75% in third month. While prognosis of patients applied earlier than 270 minutes was better than patients those applied later than 270 minutes, funcitonal outcomes of patients who have given trombolytic therapy was not different from patients who have not recieved this therapy, after third month. It was observed that symptom-door time was significantly longer in patients who experienced sensorial disturbances as the first symptomes. Motor symptom as warning signs caused shorter sympthom-door times. It has been found that door-to-brain imaging and door-to-angiography times were shorter in patients whose symptom-door time was shorter than 270 minutes. Prognosis was poor in patients with long length in hospital, and with blood glucose level above 120 mg/dl and the body temperature above 36.7°C in the first day.In conclusion, patients with acute ischemic stroke had poor prognosis when stroke began with sensory symptoms. Motor symptoms as warning sign had positive effects on prognosis. Longer length of stay in hospital, unregulated blood glucose, higher body temperature had negative effects. 65
