Background/Objectives: Misophonia, characterized by strong emotional reactions to specific sounds, poses significant challenges, particularly in academic settings. This study aimed to examine the psychometric properties of the Brazilian version of the MisoQuest in a sample of high school and university students. The primary objective was to assess its reliability and structural validity to enhance understanding of misophonia in young adults. Methods: A total of 549 students (Mean age = 23.2 years, SD = 9.3; 285 females, 260 males, 4 individuals who did not disclose their gender) participated. Both exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were employed to evaluate the MisoQuest. Fit indices for unifactorial and trifactorial models were compared. Internal consistency was assessed using Cronbach's alpha. Results: The EFA suggested a dominant single-factor structure with high factor loadings (ranging from 0.60 to 0.79). However, the CFA revealed excellent fit for both unifactorial (CFI and TLI = 1.00, RMSEA close to zero) and trifactorial models (CFI and TLI = 1.00, RMSEA = 0.037). The MisoQuest demonstrated high internal consistency (Cronbach's alpha = 0.93). Additionally, 4.5% of participants were identified as positive for misophonia based on a predefined cutoff score of 61. Conclusions: The Brazilian version of the MisoQuest is a reliable and valid tool for assessing misophonia. These findings suggest that the instrument may capture multiple dimensions of the disorder. Given the observed prevalence of misophonia and its impact on students, early identification and tailored interventions are crucial for providing adequate support. Introduction: Misophonia, not yet classified within diagnostic manuals, triggers strong emotional, physiological, and behavioural reactions to specific sounds. This study examines its correlations with attention deficient/hyperactivity disorder (ADHD) traits, obsessive-compulsive traits, and autism-related traits in adolescent outpatients with non-psychotic disorders. We hypothesize a positive association between misophonic symptoms and these psychological traits. Methods: This study was conducted at a Turkish psychiatric centre from January to July 2023 in adolescents aged 12-18. Parents completed the Autism Spectrum Quotient-Adolescent (AQ-Adolescent), and Conner's ADHD Parent Rating Scale-48 (CPRS-48), while the adolescent filled out the Misophonic Symptom Checklist (MCL) and Maudsley Obsessive- Compulsive Inventory (MOCI). Using non-parametric statistical tests, the research found associations between the scales, with a total sample size of 348. Results: Females had higher scores on MCL. There is a negative correlation between AQ-Adolescent and MCL, positive correlations between MCL-MOCI and MCL-CPRS-48. In gender specific correlation analysis found that AQ-Adolescent and MCL were negatively correlated, MCL and MOCI were positively correlated in males. MCL, CPRS-48 and MOCI were positively correlated in females. In regression AQ-Adolescent, MOCI and CPRS-48 significantly predicted the levels of MCL. Conclusions: Our study unveils a link between ADHD, obsessive-compulsive symptoms, autistic traits, and misophonic symptoms in adolescent psychiatric outpatients, highlighting sex differences. Misophonia is a disorder characterized by emotional and physiological responses that occur in response to certain auditory stimuli. Visual, tactile, and olfactory stimuli, primarily oral and nasal sounds such as eating, nose, and respiratory sounds, reveal misophonic responses. People with misophonia may have difficulty in social interactions, and work or school performance may be adversely affected. Misophonia symptoms can also cause anxiety, depression, and other mental health problems. Physiological reactions in the body, such as pain, sweating, tachycardia, hot flashes, and breathing difficulties, negatively affect people's physical health. For this reason, people's physical and mental health and quality of life are significantly affected. The absence of agreed diagnostic criteria for the diagnosis of misophonia and the lack of sufficient data to classify it as a psychiatric disorder has led to the lack of validated treatment guidelines. However, there are treatment recommendations discussed in the literature for misophonia, which has a prevalence of 20% in a limited number of studies and negatively affects the functionality of the individual. In the management of misophonia, tinnitus re-education therapy (TRT) was used as audiological treatment, antidepressants, and anxiolytics were used as pharmacological treatment and cognitive behavioral therapies were used as therapy. Other suggested treatment recommendations are neural remodeling technique, sequential remodeling hypnotherapy, trauma prevention technique, and trauma and tension reduction exercises. This review aims to present the treatment approaches available in the literature together and to understand the need for experimental evidence for treatment methods.

Background: Misophonia is a chronic condition in which the exposure to specific sounds increases the arousal and recurrence of specific intense negative emotions. We hypothesized that misophonia may be strongly related to maladaptive interpersonal schemas that create difficulties in interpersonal relationships. Subjects with maladaptive interpersonal schemas think that other people try to subjugate, criticize, dominate, exploit, deceive, disregard, and humiliate them. Furthermore, these patients typically endorse a representation of self as mistreated, constricted, harmed, damaged, humiliated, impotent, inadequate, or fragile. Methods: We describe the course of a treatment of Metacognitive Interpersonal Therapy (MIT) in a young man presenting misophonia and co-occurrent obsessive–compulsive personality disorder (OCPD) and avoidant personality disorder (AvPD), with narcissistic traits and normal hearing. We collected qualitative and quantitative data at the beginning of the intervention and at 2 years follow-up. Results: The therapy aimed at increasing awareness of maladaptive interpersonal schemas and promoting a healthy self. The results reported a significant decrease in misophonia; behavioural experiments were used to increase the quality of social relationships and tolerance to the trigger sounds. Even in cases of subclinical misophonia, where individuals do not meet the disorder criteria, the experience can still be burdensome, despite not currently causing significant distress or impairment. The S-Five is a psychometric tool for comprehensive assessment of five aspects of misophonic experience: internalizing, externalizing, impact, threat, and outburst, and includes S-Five-T section to evaluate feelings evoked by triggering sounds and their intensity. We examined whether the five-factor structure developed in the UK could be replicated in a Polish sample, including individuals with and without self-identified misophonia. The Polish version of the S-Five was translated and tested on 288 Polish-speaking individuals. Comprehensive psychometric evaluation, including factor structure, measurement invariance, test-retest reliability, internal consistency, and concurrent validity evaluations, was conducted on the translated scale. Exploratory factor analysis suggested similar structure to the original English study, while bootstrap exploratory graph analysis showed the factor structure to be reproducible in other samples. The scale was found to be bias free with respect to gender, internally consistent and stable in time, and evidence of validity was provided using MisoQuest and Misophonia Questionnaire. These results offer support for the cross-cultural stability of the five factors and provide preliminary evidence for the suitability of the Polish version for clinical and research purposes. The study also investigated five facets of misophonia, triggering sounds, emotional responses, and their associations with symptoms of psychopathology across various cultures. It underscores the central role of anger, distress, and panic, while also highlighting the mixed role of irritation and disgust in misophonia across different cultural contexts. Gilles de la Tourette syndrome (GTS) is a neurodevelopmental psychiatric disorder with complex and elusive etiology with a significant role of genetic factors. The aim of this study was to identify structural variants that could be associated with familial GTS. The study group comprised 17 multiplex families with 80 patients. Structural variants were identified from whole-genome sequencing data and followed by co-segregation and bioinformatic analyses. The localization of these variants was used to select candidate genes and create gene sets, which were subsequently processed in gene ontology and pathway enrichment analysis. Seventy putative pathogenic variants shared among affected individuals within one family but not present in the control group were identified. Only four private or rare deletions were exonic in LDLRAD4, B2M, USH2A, and ZNF765 genes. Notably, the USH2A gene is involved in cochlear development and sensory perception of sound, a process that was associated previously with familial GTS. In addition, two rare variants and three not present in the control group were co-segregating with the disease in two families, and uncommon insertions in GOLM1 and DISC1 were co-segregating in three families each. Enrichment analysis showed that identified structural variants affected synaptic vesicle endocytosis, cell leading-edge organization, and signaling for neurite outgrowth. Misophonia is a neurophysiological disorder with behavioral implications, is complex and multifactorial in origin, and is characterized by an atypical and disproportionate emotional response to specific sounds or associated visual stimuli. Triggers include human-generated sounds, mainly sounds related to feeding and breathing processes, and repetitive mechanical sounds. In response to the triggering stimulus, the patient experiences immediate, high-intensity, disproportionate physical and emotional reactions that affect their quality of life and social functioning. The symptoms of misophonia can occur at any age, but onset in childhood or adolescence is most common. Affected children live in a constant state of anxiety, suffer continuous physical and emotional discomfort, and are thus exposed to significant chronic stress. Chronic stress, especially during childhood, has consequences on the main biological systems through the dysregulation of the hypothalamic–pituitary–adrenal axis, including the gastrointestinal tract. Here, we provide arguments for a positive correlation between misophonic pathology and gastrointestinal symptoms, and this hypothesis may be the starting point for further longitudinal studies that could investigate the correlations between these childhood vulnerabilities caused by misophonia and their effect on the gastrointestinal system. Misophonia is a condition characterized by negative affect, intolerance, and functional impairment in response to particular repetitive sounds usually made by others (e.g., chewing, sniffing, pen tapping) and associated stimuli. To date, researchers have largely studied misophonia using self-report measures. As the field is quickly expanding, assessment approaches need to advance to include more objective measures capable of differentiating those with and without misophonia. Although several studies have used sounds as experimental stimuli, few have used standardized stimuli sets with demonstrated reliability or validity. To conduct rigorous research in an effort to better understand misophonia, it is important to have an easily accessible, standardized set of acoustic stimuli for use across studies. Accordingly, in the present study, the International Affective Digitized Sounds (IADS-2), developed by Bradley and Lang (Bradley MM et al., 2007), were used to determine whether participants with misophonia responded to certain standardized sounds differently than a control group. Participants were 377 adults (132 participants with misophonia and 245 controls) recruited from an online platform to complete several questionnaires and respond to four probes (arousal, valence, similarity to personally-relevant aversive sounds, and sound avoidance) in response to normed pleasant, unpleasant, and neutral IADS-2 sounds. Findings indicated that compared to controls, participants with high misophonia symptoms rated pleasant and neutral sounds as significantly more (a) arousing and similar to trigger sounds in their everyday life, (b) unpleasant and (c) likely to be avoided in everyday life. For future scientific and clinical innovation, we include a ranked list of IADS-2 stimuli differentiating responses in those with and without misophonia, which we call the IADS-M. Misophonia, a heightened aversion to certain sounds, turns common cognitive and social exercises (e.g., paying attention during a lecture near a pen-clicking classmate, coexisting at the dinner table with a food-chomping relative) into challenging endeavors. How does exposure to triggering sounds impact cognitive and social judgments? We investigated this question in a sample of 65 participants (26 misophonia, 39 control) from the general population. In Phase 1, participants saw faces paired with auditory stimuli while completing a gender judgment task, then reported sound discomfort and identification. In Phase 2, participants saw these same faces with novel ones and reported face likeability and memory. For both oral and non-oral triggers, misophonic participants gave higher discomfort ratings than controls did–especially when identification was correct–and performed slower on the gender judgment. Misophonic participants rated lower likeability than controls did for faces they remembered with high discomfort sounds, and face memory was worse overall for faces originally paired with high discomfort sounds. Altogether, these results suggest that misophonic individuals show impairments on social and cognitive judgments if they must endure discomforting sounds. Music is generally considered a pleasant and positive stimulus. However, constant exposure to loud music, especially in acoustic environments where individuals have limited control, can transform music listening into a potentially distressing and detrimental experience. In certain contexts, music can therefore be classified as noise, with implications for mental well-being and cognitive functioning. In our paper, we delve into the complexities of this paradoxical phenomenon, discussing the implications of commonly experienced situations in which music becomes a source of discomfort due to its intensity or forced exposure. Our exploration of the effects of music exposure focuses on individual differences, encompassing clinical sensitivity (hyperacusis, misophonia), non-clinical sensitivity (noise sensitivity, sensory-processing sensitivity), and music-related individual factors (sensitivity to music pleasure). We argue that only by recognizing these aspects, significant progress can be made in prevention strategies and interventions. Our aim is to identify regulatory gaps and propose strategies to mitigate potential music-related discomfort in order to promote the creation of auditory environments that align with individual preferences and sensitivities. The aims of the current study were to develop the Turkish version of MisoQuest, a fully validated misophonia questionnaire, to evaluate the relationships between misophonia, anxiety, and quality of life, and to examine the mediating role of anxiety in the relationship between misophonia and quality of life. Methods: The reliability of the Turkish version of MisoQuest was conducted using data from 548 participants (Mean age = 28.06 ± 9.36). Then, the relationships between misophonia, anxiety, and quality of life were evaluated in a separate sample of 117 participants (Mean age = 25.50 ± 6.31) using the State-Trait Anxiety Inventory (STAI) and the Short Form 36 (SF-36) questionnaire. Results: The results showed that the Turkish version of MisoQuest has good psychometric properties. Close-to-moderate positive correlations were found between misophonia and anxiety, and weak negative correlations were found between misophonia and quality of life. Anxiety mediated the relationships between misophonia and quality of life. The relationship between misophonia, stress, and traumatic stress has not been well characterized scientifically. This study aimed to explore the relationships among misophonia, stress, lifetime traumatic events, and traumatic stress. A community sample of adults with self-reported misophonia (N = 143) completed structured diagnostic interviews and psychometrically validated self-report measures. Significant positive correlations were observed among perceived stress, traumatic stress, and misophonia severity. However, multivariate analyses revealed that perceived stress significantly predicted misophonia severity, over and above traumatic stress symptoms. The number of adverse life events was not associated with misophonia severity. Among symptom clusters of post-traumatic stress disorder, only hyperarousal was associated with misophonia severity. These findings suggest that transdiagnostic processes related to stress, such as perceived stress and hyperarousal, may be important phenotypic features and possible treatment targets for adults with misophonia. Introduction: Misophonia is a recently defined disorder characterized by distressing responses to everyday sounds, such as chewing or sniffling. Individuals with misophonia experience significant functional impairment but have limited options for evidenced-based behavioral treatment. To address this gap in the literature, the current pilot trial explored the acceptability and efficacy of a transdiagnostic cognitive-behavioral approach to treating symptoms of misophonia. Methods: This trial was conducted in two studies: In Study 1, the Unified Protocol for Transdiagnostic Treatment of Emotional Disorders (UP) was delivered to eight patients in order to receive feedback to guide revisions to the treatment to suit this population. In Study 2, ten patients received the revised UP treatment to explore its acceptability and preliminary efficacy. This study used a singlecase experimental design with multiple baselines, randomizing patients to either a 2-week baseline or 4-week baseline prior to the 16 weeks of treatment, followed by four weeks of follow-up. Results: The findings from these studies suggested that patients found both the original and adapted versions of the UP to be acceptable and taught them skills for how to manage their misophonia symptoms. Importantly, the findings also suggested that the UP can help remediate symptoms of misophonia, particularly the emotional and behavioral responses. Misophonia, a disorder characterised by an extreme sensitivity to certain sounds, is increasingly being studied in cross-cultural settings. The S-Five scale is a multidimensional psychometric tool initially developed to measure the severity of misophonia in English-speaking populations. The scale has been validated in several languages, and the present study aimed to validate the European Portuguese S-Five scale in a Portuguese-speaking sample. The scale was translated into Portuguese using a forward-backwards translation method. The psychometric properties of the S-Five scale were evaluated in a sample of 491 Portuguese-speaking adults. Confirmatory factor analysis supported a five-factor structure consistent with previous versions of the S-Five scale. The five factors were as follows: (1) internalising appraisals, (2) externalising appraisals, (3) perceived threat and avoidance behaviour, (4) outbursts, and (5) impact on functioning. The satisfactory psychometric properties of the S-Five scale further indicated its cross-cultural stability. As a psychometrically robust tool, the S-Five can measure misophonia in Portuguese-speaking populations, allowing future studies to explore and compare misophonia in this population. Misophonia and misokinesia are disorders characterized by intensely negative physical and emotional reactions to specific auditory and visual stimuli. The availability of effective treatments, especially pharmacological ones, is limited. This report presents a case of a 35-year-old male with severe misophonia and misokinesia who experienced nearly complete resolution of symptoms while undergoing high-dose steroid therapy for an unrelated muscular injury. Two days after starting a 20 mg oral prednisone taper pack (in which the steroid dose is reduced by 4 mg daily), his Amsterdam Misophonia Scale (A-Miso-S) score drastically reduced from a baseline of 23 (i.e., extreme symptoms) to 1, with symptom relief persisting for approximately 2 weeks after completing the taper. Months later, a daily dose of prednisone (4 mg) was reintroduced. This again resulted in a marked reduction in symptoms (A-Miso-S of 6), enabling him to resume working in an office setting despite his triggers. Symptom improvement remained stable over several months. This case raises the possibility of the steroid prednisone as a novel treatment for misophonia and misokinesia. Misophonia has been characterized as intense negative reactions to specific trigger sounds (often orofacial sounds like chewing, sniffling, or slurping). However, recent research suggests high-level, contextual, and multisensory factors are also involved. We recently demonstrated that neurotypicals negative reactions to aversive sounds (e.g., nails scratching a chalkboard) are attenuated when the sounds are synced with positive attributable video sources (PAVS; e.g., tearing a piece of paper). To assess whether this effect generalizes to misophonic triggers, we developed a Sound-Swapped Video (SSV) database for use in misophonia research. In Study 1, we created a set of 39 video clips depicting common trigger sounds (original video sources, OVS) and a corresponding set of 39 PAVS temporally synchronized with the OVS videos. In Study 2, participants (N = 34) rated the 39 PAVS videos for their audiovisual match and pleasantness. We selected the 20 PAVS videos with best match scores for use in Study 3. In Study 3, a new group of participants (n = 102) observed the 20 selected PAVS and 20 corresponding OVS and judged the pleasantness or unpleasantness of each sound in the two contexts accompanying each video. Afterward, participants completed the Misophonia Questionnaire (MQ). The results of Study 3 show a robust attenuating effect of PAVS videos on the reported unpleasantness of trigger sounds: trigger sounds were rated as significantly less unpleasant when paired with PAVS with than OVS. Moreover, this attenuating effect was present in nearly every participant (99 out of 102) regardless of their score on the MQ. In fact, we found a moderate positive correlation between the PAVS-OVS difference and misophonia severity scores. Overall our results provide validation that the SSV database is a useful stimulus database to study how misophonic responses can be modulated by visual contexts. Here, we release the SSV database with the best 18 PAVS and 18 OVS videos used in Study 3 along with aggregate ratings of audio-video match and pleasantness (https://osf.io/3ysfh/). We also provide detailed instructions on how to produce these videos, with the hope that this database grows and improves through collaborations with the community of misophonia researchers.

Background: Misophonia is a chronic condition in which the exposure to specific sounds increases the arousal and recurrence of specific intense negative emotions. We hypothesized that misophonia may be strongly related to maladaptive interpersonal schemas that create difficulties in interpersonal relationships. Subjects with maladaptive interpersonal schemas think that other people try to subjugate, criticize, dominate, exploit, deceive, disregard, and humiliate them. Furthermore, these patients typically endorse a representation of self as mistreated, constricted, harmed, damaged, humiliated, impotent, inadequate, or fragile. Methods: We describe the course of a treatment of Metacognitive Interpersonal Therapy (MIT) in a young man presenting misophonia and co-occurrent obsessive–compulsive personality disorder (OCPD) and avoidant personality disorder (AvPD), with narcissistic traits and normal hearing. We collected qualitative and quantitative data at the beginning of the intervention and at 2 years follow-up. Results: The therapy aimed at increasing awareness of maladaptive interpersonal schemas and promoting a healthy self. The results reported a significant decrease in misophonia; behavioural experiments were used to increase the quality of social relationships and tolerance to the trigger sounds. Conclusions: MIT can be an effective therapy for the treatment of misophonia.

Misophonia is a neurophysiological disorder with behavioral implications, is complex and multifactorial in origin, and is characterized by an atypical and disproportionate emotional response to specific sounds or associated visual stimuli. Triggers include human-generated sounds, mainly sounds related to feeding and breathing processes, and repetitive mechanical sounds. In response to the triggering stimulus, the patient experiences immediate, high-intensity, disproportionate physical and emotional reactions that affect their quality of life and social functioning. The symptoms of misophonia can occur at any age, but onset in childhood or adolescence is most common. Affected children live in a constant state of anxiety, suffer continuous physical and emotional discomfort, and are thus exposed to significant chronic stress. Chronic stress, especially during childhood, has consequences on the main biological systems through the dysregulation of the hypothalamic–pituitary–adrenal axis, including the gastrointestinal tract. Here, we provide arguments for a positive correlation between misophonic pathology and gastrointestinal symptoms, and this hypothesis may be the starting point for further longitudinal studies that could investigate the correlations between these childhood vulnerabilities caused by misophonia and their effect on the gastrointestinal system. Further research to study this hypothesis is essential to ensure correct and timely diagnosis and optimal psychological and pharmacological support.

Aim: People with Misophonia disorder are prone to suffering from other psychological problems, such as anxiety and mood disorders, due to avoidance of various situations. Therefore, the goal of this study is to assess an Experiential Avoidance model with Misophonia disorder through the mediating role of Cognitive Fusion and Emotion Dysregulation in university students. Methods: The method of the present research is a descriptive-correlation study. The population included whole students of the Shahid Chamran University of Ahvaz in the academic year 2022-2023. 251 university students were selected using a random cluster sampling method. Data were collected with a demographic questionnaire, "Misophonia Questionnaire", "Brief Experiential Avoidance Questionnaire", "Cognitive Fusion Questionnaire" and "Difficulties in Emotion Regulation Scale". Results: Experiential Avoidance is indirectly correlated with the symptoms of Misophonia disorder through Emotion Dysregulation. Conclusion: Emotion Dysregulation plays a mediating role in the relationship between Experiential Avoidance and symptoms of Misophonia disorder, unlike Cognitive Fusion. It is suggested that approaches based on emotion regulation, such as transdiagnostic, be used in the development of preventive interventions and treatment for people with symptoms of Misophonia disorder

Purpose: Misophonia is not investigated much from an audiological perspective. Our study aims to examine the processing of the auditory retro-cochlear pathways in individuals with misophonia. Methods: A cross-sectional study was conducted among university students who had misophonia. The revised Amsterdam Misophonia Scale was used to determine the severity of misophonia. Participants were divided into mild and moderate-severe misophonia and compared with the healthy control group. Auditory Brainstem Response testing was recorded from all the individuals with misophonia. The absolute latency, amplitude, inter-peak latency difference, and inter-rate latency difference were compared between the groups. Results: One-way ANOVA result showed no significant difference in all the parameters of auditory brainstem response between the groups. These results are suggestive of normal brainstem processing in individuals with misophonia. Conclusions: The study concludes that the auditory pathway up to brainstem areas is intact in individuals with misophonia. Further studies are essential on a larger population for generalizing the results.

IntroductionMisophonia is a recently defined disorder characterized by distressing responses to everyday sounds, such as chewing or sniffling. Individuals with misophonia experience significant functional impairment but have limited options for evidenced-based behavioral treatment. To address this gap in the literature, the current pilot trial explored the acceptability and efficacy of a transdiagnostic cognitive-behavioral approach to treating symptoms of misophonia.MethodsThis trial was conducted in two studies: In Study 1, the Unified Protocol for Transdiagnostic Treatment of Emotional Disorders (UP) was delivered to eight patients in order to receive feedback to guide revisions to the treatment to suit this population. In Study 2, ten patients received the revised UP treatment to explore its acceptability and preliminary efficacy. This study used a single-case experimental design with multiple baselines, randomizing patients to either a 2-week baseline or 4-week baseline prior to the 16 weeks of treatment, followed by four weeks of follow-up.ResultsThe findings from these studies suggested that patients found both the original and adapted versions of the UP to be acceptable and taught them skills for how to manage their misophonia symptoms. Importantly, the findings also suggested that the UP can help remediate symptoms of misophonia, particularly the emotional and behavioral responses.DiscussionThese findings provide preliminary evidence that this transdiagnostic treatment for emotional disorders can improve symptoms of misophonia in adults.

Abstract Aim Misophonia is a sound sensitivity disorder characterized by an unusually strong aversion to a specific class of sounds—often human bodily sounds (e.g., chewing). These sounds can cause intense negative emotions which can cause profound difficulties in everyday life. The condition is linked with higher rates of anxiety and depression, and here we ask whether it co‐occurs with elevated self‐harm and suicidal thinking. Methods We measured self‐harm and misophonia in the general population by examining a birth cohort sample from the Avon Longitudinal Study of Parents and Children (ALSPAC). We screened them for misophonia as adults, then analyzed their earlier data on well‐being, self‐harm, and suicidal thinking. Results Adults with misophonia had significantly higher rates of self‐harm and suicidal ideation, as well as poorer well‐being in a number of different measures at ages 16–17 and 23–24 years. Female misophonics were particularly at risk, from as early as their teenage years, though males, too, show elevated self‐harm at 24 years compared to nonmisophonic peers. Conclusion Our data provide evidence of elevated risks of self‐harm associated with misophonia and suggest the need for greater recognition and treatment pathways.

Objective: In the current study, the authors sought to examine whether the link between moral and sexual disgust and misophonia is mediated by mental contamination. Method: An internationally diverse sample of 283 adults (193 females, 76 males, and 14 non-binary individuals) ranging in age from 18 to 60 years old was recruited from online social media platforms and survey recruitment sites. The sample completed an online battery of scales that consisted of the New York Misophonia Scale, State Metal Contamination Scale, and the Three-Domain Disgust Scale. The hypotheses were evaluated using a series of mediations. performed using the PROCESS add-on in SPSS. Results: Correlations were found between emotional and aggressive-avoidant reactions in misophonia, mental contamination, pathogen disgust, and sexual disgust. Moral disgust and non-aggressive reactions in misophonia failed to correlate significantly with any of the other constructs. Sexual disgust had direct and indirect effects while pathogen disgust had only direct effects on aspects of misophonia. Discussion: These findings partially support our hypothesis that mental contamination mediates the link between disgust propensity and misophonia while also confirming that pathogen-based disgust is not associated with mental contamination. Background: The assessment and management of misophonia need a team approach, and audiologists are essential team members. However, the role of an audiologist in this condition is not well understood, and there is a lack of awareness even among professionals about their role in the assessment and management of misophonia. Purpose: The main aim of our study is to document the present level of awareness and knowledge about misophonia assessment and management among audiologists in India. Methods: A descriptive cross-sectional study was carried out among audiologists from all over India. Descriptive statistical procedures were measured based on the type of questions being addressed, and a non-parametric chi-square test was done to see the association among variables. Results: The results show a lack of knowledge about misophonia even among audiologists, as only 15.3% of the audiologist reported being confident in handling cases with misophonia. Conclusion: Although the exact assessment and management of misophonia is still the topic of debate, it is clear that audiologists are the team's key members. However, the results clearly show a lack of confidence in handling cases of misophonia among audiologists in India. This result shows the future need for Research in misophonia from an audiological perspective.

Objective: Hyperacusis and misophonia are frequently associated with psychiatric comorbidities. The relationship between symptom severity and depression and anxiety has not been directly demonstrated. Aim: The aim of this study was to investigate whether there is a correlation between the severity of hyperacusis and mis-ophonia and depression and anxiety. Material and methods: Participants consisted of 12 adults who presented to the audiology clinic with complaints of decreased sound tolerance. Each participant underwent audio-logic evaluation (pure tone audiometry, speech audiometry, immittancemetric examination, speech tests) and decreased sound tolerance assessment (scales and questionnaires, loudness discomfort level measurement). The Khalfa Hyperacusis Questionnaire (HQ), Misophonia Symptom List (MSL) and loudness discomfort level measurement (LDL) were used together to differentiate decreased sound tolerance. Individuals with both hyperacusis and misophonia who were reported to have normal hearing on audiologic evaluation were included in the study. Hyperacusis Handicap Questionnaire, Misophonia Scale, Beck Depression Scale and Beck Anxiety Scale were administered to all participants. The correlation between the severity of hyperacusis and misophonia and anxiety and depression scores were evaluated. Results: A moderate positive significant correlation was found between hyperacusis severity and depression score (r = 0.633; p = 0.03). No significant relationship was observed between hyperacusis severity and anxiety score (p > 0.05). No significant correlation was observed between the number of misophonic triggers and the score of misophonia scale and depression and anxiety (p > 0.05). Conclusions: Increased severity of hyperacusis may increase depression. Future studies should be continued by increasing the number of data for a reliable interpretation of the findings. Misophonia, characterized by hypersensitivity to specific sounds, significantly impacts family dynamics and children's school experiences. This study aims to understand this condition from teachers' and parents' viewpoints, providing insights into coping strategies within educational and family contexts. Research Questions: 1. Are teachers able to recognize symptoms of misophonia in students at school? 2. How do teachers manage the challenges posed by misophonia? Do they view it as a challenge affecting their professional experience? 3. How do parents perceive and respond to their children's symptoms of misophonia at home? 4. What coping strategies do parents employ to manage misophonia in both family and school settings? 5. What support and intervention strategies do teachers and parents prefer for dealing with misophonia? Research Methodology: Participant Selection: The study involves teachers and parents. Teachers working with students of various age groups and parents of school-aged children are included. Sample: Approximately 300 individuals will be surveyed, including parents (both mothers and fathers). Additionally, 20 participants will be selected for in-depth interviews. Surveys: Participants will complete anonymous surveys. In-Depth Interviews: Selected participants will engage in detailed interviews to discuss their experiences and coping strategies. Expected Outcomes: This study aims to provide information on how teachers and parents perceive misophonia and identify effective coping strategies for managing this disorder in school and family settings. This exploratory research addresses a significant gap, as little is known about the experiences of those dealing with misophonia in children and adolescents. Study Objectives: The primary objective is to investigate the perceptions and experiences of misophonia in children and adolescents from the perspectives of teachers and parents. The study will explore its impact on family functioning and children's school performance. Significance of the Study: 1. Children with Misophonia: Highlighting the need for support due to the disorder's relative obscurity and the subsequent impact on students. 2. Parents' Experience: Examining how parents perceive their children's symptoms and their connections with school functioning, including interactions with teachers regarding misophonia. 3. As a solo practice clinician, over the past dozen years I have seen many hundreds of individuals and families affected by misophonia. I have taken their stories to heart and have conducted several informal surveys to consider whether their experiences are shared by others dealing with this complex condition. As I am not in a university or medical setting, I do not have the equipment or staff to conduct formal studies to test my observations, questions, and conclusions. This session will present the results of some of my informal surveys in the hope that researchers might take this raw data and conduct studies to verify or dismiss my informal results. Included will be my observations and surveys in the US addressing the following questions: 1. Is there a larger percentage of people with misophonia in the US who were raised in families of origin with more highly structured religions (mainly Catholicism, but also other more rigidly structured religions such as Orthodox Jewish, Seventh Day Adventist, Southern Baptist, etc.) than in other less structured religions? [Reasoning: Possibly more rigidity in family rules around behavior, especially at the table, emphasis on right/wrong, guilt, shame]. 2. Is there a larger proportion of Neanderthal DNA in people with misophonia than those without misophonia? Are people with misophonia more likely to have Myers-Briggs types IJ (Introversion/Judgment) than the average reported percentages of those types in the general population? Would they have been in those categories prior to or only resulting from misophonia? 4. Are people with misophonia more likely to have a family lineage including this or other neurological disorders, as compared to families without misophonia? We hypothesized that misophonia may be strongly related to maladaptive interpersonal schemas which create difficulties in interpersonal relationships. Maladaptive interpersonal schemas are typically present in personality disorder. Persons with maladaptive interpersonal schemas think that other people try to subjugate, criticize, dominate, exploit, deceive, disregard and humiliate them. Furthermore, these patients typically endorse a representation of self as mistreated, constricted, harmed, damaged, humiliated, impotent, inadequate or fragile. We describe the course of a treatment of Metacognitive Interpersonal Therapy (MIT) with a young man presenting misophonia and co-occurrent obsessive-compulsive personality disorder (OCPD) and avoidant personality disorder (AVP), with narcissistic traits. He presents self-criticism, perfectionism and emotional overcontrol. Therapy aimed increasing awareness of maladaptive interpersonal schemas and promoting a healthy self. Behavioural experiments were used to increased quality of social relationships and tolerance to the trigger sounds. Brain imaging of humans with loudness hyperacusis or misophonia remains one of the primary methods to discover the brain networks underlying these sound tolerance disorders, to differentiate them from each other and to dissociate them from related comorbid conditions of tinnitus or hearing loss. Despite the challenges associated with the expense and noise of some imaging tools, results of such studies have informed the existing experimental and theoretical framework of hy-peracusis and misophonia. In a pioneering functional MRI study, Melcher and colleagues (2010) parsed out the contribution of co-occurring hyperacusis to brain imaging findings of tinnitus. They noted that while both subcortical and cortical auditory areas were responsive in those with hyperacusis (relative to controls), such an elevated response was only noted in the auditory cortex for those with tinnitus. Recent studies (e.g., Hofmeier et al., 2021; Koops and van Dijk, 2021) further explored this dissociation using both fMRI and auditory brain-stem responses (ABRs). Brain imaging studies of misophonia (e.g., Kumar et al., 2021; Schroder et al., 2019) are more recent but have begun to change how we think of the condition, pointing to regions beyond the auditory cortex (such as motor cortex, insula) as playing a key role in this condition. In a recently completed study in my lab, we have collected both auditory brainstem response and fMRI data on young adults with hyperacusis or misophonia and their controls, results of which will be reported at the meeting. In summary, current and future non-invasive brain imaging studies continue to expand our understanding of the pathophysiol-ogy of hyperacusis and misophonia and will allow us eventually to develop and test new therapies that help patients. Misophonia typically emerges during childhood, yet research in this demographic is limited. This study aimed to investigate a wide range of characteristics in young individuals with misophonia. We examined 90 Polish children between the ages of 7 and 18, including a control group, along with their parents, utilizing interviews, audiological assessments, questionnaires, and performance-based tests. The first set of results has already been published. Among the main findings, it was observed that children with misophonia did not significantly differ from their peers in terms of developmental disorders, emotional and social competencies, head injuries, epilepsy, tinnitus, or perinatal factors. However, they exhibited higher levels of anxiety and depression, an increased incidence of OCD, migraines, and psychosomatic complaints. Additionally, their mothers more frequently self-reported postpartum depression compared to mothers in the control group. The results not yet published can be categorized into three aspects of functioning: 1) Regarding hearing loss and the seven assessed tests of so-called central auditory processing, no significant group differences were found (p > .05). However, the misophonia group demonstrated faster reactions to visual stimuli tests. 2) Concerning cognitive functioning, children with misophonia outperformed the control group in divided attention (p = .038, Cohen's d = .62) and inhibitory control (p = .004, Cohen's d = .76). In logistic regression, better inhibition emerged as a significant predictor for the likelihood of having misophonia, even after adjusting for gender and IQ. No significant group differences were observed in cognitive speed processing, auditory short-term memory, abstract reasoning, verbal reasoning, mean grades for school performance, and behavior. 3) The data on parental stress and the relationship between the child and parent are currently under analysis and will be presented during the conference. The results of this study are in line with most of the previous preliminary reports and the most recent outcomes. However, they also shed light on new aspects of the early development of misophonia and indicate the need for further exploration, particularly in examining better attentional and inhibitory aspects as potential mechanisms underlying misophonia. Misophonia and hyperacusis are typically defined in terms of a narrow versus broad range of trigger sounds and are often linked to different aspects of a sound (sound meaning for misophonia, loudness for hyperacusis). However, co-morbidity between these conditions suggests possible mechanistic overlap. In two studies we examine the neurochemical and psychophysiological basis of misophonia (using a validated questionnaire) and hyperacusis (based on uncomfortable loudness thresholds). The technique of magnetic resonance spectroscopy (MRS) uses MRI to estimate the concentration of excitatory and inhibitory neurotransmitters in the brain, enabling us to explore whether either type of sound intolerance is linked to general levels of excitability in key regions of the brain (auditory cortex, insula). No differences were found. In a separate experimental study, participants engaged in an attention-demanding visual task whilst concurrently measuring responsiveness to task-irrelevant sounds (in terms of level of distraction, psychophysiological responsiveness). Here we do find that different aspects of the task are differentially sensitive to misophonia and hyperacusis. Misophonia, meaning "hatred of sound", is a proposed neurological condition in which certain sounds trigger emotional or physiological responses others may deem unreasonable. The studies on prevalence of misophonia show that almost 20% of college going students exhibit experience misophonia like symptoms worldwide. These studies help us understand that decreased tolerance towards certain sounds has a high prevalence rate. In a country like India, the diversity in terms of exposure to various levels of noise and traditional habits spans across different age groups, locations, socio-economic statuses, and communities. This study aims to establish the prevalence rate and severity of misophonia in college going-students of India and also an attempt to determine gender dominance. The total participants were 328 undergraduate students all over India, with diverse cultural, linguistic, and economic backgrounds. An online survey was conducted through Google forms, wherein the participants filled the self-rating Amsterdam Misophonia Scale and Misophonia Questionnaire. The results of the study showed that the prevalence of misophonia was approximately 15.85%, with a moderate to severe degree of misophonia. Background: Misophonia is a severe emotional response to repetitive sounds. This disorder may limit a person’s communication, reduce his/her ability, or disrupt his/her social and personal life. This study aimed to determine the prevalence of Misophonia and its relationship with obsessive-compulsive disorder, anxiety, and depression in undergraduate students of Shiraz University of Medical Sciences.Methods: The present study is an analytical descriptive study conducted in October 2020. The study samples consisted of 390 undergraduate students of Shiraz University of Medical Sciences. A relative and systematic sampling method was used. In this study, demographic questionnaire, misophonia questionnaire (A score of 7 or higher is considered as misophonia), Beck anxiety questionnaire, Beck depression questionnaire, and Maudsley obsessive-compulsive inventory questionnaire were used, and the data were analyzed using SPSS 24 software. In this study, chi-square test was used to examine the relationship between the variables. Due to the non-normality of the data, the Spearman correlation coefficient was used for data analysis. The significance level was considered equal to and less than 0.05.Results: Of the 390 participants in the study, 93 (23.8%) had experienced misophonia. Among these 93 students, 37 (39.8%) had obsessive-compulsive disorder, 8 (8.6%) suffered anxiety, and 9 (9.7%) were depressed. The problem of hyperacusis a condition in which sensitivity to sounds is increased is common in world practice. According to the ICD classification of diseases of the World Health Organization (WHO), the concept of hyperacusia is used as a general term for abnormal auditory sensations, (ICD code H93.2). Up to 17% of children and adolescents are disturbed by unpleasant sensations in the sounds of the environment. In this review, we would like to analyze the current state of the issue, to understand the terminology of similar conditions. To study the nature of complaints, varieties and causes of this condition, the relationship with concomitant diseases. Designate diagnostic principles, algorithms for managing patients with hyperacusis by different specialists. Consider the options for the treatment and rehabilitation of children and adolescents suffering from hyperacusis proposed in world practice.

Clinical observations of hundreds of patients who exhibited decreased tolerance to sound showed that many of them could not be diagnosed as having hyperacusis when negative reactions to a sound depend only on its physical characteristics. In the majority of these patients, the physical characteristics of bothersome sounds were secondary, and patients were able to tolerate other sounds with levels higher than sounds bothersome for them. The dominant feature determining the presence and strength of negative reactions are specific to a given patient's patterns and meaning of bothersome sounds. Patients with misophonia could not control internal emotional reactions (even when fully realizing that these reactions are disproportionate to benign sounds evoking them) suggesting that subconscious, conditioned reflexes linking the auditory system with other systems in the brain are the core mechanisms of misophonia. Consequently, the strength of functional connections between various systems in the brain plays a dominant role in misophonia, and the functional properties of the individual systems may be perfectly within the norms. Based on the postulated model, we proposed a treatment for misophonia, focused on the extinction of conditioned reflexes linking the auditory system with other systems in the brain. Misophonia is characterized by a preoccupation with and strong emotional and behavioral reactions to certain triggers, mostly sounds related to eating and breathing. We applied functional impairment due to misophonic symptoms as a central criterion to investigate differences between clinical misophonia and normative decreased sound tolerance in a large non-random sample of n = 1,881 individuals from an online survey. We assessed the frequency of self-reported misophonia symptoms across various symptom measures, compared severity, triggers and emotional reactions, general psychopathology, interpersonal emotion regulation, and quality of life between both groups with and without functional impairing misophonia. Individuals with functional impairment due to misophonia (n = 839) revealed significantly higher general psychopathology symptoms, lower interpersonal emotion regulation skills, and lower quality of life than individuals without impairment (n = 1,042). Anxious/distressed and annoyed reactions to triggers were experienced more frequently compared to emotional reactions of disgust and sadness or depression in both groups. Overall, the group differences were primarily quantitative in nature. What is the reality of the misophonic experience in the general population? This is a study on misophonia in a large sample, representative of the UK general population. The study utilises a multidimensional psychometric tool, the S-Five, to study the intensity of the triggering misophonic sounds in everyday activities, the emotions/feelings related to them, and the norms of the key components of the misophonic experience: internalising and externalising appraisals, perceived threat and avoidance behaviours, outbursts, and the impact on functioning. Based on the S-Five scores and a semi-structured interview delivered by clinicians who specialise in misophonia, the estimated prevalence of people for whom symptoms of misophonia cause a significant burden in their life in the UK was estimated to be 18%. The psychometric properties of the S-Five in the UK general population were also evaluated and differences across gender and age were explored. Our results show that the five-factor structure is reproduced, and that the S-Five is a reliable and valid scale for the measurement of the severity of the misophonic experience in the general UK population. Background: Misophonia is a recently identified disorder in which individuals experience intense, uncontrollable and disproportional irritation, anger or disgust when confronted with specific sounds or stimuli associated with these sounds. Prevalence rates in children and adolescents are currently still to be investigated. The reported average age of onset is around 13 years, in clinical practice children from 8 years old are referred.Misophonia is associated with avoidance and anticipation anxiety, possibly leading to serious educational and social consequences for children and families. Worldwide, no evidence-based treatment exists specifically for children and adolescents with misophonia.This article presents the design of a randomized controlled trial testing the effectiveness of cognitive behavioral therapy (CBT) combined with psychomotor therapy (PMT) for misophonia in children and adolescents (aged 8–18). Methods: In total, 82 patients will be randomly assigned to a treatment condition or waiting list condition of 3 months (WCG). Treatment consists of 7 weekly group therapy sessions (1.5 h CBT plus 1.5 h PMT) and a follow-up after 3 weeks. Pre and post treatment assessments will be conducted during a baseline assessment, after 3 and 6 months. The primary outcome will be assessed by the Amsterdam Misophonia Scale – Youth (AMISOS-Y) and secondary outcomes (e.g. quality of life) and putative predictors (e.g. parenting burden) will be studied. Conclusion: This trial is the first study worldwide testing the effectiveness of a combined CBT plus PMT protocol for misophonia in children and adolescents. If proven effective, this protocol provides an innovation to improve care for youth with misophonia.

Misophonia, a disorder characterised by an extreme sensitivity to certain sounds, is increasingly being studied in cross-cultural settings. The S-Five scale is a multidimensional psychometric tool initially developed to measure the severity of misophonia in English-speaking populations. The scale has been validated in several languages, and the present study aimed to validate the European Portuguese S-Five scale in a Portuguese-speaking sample. The scale was translated into Portuguese using a forward-backwards translation method. The psychometric properties of the S-Five scale were evaluated in a sample of 491 Portuguese-speaking adults. Confirmatory factor analysis supported a five-factor structure consistent with previous versions of the S-Five scale. The five factors were as follows: (1) internalising appraisals, (2) externalising appraisals, (3) perceived threat and avoidance behaviour, (4) outbursts, and (5) impact on functioning. The satisfactory psychometric properties of the S-Five scale further indicated its cross-cultural stability. As a psychometrically robust tool, the S-Five can measure misophonia in Portuguese-speaking populations, allowing future studies to explore and compare misophonia in this population.

Introduction: Misophonia is a recently defined disorder in which certain aversive repetitive sounds and associated stimuli elicit distressing and impairing affective, behavioral, and physiological responses. The responses in misophonia may be stronger when the sound is produced by close friends and family, suggesting that the context in which a triggering cue occurs may have an important role in misophonia. As such, the goal of this study was to test experimentally whether the context of the sound source influences affective and psychophysiological responses to triggering stimuli in misophonia. Methods: Sixty one adults with misophonia and 45 controls listened to audio recordings (8 s) of human eating, animals eating, and human mouth smacking sounds (without eating). After a break, the same audio recordings were presented embedded within videos of human eating (congruent stimuli), animals eating (congruent stimuli), and, in the mouth smacking condition, with visually incongruent stimuli (hands playing in mud or in a bowl with a watery dough). Psychophysiological responses--skin conductance response (SCR) and heart rate (HR), and self-reported affective responses (valence, arousal, dominance) were gathered during the experiment in a laboratory. Results: Participants with misophonia assessed all the stimuli as more negative and arousing than the controls, and reported feeling less dominant with respect to the sounds. Animal and mouth smacking sounds were assessed by all the participants as less negative and arousing than human eating sounds, but only in the audio-video conditions. SCR data partially confirmed increased psychophysiological arousal in misophonia participants during an exposure to mouth sounds, but did not reflect the self-report changes in response to different contexts. Misophonia participants had deeper deceleration of HR than controls during human eating sound with congruent video stimuli, while there was no group difference during human mouth smacking with incongruent video stimuli. Conclusion: Results suggest that the context of mouth sounds influences affective experiences in adults with misophonia, but also in participants without misophonia. Presentation of animal eating sounds with congruent visual stimuli, or human mouth smacking sounds with incongruent stimuli, decreased self-report reaction to common misophonic triggers. Misophonia is a disorder of decreased tolerance to certain "trigger" sounds (e.g., chewing, tapping, clicking). While misophonia research is scant in general, studies presenting sounds are especially rare and methodologically variable, likely due to the labor and time required to create stimuli. Thus, we introduce FOAMS: Free Open-Access Misophonia Stimuli, a sound bank publicly available on Zenodo, accompanied by pilot discomfort ratings for 32 of these sounds (4 exemplars of 8 classes). The FOAMS database aims to decrease the burden on researchers, facilitating reproducibility and the pursuit of nuanced research questions to better understand this perplexing disorder.
