Abstract

We measure the nonlinearity of a telecom-wavelength superconducting nanowire single-photon detector via incoherent beam combination. At typical photon count rates and detector bias current, the observed relative deviation from a perfectly linear response is in the order of 0.1% when the flux is doubled. This arises from a balance between the counteracting nonlinearities of deadtime-induced detector saturation and of multi-photon detections. The observed behaviour is modelled empirically, which suffices for a correction of measured data. In addition, statistical simulations, taking into account the measured recovery of the detection efficiency, provide insight into possible mechanisms of multi-photon detection., Comment: 9 pages, 8 figures

Abstract

RATIONALE & OBJECTIVE: Large differences between estimated glomerular filtration rate (eGFR) based on cystatin C (eGFRcys) and creatinine (eGFRcr) occur commonly. A comprehensive evaluation of factors that contribute to these differences is needed to guide the interpretation of discrepant eGFR values. STUDY DESIGN: Cohort study. SETTING & PARTICIPANTS: 468,969 participants in the UK Biobank. EXPOSURES: Candidate sociodemographic, lifestyle factors, comorbidities, medication usage, and physical and laboratory predictors. OUTCOMES: eGFRdiff, defined as eGFRcys minus eGFRcr, categorized into 3 levels: lower eGFRcys (eGFRdiff, less than -15 mL/min/1.73 m2), concordant eGFRcys and eGFRcr (eGFRdiff, -15 to 300 mg/g. Odds ratio estimates for these predictors were largely inverse of those in the lower eGFRcr group. The models area under the curve was 0.75 in the validation set, with good calibration (1.00). LIMITATIONS: Limited generalizability. CONCLUSIONS: This study highlights the multitude of demographic, lifestyle, and health characteristics that are associated with large eGFRdiff. The clinical model may identify individuals who are likely to have discrepant eGFR values and thus should be prioritized for cystatin C testing.

Abstract

Abstract Background The Coronavirus disease (COVID-19) pandemic caused significant morbidity and mortality in Africa, in addition to other socio-economic consequences. Across the continent, Schools of Public Health (SPHs) played several roles in supporting national, regional, and global response to the pandemic. Following a published and grey literature search, this paper reviews and analyses the contribution of SPHs in Africa during the COVID-19 pandemic. Contribution of the Schools of Public Health SPH faculty in most countries contributed their expertise through COVID-19 task forces and advisory committees where they guided and supported decision-making. Faculty also supported the identification, review, and synthesis of rapidly evolving global and local evidence, adapting it to the local context to guide policy decisions. Through research, SPHs contributed to a better understanding of the disease epidemiology, response interventions, as well as prevention and control measures. SPHs engaged in training field epidemiologists, frontline health workers, and district response teams. SPH staff, students and field epidemiology trainees also supported field activities including surveillance, contact tracing, as well as managing quarantine facilities and points of entry. SPHs engaged in public education and awareness-raising initiatives to share information and dispel misinformation. In partnership with other stakeholders, SPHs also developed important innovations and technologies. Conclusion SPHs are a critical pillar for pandemic prevention, preparedness, and response, that support health systems with important functions. To further enhance their capacity, efforts to improve coordination of SPHs, strengthen collaboration among schools, harmonize training and curricula, and enhance capacity for advanced research are needed. There is also a need to bridge the inequities in capacity and resources that exist among SPHs across regions and countries.

Abstract

The scaling of Transformers has driven breakthrough capabilities for language models. At present, the largest large language models (LLMs) contain upwards of 100B parameters. Vision Transformers (ViT) have introduced the same architecture to image and video modelling, but these have not yet been successfully scaled to nearly the same degree; the largest dense ViT contains 4B parameters (Chen et al., 2022). We present a recipe for highly efficient and stable training of a 22B-parameter ViT (ViT-22B) and perform a wide variety of experiments on the resulting model. When evaluated on downstream tasks (often with a lightweight linear model on frozen features), ViT-22B demonstrates increasing performance with scale. We further observe other interesting benefits of scale, including an improved tradeoff between fairness and performance, state-of-the-art alignment to human visual perception in terms of shape/texture bias, and improved robustness. ViT-22B demonstrates the potential for "LLM-like" scaling in vision, and provides key steps towards getting there.

9. Investigation of the effect of chlorine in different additives on dioxin formation during high temperature processing of iron ore

Abstract

Ironmaking can have an important role in the circular economy by utilising alternative carbon bearing materials, which are otherwise of low value in use or landfilled. Biomass and plastic wastes have been proposed as alternatives for coke and coal in sintering, blast furnace ironmaking and direct iron ore reduction processes. However, introduction of the alternative carbon sources with higher chlorine contents may influence the formation of some pollutants, most notably, dioxins. This study investigated the effects of carbon bearing materials, i.e., coke, biomass and polyvinyl chloride (PVC), on formation of polychlorinated dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs) and polychlorinated biphenyls (PCBs) during high temperature processing of iron ore. The effect of copper (Cu) as a known dioxin synthesis catalyst, on formation of the dioxins was also investigated. The study found an exponential increasing relationship between the chlorine content in the fuel source and the dioxin emissions. The addition of both biomass and PVC materials contributed to significantly increased dioxin formation, especially for the case of plastics. PCBs had higher concentrations than PCDD/Fs in all cases except when CuCl2 was added due to its catalytic properties to synthesise PCDD/Fs. PCDF concentrations far outweighed the PCDDs, suggesting that de novo reaction mechanism, where carbon particles are oxidised by a transition metal in presence of chlorine, dominated the formation of the dioxins in the process.

23. Using virtual experiences to facilitate refugees' integration in third countries

Abstract

Virtual Experiences (VEs) have significant potential for a wide range of applications, including the potential to enrich the level of emotional interactions and socialisation, facilitate communication, treat phobias and PTSD, eliminate biases, deepen our understanding of ourselves and others, and enhance our appreciation of our environment. They also have the immense ability to elicit knowledge transfer and to change attitudes and behaviour. This research investigates whether VEs can play a key role in assisting refugees to learn how to navigate the complexities of the UK health system, a substantial stumbling block for their integration into society and for their own health and well-being. First, a pilot study involving University of Warwick students was conducted. During this study, the participants (N = 37) were exposed to material presented via three different media. The three media were descriptive text, a 360_video and a Virtual Environment (VE). All three media used in the experiment depicted a surgery in London. The experiment investigated whether any of the three media was superior in achieving knowledge transfer and whether there were specific attributes of the medium that could facilitate knowledge transfer. The results showed important tendencies and correlations between knowledge transfer and interactiveness of the media, with VE significantly superior across the four tested attributes. Furthermore, the results for the 360_video, demonstrated that the quality of the image was an important factor in achieving participants' appreciation of knowledge transfer. A second experiment build on these initial results. The participants (N=122) in this experiment were refugees in Senegal awaiting, what the UN Refugee Agency calls, "durable solutions" (repatriation to their country of origin, resettlement to third countries, or local integration in the country of asylum). They were exposed to material presented via three different media. These included a descriptive text, 360_images (superior to the previous 360_video) and a VE using a multi-factor approach where two of the three different media (360 and VE) were further sub-divided into six different factors. In both studies, we tested the hypothesis that interactive experiences have a greater propensity to elicit knowledge transfer because Using Virtual Experiences to Facilitate Refugees' Integration in Third Countries of their psychological and physiological effect on areas of the brain, which are crucial for recollection-based memories, memory for spatial and episodic context, and scene perception. An assessment of participants appreciation of the information provided was done at the end of both experiments. The results show that interactive media, VE, was significantly better across all tested attributes. This suggests that VEs do indeed provide a far better understanding of a complex problem, such as navigating the UK health system. UNHCR is already looking at innovative ways to help refugees in their quest for durable solutions and future work will look to deploy such VEs at a number of UNHCR offices.

24. Reception of John Chrysostom in the Middle Byzantine period (9th-13th centuries) : a study of the Catechetical homily on Pascha (CPG 4605)

Abstract

John Chrysostom was the most prolific Byzantine Church father, reaching high levels of prominence in the Byzantine Church for both his rhetorical prowess and spiritual instruction. Nevertheless, the process itself of the development of his cult in Byzantium has never been systematically studied. Moreover, one of the single most popular texts attributed to him, the so-called Catechetical Homily on Pascha, has also never been systematically analysed, despite scholarly insistence for centuries that the attribution to Chrysostom must be mistaken. The present dissertation addresses these two gaps in research together. Part 1 offers the first ever critical edition of the Catechetical Homily on Pascha, as well as the first edition of a related Byzantine text by the boisterous Constantinopolitan abbot, Theodore of Stoudios. The primary manuscript witnesses to the Catechetical Homily, and all related texts, are presented in detail and the relationships between these texts are investigated in an effort to bring to light as much of the history behind the Catechetical Homily as possible from the point of view of the manuscripts preserving it. Part 2 builds on the research in Part 1 by examining the context of John Chrysostom's Byzantine reception and the people involved in the process. As noted above, Theodore of Stoudios plays a critical role in 9th century Byzantium. Part 2 begins by tracing the major lines of Chrysostom's reception from his death in the early 5th century until the time of Theodore of Stoudios' birth in the mid-8th century. From there, it investigates Theodore's understanding of Chrysostom, his use of the Catechetical Homily among other aspects of the heritage attributed to Chrysostom, and how Theodore's promotion of a particular interpretation of Chrysostom sparked a chain of receptions that stretched over the course of centuries. The chapters in Part 2 all address aspects of this chain of receptions, beginning with Theodore's use of the Catechetical Homily in the 9th century and stretching to the early 13th century, when Neophytos Enkleistos, a recluse living on Byzantine and then Lusignan Cyprus, imitates Theodore, although under different circumstances. This thesis focuses on the reception of John Chrysostom during the middle Byzantine period, 9th-13th centuries, through the lens of the so-called Catechetical Homily on Pascha (CPG 4605). The earliest surviving attribution of this text to John Chrysostom comes from the monastic leader, Theodore of Stoudios in the 9th century. Theodore began the tradition that has continued over a millennium up to the present of reading the Catechetical Homily on Pascha at the Easter vigil service. In this way, the text became one of the most frequently copied and well known ascribed to Chrysostom. The text has two indirect textual traditions, as well: one from Theodore of Stoudios and the other from Neophytos Enkleistos in the late 12th-early 13th centuries. Both abbots have left paschal homilies in which they incorporate the text of the Catechetical Homily on Pascha. Part 1 of this thesis gives the first critical edition of the Catechetical Homily on Pascha, as well as of Theodore of Stoudios' Oratio IV, incorporating the former. Neophytos' text already has a critical edition. Principal manuscript witnesses are presented and a stemma codicum is offered, while intertextual relationships are extensively discussed. Part 2 of the thesis focuses on the historical context of the Catechetical Homily on Pascha during the middle Byzantine centuries. A chain of receptions is traced that were inspired by, and responded to, Theodore of Stoudios' 9th century understanding and deployment of Chrysostom's heritage. In each century the image of Chrysostom played a slightly different role, building on what had preceded, while also innovating in response to changed circumstances and agendas of the political and religious actors of the time. Thus, each chapter of Part 2 focuses on one of the centuries from ca. 800-1204, complementing the critical edition of the text given in Part 1.

30. Influence of iron ore properties on dioxin emissions during iron ore sintering

Abstract

Abstract Iron ores are principal input materials for iron and steel-making industries. Quality of iron ores is one of the critical parameters for formation of environmental pollutants related to the steel-making process. Dioxins are identified as one of the most toxic pollutants emitted during ironmaking, specifically during the sintering process. This study applied four types of iron ores and analyzed their moisture, density, particle size distribution and element concentrations to investigate their effect on the dioxin formation during sintering. Each type of iron ore was processed in a sinter pot grate. During each processing route, exhausted dust and generated sinter products were collected and subjected to PCDD/F and PCB analysis. Statistical analysis was applied to assess correlations between properties of iron ores and exhausted dioxin emissions, identifying key contributors to dioxin formation during sintering process. Results showed that Fe in iron ores was positively and significantly related to PCB 114 formation in dust and confirmed its co-catalytic effect on dioxin formation. Concentrations of Al, Ti and Cl in iron ores greatly increased PCDD/F and PCB emissions in the sintered products compared to dioxins in dust samples. The S levels and density of iron ores were highly related to the increasing PCDD/F and PCB emissions in both sinter and dust samples. Abstract

Background Prior nonrandomized studies have suggested nephroprotective effects of arteriovenous fistula (AVF) formation, but these are plausibly susceptible to immortal time and selection biases. Methods We studied patients attending nephrology clinics in the West of Scotland during 2010–22 with an estimated glomerular filtration rate (eGFR) ≤15 mL/min/1.73 m2 and no prior AVF. Using target trial emulation and a sequential trial design, we simulated a hypothetical trial that would randomize patients to either undergo AVF formation immediately or not to undergo AVF formation. The primary outcome was the difference in eGFR slope for the first 6 months of follow-up, estimated using a mixed-effects model. The secondary outcomes were 5-year absolute risks of dialysis and death, estimated using the Aalen–Johansen and Kaplan–Meier estimators respectively. Results A total of 1364 unique patients (mean age 51.1 years, 55.7% male) contributed 3125 person-trials, with 561 in the AVF and 2564 in the no AVF group. Mean eGFR was 12.6 mL/min/1.73 m2 and the median number of eGFR measurements per person-trial was 7 (interquartile range 4–12). Slope of eGFR decline did not differ significantly between the AVF and no AVF groups (between-group difference –0.67 mL/min/1.73 m2/year, 95% CI –1.43, 0.10). The 5-year absolute risk of dialysis was 87% (95% CI 84, 91) in the AVF group and 75% (95% CI 73, 77) in the no AVF group, and the 5-year survival probability was 77% (95% CI 70, 83) in the AVF group and 67% (95% CI 64, 69) in the no AVF group. Conclusions In this study of patients with advanced chronic kidney disease, there was no evidence of a nephroprotective effect of AVF formation. [ABSTRACT FROM AUTHOR]

Abstract

Background Sodium zirconium cyclosilicate (SZC) reduces serum potassium in patients with chronic hyperkalaemia in clinical trials, but its role in the emergency treatment of hyperkalaemia is unproven. We hypothesized that SZC use for emergent hyperkalaemia would be associated with a reduction in rates of emergency interventions for hyperkalaemia. Methods This was a single-centre, propensity score–weighted case–control study of patients admitted with hyperkalaemia to a specialist renal centre. We randomly selected 250 patients admitted between April 2021 and September 2022 (post-SZC era) with a potassium level ≥5.5 mmol/l treated with at least one ≥10 g dose of SZC (treatment group). We randomly selected a comparator group of 250 patients admitted between January 2018 and December 2019 (pre-SZC era) with a potassium level ≥5.5 mmol/l (control group). Baseline demographic and clinical characteristics were recorded and used as covariates for propensity scoring and inverse probability treatment weighting (IPTW). Our primary outcome measure, rates of emergency haemodialysis (HD), was tested using unadjusted models and multivariable logistic regression models on unweighted data in addition to unadjusted models on weighted data. We also reviewed rates of emergency temporary central venous access as a secondary outcome. Results A total of 59% were male, the mean age was 67 years (standard deviation 14) and 149 (30%) were receiving maintenance dialysis. IPTW achieved satisfactory balance of covariates between the treatment and control groups. In the treatment group, patients were 77% less likely to need emergency HD {odds ratio [OR] 0.23 [confidence interval (CI) 0.17–0.31]}. This result was consistent following analysis of weighted and unweighted data. Similarly, patients treated with SZC were 73% less likely to require emergency temporary central venous access [OR 0.27 (CI 0.20–0.36)]. Conclusion SZC was associated with a significant reduction in the rates of emergency HD and emergency temporary central venous access in patients admitted to a specialized renal centre with emergent hyperkalaemia.
