Objective: The mandibular advancement device (MAD) is currently suggested in patients with mild to moderate obstructive sleep apnea (OSA). This study investigated the effects of a new fully customizable MAD-type device called, "It Makes You Sleep" (IMYS), in patients with mild to moderate OSA. Methods: Sixteen patients (14 men and 2 women; mean age 62; SD ± 11 years) were retrospectively enrolled. Each patient received home sleep apnea testing (HSAT) at baseline (T0) and after three months (T1) of IMYS treatment. The Apnea-Hypopnea Index (AHI), the Oxygen Desaturation Index (ODI), the Minimum Oxygen Saturation (minSO2) and the Medium Oxygen Saturation (medSO2) were analyzed. Results: From T0 to T1, the IMYS treatment showed a significant reduction of AHI and ODI and a significant increase of minSO2 and medSO2. To observe the biomechanical characteristics of the muscles and TMJ during orthodontic appliance wearing and the retention phase, two different protocols were used: Model 1: The mandibular advancement using FCA; Model 2: The mandibular advancement using Class II elastics. Results: The FCA group produced greater and more coordinated masticatory muscle forces compared to the Class II elastics group. Temporal and masseter muscles exhibited the most pronounced variation in muscle strength during mandibular advancement. The FCA group exhibited greater TMJ region stress compared to the Class II elastics group. Interestingly, the stress on the articular discs in both models decreased over time. Tensile stresses were observed in both the condyle and the posterior region of the articular fossa. Conclusion: During skeletal Class II malocclusion treatment, masticatory muscle forces and stress on the TMJ were higher in the FCA group compared to the Class II elastics group. Materials and methods: A systematic literature search was conducted across PubMed, Cochrane Central, Web of Science, Embase, Scopus databases, Chinese Biomedical Database, Chinese National Knowledge Infrastructure, and Wanfang. The research involved children and adolescents (under 16 years old) who received mandibular advancement and maxillary expansion functional orthopedic appliances for OSA treatment. We performed narrative reviews and subsequently amalgamated the findings from the studies. Results: Six articles were included for review. Although a small number of studies were included, the research suggested the potential advantages of mandibular advancement for children with OSA. Following treatment, there was a decrease in AHI/RDI, an improvement in sleep quality, and the increase in oxygen saturation. Conclusions: The limited quantity and quality of existing studies necessitate caution when drawing conclusions about the effectiveness of mandibular advancement and maxillary expansion for OSA. In the future, larger and well-designed randomized controlled trials (RCTs) are needed to provide more robust evidence. Patients should be carefully selected, and their orthodontic indications should be thoroughly evaluated before inclusion in such trials.We encourage researchers to design studies that monitor patients over several years to provide a comprehensive understanding of the long-term effectiveness. Background and Objectives: The most popular surgical procedures among orthognathic surgeries for Class II and III patients are Le Fort 1 osteotomy for the maxilla and bilateral sagittal split ramus osteotomy (BSSRO) for the mandible. Keeping the condyle in its proper place during fixation is one of the difficulties of orthognathic surgery. One of the worst post-orthognathic surgery consequences in the temporomandibular joint (TMJ) area may be condylar resorption. Condylar remodeling refers to a group of processes that occur in reaction to forces and stress placed on the temporomandibular joint in order to preserve morphological, functional, and occlusal balance. A systematic review of the literature was performed with the aim of identifying the mandibular condylar component of TMJ changes after orthognathic surgery in class II and III patients. Materials and Methods: An electronic search was carried out using the PubMed, Cochrane Library, and Google Scholar, databases. The inclusion criteria included trials in non-growing patients upon whom orthognathic surgery was performed due to Angle II or Angle III classes malocclusion; in addition, a CT or cone beam computed tomography (CBCT) scan was performed before and after surgery to track the mandibular condylar component of TMJ changes. The quality of the studies was evaluated by two independent authors. The risk of bias was assessed by using the Downs and Black checklist. Results: The electronic and manual literature search yielded 12 studies that fulfilled all necessary inclusion criteria. Observed studies were evaluated as good (3), fair (8), and poor (1) quality. Two studies evaluated class II patients, six studies observed class III patients, and four studies were comparative. Most of the studies evaluated condyle angle and space changes, and the condylar surface and volume changes were also observed. However, the methodology of evaluation in the publications differs. Conclusions: Reduction of bone density, especially in class II patients, and morphological condyle reshaping, with the apposition of the bone, is the main adaptive mechanism after orthognathic surgery. Objective: To retrospectively evaluate the effects of single mandibular advancement (MA) and two-jaw surgery (2J-S) on the pharyngeal airway space (PAS) and hyoid position for the correction of skeletal Class II malocclusion. Methods: Eleven adult patients who underwent only MA surgery and twelve adult patients who underwent Le Fort I maxillary impaction-MA surgery (2-JS) were included in the retrospective study. A total of 46 cephalometric recordings obtained before (T1) and after treatment (T2) were examined. Craniofacial changes, area, and linear measurements of the pharyngeal airway and hyoid bone position were obtained in both groups. The article presents a case of a 13‐year‐old adolescent male patient who started orthodontic treatment at the age of 12. Before treatment, he was diagnosed with narrow maxilla, proclination of upper incisors, deep overbite, distal occlusion bilaterally with significant sagittal overjet in frontal area, skeletal Class II, and hypodivergent growth pattern. During treatment, the patient is in his pubertal growth spurt. About 2 months after intermaxillary Class II elastics (1/4 heavy, 6.5 Oz) were applied, he complained of pain during mastication, wide opening of the mouth, and sometimes during protrusive and lateral movements in the right TMJ. The TMJ X‐ray examination did not reveal abnormal morphological changes. Occlusion was evaluated by an electromyographic device, Teethan. The result was typical for Class II malocclusion. During the bilateral palpation of the zones of TMJ and opening of the mouth and chewing, the patient reported pain on the right side. There was no clicking in the joint. The elastic wear was stopped, and soon afterwards, the pain disappeared. These complaints point to a possible relationship between orthodontic treatment and TMJ pain. However, the disappearance of complaints after the removal of the Class II elastics points that the temporomandibular joint disorder (TMD) symptoms are reversible and resolved. The aim of this study was to examine the relationship between the presurgical maxillomandibular transverse index and post-surgical stability one year after mandibular advancement. For the material and methods, twenty-two subjects who were treated with mandibular advancement were enrolled in this study. Postsurgical stability was defined as the horizontal mandibular position change of <2 mm in lateral cephalogram 1 year after surgery. Subjects were divided into two groups according to the maintenance of postsurgical stability: a stable group (group S) and a less stable group (group LS). Presurgical maxillomandibular transverse index was determined as Yonsei transverse index (YTI) one month before surgery. A logistic analysis was performed on the postsurgical stability according to the YTI value. The presurgical, post-expansion target YTI value was obtained using receiver operating characteristic (ROC) curve. There were no notable differences in the baseline characteristics of the two groups except for vertical positions of point A, B, and gender distribution. Before surgery, however, there was a significant difference in YTI at both the fossa and CR level between the groups. The amount of mandibular advancement did not show a significant difference. The odds ratio for YTI was 0.35 (p = 0.024). The prediction of stability of presurgical YTI yielded an area under the ROC curve of 0.88. The cut-off value for YTI was 1.45 mm. It can thus be concluded that presurgical transverse index showed a correlation with postsurgical stability, and correcting it in the presurgical phase to a certain level appears to aid in securing postsurgical stability. This study analyzed the biomechanical behavior under simulated conditions between a novel modification of the Low Z plasty (NM-Low Z) technique and the conventional Hunsuck–Epker (HE) technique on mandibular advancement surgery. The NM-Low Z technique was developed with the purpose of reducing post-operative complications by facilitating the operative method of lower jaw surgery. The models were investigated under physiological muscular and occlusal loads at 1, 2, 4, 6, and 8 weeks post-operation. Finite element analysis was used to analyze stress distribution, elastic strain, and model displacement. Maximum equivalent von Mises stresses were observed on the fixation system and bone. The elastic strain at the fracture site represented the optimal bone-healing capacity. The NM-Low Z model showed lower stress than the HE model at the 2-week post-operation stage and onwards. The elastic strains observed in both models were situated within the normal range for bone healing. The ranges of displacement for the NM-Low Z model were less than those in the HE model. Based on the data sets studied, the biomechanical study of the NM-Low Z technique can be favorably compared to the conventional HE technique. The NM-Low Z technique promotes post-operation skeletal stability by lowering stress on fixation and bone, with less displacement of the segment during bone-healing periods. [ABSTRACT FROM AUTHOR]

ABSTRACT Introduction: Mandibular advancement surgery corrects bone bases while establishing patients’ functional and aesthetic rehabilitation. However, little is known about the results of this procedure in the structures that make up the stomatognathic system, as the condyles. Objective: This study aimed to evaluate the structural and positional changes of mandibular condyles in ortho-surgical patients who underwent mandibular advancement surgery. Material and Methods: A prospective investigation was conducted with cone-beam computed tomography images. Using Dolphin Imaging® software, seven ortho-surgical patients with Angle Class II malocclusion and mandibular deficiency were evaluated. The images assessed were obtained at pre-surgical phase and after, at least, 1 year of the procedure. To study the structural and positional changes of condyles, linear and angular measurements were obtained, and the right and left sides of patients were compared. Descriptive statistical analysis was performed and, in order to verify possible significant differences, normality tests (Kolmogorov-Smirnov) were applied, followed by a paired t-test to define significance. Results: For all measures evaluated in this study, no statistically significant differences were found. Conclusion: The ortho-surgical procedure performed did not change the structure and position of the condyles of patients who underwent surgical mandibular advancement. Right and left mandibular condyles behaved similarly, suggesting stability and condylar adaptation after surgery.

Objective: This study evaluated the dentoskeletal and soft tissue changes for Class II malocclusion patients treated with Invisalign clear aligners with mandibular wings (IAMW). Methods: This retrospective study included 50 skeletal Class II patients treated with Invisalign clear aligner with mandibular wings. Records of 20 subjects were collected from the AAOF Legacy Collection (The Case Western Bolton Brush Growth Study) and were used as a control. The dental, skeletal, and facial soft tissue changes were assessed by digitizing and analyzing lateral cephalograms using Dolphin Imaging software (version 11.95 Premium; Dolphin Imaging & Management Solutions, Chatsworth, Calif). Paired t-tests and independent t-tests were used to assess the changes before and after and to compare between the IAMW and control groups. Results: The different measurements of the maxilla have shown that IAMW effect on the maxilla included minimal, non-significant retraction compared to the control group. The SNB and mandibular base position increased by 1.17° (±2.63) and 3.79 (±8.13), respectively. The mandible advanced significantly in the treatment group compared to the control group. Dentally, the lower incisors tipped slightly buccally, but the change was not significant (p > 0.05). The facial convexity angle decreased by 1.16° (±4.36). Conclusion: Invisalign clear aligner with mandibular advancer wings was able to correct the Class II malocclusion. This correction was mainly skeletal with some dental changes. This device can be used to address the growth modification problem in Class II malocclusion at the same time as addressing the other occlusal problems.

Background: This study aimed to determine the skeletal and dental effects in pediatric and adolescent Korean patients with Class II Division 1 malocclusion treated using the Invisalign Mandibular Advancement (MAⓇ) appliance. Methods: The study included patients aged 6 to 18 years who received orthodontic treatment with the MAⓇ appliance for Class II Division 1 malocclusion at the Department of Pediatric Dentistry, Wonkwnag University Daejeon Dental Hospital, between July 1, 2018, and December 31, 2021. The treatment group consisted of 20 patients, 10 boys and 10 girls. The control participants were also 10 boys and 10 girls. Lateral cephalometric radiographs were taken before and after treatment, and 41 measurements of skeletal and dental changes were measured and analyzed using the V-CephTM 8.0 (Osstem Implant). All analyses were performed using SPSS software (IBM SPSS for Windows, ver 26.0; IBM Corp.), and statistical significance was tested using paired and independent samples t-tests for within-group and between-group comparisons, respectively. Results: The patients in the treatment group showed significant decreases in ANB (A point, Nasion, B point), maxillary protrusion, maxillary anterior incisor labial inclination, and maxillary protrusion after treatment. However, when compared with the growth changes observed in the control group, only ANB and maxillary protrusion decreased, with no significant differences in SNA, SNB, and mandibular length. Conclusion: Collectively, the results of this study confirm that the use of MAⓇ appliance in pediatric and adolescent Korean patients with Class II Division 1 malocclusion results in a reduction of anteroposterior skeletal and dental disharmony.

Introduction: Full arch reconstruction with dental implants is considered as a reference for dental rehabilitation. However, there are some limitations with patient affected by severe maxilla, mandible atrophy and dento-skeletal-disharmony. These defaults can be corrected with orthognathic surgery and several bone grafts. Observations: A partially edentulous 65-year-old female patient with a skeletal class II by mandibular retrognathia was treated with mandibular advancement osteotomy and simultaneous insertion of four fully guided implants with immediate screwed zirconia prostheses loading in just one session. Conclusion: Simultaneous bilateral sagittal split osteotomy with a full arch rehabilitation by implant with immediate loading by zirconia prosthesis seems to be a reliable and predictable approach to treat skeletal class II patient with a fixed solution. Objectives To evaluate the efficacy of mandibular advancement devices (MADs) in improving apnea-hypopnea index (AHI) in positional obstructive sleep apnea (POSA), compared with a control group of nonpositional OSA (NPOSA) patients, from mild to very severe degree, in order to to find the main variables characterizing the examined group as potential predictors of treatment success.

Objective To evaluate, through a tomographic analysis, the positional changes of the condyle when using a mandibular advancement device (MAD) for the treatment of obstructive sleep apnea (OSA), and to assess if the condylar positions influence OSA polysomnographic patterns.

Introduction Obstructive sleep apnea (OSA) is defined as intermittent partial or complete collapse of the upper airway during sleep. It is a common condition in childhood, with an incidence ranging from 1.2% to 5.7%, and it can harm several aspects of children's life, such as cognitive, metabolic and cardiovascular functions, among others.

The main objective of this prospective study was the evaluation of 1 mm step titration of mandible advancement in the success of treating obstructive sleep apnea (OSA). For that, a multicenter prospective study was designed to recruit patients with OSA who were eligible to receive a mandibular advancement device. Gradual titration of mandibular advancement (steps of 1 mm) from maximum intercuspidation was performed to determine the optimal mandibular advancement (highest reduction in the apnea-hypopnea index (AHI)). The principal variable was the percentage of patients where a reduction ≥50% of the AHI was achieved at the end of the titration phase. A total of 102 patients participated in this study. Fifty-six percent of the participants were males and 67% had a BMI ≥ 25 kg/m2. Most of the patients (79%) had an age ≥ 50 years and the majority (74%) were either non-smokers or ex-smokers. Excessive daytime sleepiness was reported by 40% of the patients. The mean AHI at baseline was 20.6 ± 12.7 events/h. The mean advancement of the mandible was 3.1 ± 1.6 mm. The device achieved a reduction in the AHI in 93% of the patients and success (≥50% reduction in the AHI) in 69% of the patients. Success was achieved in 50%, 81.6%, and 73.3% of the patients with mild, moderate, and severe OSA, respectively. Decreasing the magnitude of mandibular advancement could be possible by controlling the vertical mouth opening and step-by-step titration. [ABSTRACT FROM AUTHOR]

Objectives The aim was to analyse the costs and duration of orthodontic-surgical treatment with mandibular advancement in the public health care sector in Finland. Materials The study was conducted as a retrospective registry study in a public district hospital on all nonsyndromic patients that were ethnic Finns and treated with full fixed appliances and mandibular advancement surgery in 2016–2020. Results The mean treatment duration of the included 45 patients was 28.1 months, including 18.9 months pre and 9.2 months postoperative orthodontics. The median number of visits was 27, including 17 visits before and 9 visits after surgery. The mean total treatment time was 14.5 h. The mean total direct costs per course of treatment were 7574 € to the municipality and 947 € to the patient. The costs positively correlated with the duration of the treatment (rho = 0.71, P  = .000), but were not associated to gender or age of patient. The mean surgery time was 78 minutes, and significantly less with an experienced surgeon (P  = .002). It was calculated that the mean minimum treatment costs would be 45% of the present total, achievable with a patient with optimum dental arches at the start of treatment. Limitations The major limitation of the study is the relatively small number of study subjects. Conclusion A 55% share of the costs is influenced by case- and operator-dependent factors. The overclosure of the mandible and a potential upper airway collapse during sleep creates challenges in treating edentulous sleep apneic patients.Purpose: To evaluate complete dentures and mandibular advancement devices as potential oral appliances in the management of sleep apnea in completely edentulous patients.Materials and Method: The study design was a systematic review with meta-analysis. Randomized controlled trials and cohort studies were included that compared the effectiveness of oral appliances on apnea-hypopnea index (AHI), airway space, and quality of sleep in edentulous sleep apneic patients.Results: 1785 articles were derived from the initial search and based on inclusion criteria, 10 articles were systematically filtered for qualitative analysis and assessed for risk of bias using the Cochrane risk of bias tool and ROBINS-I tool. Out of the 10 articles, 5 articles were taken for quantitative analysis. The use of a mandibular advancement device (MAD) showed a decrease in AHI score, but the available data was heterogeneous to conduct a meta-analysis. The mean difference of AHI for the random effect model between the non-complete denture and complete denture wearers at sleep was -0.49[95% CI (-1.47,0.48)] events per hour, but the change was non-significant (p>.05).Conclusion: The complete dentures as an oral appliance had reduced apneic episodes in completely edentulous sleep apneic patients, but the effectiveness cannot be solely attributed to the prosthesis in the treatment of OSA. MAD showed greater improvement in reducing AHI, however, the level of evidence was inadequate to provide a conclusive statement.

The aim of this study was to examine the relationship between the presurgical maxillomandibular transverse index and post-surgical stability one year after mandibular advancement. For the material and methods, twenty-two subjects who were treated with mandibular advancement were enrolled in this study. Postsurgical stability was defined as the horizontal mandibular position change of p = 0.024). The prediction of stability of presurgical YTI yielded an area under the ROC curve of 0.88. The cut-off value for YTI was 1.45 mm. It can thus be concluded that presurgical transverse index showed a correlation with postsurgical stability, and correcting it in the presurgical phase to a certain level appears to aid in securing postsurgical stability.

Fixed functional appliances are used at either the fag end of the growth spurt or after the completion of the same. We orthodontists are yet to come across an appliance which satisfies all the ideal requirements for a fixed functional appliance. Most disadvantages of the fixed functional appliances such as increase in lower anterior facial height, proclination of lower anteriors, constant breakages, and huge expenses were taken into consideration in the basic designing of this ingenious appliance. In this article, a simple fixed functional appliance was designed to function along with fixed appliance therapy to stimulate mandibular growth. After the initial leveling and aligning, 0.019 × 0.025 stainless steel posted arch wires were placed. The bite jumping blocks were fabricated in the upper and lower arches with light cure blue composite. Using indirect bonding trays they were bonded in position. The case finished in an ideal overjet and overbite with a balanced face. Cephalometric values demonstrate an advancement of the mandible. [ABSTRACT FROM AUTHOR]

Aim: The current study described an 11-year-old female patient with Angle's Class II relationship and skeletal Class II base relationship with retrognathic mandible and orthognathic maxilla. Methods: The patient was treated by mandibular advancement (MA). She was instructed to wear each aligner 20 h a day and to change aligners every 14 days. After the deciduous teeth had been replaced by the permanent teeth, 30 additional appliances were produced for MA. Results: During the 28-month treatment period, MA led to widening of the upper dental arch, alignment of the upper teeth, forward movement of the lower jaw, reduction of the anterior teeth coverage and improvement of the molar relationship. Conclusion: Invisalign MA is simple and comfortable. It can expand the arch, align and even the dentition at the same time, therefore, it can shorten the treatment course effectively.

The aim of this study was to perform a three-dimensional evaluation of Herbst appliance effects on the mandibular canines.The subjects consisted of 23 Class II:1 patients (12 men, 11 women), mean age of 15.76± 1.75 years, consecutively treated with a Flip-Lock Herbst®appliance (TP Orthodontics, Inc., La Porte, IN, USA). The lower anchorage unit for the Herbst appliance consisted of two anchor bands connected by a lingual arch with 3mm distance from the incisor’s lingual surface. Treatment changes in mandibular canine inclination and mandibular intercanine width at the cusp and apex levels were evaluated by means of cone-beam computed tomography images (i-CAT® Classic unit, Imaging Sciences International, Hatfield, PA, USA) obtained before and after treatment with the Herbst appliance. There were no statistical differences between genders. Herbst appliance treatment did not result in any statistically significant changes for mandibular canine inclination and mandibular intercanine width. There were associations between mandibular canine inclination and mandibular intercanine width at the cusp (/r/ = 0.43 to 0.66) and apex levels (/r/ = 0.34 to 0.60). The three-dimensionalcone-beam computed tomography analysis of the mandibular canine segment revealed that the Herbst appliance with a mandibular anchorage unit distant from the incisor’s lingual surface does not change mandibular canine inclination and mandibular intercanine width significantly during treatment. However, large interindividual differences may result in undesired amount of mandibular canine anchorage loss in individual patients.

The main objective of this prospective study was the evaluation of 1 mm step titration of mandible advancement in the success of treating obstructive sleep apnea (OSA). For that, a multicenter prospective study was designed to recruit patients with OSA who were eligible to receive a mandibular advancement device. Gradual titration of mandibular advancement (steps of 1 mm) from maximum intercuspidation was performed to determine the optimal mandibular advancement (highest reduction in the apnea-hypopnea index (AHI)). The principal variable was the percentage of patients where a reduction ≥50% of the AHI was achieved at the end of the titration phase. A total of 102 patients participated in this study. Fifty-six percent of the participants were males and 67% had a BMI ≥ 25 kg/m2. Most of the patients (79%) had an age ≥ 50 years and the majority (74%) were either non-smokers or ex-smokers. Excessive daytime sleepiness was reported by 40% of the patients. The mean AHI at baseline was 20.6 ± 12.7 events/h. The mean advancement of the mandible was 3.1 ± 1.6 mm. The device achieved a reduction in the AHI in 93% of the patients and success (≥50% reduction in the AHI) in 69% of the patients. Success was achieved in 50%, 81.6%, and 73.3% of the patients with mild, moderate, and severe OSA, respectively. Decreasing the magnitude of mandibular advancement could be possible by controlling the vertical mouth opening and step-by-step titration.

Background: To investigate whether fixed functional therapy for mandibular advancement with the Forsus™ appliance would produce any changes in the cervical spine posture. Materials and Methods: This prospective clinical exploratory study was conducted on 12 patients (six females and six males) with a mean age of 15 ± 1.3 years, having mandibular retrusion, Class II malocclusion, who were treated with the Forsus™ appliance. Lateral cephalogram was taken twice, once at the baseline (T1) before the commencement of the treatment and once following termination of fixed functional treatment (T2). Eleven measurements representing the vertical and the sagittal craniofacial proportions, and the head posture, were taken into account. Data were analyzed using SPSS version 22. The variations between before and after treatment measurements were collated using paired t-test. P < 0.05 was considered statistically significant. Results: No significant differences were noticed in the angle linking the horizontal lines of the head and the superior crest of the spinal column before and after the treatment, with P = 0.73. The cervical curvature angle also failed to show any significant difference with P = 0.14. Conclusion: Fixed functional therapy with the Forsus™ device resulted in dentoalveolar and soft tissue alterations alone but did not alter the cervical spine posture.

Objective: The internal fixation has been purpose of study for many years, but there is still no consensus on the best method of fixation in relation to resistance for bilateral sagittal split ramus osteotomy (BSSO) using plates. Therefore, the aim of this study was to assess five different methods of osteosynthesis using resorbable and non-resorbable plates and screws in simulated sagittal split osteotomy (SSO) of the mandibular ramus. Materials and Methods: SSO was performed in 25 polyurethane synthetic mandibular replicas. The distal segments were moved forward 5 mm, and the specimens were grouped according to the fixation method: Inion resorbable plate, KLS resorbable plate, standard four-hole titanium miniplate (Medartis), two standard four-hole titanium miniplates (Medartis) and an adjustable titanium miniplate (Slider/Medartis). Mechanical evaluation was performed by applying compression loads to first molar using an Instron universal testing machine up to a 5 mm displacement of the segments. Resistance forces were obtained in Newtons (N), and statistical analysis was performed using the software R v. 3.5 with significance level of 0.05. Linear mixed models were used to compare the force required to move each type of plate. Results: The results showed that the resistance of SSO was better accomplished using two titanium miniplates and KLS resorbable plate showed the least resistance. However, both titanium and resorbable plates behaved similarly in small displacements, which are most commonly observed in BSSO postoperative time. Conclusion: It can be concluded that both resorbable and non-resorbable systems might offer suitable mechanical resistance in the procedures where there are no mechanical postoperative complications. Aims: The current manuscript explores the viability of reverse sagittal split osteotomy technique for correction of ankylotic cases with post gap arthroplasty mandibular retrognathia to achieve socially acceptable esthetic results. Method: Reverse sagittal split osteotomy which was introduced by Collins et al in 1983 was performed with certain modifications on two cases to correct mandibular hypoplasia in post gap arthroplasty cases. The paper also highlights intraoral as well as extraoral approach for performing the osteotomy along with better management of bad split under direct vision Result: It was observed that the reverse sagittal split technique for advancement of mandible in cases of tmj ankylosis-induced dentofacial deformity provided better proximal control, reduced chances of bad split, greater range of advancement (11–14 mm) with esthetically acceptable results. The osteotomy cuts on lateral surface of mandible make the procedure effectively easier and quicker with better control over proximal segment and management under direct vision. Conclusion: When Distraction Osteogenesis and conventional orthognathic is not a choice in management of dentofacial deformity of post-release ankylosis cases due to the poor proximal control and concern over bad split, reverse sagittal split can be an appropriate choice to manage these deformities without any donor site morbidity. Background: The surgical treatment for mandibular repositioning using a bilateral sagittal split osteotomy (BSSO) favours the development of techniques that result in adequate repair and stability. In Puricelli's mandibular sagittal split osteotomy (PMSSO) proposal, the vertical lateral cut osteotomy is located in the interradicular space between the lower first molar and second premolar., Objectives: This in silico study aimed to investigate the mechanical stability of PMSSO and compare it with the classical Obwegeser-Dal Pont technique for mandibular advancement., Materials and Methods: A computational geometric model of the mandible was created in a virtual environment using computer-aided design (CAD) software. After reproducing the advancements, two test groups were developed: GTOD10, Obwegeser-Dal Pont osteotomy, and GTP10, Puricelli osteotomy, both simulating a 10-mm mandibular advancement, allowing for measuring the area of overlap between bone segments. With the geometric changes promoted by the osteotomy, boundary conditions of displacement and force were applied to a CAD software based on finite element analysis (FEA), allowing for quantitative and comparative analysis of the stress and vertical displacement of the mandible, mechanical measurements that may be associated with strength and stiffness., Results: A 17.48% higher stress was observed in the GTP10 group than in GTOD10. However, the region of highest stress in GTP10 was found in a part of the bone that was still intact and far from the area of fragility caused by lateral vertical osteotomy. In contrast, in GTOD10, the region with high stress was in a less resistant bone region. The GTP10 group showed a 28.73% lower displacement than GTOD10. The area of overlap between the proximal and distal segments of the mandible was 33.13% larger in the GTP10 than in the GTOD10 group., Conclusion: The PMSSO method, performed in large mandibular advancements, keeps the point of highest stress away from the mandibular fragility zone. Objective: The mandibular advancement device (MAD) is currently suggested in patients with mild to moderate obstructive sleep apnea (OSA). This study investigated the effects of a new fully customizable MAD-type device called, "It Makes You Sleep" (IMYS), in patients with mild to moderate OSA., Methods: Sixteen patients (14 men and 2 women; mean age 62; SD ± 11 years) were retrospectively enrolled. Each patient received home sleep apnea testing (HSAT) at baseline (T0) and after three months (T1) of IMYS treatment. The Apnea-Hypopnea Index (AHI), the Oxygen Desaturation Index (ODI), the Minimum Oxygen Saturation (minSO2) and the Medium Oxygen Saturation (medSO2) were analyzed., Results: From T0 to T1, the IMYS treatment showed a significant reduction of AHI and ODI and a significant increase of minSO2 and medSO2., Conclusion: The IMYS appliance is an effective device for treating mild or moderate OSA.

Objective: To retrospectively evaluate the effects of single mandibular advancement (MA) and two-jaw surgery (2J-S) on the pharyngeal airway space (PAS) and hyoid position for the correction of skeletal Class II malocclusion., Methods: Eleven adult patients who underwent only MA surgery and twelve adult patients who underwent Le Fort I maxillary impaction-MA surgery (2-JS) were included in the retrospective study. A total of 46 cephalometric recordings obtained before (T1) and after treatment (T2) were examined. Craniofacial changes, area, and linear measurements of the pharyngeal airway and hyoid bone position were obtained in both groups. The Wilcoxon signed-rank test was used to evaluate time-dependent changes within groups. The Mann-Whitney U test was used to compare differences between groups., Results: Hyoid-Vert values increased significantly in both groups (MA, p<0.01; 2J-S, p<0.05); however, Hyoid-Hor values decreased significantly only in the 2J-S group (p<0.01). The anteroposterior dimensions of the airway increased in both groups, except for the PNS-P and PPS groups (p<0.01). Although a significant increase was observed in the nasopharyngeal area (A1) in the MA group (p<0.05), the decrease was found to be statistically significant in 2JG (p<0.01). Fibromyalgia (FM) is a chronic, often disabling disorder characterized by multisite pain along with sleep problems and fatigue. Pain and sleep exhibit a reciprocal relationship. When FM and obstructive sleep apnea/hypopnea (OSA) co-exist, treatment options include continuous positive airway pressure or mandibular advancement device. We present a patient experiencing fibromyalgia and OSA whose symptoms vanished wearing a Mandibular Advancement Device (MAD) during sleep. To our knowledge, this is the first documented case of FM symptom resolution by MAD treatment.

Objective: Maxillofacial Orthognathic surgery is performed to repair or correct the skeletal anomalies of the jaw and its associated dental and facial structures. There is a conflict on whether orthognathic surgery has a negative or positive effect on temporomandibular disorders (TMD). The aim of this study is to review the disorders of the temporomandibular joint after orthognathic surgery. Materials and Methods: Data for this review was obtained from the articles published between 2010-2020 via PubMed, Google scholar, Web of Sciences, and Scopus engines. The content keywords matched those used in PubMed and Mesh engines. Based on the inclusion and exclusion criteria; 27 articles were included. Results: Most of the selected articles were retrospective reviews and performed on class II and class III patients. Ages ranged from 19- 47 years. Pain reduction was reported in 11 studies, while 8 studies reported a click reduction post orthognathic operation. In 2 studies, decreased joint noises was reported after orthognathic operation, and 7 articles reported a decrease in maximum mouth opening. Three studies reported a Bilateral Sagittal split Osteotomy (BSSO) and in one study, reduced and improved symptoms after Le Fort I + (BSSO) were reported. One study exhibited that BSSO orthognathic surgery is less predictable in reducing TMD symptoms in retrognathic patients. Three articles showed that orthognathic patients with TMJ click have a high predictive value. Conclusion: To accomplish accurate results regarding temporomandibular joint disorders post orthognathic surgery; a larger number of subjects and clinical trial studies are required, as well as extended long term follow-up. Objective: To examine the skeletal, dental, and soft-tissue cephalometric effects of class II correction using Invisalign's mandibular advancement feature in growing patients. Materials and Methods: A retrospective cohort clinical study was performed on cases that were started between 2017 and 2019. A total of 32 patients (13 females, 19 males), with an average age of 13 years old (9.9-14.8 years) had undergone Invisalign treatment (Align Technology, Inc., San Jose, CA) wherein the mandibular advancement phase was completed were included. Photos, digital study models, and cephalograms were taken once during the patients' initial visit and again upon completing the mandibular advancement phase of treatment. The number of aligners worn and the time of treatment in months was recorded for each subject. Cephalometric analysis was performed and overjet and overbite were measured. Statistical analysis was performed using SPSS statistical software (version 25; SPSS, Chicago, Ill) and the level of significance was set at P <0.05. Descriptive statistics were performed to generate means and differences for each cephalometric measurement as well as patient data including age, treatment time, and aligner number. Differences between measurements from patients before treatment (T1) and after treatment (T2) with the mandibular advancement feature were evaluated using a paired t-test. Results: All 32 patients had multiple jumps staged for the precision wings, i.e., incremental advancement. The average length of treatment for the MA phase was 9.2 months (7.5-13.8 months) and the average number of aligners used during this time was 37 (30-55). Statistically significant differences between T1 and T2, in favor of class II correction, were observed in the ANB angle, WITS appraisal, facial convexity, and mandibular length. The nasolabial angle, overjet, and overbite also showed statistically significant changes between T1 and T2. Conclusion: Invisalign aligners with the mandibular advancement feature took approximately 9 months for 1.5 mm of overjet correction. The lower incisor angulation was maintained during class II correction. The minimal skeletal changes are in favor of class II correction. [ABSTRACT FROM AUTHOR]

Purpose The aim of this study was to compare the effects of two different mandibular advancement methods on skeletal, dentoalveolar, and soft tissue structures through cephalometric measurements. Materials and Methods Twenty-four Class II division 1 patients (10 males, 14 female) treated with twin block (TB) or aesthetic approach (EA: Essix plates + Class II elastics) from the archive of our faculty were included in the study. There were 12 individuals in the EA group (mean age: 12.2 ± 1.0) and 12 individuals in the TB group (mean age: 11.8 ± 1.1 years). The skeletal, dentoalveolar, and soft tissue effects of the appliances were evaluated by performing 28 measurements, 14 linear and 14 angular, on the pre and post-treatment cephalometric radiographs. AudaxCeph 5.0 software (Ljubljana, Slovenia) was used for the analysis. A paired sample t-test was employed to assess the changes after one year of utilizing the appliance for each group. Intergroup comparison was performed by using student t test. Results The mandibular base was observed to move forward significantly in both groups (p<0.05). However, the forward movement of the mandibular base was greater in the TB group than in the EA group (p<0.05). There was no difference in lower incisor protrusion between the two treatment methods. The EA device was found to cause a significant increase in vertical direction parameters (p<0.05). Conclusion Both methods resulted in Class II malocclusion correction as well as an acceptable occlusion plus profile. The effects of EA were primarily dentoalveolar. In patients with high aesthetic expectations, EA could be an alternative for TB. [ABSTRACT FROM AUTHOR]

Objective:Strontium ranelate (SR), unlike other anti-osteoporotic agents, might not only prevent bone resorption but also might induce bone formation. The aim of this experimental study was to evaluate the effects of systemic SR on condylar growth during mandibular advancement (MA) in growing rats.Methods:Fifty-six, 8-week-old Wistar male rats weighting 160-190 g were randomly divided into four groups; one control (n=14) and three experimental (n=14). Group 1: Control group, Group 2: SR (900mg/kg daily dose), Group 3: MA, Group 4: SR +MA. The amount and direction of mandibular growth were assessed by linear measurements on the computed tomography (CT) images taken on days 1, 15, and 30. Background and Objective: Obstructive sleep apnea (OSA) needs early detection and effective treatments to reduce the risk of its harmful consequences. The aim of this study was to assess the knowledge and practice of prosthodontists about OSA and oral appliances (OAs) after a period of training and comparative evaluation between two types of virtual education. Materials and Methods: This study was a randomized clinical trial with two types of educational interventions (Pow-erPoint and podcast) performed among the members of the Association of Prosthodontists (dentists who are specialist in prosthodontics) in 2020. The participants answered to a questionnaire which assessed their knowledge and practical actions about OSA. Data were analyzed using SPSS software and independent-sample t-test. Results: Group A (PowerPoint) obtained higher scores in all knowledge sections compared to group B (podcast). Totally, the mean scores of group A in knowledge and practical sections were 77.56 ± 9.09 and 81.75 ± 12.39, respectively. In ad-dition, the mean scores of group B in knowledge and practical sections were 74.72 ± 10.79 and 80.69 ± 14.05, respectively. The difference between the mean scores of the two groups in knowledge and practical sections was not significant. Conclusion: The virtual educational intervention had positive effects on the knowledge and practice of prosthodontists about OSA and OAs. Although the power Point was more effective than podcasts, there was not significant difference between them.

Deep curve of Spee is a very common problem among Class II malocclusion patients associated with mandibular deficiency. The curve of Spee is maintained and leveled following surgical mandibular advancement. The same surgical splint which is used to guide the mandibular advancement can also be used for the leveling of deep curve of Spee. This case report highlights the leveling of deep curve of Spee by the same surgical splint used during the surgical mandibular advancement. Background: To investigate whether fixed functional therapy for mandibular advancement with the Forsus™ appliance would produce any changes in the cervical spine posture.Materials and Methods: This prospective clinical exploratory study was conducted on 12 patients (six females and six males) with a mean age of 15 ± 1.3 years, having mandibular retrusion, Class II malocclusion, who were treated with the Forsus™ appliance. Lateral cephalogram was taken twice, once at the baseline (T1) before the commencement of the treatment and once following termination of fixed functional treatment (T2). Eleven measurements representing the vertical and the sagittal craniofacial proportions, and the head posture, were taken into account. Data were analyzed using SPSS version 22. The variations between before and after treatment measurements were collated using paired t-test. P < 0.05 was considered statistically significant.Results: No significant differences were noticed in the angle linking the horizontal lines of the head and the superior crest of the spinal column before and after the treatment, with P = 0.73. The cervical curvature angle also failed to show any significant difference with P = 0.14.Conclusion: Fixed functional therapy with the Forsus™ device resulted in dentoalveolar and soft tissue alterations alone but did not alter the cervical spine posture. Obstructive Sleep Apnea Syndrome (OSAS) is a respiratory syndrome and oral devices can be used for its treatment. This study aimed to evaluate the opinions of a generic subject about being treated by a dentist for a general health problem and the association between personality traits and the predisposition to use a MAD for the treatment of OSAS and snorting. One hundred and fortyeight participants were enrolled in the study and were asked to fill in the questionnaires. Personality traits were evaluated using NFC (Need for Closure), PER (openness to new experiences), STAITrait and STAI-Stat questionnaires (State-Trait Anxiety Inventory). The propensity to be treated with dental devices for a general health problem such as OSAS and snoring was evaluated with a specific questionnaire. Eight out of ten participants would accept to use dental device to be kept at night for the solution of a health problem or the treatment of a disease that does not affect the teeth. A positive opinion on device used to treat OSAS was associated with higher PER and lower Mad-related distress, while the opinion of usefulness of the device was positively associated with higher PER and STAI-Trait. A positive opinion about treatment of snorting and OSAS using dental devices was associated with higher PER, while lower STAI-Trait was associated with positive opinion on treatment of snorting using dental-devices. The results suggest that some personality traits are associated with the propensity to use MAD to treat a general pathology as OSAS.
