Abstract: The optimized superconducting stellarator device Wendelstein 7-X (with major radius , minor radius , and plasma volume) restarted operation after the assembly of a graphite heat shield and 10 inertially cooled island divertor modules. This paper reports on the results from the first high-performance plasma operation. Glow discharge conditioning and ECRH conditioning discharges in helium turned out to be important for density and edge radiation control. Plasma densities of with central electron temperatures were routinely achieved with hydrogen gas fueling, frequently terminated by a radiative collapse. In a first stage, plasma densities up to were reached with hydrogen pellet injection and helium gas fueling. Here, the ions are indirectly heated, and at a central density of a temperature of with was transiently accomplished, which corresponds to with a peak diamagnetic energy of and volume-averaged normalized plasma pressure . The routine access to high plasma densities was opened with boronization of the first wall. After boronization, the oxygen impurity content was reduced by a factor of 10, the carbon impurity content by a factor of 5. The reduced (edge) plasma radiation level gives routinely access to higher densities without radiation collapse, e.g. well above line integrated density and central temperatures at moderate ECRH power. Both X2 and O2 mode ECRH schemes were successfully applied. Core turbulence was measured with a phase contrast imaging diagnostic and suppression of turbulence during pellet injection was observed.

Abstract: Wendelstein 7-X (W7‐X) is the world’s largest superconducting nuclear fusion experiment of the optimized stellarator type. In the first Operation Phase (OP1.1) helium and hydrogen plasmas were studied in limiter configuration. The heating energy was limited to 4 MJ and the main purpose of that campaign was the integral commissioning of the machine and diagnostics, which was achieved very successfully. Already from the beginning a comprehensive set of diagnostics was available to study the plasma. On the path towards high-power, high-performance plasmas, W7‐X will be stepwise upgraded from an inertially cooled (OP1.2, limited to 80 MJ) to an actively cooled island divertor (OP2, 10 MW steady-state plasma operation). The machine is prepared for OP1.2 with 10 inertially cooled divertor units, and the experimental campaign has started recently. The paper describes a subset of diagnostics which will be available for OP1.2 to study the plasma edge, divertor and scrape-off layer physics including those already available for OP1.1, plus modifications, upgrades and new systems. The focus of this summary will be on technical and engineering aspects, like feasibility and assembly but also on reliability, thermal loads and shielding against magnetic fields.

Abstract: Équipe 107 : Physique des plasmas chauds; International audience; In JET-ILW dedicated melt exposures were performed using a sequence of 3MA/2.9T H-Mode JET pulses with an input power of P-IN = 23 MW, a stored energy of similar to 6 MJ and regular type I ELMs at Delta W-ELM = 0.3 MJ and f(ELM) similar to 30 Hz. In order to assess the risk of starting ITER operations with a full W divertor, one of the task was to measure the consequences of W transients melting due to ELMs. JET is the only tokamak able to produce transients/ ELMs large enough (>300 kJ per ELM) to facilitate melting of tungsten. Such ELMs are comparable to mitigated ELMs expected in ITER. By moving the outer strike point (OSP) onto a dedicated leading edge the base temperature was raised within similar to 1 s to allow transient ELM-driven melting during the subsequent 0.5 s. Almost 1 mm (similar to 6 mm(3)) of W was moved by similar to 150 ELMs within 5 subsequent discharges. Significant material losses in terms of ejections into the plasma were not observed. There is indirect evidence that some small droplets (similar to 80 mu m) were ejected. The impact on the main plasma parameters is minor and no disruptions occurred. The W-melt gradually moved along the lamella edge towards the high field side, driven by j x B forces. The evaporation rate determined is 100 times less than expected from steady state melting and thus only consistent with transient melting during individual ELMs. IR data, spectroscopy, as well as melt modeling point to transient melting. Although the type of damage studied in these JET experiments is unlikely to be experienced in ITER, the results do strongly support the design strategy to avoid exposed edges in the ITER divertor. The JET experiments required a surface at normal incidence and considerable pre-heating to produce tungsten melting. They provide unique experimental evidence for the absence of significant melt splashing at events resembling mitigated ELMs on ITER and establish a unique experimental benchmark for the simulations being used to study transient shallow melting on ITER W divertor PFUs

5. Theoretical Investigation of Molecular and Electronic Structures of Buckminsterfullerene-Silicon Quantum Dot Systems

Abstract: Density functional theory (DFT) and density functional tight binding (DFTB) molecular dynamics (DFTB/MD) simulations of embedding and relaxation of buckminsterfullerene C60 molecules chemisorbed on (001) and (111) surfaces and inside bulk silicon lattice were performed. DFT calculations of chemisorbed fullerenes on both surfaces show that the C60 molecule deformation was very small and the C60 binding energies were roughly ∼4 eV. The charge analysis shows that the C60 molecule charges on (001) and (111) surfaces were between -2 and -3.5 electrons, respectively, that correlates well with the number of C-Si bonds linking the fullerene molecule and the silicon surface. DFT calculations of the C60 molecule inside bulk silicon confirm that the C60 molecule remains stable with the deformation energy values of between 11 and 15 eV for geometries with different C60 configurations. The formation of some C-Si bonds causes local silicon amorphization and corresponding electronic charge uptake on the embedded fullerene cages. Charge analysis confirms that a single C60 molecule can accept up to 20 excessive electrons that can be used in practice, wherein the main charge contribution is located on the fullerene's carbon atoms bonded to silicon atoms. These DFT calculations correlate well with DFTB/MD simulations of the embedding process. In this process, the C60 molecule was placed on the top of the Si(111) surface, and it was further exposed by a stream of silicon dimers, resulting in subsequent overgrowth by silicon.

Abstract: The results of a comprehensive and systematic ab initio based ground-state search for the structural arrangement of oxygen vacancies in rutile phase ${\mathrm{TiO}}_{2}$ provide new insights into their memristive properties. We find that O vacancies tend to form planar arrangements which relax into structures exhibiting metallic behavior. These metastable arrangements are structurally akin to, yet distinguishable from, the Magn\'eli phase. They exhibit a more pronounced metallic nature but are energetically less favorable. Our results confirm a clear structure-property relationship between segregated oxygen vacancy arrangement and metallic behavior in reduced oxides.

Abstract: The high heat flux ion beam test facility MARION (Material Research ION Beam test Stand) has been used for ion source conditioning and material testing within the IEK-4 for several years. Mainly small samples have been tested in the past by perpendicular irradiation. In these experiments only 25% of the available power density of MARION (>80 MW/m 2 ) was used so far. The activation of these resources for material testing was realized in the first test phase of the new tungsten divertor element for the JET tokamak. The enhancement of MARION by a new protection scraper is shown. During the last experiments the need of a more flexible handling of the sample itself and of the facility became clear. Based on those experiences an upgrade concept was developed to prepare MARION for future demands allowing the handling of much bigger prototypes. Additionally main emphasis is given to the access to improved (surface) diagnostics for all orientations of the target.

Abstract: In the frame of the ITER-like Wall (ILW) for the JET tokamak, a divertor row made of bulk tungsten material has been developed for the position where the outer strike point is located in most of the foreseen plasma configurations. In the absence of active cooling, this represents a formidable challenge when one considers the temperature reached by tungsten (TW,surf > 2000 °C) and the vertical gradient ∂T/∂z = 5 × 104 K/m. As the development is drawing to an end and most components are in production, actual 1:1 prototypes are exposed to an ion beam with a power density around 7 MW/m2 on the plasma-facing surface. Advantage is taken of the flexibility of the Marion facility to bombard the tungsten stack under shallow angles of incidence (∼6°) with a powerful beam of ions and neutrals (>70 MW/m2 on axis). The shallow angles are important, with respect to the toroidal wetted surface, for properly simulating the expected performance under actual tokamak conditions. The M arion tests have been used to validate for a few typical cases the thermal calculations that were steadily developed along with the tungsten tile and, at the same time, to gather information on the actual temperatures of individual components. The latter is an important factor to a finer estimation of the power handling capabilities.

Abstract: Atomistic simulations show that organosilicates, used as low permittivity dielectric materials in advanced integrated circuits, can be made substantially stiffer than amorphous silica, while maintaining a lower mass density. The enhanced stiffness is achieved by incorporating organic cross-links to replace bridging oxygen atoms in the silica network. To elucidate the mechanism responsible for the enhanced stiffness, the conformational changes in the network upon hydrostatic and shear loading are examined. The structural and mechanical impact of terminal methyl groups is also assessed quantitatively and compared with continuous random network theory.

Abstract: Vacancy dynamics in oxides are vital for understanding redox reactions and resulting memristive effects or catalytic activity. We present a method to track and drive vacancies which we apply to metadynamics simulation of oxygen vacancies (V${}_{\mathrm{O}}{}^{2+}$) in rutile, demonstrating its effectiveness. Using the density-functional based tight-binding method, it is possible to explore the free energy hyperplane of oxygen vacancies in TiO${}_{2}$. We show that the migration of V${}_{\mathrm{O}}{}^{2+}$ in TiO${}_{2}$ is governed by the jump with the highest degree of topological interconnection. Free energy profiles are consistent with minimum energy paths.

Abstract: For the dynamic behaviour of a target under irradiation with dense particle beams, it is essential to study static and transport properties of matter over a wide range of densities and temperatures. Of particular interest are the formation of neutral atoms and molecules through recombination in a dense plasma, and the eventual emergence of ordered structures, as well as the delocalization and the lowering of the continuum edge that precurses the transition to a metallic state. As a theoretical tool, we employ wave packet molecular dynamics, in which electrons are represented by Gaussian wave packets, whose parameters follow pseudo-Hamiltonian dynamics. To exclude artifacts from the finite size of the simulation box, about 103 particles must be simulated. For reasons of numerical economy, the wave function is only pairwise symmetrized at present. As stationary observables, we study the energy-resolved electron-proton pair distribution function for hydrogen and the spectral density of the electrons. With increasing density, we observe the delocalization of the states and the lowering of the continuum edge. As a dynamical observable, we consider the stopping of ions. In comparison with classical point particles, the finite width of the wave packet leads to a smoothing of the singular Coulomb potential and a reduction of the stopping power. This is confirmed by actual calculations of the stopping of ions in matter.

Abstract: Organosilicate glasses (OSGs) are used as low-k intermetal dielectrics for advanced integrated circuits. In this application, the material must fulfill two conflicting requirements: It has to have low density to reduce the dielectric constant while being sufficiently mechanically stable to withstand thermomechanical and other stresses during subsequent steps of integrated circuit manufacture. Recent experimental advances in improving the mechanical and electrical properties of these materials have not yet been systematically studied theoretically at the ab initio level due to the large model sizes necessary to realistically describe amorphous materials. In this paper we employ the density-functional-based tight-binding method to achieve an accurate description of OSG properties at different compositions. We analyze the influence of composition and local network defects on the density and bulk modulus of nonporous OSG. We find that the dependence of density and that of mechanical stiffness on chemical composition are of different natures. This difference is traced to a transition between mechanisms of elastic deformation in silica glass and in silicon hydrocarbide, which is also the reason for the two materials' different sensitivities to network defects.

Abstract: Oxide materials might form the basis for the next technological revolutions in fields such as electronics, photovoltaics or heterogeneous catalysis by enabling the production of inexpensive, high capacity and fast nonvolatile memories, neuromorphic computers and control systems, and efficient energy conversion devices. All these possible applications have in common that they promise to expand the limits of existing technologies by exploiting the inherent material-level complexity of oxides. However, the enhanced opportunities of this added complexity come at the price of significantly greater difficulties in understanding the materials' properties and their implications. Among many, the peculiar and variable mixture of ionic and covalent bonding behavior and the relative ease of changing the oxygen content of many compounds lead to fascinating effects. Furthermore, a deep level of understanding of the materials properties and their relation to synthesis is crucial for eventual applications. In recent years, an enormous amount of work has been devoted to the development of new experimental and theoretical tools, which have enabled much improved understanding of oxide behavior. Materials science is now becoming able to predict and engineer the behavior of oxide materials and explore brave new worlds of functionality. The recent advances in functional oxide science and technology prompted us to hold the CECAM workshop on “Functional Oxides for Emerging Technologies” at the Bremen Center for Computational Materials Science in October 2013, and it was during the preparation for this workshop that the Focus Issue on functional oxides was conceived. The three Review@RRL articles introduce the reader to innovative, low energy electron microscopy [1], controllable electric conduction at oxide interfaces [2], and ionic switching based RRAM [3]. The 20 original Letters present exciting new research on the topics of transport properties [4], super-reducible oxides [5], ferroelectricity [6], point defects [7], memory applications [8–10], bulk properties [11–14], interfaces [15–17], surfaces [18–20] and catalytic properties [21–23]. We sincerely hope that you will find our collection of articles from the forefront of functional oxide research to be informative and that reading our Focus Issue inspires you to come up with more and even better ideas and insights. Abstract: A plug for the ITER core charge exchange recombination spectroscopy (core CXRS) is located in the upper port 3. It transfers the light emitted by interaction of plasma ions with the diagnostic neutral beam (DNB). The plug consists of a main shell, a shielding cassette and a retractable tube. The tube carries the first mirror (M1) with a shutter and allows multiple replacements of M1. A large uncertainty in the M1 lifetime has initiated a new design option with M1 located as far as possible from the first wall. A fast shutter has been proposed to enhance M1 protection. It operates consistently with a pulsed DNB and uses elastic hearings. Three-point attachment scheme of blanket shield modules (BSMs) has been suggested. It is capable to react electromagnetic loads caused by eddy- and halo-currents for any BSM option with a good design margin. The concern has been raised with regards to the forces caused by halo current and applied to the entire plug. Possible solutions are either to shunt and support the plug to the port in the frontal part, or re-attach all BSM in this belt from the plugs to the vacuum vessel. (C) 2009 Elsevier B.V. Abstract: We calculate the minimum energy paths and reaction energies of the adsorption of the epoxide adhesive components diglycidylesterbisphenol A (DGEBA), diethyltriamine (DETA), and the adhesion promoter 3-aminopropylmethoxysilane (AMEO) at two different sites on a model of the native Al2O3 surface, using the nudged elastic band algorithm in conjunction with self-consistent charge-density functional based tight binding. Our results show that the chosen combination of methods is well suited to obtain an overview of the reaction mechanisms and kinetics of the adsorption of organic molecules on inorganic surfaces. The obtained MEP-s show that there is preference for the adsorption of the adhesion promoter, AMEO, over the resin, DGEBA, while the adsorption of the curing agent, DETA, is unfavorable. Our approach also gives an insight into the ranges of the mechanical and electronic influences of the adsorption process on the interface, which neither full ab initio methods nor force field approaches can provide. These results will help to develop a quantum mechanics-molecular mechanics multiscale embedding scheme for more detailed studies of organic/inorganic hybrid interface reactions.

Abstract: A systematic study of the level positions of intrinsic and carbon defects in ${\mathrm{SiO}}_{2}$ is presented, based on density functional calculations with a hybrid functional in an $\ensuremath{\alpha}$-quartz supercell. The results are analyzed from the point of view of the near interface traps (NIT), observed in both $\mathrm{SiC}∕{\mathrm{SiO}}_{2}$ and $\mathrm{Si}∕{\mathrm{SiO}}_{2}$ systems, and assumed to have their origins in the oxide. It is shown that the vacancies and the oxygen interstitial can be excluded as the origin of such NIT, while the silicon interstitial and carbon dimers give rise to gap levels in the energy range inferred from experiments. The properties of these defects are discussed in light of the knowledge about the $\mathrm{SiC}∕{\mathrm{SiO}}_{2}$ interface.

Abstract: Possible defect structures, arising from the interaction of ${\mathrm{O}}_{2}$ molecules with an ideal portion of the $\mathrm{SiC}∕\mathrm{Si}{\mathrm{O}}_{2}$ interface, have been investigated systematically using density functional theory. Based on the calculated total energies and assuming thermal quasiequilibrium during oxidation, the most likely routes leading to complete oxidation have been determined. The defect structures produced along these routes will remain at the interface in significant concentration when stopping the oxidation process. The results obtained for their properties are well supported by experimental findings about the $\mathrm{SiC}∕\mathrm{Si}{\mathrm{O}}_{2}$ interface. It is found that carbon-carbon bonds can explain most of the observed interface states but not the high density near the conduction band of $4H$-SiC.

Abstract: Since the last IAEA conference, the scientific programme of JET has focused on the qualification of the integrated operating scenarios for ITER and on physics issues essential for the consolidation of design choices and the efficient exploitation of ITER. Particular attention has been given to the characterization of the edge plasma, pedestal energy and edge localized modes (ELMs), and their impact on plasma facing components (PFCs). Various ELM mitigation techniques have been assessed for all ITER operating scenarios using active methods such as resonant magnetic field perturbation, rapid variation of the radial field and pellet pacing. In particular, the amplitude and frequency of type I ELMs have been actively controlled over a wide parameter range (q(95) = 3-4.8, beta(N)

Abstract: nuclear run on, Northern btotting, Western btotting and [uciferase reporter assay, respectivety. Transcription factor binding activity was assessed by EMSA and chromatin immunoprecipitation (CHIP). Results: TSA strongty reduced MIF mRNA and protein revers in whore brood and cet[ tines. Nuctear run on anatyses showed that TSA reduced MIF gene transcription. Yet, TSA affected neither the activity of an ectopicatty expressed MIF promoter, nor the nuclear content of Spl and CREB, two transcription factors previousty shown to be indispensabte for MIF gene expression. Surprisingty, ChIP anatyses rereared that, even though gtoba[ histone acetytation was strongty increased by TSA, TSA deacetytated the histories associated with the MIF promoter. This effect required protein synthesis and was coupted with a decreased recruitment of Spl and CREB to the bona fide MIF promoter. Conclusions: TSA down-regutates MIF expression by a motecutar mechanism invotving a [oca[ deacetyration of MIF-associated histories and a reduced transcription of the MIF gene. Considering that MIF is over-expressed in human tumors and required for tumor associated angiogenesis, our findings suggest that the anti-tumora[ effects of HDIs may be mediated by a down-regutation of MIF expression.

Abstract: After completing the main construction phase of Wendelstein 7-X (W7-X) and successfully commissioning the device, first plasma operation started at the end of 2015. Integral commissioning of plasma start-up and operation using electron cyclotron resonance heating (ECRH) and an extensive set of plasma diagnostics have been completed, allowing initial physics studies during the first operational campaign. Both in helium and hydrogen, plasma breakdown was easily achieved. Gaining experience with plasma vessel conditioning, discharge lengths could be extended gradually. Eventually, discharges lasted up to 6 s, reaching an injected energy of 4 MJ, which is twice the limit originally agreed for the limiter configuration employed during the first operational campaign. At power levels of 4 MW central electron densities reached 3 × 1019 m−3 , central electron temperatures reached values of 7 keV and ion temperatures reached just above 2 keV. Important physics studies during this first operational phase include a first assessment of power balance and energy confinement, ECRH power deposition experiments, 2nd harmonic O-mode ECRH using multi-pass absorption, and current drive experiments using electron cyclotron current drive. As in many plasma discharges the electron temperature exceeds the ion temperature significantly, these plasmas are governed by core electron root confinement showing a strong positive electric field in the plasma centre. Abstract: For the first operation phase of the optimized stellarator W7-X, a heterodyne poloidal correlation reflectometry diagnostic is installed and put into operation. The system is intended to measure the perpendicular (with respect to the magnetic field) turbulence rotation and turbulence properties, such as the decorrelation time and correlation length at the plasma edge. Furthermore, it can give information on the magnetic field line pitch angle. The system consists of an array of microwave antennas distributed in the poloidal and toroidal directions. The frequency range of 22 GHz–40 GHz allows us to access local plasma densities of m−3– m−3. During the first operation phase the turbulence rotation is measured in almost all plasmas. In addition, the radial electric field is estimated and compared to that in neoclassical theory. The relatively low plasma density allows us to cover up to 80% of the plasma radius during OP1.1. The obtained data cover various experimental programs and are partly presented in the paper.

Abstract: Équipe 107 : Physique des plasmas chauds; International audience; The original goals of the JET ITER-like wall included the study of the impact of an all W divertor on plasma operation (Coenen et al 2013 Nucl. Fusion 53 073043) and fuel retention (Brezinsek et al 2013 Nucl. Fusion 53 083023). ITER has recently decided to install a full-tungsten (W) divertor from the start of operations. One of the key inputs required in support of this decision was the study of the possibility of W melting and melt splashing during transients. Damage of this type can lead to modifications of surface topology which could lead to higher disruption frequency or compromise subsequent plasma operation. Although every effort will be made to avoid leading edges, ITER plasma stored energies are sufficient that transients can drive shallow melting on the top surfaces of components. JET is able to produce ELMs large enough to allow access to transient melting in a regime of relevance to ITER. Transient W melt experiments were performed in JET using a dedicated divertor module and a sequence of I-P = 3.0 MA/B-T = 2.9 T H-mode pulses with an input power of P-IN = 23 MW, a stored energy of similar to 6 MJ and regular type I ELMs at Delta W-ELM = 0.3 MJ and f(ELM) similar to 30 Hz. By moving the outer strike point onto a dedicated leading edge in the W divertor the base temperature was raised within similar to 1 s to a level allowing transient, ELM-driven melting during the subsequent 0.5 s. Such ELMs (delta W similar to 300 kJ per ELM) are comparable to mitigated ELMs expected in ITER (Pitts et al 2011 J. Nucl. Mater. 415 (Suppl.) S957-64). Although significant material losses in terms of ejections into the plasma were not observed, there is indirect evidence that some small droplets (similar to 80 mu m) were released. Almost 1 mm (similar to 6 mm(3)) of W was moved by similar to 150 ELMs within 7 subsequent discharges. The impact on the main plasma parameters was minor and no disruptions occurred. The W-melt gradually moved along the leading edge towards the high-field side, driven by j x B forces. The evaporation rate determined from spectroscopy is 100 times less than expected from steady state melting and is thus consistent only with transient melting during the individual ELMs. Analysis of IR data and spectroscopy together with modelling using the MEMOS code Bazylev et al 2009 J. Nucl. Mater. 390-391 810-13 point to transient melting as the main process. 3D MEMOS simulations on the consequences of multiple ELMs on damage of tungsten castellated armour have been performed. These experiments provide the first experimental evidence for the absence of significant melt splashing at transient events resembling mitigated ELMs on ITER and establish a key experimental benchmark for the MEMOS code.

26. Assessment of Operator Reliability in Ultrasound of the Median and Ulnar Nerve Using Bland-Altman Analysis.

Abstract: Currently, there is no standardized method to evaluate operator reliability in nerve ultrasound. A short prospective protocol using Bland-Altman analysis was developed to assess the level of agreement between operators with different expertise levels. A control rater without experience in nerve ultrasound, three novices after two months of training, an experienced rater with two years of experience, and a reference rater performed blinded ultrasound examinations of the left median and ulnar nerve in 42 nerve sites in healthy volunteers. The precision of Bland-Altman agreement analysis was tested using the Preiss-Fisher procedure. Intraclass correlation coefficients (ICC), coefficients of variation, and Bland-Altman limits of agreement were calculated. The sample size calculation and Preiss-Fisher procedure showed a sufficient precision of Bland-Altman agreement analysis. Limits of agreement of all trained novices ranged from 2.0 to 2.9 mm 2 and were within the test's maximum tolerated difference. Ninety-five percent confidence intervals of limits of agreement revealed a higher precision in the experienced rater's measurements. Operator reliability in nerve ultrasound of the median and ulnar nerve arm nerves can be evaluated with a short prospective controlled protocol using Bland-Altman statistics, allowing a clear distinction between an untrained rater, trained novices after two months of training, and an experienced rater.

Abstract: Newborns are particularly susceptible to bacterial infections due to qualitative and quantitative deficiencies of the neonatal innate immune system. However, the mechanisms underlying these deficiencies are poorly understood. Given that fetuses are exposed to high concentrations of estradiol and progesterone during gestation and at time of delivery, we analyzed the effects of these hormones on the response of neonatal innate immune cells to endotoxin, bacterial lipopeptide, and Escherichia coli and group B Streptococcus, the two most common causes of early-onset neonatal sepsis. Here we show that at concentrations present in umbilical cord blood, estradiol and progesterone are as powerful as hydrocortisone for inhibition of cytokine production by cord blood mononuclear cells (CBMCs) and newborn monocytes. Interestingly, CBMCs and newborn monocytes are more sensitive to the effects of estradiol and progesterone than adult peripheral blood mononuclear cells and monocytes. This increased sensitivity is associated with higher expression levels of estrogen and membrane progesterone receptors but is independent of a downregulation of Toll-like receptor 2 (TLR2), TLR4, and myeloid differentiation primary response gene 88 in newborn cells. Estradiol and progesterone mediate their anti-inflammatory activity through inhibition of the NF-κB pathway but not the mitogen-activated protein kinase pathway in CBMCs. Altogether, these results suggest that elevated umbilical cord blood concentrations of estradiol and progesterone acting on mononuclear cells expressing high levels of steroid receptors contribute to impair innate immune responses in newborns. Therefore, intrauterine exposure to estradiol and progesterone may participate in increasing susceptibility to infection during the neonatal period.

28. Monitoring procalcitonin in febrile neutropenia: what is its utility for initial diagnosis of infection and reassessment in persistent fever?

Abstract: Background: Management of febrile neutropenic episodes (FE) is challenged by lacking microbiological and clinical documentation of infection. We aimed at evaluating the utility of monitoring blood procalcitonin (PCT) in FE for initial diagnosis of infection and reassessment in persistent fever., Methods: PCT kinetics was prospectively monitored in 194 consecutive FE (1771 blood samples): 65 microbiologically documented infections (MDI, 33.5%; 49 due to non-coagulase-negative staphylococci, non-CNS), 68 clinically documented infections (CDI, 35%; 39 deep-seated), and 61 fever of unexplained origin (FUO, 31.5%)., Results: At fever onset median PCT was 190 pg/mL (range 30-26'800), without significant difference among MDI, CDI and FUO. PCT peak occurred on day 2 after onset of fever: non-CNS-MDI/deep-seated-CDI (656, 80-86350) vs. FUO (205, 33-771; p<0.001). PCT >500 pg/mL distinguished non-CNS-MDI/deep-seated-CDI from FUO with 56% sensitivity and 90% specificity. PCT was >500 pg/ml in only 10% of FUO (688, 570-771). A PCT peak >500 pg/mL (1196, 524-11950) occurred beyond 3 days of persistent fever in 17/21 (81%) invasive fungal diseases (IFD). This late PCT peak identified IFD with 81% sensitivity and 57% specificity and preceded diagnosis according to EORTC-MSG criteria in 41% of cases. In IFD responding to therapy, median days to PCT <500 pg/mL and defervescence were 5 (1-23) vs. 10 (3-22; p = 0.026), respectively., Conclusion: While procalcitonin is not useful for diagnosis of infection at onset of neutropenic fever, it may help to distinguish a minority of potentially severe infections among FUOs on day 2 after onset of fever. In persistent fever monitoring procalcitonin contributes to early diagnosis and follow-up of invasive mycoses.

Abstract: Modified vaccinia virus Ankara (MVA) is an attenuated double-stranded DNA poxvirus currently developed as a vaccine vector against HIV/AIDS. Profiling of the innate immune responses induced by MVA is essential for the design of vaccine vectors and for anticipating potential adverse interactions between naturally acquired and vaccine-induced immune responses. Here we report on innate immune sensing of MVA and cytokine responses in human THP-1 cells, primary human macrophages and mouse bone marrow-derived macrophages (BMDMs). The innate immune responses elicited by MVA in human macrophages were characterized by a robust chemokine production and a fairly weak pro-inflammatory cytokine response. Analyses of the cytokine production profile of macrophages isolated from knockout mice deficient in Toll-like receptors (TLRs) or in the adapter molecules MyD88 and TRIF revealed a critical role for TLR2, TLR6 and MyD88 in the production of IFNbeta-independent chemokines. MVA induced a marked up-regulation of the expression of RIG-I like receptors (RLR) and the IPS-1 adapter (also known as Cardif, MAVS or VISA). Reduced expression of RIG-I, MDA-5 and IPS-1 by shRNAs indicated that sensing of MVA by RLR and production of IFNbeta and IFNbeta-dependent chemokines was controlled by the MDA-5 and IPS-1 pathway in the macrophage. Crosstalk between TLR2-MyD88 and the NALP3 inflammasome was essential for expression and processing of IL-1beta. Transcription of the Il1b gene was markedly impaired in TLR2(-/-) and MyD88(-/-) BMDM, whereas mature and secreted IL-1beta was massively reduced in NALP3(-/-) BMDMs or in human THP-1 macrophages with reduced expression of NALP3, ASC or caspase-1 by shRNAs. Innate immune sensing of MVA and production of chemokines, IFNbeta and IL-1beta by macrophages is mediated by the TLR2-TLR6-MyD88, MDA-5-IPS-1 and NALP3 inflammasome pathways. Delineation of the host response induced by MVA is critical for improving our understanding of poxvirus antiviral escape mechanisms and for designing new MVA vaccine vectors with improved immunogenicity.

Abstract: Background: Invasive fungal infections (IFIs) are life-threatening complications in neutropenic patients with hematological malignancies. Because early diagnosis of IFI is difficult, new noninvasive, culture-independent diagnostic tools are needed to improve clinical management. Recent studies have reported that detection of 1,3-beta-D-glucan (BG) antigenemia may be useful for diagnosis of IFI. The aim of the present prospective study was to evaluate the usefulness of monitoring BG in patients undergoing chemotherapy for acute leukemia., Methods: BG antigenemia was measured by a colorimetric assay twice weekly in the absence of fever and daily in the presence of fever. IFIs were classified according to the criteria of the European Organization for Research and Treatment of Cancer/Mycoses Study Group., Results: During 190 consecutive neutropenic episodes (median duration, 22 days; range, 7-113 days) in 95 patients, 30 proven or probable IFIs (13 aspergillosis, 15 candidiasis, and 2 mixed IFIs) were diagnosed. Sensitivity, specificity, positive predictive value, negative predictive value, and efficiency of 2 consecutive BG values > or =7 pg/mL for diagnosis of proven or probable IFI was 0.63 (95% confidence interval, 0.44-0.79), 0.96 (95% confidence interval, 0.89-0.98), 0.79 (95% confidence interval, 0.57-0.92), 0.91 (95% confidence interval, 0.84-0.95), and 0.89, respectively. The time interval between onset of fever as first sign of IFI and BG antigenemia was significantly shorter than the time to diagnosis of IFI by clinical, microbiological, radiological, and/or histopathological criteria (P < .001). BG values >50 pg/mL were observed in only 2 patients, both of whom experienced failure of antifungal therapy., Conclusion: Monitoring of BG antigenemia is a useful noninvasive method for early diagnosis of IFI in patients with acute leukemia.

Abstract: Background: Identification of new therapeutic targets remains an imperative goal to improve the morbidity and mortality associated with severe sepsis and septic shock. Macrophage migration inhibitory factor (MIF), a proinflammatory cytokine and counterregulator of glucocorticoids, has recently emerged as a critical mediator of innate immunity and experimental sepsis, and it is an attractive new target for the treatment of sepsis., Methods: Circulating concentrations of MIF were measured in 2 clinical trial cohorts of 145 pediatric and adult patients who had severe sepsis or septic shock caused predominantly by infection with Neisseria meningitidis or other gram-negative bacteria, to study the kinetics of MIF during sepsis, to analyze the interplay between MIF and other mediators of sepsis or stress hormones (adrenocorticotropic hormone and cortisol), and to determine whether MIF is associated with patient outcome., Results: Circulating concentrations of MIF were markedly elevated in 96% of children and adults who had severe sepsis or septic shock, and they remained elevated for several days. MIF levels were correlated with sepsis severity scores, presence of shock, disseminated intravascular coagulation, urine output, blood pH, and lactate and cytokine levels. High levels of MIF were associated with a rapidly fatal outcome. Moreover, in meningococcal sepsis, concentrations of MIF were positively correlated with adrenocorticotropic hormone levels and negatively correlated with cortisol levels and the cortisol:adrenocorticotropic hormone ratio, suggesting an inappropriate adrenal response to sepsis., Conclusions: MIF is markedly and persistently up-regulated in children and adults with gram-negative sepsis and is associated with parameters of disease severity, with dysregulated pituitary-adrenal function in meningococcal sepsis, and with early death.

Abstract: Macrophage migration inhibitory factor (MIF) is a mediator of innate immunity and important in the pathogenesis of septic shock. Lipopolysaccharide (LPS) and tumor necrosis factor (TNF) alpha are reported to be inducers of MIF. We studied MIF and cytokines in vivo in patients with meningococcal disease, in human experimental endotoxemia, and in whole blood cultures using a newly developed sensitive and specific enzyme-linked immunosorbent assay. Twenty patients with meningococcal disease were investigated. For the human endotoxemia model, 8 healthy volunteers were intravenously injected with 2 ng/kg Escherichia coli LPS. Whole blood from healthy volunteers was incubated with LPS or heat-killed meningococci. Macrophage migration inhibitory factor concentration in blood was increased during meningococcal disease and highest in the patients presenting with shock compared with patients without shock. Plasma concentration of MIF correlated with disease severity, the presence of shock and with the cytokines interleukin (IL) 1beta, IL-10, IL-12, and vascular endothelial growth factor, but not with TNF-alpha. MIF was not detected in blood in experimental endotoxemia, nor after stimulation of whole blood with LPS or meningococci, although high levels of TNF-alpha were seen in both models. In conclusion, MIF is increased in patients with meningococcal disease and highest in the presence of shock. Macrophage migration inhibitory factor cannot be detected in a human endotoxemia model and is not produced by whole blood cells incubated with LPS or meningococci.
