1. Diabetes Text-Message Self-Management Support Program (SMS4BG): A Pilot Study

Abstract

BackgroundThe increasing prevalence of diabetes and costly long-term complications associated with poor glycemic control are issues facing health services worldwide. Diabetes self-management, with the support of health care providers, is critical for successful outcomes, however, frequent clinical contact is costly. Text messages via short message service (SMS) have the advantage of instant transmission at low cost and, given the ubiquity of mobile phones, may be the ideal platform for the delivery of diabetes self-management support. A tailored text message-based diabetes support intervention called Self-Management Support for Blood Glucose (SMS4BG) was developed. The intervention incorporates prompts around diabetes education, management, and lifestyle factors (healthy eating, exercise, and stress management), as well as blood glucose monitoring reminders, and is tailored to patient preferences and clinical characteristics. ObjectiveTo determine the usability and acceptability of SMS4BG among adults with poorly controlled diabetes. MethodsAdults (aged 17 to 69 years) with type 1 (n=12) or type 2 diabetes (n=30), a hemoglobin A1c (HbA1c) over 70 mmol/mol (8.6%), and who owned a mobile phone (n=42) were recruited to take part in a 3-month pilot study of SMS4BG. At registration, participants selected the modules they would like to receive and, where appropriate, the frequency and timing of blood glucose monitoring reminders. Patient satisfaction and perceptions of the usability of the program were obtained via semistructured phone interviews conducted at completion of the pilot study. HbA1c was obtained from patient records at baseline and completion of the pilot study. ResultsParticipants received on average 109 messages during the 3-month program with 2 participants withdrawing early from the study. Follow-up interviews were completed with 93% of participants with all reporting SMS4BG to be useful and appropriate to their age and culture. Participants reported a range of perceived positive impacts of SMS4BG on their diabetes and health behaviors. HbA1c results indicated a positive impact of the program on glycemic control with a significant decrease in HbA1c from baseline to follow-up. ConclusionsA tailored text message-based intervention is both acceptable and useful in supporting self-management in people with poorly controlled diabetes. A randomized controlled trial of longer duration is needed to assess the efficacy and sustainability of SMS4BG.

9. The constitution of transgender masculinities through performance : a study of theatre and the everyday

Abstract

This doctoral project is concerned with gender and the way that transgender masculinities are manifested, articulated and debated through drama, theatre and performance. The central question of the research is how `performance' contributes to the process of constituting individual identities and communities, specifically transgender masculinities. The research engages with the multiple ways that the concepts or categories of the individual, of community and of performance are defined, and how they function and are experienced when transgender identities or transgender masculinities are central to a `performance event'. The particular individuals and groups of transgender-identified people, or people who might be described in relation to a trans framework of identity, are those for whom gender is not a fixed state rooted in a binary system, but a state that can be bent, moved or made malleable in order to fit according to individual need. The individuals and groups on whom I focus tend to have had their sex assigned female at birth and at some point in their lives have identified themselves as male rather than female. There are also individuals who do not self-identify as male but refute gender categorisation, thereby not identifying as female either. Moreover, there are people who still self-identify as female but have developed or produced markers of masculinity on their body that have a significant impact on their day-to-day living and in their performance work. In this thesis I will be referring to this range of varied identities as transgender masculinities. This research will be of relevance to contemporary theatre scholars, particularly those with an interest in the creation of avant-garde and community-generated practices. The research will also be of use to those interested in queer and non-normative identities as manifested through drama, theatre and performance, whether this is by solo artists or within project work with groups of people who identify as transgender, genderqueer or have an otherwise complex relationship to gender.

10. Celebrating the 'Intellectual Heart': Emergent Meanings from a Post-Earthquake Organizational Communication Culture Perspective.

Abstract

The Organizational Communication Culture (OCC) method, designed by Charles A. Bantz (1993) was utilized as the primary method for analyzing messages coming from the administration of California State University, Northridge, after the earthquake. Bantz feels that communication interactions in an organization bring meanings and expectations to that organization and that by analyzing these symbolic forms, the researcher has some insight into the culture of the group being studied. Results of the study were organized around three key constructs: harmony, family community, and spirit/dedication. It is important to note, however, that no single construct is representative of the entire culture of the university--this study concerns only the administration's vision as it is represented in its documents. First, the documents emphasized team work and collaboration as though the harmony displayed among the participants in the project reorganization were representative of an organism back in harmony with nature and not at odds with nature. Second, the university became a refuge or family for students who had lost their homes or were in some way displaced by the earthquake. Third, letters from the president's office explained, defined, and expressed how the slogan, "Not Just Back...Better" reflected an attitude of what the administration was trying to do. 11. Optimisation of catalytic systems using design of experiment methodologies

Abstract

A Sharpless ligand with a novel triazene core was synthesised in two simple, high yielding steps from cheap, readily available starting materials. The ligand showed good selectivity in the asymmetric dihydroxylation of alkenes, and its optimisation was undertaken in collaboration with Dr D.C. Woods and Professor S.M. Lewis of Southampton University's Statistical Sciences Research Institute. Using a number of statistical designs and experimental methods, the results showed that the conditions reported and currently used were optimal. In the process of the investigation a greater understanding of the factors affecting the reaction was obtained. In a second example, C-C bond formation was optimised using a supported palladium catalyst. Four different sulfur containing pincer ligands were synthesized and attached to four different solid supports. Subsequent complexation with palladium gave 16 different polymer supported catalysts which were active catalysts in the Heck reaction.  The optimisation of this system with regards to both conversion and palladium leaching was undertaken using the experimental design software, MODDE which showed that solvent had the largest effect on both responses. The cHexyl thiol moiety was superior to its phenyl counterpart which is most commonly adopted. In the Heck reaction of 4-bromo acetophenone the conversion was improved from 30% with large observable leaching to 88% with no observable leaching varying 6 factors corresponding to 5760 possibilities.

12. Early metabolic predictors for the development of Type 2 diabetes in 1st degree relatives : results of a 10-year follow-up study (1984-1994), together with cross-sectional analyses in 1994

Abstract

The relative contributions of reduced insulin sensitivity and beta-cell dysfunction to the pathogenesis of Type 2 diabetes are unknown. In addition, the influence of islet amyloid polypeptide (IAPP) and proinsulin secretion upon disease expression have not been fully characterised. In 1994 a 10 year-follow-up study of 96 previously non-diabetic, 1st degree relatives of Type 2 diabetic subjects was undertaken, with a physiological test-CIGMA(Continuous infusion of glucose with model assessment). The aims were to determine the conversion rate to diabetes, together with the most sensitive baseline predictors for disease progression. Results showed a cumulative prevalence for diabetes of 29% in the case of the siblings by mean age 60(10)y with an additional 8% having Impaired Fasting Glucose (fasting plasma glucose 6.1mmol/1 (IFG), and in the offspring, 10% by mean age 44(8)y with an additional 5% having IFG. The major predictor for conversion to diabetes was the degree of fasting glycaemia at baseline. Neither beta cell function nor insulin sensitivity were predictive. In a separate study, cross-sectional analyses of plasma IAPP and proinsulin concentrations in 58 non-diabetic relatives, 19 control subjects and 39 Type 2 diabetic patients, showed both peptides increased proportionally to C-peptide from normoglycaemia (FPG < 5.5mmol/l) to IFG. Once diabetes was established, disproportionate hyperproinsulinaemia occurred, particularly in relation to the lower C-peptide concentrations in the insulin treated diabetics. IAPP levels were highest in relatives with IFG and in the diet and tablet treated diabetics and, like the C-peptide levels were significantly reduced in the insulin treated group. In summary, this study confirms the high prevalence and incidence of Type 2 diabetes in 1st degree relatives of white Caucasian diabetic patients. Raised fasting plasma glucose was the most sensitive predictor of diabetes. It is unlikely that routine assessment of beta-cell function or insulin sensitivity will give clinically useful information indicating which patients with IFG are at greater risk of developing diabetes. Disproportionate secretion of proinsulin did not occur until diabetes was established and is likely to be a secondary phenomenon.

Abstract

This short piece discusses a project on Child Criminal Exploitation (CCE),County Lines, gangs and youth violence for which I am Project Lead. In working in partnership with Active Communities Network the aim is to complement the multi-agency work already in place in the region of Hampshire. The teamincludes artists, students, academics and professionals in policing, safeguarding and youth offending. The project exchanges knowledge in order to create a resource that will meet the needs of all partners in their mission to raise awareness and prevent children from being exploited.

Abstract

Effectiveness in education is a national concern and reform efforts continue to be championed with the hope of stimulating improvement to more effectively meet the needs of all students. Many reform efforts include a focus on teacher professional development to strengthen teacher pedagogy and positively impact student achievement. Rapid expansion and increased use of technology have created opportunities that never existed before to support teachers. Online, as an alternative to traditional face-to -face professional development, is currently underutilized in K-12 school districts. This study intended to investigate successful models of online professional development to inform district decision-making at the local level. K-12 teachers from fifteen states, who participated in a successful online professional development experience, were invited to complete a survey to provide insight about how a professional development experience positively impacted their teaching, learning, and classroom. Of the 328 teachers who responded, three people were interviewed to learn more about their particular online professional development experience. Study findings show that participants highly value online professional development because of the convenience, accessibility, and ability to self-pace and differentiate learning. Online professional development has the unique potential to foster reflection, deep thought, and analysis particularly when there is the potential to interact with an online professional learning community over time. A significant finding is that teachers thrive on the interaction and sharing of ideas between colleagues in job -alike situations. Additionally, findings show that when teachers participate with face-to-face school or grade- level teams online, they reap even greater learning benefits. The majority of participants found that online professional development helped them improve their knowledge of curriculum and instructional strategies and improved their technology skills. They learned ways to improve their classrooms to meet the needs of diverse student populations and helped validate effective practices that were already firmly in place. Educational leaders should consider online professional development as a strong viable option to improve teacher practice. Findings suggest that district leaders should recommend State Department of Education and commercial online learning opportunities that are subject-area focused and aligned to district/school goals. They should encourage teacher teams to participate together online and face-to- face to deepen learning. By recommending specific online learning experiences and encouraging teachers to participate together, districts will be well served by the effective online professional development offerings that are available

Abstract

The study objective was to measure dental caries experience in the cleft lip and/or palate population in the West of Ireland by undertaking a retrospective review of clinical records of all cleft patients attending between 2000 and 2016. The main outcome measures were dmft/DMFT index, ethnicity, age, gender, cleft type, medical, social and family history, preventative dental measures provided. One hundred and twenty patients were identified: 116 Caucasian and 4 non-Caucasian. Age range: 7 months to 17 years. Male:Female 56:64. Cleft lip and palate (CLP): Cleft palate (CP) 52:68, Male:Female CLP 30:22, M:F CP 26:42, Syndromic: Non-Syndromic 19:101. Mean age at first dental visit 2.6 years. Dentally fit: 96.7%. Zero dmft/DMFT 66%. Mean dmft/DMFT 1.57 (range 0-18). With a paediatric dental service in existence for 21 years, the dmft/DMFT has been significantly reduced (1.57). Where caries did present, it was related to late referral.

This complication has been reported in acne vulgaris and other systemic metabolic disorders. This paper presents a rare case of calcinosis cutis in a 14-year-old male which was found at a routine orthodontic assessment.

Abstract

BACKGROUND: Addressing the increasing prevalence, and associated disease burden, of diabetes is a priority of health services internationally. Interventions to support patients to effectively self-manage their condition have the potential to reduce the risk of costly and debilitating complications. The utilisation of mobile phones to deliver self-management support allows for patient-centred care at the frequency and intensity that patients desire from outside the clinic environment. Self-Management Support for Blood Glucose (SMS4BG) is a novel text message-based intervention for supporting people with diabetes to improve self-management behaviours and achieve better glycaemic control and is tailored to individual patient preferences, demographics, clinical characteristics, and culture. This study aims to assess whether SMS4BG can improve glycaemic control in adults with poorly controlled diabetes. This paper outlines the rationale and methods of the trial. METHODS/DESIGN: A two-arm, parallel, randomised controlled trial will be conducted across New Zealand health districts. One thousand participants will be randomised at a 1:1 ratio to receive SMS4BG, a theoretically based and individually tailored automated text message-based diabetes self-management support programme (intervention) in addition to usual care, or usual care alone (control). The primary outcome is change in glycaemic control (HbA1c) at 9 months. Secondary outcomes include glycaemic control at 3 and 6 months, self-efficacy, self-care behaviours, diabetes distress, health-related quality of life, perceived social support, and illness perceptions. Cost information and healthcare utilisation will also be collected as well as intervention satisfaction and interaction. Abstract

The prison population experiences significant health inequality and social exclusion (Department of Health 2002). Once incarcerated, prisoners are at increased risk of mental illness and have higher exposure to communicable diseases (WHO 2008). Prisoners generally have poorer health because imprisonment negatively impacts upon the health of the individual (WHO 2008). However, the prison setting offers a unique opportunity to implement targeted health promotion initiatives with a population that may have had very limited experience of accessing health services prior to incarceration. The World Health Organisation recognises the need for health promotion in prisons. This is evidenced in the Health in Prisons Project (HIPP), the aim of which is to support member states in improving health and health care in prisons, and to facilitate the links between prison health and public health systems at both national and international levels. In practice however, implementation of the HIPP is complex. This paper examines the challenges in promoting health in prisons globally and in Irish prisons specifically, given that the prison setting is omitted from the Irish National Health Promotion Strategy. Based on a qualitative research design that incorporated semi-structured interviews with a purposive sample of twelve leading figures in prison policy in Ireland, this paper discusses the need for health promotion interventions in Irish prisons. The paper also discusses the influence of organisational culture on attitudes of prison staff towards engaging in health promotion with prisoners and explores the problems posed by lack of national policy and funding for health promotion initiatives in the prison setting.
